Abstract:

La sindrome di Pendred è, in ordine di frequenza, la seconda causa di ipoacusia su base genetica autosomica recessiva. Si manifesta con un ipoacusia accompagnata dalla presenza di un gozzo tiroideo con eventuale ipotiroidismo. Tali caratteristiche si accompagnano a malformazioni dell’orecchio interno, quali l’acquedotto vestibolare largo. Nel 50% dei casi vi è una mutazione del gene SLC26A4. Riportiamo nel presente lavoro il caso di una paziente portoghese di 47 anni affetta da ipoacusia di grado severo/profondo e ipotiroidismo. La madre e la sorella della paziente, entrambe decedute, erano a loro volta affette da ipoacusia associata a gozzo tiroideo. La risonanza magnetica e la TC hanno entrambe evidenziato un allargamento dell’acquedotto vestibolare e del sacco endolinfatico. La paziente è stata sottoposta a uno studio di GJB2 e GJB6 seguiti da uno screening di tutti gli esoni di SLC26A4 e delle regioni introniche 8 e 14. È stata rilevata, per la prima volta in omozigosi, una mutazione c.918 + 2T>C nella regione intronica 7 del gene SLC26A4. Sequenziando i campioni di controllo è stata rilevata una nuova mutazione c.821C>G presente in eterozigosi nell’estone 7 del gene SLC26A4, per la quale si è ipotizzato un ruolo dannoso. Il presente studio ha condotto alla scoperta di due nuovi genotipi di SLC26A4, e alla miglior definizione degli aspetti fenotipici associati alla sindrome di Pendred.

ESOI for the experimental condition was greater than for the control and within the experimental condition at 12, 16, and 20 min. No other differences were found for the IPT measures. There were no differences for ESOI and IPT between genders.Conclusions:Cryotherapy enables patients to tolerate greater output intensities but does not result in increased peak torque

Abstract:

Methanolic extracts from corpora cardiaca of three moth species, Hippoteon eson (Sphingidae), Imbrasia cytherea (Saturniidae) and Bombyx mori (Bombycidae) show adipokinetic activity in conspecific bioassays. Haemolymph carbohydrates in these moths are not affected. These extracts are also active in heterologous bioassays: haemolymph lipids are increased in Locusta migratoria, whereas a small effect on haemolymph carbohydrates was observed in Periplaneta americana. Therefore, locusts can be used to monitor adipokinetic activity in corpora cardiaca from moth extracts during isolation. The three moth species possess an adipokinetic peptide with the same retention time on reversed phase high performance liquid chromatography (RP -HPLC) as a peptide isolated previously from Manduca sexta, which was code-named Mas-AKH. H. eson contains a second active peak with a similar retention time on RP-HPLC as the hypertrehalosaemic peptide isolated previously from Helicoverpa zea , code-named Hez-HrTH. Both synthetic peptides, Mas-AKH and Hez-HrTH, produce an adipokinetic effect in the three experimental moth species. In H. eson, the haemolymph concentration of Mas-AKH or Hez-HrTH needed to elicit a maximum hyperlipaemic response is about 20 to 30 nᴍ.Flight behaviour in the three moth species is quite different: H. eson is a good hovering flyer, I. cytherea is a comparatively bad flyer and B. mori males show only degenerate flight movements during their mating dance. Haemolymph lipid levels in H. eson decrease drastically during 15 min of flight and return to pre-flight levels in a subsequent rest period. The amount of lipids metabolized during flight is 10.9 mg/gxhr. Haemolymph carbohydrate levels drop during flight, but remain low during the 45 min of recovery. Haemolymph lipids in “dancing” males of B. mori remain constant. In individuals, however, which have low initial lipid levels in the blood, lipid concentrations increase significantly in a subsequent 15 min rest period after “dancing”. Metabolic changes during flight in I. cytherea were not investigated due to this species’ poor flight performance.

Abstract:

P2X7R is a purinergic receptor with broad expression throughout the body, especially in immune system cells. P2X7R activation causes inflammatory mediators to release, including interleukin-1β (IL-1β), the processing and release of which are critically dependent on this ion channel activation. P2X7R’s therapeutic potential augments the discovery of new antagonistic compounds. Thus, we investigated whether the Eugenia sulcata essential oil could block P2X7R activity. The essential oil (ESO) dose-dependently inhibited ATP-promoted PI uptake and IL-1β release with an IC50 of 113.3 ± 3.7 ng/mL and 274 ± 91 ng/mL, respectively, and the essential oil nanoemulsion (ESON) improved the ESO inhibitory effect with an IC50 of 81.4 ± 7.2 ng/mL and 62 ± 2 ng/mL, respectively. ESO and ESON reversed the carrageenan-activated peritonitis in mice, and ESON exhibited an efficacy higher than ESO. The majority substance from essential oil, β-caryophyllene, impaired the ATP-evoked PI uptake and IL-1β release with an IC50 value of 26 ± 0.007 ng/mL and 97 ± 0.012 ng/mL, respectively. Additionally, β-caryophyllene reduced carrageenan-induced peritonitis, and the molecular modeling and computational simulation predicted the intermolecular interactions in the P2X7R situs. In silico, results indicated β-caryophyllene as a potent allosteric P2X7R antagonist, although this substance may present toxic effects for humans. These data confirm the nanoemulsion of essential oil from E. sulcata as a promisor biotechnology strategy for impaired P2X7R functions and the inflammatory response.

Abstract Decadal and longer time-scale variabilities of the best known El Niño–Southern Oscillation (ENSO) indexes are poorly correlated before 1950, and so knowledge of interdecadal variability and trend in ENSO indexes is dubious, especially before 1950. To address this problem, the authors constructed and compared physically related monthly ENSO indexes. The base index was El Niño index Niño-3.4, the sea surface temperature (SST) anomaly averaged over the equatorial box bounded by 5°N, 5°S, 170°W, and 120°W; the authors also constructed indexes based on the nighttime marine air temperature over the Niño-3.4 region (NMAT3.4) and an equatorial Southern Oscillation index (ESOI). The Niño-3.4 index used the “uninterpolated” sea surface temperature data from the Second Hadley Centre Sea Surface Temperature dataset (HadSST2), a dataset with smaller uncertainty and better geographical coverage than others. In constructing the index, data at each point for a given month were weighted to take into account the typical considerable spatial variation of the SST anomaly over the Niño-3.4 box as well as the number of observations at that point for that month. Missing monthly data were interpolated and “noise” was reduced by using the result that Niño-3.4 has essentially the same calendar month amplitude structure every year. This 12-point calendar month structure from April to March was obtained by an EOF analysis over the last 58 yr and then was fitted to the entire monthly time series using a least squares approach. Equivalent procedures were followed for NMAT3.4 and ESOI. The new ESOI uses Darwin atmospheric pressure in the west and is based on theory that allows for variations of the atmospheric boundary layer depth across the Pacific. The new Niño-3.4 index was compared with NMAT3.4, the new ESOI, and with a record of δ18O from a coral at Palmyra, an atoll inside the region Niño-3.4 (Cobb et al.). Correlation coefficients between Niño-3.4 and the three monthly indexes mentioned above before 1950 are 0.84, 0.87, 0.73 and 0.93, 0.86, 0.73 for decadal time scales. These relatively high correlation coefficients between physically related but independent monthly time series suggest that this study has improved knowledge of low-frequency variability. All four indexes are consistent with a rise in Niño-3.4 SST and the weakening of the equatorial Pacific winds since about 1970.

Abstract:

The Interface Object Library based on the Motif extension of the X Windows system and on the ESONE SVIC-VCC Library is presented. Some features of the applications for controlling a synchrotron radiation experiment are discussed. The Interface Object Library is written in the object-oriented C++ language. The library class-hierarchy structure is presented and discussed. Several interfaces were realized in the Interface Object Library: the Windows interface, the CAMAC interface and the interface for supporting the experiment. The behaviour of the objects describing the CAMAC crate and CAMAC block is discussed. The application of these protocols for controlling the fast one-coordinate position-sensitive X-ray detector OD3 is presented.

e20524 Background: over the past decade, personalized management based on the molecular features of tumours in patients with advanced non small-cell lung cancer (NSCLC) has entered routine clinical practice. The poor performance of many advanced NSCLC patients may limit invasive biopsies. The liquid biopsy is a diagnostic procedure performed on cancer-derived material obtained in blood samples. In this abstract, we will describe our experience with liquid biopsies. Methods: In the Reggio Emilia Clinical Cancer Centrefrom March 2016 to December 2016, 42 patients with advanced NSCLC were analyzed that had had or had already started first line therapy. The liquid biopsy was repeated at each imaging response evaluation by thoracic-abdominal compound tomography (CT) scan performed every 3 months. In the liquid biopsy, the mutational status of EGFR was analyzed with real time PCR (KIT cobas EGFR mutation test v2 CE-IVD Roche); in tissue, it was evaluated by pyrosequencing. Results: 21/42 liquid biopsies were EGFR-mutated (12/21 eson 19 and 9/21 eson 21). In 3/21 (14.3%) cases, the tissue biopsies showed wild type (WT) EGFR. 6 liquid biopsies were also performed at time 0 (diagnosis). All liquid biopsies of EGFR WT remained WT during treatment and imaging evaluation. The median number of liquid biopsy tests for patients was 2 (range 1-3). In 4/21 cases, T790M was performed: 3 cases in both liquid biopsies and tissue, and 1 case in tissue but not in liquid biopsy. TKi therapy was ineffective in this patient with T790M mutation detected in tissue, but not in liquid biopsy. In all patients, the disappearance of the T790M mutation during TKi therapy was related to disease progression. In 11 cases, modification of EGFR mutation status during treatment anticipated CT scan evidence of disease progression (median = three months). Conclusions: the liquid biopsy is an excellent resource. In our experience the liquid biopsy is the sensitive method of choice during treatment of advanced NSCLC patients. EGFR modification status during TKi therapy showed advanced disease progression.

O artigo analisa a formação dos agentes de informação e/ou repressão que atuaram em dependências públicas civis e/ou militares durante a última ditadura brasileira (1964-1985). Para isso, analisamos as mudanças ocorridas no pensamento militar sob a influência estadunidense e francesa, bem como a criação de cursos, como os ministrados na Escola Superior de Guerra (ESG) e na Escola Nacional de Informações (ESNI), esta última uma criação da ditadura civil-militar brasileira. Um terceiro aspecto analisado diz respeito ao contato estabelecido com oficiais estrangeiros através do oferecimento de treinamento em território nacional e/ou no envio de brasileiros para a realização de cursos fora do país. Consideramos este último ponto como fundamental para a criação do Plano Condor, a ação conjunta das ditaduras do Cone Sul na perseguição aos oponentes políticos além das suas fronteiras.

Los estudiosos reconocen que las migraciones se han convertido en un desafío fundamental de la vida política, social e imaginaria de las regiones del «Tout-Monde» (Glissant, 1993). Por lo mismo hay una innegable acentuación de las migraciones intraafricanas, transafricanas y transcontinentales. A las migraciones Sud-Sud, se añaden las migraciones hacia Europa/Occidente que captan todavía más la atención de los medios de comunicación masiva. El presente artículo analiza, a partir de las novelas Juicio a un Estado y a un continente de Inongo Vi-Makomè y Ecos de Malabo de Maximiliano Nkogo Esono, la perpetuación por los migrantes subsaharianos de la única imagen positiva/idílica que Europa/Occidente intenta, con éxito, proyectar a través del mundo. El estudio ha demostrado que la mayoría de los migrantes (o candidatos a la migración) particularmente negro africanos, como Mvumba Essono y Miko, en el caso que nos ocupa, se convierten consciente o inconscientemente en fieles “embajadores” o defensores del Eldorado que los europeos/occidentales se esfuerzan a difundir de sus países, puntos de caída de los migrantes.

Abstract:

In this article, a Quasi Z-source inverter (QZSI)-based unified power quality conditioner (UPQC) backed by the solar photovoltaic (SPV) is presented in order to enhance power quality. The UPQC consists of converters connected in parallel and series. The active power filters (APFs) connected in parallel and series is one of the versatile custom power circuitries to reduce current and voltage instabilities. The main functions of QZSI can increase the variable direct current (DC) voltage to any given alternating current (AC) output voltage, reduce the necessary elements, and alleviate harmonic content. The UPQC’s compensation function primarily relies on the control system used for the generation of reference current and voltage. The enhanced second order generalized integrator (ESOGI) is used in this proposed system to extract the reference current of QZSI-UPQC. The proposed UPQC uses the SPV system, which has an energy storage unit to offset long-term current and voltage disruptions and fulfill the active power demands of the grid. Abstract:

The aim of the present study was to identify and assess the determinants of the content of Ca, Mg, Zn, Fe, Cd, and Pb in longissimus lumborum (LL) and semimembranosus (SM) muscles of fallow deer and red deer reared in the organic system. The concentrations of 6 elements (Ca, Mg, Zn, Fe, Cd, Pb) in fallow deer and red deer meat produced in the organic system in Poland were analyzed with the use of inductively coupled plasma mass spectrometry. The animals, i.e. 12 ca. 2- and 3-year-old fallow deer (Dama dama) and 12 ca. 3- and 4-year-old red deer (Cervus elaphus), were culled during the autumn season. Both groups of animals comprised 6 males and 6 females. A significant effect of the species on the content of selected minerals in the meat was noted. It was found that the LL muscle from the fallow deer was a rich source of Ca, Mg, and Zn, and the meat from the red deer had high amounts of Zn and Fe. The age of the animals had a significant effect on the level of Mg in the red deer meat.

Abstract:

Recent work on intelligent agents is a popular topic among the artificial intelligence community and robotic system design. The complexity of designing a framework as a guide for intelligent agents in an unknown built environment suggests a pressing need for the development of autonomous agents. However, most of the existing intelligent mobile agent design focus on the achievement of agent’s specific practicality and ignore the systematic integration. Furthermore, there are only few studies focus on how the agent can utilize the information collected in unknown build environment to produce a learning pipeline for fundamental task prototype. The hierarchical framework is a combination of different individual modules that support a type of functionality by applying algorithms and each module is sequentially connected as a prerequisite for the next module. The proposed framework proved the effectiveness of ESNI system integration in the experiment section by evaluating the results in the testing environment. By a series of comparative simulations, the agent can quickly build the knowledge representation of the unknown environment, plan the actions accordingly, and perform some basic tasks sequentially. In addition, we discussed some common failures and limitations of the proposed framework.

Indonesia merupakan salah satu Negara dengan kasus kecacingan yang cukup tinggi. Kasus kecacingan ini disebabkan oleh infeksi cacing usus yang proses menularnya melalui perantara tanah (Soil Transmitted helmint). Penyakit kecacingan banyak ditemukan di daerah dengan kelembaban tinggi terutama pada kelompok masyarakat dengan kebersihan diri dan keadaan lingkungan yang kurang baik. Cacing yang merupakan penyebab dari infeksi adalah Ascaris lumbricoides, Trichuris trichiura dan cacing tambang. Infeksi cacing ini mengakibatkan beberapa masalah yakni gangguan pencernaan, penyerapan serta metabolisme makanan sehingga protein, karbohidrat, lemak, vitamin dan darah dapat hilang dalam jumlah yang banyak. Eosin 2% merupakan pewarna yang selama ini digunakan dalam pewarnaan telur cacing nematode usus. Daun jati (Tectona grandis) merupakan tanaman yang memberikan warna alami karena mengandung.pigmen Antosianin. Antosianin mampu memberikan warna merah, biru, orange atau ungu. Tujuan dari penelitian ini adalah yaitu untuk mengetahui gambaran cacing STH (Soil Transmitted Helminth) menggunakan ekstrak daun jati (Tectona.grandis). Penelitian ini merupakan penelitian dekskriptif dengan pemeriksaan cacing menggunakan metode sedimentasi yang dilakukan dengan pewarna ekstrak daun jati dengan pelarut ethanol dan esoin 2% sebagai kontrol positif. Hasil penelitian menujukkan bahwa ekstrak daun jati konsentrasi 60% lebih baik dan mampu dijadikan sebagai pewarna pengganti eosin. Hal ini dapat disimpulkan bahwa ekstrak daun jati dapat digunakan sebagai pewarna alami dalam pemeriksaan telur cacing. Kata Kunci: Daun jati, Eosin, Soil Transmitted Helminth, Telur cacing

One of the predators of the ichthyofauna of the Lower Tobol is Esox lucius (Linnaeus, 1758) (Esociformes, Esocidae). The purpose of this work is to study the current state of the E. lucius parasite fauna in the lower reaches of the Tobol River. In the present study 47 individuals of the northern pike of different sex aged from 2+ to 8+ years were examined by the method of complete parasitological dissection. As a result of the research, 23 types of parasites were found in the pike. The largest number of species of parasitic organisms – 20 – were found in May, 12 were found in December and 11 in September. Of these, 10 species are specific to northern pike: Haemogregarina esoci (Nawrotzky, 1914), Chloromyxum esocinum (Dogiel, 1934), Myxidium lieberkuhni (Biitschli, 1882), Myxosoma anurum (Cohn, 1895), Henneguya psorospermica (Thelohan, 1995), Tetraonchus monenteron (Wagener, 1857), Gyrodactylus lucii (Kulakowskaja, 1951), Triaenophorus nodulosus (Pallas, 1781), Azygia lucii (Miiller, 1776), Raphidascaris acus (Bloch, 1779). For the first time an intraerythrocyte parasite – H. esoci – was found in pike in the Lower Tobol, the extensiveness of invasion was 18.7% in December, and 6.2% in May. Epistylis sp. and Trichodinella epizzotica parasitized on the surface of the body of fish only in May, whereas members of the genus Trichodina were found in winter and spring. In all seasons, parasitization by M. anurum and H. psorospermica was established on the gills. The greatest occurrence of spores of M. lieberkuhni and C. esocinum was observed in the spring. Myxosporidia parasitized in the kidneys and T. monenteron parasitized on the gills during all periods of the study (the latter with an extensivity of invasion of 100.0%). All examined fish are infected with T. nodulosus 100.0% of cases. In May, other types of cestodes were found: P. esocis and Dibothriocephalus latus. Three pike were infected with proteocephalis. The larval stage of diphyllobothriid was found in one fish in the liver and gonads in an amount of 139 individuals. In the pikes’ stomach, during all periods of the study, A. lucii was found in all the fish examined. In the autumn period of the study immature R. campanula were found in in the intestine of 4 fish specimens. The nematode R. acus was found in pike only in spring. This nematode was found in fish aged 4+–8+. In May, glochidia with a high intensity of invasion were found on fins, gill covers and gills of fish. E. sieboldi copepods were found on the gills; in May, one specimen was found in one pike and in September in 6 with AI of 0.7. Tetraonchus monenteron was a dominant species, in May its degree of dominance decreased with the greatest uniformity of species in this study period. Having considered the age dynamics of pike infection by various types of parasites, it was found that in the age group 4+–5+ the number of species of parasites was greatest (20), while in groups 2+–3+ and 6+–8+ it was 14. In the pike, the core parasitic fauna were M. anurum, H. psorospermica, T. monenteron, T. nodulosus, A. lucii. Seasonality has virtually no effect on the degree of infection with specific parasites. The age of fish largely determines the qualitative and quantitative composition of the parasite fauna.

Traditional Ghanaian seats have existed for centuries. As of now, there is no complete research material detailing its history, nature, types, functions, appropriate materials for production, and the symbolic designs conveyed in the craft making it incomplete to tell the rich culture of Ghana. The paper aims at providing a more detailed overview of the craft to serve as complete and reliable reference material to learners. This qualitative study employed the narrative research design. The study was conducted at Ahwiaa and Bomani Tyehimba traditional wood carving communities in the Ashanti and Eastern regions of Ghana respectively. Two historians; one each, and ten traditional stool carvers; five each from the visited carving communities were purposively selected and interviewed. The craft was initiated by the Ashantis and traveled to other regions. The craft has a flat rectangular base, a center section, and a crescent-shaped top. The center section contains distinctive motifs and symbolic meanings that are the source of the numerous stools' names. Some symbolic wisdom conveyed in the craft includes; Gye Nyame, Bi nka bi, Akofena, Sankofa, etc. Ahemmaa dwa, Mmarima dwa, Mmaa dwa, Osram dwa, Kotoko dwa, Esono dwa, etc. are some common types of the craft. It functions as practical items in every home, as a connection to rites of passage, as religious artifacts, and as a political allegory. Wood like Osese (Funtumia sp.), Nyame dua (Alstonia gongensis), Cedrela (Cedrela odorata), Mahogany (Swietenia mahagoni), teak (Tectona grandis), etc. are used for the production. The study will add up to the Ghanaian citizens, foreigners, and researchers’ knowledge to know much about the craft and the cultural heritage of the country.

Abstract:

The purpose of the research is study of four fish species infected with haematophagous parasites in the rivers of the Lower Irtysh basin. Materials and methods. The work was performed in the rivers of the Lower Irtysh basin in 2017–2020. A total of 390 fish of different sex and age were examined including 47 roach (Rutilus rutilus lacustris, (Pallas)), 41 ide (Leuciscus idus, (Linnaeus, 1758)), 47 pike (Esox lucius, (Linnaeus, 1758)) and 255 Siberian sterlet (Acipenser ruthenus marsiglii (Brandt, 1833)). We took blood from the tail artery, and examined its fresh drop under a microscope, and prepared a thin smear, then dried it in the air and fixed in a mixture of 70% ethyl alcohol and diethyl ether (1 : 1) for 30 minutes. The fixed smears were dried in the air at room temperature and stained with azure and eosin at a dilution of 1 : 10 for 40 minutes. We identified species of the found parasites and calculated the prevalence of infection with standard error of the mean.Results and discussion. We found representatives of the Coccidea and Kinetoplastea classes parasitizing in fish blood. Trypanoplasma acipenseris and Haemogregarina acipenseris were recorded in the Siberian sterlet. The pike was infected with T. schulmani and H. esoci. T. schulmani was found in the blood of the ide and roach. Piscicola geometra is the main host of flagellates and sporozoans that parasitize in fish blood in the basin of the Lower Irtysh. The roach and the Siberian sterlet become infected with parasitic flagellates from the age of 1+. All examined fish were infected in age groups 2+ – 4+. Haemogregarina parasitize in the pike aged 4+ and 5+, and in the Siberian sterlet aged 1+ – 4+ and 6+ – 7+.

Abstract:

Background and aims – A recent taxonomic and molecular study of the genus Cyrtorchis revealed three new species. Here, we describe one of these new species, endemic to the Cameroon Volcanic Line (CVL), and compare it to its closest relatives. Its conservation status is provided along with detailed information on its ecology and reproductive biology.Material and methods – A detailed examination of 21 herbarium specimens was performed using standard practices of herbarium taxonomy. The conservation status of the new species was assessed using the IUCN Red List Categories and Criteria. Furthermore, we investigated the reproductive biology of the new species in an easily accessible subpopulation in Cameroon and tested the hypothesis of hawkmoth pollination in the genus Cyrtorchis. Key results – The newly discovered species, here named C. okuensis, is restricted to the montane forest (1600 to 2500 m elevation) of the CVL and is currently assessed as Near Threatened (NT). Among angraecoid orchids in which hawkmoth pollination is recorded (~16 species), C. okuensis has the shortest nectar spur (19–40 mm). In situ observations revealed hawkmoths with short proboscis of the genus Hippotion (H. celerio, H. eson, and H. osiris) as the main pollinators. The rate of pollination and fruiting under natural conditions was low and among the 448 surveyed flowers only 38.8% (174/448 flowers) had their pollinia removed, 19% produced fruits (87/448 flowers), and 16% (70/448 flowers) set mature fruits. Conclusion – Cyrtorchis okuensis is clearly differentiated from C. submontana by its longer and wider floral bracts, longer anther caps, and wider triangular dorsal sepal.

A terhesség alatt nagy a magas vérnyomás kialakulásának kockázata, a szérum-kretaininszint emelkedik, és a terhesség végére proteinuria is kialakulhat. Summary. Kidney transplantation offers the best hope to women with end-stage renal disease who wish to become pregnant. Pregnancy in a kidney transplant recipient continues to remain challenging due to side effects of immunosuppressive medication, risk of deterioration of allograft function, risk of adverse maternal complications of preeclampsia and hypertension, and risk of adverse fetal outcomes of premature birth, low birth weight, and small for gestational age infants. The factors associated with poor pregnancy outcomes include the presence of hypertension, serum creatinine greater than normal range and proteinuria. The recommended maintenance immunosuppression in pregnant women is calcineurin inhibitors (tacrolimus/cyclosporine) and low-dose steroid which are considered safe. It is important that counseling for childbearing should start as early as prior to getting a kidney transplant and should be done at every clinic visit after transplant. Abstract:

A COVID-19 járvány hatásai miatt az autóipar hosszúra nyúlt, globális, többlépcsős, az egyes tevékenységeket egyes térségekbe kiszervező beszállítói láncain célszerű rövidíteni. A helyi gyártókhoz közelebb lévő beszállítói láncok kevésbé sérülékenyek a negatív globális folyamatokkal szemben, jobban menedzselhetők a határon átnyúló együttműködések keretében a tudás és technológia transzferrel, szellemi tulajdonjoggal kapcsolatos kérdések is. Alapvető fontosságú a legmegfelelőbb beszállítók kiválasztása. Ehhez a termékeket, szolgáltatásokat folyamatosan ellenőrizni kell, és szükség van a követelmények és a beszállítók teljesítőképességének ismeretére, valamint a beszállítókkal történő folyamatos kapcsolattartásra. Azon hazai vállalatok válhatnak beszállítóvá, amelyek korábban bizonyítottak a működési területükön. Ezen cégeknek a következő feltételeknek kell megfelelniük: a közvetlen vevők által igényelt termékek előállítására alkalmas technológiák és ezek működtetéséhez szükséges kapacitások, megfelelő tőkeerő és vállalatméret, többnyelvű kommunikáció; K+F képességek, képesek maguk is finanszírozni a piacra lépési költségeket (gépi és egyéb beruházások, vállalatirányítási rendszerek). Jelen esettanulmány áttekinti a direkt és indirekt autóipai beszállítói tevékenységeket, bemutatva a helyi kis-és középvállalatok (KKV) számára az indirekt beszállításban rejlő lehetőségeket egy Komárom-Esztergom megyei sikeres indirekt beszállító vállalat az Insol Kft. példáján keresztül. Általánosságban a hazai KKV-k könnyebben tudnak bekerülni egy új projektbe, mint egy már meglevő, minősített beszállító helyét átvenni a direkt „széria” beszállítások esetében (ahol az alkatrészek beépülnek a végtermékbe). Másfelől könnyen ki lehet esni a direkt beszállítói körből: elég néhány minőségi hibás teljesítés, nem az elvárások szerinti költséghatékony megoldás vagy határidőcsúszás. Ezért reálisabb cél lehet számukra az indirekt beszállítóként (amely a termelés működtetéséhez biztosít gépeket, alkatrészeket, szolgáltatásokat) integrálódás egy multinacionális vállalat ellátási láncába, mint a direkt beszállítói pozíció megszerzése. Ha indirekt beszállítóként egy helyi KKV beválik, akkor az adott KEM TIER1 vagy TIER2 ajánlása (referencia) alapján a többi európai gyárba is bekerülhet és megnyílik az út a nagyvállalattá válás előtt.

Desain penelitian ini menggunakan metode pendekatan kualitatif dengan metode analisis system dari Data <em>input ,proses ,outpu .</em>Penelitian ini dilakukan di Dinas Kesehatan Kota.Hasil penelitian menunjuk kan bahwa belum adanya kebijakan lokal yang mengatur tentang pelaksanaan program penanggulangan HIV dan AIDS ,sumberdaya tenaga yang masih kurang dan tugas rangkap ,Dinas Kesehatan tidak memiliki buku pedoman nasional manajemen HIV, dan ada dana dari APBD tidak terfokus untuk program penanggulangan HIV dan AIDS menyatu dengan dana pada bidang P2P, Masalah teknis peran serta dan koordinasi dengan linta sprogram dan lintas sektor belum berjalan dengan baik. Disaran kan agar pihak pemerintah daerah khususnya dinas kesehatan kota Bukittinggi membuat regulasi atau peraturan yang mengatur secara teknis pelaksanaan program penanggulangan HIV dan AIDS ini baik berupa kebijakan atau peraturan walikota sehingga program ini dapat berjalan secara maksimal, alokasi dana dari APBD harus terfokus khusus untuk program ini ,diharapkan juga pemerintah khususnya lebih memperhatikan terutama masalah nutrisi para ODHA yang bertugas sebagai penjangkau kasus dilapangan meningkatkan kerja sama lintas program dan sektor serta menguatkan komitmen demi keberhasilan dari program ini.</p><p><em><br /></em></p>
