Full textAs part of the study of a site in Northern France polluted by lead and cadmium from 2 smelten (ME and UM), we sought to examine the burden of these metals in the populations subjected to this pollution. The objectives of this dissertation were to quantify these burdens and to seek the factors associated with excess levels. A cross-sectional survey included 400 children (200 living in the exposed municipalities and 200 in non-exposed municipalities) and 600 adults (400 exposed and 200 not exposed). The factors associated with increased blood lead levels in children were: place of residence ≤ 3 kilometers from the smelters, drinking tap water, eating organ meats, male sex, and warm weather (compared with winter). The blood cadmium concentration was significantly higher in children who lived ≤ 4 kilometers from UM and drank > 500 ml tap water daily, who ate fish ≥ once a week and who ate garden vegetables ≥ once a day. The factors associated with elevated urinary cadmium levels were similar to those for blood cadmium, but also included paternal employment at one of the two smelters. Adults had a higher blood lead level when they lived ≤ 1 kilometer from one of the smelters. Most of the other factors associated with increased blood lead levels differed by sex. For men, they were: occupational exposure to lead, alcohol consumption and smoking, low educational level, and age older than 30 years; for women: drinking alcohol, eating organ meats, living in a home constructed before 1948, and using a broom for housework. An increase in the mean blood cadmium concentration was observed in subjects living ≤ 4 kilometers from ME, among smokers, women, subjects with a low educational level, and adults older than 40 years

Full textThis thesis presents a methodological approach concerning the determination of the sphere of influence of hospitals. First part recalls different methods able to determine the sphere of attraction of hospitals: data analysis methods (correspondance factor analysis and "percolation" method of r. Tremolieres), and spatial interaction models. The usefulness of sophisticated gravitational models is demonstrated by the construction of attraction sphere plans, calculated point by point for each establishment. Second part presents some aspects of fuzzy subsets mathematical theory, in connection with applications in the field of medicine and economy: models of medical decision making and consumer's spatial behaviour. A fuzzy model of determination of consumer's spatial preferences, concerning the choice of a medical establishment is presented. Third part presents a cross sectional study concerning hospitalisation in the french department of "bouches-du-rhone" and recalls the principles of the french sanitary chart. Methodological problems, in connection with cross-sectional types of sampling are analyzed and appropriate estimators are proposed. Some results of the methods presented in part i and applied to this study are discussed

Full textBackground: The aim of this study is to explore the geographical differences in the practice of Leisure Time Physical Activity (LPA) among young Canadians and how it is influenced by contextual features of the living environment. Methods: This study relies on LPA behaviours of 54,832 Canadian aged 12 to 17. To investigate the influence of the contextual features, multilevel logistic regressions are conducted. Observations are structured according to a four-level geographical hierarchy: individual, neighbourhood, health region and province. The outcome studied is a dichotomous indicator which refers to the energetic equivalent of achieving (or not) the recommended daily LPA level. Results: Girls show lower odds of achieving sufficient LPA level during winter and when living in an urban setting. Although boys also show lower odds of achieving sufficient LPA level during winter, living in an urban setting is found to have no impact on them. Among both sexes, significant variations are observed between health regions and between neighbourhoods within the provinces, whereas no significant between-province variation is observed. The indices of social and material deprivation analysed are not significant. Interpretation: Winter is observed as a major barrier to performing LPA, suggesting to account for seasonality when developing interventions targeting LPA. The association of suburban and rural areas with higher odds to meet LPA guidelines among girls suggests that facilities available in urban areas might be more suitable for boys, thus recalling the needs for equity. Whereas the between-province variance distribution shows no differences globally, residual analyses highlight provinces that are significantly different from the Canadian average, and provide rationale for further investigation on how LPA is promoted in these provinces.

Full textThe aim of this thesis is to study the consumption behavior of households from a microeconomic point of view, using pseudo panels built on three surveys of Insee (1979, 1984, 1989), in order to identify the possible reasons for divergences generally observed among cross-section estimates and time-series estimates. The first part of the thesis is about demand theory and econometrics for demand models, where we present the usual consumption models (chapter I) and econometrical technics for estimating models over a panel or a pseudo panel (chapter II). The second part consists of empirical applications over pseudo panels. After presenting the data set (chapter iii), we estimate two demand models in chapter IV. The third part of the thesis studies more precisely a group of low-income households (chapter V) and the demand for specific goods : food and transports (chapter vi). The main conclusion of the thesis is that the positionning of a household in the income distribution is an important explaining factor whose impact over the estimation of usual consumption models differs depending on the data dimension, longitudinal or transversal this would explain the observed divergences among estimations made along those two dimensions
