Full textBackground: Nurses act on ethical and moral values to be able to give their patient ethical care. The foundation of the ethical care that nurses provide is based on their empathic and sympathetic abilities. The ethical ability can negatively be affected by stress. The definition of stress is a wide concept with different meanings. One specific type of stress that nurses encounters is the moral distress which is hard to explain and difficult to handle. The moral distress emerges when nurses are unable to act according to their moral judgment. Moral distress is emotional challenging and may affect patient care. Aim: To illustrate nurses’ experiences of moral distress. Method: The study is a literature review containing ten articles of qualitative design. The articles were analyzed according to Friberg’s method. In which results relevant to the aim were categorized and subsequently referred to themes and sub-themes. Results: The analysis resulted in four major themes: Experienced emotions of moral distress, Situations where feelings of moral distress is experienced, Encounters that’s makes nurses vulnerable and Handle the emotions of moral distress. The latter major theme included two sub-themes: The importance of dialog and Personal abilities and experiences. Discussion: The results of the literature review have been discussed in relation to Travelbees nursing theory and perception of the nurse and other relevant studies. The importance of mutual ethical guide lines between nurses and doctors is discussed and the need of ethical guidance and support in difficult care situations. The meaning of dialog and communication to make moral distress manageable for nurses is discussed.

Full textBackground: Moral distress arises when the nurse cannot implement a course of action that is considered to be right due to institutional restraints. This phenomenon is probably as old as the profession itself, yet it was first defined in the 80’s by Andrew Jameton. It is his definition that is used in this paper. The nurse is working close to the patient, the work also takes place within the frames of an organisation with hierarchies and demands of efficiency. The professional role is strongly associated with doing good and helping others, but in some situations the nurse is restrained from doing so due to external factors. Moral distress is one of the individual consequences of these situations. Aim: To describe situations that cause moral distress among nurses. Methods: To reach the aim a literature review was done. A search for relevant scientific articles that had used Jameton’s definition was done i databases. Articles with both qualitative and quantitative methods were used. Eleven articles were chosen, processed and analyzed. Studies taking place in Asia, Europe, USA and Africa were used in this literature review. Results: From the situations described in the literature three different main themes could be discerned: moral distress in encounters with other health care professions (1), with relatives (2) and with external circumstances (3). Common for these encounters is that they place the nurse in a constricted situation wtih the experience of having no choice. Discussion: Moral distress among nurses was interpreted as an expression of nurses' doubble roles; the protective role over to the patient and the subordinate role of the doctor. The situations that caused moral distress were experienced in similar ways in all countries where the studies took place regardless of the access of resources and other external circumstances. The result was also discussed in the context of Ida Jean Orlando's theory of patient relationships and the reflexive nature of caring practice. The moral distress was placed in a larger structural context to increase the understanding of the phenomenon and to make it possible to find institutional solutions to the problem.

Full textMoral stress affects the nurse and can be seen as a result of external factors. There is a growing demand that the nurse should be able to handle moral stressful situations. Through clinical supervision the nurse may be helped to verbalize her experiences. The aim of this study is to illustrate the way in which clinical supervision can help the nurse to deal with moral stress, focusing on the structure and content of clinical supervision. A literature study has been conducted. Ten articles have been reviewed and analyzed. To create an understanding for clinical supervision a theoretical framework has been used focusing on the structure and content of clinical supervision. The results show that the clinical supervision may affect the nursing staff’s moral stress, in both a positive and negative way, when the ethical competence of participants increases. Furthermore, it appears that staff sometimes chooses to turn to colleagues for support instead of organized clinical supervision. Ethical competence may increase through clinical supervision when increasing the participant's ability to reflect upon an ethical situation. However, this can harm the nurses’ ability to deal with future situations involving ethical issues when seeing a situation from a critical approach. Supervision, however, strengthens the nurses’ decision-making capacity and strengthens her confidence to act in ethically difficult situations.
