Abstract:
Zusammenfassung Ziel der Studie: Expositionen in suizidaler Absicht stellen besondere Anforderungen an Krankenhäuser und Giftinformationszentren. Deswegen wurden die zeitlichen Charakteristika ihrer Häufigkeit studiert. Methodik: Retrospektive Analyse aller humanen Expositionen von Anfang 2004 bis Ende 2013 im Einzugsbereich des Giftnotrufs Erfurt nach Häufigkeit nach Jahren, Jahreszeiten, Monaten, Wochentagen, Tageszeiten sowie nach Expositionsumständen, Altersgruppen und Geschlecht. Ergebnisse: 59,7% aller Expositionen (n=137 104) waren akzidentell, 23,4% in suizidaler und 3,3% in missbräuchlicher Absicht. 0,3% der Suizidversuche verliefen tödlich. Während die absolute Häufigkeit der Suizidversuche von 2 422 im Jahre 2004 auf 3 458 im Jahr 2013 anstieg, blieb die relative Häufigkeit konstant bei 23,4%. Die absolute Häufigkeit der Suizidversuche war am höchsten im Frühjahr und Sommer. Suizidversuche wurden am häufigsten im Juli und August und am seltensten im Februar und September beobachtet. Die meisten Suizidversuche wurden sonntags bis dienstags und die wenigsten freitags registriert. Die Tagesspitze der Suizidversuche wurde um 22 Uhr erreicht und der Tiefpunkt um 6 Uhr. Der Altersmedian war 39 Jahre (1. Quartil 24 Jahre, 3. Quartil 50 Jahre). Der Frauenanteil war fast doppelt so hoch wie der Männeranteil. Schlussfolgerung: Kliniken im Einzugsbereich des Giftnotrufs Erfurt sollten im Frühjahr und Sommer, insbesondere im Juli und August, am Wochenanfang und kurz vor Mitternacht besonders auf Vergiftungsfälle in suizidaler Absicht vorbereitet sein.

Abstract:
The paper presents the initial results of the study of volcanic cave system and its typical formations in Krongno Volcano Geopark (KVG), Dak Nong, Vietnam. The volcanic caves have been discovered since 2007, under UNESCO sponsored the scientific project, are seen as unique geological heritages. The collaborative surveys and studies between Vietnamese geologists and the members of the Non-Profit Organization, Vulcanospeleological Society of Japan have discovered and surveyed 45 caves, and detailed mapping 20 caves. Using a complex of adequate methods, especially Remote Sensing image interpretation method, Surveying and mapping lava cave method, K/Ar dating isotopic analytical method and Current methodology, the studies aim to affirm endogenous origin of the lava cave system, the formation mechanism, as well as the typical formations of the caves. Up to date, the lava caves and interior formation in lava caves in KVG have been examined and evaluated in term of geological nature and recognized as pillar geological heritages of the Geopark.References Allred K., AllredC., 1997. Abstract:
Zusammenfassung Ziel der Studie: Durch eine Fülle von Informationen im Internet sind psychotrope Pflanzen und Pilze von steigender Bedeutung als legale und billige Alternative zu klassischen Rauschdrogen. Da Daten zum Missbrauch biogener Rauschdrogen in Deutschland bisher fehlen, wurden entsprechende Fallberichte von drei deutschen Giftinformationszentren (GIZ) untersucht. Methodik: Die Daten der durch die GIZ in Berlin, Erfurt und Freiburg zwischen 2007 und 2013 erfassten Missbrauchsverdachtsfälle (MVF) mit ausgewählten biogenen Rauschdrogen wurden retrospektiv ausgewertet. Ergebnisse: 602 MVF mit biogenen Rauschdrogen wurden durch die GIZ dokumentiert. Davon entfielen 52% auf die Exposition tropanalkaloidhaltiger Pflanzen (Engelstrompete, Stechapfel, Tollkirsche), 25% auf psilocybinhaltige Pilze und 10% auf Muskatnuss. Am häufigsten wurden Expositionen mit Engelstrompete berichtet. Tropanalkaloidhaltige Pflanzen werden überwiegend von Jugendlichen konsumiert, wohingegen psilocybinhaltige Pilze in der Mehrzahl von jungen Erwachsenen und Muskatnuss mehrheitlich von Schulkindern zu Missbrauchszwecken aufgenommen werden. Nach dem Abusus von Engelstrompete, Stechapfel und Tollkirsche traten in 55% der Fälle mittelschwere und schwere Symptome auf. Durch die GIZ erfolgte in mehr als 90% der MVF mit tropanalkaloidhaltigen Pflanzen und psilocybinhaltigen Pilzen eine Therapieempfehlung zur stationären Behandlung. Mydriasis, Halluzinationen, Tachykardie und Agitiertheit wurden als häufigste Symptome nach der missbräuchlichen Aufnahme biogener Drogen identifiziert. Schlussfolgerung: Die Daten der 3 GIZ lassen einen Rückschluss zum Missbrauch biogener Drogen in Teilen Deutschlands zu. Der Abusus biogener Drogen zeigt insgesamt eine abnehmende Tendenz, was wahrscheinlich mit dem zunehmenden Aufkommen neuer synthetischer Designerdrogen („Legal Highs“) zusammenhängt. Engelstrompete wurde am häufigsten missbräuchlich aufgenommen, was die Bedeutung einer leichten Verfügbarkeit beim Abusus von Pflanzen und Pilzen unterstreicht. Die steigende Tendenz der missbräuchlichen Exposition von Muskatnuss kann als überraschendes Ergebnis bewertet werden.

Zusammenfassung Ziel der Studie Substanzmissbrauch war in den letzten Jahren starken Veränderungen unterworfen und ist eine große Herausforderung für die Klinische Toxikologie. Deswegen wurden Substanzmissbrauchsfälle der letzten 15 Jahre im Einzugsbereich des Giftnotrufs Erfurt analysiert und mit humanen Expositionsfällen anderer Ursache verglichen. Methodik Retrospektive Analyse aller Humanexpositionsfälle (Expositionsfälle des Menschen gegenüber einer Substanz unter akzidentellen, missbräuchlichen oder unbekannten Umständen oder im Rahmen eines Suizidversuches) von Anfang 2002 bis Ende 2016 nach Substanzklassen, Symptomschwere, Altersgruppen und Geschlecht. Ergebnisse Substanzmissbrauchsfälle (7237, 3,5% aller Expositionsfälle) stiegen kontinuierlich von 250 (Einzelsubstanzmissbrauch: 159; Mischkonsum: 91) in 2002 auf 830 in 2016 (Einzelsubstanzmissbrauch: 398; Mischkonsum: 432) an. Die Fallzahl von Methamphetaminmissbrauch stieg von 10 in 2003 auf 100 in 2012 und fiel auf 52 in 2016. Die Anzahl der Missbrauchsfälle mit Neuen Psychoaktiven Substanzen (NPS) erhöhte sich von 1 in 2008 auf 130 in 2015 und sank auf 90 in 2016. Substanzmissbrauchsfälle verursachten signifikant (p<0,001) häufiger mittelschwere (29,1%) und schwere Symptome (5,8%) als Expositionsfälle in suizidaler Absicht (11,6%; 4,9%). Missbrauchsfälle mit NPS und der Untergruppe synthetischer Cannabinoide zeigten signifikant (p<0,001 u. 0,025) öfter mittelschwere und schwere Symptome (46,9% u. 43,6%; 7.9% u. 6,0%) als Missbrauchsfälle mit Cannabis (19,7%; 2,1%). Schlussfolgerung Die klinische Bedeutung des Substanzmissbrauchs wird daran erkennbar, dass er häufiger mittelschwere und schwere Symptome verursachte als Suizidversuche. Daten zu Substanzmissbrauch von Giftnotrufzentralen könnten offizielle Drogenberichte hinsichtlich klinisch-toxikologischer Aspekte ergänzen.

Abstract:
Der SC Motor/FC Carl Zeiss Jena war seit Ende der 50-er Jahre bis in die 80-er Jahre hinein ein vom DFV der DDR und vom DTSB immer wieder benannter und bestätigter Schwerpunktclub innerhalb der sogenannten zivilen Clubs. Der SC Turbine/FC Rot-Weiß Erfurt konnte diesen Status innerhalb des Fußballverbands dagegen nie erreichen. Die zentrale Frage dieser Dissertation nach den spezifischen Bedingungsgefügen des zivilen Schwerpunktclubs FC Carl Zeiss Jena (und Vorgänger) und des zivilen Nichtschwerpunktclubs FC Rot-Weiß Erfurt (und Vorgänger) im DDR-Fußballsystem ergab sich aus dieser unterschiedlichen Privilegierung und den ungleichen Erfolgsbilanzen dieser beiden Clubs. Die Hypothese der komparativ angelegten Fallstudie vermutete einen unmittelbaren Zusammenhang zwischen diesen deutlich sichtbaren Erfolgsunterschieden der beiden Mannschaften in der DDR und den erfolgten Schwerpunktfestlegungen. Zusätzlich konnte vermutet werden, dass ein beträchtlicher Anteil an den Jenaer Erfolgen auf die besonders starke Unterstützung des wirtschaftlich mächtigen VEB Carl Zeiss Jena zurückzuführen war. Um diesen Zusammenhängen nachzugehen, fragte der Autor nach den konkreten Bevorzugungen des Jenaer Schwerpunktclubs und den Benachteiligungen des Erfurter Nichtschwerpunktclubs und nach den spezifischen Bedingungen und Handlungsspielräumen der beiden Thüringer Mannschaften in der DDR. Daraus ergaben sich eine Reihe von detaillierten, auf einen Vergleich der verschiedenen Bedingungen in Erfurt und in Jena hin orientierte, Fragen, welche in der vorliegenden Untersuchung detailliert beantwortet werden: Wie sah die besondere Förderung des DFV bzw. des DTSB für einen Schwerpunktclub wie Jena überhaupt aus? Wer nahm Einfluss auf die Clubs, von wem waren diese abhängig, wer förderte sie durch welche Leistungen? Wie wurden diese Beschlüsse vor Ort umgesetzt? Wer waren die Trägerbetriebe und in welchem Maße und wodurch engagierten sich diese für den Fußball in Erfurt und Jena? Wie kamen die häufigen Wechsel der besten Spieler Erfurts nach Jena zustande? Warum war die Richtung dieser Wechsel insgesamt einseitig in Richtung Jena? Welche finanziellen, materiellen und sozialen Bedingungen konnten den Spielern in Jena und Erfurt geboten werden? Die vorliegenden Ergebnisse dieser erstmals für die zivilen Clubs auf der Mikroperspektive angelegten systematischen Untersuchung bestätigen das bereits von Hans Joachim Teichler als grundlegend für den DDR-Fußball beschriebene Konfliktmuster des „Fußball-Lokalpatriotismus versus Parteiräson“. Eigenmächtige Handlungen vieler Betriebsleiter und zahlreicher Partei- und Gewerkschaftsfunktionäre in den Trägerbetrieben konnten beispielsweise in Erfurt bei der eigenmächtigen Erhöhung der Aufnahmezahlen von Fußballern an die KJS Erfurt oder in Jena bei der Anstellung der Fußballer im Zeisswerk nachgewiesen werden. Das am sowjetischen Vorbild orientierte Sportsystem der DDR mit seinen engen Bindungen an die Trägerbetriebe provozierte geradezu verdeckte Zuwendungen der Betriebe, die über die Clubs an die Spieler weitergereicht wurden. Für die zentralen Instanzen des DDR-Fußballs war das ein Dauerproblem, weil sich damit ein Großteil der Vorgänge vor Ort der Steuerung entzog. Wie in der vorliegenden Arbeit beschrieben wird, war genau dies jedoch der Schlüssel für den Erfolg des SC Motor/FC Carl Zeiss Jena vom Ende der 50-er bis in den Anfang der 80-er Jahre bzw. für den vergleichsweisen Misserfolg des SC Turbine/FC Rot-Weiß Erfurt im gleichen Zeitraum. Dass letztlich die finanziellen, materiellen und sozialen Möglichkeiten die entscheidende Gründe für die Spieler waren, zu einem anderen Club oder einer BSG zu wechseln, mithin demnach Marktmechanismen, und hier in erster Linie der Grund für die Stärke des SC Motor/FC Carl Zeiss Jena zu suchen ist, ist eine zentrale Erkenntnis dieser Arbeit.<br>The SC Motor/FC Carl Zeiss Jena was in the late 50's to the 80's one of the DFV der DDR and of the DTSB repeatedly nominated and confirmed priority club within the so-called civilian clubs. The SC Turbine/FC Rot-Weiss Erfurt could never reach this status within the Football Association. The central question of this thesis to the specific structure of conditions of the civilian priority club FC Carl Zeiss Jena (and earlier) and of the civilian non-focal clubs FC Rot-Weiss Erfurt (and earlier) in the East German football system resulted from these different privileges, and the uneven track records of these two clubs. The hypothesis of the comparative case study to suspected a direct relationship between these highly visible success differences between the two teams in the East and made the key requirements. Additionally it was suggested that a significant proportion of the Jena successes to the particularly strong support of the economically powerful VEB Carl Zeiss Jena was due. To investigate these relationships, the author asked about the specific preferences of the Jena focal club and the disadvantages of the Erfurt non-focal clubs and on the specific conditions and scope for action of the two Thuringian teams in the GDR. This resulted in a series of detailed, based on a comparison of the different conditions in Erfurt and Jena out questions that are answered in detail in this study: What was the specific support of the DFV or the DTSB for a priority club like Jena at all from? Who took effect on the club, by whom they were dependent on who they supported by what is included? How these decisions were implemented on site? Who were the carrier companies and to what extent and how dedicated these for football in Erfurt and Jena? How did the frequent change of the best players reached Erfurt in Jena? That were the financial, physical and social opportunities the key reason for the players, ultimately, to move to another club or BSG, consequently, therefore market mechanisms, and this is primarily the reason for the strength of the SC Motor/FC Carl Zeiss Jena looking for is is a central finding of this study.

Bakgrund: Den 1 januari 2018 infördes en ny lag om samverkan vid utskrivning från slutenvården. Lagen reglerade samordnad vårdplanering samt kommunernas betalansvar. Forskningen visade att kommunikation mellan vårdgivare gick att förbättra för att öka förståelse för varandras arbete. Genom uppföljning av patienter kunde en trygg hemgång säkras och återinläggningsfrekvensen kunde minskas. Syfte: Syftet var att belysa hur sjuksköterskor inom kommunal hemsjukvård erfor de förändringar i vårdplaneringsprocessen som infördes vid utskrivning av patienter från somatisk slutenvård efter lagändringen den 1 januari 2018. Metod: Studien genomfördes med en kvalitativ design. Semistrukturerade intervjuer genomfördes efter ett ändamålsenligt urval samt kompletterades med ett snöbollsurval. Studien innefattade åtta intervjuer med informanter från två kommuner i Kronobergs län. Det insamlade materialet analyserades därefter med en kvalitativ innehållsanalys. Resultat: Analysen resulterade i sex kategorier och tio underkategorier. Efter lagändringen gick vårdplaneringsprocessen snabbare vilket gav negativ inverkan på delaktigheten och patientsäkerheten. Kommunikation och samarbete både inter- och intraprofessionellt hade stor vikt under vårdplaneringsprocessen. Trots en god vårdplanering kunde organisatoriska faktorer orsaka problem i samband med utskrivningar från slutenvården. Resultatet påvisade vissa brister och förbättringsområden i samband med vårdplaneringsprocessen. Studien bidrog med ökad förståelse och ny kunskap om hur sjuksköterskor erfor förändringar i vårdplaneringsprocessen. Slutsats: De kortare vårdtiderna inom slutenvården ledde till att hemsjukvården fick ta ett större ansvar för patienters vård. Det framkom även att det var viktigt att bibehålla en god patientsäkerhet och möjliggöra en trygg hemgång för patienter. Det framkom flera förslag på förbättringsåtgärder inom området.

In 2016, the first German bimodal bilingual co-enrollment program for deaf and hard-of-hearing (DHH) students, CODAs, and other hearing children was established in Erfurt, Thuringia. There is a tradition of different models of co-enrollment for DHH children in a spoken language setting in Germany, but there has been no permanent program for co-enrollment of DHH children who use sign language so far. This program draws from the experience of an existing model in Austria to enroll a group of DHH children using sign language in a regular school and from two well-documented bimodal bilingual programs in German schools for the deaf. The chapter describes the preconditions for the project, the political circumstances of the establishment of bimodal bilingual co-enrollment, and the factors that seem crucial for successful realization.
