Full textThe main objectives of this thesis consist in making a comparative analysis of the specified health care of people with mental disabilities in France and in Russia, using a multidisciplinary follow-up programme aimed at young adult patients. This doctoral study begins with a history of the theoretical advances in the health care organisation for people with mental health conditions as well as an overview of the legislation evolution. This exploratory and comparative study includes 20 people (10 French and 10 Russian) with light or medium intellectual disabilities, aged 19 to 33, who are placed in specialized institutions. A comparison is made between the current legislations in France and Russia and shows that though alike, they result from a very different history. The health-care organisation for people with mental disorders is also under close scrutiny. The analysis shows the great diversity in specialized mental-health-care infrastructures available both in France and Russia. The collection of data has been carried out through the study of medical and educational files and semi-structured interviews with the supervising staff and young adults in specialized centres. In this doctoral thesis, the study methodology as well as the results obtained in psychometric and projective tests are provided. Findings are discussed in light of current research and criticized with regard to methodological limitations. This research work exhibits the impact of the institutional health-care on the social adjustment of those young people with mental-health issues, according to their psychological potential. This study analyses the results of different mental-health-care programmes in two countries - France and Russia — with distinct social and societal patterns. However, this bibliographical and historical research shows that practices result from clinical and therapeutic values that have sometimes been conflictory, and which for historical reasons are often close, but also at times disjointed. In fact, this inquiry shows that the exchanges between France and Russia must be maintained, as they used to be, in relation to practices and research. The approaches of the two cultures appear quite complementary and can learn from each other. Comparative studies must therefore continue. This enlargement would provide other examples of social situations or institutional environments which would help to understand the impact of the various health-care programmes in both countries

Full textThe nosographical vagueness of the concept of slight intellectual deficiency has entailed the absence of a notional definition in every aspect of institutional consideration since 1909, as can be seen through official texts, as well as through the organisation of the school system. The ensuing teaching methods have not proved really efficient in helping children decrease their difficulties ; they sometimes may add to them, or, even, create new ones. On the one hand, we find psychometry and non-analytic psychopathology, which mark out the field of deficiency, with the statistically defined milestones of intelligence and normality ; on the other hand, specialized educational methods and psychoanalysis free us from them, to give the subject's own history more importance. A new approach to this concept is therefore likely to provide a better understanding and a more suitable educational treatment. Studying the concept of slight intellectual deficiency through official texts, the way it is dealt with in the Haute Loire, as well as the description of the way the District Commission for Secondary Schools works, enables us to grasp the concept. An analysis of the school and psychological records of children directed to a special educational section, provides a more accurate approach, as it singles out the strong points which are examined through three case studies. These examples clearly demonstrate the developpemnt of a deficient structuring and point out a psychological process which strongly fits in with a relation of dependency. The model of communication allows us to go beyond the dialectics of normality and pathology. Slight intellectual deficiency must then be regarded as a difference and our approach towards others, can no longer be the mere objectification of a category or a simple pathology, but rather, a thorough search for the significance of this otherness and of its global and inter-relational effect

From that elsewhere of human kind, in that space of transitivity, out of standard, dialectical transformations will appear between the social worker and the subject so-called mentally handicapped. The locating in the poetical field of what we may call "withdrawing into oneself" confirms the rights of difference. It makes us think about the rights of resemblance, drives us to consider strangeness and thus questions the educational sciences about any person, any individual. The essential stake of the actual thesis shows that mentally handicapped person is not totally apprehended on the typical, atypical and utopian aspects. This questions again the possibility of his being educated and makes it necessary to clarify the objectives. A reflection to extend to the whole society so as to put an end to the ill treatment of differences and resemblances, always a topical subject

In order to achieve this, an investigation by quiz was done among institutions for mind-handicaped child hood in algeria. The thematic analysis of quiz answers has allowed us to schow in the studied population of educators the existence of : - a heterogeneity in the representation made by educators of their own job, - a noticeable importance given to the reeducational and pedagogical aspect of the job, - a lack of interrest expressed in the aspects psychological and professional, in the enlivening and keeping. These results led us to suggest the following things : - change totally the present system of educators training, - wide the field of skills of special education teachers, - change the educational projects in institutions for mind-handicaped children

Full textThe subject matter of this thesis is the study and the setting up of a computerised learning programme for mentally deficient people working in a specialised assisted work centre. The fundamental idea behind this programme, which includes series of specialised software specially adapted to this population, both with regard to the contents to be learned and with regard to its presentation, is cognitive remedial work on intellectual handicaps by means of learning and training with learning tasks which closely resemble real tasks from professional and everyday life. The learning principle consists of presenting the information to be learned to be taken up, treated and controlled through facilitating the use of attention span, mental imagery and memorisation. Full textMentally-handicapped adults, like any other human beings, develop amorous sentiments and therefore, are entitled to an emotional, intimate and sexual life.In institution (living space), brain-damaged adults are often dependent on social representations which built them, of those of caregivers who cared them, or still of those of caregivers-loving-accompanying and families which accompanying them.This research tried to target the central cores and peripheral elements of all these social representations, by means of the “free association” and the “emotional cards”, to estimate for all actors, the possible progress regarding acceptance, and consequently accompaniment of intimacy and sexuality about institutionalized mentally-handicapped adults.Brain-damaged adults (acquired handicap) and people with mental disabilities (congenital handicap) were the main protagonists of this research.The results show that according to the type of handicap (acquired or congenital), the caregivers’ posture is almost the same; that of the resident differ themselves slightly; as for that some caregivers-loving-accompanying and families are radically different: people with congenital disabilities do not create an intimate imagination and/or sexual from them, to their mentally-handicapped adult.What perspectives for the implementation of an education for the sexual health for these cerebral disabled people? What sort of companying for privacy and sexual life in institution?

Full textThe situation of the mentally challenged people inside the social space is a multiple-themes issue. This vast subject allows us to question the origin of every social organization. The text offers a study starting from the observation of a ritual which function is to prepare someone getting into adulthood. The participation of members from an organization specialized in the activities of a group called "Inter-unit of young adults" is the first step of an initiatory period which eventually leads to another stage of life. This study observes the ways of acquiring this new "adult" status and questions its contents. It tends to show that the development of a ritualized space, based on a principle of age, which is being of age, changes the organization of the institutional taking charge, making easier transmission, elaboration and adherence to the common values of the members of the institution. The ritual puts in a symbolical light the initiate in a social space giving him access to a process helping him towards autonomy. It conforms to the requirements of the initiation, as it is defined by ethnology, taking the initiate away from the childhood world. But the initiation stays "ambiguous" because its organization is restricted to the inside of the institution, therefore sending an individual back to the status of a mentally-handicapped person

Full textThe present thesis has an epistemological and sociohistoric nature. It is about the care of the intellectual deficient children described, in the 19th century, as « idiotic children », «fools », « retarded » and regarded as patients having mental insanity. The research concerns more specifically the processes which has led to the emergence, to the development then to the disappearance of gymnastics called medico-educational for the idiotic children in mental homes. These gymnastics begin in the field of medicine when medical specializations and gymnastics are worked out. It develops during all the 19th century in the framework of an elaborate system of care of medico-educational type. This treatment, used in some Parisian asylums, appears, at the end of this century, to be a very accomplished form of education and instruction for this population. It comes from the work of a movement of reforming agents (gymnasts, teachers, nurses, etc), sponsored by doctors who try to institutionalize this prototype in the medical domain then in the school field. The development of experimental psychology through the creation in particular of metric scale of the intelligence is a breakthrough which leads in 1909, to a distinct treatment of the mentally handicapped patients and the mentally handicapped schoolchildren. The first are pushed in insane asylums with a treatment based on medical attention. The seconds, considered retarded children are handed over specific schools’ care in the regular school system. The medico-educational model and its gymnastics in asylums disappear permanently in the early 20th century

Full textCe mémoire concerne les options résidentielles s’offrant aux personnes aînées présentant une déficience intellectuelle (PADI) de la Ville de Québec. Sachant que l’espérance de vie des personnes présentant une DI a considérablement augmenté et qu’elles survivent désormais bien souvent à leurs parents, il devient primordial de s’intéresser à la planification de leur futur, notamment sur le plan résidentiel. Les objectifs de cette recherche sont : (1) connaître les options résidentielles actuellement disponibles pour les PADI; (2) comprendre leurs besoins comblés et non comblés en matière d’options résidentielles actuelles; (3) identifier des recommandations pour optimiser la satisfaction des PADI à l’égard de leur milieu de vie et soutenir l’exercice de leur pleine participation sociale. Premièrement, une analyse documentaire a permis d’identifier les différentes options résidentielles qui s’offrent aux PADI dans la Ville de Québec et de dresser le portrait de cette offre. Deuxièmement, des entrevues semi-dirigées ont été réalisées auprès de dix personnes présentant une DI et âgées de plus de 50 ans. Ensuite, l’analyse réalisée à l’aide du logiciel de données qualitatives Nvivo 11 a permis d’identifier des informations à propos du milieu de vie habité, l’appréciation de celui-ci et le milieu de vie souhaité par les PADI pour leurs vieux jours. Enfin, les différents constats mis en lumière aux termes de cette étude sont discutés. Il est question de l’absence de planification du futur, de la participation sociale, des facteurs influençant positivement et négativement la qualité de vie, de l’intérêt des participants pour les soins de santé et des options résidentielles souhaitées. Finalement, deux recommandations principales sont émises afin de faciliter la transition résidentielle des personnes présentant une DI dans leur avancée en âge. Il s’agit de sensibiliser les familles à la planification du futur et d’offrir des options résidentielles favorisant la participation sociale et le bien-être des PADI.

Knowing that the life expectancy of people with ID has increased significantly and that they now outlive their parents, it is essential to take an interest in the planning of their future, which includes housing. The goals of this project are to: (1) be made aware of the housing options currently available for seniors with ID; (2) understand their needs, met and unmet, with respect to the currently available housing options; (3) identify strategies to optimize the satisfaction of seniors with ID concerning their living environment and to support the exercise of their full social participation. First, a literature review was completed. This review made it possible to identify the various housing options available to seniors with ID in Quebec City and to get an accurate picture of what is offered. Second, semi-directed interviews were conducted with ten people with ID who were over 50 years of age. Full textIntroduction: Adolescence is the age of transformations and changes in balance between the inside and the outside. The program "My Roof and I!" aims at exploring the theme of “the home environment”, as a marker of identity, through the setting up of a program of therapeutic mediation, based on the assumption of the existence of a link between the appropriation of one’s living space and the construction of identity. Method - 65 teenagers with a mental disability took part in the study, being taken care of in a Medical Special Needs Educational Institute presenting a mental disability (average age: 16.58; S.D.: +/-1.55) with an experimental group (n=32) who followed a therapeutic mediation program consisting of 6 sessions and a control group (n=33). The following investigation tools were used: the Toulousian Self-Esteem Scale (E.T.E.S), the Big Five Questionnaire for Children (B.F.Q.-C) and the Multidimensional Anxiety Scale for Children (M.A.S.C), following a test-retest protocol. This quantitative approach is completed by a qualitative approach, focused on sociodemographic, diagnostic aspects and the therapeutic process in workshops. Results – The study includes 3 parts: 1. Singular typology of teenagers in a Medical Special Needs Educational Institute; 2. Development and assessment of an art therapy program from a comparative perspective. Among the main results, two therapeutic working axes can be seen: the somato-emotional axis and the psycho-social axis; 3.Clinical assessment of the workshop process and creative work made by the teenagers. Conclusion - The program of art therapy enables a whole set of dimensions of teenage identity to be improved. The teenagers who benefited from the program gradually appropriate spaces which help them fix their identity. This approach should be developed within the institutions caring for teenagers presenting a mental disability

Full textLong ignored or denied, incidents of abuse are now regularly reported and recognized as commonplace in institutions catering for people with disabilities. It is widely recommended that staff should receive training in order to help prevent such abuse, and even that such training modules should now be made compulsory in initial training programmes for all staff working directly with residents. However, virtually nothing is said about the content of such courses, nor about the pedagogical approaches that should be adopted. This thesis analyses such a training program and its outcomes. The programme was conceived with a view to improving active prevention, taking into account the evolution of the concept of abuse, the need for prior work on representations of abuse and the option of an eco-systemic approach to the issue. In order to better understand outcomes, three distinct pedagogical approaches were included. The programme is proposed to social workers during the final year of their initial training. Our approach tests the hypothesis that changes in representations of what constitutes abuse and its factors of protection occur during the training course and may result in specific preventive action. It allows us to develop the methodological approaches needed to analyse such a training programme and also to make recommendations regarding the content and the adoption of appropriate pedagogical methods
