Full textOBJECTIVES 1. Evaluate the oral health status of the patients under allogeneic-HPT. 2. Study the frequency of occurrence of oral GVHD. 3. Study the relationship between the appearance of oral GVHD and variables such as presence of bacterial plaque, periodontal status and rates of caries, absences and fillings. 4. Make a review of the literature on the treatment protocol of oral GVHD. 5. Confirm the need for guidelines of oral hygiene and regular visits to the dentist, pre and post transplant. MATERIAL There were reviewed the medical records and performed oral examinations to 73 patients who had received an allo-HPT for different pathologies. Patients were referred to the unit of special patients of the University Clinic of Dentistry, from the service of oncohematology at “Morales Meseguer" Hospital from October 2000 until June 2008, to assess the oral health status and the presence or absence of lesions from oral GVHD. Inclusion criteria: - Having received an allo-HPT in the Region of Murcia. -Presence of teeth to assess their oral health status. METHODS The data were obtained from clinical and radiological examinations. We also collected data from the medical files of the service of Hematology of "Morales Meseguer" Hospital (Murcia). Collection and registration of data were performed by a single operator. RESULTS In our study, 82.5% of the patients presented dental caries, with an average of 4.47 caries. 74% of the patients had at least one missing piece. The average of dental absences was 4.52. We found that 29 patients (39.7%) did not have any type of restaurative treatment. The mean of fillings was 2.89. The largest number of fillings was obtained in the 35-44 age group. This may be due to better health education, improved access to treatment centers and greater purchasing power of this population range. The mean of the P.I. was 76.28%. 23 patients (37.1%) had a P.I. of 100%, indicating a very poor hygiene, what would lead to increased caries, gingivitis, periodontal problems, dental problems, increased risk of infections,... 43 patients (52.4%) did not present any type of injury compatible with systemic GVHD, while 30 (36.6%) were diagnosed of this injury. 34 (41.5%) were free from oral GVHD and 39 patients (47.6%) showed those lesions. The presence of caries in patients with GVHD had statistically significant relationship (p < 0.05), which would strengthen the hypothesis that cavities progress faster and more aggressive in these patients than in the normal population. Group with oral and/or systemic GVHD have a higher average of tooth fillings, maybe due to the greater care, and more serious systemic symptoms that make these patients be more aware of possible complications after the TPH. CONCLUSIONS 1. Both the periodontal and dental health in our study sample are worse than the Spanish population in the oral health survey of 2010. 2. Patients undergoing TPH have a high number of carious teeth, higher than the general population, and a lower rate of restoration. 3. Periodontal health is very poor and the patient is little awareness for the maintenance of their oral status. 4. The HPT and salivary flow have little influence or interdependence among themselves. 5. It should make a protocol for the first visit, monitoring and dental treatment of patients that will undergo a HPT from Oncohematology Unit to reduce the subsequent oral complications. 6. Specific training and a specialized unit in the treatment of these patients is needed, for the early detection of oral symptoms of GVHD.

Full textEl esófago es una estructura tubular que tiene como función el transporte del alimento desde la faringe hasta el estómago. Las enfermedades de resolución quirúrgica son las obstrucciones esofágicas por cuerpos extraños, el megaesófago y la estenosis esofágica. Obstrucciones causadas por la persistencia del anillo vascular, las neoplasias esofágicas y las hernias del hiato son menos frecuentes. Las cirugías más conocidas que se realizan en el esófago son la esofagotomía y la esofagectomía parcial. Cirugías que se han desarrollado y aún se encuentran en perfeccionamiento son el reemplazo del esófago con yeyuno o tubo cutáneo ante una resección total y la cardioplastía esofagodiafragmática como tratamiento para megaesófago. El estómago es un órgano músculo glandular que conecta al esófago con el duodeno, cuya función es almacenar, triturar y digerir el alimento antes de pasar al intestino. Las enfermedades gástricas de resolución quirúrgicas son la presencia de cuerpos extraños, el síndrome vólvulo/dilatación gástrica, las úlceras y erosiones, la estenosis del píloro y las neoplasias. Las cirugías del estómago de frecuente desarrollo son la gastrotomía, la gastrectomía, la gastropexia, la pilorotomía y la piloroplastía. El desarrollo de un tubo gástrico como reemplazo del esófago se ha realizado con cierto éxito en humanos por lo que aún falta determinar el procedimiento en pequeños animales y en este trabajo es considerado como cirugía gástrica

Full textThe present Doctoral Thesis studies the optimization of the larval culture and the morphology and ontogeny of the digestive system of the common spider crab (Maja brachydactyla Balss, 1922). The larval culture was realized in 200 L aquaria used as incubation chambers. Inside the aquaria were located 600 mL glass beakers as incubation jars. The parameters studied were: temperature, salinity, light conditions and settlement inductors. Our results conclude that the optimal parameters were: 21 ± 1 °C, 35 ± 1 and a daily light-dark cycle. Temperature mainly influences duration of the larval development, salinity mainly influences survival and light conditions have a poor influence on the larvae. M. brachydactyla does not have special requirements for settlement and can be cultured in communal conditions. Digestive system was studied in larvae (zoeae and megalopae), juvenile and adults, employing: dissection, tomography and micro computed tomography, histological techniques and electron microscopy. The basic morphology of the digestive system is conserved during all the life stages. The main differences rely on the higher development of connective and muscle tissues in adult specimens comparing to larval stages. The biggest change was observed in the stomach, since acquires cardiac sacs, ossicle system and gastric mill after the molt to megalopa, then the adult shape of these structures is acquired in subsequent molts. In this sense, the caeca of the midgut gland (hepatopancreas) is very similar between larvae and adults, but their number increases from fourteen in the larvae to tens of thousands in the adults. The esophagus and hindgut tract are characterized by the presence of a cuticle with microspines and rosette glands in the connective. The midgut gland, midgut tract and midgut caeca are very rich in microvilli. The midgut gland realizes absorptive and secretory functions, the midgut tract secretes the peritrophic membrane and the midgut caeca realizes an important secretory activity. For first time, the "micro-apocrine" secretion is described in decapods.

Full textIn the last 10 years an important development of endoscopic systems has taken place. High resolution white light endoscopy (HRE) provides a highly detailed image. Narrow Band Imging (NBI) uses a filter to modify the bandwith of white light, providing a highly contrasted image that enhances the small details of gastrointestinal mucosa. Digestive adenocarcinoma is the paradigm of preventable cancers because its precursor lesions are detectable and resectable by endoscopy. However, there is a group of diseases with an increased risk of developing cancer whose precursor lesions are difficult to detect because they are similar to surrounding mucosa. Besides they are hard to differentiate from others wit lower risk. Therefore conventional white light shows some limitations in these patients’ endoscopic exams. NBI might improve the detection and characterization of precursor lesions in high risk patients. This Doctoral Thesis investigated the role of NBI in three diseases with high risk of digestive cancer: adenomatous familial polyposis (FAP), serrated polyposis syndrome (SPS), and long-term inflammatory bowel disease (IBD). In the first study HRE and NBI were compared for the detection of gastric and duodenal polyps in FAP patients. No additional clinically relevant lesions were detected by NBI. In addition, a systematized design was used to identify advanced histology features in duodenal adenomas with HRE and NBI. Only size larger than 1 cm was associated with this histologic diagnosis. In the second study a systematized design was used to identify endoscopic features to differentiate sessile serrated adenomas (SSA) from hyperplastic polyps (HP) with HRE and NBI in SPS patients. A cloud-like surface, indistinct borders, irregular shape and dark spots inside the crypts were significantly associated to SSA histologic diagnosis. These features were able to differentiate SSA from HP with a high accuracy. The third study compared the use of chromoendoscopy (CE) with NBI for the detection of dysplastic lesions in long-term IBD. NBI No differences were found in terms of lesion detection, but NBI had a higher lesion miss-rate in comparison with CE in the back-to-back colonoscopies analysis. Therefore NBI cannot be recommended as the technique of choice.

Los cánceres del aparato digestivo (CAD) son, en su conjunto, la principal causa de defunciones por cáncer a nivel mundial. Pese al gran desarrollo de diversas terapias, el tratamiento contra este tipo de cáncer continúa siendo poco eficaz y conlleva un amplio número de efectos secundarios que disminuyen la calidad de vida del paciente. Esto hace necesario investigar y desarrollar nuevas terapias que combatan de forma específica y eficaz el desarrollo de tumores. En la actualidad, la inmunoterapia es una de las estrategias terapéuticas más promisorias. El sistema inmune posee el potencial de reconocer y eliminar específicamente a las células tumorales mediante el reconocimiento de proteínas expresadas diferencialmente en estas células, denominadas antígenos tumorales. Sin embargo, el tumor genera un microambiente que inhibe la respuesta inmune antitumoral, por lo que el desarrollo de terapias capaces de interrumpir esta inhibición puede permitir que el sistema inmune elimine eficientemente las células tumorales. Actualmente existen dos tratamientos de este tipo aprobados para su uso en humanos. El antígeno carcinoembrionario (CEA) es una proteína de superficie altamente expresada en una gran variedad de CADs y por ende, un blanco atractivo para el desarrollo de inmunoterapias específicas contra este conjunto de cánceres. En este trabajo se generó un bacteriófago filamentoso M13 específico contra CEA para ser usado en una futura inmunoterapia como un agente inmunogénico que se una a CEA en la superficie de las células tumorales. Así, el fago, a través de su ADN genómico de hebra simple, activará a las células del sistema inmune innato infiltrantes del tumor y promoverá un microambiente proinflamatorio capaz de revertir la inmunosupresión del tumor y permitir que la respuesta inmune adaptativa elimine a las células tumorales de forma específica. El bacteriófago anti-CEA se generó en cultivos de E. coli transformadas con fagomio e infectadas con un fago auxiliar. El fagomidio pSEX81 permitió expresar un fragmento variable de anticuerpo de cadena simple (scFv) específico contra CEA fusionado a la proteína pIII. El fago auxiliar (denominado hiperfago) permitió expresar las proteínas que conformarán la nueva partícula viral. La presencia de la proteína de fusión scFv-pIII en el bacteriófago M13 anti-CEA se confirmó mediante western blot usando un anticuerpo específico contra la proteína pIII. La especificidad del fago anti-CEA contra el antígeno CEA purificado se confirmó mediante ensayos de ELISA. Por otra parte, se comprobó in vitro que el bacteriófago anti-CEA se une a la proteína CEA expresada en la superficie de líneas celulares tumorales de origen humano y murino mediante citometría de flujo. Los resultados obtenidos en esta tesis sugieren que este fago se unirá al antígeno tumoral CEA in vivo, convirtiéndolo en una potencial inmunoterapia contra CADs
Gastrointestinal cancers are the leading cause of cancer-related deaths worldwide. Despite the great development of cancer therapies, the treatment of this type of cancer remains ineffective and involves a large number of side effects that negatively impact the life quality of patients. In order to solve these problems, development of new therapies able to specifically and effectively eradicate tumors is needed. Currently, immunotherapy is one of the most promising therapeutic strategies. The immune system has the potential to specifically eliminate tumor cells by recognizing tumor antigens, which are proteins differentially up-regulated in these cells. However, tumors create an immunosuppressive microenvironment that inhibits the immune responses against the tumor, therefore development of therapies capable of disrupting this inhibition may allow the immune system to eliminate tumor cells efficiently. Currently, two drugs following this principle have been approved for human use. Carcinoembryonic antigen (CEA) is a surface protein highly expressed in a variety of gastrointestinal cancers, thus it is an attractive target for the development of immunotherapies targeting this type of cancers. In this thesis, a M13 filamentous bacteriophage able to bind CEA at the surface of tumor cells was generated as an immunogenic agent for a future immunotherapy. This CEA-specific M13 filamentous bacteriophage, through its genomic single-stranded DNA, is expected to activate tumor-infiltrating innate immune cells promoting a proinflammatory microenvironment capable of disrupting tumor-induced immunosuppression and allowing adaptive immune responses to efficiently eliminate tumor cells. Furthermore, this strategy will promote the presentation of tumor antigens and the generation of tumor-specific adaptive immune responses The CEA-specific bacteriophage was generated in E. coli cultures transformed with a phagemid and infected with a helper phage. The phagemid vector pSEX81 was used for generating the CEA-specific single chain variable fragment (scFv) fused to the pIII protein on the surface of M13 bacteriophages. The helper phage (or hyperphage) allowed the expression of the other viral proteins needed to form the bacteriophage particles. The presence of scFv-pIII fusion protein in CEA-specific bacteriophage preparations was confirmed by western blot analysis using an M13 pIII-specific antibody. The capability of the bacteriophage to specifically bind purified CEA in vitro was confirmed by ELISA. Moreover, it was demonstrated that the CEA-specific bacteriophage binds CEA expressed on the surface of mouse and human tumor cell lines. The results obtained in this thesis suggest that this CEA-specific bacteriophage will bind to CEA-expressing tumors in vivo, and it represents a potential immunotherapy against gastrointestinal cancer. The in vitro observed specificity of the anti-CEA M13 bacteriophage suggests that this phage will bind to this tumor antigen in vivo, making it a powerful tool for potential gastrointestinal cancer targeted immunotherapies
