Full textThe structure of biological tissues on submicron scale can be efficiently caracterized by optical Mueller polarimetry, which gives this technique a great interest for an aid to early medical diagnosis of diverse pathologies affecting the extracellular matrix, such as some cancers. In order to extend the application field to the caracterization of internal organs, Mueller polarimetry can be developed through an optical fiber as an endoscope, thanks to a two wavelengths differential measurement method (called“bichromatic method”) developed at the Xlim institute. The work carried out in this thesis aims to design and realize a fiber based Mueller polarimetric endomicroscope, based on the bichromatic method, meeting the requirements of in vivo endoscopic imaging. In the first place, we develop an accelerated data acquisition procedure allowing to reduce from several hours to a few seconds the realization time of a Mueller image. Thereafter, we develop a microprobe located at the distal tip of the endoscopic fiber, with dimensions sufficiently small to be used in endoscopy (length : 30 mm, diameter : 3 mm), including a fast scanning system (microscanner) done by the mechanical resonance of the fiber tip. The miniaturized dichroic filter required to implement the bichromatic method is a fiber Bragg grating photowritten into the fiber core. The design, the fabrication, the spectral and polarimetric characterizations of this key component are the subject of an in-depth study. Finally, after the detailed description of the microprobe, we present and comment on the Mueller polarimetric images of a known waveplate and a biological sample (type I collagen) realized with the endomicroscope equiped with this microprobe (1 image/2s, 250 x 250pixels, field of view : 400μm, resolution : 4μm). These images confirm the validity of the realized instrument. At last, we propose and experiment a solution to determine spatial depolarization through the endoscopic fiber, based on the decomposition of the sum of Mueller matrices of neighbouring pixels

Full textIntroduction - Bariatric surgery, despite its effectiveness, remains morbid. Digestive endoscopy has developed bariatric techniques. The realization of an endoscopic by-pass (EBP) is our research topic. We present its metabolic and technical results for a transition to humans. Materials and methods - The studies were conducted at the CERC from 2012 to 2019. The initial phase resulted in an experimental AGJ model by lumen apposing stent (Axios® stents) with metabolic evaluation on normal minipig and the first humans’ cases. In the 2nd part, the metabolic validation was performed on 24 obese Yucatan minipigs with 4 groups: gastro-jejunal anastomosis (GJA) alone, full BPE, surgical bypass, and sham procedure. The equipment improvements included: the apposing stent, endoscopic pyloric closure, grasping and selection of the bypassed limb. Results - Material: We have improved or acheuved: i) the reliability of the anastomosis by modifying the stent (bi-concave shape); ii) the pyloric closure by creating a central "twist" of the Axios® stent; iii) the grasping of the limb by creating a dedicated atraumatic forceps; iv) its selection at 150cm from the pylorus by introducing a luminous catheter, identified during flexible peritoneoscopy. Metabolic validation: results are pending, but comparison of endoscopy groups showed that duodenal exclusion allows for greater weight loss than GJA alone. Conclusion - This experimental research allows the metabolic and technical validation of an exclusive experimental endoscopic bypass model. A transition to man is planned for 2021

Full textIntroduction - The aim of this study was to develop an endoscopic experimental model of NOTES bariatric surgery using a simple, reproducible technique and to validate it both clinically (feasibility, tolerance) and metabolically (pathophysiology and action plan). Materials and Methods - The studies were conducted within the CERC and INSERM UMRS 1260 unit. The animal was the pig, aged 3 to 4 months, healthy and not obese. Endoscopies were performed using a dual channel gastroscope operator and the endoscopy equipment currently available. Several development stages were set up from the development of an animal model of gastrointestinal anastomosis using an exclusive endoscopic NOTES procedure to the evaluation of the metabolic impact of gastric bypass bariatric model described using the gastrointestinal anastomosis by luminal apposing stent technique. Results - The gastro-jejunal anastomosis model chosen for its feasibility and safety was developed by luminal apposing gastric and small bowel digestive lumens with a specific metallic covered stent and NOTES technique. The experimental evaluation of the metabolic gastric bypass using this procedure allowed to observe a significant improvement in insulin resistance none mediated by GLP-1 and incretin effect. Conclusion - The concept of anastomosis with luminal apposing stent and exclusive NOTES technique is established and endoscopic bypass model using this procedure is feasible pretend to present a significant metabolic impact

Full textWe present the development of a fiber-based, scanning-tip endoscope for nonlinear imaging. The flexible endoscope developed in the framework of this thesis, has an outer diameter of 2.2 mm. The innovation of the technology lies in its high scanning speed (8 frames/second) and large field-of-view (FOV>300 microns). The miniaturized endoscope enables high resolution coherent anti-Stokes Raman (CARS), 2photon fluorescence and second harmonic generation (SHG) imaging contrasts. We show the potential of the endoscope to image different biological samples and specimens with and without endogenous labeling. We have obtained CARS images of fresh, label-free human colon ex-vivo and SHG images of collagen from rat tail tendon. Finally, we managed to perform in-vivo imaging of calcium flow indicators from hippocampal neurons labeled with Green Fluorescent Protein (GFP). We are convinced that this imaging device will have a strong impact on biomedical imaging

Full textLa NOTES (Natural Orifice Translumenal Endoscopic Surgery) est une technique chirurgicale qui consiste à aborder la cavité péritonéale à l’aide d’un endoscope souple, à travers les orifices naturels puis à travers la paroi du tube digestif ou des voies uro-génitales. La maîtrise d’un abord per-oral et transgastrique (disponible pour les deux sexes, par opposition à l’abord transvaginal) est un objectif hautement souhaitable pour le développement de cette technique. Les travaux présentés dans cette thèse démontrent la faisabilité d’interventions endoscopiques souples par voie transgastrique sur modèle porcin vivant. Ils mettent en évidence : (1) l’intérêt de protocoles anti-infectieux avant une NOTES transgastrique ; (2) la nécessité de réaliser un pneumopéritoine avant création d’un abord transgastrique pour prévenir la survenue de plaies d’organes de voisinage; (3) la meilleure tolérance d’une dilatation au ballon plutôt que d’une incision au sphinctérotome lors de la création de la voie d’abord transgastrique ; (4) la possibilité d’introduire de façon stérile par voie transgastrique du matériel prothétique pour un usage intrapéritonéal (telle qu’illustrée par une technique de cure d’éventration ombilicale par NOTES transgastrique) ; (5) le caractère encore imparfait et hautement dépendant des avancées technologiques des méthodes endoscopiques de création (par exemple à l’aide d’un laser de 2 µm de longueur d’onde) et de fermeture (par exemple par clips, points en T, agrafes) de la voie d’abord transgastrique ; et (6) la faisabilité de tests d’étanchéité mini-invasifs et hautement spécifiques (utilisant par exemple de l’hydrogène dilué)

However, it requires a high level of technical skill and a level of training that is difficult to apply outside Japan. The development and use of animal models have allowed our team to train in ESD, to improve the device and strategies allowing a democratization of the technique initially limited to rare expert centers.This strategy can be extrapolated to many new endoscopic procedures where the dexterity required is inversely proportional to recruitment and the supervisors and industrialists will have to integrate the importance of these animal research and training structures in their support of academic institution in digestive endoscopy

Full textVision-based metrology devices generally embed stereoscopic sensors or active measurement systems. Most of the passive 3D reconstruction techniques (Structure-from-Motion, Shape from-Shading) adapted to monocular vision suffer from scale ambiguity. Because the processing of image acquisition implies the loss of the depth information, there is an ambiguous relationship between the depth of a scene and the size of an imaged object. This study deals with the estimation of the absolute scale of a scene using passive monofocal vision. Monofocal vision describes monocular system for which optical parameters are fixed. Such optical systems are notably embedded within endoscopic systems used in colonoscopy. This minimally invasive technique allows endoscopists to explore the colon cavity and remove neoplasias (abnormal growths of tissue). Their size is an essential diagnostic criterion for estimating their rate of malignancy. However, it is difficult to estimate and erroneous visual estimations lead to neoplasias surveillance intervals being inappropriately assigned. The need to design a neoplasia measurement system is the core motivation for our study. In the first part of this manuscript, we review state-of-the-art vision-based metrology devices to provide context for our system. We then introduce monofocal optical systems and the specific image formation model used in our study. The second part deals with the main contribution of our work. We first review in detail state of the art DfD (Depth-from-Defocus) and DfF (Depth-from-Defocus) approaches. They are passive computer vision techniques that enable us to resolve scale ambiguity. Our core contribution is introduced in the following chapter. We define the Infocus-Breakpoint (IB) that allows us to resolve scale from a regular video. The IB is the lower limit of the optical system’s depth of field. Our system relies on two novel technical modules: Blur-Estimating Tracking (BET) and Blur-Model Fitting (BMF). BET allows us to simultaneously track an area of interest and estimate the optical blur information. BMF allows us to robustly extract the IB by fitting an optical blur model to the blur measurement estimated by the BET module. For the optical system is monofocal, the IB corresponds to a reference depth that can be calibrated. In the last chapter, we evaluate our method and propose a neoplasia measurement system adapted to the constraints in colonoscopy examination. The last part of this manuscript is dedicated to a prospect of extension of our method by a generative approach. We present, as a preliminary study, a new NRSfM (Non-Rigid Structure-from-Motion) method allowing the scaled Euclidean 3D reconstruction of deformable surfaces. This approach is based on the simultaneous estimation of dense depth maps corresponding to a set of deformations as well as the in-focus color map of the flattened surface. We first review state-of-the-art methods for 3D reconstruction of deformable surfaces. We then introduce our new generative model as well as an alternation method allowing us to infer it

Full textThe diagnosis of gut-inflammatory conditions such as inflammatory bowel disease (IBD), that may lead into colorectal cancer (CRC), is a major public health issue. The emergence of novel imaging-based diagnosis technics helps to improve patients’ following-up but also the growth mechanisms of such conditions. In this context, the present manuscript describes a protocol that includes sequentially endoluminal MRI examination with conventional endoscopy, optical spectroscopy and confocal endomicroscopy. The dedicated developed instrumentation has been first characterized on organic models and then evaluated in vivo on a mouse model of colitis during two longitudinal studies. This project aims to propose methodological developments offering new ways of investigating complex mechanisms on mouse models and that could be further transposed to Human. The first part of this manuscript describes an overview of the colon and rectum anatomy as well as the main characteristics of IBD and CRC. The diagnostic imaging tools that are mainly use in clinical environment for the detection, description and staging of colorectal abnormalities are then presented. The second part describes the development of endoluminal MRI with dedicated coils and the development of optical modalities. In order to assess the potential of the proposed protocol, the different in vivo studies carried out during this project are detailed. Finally, the last part of the document shows and discusses the results obtained for each and cross modalities in this context. In conclusion, bringing new ways of imaging gut diseases to increase the sensitivity of the diagnosis and enhance the morphological description of colorectal abnormalities was demonstrated

Full textEarly diagnosis of a cancerous lesion and complete surgical resection of the diseased areas are both crucial points in order to greatly improve the chances for recovery of a patient. However, early detection of cancer is a very difficult task. It relies on random biopsies of suspicious areas which are not easy to identify at this stage of the disease using conventional imaging techniques (visible imaging, ultrasound, magnetic resonance, X-ray scanner, positron emission tomography). In addition, the correct evaluation of surgical resection margins remains often very difficult or even impossible in some cases.Polarimetric imaging is a promising technique for the early detection of cancerous lesions on the surface of the organs and for a better definition of the resection limits during surgery. Biomedical research activity, conducted within the 'Applied Optics and Polarimetry' team of the LPICM, focuses on the development of Mueller polarimetric imaging systems for improving the management of epithelial cancers, also known as carcinomas, which represent 80-90% of all cancers. In this regard the LPICM leads a project funded by the “Institut National du Cancer (INCa)”, on the use of Mueller polarimetric imaging for improving the management of cervical cancer at different stages of its evolution. At present an extensive series of ex vivo measurements is in progress in three different hospitals of Paris (Institut Gustave Roussy, Kremlin Bicêtre and Institut Mutualiste Montsouris). The final goal of this study is to evaluate the performance of Mueller polarimetric imaging technique in terms of sensitivity and specificity, while using an interpretation of corresponding histology slides by pathologists as a “golden standard” of cancer diagnostics. Ex vivo measurements provide a precise knowledge of the systematic effects which can negatively affect image quality. Hence, the results of this study represent a good starting point for in vivo applications of polarimetric imaging technique. Within the frame of INCa project the analysis of uterine cervix in vivo is planned, using a classical colposcope modified to obtain polarimetric Mueller images.The endoscope is another medical instrument used also to detect cancerous or precancerous lesions in the internal cavities of human body (esophagus, colon, rectum, etc.). The proposed thesis subject consists in developing a Mueller polarimetric rigid endoscope and evaluating its performance in terms of sensitivity and specificity. The work of PhD student will be concerned with instrumentation in optics, acquisition of data, signal processing and statistical evaluation of the performance of technique. Thus, the subject of this thesis is on the interface between physics and medical diagnostics and it shows a strong potential for industrial development with a significant societal impact

Full textThe work presented in this manuscript is part of the ANR project EMMIE. This project aims to develop an innovative multimodal system for the detection of inflammatory lesions in the stomach. To this purpose, a prototype has been developed to be able to acquire NBI endoscopic images and multispectral images during human's antrum exploration. The prototype is made of a standard endoscope and multispectral images.The prototype can acquire two types of data: NBI images and spectra. These two modalities are processed independently. Common image processing features are used to recognize four kind of diseases: active gastritis, chronic gastritis, metaplasia and atrophy. In addition, visual based features have been developed with an experienced endoscopist. These criteria were successfully used to classify endoscopic image patches.NBI images are pre-processed with "intrinsic image decomposition". This decomposition is made by the dichromatic model. We developed two methods to inverse this model, respectively with quadratic programming and deconvolutions. Our algorithms are tested on three datasets. Equivalent or better results are found compare to the state of the art.Acquisitions are also made on mice model. Reflectance spectra have been acquired on resected stomach from control and inflammed mice. A pipeline was build to classify the two groups. The pipeline allowed us to obtain good classification results and to identify two interesting wavelengths range in the near infrared.This pipeline is re-used to process~ extit{in vivo} spectra from human's stomach with the developed acquisition prototype. The aim is to characterize the inflammation and being able to classify spectra according to the disease. Obtained results are compared with biopsies and show good diagnostic prognosis for chronic gastritis and intestinal metaplasia

Full textDiagnostic techniques currently used in gastrointestinal endoscopy do not allow a completeand accurate observation of the small bowel. Endoscopic capsules were designed to solvethis problem, but the doctor cannot control their movement once they are swallowed bypatients. In that context, the work developed in this thesis focuses on the use of magneticfields to manipulate an object (such as a capsule) without contact, on a large workspace.For this, we study magnetic systems with mobile electromagnets. We study first how tomodel the magnetic field created by an electromagnet. We develop an analytical modeltaking into account the ferromagnetic core of electromagnets. Then, we study a genericsystem composed of n electromagnets which can move throughout space. We develop amodel of the system, integrating the mobility of each of the electromagnets. This modelis then linearized, allowing us to introduce a linearizing control. This command allowsindependent management of the movement of each of the electromagnets, and the currentflowing in it. Finally, the model and the command are tested in simulation and on anexperimental device.Keywords: Magnetism
