Abstract:
Background: Stroke is a leading cause of perinatal brain injury and cerebral palsy. Term neonatal hemorrhagic stroke (NHS) is a common syndrome with poorly defined epidemiology. We aimed to determine incidence and mechanisms within a large population-based NHS sample. Methods: The Alberta Perinatal Stroke Project (APSP), a provincial registry ascertained NHS cases using exhaustive ICD-9/10 code searching (1992-2012, >2400 chart reviews). Prospective cases were captured through the Calgary Pediatric Stroke Program from 2007-2014 (n=387). All NHS cases underwent structured chart review using a data capture form and blinded review of neuroimaging. Provincial live births were obtained from statistics Canada. Outcomes were extrapolated to the Pediatric Stroke Outcome Measure (PSOM). Results: We identified 74 cases: 49 NHS (26 retrospective, 27 prospective), 4 presumed perinatal HS (PPHS), and 21 hemorrhagic transformation (HT) of ischemic injury. Incidence of NHS was 1:8800 live births (1:5820 for all forms). HT was common (28.4%) including global, arterial venous ischemic lesions. Presumed perinatal hemorrhagic stroke presented with epilepsy. No risk factor was identified in 68% of cases. Outcomes were abnormal (PSOM 1 or more) in 30% and better in the HT group. Conclusion: NHS occurs in 1:8800 live births. Imaging classification is essential to define mechanisms.

Total of 170 samples were collected from Al-Chibayish Marsh reality in Dhi-Qar governorate southern of Iraq to study the epidemology of viral hepatitis in these areas and to detect the type of hepatitis viruses which include A ,B,C,D .The percentage of hepatitis A was 1.17% and most of them below age of ?10 (66.6%) while infection with hepatitis B account 5.29% and includes all age groups. There was no detected cases of hepatitis C,D. The laboratory study showed that the incidence of hepatitis B higher in male (4.11%) compared to female ( 2.35%)

Abstract:
Fast-evolving RNA viruses are a leading cause of morbidity and mortality among human and animal populations, contributing significantly to both global health and economic burden. The advent and revolution of high-throughput sequencing has empowered phylogenetic analyses with increasing amounts of temporally and spatially sampled viral data. Moreover, the parallel advancement in molecular evolution and phylogenetic methods has provided investigators with a unique opportunity to gain detailed insight into the evolutionary and epidemiological dynamics of emerging viral pathogens. Using state-of-the-art statistical approaches, this thesis addresses some of the important but controversial questions in viral emergence. Chapter 2 introduces a new framework to quantify and investigate reassortment events in influenza A viruses. By developing a computationally efficient algorithm to calculate the largest common subtree for a pair of tree sets, which are estimated from diffe rent parts of the genome for the same taxa set, the level of phylogenetic incongruency due to reassortment can be appropriately ascertained. Chapters 3, 4 and 5 investigate the evolutionary origins of three diff erent viruses: the novel emergence and cross-species transmission of SARSCoV, the genesis and dissemination of the unique HCV circulating recombinant form, and the ancient divergence of all influenza viruses, respectively. Moreover, Chapter 4 presents an improved statistical framework, which provides more precise evolutionary estimates, by utilizing the hierarchical bayes approach to investigate recombination events in emerging RNA viruses. The last empirical study, presented in Chapter 6, applies the recently developed Bayesian phylogeography models to a large viral sequence dataset sampled from southern Viet Nam to examine the fine-scale spatiotemporal dynamics of endemic dengue in Southeast Asia. The work presented here reflects both the advancements made in sequencing technology and statistical phylogenetics, along with some of the challenges that remain in studying the emergence of fast-evolving RNA viruses. This thesis proposes new and improved solutions to these evolutionary problems, such as incorporating non-vertical evolution (i.e. homologous recombination and reassortment) into the phylodynamic framework, with the aim of facilitating future investigations of emerging viral diseases.

Abstract:
Purpose: The purpose of this study was to investigate the relationship between anthropometric measurements, proximal and distal lower extremity muscle performance, core muscle endurance, lower extremity flexibility, and neuromuscular control with the incidence of injury in recreational runners over one season. Also, when a relationship was established, we sought to evaluate the predictive validity for any of the variables being investigated for risk of injury in this population. Study Design: Prospective cohort Methods: Anthropometric measurements, proximal and distal isometric lower extremity muscle performance, isometric core muscle endurance, lower extremity flexibility and neuromuscular control were measured in 75 recreational runners prior to the start of a graded marathon training program. Incidence of injury was tracked over the course of 18 weeks, May 2014 – October 2014. Data was analyzed comparing the differences between injured and non-injured groups. Results: There were 33 repetitive stress injuries yielding a gross injury rate of 46% (male n=13, female n=20). Of all the variables analyzed, 5 variables emerged as possible a predictors including age, dominant limb rear foot posture, non dominant limb ankle DF ROM (extended), limb difference of Y balance scale composite scores and limb difference in the 6 M hop test. These variables were entered into a binary logistic regression analysis. Results of the regression indicated only the composite Y balance score difference variable as yielding a significant contribution (p = 0.01), with and predictive validity, (OR = 1.46, 95% CI =1.127 – 1.892). The model predicted 69.2% of the injuries with a specificity of 82% and sensitivity of 54.5%. A cutoff point of 3.6% was determined using a receiver operating characteristic curve. Runners were 3 times more likely to get injured with an asymmetry ≥ 3.6%. Conclusions: An asymmetry of lower extremity neuromuscular control ≥ 3.6% measured by the Y balance scale has been identified as a potential risk factor for injury in recreational runners. Clinical Relevance: This test can be performed as part of a pre-training screening or physical and may be helpful in identifying recreational runners at risk for injury.

A mastite é uma doença endémica considerada a mais frequente em vacas leiteiras e que causa maiores perdas económicas na produção leiteira, especialmente na forma subclínica. O contraste leiteiro é um registo, que consiste na avaliação da quantidade e qualidade do leite produzido por cada animal de uma exploração e que permite também monitorizar a eliminação de células somáticas. Através da aplicação de um programa de análise de contagem de células somáticas, pretendeu-se estudar, vários padrões epidemiológicos de mastites numa amostra de explorações do Entre Douro e Minho. Os resultados obtidos demonstram que apesar das variações sazonais esperadas, o ano de 2016 foi atípico relativamente à ocorrência de mastites, sobretudo pelos ajustes realizados na produção leiteira, exigidos pelos contratos de recolha de leite na região. Porém, a amostra em estudo apresenta bons índices epidemiológicos para mastites subclínicas; The use of milk recording as a tool for the analysis of subclinical mastitis in dairy farms in Entre Douro e Minho Abstract: Mastitis is an endemic disease considered to be the most frequent in dairy cows and causes greater economic losses in milk production, especially in the subclinical form. The milk recording is a register, which consists of the evaluation of the quantity and quality of milk produced by each farm animal and also allows to monitoring of somatic cell elimination. Through the application of a somatic cell count analysis program, it was intended to study several epidemiological patterns of mastitis in a sample of Entre Douro and Minho farms. The results show that despite the expected seasonal variations, the year of 2016 was atypical regarding the occurrence of mastitis, mainly due to adjustments made in milk production, required by the milk collection contracts in the region. However, the sample under study presents good epidemiological indices for subclinical mastitis.

Abstract:
The present doctoral thesis is a field epidemiology investigation carried in Myanmar for trialling a surveillance scheme for Avian Influenza (AI). The study was completed in Myanmar between 2009 and 2010 and was conducted in collaboration with the Livestock Breeding and Veterinary Department of the local Ministry of Agriculture and Fishery. The area selected for the study is the Moeyingyi Wetland including its wild bird sanctuary. This area produces the 60% of the country domestic ducks using traditional technology. After hatching, ducklings are grown up locally or sold in other area. An array of duck products (eggs, embryonated food eggs, drakes, spent laying ducks) are distributed all over the country. The previous history of AI epidemics occurred in Myanmar given by the H5N1 Highly Pathogenic Avian Influenza Virus (HPAIV) subtype increased the suspicion on the Moeyingyi area as a reservoir of AI viruses. Previous attempts to investigate H5N1 HPAIV were carried out by local veterinary services but results were not considered sound enough. This doctoral research proposed and tested a prospective surveillance scheme sensitive to territorial characteristics and local resources which were thoroughly assessed prior the study commenced. The scheme was designed to be effective and to maximise data and information gathered. Special targeted attempts to virus isolation were carried out. This surveillance scheme proved to produce information useful for planning control measures also based only on serological results. The data produced and the information generated showed that almost all the newly housed negative flocks got infected in the time span of the first egg laying cycle (August – April).
La presente tesi di dottorato si basa su uno studio epidemologico di campo eseguito in Myanmar per testare uno schema di sorveglianza dell’Influenza Aviare (IA). Lo studio è stato completato in Myanmar tra il 2009 ed il 2010 ed è stato eseguito in collaborazione con il Livestock Breeding and Veterinary Department del Ministry of Agriculture and Fishery. L’area selezionata per lo studio è la zona umida di Moeyingyi e comprende anche la omonima riserva protetta di uccelli selvatici. Questa area produce il 60% delle anatre domestiche del paese usando metodi di allevamento e riproduzione tradizionali. Dopo la schiusa, gli anatroccoli sono fatti crescere localmente o venduti in altre aree. Il settore locale di produzione delle anatre produce una varietà di prodotti (uova, uova embrionate per consumo alimentare umano, maschi di anatra, e femmine ricostituite a fine carriera di deposizione) che sono distribuiti in tutto il paese. La storia di epidemie precedenti sostenute dal sottotipo H5N1 del virus influenzale ad alta patogenicità (HPAIV) hanno generato preoccupazione sul rischio che la zona studiata fosse un reservoir geografico di virus influenzali aviari. Tentativi di studio dell’area precedenti eseguiti dai servizi veterinari non hanno generato risultati ritenuti soddisfacenti. La presente ricerca di dottorato propone e testa uno schema di sorveglianza longitudinale che tiene in considerazione le caratteristiche territoriali e le risorse locali per raggiungere una soddisfacente sostenibilità. Gli aspetti legati all’ambiente ed al territorio sono stati studiati sistematicamente prima di pianificare lo schema di sorveglianza. Il programma di sorveglianza è stato progettato per massimizzare i dati e le informazioni generati ed ha compreso una attività di sorveglianza virologica guidate dall’interpretazione dei risultati sierologici. La ricerca di questo dottorato ha dimostrato che lo schema proposto è in grado di generare informazioni utili alla pianificazione di misure di controllo anche sul solo dato sierologico. I dati e le informazioni prodotte hanno dimostrato che quasi il 100% dei nuovi gruppi negativi accasati in questa zona si infettano con il sottotipo H5N1 del virus dell’IA entro la fine del primo ciclo di deposizione (Agosto – Aprile). Le dinamiche di infezione sono descritte.
