Additionally, we will highlight the importance of preparing this business plan in order to have an excellent chance of a successful company and to allow us to incorporate innovation into all designed processes. We will have an administrative office located in San Borja, where we will be able to attend our target clients of zones 6, 7 and 8 of Lima Metropolitana from socioeconomic level A, B and C. Our project will have a web page which will be the link communication between our clients and our services and the recruitment of technical nurses as well. The technical nurses providing the service will be professional and vocational people who will be paid 65% of the contracted service. We will have 3 types of services where prices range are from 60 to 120 soles. According our projections, we will start this business plan started with an investment of 31,714 soles, where in the first year our estimated profit will be 9,787 soles, in the third year 22,640 soles, and 51,082 soles for the fifth year.

Full textThe progressive aging of the population has political, economic, and social and health repercussions, especially for families and their environment. Within this aging, highlights both the percentage of people of extreme age (>80 years) and the feminization of the population >65 years in Europe, where women in their >80 years are double of men. Aging may involve living together with chronic diseases and geriatric syndromes that results in worse functional status, less social activity and greater morbidity and mortality. The concurrency of chronic diseases in the elderly causes a difficult management for health professionals. To improve the management of these patients, they are identified as chronic patients in a complex situation. These people present polypharmacy, multimorbidity, advanced age, accompanying syndromes and dependence, having a higher risk of falls and disability. All this causes that a small proportion of the population consumes a high volume of health resources, has comorbidity and dependence, and needs home care. The basic coverage of services to these people is guaranteed, but it is usually fragmented, which means that care needs are not identified and prevents the success of health actions. PC teams, and especially nurses, can and should carry out actions such as CGA to identify the care needs of this population. Therefore, the goal of this research is to evaluate the efficacy of a nursing intervention in the field of home care for the elderly population in a complex chronic situation. In doing so, a multicenter quasi-experimental study, with a control group and a non-probabilistic convenience sampling technique, in six urban KAPs was carried out. The population analyzed were all those chronic patients >65 years in home care, excluding those with cognitive impairment; 138 participants (GC: 75, GI: 63). The study was carried out from March 2017 to January 2019, performing two identical assessments on all participants with an ad-hoc questionnaire. One at the beginning of the study, prior to 12 months intervention, and the other at the end. Three essential issues shape the intervention: continuity of home care, empower the patient, and recording and improving prescribed medication control. The mean age of the population analyzed was 85.92 years; 69.5% were women (but, regarding population that lived alone, represented 72.5%); more than 75% of the population was correctly vaccinated; 90% of the participants presented polypharmacy linked with a worse self-perception of HRQOL; there was greater comorbidity among those <80 years; 22% suffered some fall associated with polypharmacy and low dependence for BADL; and, COPD and HF were the two pathologies that caused the highest number of hospital admissions. There were 29% losses during the study. The higher percentage of death was in the CG due to in the IG, falls, visits to emergency services and hospital admissions were reduced. Comorbidity, poorer performance status for BADL, living alone, and poorer self-perception of health turned out to be a good predictor of mortality among study participants. Greater control of polypharmacy is essential to avoid or minimize adverse effects. Nurses, with their caregiving and educational role, can help to reduce and control drug use and promote lifestyles that foster a healthy aging. This research brings useful results on home care for elderly patients who are also complex chronic, through a nursing intervention and using, among other tools, the CGA.

Los últimos estudios han demostrado que la sociedad Chilena va envejeciendo rápidamente y las instituciones de salud cada vez se han visto más colapsadas, repercutiendo directamente en la calidad de vida de las personas. Esto también alcanza a los centros médicos de las Fuerzas Armadas donde cada vez es más frecuente encontrar estos centros colapsados en horas de atención médica. El desarrollo de este plan de negocios tiene por objetivo crear FASTCARE una empresa dedicada a la prestación de servicios médicos a domicilios para las Fuerzas Armadas y de Orden Público. Sus afiliados cuentan con un sistema previsional exclusivo (DIPRECA Y CAPREDENA), el cual debe garantizar los servicios de salud y disponer de una oferta variada de servicios médicos. Los servicios médicos otorgados por FastCare serán usados por personal de las Fuerzas Armadas y de Orden Público de ambos sexos, cuya edad fluctúa entre los 51 y 80 años, pertenecientes a la región metropolitana. Básicamente FastCare tiene una estrategia diferenciadora en la entrega de servicios médicos integrales abordando gran parte de la necesidad que puedan tener los usuarios y también el cliente (FFAA) entendiendo la necesidad de la institución de descongestionar sus centros de atención y derivando a domicilio los tratamientos y procedimientos de menor complejidad. Entre estos se encuentran por ejemplo: Procedimientos médicos de enfermería, tratamientos antibióticos, curaciones de heridas, toma de muestras, Kinesiología, arriendo de equipamiento médico, cuidadoras etc. Del análisis del entorno se puede concluir que en términos políticos el escenario es favorable debido al interés que tiene el estado de garantizar y disponer de servicios complementarios de salud. En cuanto a los factores económicos se visualiza una oportunidad que apunta a disminuir el gasto incurrido por las instituciones de salud y sustituirlo por procedimientos de menor costo. Los aspectos socioculturales son favorables, ya que las tendencias son aumentar las expectativas de vida y priorizar la comodidad de las personas contratando servicios médicos a domicilio.

Full textObjective: To identify the kind of care given by informal caregivers to patients who undergo abdominal surgeries at the Consorci Hospitalari of Vic (Barcelona). To compare the responsibility chores that these caregivers have in all the different stages of the surgical process (preoperative stage, the transfer from hospital to the patient’s home, the convalescence period). To know the consequences of the care itself in the caregiver’s health. Methodology: A longitudinal recording study with only one cohort of non-paid eventual and of legal age caregivers of patients who present abdominal surgical pathologies. The sample was obtained in a consecutive but not probabilistic form. This study was carried out from February 2011 to March 2012. The studied variables were: sociodemographic characteristics of informal caregivers and patients, the care that the informal caregiver gives the patient and the consequences of his health state according to the ICUB97-R questionnaire and the NOC taxonomy. A bivaried and multivaried descriptive statistic analysis of all the variables was carried out. These had been obtained in three different stages: when the patient was admitted to hospital, when the patient was discharged home and his convalescence at home. Results: A sample of 317 caregivers and surgical patients was included. Most of the caregivers were married women, with an average age of 52,9±13,7 without previous experience as caregivers (99%). Average age of patients was of 64,1±14,9; 51,1% of which were elderly men. There was a 10,1% of these last elderly patients who were dependent before surgery and 48,3% of these suffered from neoplasm. The average care given during in the preoperative stage (min-max) was of 0,0(0-30), during transfer from hospital to the patient’s home was that of 14,0(0-32) and during the convalescence stage was 11,0(0-29) (p<0,05) and the consequences of 0,0(0-26), 6,0(0-22) y 3,0(0-23) respectively (p<0,05). The predictable variables of appearance in terms of consequences were the cáncer diagnosis (β=0,87,p<0,001) and NOC indicators: Emotional health satisfaction (β=-1,76,p<0,001), daily activity disorder (β=-1,66,p<0,001), self-control feeling (β=-1,27,p<0,001), sleeping patterns (β=-0,81,p<0,001) with a R2=0,665. Conclusions: The surgical process includes a care chore that is undertaken by caregivers. This care changes in the three different stages: during preoperative, during transfer from hospital to the patient’s domicile and during convalescence. The peak need for care is when post-surgical patients arrive home after hospital discharge. This care decreases during the convalescence stage and it changes whether these elderly patients were dependent or not prior to surgery. This care also changes if patients have been diagnosed with neoplasm or not. The appearance of care consequences on the caregiver’s health develops in the same way as in the care given to surgical patients. However, these care consequences arise when patients suffer from cancer. The predictable variables of the consequences on the caregiver’s health in the surgical process during the transfer from hospital to the patient’s domicile are: cancer diagnosis and NOC: Emotional health satisfaction, Daily activity disorder, Self-control feeling and Sleeping pattern.

Full textINFLUENCE OF NURSING HOME CARE IN THE EVOLUTION OF PREMATURE INFANTS WITH EARLY DISCHARGE The purpose is to demonstrate that the Nursing Home Care (NHC) of premature infants with early discharge is safe and beneficial. OBJECTIVES: To compare the evolution of weight, analyzing their causes, and morbidity in cases and controls. METHODOLOGY: Case-control (1:1) and observational study, preferably prospective, of newborns at the Hospital Clínic of Barcelona during the years 2007 to 2009, premature, with birth weight over 750 g, which at discharge (or matching of controls) had: corrected postmenstrual age <30 weeks, thermal stability, food by mouth, weight over 1500 g and below 2100 g with upward curve, good clinical evolution and family consent obtained. 65 cases and 65 controls were included. The NHC was administered by a neonatologist pediatrician and two nurses in neonatology, dependent on the Hospital. The nurse made the first visit in two days after discharge, and afterwards twice weekly until 2100 g, by taxi. She supervised the general state, weight gain, food tolerance, temperature, deposition, hygiene and sleep rhythm of the infant. Parents arranged exclusive telephone contact with the hospital and readmission if necessary. The doctor performed the medical examination at the end of NHC and issued a supplemental final report. RESULTS: The total stay of NHC group was 10 days less. Families in the NHC group were of a higher social level. The initial weights were similar in both groups. Regarding changes in weight in g/kg/day in the week prior to discharge and during the week of the study, cases were based on a figure similar to controls, but ended the study with a higher figure. The multivariate analysis showed that the "increase in g/kg/d during the study" increased in NHC group or in males and decreased in case of breastfeeding or peri-intraventricular hemorrhage. During the NHC no major morbidity existed. Only 9% of families did a phone call. CONCLUSION: The NHC is safe and carries a greater weight gain, being the positive factors the NHC group, artificial feeding, the absence of peri-intraventricular hemorrhage and male sex.
