Abstract:
CoordenaÃÃo de AperfeiÃoamento de Pessoal de NÃvel Superior<br>Diabetes Mellitus apresenta um estado inflamatÃrio silencioso basal e de elevado estresse oxidativo intrinsecamente relacionados com as complicaÃÃes da doenÃa. Estudos recentes evidenciam aÃÃes anti-inflamatÃrias e redutoras de estresse oxidativo, independentes do efeito glicÃmico, com uso da terapia incretÃnica. No presente estudo, objetivou-se avaliar a resposta da gliclazida, sulfonilurÃia de segunda geraÃÃo, e da vildagliptina, inibidor da DPP-4, na resposta glicÃmica, nos marcadores inflamatÃrios e de estresse oxidativo em diabÃticos tipo 2 nÃo controlados com metformina. Trata-se de um estudo clÃnico, prospectivo, randomizado, aberto e comparativo com 35 pacientes e com duraÃÃo de 22 semanas. Dezoito pacientes utilizaram gliclazida MR, 60-120mg/dia, e 17 pacientes fizeram uso da vildagliptina, 100mg/dia. Glicemia de jejum e pÃs-prandial, HbA1c, insulina, glucagon, GLP-1, HOMA-IR, marcadores solÃveis de adesÃo celular (VCAM, ICAM e E-selectina), marcadores inflamatÃrios (TNF-alfa, IL-6 e PAI-1) e marcadores de estresse oxidativo (TBARS e TAOS) foram aferidos ao inÃcio e ao tÃrmino do estudo. O estudo foi composto por 51,4% pacientes do sexo masculino e a mÃdia de idade foi de 56,9 Â 10.0 anos. NÃo se observou diferenÃa estatÃstica ao final do estudo no peso e no IMC, porÃm houve variaÃÃo positiva nas mÃdias do peso e IMC encontradas no grupo usuÃrio de gliclazida (+ 1,4kg e + 0,6kg/m2, respectivamente) e variaÃÃo negativa no grupo usuÃrio de vildaglitpina (-0,6Kg e -0,3Kg/m2, respectivamente). Houve melhor controle da glicemia de jejum e pÃs-prandial no grupo da gliclazida (pajustado< 0,001 e = 0,008, respectivamente) e as duas drogas apresentaram melhora semelhante da glicada (p=0,068). A sE-selectina foi o Ãnico marcador que apresentou declÃnio significante e apenas intragrupo na vildagliptina (p=0,009). A insulina reduziu de forma significante a favor do grupo da vildagliptina (23,4 Â 11,1 vs 17,8 Â 11,9ÂU/ml, pajustado=0,020) e o glucagon tambÃm (77,7 Â 44,0 vs 46,4 Â 31,7pg/ml, pajustado=0,012). Houve elevaÃÃo significante do GLP-1 intragrupo na vildagliptina (p=0,028) e declÃnio significante do HOMA intragrupo na vildagliptina tambÃm (p=0,003). O PAI-1 nÃo apresentou alteraÃÃes significantes no estudo; o TNF-alfa reduziu significativamente intragrupo em ambos os tratamentos (3.9 Â 0.7 to 3.5 Â 1,1pg/ml, p= 0,006, grupo da gliclazida, e 3.6 Â 1.2 to 3.0 Â 1,5pg/ml, p<0,001, grupo vildagliptina), com significÃncia intergrupo favorecendo a vildagliptina (pajustado=0,001). A IL-6 teve elevaÃÃo nÃo significante no grupo gliclazida e reduÃÃo tambÃm nÃo significante no grupo da vildagliptina. O TBARS apresentou melhora em ambos os grupos, com significÃncia na vildagliptina tanto intragrupo (9,1 Â 1,6 para 8,0 Â 0,7 nmol/MDA/ml), p= 0,039), quanto intergrupo (8,4 Â 0,9 vs 8,0 Â 0,7 nmol/MDA/ml, pajustado <0,001, gliclazida e vildagliptina respectivamente) e o TAOS apresentou elevaÃÃo nÃo significativa nos dois grupos. Gliclazida apresentou melhor resposta nas glicemias de jejum e pÃs-prandial e as duas drogas melhoraram a glicada de forma semelhante. Vildagliptina obteve melhora glicÃmica com menor necessidade de insulina e com menor HOMA-IR e foi mais efetiva em reduzir a inflamaÃÃo e em melhorar o estresse oxidativo.

Abstract:
O efeito de refeições ricas em carboidratos e lipídeos sobre a sensibilidade à insulina foi avaliado. Além disso, investigou-se o conteúdo da proteína GLUT4 em músculo esquelético e tecido adiposo branco. Ratos foram realimentados por 1, 2, 4 e 6 horas com: refeição balanceada (B); rica em carboidrato (C) e rica em lipídeo (L). O índice glicose/insulina revelou que C e L apresentavam resistência à insulina 2 horas após o início da ingestão. No teste de tolerância à insulina, uma redução (~47%) na sensibilidade à insulina foi observada em C após 2 e 4 horas de realimentação. O teste de tolerância à glicose confirmou a resistência à insulina em C e L após 2 horas de ingestão. Não houve alteração no conteúdo de GLUT4, nos momentos em que se verificou alteração na sensibilidade à insulina. Tais resultados indicam que, em ratos, refeições não balanceadas (alto teor de carboidrato ou alto teor de lipídeo), induzem menor sensibilidade à insulina 2 horas após o início da ingestão, e este fenômeno não envolve alterações no conteúdo de GLUT4 nos tecidos avaliados.<br>The effect of high carbohydrate and fat meals on the insulin sensitivity was evaluated. Furthermore, it was investigated the content of GLUT4 protein on the skeletal muscle and white adipose tissue. Rats were refed for 1, 2, 4 and 6 hours with: balanced meal (B); high carbohydrate meal (C) and high fat meal (L). The glucose/insulin index shows that C and L meals exhibited insulin resistance after 2 hours of ingestion. In the insulin tolerance test, a reduction (~47%) in the insulin sensitivity was observed in C group after 2 and 4 hours of refeeding. The glucose tolerance test confirmed the insulin resistance in C and L-groups after 2 hours of ingestion and such phenomena did not involve alterations in the GLUT4 content on both skeletal muscle or white adipose tissue.

O objetivo deste estudo foi avaliar as concentrações plasmáticas de estradiol, inibina, FSH e LH em éguas superovuladas com eFSH e EPE (em doses constantes ou decrescentes): antes, durante e imediatamente após o término da administração hormonal. Foram utilizadas seis éguas as quais foram submetidas aos seguintes tratamentos: Grupo I (GI) recebeu 25mg de EPE intramuscular (I.M.), Grupo II (GII) foram tratadas com EPE (I.M.) em doses decrescentes. No Grupo IIl (GIII), o EPE foi substituído por 12,5mg de eFSH (I.M.). O Grupo IV (GIV) foi utilizado como controle. Sangue foi coletado diariamente para avaliar as concentrações hormonais por radioimunoensaio. As coletas foram iniciadas dois dias antes da primeira aplicação medicamentosa, encerrando 48horas após a última ovulação do estro. As diferenças nos níveis plasmáticos hormonais foram comparadas utilizando-se o ANOVA, sendo considerada diferença estatística quando P0,05. O número médio de ovulações foi de 3,3; 4,8; 5,0 e 1,0 nos grupos 1 a 4, respectivamente, tendo sido maior (P<0,05) em todos os grupos superovulados em relação ao controle. As concentrações de estrógeno, inibina, FSH e LH não diferiram entre os grupos durante o período 1. No período 2, os valores de estradiol foram maiores (P<0,05) em éguas sob tratamento superovulatório com eFSH (GIII) 29,52pg/ml, os grupos I e II, apresentaram valores intermediários (GI: 16,75pg/ml, GII: 16,26pg/ml) e o GIV (7,78pg/ml) menor estatisticamente aos demais. Foi verificada uma correlação altamente positiva (R=0,615; P=0,01) entre os valores de estradiol e inibina, os quais, durante os tratamentos, foram maiores (P<0,05)...<br>Satisfactory superovulatory responses have been reported in mares with the use of equine pituitary extract (EPE) and purified equine FSH (eFSH), however embryo recovery rates are still inconsistent. Recent studies demonstrated that the superovulatory treatment disturbs oocyte maturation and transport. The main objective of the present study was to evaluate the plasmatic concentrations of estradiol, inhibin, FSH and LH in super ovulated mares treated with eFSH and EPE (with constant or decreasing doses): Six mares were used in a crossover experiment in four different groups: Group I mares received 25mg of EPE every twelve hours, Group II received EPE in decreasing doses and Group IlI mares received 12.5mg of eFSH (IM). Group IV served as control. Embryo flushes were performed 8 days pos ovulation. Blood was collected daily to evaluate hormonal concentrations through radioimuneassay, starting two days before the first administration, until 48 hours post-ovulation. Mean and daily hormonal concentrations of estradiol, inhibin, FSH and LH were evaluated. Hormonal concentrations were submitted to statistical analysis using ANOVA. Significance levels were set as P<0.05. Mean number of ovulations was 3.3; 4.8; 5.0 and 1.0 in groups 1 to 4, respectively, being higher (P<0.05) in all super ovulated groups, when compared to control. Concentrations of estrogen, inhibin, FSH and LH did not differ between groups during the first period. In period 2, estradiol levels were higher (P<0.05) in mares with superovulatory treatment eFSH (GIII) 29.52pg/ml, the groups GI and GII (GI: 16.75pg/ml, GII: 16.26pg/ml) values intermediary, and GIV (7.78pg/ml) levels lower. A positive correlation between estradiol levels and number of follicles ≥30mm (R=0.917; P<0.05), recovered embryos (R=0.945; P<0.05) and ovulations (R=0.75; P<0.05) was identified in super ovulated mares. A positive correlation (R=0.615; P=0.01) between estradiol and inhibin concentrations.

Abstract:
Lymphoid neoplasms are a heterogeneous group of cancers characterized by the neoplastic or clonal proliferation of lymphoid cells in different stages of differentiation. The incidence rate of these neoplasms has seen a rise in some western countries since the 1970s and it seems to have reached a plateau during the last decade. Incidence rates are higher in men than in women for most lymphoma subtypes; however, the causes explaining these differences by sex are unknown. We hypothetised that hormonal factors could have a role in lymphoma etiology. The general aim of this thesis was to assess the risk of lymphoid neoplasms in relation to reproductive factors and occupational exposure to endocrine disruptors. We used different studies and populations to evaluate our hypothesis: the EpiLymph study, the InterLymph consortium, the International Multiple Myeloma Consortium, and a systematic review. As well, we developed a new tool to estimate occupational exposures to a specific type of endocrine disruptors. We observed contradictory findings across studies and lymphoma subtypes concerning the association between lymphoma and parity, as well as hormonal contraceptives. We observed inverse associations between postmenopausal hormone therapy and lymphoma, although we noticed in our systematic review that cohort studies usually found null associations. We observed associations with lymphoma and prolonged (≥30 years) occupational exposure to endocrine disrupting chemicals, in particular for multiple myeloma and chronic lymphocytic leukemia. Associations were observed between lymphoma and prolonged occupational exposures to organic solvents, pesticides, brominated flame retardants, alkylphenolic compounds, and metals. To further explore the associations with alkylphenolic compounds, we developed a job-exposure matrix on these compounds considering relevant changes in use over time. In conclusion, our results indicate that reproductive factors and exogenous hormone use are unlikely to play a role in lymphomagenesis. The associations between occupational exposure to endocrine disrupting chemicals and lymphoma need to be further explored in studies using a more detailed exposure assessment.

Em função da cinética da espermatogênese e do tipo de reprodução (contínua ou sazonal), o epitélio germinativo pode se desenvolver de forma constante ou sofrer modificações cíclicas durante o ciclo reprodutivo. O presente trabalho teve como objetivo analisar, através de parâmetros histológicos, ultra-estruturais e imuno-histoquímicos, as alterações do epitélio germinativo em duas espécies de ciclos reprodutivos diferentes (Serrasalmus spilopleura - reprodução contínua e Pimelodus maculatus - reprodução sazonal) e correlacioná-las com as células esteroidogênicas e as células gonadotrópicas, no intuito de melhor compreender a fisiologia reprodutiva dos teleósteos tropicais de água doce. Em S. spilopleura, o epitélio germinativo organiza-se em cistos basais de espermatogônias e cistos apicais de células germinativas em desenvolvimento mais avançado. As espermatogônias basais apresentam intensa atividade proliferativa durante todo o ciclo reprodutivo. As células de Sertoli proliferam no intuito de acompanhar o crescimento dos cistos e aumentar o tamanho do testículo a cada ciclo reprodutivo. Quanto à esteroidogênese, a atividade 3"beta"-HSD é maior na região espermatogênica do que na região relacionada à espermiação. Em relação às células gonadotrópicas, as células LH são requeridas durante a espermatogênese e na espermiação, enquanto que as células FSH são mais ativas na liberação dos espermatozóides e provavelmente na proliferação das células sle Sertoli. Em P. maculatus, o epitélio germinativo passa por modificações morfofuncionais, categorizadas em quatro classes reprodutivas: regredida, maturação, maturação final e regressão. A proliferação das espermatogônias é intensa durante a maturação e depois diminui na maturação final e regressão. As células de Sertoli proliferam em maior freqüência na classe regredida, o que contribui para o crescimento do testículo durante o ciclo reprodutivo. A atividade 3"beta"-HSD é intensa na maturação e na maturação final, e depois diminui na regressão e na classe regredida. Os processos de espermatogênese e liberação de espermatozóides são dependentes de andrógenos. As células LH e FSH estão em maior quantidade durante a maturação e a maturação final, e depois diminuem na regressão e na classe regredida. A espermatogênese e a liberação dos espermatozóides são mais dependentes da atividade gonadotrópica do que os processos de proliferação espermatogonial e regressão testicular<br>Abstract: In teleost fish, the morphofuncional unit of the spermatogenesis is the spermatocyst or cyst, which are constituded by synchronous developmental germ cells surrounded by Sertoli cells. Depending on the spermatogenesis kinetics and the type of reproduction (continuous/non-seazonal or seazonal), the germinal epithelium may be constant or undego cyclical changes during the reproductive cyc1e. The aim, of the present work is to analyze, through histological, ultrastructural and immunohistochemical evaluation, the changes of the germinal epithelium in two different reproductive cycles (Serrasalmus spilopleura continuous/non-seazonal reproduction and Pimelodus maculatus - seazonal reproduction) and to correlate them with steroidogenic cells and gonadotropic cells, for a better knowledge on the reproductive physiology of the tropical fresh water teleosts. In S. spilopleura, the germinal epithelium was organized in basal cysts of spermatogonia and apical cysts of more advanced developmental germ cells. The basal spermatogonia had intense proliferative activity during the whole reproductive cycle. Sertoli cells proliferation occured in order to accompany the cyst growth and to increase the testis size during each reproductive cycle. For steroidogenic activity, 3"beta"-HSD was stronger in the spermatogenic area than in the area related to spermiation. In re1ation to gonadotropic cells, LH cells were requ~sted during spermatogenesis and spermiation, whereas FSH cells were more active in spermatozoa liberation and probably in the Sertoli celI proliferation. In P. maculatus, the germinal epithelium undergoes morphological changes, classified in 4 reproductive classes: regressed, maturation, final maturation and regression. The spermatogonial prolif~ration was intense during maturation and decreased in final maturation and regression. Sertoli cells proliferated more in regressed class, contributing to testis growth during the reproductive cycle. The 3"beta"-HSD activity was strong in maturation and final maturation, and decreased in regression and in regressed class. Then, spermatogenesis processes and spermatozoa release are dependent of androgenic. LH and FSH cells were in large number during maturation and final maturation, and decreased in regression and in regressed class. Abstract:
O GLUT4 (gene Slc2<font face=\"symbol\">a4) é responsável pela captação de glicose sob estímulo insulínico, e alterações na sua expressão se relacionam à resistência à insulina (RI). Variações na concentração de estradiol (E2) estão relacionadas a RI e menor expressão deste transportador, mecanismo que pode ser mediado pelo fator transcricional NF<font face=\"symbol\">k-B, um repressor de Slc2<font face=\"symbol\">a4. Avaliou-se em células 3T3-L1 a regulação da expressão de Slc2<font face=\"symbol\">a4/GLUT4, a atividade de ligação de NF<font face=\"symbol\">k-B e a captação de glicose pelo E2 e o papel de ESR1 (isoforma 1 do receptor de E2) nesta regulação. Tratou-se as células por 1 dia com E2 e PPT (agonista de ESR1). O PPT aumentou a expressão de Slc2<font face=\"symbol\">a4/GLUT4 na ausência ou presença de E2 bem como a captação de glicose e diminuiu a atividade de ligação de NF<font face=\"symbol\">k-B. Os resultados apresentados demonstram que o E2, atuando via ESR1 aumenta a expressão de Slc2<font face=\"symbol\">a4/GLUT4, efeitos estes parcialmente mediados por NF<font face=\"symbol\">k-B, resultando em alteração na captação de glicose.<br>GLUT4 (gene Slc2<font face=\"symbol\">a4) is responsible by insulin-induced glucose uptake and alterations in its expression are related to insulin resistance (IR). Variability in estradiol levels (E2) is related with IR and lower glucose transporter expression and this mechanism can be mediated by transcriptional factor NF<font face=\"symbol\">k-B, which is an Slc2<font face=\"symbol\">a4 repressor. Our aim was to evaluate in 3T3-L1 adipocytes the role of E2 in Slc2<font face=\"symbol\">a4/GLUT4 expression, NF<font face=\"symbol\">k-B binding activity and glucose uptake as well as the ESR1 (estrogen receptor 1) role in this regulation. For this, 3T3-L1 cells were treated for 1 day with E2 and PPT (ESR1-agonist). PPT enhanced Slc2<font face=\"symbol\">a4/GLUT4 expression in the absence or presence of E2 as well as the glucose uptake and decreased NF<font face=\"symbol\">k-B binding activity. Our results show that E2 increases Slc2<font face=\"symbol\">a4/GLUT4 expression via ESR1 and this effect is partially mediated by NF<font face=\"symbol\">k-B, and allow parallel changes in glucose uptake.

Orientador: Marco Antonio Alvarenga<br>Resumo: Respostas superovulatórias satisfatórias têm sido reportadas em éguas com o uso do Extrato de Pituitária Eqüina (EPE) e FSH purificado eqüino (eFSH), contudo as taxas de recuperação embrionária permanecem inconsistentes. O objetivo deste estudo foi avaliar as concentrações plasmáticas de estradiol, inibina, FSH e LH em éguas superovuladas com eFSH e EPE (em doses constantes ou decrescentes): antes, durante e imediatamente após o término da administração hormonal. Foram utilizadas seis éguas as quais foram submetidas aos seguintes tratamentos: Grupo I (GI) recebeu 25mg de EPE intramuscular (I.M.), Grupo II (GII) foram tratadas com EPE (I.M.) em doses decrescentes. No Grupo IIl (GIII), o EPE foi substituído por 12,5mg de eFSH (I.M.). O Grupo IV (GIV) foi utilizado como controle. Sangue foi coletado diariamente para avaliar as concentrações hormonais por radioimunoensaio. As coletas foram iniciadas dois dias antes da primeira aplicação medicamentosa, encerrando 48horas após a última ovulação do estro. As diferenças nos níveis plasmáticos hormonais foram comparadas utilizando-se o ANOVA, sendo considerada diferença estatística quando P0,05. O número médio de ovulações foi de 3,3; 4,8; 5,0 e 1,0 nos grupos 1 a 4, respectivamente, tendo sido maior (P<0,05) em todos os grupos superovulados em relação ao controle. As concentrações de estrógeno, inibina, FSH e LH não diferiram entre os grupos durante o período 1. No período 2, os valores de estradiol foram maiores (P<0,05) em éguas sob tratamento superovulatório com eFSH (GIII) 29,52pg/ml, os grupos I e II, apresentaram valores intermediários (GI: 16,75pg/ml, GII: 16,26pg/ml) e o GIV (7,78pg/ml) menor estatisticamente aos demais. Foi verificada uma correlação altamente positiva (R=0,615; P=0,01) entre os valores de estradiol e inibina, os quais, durante os tratamentos, foram maiores (P<0,05)... Recent studies demonstrated that the superovulatory treatment disturbs oocyte maturation and transport. The main objective of the present study was to evaluate the plasmatic concentrations of estradiol, inhibin, FSH and LH in super ovulated mares treated with eFSH and EPE (with constant or decreasing doses): Six mares were used in a crossover experiment in four different groups: Group I mares received 25mg of EPE every twelve hours, Group II received EPE in decreasing doses and Group IlI mares received 12.5mg of eFSH (IM). Group IV served as control. Embryo flushes were performed 8 days pos ovulation. Blood was collected daily to evaluate hormonal concentrations through radioimuneassay, starting two days before the first administration, until 48 hours post-ovulation. Mean and daily hormonal concentrations of estradiol, inhibin, FSH and LH were evaluated. Hormonal concentrations were submitted to statistical analysis using ANOVA. Significance levels were set as P<0.05. Mean number of ovulations was 3.3; 4.8; 5.0 and 1.0 in groups 1 to 4, respectively, being higher (P<0.05) in all super ovulated groups, when compared to control. Concentrations of estrogen, inhibin, FSH and LH did not differ between groups during the first period. In period 2, estradiol levels were higher (P<0.05) in mares with superovulatory treatment eFSH (GIII) 29.52pg/ml, the groups GI and GII (GI: 16.75pg/ml, GII: 16.26pg/ml) values intermediary, and GIV (7.78pg/ml) levels lower. CoordenaÃÃo de AperfeiÃoamento de Pessoal de NÃvel Superior<br>Thyroid hormones regulate the body metabolism, with changes in cellular oxygen usage profile. As a result, there may be production of reactive oxygen species that induce oxidative stress and cellular dysfunction in various tissues. The study aims to evaluate the levels of oxidative stress in women diagnosed with primary hypothyroidism. The study included 25 female patients from the Endocrinology Clinic of the University Hospital Walter CantÃdio (HUWC) in Fortaleza, CearÃ, with primary hypothyroidism. Blood samples of patients were collected in the hypothyroid condition and euthyroid. We evaluated the thyroid hormone function, biochemical and haematological profile of patients, beyond the parameters of oxidative stress from measurements of malondialdehyde, nitrite/nitrate, superoxide dismutase and catalase. We observed a significant reduction of total cholesterol, HDL, LDL and triglycerides and leukocytes after correction of hypothyroidism. In relation to oxidative stress, there was a significant reduction of catalase activity (50,12 Â12,75 vs 63,77  23,8 atv/min; p = 0.03) when we compared two moments. There was no change in nitrite / nitrate concentration (p = 0.18), malonaldehyde (p = 0.67) and superoxide dismutase activity (p = 0.93) when we analyzed the whole group. There was no significant difference in the concentration of malonaldehyde in both situations (P = 0.67). The concentration of nitrite / nitrate was increased in cases of hypothyroidism with metabolic syndrome (34.07  10.89 Vs 30.12  10.12, p = 0.03). These data suggest that hypothyroidism alters oxidative stress, and catalase activity is the most sensitive marker. When hypothyroidism occurs with metabolic syndrome, NO levels increase, suggesting different mechanisms in oxidative stress in these two clinical conditions.<br>Os hormÃnios tireoidianos regulam o metabolismo corporal, com mudanÃa no perfil de utilizaÃÃo de oxigÃnio celular. Como consequÃncia, pode ocorrer a produÃÃo de espÃcies reativas de oxigÃnio que induzem o estresse oxidativo e disfunÃÃes celulares em diversos tecidos. O estudo tem como objetivo avaliar os nÃveis de estresse oxidativo em mulheres diagnosticadas com hipotireoidismo primÃrio. Participaram do estudo 25 pacientes do sexo feminino, atendidas no ambulatÃrio de Endocrinologia do Hospital UniversitÃrio Walter CantÃdio (HUWC) em Fortaleza-CearÃ, com hipotireoidismo primÃrio. As amostras de sangue dos pacientes foram coletadas na condiÃÃo de hipotireoidismo e em eutireoidismo. Avaliou-se a funÃÃo hormonal tireoideana, o perfil bioquÃmico e hematolÃgico dos pacientes, alÃm dos parÃmetros do estresse oxidativo a partir das dosagens de malonaldeÃdo, nitrito/nitrato, catalase e superÃxido dismutase. Observamos reduÃÃo significativa do colesterol total, LDL e triglicÃrideos apÃs a correÃÃo do hipotireoidismo. Em relaÃÃo ao estresse oxidativo, houve aumento da atividade da catalase (50,25  12,75 vs 63,77  23,8 atv/min; p = 0,03) quando comparamos os dois momentos. NÃo houve alteraÃÃo no nitrito/nitrato (p=0,18), malonaldeÃdo (p=0,67) e na atividade da superÃxido dismutase (p=0,93) quando analisamos o grupo inteiro. A concentraÃÃo de nitrito/nitrato estava aumentada nos casos de hipotireoidismo com sÃndrome metabÃlica (34,07  10,89 vs 30,12 Â10,12; p=0,03). Esses dados sugerem que o hipotireoidismo altera o estresse oxidativo, sendo que a atividade da catalase à o marcador mais sensÃvel. Quando o hipotireoidismo està associado à sÃndrome metabÃlica, os nÃveis de NO aumentam, o que sugere mecanismos diferentes no estresse oxidativo nessas duas condiÃÃes clÃnicas.

Abstract:
Introduction: Psychiatric disorders occurring during endocrine dysfunction and, conversely, endocrine dysfunctions associated with mental disorders were the emergence of a new discipline, psychoendocrinology. Psychiatric disorders correlated with endocrine diseases are defined as psychopathological manifestations of variable intensity and clinical symptomatology, determined by complex psycho-neuro-endocrinological interrelationships. Defining elements consist of the association between diagnosis of mental disorders and specific symptoms for endocrine dysfunction. Methods: We conducted a prospective one-year study (January 2018 - December 2018 on 112 patients hospitalized in the Clinic of Psychiatry who also had an endocrinological comorbidity. We investigated the frequency and severity of psychoendocrinological associations by studying a number of demographic and clinical items. Results: The results showed that the highest incidence belongs to thyroid disorder - 55.36%, followed by gonadal disorders - 24.11%, and, rarely, pituitary diseases and diabetes. Hyperthyroidism was associated most frequently with manic episodes, while unipolar depression prevailed in patients with hypothyroidism. In gonadal disorders, present in majority in female patients (secondary amenorrhea, menopause or erectile dysfunction in males), depression accompanied by anxiety, often severe in intensity, was the most frequent psychiatric diagnosis. Psychotic disorders were met in a smaller number of cases, especially in patients with long history of endocrine disorders and instability of biological constants. Conclusions: We may state that affective disorders are the most frequent nosologically category in patients with endocrine dysfunctions. It requires a better collaboration between specialists in endocrinology and psychiatry, to highlight the determinants which contribute to the development of psychopathological manifestations in endocrine diseases and to individualize the treatment depending on cases’ particularities.

Introdução: A distocia é definida como dificuldade para expelir os fetos pelo canal do parto. Esta afecção é mais frequente em cadelas do que em gatas. Normalmente, a distocia pode ocorrer devido origem materna ou fetal e de modo geral, raças de porte pequeno ou braquicefálicas possuem maior predisposição a afecção. Objetivos: Neste contexto, o objetivo deste estudo é revisar as principais causas da distocia em pequenos animais, métodos diagnósticos e terapêutica. Materiais e métodos: Para elaboração da revisão de literatura, foram utilizados trabalhos e artigos científicos retirados do Google Acadêmico dos últimos 6 anos. Resultados: A distocia de origem materna é recorrente principalmente em fêmeas primíparas ou com múltiplos fetos. Dentre as causas primordiais da distocia de origem materna destaca-se a inércia uterina (primária e secundária), estreitamento das vias moles e duras, torção uterina e contrações excessivas sem sucesso. Já a distocia de origem fetal pode ser acarretada por deficiência de corticosteroides adrenais do feto, tamanho do feto, gestação prolongada, ascites, anasarca e hidrocefalia ou alterações na estática fetal. Por se tratar de afecção rotineira na clínica de pequenos animais, o diagnostico ocorre por meio de exames radiográficos ou ultrassonográficos, palpação abdominal e exame digital vaginal. O gluconato de cálcio a 10% é capaz de promover aumento da força uterina. O tratamento divide-se em medicamentoso ou por intervenção cesariana, sendo que os fármacos podem ser administrados caso a fêmea esteja em boas condições clínicas e apresente inércia uterina secundária. Contudo, em situações de falha de estática fetal onde a correção não é possível, ou falha na indução da contração uterina, a cesariana é indicada. Conclusão: O conhecimento da fisiologia e endocrinologia da fêmea e sua gestação é de sua importância para que o diagnóstico e tratamento corretos sejam efetuados, garantindo assim, a sobrevida da mãe e fetos.

Ovariectomy is the most popular and reliable method of reproductive control in dogs. There are many health benefits to spaying. If spayed early enough, the risk of mammary gland neoplasia de-creases significantly, and ovariectomy also eliminates the possibility of pyometra. On the other hand, spaying and the absence of gonadal hormones is also associated with an increased prevalence of many health problems. Urinary incontinence, various orthopaedic disorders, endocrinologic disor-ders such as hypothyroidism, many neoplastic disorders (including hemangiosarcoma, osteosar-coma, lymphoma) and immune-mediated disorders are among the most common health problems associated with early spaying and spaying in general. Review question / Objective: To identify and review studies published in the last ten years, presenting the efficacy and outcomes of EEA and TCA for patients with cranio-pharyngiomas. Eligibility criteria: Studies meeting the following criteria were included: (a) retrospective and prospective studies and (b) observational studies (i.e., cross-sectional, case-control, case-series). Abstract:
Objectives. To assess the use, effectiveness, and implementation of telehealth-supported provider-to-provider communication and collaboration for the provision of healthcare services to rural populations and to inform a scientific workshop convened by the National Institutes of Health Office of Disease Prevention on October 12–14, 2021. Data sources. We conducted a comprehensive literature search of Ovid MEDLINE®, CINAHL®, Embase®, and Cochrane CENTRAL. We searched for articles published from January 1, 2015, to October 12, 2021, to identify data on use of rural provider-to-provider telehealth (Key Question 1) and the same databases for articles published January 1, 2010, to October 12, 2021, for studies of effectiveness and implementation (Key Questions 2 and 3) and to identify methodological weaknesses in the research (Key Question 4). Additional sources were identified through reference lists, stakeholder suggestions, and responses to a Federal Register notice. Review methods. Our methods followed the Agency for Healthcare Research and Quality Methods Guide (available at https://effectivehealthcare.ahrq.gov/topics/cer-methods-guide/overview) and the PRISMA reporting guidelines. We used predefined criteria and dual review of abstracts and full-text articles to identify research results on (1) regional or national use, (2) effectiveness, (3) barriers and facilitators to implementation, and (4) methodological weakness in studies of provider-to-provider telehealth for rural populations. We assessed the risk of bias of the effectiveness studies using criteria specific to the different study designs and evaluated strength of evidence (SOE) for studies of similar telehealth interventions with similar outcomes. We categorized barriers and facilitators to implementation using the Consolidated Framework for Implementation Research (CFIR) and summarized methodological weaknesses of studies. Results. We included 166 studies reported in 179 publications. Studies on the degree of uptake of provider-to-provider telehealth were limited to specific clinical uses (pharmacy, psychiatry, emergency care, and stroke management) in seven studies using national or regional surveys and claims data. They reported variability across States and regions, but increasing uptake over time. Ninety-seven studies (20 trials and 77 observational studies) evaluated the effectiveness of provider-to-provider telehealth in rural settings, finding that there may be similar rates of transfers and lengths of stay with telehealth for inpatient consultations; similar mortality rates for remote intensive care unit care; similar clinical outcomes and transfer rates for neonates; improvements in medication adherence and treatment response in outpatient care for depression; improvements in some clinical monitoring measures for diabetes with endocrinology or pharmacy outpatient consultations; similar mortality or time to treatment when used to support emergency assessment and management of stroke, heart attack, or chest pain at rural hospitals; and similar rates of appropriate versus inappropriate transfers of critical care and trauma patients with specialist telehealth consultations for rural emergency departments (SOE: low). Studies of telehealth for education and mentoring of rural healthcare providers may result in intended changes in provider behavior and increases in provider knowledge, confidence, and self-efficacy (SOE: low). Patient outcomes were not frequently reported for telehealth provider education, but two studies reported improvement (SOE: low). Evidence for telehealth interventions for other clinical uses and outcomes was insufficient. We identified 67 program evaluations and qualitative studies that identified barriers and facilitators to rural provider-to-provider telehealth. Success was linked to well-functioning technology; sufficient resources, including time, staff, leadership, and equipment; and adequate payment or reimbursement. Some considerations may be unique to implementation of provider-to-provider telehealth in rural areas. These include the need for consultants to better understand the rural context; regional initiatives that pool resources among rural organizations that may not be able to support telehealth individually; and programs that can support care for infrequent as well as frequent clinical situations in rural practices. An assessment of methodological weaknesses found that studies were limited by less rigorous study designs, small sample sizes, and lack of analyses that address risks for bias. A key weakness was that studies did not assess or attempt to adjust for the risk that temporal changes may impact the results in studies that compared outcomes before and after telehealth implementation. Conclusions. While the evidence base is limited, what is available suggests that telehealth supporting provider-to-provider communications and collaboration may be beneficial. Telehealth studies report better patient outcomes in some clinical scenarios (e.g., outpatient care for depression or diabetes, education/mentoring) where telehealth interventions increase access to expertise and high-quality care. In other applications (e.g., inpatient care, emergency care), telehealth results in patient outcomes that are similar to usual care, which may be interpreted as a benefit when the purpose of telehealth is to make equivalent services available locally to rural residents. Most barriers to implementation are common to practice change efforts. Methodological weaknesses stem from weaker study designs, such as before-after studies, and small numbers of participants. The rapid increase in the use of telehealth in response to the Coronavirus disease 2019 (COVID-19) pandemic is likely to produce more data and offer opportunities for more rigorous studies.
