Full textWilm's tumour (WT) or nephroblastoma is the most frequent cancer of kidney in children (1/10000 births). This tumour is overall of good pronostic. However, 20 to 25% of the tumours resist and relapse without any predictive factor explaining this bad outcome. The only cloned gene, clearly implicated in the tumorigenesis of the nephroblastoma is WT1. The purpose of the work carried out during my thesis was (1) to searchnew genes, particularly those located on the chromosome 16 ; (2) to determine, more clearly, the role of WT1 in the tumorigenesis of WT. The study of chromosome 16 allowed us to highlight the 10% frequency of 16q LOH and the implication of two cadherin genes (E and KSP) wich are down-expressed

A number of hypotheses have been presented to explain the mechanism of action of thalidomide. The deficient children damaged by the drug thalidomide lived without help. The tragedy showed the difficulties in assessing the teratogenic potential of drugs used during pregnancy. We studied seventeen French newspapers, five British newspapers and weeklys of the sixties, and we tried to explain how the readers discovered the tragedy. Nowadays, the chemical is a very good treatment for the AIDS victims and for the lepromatous people. In Brazil and in India, there is today a second generation of thalidomide children

Full textThe notion of authority appears as one of pillars of family's law. The bases of the authority in the family have very evolved all long history period. At first, the authority was bound to the quality of man, then at the existence of marriage to be today based on the kinship. This evolution shows the way of an organization of patriarchal's type in which the power was the expression of a juridical authority on the wife and on the children to a familial organization of equalitarian type in the bosom on which the parental responsability takes its origin in the only one kinship. At the end of this evolution, the authority is above all that of parent's. The authority in the family expresses therefore today for the main point via the concept of parental responsability. Two elements render an account to this authority in the family : the legal power and the legal duty. Henceforth, the parental responsability surpasses all the law family and represents in that a new familial reference

Full textWe compare the communicative behaviour within the duos fathers-infant and mothers-infant within one culture (intracultural comparison) then those between France and the Czech Republic (intercultural comparison). Thus we examined the structural, functional and pragmatic aspects of the parents-child communication. To realise this approach we have chosen the situation of the reading of an illustrated book. We worked with middle-class families. The infants are first born, aged between 30 and 36 months, living with their two parents. The total number of subjects is 28. The children are divided in a balanced way: 14 French and 14 Czech, and in every sample 7 boys and 7 girls. Thus our results indicate, that indeed in the one hand, the culture plays the role of universal factor which determines the way of the communicating in the family and, in the other hand the culture seems to act as a factor of specification which is linked to the country of origin

Full textThis doctorate envisions the question of the function of play in the treatment of children, from a clinical and theoretical point of view. First, three psychoanalytic approaches are put into perspective. The approach pioneered by Donald Woods Winnicott – who made play the paradigm of the psychoanalytic cure – is considered through the notion of playing. We consider the theoretical paradoxes of this notion, as well as the impasses to which it can lead if it is not clearly distinguished from the common notion of play. Next, we unpack two of Melanie Klein's concepts : firstly, her “psycho-analytic play technique,” a discovery that opened up the cure to children, and that exposed the difficulty of reducing play to technè, and secondly, a theory of play conceived of as “personification” whose mechanism, by definition schizo-paranoid, consists in the combination of Spaltung and “projection.” Finally, the Freudian model conceives of play as a formation of the unconscious, just like dreams or faulty acts. For Freud, the game is a space where fantasies are created. We will compare Ernst's and Hans' play with that of Arpad, Sándor Ferenczi's “little chanticleer”, in order to tease out two possible tendencies of play: on the one hand, negation, and on the other hand, disavowal. Following this exposition, we propose a Freudian-inflected tripartite structure of play. According to this classification, we distinguish play based on the mechanism of negation (Verneinung) that we call “trompe-l'oeil” games; play based on disavowal (Verleugnung) that we call “decoy” games; and finally “suppléance” games based on the mechanism of foreclusion (Verwerfung). We maintain that detecting the principal mechanism at play in a child's game is an indispensable moment for receiving him as a patient. A second line of thought, in counterpoint to this triple classification, attempts to describe the “three dimensions” of play. If the first classification divided the notion of play, we now attempt to illuminate play's multiple facets. First of all, we interrogate the “frame” of playing, in other words, its structure. What makes a game a game and what is the clinical interest of this distinction? Next, a phenomenology of the “being-at-play” induces a study of the content of play. Play will be considered along two axes: on the one hand, the game as world-making, and on the other hand, the game as “untamed thought” (Levi-Strauss) with a mythical structure. Our hypothesis is that the determinants of the subject are enunciated in the form taken by the game. Finally, we consider the active dimension of play: the child as playing and not as plaything. In this section, we explore the creative and innovative dimension of playing in contrast with the content of his play, allowing us to move beyond an approach to playing that remains colored, from a clinical point of view, by psychopathology.
