Abstract:
Background A new blood gas analyzer (epoc™, Epocal Inc.) has recently been developed for the ambulatory monitoring of respiration and electrolyte balance. However, the accuracy of this instrument has not been fully elucidated. The present study compares the performance of the epoc™ analyzer and a conventional bench-top analyzer in operating rooms. Materials and Methods Fifty blood samples were collected from anesthetized surgical patients and three samples were collected from volunteers into syringes containing lithium heparin. pH, pCO2, pO2, Na+, K+ Ca2+, glucose, lactate and hemoglobin levels were measured using both the epoc™ and calibrated ABL700™ analyzers (Radiometer). Data were statistically analyzed using Pearson correlation coefficients and Bland-Altman plots. Results Results showed excellent agreement between the values measured using the epoc™ analyzer and those obtained using the ABL700™ analyzer, except for Na+. Conclusions The epoc™ analyzer is useful in clinical settings, including operating rooms.

Biochemical analytes were also measured with the EPOC, i-STAT Alinity v, and a veterinary diagnostic laboratory in the same animals. Analytes assessed included blood urea nitrogen, chloride, creatinine, glucose, ionized calcium, potassium, and sodium. There were no differences for hematocrit values for African savanna elephants or southern white rhinoceros (p ≤ 0.05). In African savanna elephants, there were no differences between the EPOC and i-STAT Alinity v analyzers for any measured analytes except ionized calcium. When compared to a veterinary diagnostic laboratory, there were differences for a majority of the biochemical analytes measured on the EPOC and i-STAT Alinity v analyzers in African savanna elephants. In southern white rhinoceros, there were differences for a majority of analytes among all three analyzers. Abstract:
Introduction: This study addressed the metabolic demands of running by analyzing the contributions of oxygen consumption during exercise, excess post-exercise oxygen consumption (EPOC), and lactate accumulation to total energy expenditure. Understanding the interaction between these components is essential for optimizing training strategies and improving physiological monitoring in elite athletes. Objective: To evaluate the relative contributions of aerobic and anaerobic energy systems across different running distances using a validated physiological approach in elite-level male runners. Methodology: The study involved sixteen elite male runners from Nineveh, Iraq, who completed 100-meter, 400-meter, and 3000-meter running trials under controlled environmental conditions. Metabolic data were collected using a portable gas analyzer, while lactate concentrations were assessed via capillary blood sampling. Results: The results revealed significant differences in energy expenditure components across the running distances (p < 0.001). During the 100-meter sprint, anaerobic metabolism dominated, with oxygen consumption accounting for 8.57%, EPOC 68.87%, and lactate 22.56%. In the 400-meter trial, energy contributions were more balanced, with 16.78% from oxygen, 53.04% from EPOC, and 30.17% from lactate. The 3000-meter run was characterized by aerobic dominance, with oxygen contributing 68.00%, EPOC 25.96%, and lactate 6.04%. Statistical tests confirmed the significant role of anaerobic metabolism in short-duration efforts. Discussion: The discussion emphasized that earlier studies often underestimated anaerobic contributions. Incorporating lactate kinetics and EPOC provides a more complete understanding of energy demands during intense exercise. Conclusions: It is concluded that integrating anaerobic components yields a more accurate estimation of total energy expenditure, supporting improved performance modeling and health-oriented interventions.

Electrolyte problems are one of the most common cause of morbidity and mortality in the sick patients coming to the Emergency. Electrolytes measured in central lab usually takes 30-60 minutes or more whereas point of care device can give results within 5 to 10 minutes, however different instruments available and their values varies accordingly. Therefore, this study was done to investigate whether electrolyte i.e. sodium and potassium levels assessed using point of care device and auto analysers were equivalent. Methods: We prospectively studied patients admitted from Emergency Department between January 2021 to October 2021. We had taken 60 paired convenience samples and analysed sodium and potassium levels in point of care device (epoc POC device, Siemens) and in Central Lab Auto Analyser (Siemens Dimension RxL Max Integrated chemistry analyzer or in Johnson & Johnson chemistry analyser model number 51 FS). Statistical method to compare the data included Boxplot view, Regression line, Spearman’s correlation coefficient’s, paired t-test for potassium and Wilcox test for sodium correlation, Regression equation, Deming regression and Bland Altman plots.

Different blood gas analyzers are used in equine practice. Every machine needs to be validated, as they have not been designed for use in horses. The aim of this study was to compare the newly marketed GEM5000 machine to the formerly validated epoc machine for blood gas analysis in horses. In this prospective, non-blinded, comparative laboratory analyzer study, 43 equine blood samples were analyzed on both analyzers and values were compared between the two machines via Lin’s concordance analysis, Passing–Bablok regression analysis and Bland–Altman plots. Duplicate measurements were conducted on the GEM5000 machine to evaluate precision. The GEM5000 failed to achieve the required precision for tHb, Hct and iCa2+, but achieved acceptable precision for all other parameters. Concordance correlation analysis revealed poor correlation for Na+, Cl−, iCa2+, K+, Hct and tHb, while there was an at least moderate agreement for all other parameters. Passing–Bablok regression revealed significant constant bias for pCO2, pO2, Cl−, and iCa2+ and significant proportional bias for pCO2, iCa2+ and SO2. Bland–Altman analysis revealed significant systematic bias for Na+, Cl−, iCa2+, K+, Hct, tHb and SO2. This study shows that while precision of the GEM5000 is good, values should not be used interchangeably with data obtained from other blood gas analyzers.

Abstract:
Our study hypothesis is that the interline registered pH of the cow reticulum can be used as an indicator of health and reproductive status. The main objective of this study was to examine the relationship of pH, using the indicators of the automatic milking system (AMS), with some parameters of cow blood components. The following four main groups were used to classify cow health status: 15–30 d postpartum, 1–34 d after insemination, 35 d after insemination (not pregnant), and 35 d (pregnant). Using the reticulum pH assay, the animals were categorized as pH < 6.22 (5.3% of cows), pH 6.22–6.42 (42.1% of cows), pH 2.6–6.62 (21.1% of cows), and pH > 6.62 (10.5% of cows). Using milking robots, milk yield, fat protein, lactose level, somatic cell count, and electron conductivity were registered. Other parameters assessed included the temperature and pH of the contents of reticulorumens. Assessment of the aforementioned parameters was done using specific smaX-tec boluses. Blood gas parameters were assessed using a blood gas analyzer (EPOC (Siemens Healthcare GmbH, Erlangen, Germany). The study findings indicated that pregnant cows have a higher pH during insemination than that of non-pregnant ones. It was also noted that cows with a low fat/protein ratio, lactose level, and high SCC had low reticulorumen pH. They also had the lowest blood pH. It was also noted that, with the increase of reticulorumen pH, there was an increased level of blood potassium, a high hematocrit, and low sodium and carbon dioxide saturation.

Abstract We evaluated the analytical performance of the EPOS (Eppendorf Patient Oriented System) Automated Selective Chemistry Analyzer, using the following tests for serum analytes: alanine and aspartate aminotransferases, lactate dehydrogenase, creatine kinase, gamma-glutamyltransferase, alkaline phosphatase, and glucose. Results from the EPOS correlated well with those from comparison instruments (r greater than or equal to 0.990). Precision and linearity limits were excellent for all tests; linearity of the optical and pipetting systems was satisfactory. Reagent carryover was negligible. Sample-to-sample carryover was less than 1% for all tests, but only lactate dehydrogenase was less than the manufacturer's specified 0.5%. Volumes aspirated and dispensed by the sample and reagent II pipetting systems differed significantly from preset values, especially at lower settings; the reagent I system was satisfactory at all volumes tested. Minimal daily maintenance and an external data-reduction system make the EPOS a practical alternative to other bench-top chemistry analyzers.

Dans la première partie, je dresse un portrait de l'astronomie contemporaine dans les hautes énergies, puis je résume le contexte, notamment celui de l'observation des grandes structures et des amas de galaxies, ayant mené au développement d'un observatoire spatial tel qu'XMM-Newton. Une description de ses télescopes, aussi détaillée que nécessaire compte-tenu des études présentées par la suite, est faite dans la seconde partie. Je décris dans la troisième partie comment se sont déroulées les campagnes d'étalonnage au sol des instruments EPIC sur le banc de test synchrotron d'Orsay, ainsi que ma contribution à l'analyse des données subséquentes. Dans une quatrième partie, je confronte des résultats d'étalonnage à des données de vol, fais le bilan des méthodes d'analyse de ces données et présente une application de ces méthodes à l'observation d'amas de galaxies. Je conclus enfin quant aux perspectives en matière d'observations de sources étendues avec XMM-Newton ainsi qu'en matière d'instrumentation pour les hautes énergies en général et de techniques de traitement de données massives dont les Observatoires Virtuels pourront être demandeurs<br>The XMM-Newton satellite is one of the four cornerstones on which the Euro-pean Space Agency based its sky exploration program, and is aimed at opening further the X-rays window and map the high energies population from the Galaxy as well as from the deep Universe. Within the first part, I make a review of the high energies astrophysics current main topics, like the observations of clusters of galaxies, and summarize the. . . Having brought forward such an X-ray space observatory. A description of the XMM-Newton X-ray telescopes is then presented in the second part, as detailed as needed by the following. I explain in the third part how were conducted the EPIC instruments ground calibration campaigns within the synchrotron test facility in Orsay, and the analysis of the resulting data. In the fourth part, ground calibrations results are compared to some in-flight calibrations datasets, and methods for analysing data from extended sources (like clusters of galaxies) observations are extensively described. Finally, my conclusions regarding future extended sources observations with XMM-Newton and relevant data analysis strategies are drawn in the light of X-rays instrumentation general perspectives as well as the development of Virtual Observatories

Au lendemain de l'accession du tchad a l'independance, les pouvoirs publics ont tres vite manifeste leur volonte d'accompagner la decolonisation politique par le developpement economique du pays. Mais l'initiative privee etant defaillante, c'est l'administration publique qui est amenee a jouer le role de substitution : elle devient l'instrument privilegie de la politique de developpement economique et social du gouvernement. C'est a ce titre qu'elle intervient pour planifier l'economie, mettre en place les mecanismes d'incitation a l'investissement ou prendre directement en charge la gestion et l'exploitation des unites de production. Ce role fondamental attribue a l'administration publique implique que celle-ci soit exceptionnellement dynamique. Or force est de constater que l'organisation et le fonctionnement de l'administration tchadienne revelent de nombreuses carences: centralisation excessive des structures, mimetisme administratif, corruption et nepotisme, qui entravent durablement l'efficacite de son action. Le redresse ment de cette situation implique une reforme globale des institutions administratives, s'appuyant a la fois sur les structures, les methodes de travail et la mentalite des agents publics. De cette strategie de reforme, on espere voir emerger une administration tchadienne plus dynamique, plus efficace, donc veritablement tournee vers le developpement<br>As soon as chad became independant, the public authorities revealed their wishes of carrying out the economic developement of the country simutaneously with the political decolonization proceedings. As the private sector's own initiative was not strong enough, the national civil service had to fulfillthis purpose, therefore becoming the goverment'sprincipal means of action for the economic and social developement policy. Thus, the civil service set up economic planning, gave a stimulus to investments, took over management and ruled production. To accomplish its main task, the national civil service was supposed to be exceptionally active; unfortunately the working system of chad's national civil service showed several weaknesses: corruption, nepotisme, structures and decisions highly centralized, so that its action was quiteunefficient. In order to reverse such a situation, a global reform including some civil service institutions is needed on the bases of working methods and civil servant's mentality. From the bases of this strategy of reform, we hope to see coming up a chadian civil service more dynamic, more efficient, and frankly devoted to the developement of the country

La nutrition est un facteur de risque modifiable pour le cancer puisqu'environ un tiers des cas pourraient être évités en adoptant une meilleure alimentation. La relation entre nutrition et cancer est complexe, et son étude est enrichie par de nouveaux défis apportés par les récentes avancées technologiques dans le domaine des « -omiques ». Cette thèse a pour but de développer de nouvelles approches biostatistiques afin d'étudier la relation entre nutrition et cancer au sein de la cohorte EPIC. Pour ce faire, l'applicabilité de nouvelles méthodologies multivariées dans le domaine de l'épidémiologie nutritionnelle a été étudiée.Une nouvelle méthode multivariée pour la réduction de la dimensionnalité, le Treelet Transform (TT), a été examinée afin d'extraire des patterns de nutriments issus de questionnaires. Les patterns ainsi obtenus par le TT étaient plus facilement interprétables que par les méthodes classiques. Ensuite, un cadre analytique pour implémenter le concept du « meeting-in-the-middle » (MITM) a été développé et appliqué dans 2 études cas-témoin nichées sur le cancer hépatocellulaire avec des données métabolomiques. It has been estimated that about a third of cancer cases can be prevented by complying with a healthy diet and adhering to the recommendations in terms of nutrition. The nutrition-cancer relationship is a complex one, and its study is currently at a turning point with the opportunity and challenges brought by the recent technological advances in the fields of « -omics ».This thesis aims to develop new biostatistical approaches to investigate the nutrition-cancer relation within the European Prospective Investigation into Cancer and nutrition (EPIC) study. To do so, the applicability of new methodologies in the field of nutritional epidemiology has been examined.First, a new multivariate dimension reduction method, the Treelet Transform (TT) was applied to extract nutrient patterns relying on questionnaire data. The extracted patterns were more easily interpretable than those obtained with more classical methods.Then, an analytical framework was conceived for the « meeting-in-the-middle » (MITM) principle and applied to two nested case-control studies on hepatocellular carcinoma, with targeted and untargeted metabolomics data. The MITM aims to identify overlap biomarkers of certain exposures that are at the same time predictive of disease outcomes. The implementation focused on the application of partial least squares and mediation analyses. Last, the association between 60 plasma fatty acids levels assessed from biomarkers and breast cancer risk was examined in a nested case-control study in EPIC. This thesis will serve as a basis for future epidemiological applications looking into the nutrition-cancer relation

The overall aim of this research project is to study green/energy-efficient real estate from an economic perspective. The thesis summarizes the results from five different studies with a connection to green/energy-efficient real estate.The aim of the first paper (paper A) is to study how tenants perceive the indoor environment in green-rated premises, and to compare these results with tenants’ perception of a conventional building’s indoor environment. The main result is that the tenants in the green-rated building are more satisfied with the indoor environment than the tenants in the conventional building.Papers B and C assess whether energy efficiency has an impact on buildings’ income and market values using Swedish real-estate data. The key result is that although there is a small impact on building-related income, this does not seem to translate into a higher market value.The last two papers included in this thesis study hindrances to a more energy-efficient building sector. In paper D, two office buildings are used as baseline cases to provide insights into the difficulties that can arise when trying to upgrade a building to make it more energy efficient. The results indicate that changing existing leases is a prohibitive process and that it is often difficult to evaluate the final impact of an energy upgrade. The last paper focuses on why it may be rational to postpone green refurbishments even if they are profitable. The main result is that it may be rational to postpone such refurbishments. However, by introducing different financial penalties and/or subsidies, these investments could be triggered today.To sum up, the results indicate that green buildings are preferred by tenants, but that there still appear to be economic barriers to a greener building sector.<br>Det övergripande syftet med denna avhandling är att studera grön/energieffektiva byggnader ur ett ekonomiskt perspektiv. Avhandlingen består av en kappa och fem separata studier, vilka belyser tre olika forskningsfrågor. Syftet med den första studien är att studera hur hyresgäster upplever inomhusmiljön i gröna byggnader. I studien jämförs inomhusmiljön i en grön byggnad med inomhusmiljön i en likvärdig konventionell byggnad. Resultatet visar, på det stora hela, att hyresgästerna är mer nöjda med inomhusmiljön i den gröna byggnaden. De nästföljande studierna, B respektive C, undersöker om byggnadens energiprestanda har någon inverkan på dess hyra respektive marknadsvärde. Resultaten visar på en liten signifikant hyrespåverkan, dock verkar denna inte ha någon effekt på byggnadernas marknadsvärdebedömningar. Skälet till detta kan vara att hyrespremien anses för liten för att ha någon signifikant inverkan på byggnadens marknadsvärde, alternativt att fastighetsvärderare inte beaktar energiprestanda när en fastighet värderas. De två sista studierna studerar varför vissa, tillsynes lönsamma, energiinvesteringar inte genomförs. Resultaten från studie D visar på svårigheterna med att ingå ett samarbetsavtal (för att eliminera felaktiga incitament) mellan hyresgäst och hyresvärd. Sådana avtal tar lång tid att förhandla fram och det uppkommer ofta svårigheter med att utvärdera de tänkta energiinvesteringarnas ekonomiska utfall. Studie E utgår ifrån en realoptionsmodell, vilken används för att utvärdera när ”gröna” renoveringar bör genomföras i en befintlig byggnad. Studien visar att det kan vara rationellt att vänta trots att investeringen idag är lönsam. Vidare visar resultaten att det är möjligt att via byggsubventioner/finansiella ”straff” påverka aktörer att tidigarelägga energieffektiviseringsåtgärder.  Dock är det viktigt att dessa utformas korrekt så att det inte skapar några snedvridna incitament. Övergripande visar resultaten att gröna byggnader är att föredra ur ett brukarperspektiv men att det fortfarande finns ekonomiska hinder för en mer hållbar byggsektor. In this article the author analyzes how the Renaissance epic poetry of the Polish-Lithuanian Commonwealth approaches the theme of translatio imperii, which is a concept and a political stereotype of transfer of metaphysical world domination from country to country. After the fall of Constantinople (1453), the concept of translatio imperii gradually lost its universal character and was interpreted within the confines of a nation. Among the analyzed poems are: Bellum Prutenum (1516) by Ioannes Visliciensis and Radivilias (1592) by Ioannes Radvanus. The artistic expression of both the “Jagiellonian” and Lithuanian (i.e., Grand Duchy of Lithuania) patriotism, which incorporated the concept of translatio imperii, had an enormous impact on the formation of the national identity of the Belarusian, Lithuanian, and Polish peoples.

This chapter considers what a putative oral Homer means to contemporary performing artists. It is argued that (especially within devised theatre, performance storytelling, and spoken-word poetry) an imagined oral Homer can serve as inspiration for the creation of deliberately unpredictable ‘unfixed’ performances, which strive to generate present-day equivalents for the flexibility and spontaneity attributed to the earliest, preliterary, performances of epic. This chapter uses the terminology of ‘fixed’ and ‘unfixed’ to analyse such performances, with different works, and even different iterations of a single work, displaying ‘fixed’ or ‘unfixed’ characteristics to varying degrees, and at different moments. An in-depth case study examines Kate Tempest’s Brand New Ancients as a high-profile example of a work containing a significant degree of interplay between ‘fixed’ and ‘unfixed’ elements. The chapter concludes by highlighting challenging new questions concerning creative interactions between contemporary performance practices and myths of Homeric orality.

Despite the diversity of Haḍimbā’s character, in recent decades the goddess has become primarily identified with the demoness Hiḍimbā, a renowned figure from the Mahabharata. Drawing on diverse types of materials, this chapter analyzes this identification in light of the different processes indicated by the term Sanskritization, which is also closely explored. Whereas the origins of Haḍimbā’s epic associations remain uncertain, it becomes clear that their current foregrounding is the result of yet another set of encounters and interactions with local, regional, and extraregional forces and ideas. Haḍimbā again emerges here as a complex persona, who serves as a conceptual arena for her devotees to reflect on their self-perception and sense of belonging and to recast their own cultural marginality in a new, inclusive, and rather flattering light. The chapter concludes by showing how the process of Haḍimbā’s Mahabharatization projects outward in ever-growing circles.

Thapar explains that it is not necessary to prove that the events described in the epics are historically accurate, but clarifies that the historian can derive an understanding of the kind of societies represented in the epics from careful analyses of the texts. She discusses the two time concepts that prevailed then—the cyclic in the yuga theory and the linear in genealogies and the adoption of eras by rulers. There is also some discussion of the location of exile in epic literatures.

Envisioned as primordial ancestors in art, and at the heart of origin stories in epic poetry, giants were ubiquitous in sixteenth-century Europe. Establishing ancestry with giants was pervasive in the German tradition via the epic Nibelungenlied and the archaeology of Maximilian I. Later, the printmaker Hendrik Goltzius would interpret the colossal figure of Hercules not only in terms of Dutch nationalism, but also of ancestral significance. In England, within the cultural archaeology of Edmund Spenser’s epic poem The Faerie Queene several giants are invoked in order to imagine past ancestry, as much as future hopes of imperialism. This paper takes a comparative approach and analyzes how giants figure prominently in German, English, and Dutch art and epic poetry.

The article analyzes the phenomenon of chronotope with respect to M. Sholokhov’s novel And Quiet Flows the Don. The features of Sholokhovian chronotope are related to the intermediate position which the novel occupies between the two cultural epochs specific to Russian literature — between the classical novel forms and the new type of tragical epic literature created by the writer, with its considerable insertions of mythology and folklore. The elements of Sholokhovian chronotope — the time and the topos — play an important role as regards the semantics of the main conflicts rendered in the novel. To a certain extent, it is impossible — without taking into account the peculiarities of the chronotope — to analyze adequately the fate and the vicissitudes of Grigory Melekhov, the main character of the novel And Quiet Flows the Don.

Based on the results of a two-year study on the impact of agro-ecological conditions in Serbia in the regions of the Sava River and the Danube River, the possibility of introducing Khorasan wheat in the three most important agricultural regions of Libya was analyzed using the EPIC model (Erosion Productivity Impact Calculator). The EPIC method was used to analyze precipitation and temperature, which have the biggest impact on the growth and development of wheat. It has been found that by choosing the most suitable production technology, Khorasan wheat can be grown as a winter or facultative-winter crop in these areas.

Perspectives on the Development of Literature in the Context of the Colonial Regime and the Independence of the Country While the national literature has been developed for centuries under the conditions of the colonial regime, it is natural for the reader to find difficulty in distinguishing between fictional works created in the conditions of freedom and the colonial regime. If we analyze this issue with the example of our country in mind, then we must confront both the literature created under colonial rule and in the time of freedom. Indeed, it is through such a comparative analysis that we can then identify the literary peculiarities characteristic of both eras. At the same time, it is natural to consider the author's creative indivi­du­ality as well as the paradigmatic features of each epoch in which the author is inculcated. Considering all of this, we can analyze, on the one hand, the work of Shota Rustaveli (12th century, Golden Age of Georgia), and on the other hand, the work of Nikoloz Baratashvili (19th century, a country that became a province of Russia). These two literary works provide an excellent platform for discussing the most fruitful conditions for literature development. In this case, the subject of relative controversy will be the hero-knight of the epic ("Panther-skinned"), and on the other hand the lyrical "Meran" character – also a rider who, like the first, tries to cross the border of destiny over to a promised land of harmony. It is important to recognize that "Merani" is as highly acclaimed as Rustaveli’s “Vepkhistkaosani." If the classical epic tradition is perfectly found and manifest in Rustaveli's poem, then "Merani is the most brilliant example of lyrical self-expression characteristic of romantic poetry" (Guram Asatiani). Lastly, we should not assume that this research will necessarily lead us to a banal conclusion, one in which the literature produced under colonial rule inevitably reflects a process of resistance or obedience to the current situation. The solution to the current situation is often to isolate oneself from reality. An even more effective way to escape colonial influence is to focus on a different cultural space which, in turn, carves a whole new modern paradigm for considering the national interest. This research is relevant and timely and, thus, we hope its results will be read with enthusiasm and appropriate interest. And, perhaps more importantly, we hope that these dual perspectives of literature (colonial, independent) will be thoughtfully acknowledged.

National geodetic reference system is base for geodetic and geospatial data and information obtaining, maintenance and distribution in common reference system in country. It means, that it must be as accurate as possible and lockstep with time. Latvian national geodetic reference system (LNGRS) derives from Soviet Union geodetic reference system at beginning of 90-ties last century. Development of LNGRS was done as historical continuation of practice to use height, coordinates, Earth gravitational and geomagnetic systems and national networks. For coordinates was established new Latvian geodetical coordinate system using one week campaign data of GPS NAVSTAR. Heights were kept in Baltic normal height system epoch 1977. Earth gravitational system was established newly by absolute gravimetric measurements. Geomagnetic system was developed as repeat station system. Earth is constantly change system internally and outwardly More growing use of GIS and GNSS in agriculture, construction, logistic, military and everyday life put new challenge for LNGRS and it reliability to actual situation in top level. Main goal of publication is to analyse all aspects of LNGRS against nowadays criteria and requirements of dynamic national geodetic reference system. Results of analyses show feeble and powerful sides of LNGRS.

Objectives. The Agency for Healthcare Research and Quality (AHRQ), through the Evidence-based Practice Center (EPC) Program, aims to provide health system decision makers with the highest-quality evidence to inform clinical decisions. However, limitations in the literature may lead to inconclusive findings in EPC systematic reviews (SRs). The EPC Program conducted pilot projects to understand the feasibility, benefits, and challenges of utilizing health system data to augment SR findings to support confidence in healthcare decision making based on real-world experiences. Study Design and Setting. Three contractors (each an EPC located at a different health system) selected a recently completed systematic review conducted by their center and identified an evidence gap that electronic health record (EHR) data might address. All pilot project topics addressed clinical questions: infantile epilepsy, migraine, and hip fracture, respectively. EPCs also tracked additional resources needed to conduct supplemental analyses. The workgroup met monthly from 2022 to 2023 to discuss challenges and lessons learned from the pilot projects. Results. Two supplemental data analyses filled an evidence gap identified in the systematic reviews (raised certainty of evidence, improved applicability), and the third filled a health system knowledge gap. Project challenges fell under three themes: regulatory and logistical issues, data collection and analysis, and interpretation and presentation of findings. Limited ability to capture key clinical variables given inconsistent or missing data within the EHR was a major limitation. Conducting supplemental data analysis alongside an SR added considerable time and resources to the review process (estimated total hours to complete pilot projects ranged from 283 to 595 across EPCs), and the increased effort and resources added limited incremental value. Conclusion. Supplementing existing systematic reviews with analyses of EHR data is feasible, but resource intensive, and requires specialized skillsets throughout the process. While using EHR data for research has immense potential to generate real-world evidence and fill knowledge gaps, these data may not yet be ready for routine use alongside systematic reviews.

Abstract:
An experimental study was carried out to characterize the scavenging behavior of a cross-scavenged, piston-aspirated, two-stroke, natural gas engine to aid in the development of computationally inexpensive simple scavenging models for onboard engine control by (1) studying the effects of changing operational parameters on the engine's scavenging performance, and (2) identifying the underlying phenomena driving the observed effects. Tracer based methods were used to quantify the scavenging and trapping performance of the engine - CO2 was used as a tracer for combustion products and pre-mixed fuel was used as a fresh charge tracer. CO2 concentration was measured on a crank angle resolved basis both in the engine cylinder and exhaust using portable NDIR sensors, while unburned fuel concentration was measured in the exhaust using the FID module of a standard five gas analyzer. It was found that scavenging took place in three stages, an initial perfect displacement type stage, followed by a short-circuiting, and a perfect mixing type stage. Engine speed and load changes were found to have the strongest effects on the trapping and scavenging performance of the engine; spark timing effects were less significant. Changes in measured scavenging and trapping efficiencies at different operating points resulted from a combination of influences, namely (1) reduced time for gas exchange at high speeds, (2) higher expansion and scavenging pressures at high loads and retarded spark timings, and (3) phasing of the reflected 'scavenging' and 'plugging' pulses in the exhaust pipe relative to BDC and EPC, respectively. Increasing engine load made the engine scavenge significantly better and increasing engine speed resulted in a larger fraction of the delivered air being trapped. The combined effect of these scavenging changes and changes in the engine's fuel conversion efficiency resulted in the engine running leaner at high speeds (more air delivered and higher trapping efficiency) and at low loads (higher trapped residuals). The results were then used to gauge the performance of the simple scavenging model (the hybrid model) developed in phase II of the project. While encouraging results were obtained at high speed, the trapped air mass was overestimated at medium speed; suggesting the need for adding a low scavenging efficiency sub-model. Recommendations have been made about adding a short-circuiting zone to address this limitation of the model.
