Full textThis work aims at understanding hope’s beneficial effects in physical and sports activities. After ensuring the viability and sustainability of Snyder’s hope model (Snyder et al., 1991, 1996) through a validated measurement tool, we tried to identify how hope contributes to success in physical education at school, and hope’s beneficial effects on students’ performance and resilience during a cycle ergometer session. The experimentation results prove Snyder’s hope model reliability from an empirical viewpoint, despite the reservations about its theoretical validity. In a school context, trait hope enables to predict their cycle-end grades, through a mediation of state hope and perceived ability. This data corroborates the existence of a hierarchical model in the concept of hope. Furthermore, hope can also be associated to better performance in experimental situation, after receiving a failure feedback. By having a protective effect on the result perception, hope offers a better resilience, immediately after the feedback. Thanks to the diversified contexts and methods, our research brings light on hope’s effects and consequences in physical and sports activities, and more generally speaking, on the relevance of this yet unknown theory that should be developped and extended to other study populations and contexts

Full textAbstract : Background: Cardiovascular disease (CVD) is a contemporary public health issue. However, clinical research shows that some interventions are effective in its treatment and prevention: nutrition education programs prioritizing whole plant foods (WPF). These interventions promote the adoption of diets characterized by the unlimited consumption of a wide variety of plant foods (e.g. legumes, whole grains, fruits, vegetables) and a decrease in consumption of animal (e.g. meat, eggs and dairy products) and highly processed foods (e.g. high in sugar, salt or fat and low in fiber). Objectives: Using a nested concomitant mixed design, we evaluated the effects of a nutrition education program to increase the consumption of WPF in adults at risk of CVD and explored the determinants of the observed behavioral changes. Methodology: Various physiological and anthropometric parameters were measured pre-post program (n = 72) and analyzed with t test for paired samples or signed rank test Wilcoxon. Also, 10 semi-structured interviews were conducted post-program, supported by an interview guide based on the Food Choice Process Model. The full transcripts were coded according to thematic analysis method. Results: After 12 weeks, weight (-10.5 lbs, 95 % CI: 9.0-12.0), waist circumference (-7.4 cm, 95 % CI: 6.5-8.4), diastolic blood pressure (-3.2 mmHg, 95 % CI: 0.1-6.3), total cholesterol (-0.87 mmol/ L, 95 % CI: 0.57-1.17), LDL cholesterol (-0.84 mmol/ L, 95 % CI: 0.55-1.13) and HbA1c (-1.32 %, 95 % CI - 0.17-2.80) improved significantly. Thematic analysis of qualitative data reveals that the program, by stimulating values such as health, ethics and integrity, promote the transformation of food choices towards a more whole-foods plant-based dietary pattern during a key period of participants’ life course (i.e. early retirement). Other determinants that can help orient food trajectory towards more WPF have been identified, including the significant short term benefits, the lack of restriction on the amount of WPF and development of planning skills in acquiring and preparing food. Conclusion: A nutrition education program prioritizing WPF improves the cardio-metabolic profile individuals in their pre-retirement because of its intrinsic characteristics, but also because it changes their food choice values.
Resumen : Contexto: Las enfermedades cardiovasculares (ECV) son un asunto contemporáneo de salud pública. Sin embargo, la investigación clínica demuestra que algunas intervenciones son eficaces en su tratamiento y prevención. Se trata de intervenciones nutricionales educativas que priorizan los alimentos vegetales mínimamente procesados (AVMP). Estas intervenciones promueven la adopción de posturas alimenticias que se caracterizan por el consumo ilimitado de una amplia variedad de alimentos de origen vegetal (e.g. legumbres, granos enteros, frutas, verduras) y una disminución en el consumo de alimentos de origen animal (e.g. carne, huevos y productos lácteos) y altamente procesados (e.g. es decir, alto contenido de azúcar, sal o grasa y bajo en fibras). Objetivo: Con el uso de un diseño mixto concurrente se evaluaron los efectos de un programa compuesto de diversas intervenciones educativas enfocadas en el incremento del consumo de AVMP en adultos en situación de riesgo de ECV. Así mismo, se exploraron los determinantes de los cambios de comportamientos observados. Material y métodos: Se midieron varios parámetros fisiológicos y antropométricos antes y después del programa (n = 72) los cuales se analizaron con la prueba t para muestras pareadas o prueba de los rangos con signo de Wilcoxon. Ademas, se llevaron a cabo 10 entrevistas semi-estructuradas post-programa apoyados por una guía de entrevista basada en el Food Choice Process Model. Las transcripciones completas fueron codificadas de acuerdo al método de análisis temático. Resultados: Después de 12 semanas, el peso (-10.5 lbs, IC 95 %: 9.0 a 12.0), la circunferencia de la cintura (-7.4 cm, IC 95 %: 6.5 a 8.4), la presión arterial diastólica (-3.2 mmHg, IC 95 %: 0.1 a 6.3), el colesterol total (-0.87 mmol/ L; IC 95 %: 0.57 a 1.17), el colesterol LDL (-0.84mmol/ L, IC 95 %: 0.55 a 1.13) y la HbA1c (-1.32 %, IC 95 % - 0.17 a 2.80) mejoraron significativamente. El análisis temático de los datos cualitativos revela que el programa, mediante la estimulación de valores de la salud, la ética y la integridad, favorece la transformación de la elección de alimentos hacia una postura más centrada en los AVMP durante un período clave del ciclo de vida (e.g. la jubilación temprana). Otros determinantes que pueden promover la adopción de una postura alimenticia más centrada en los AVMP han sido identificados, incluyendo los beneficios significativos observados a corto plazo, la falta de restricción en la cantidad de AVMP y el desarrollo de habilidades de planificación en la adquisición y preparación de alimentos. Conclusiones: Un programa de intervenciones educativas que favorece el consumo de AVMP mejora el perfil cardio-metabólico de las personas pre-jubiladas no sólo gracias a sus características intrínsecas, sino también al cambio de valores que intervienen en la elección de los alimentos.

Full textAlthough the annual number of new HIV infections has been decreasing since the middle of the 1990s’, it remains high, and particularly in sub-Saharan Africa. Epicentre, in collaboration with “Médecins Sans Frontières”, is planning a strategy to reduce HIV incidence in several areas of high prevalence and incidence rates, such as Ndhiwa sub-county (Nyanza Province, Kenya). In this sub-county, the adult HIV prevalence was estimated at 24% in 2012 (Ndhiwa HIV Impact in Population Survey, NHIPS, 2012). In the first part of this thesis, various aspects of HIV infection as well as current issues are given. In the second part, our objective is to estimate HIV incidence in Ndhiwa sub-county using several distinct approaches. In the last part, we focus on the mathematical modeling of the spread of the HIV infection to the short term. Firstly, we develop a framework for using the same data for estimation and prediction. Secondly, the potential impacts of multiple interventions on HIV incidence in Ndhiwa sub-county are studied using our framework and performing deterministic and stochastic simulations

Full textVentilator-associated pneumonia (VAP) is the most common hospital-acquired infection in the intensive care unit (ICU setting. Despite the availability of evidence-based guidelines to prevent VAP, they remain poorly implemented. The objective was to determine the effect of a 2-yr multifaceted program aimed at preventing VAP. The program involved a healthcare workers (HCWs) and included a multidisciplinary task force, an educational session, direct observations with performance feedback, technical improvements, and reminders. It focused on eight targeted measures based on well recognized published guidelines, easily and precisely defined acts, and directly concerned HCW's bedside behavior: 1 compliance with hand-hygiene rules including use of an alcohol-based hand rub, 2) proper glove and gown use, 3) keeping patients in the semi-recumbent position, 4) keeping the endotracheal tube cuff pressure >20 cm H2O, 5) using orogastric tubes rather than nasogastric tubes, 6) avoiding gastric overdistension, 7) decontaminating the patient's mouth with 0. 12% chlorhexidine at least four times/day, and 8) eliminating nonessential tracheal suction. We studied the impact of the programme on compliance with the targeted preventive measures, on HCWs'individual factors and on VA incidence and the role of technical devices. Our preventive program produced sustained VAP rate decreases in the long term. However, VAP rates remained substantial despite high compliance with preventive measures, suggesting that eliminating VAP in the ICU may be an unrealistic goal

Full textIn its contemporary conception, the hallucination is simply defined as a ‘perception without object to perceive’. It remains however complex to highlight it in clinical practice, in particular in pediatric population. The clinical interview, during which the clinician have to avoid stumbling blocks, such as psychic immaturity and potential existence of an imaginary companion, can be completed by methods allowing a more specific caracterisation of the phenomenon. This symptom has been a renewed interest in the last 30 years in scientific domain. It is now accepted that the hallucinations are not found exclusively in schizophrenic etiological context, but they can also occur during normal development and be spontaneous resolution. Despite this, few studies currently have a specific interest in its early and transdiagnostic character, and his treatment in children and adolescents.Nevertheless, differents strategies for actions are available to help us better understand this symptom, whether its evaluation or therapeutic. In this PhD work, we initially focused on cognitive aspects of hallucination. Based on the source monitoring framework, assuming that hallucinations are the result of a tendency to attribute more often self-generated events to an external subject, we standardized a task assessing this cognitive ability.We first highlight an age effect in our healthy subjects enrolled in the study of standardization, preferentially for the reality monitoring task (distinction between an external source and an internal source). We then compare these data with those obtained from patients suffering from schizophrenia, revealing loss performance for both tasks. Our goal is then to obtain normative data for a pediatric population.In a second step, we are interested in the psychometric assessment of early-onset hallucinations. Given the lack of tools, we have combined the playfulness of new technologies in a comprehensive and rigorous assessment of the symptom. We thus developed an application for touchpad associating the assessment of cognition, including the theory of mind, and symptom severity. Theory of mind developpement have recently been identified as prognostic marker in the resolution of hallucinations in children. The first objective of this work is to provide a reliable assessment or early-onset hallucinations, the underlying objective is to allow longitudinal assessment of this symptom in children and adolescents and to better adapt its therapeutic. Finally, we are interested in the development of an innovative therapy in children and adolescents, which is the repetitive trancranial magnetic stimulation (rTMS). This neuromodulation technique has the advantage to target a symptom, not a disease, and can therefore be applied to a number of hallucinated subjects.The objective of this work was to study early-onset hallucination through diverse approaches facilitating the trust of the child and both phenomenological and cognitive caracterisation of the symptom by the clinician.The results gathered in this PhD work demonstrate that it is possible to understand the early-onset hallucinatory symptoms through cognition and new technologies. These approaches allow us to adapt our daily practice, gives us a better understanding of the symptoms and associated cognitive mechanisms, thereby improving our therapeutic management

Full textAssessing fitness to drive of individuals with medical conditions has become a major road safety issue in most industrialized countries. The aim of this thesis was to assess, from a public health point of view, the opportunity of screening for inability to drive. The first step was to examine potential issues to be solved before screening can be implemented and effective : 1/ demonstrating, using insurance data from Quebec, that there might be an opportunity to detect, during usual medical visiits, medical conditions that potentially put older drivers at higher risk of collision ; 2/ adaptating criteria proposed to judge the indication of screening for chronic diseases to the issue of screening in the ontext of road safety. Using this framework allowed us to conclude that screening for inability to drive was not indicated now, given the lack of evidence. This result has justified developing a decision analysis model to assess the potential effectiveness and feasibility of three possible strategies to deal with individuals potentially unfit to drive. From this decision analysis, based on a Markov model, we concluded that, whatever the medical condition, the age when detection starts, and the rythm of screening, a no-screening strategy was always better than a single-test screening strategy. By providing a better understanding of the issue of screening for inability to drive, this work underlines the need for the organization of a task force to develop recommendations for possible actions and researches to answer unresolved emerging questions

Full textDisability is a major barrier to the labor market. In Canada, federal and provincial initiatives aim for a better integration of people with disabilities. The province of Quebec in particular has an unique programme to promote the hiring and retaining of people with disabilities in a non sheltered workplace: the job integration contract. The objective of this research to draw a portrait of the Canadian population with disabilities and to assess the impact of the job integration contract on employment. We use data from the Survey of Labor and Income Dynamics and the Canadian Survey on Disability. We apply the difference-in-difference method using people with disabilities in the rest of Canada as our control group. We find that education, severity and the need for the workplace adaptations are the most important factors in the job market for people with disabilities. The first results of the evaluation suggest that the policy had no significant effect for men with disabilities but that it contributed to an increase in the participation of women with disabilities by 9 percentage points over the studied period. However, additional results from the triple difference are not significant for both men and women.

Full textAbstract : Sports training is an athletic development process based on scientific principles aimed at enabling athletes to achieve enhanced or superior performance in a sport through planned and systematic influences (training loads) on their performance ability (Harre, 1982). An appropriate sports training program should start at an early age, so young athletes can progressively and systematically develop their body and mind to achieve sporting excellence (Bompa, 2000; Weineck, 1997). Yet, as they try to rapidly achieve superior results, many coaches impose a very specific and rigorous training regime on young athletes without taking the time to properly develop the basic physical and motor abilities underlying the specific sports skills (Bompa, 2000) – hence the term “early sport specialization”. New approaches have been put forward to address the negative consequences of early sport specialization. One approach is to participate in several sports at an early age (FraserThomas, Côté and Deakin, 2008; Gould and Carson, 2004; Judge and Gilreath, 2009; LeBlanc and Dickson, 1997; Mostafavifar, Best and Myer, 2013) – hence the term “sport diversification”. Many professional and sports organizations have decided to promote and implement programs based on sport diversification (Kaleth and Mikesky, 2010). Aware of the adverse effects of early specialization, some Quebec high school physical education professionals (a physical educator, a kinesiologist and a sports agent) have implemented an innovative junior high school multisport program drawing on sport sciences and the Long-Term Athlete Development (LTAD) model guidelines (Balyi et al., 2005). This research project examines physical and motor abilities development among young athletes enrolled in a sport specialization program as well as young athletes enrolled in a sport diversification program who have reached the “Train to Train” stage of the LTAD model (12 to 16 years old) (Balyi et al., 2005). The main objective of this research project is to document the changes in the physical and motor abilities of young student-athletes enrolled, on one hand, in a sport studies program in soccer (specialization) and, on the other hand, in a multi-sport program (diversification). More specifically, this research attempts to (a) provide a comprehensive picture of the changes in physical and motor abilities of the studentathletes in each program, and hopes to draw a parallel with the annual planning of each sports program, and (b) identify any contrasts between the physical and motor abilities of the student-athletes in each program. 9 The results indicate that (a) the student-athletes in both sports programs improved all their physical and motor abilities during the school year; (b) it is relatively easy to draw a parallel between the development of the physical and motor abilities of the student-athletes and the annual planning of each sports program; (c) overall, the student-athletes enrolled in the multi-sport program demonstrated similar or better performance than the student-athletes enrolled in the sport studies program in soccer; and (d) during the school year, student-athletes enrolled in the sport studies program in soccer showed more improvement of their cardio-respiratory endurance, whereas the student-/athletes enrolled in the multi-sport program showed more improvement in (a) the segmental speed of their arms, (b) their agility during the circle run test and (c) the muscle power of their lower limbs, confirming that young athletes who specialize early, develop a set of physical and motor abilities that are rather sportspecific (Balyi et al., 2005; Bompa, 1999; Cloes, Delfosse, Ledent and Piéron, 1994; Mattson and Richards, 2010), whereas the skills developed through sport diversification are more varied (Coakley, 2010; Gould and Carson, 2004; White and Oatman, 2009). The research project was realized in a Quebec high school. In total, 53 secondary one student-athletes were selected for the research project based on their willingness to participate in the study, 23 student-athletes enrolled in sports study soccer program and 30 student athletes enrolled in the multi-sport study program. All the student-athletes were aged between 11 and 13 years old. Thirteen physical and motor skills standardized tests were conducted to all the student-athletes in both programs. The tests were administered at the beginning, in the middle and at the end of the school year. Data processing was performed by using descriptive statistics and ANOVA repeated mesures. These findings may be explained by (a) the level of specificity or of diversity of the exercises proposed during the training sessions, (b) the time spent on each exercise, and (c) the requirements of the game of soccer compared to the requirements of the participation in several sports. However, these findings remain difficult to interpret due to various factors: (a) physical maturity, (b) the number of hours of training during the previous school year, (c) the number of hours of training of both sports programs, and (d) the participation in physical activities and sports outside of school programs. Also, this research does not assess the quality of the interventions of the exercises proposed during the training sessions,nor the motivation of the student athletes taking part in the trainings sessions and during the physical and motor skills tests. Finally, it would be interesting to extend this research to various sports to demonstrate the specific contributions of each discipline to the development of the physical and motor abilities of young athletes.

Full textThis thesis contributes to the advancement of knowledge in the area of program evaluation by presenting a new strategy for the development of outcome indicators. A research hypothesis was formulated following a literature review on program evaluation and some of the shortcomings observed in practice, a research hypothesis regarding the method used by the expert groups involved in the evaluation, as well as a lack of support and collaboration between these groups to "make sense" of all the information needed for this exercise was uncovered. Firstly, a relevant, feasible and effective strategy for identifying outcome indicators was developed in order to support the work of the evaluator and, secondly, to promote the use of the evaluation results in decision-making for program managers. The proposed strategy, which hopes to be an innovative and iterative process, is contingent on a specific procedure of implementation based on accompaniment and four important stages : 1 st stage : use the detailed logical model of the program in order to document the program, the context of its implementation and to communicate; 2 nd stage : consult the stakeholders concerned with the program to design a list of potential indicators which will serve to appraise the program; 3 rd stage : identify and classify the indicators according to the components of the relevant program and 4 th stage : appraise the retained indicators according to the characteristics of relevance, feasibility and weight accorded to by the stakeholders concerned with the program. To carry out this research, a program to be evaluated, as well as a practice environment were chosen to experiment the strategy of identifying program results. From a total of 39 participants, 27 experimented the strategy and 12 validated the outcome indicators that were identified by the experimental group. The main results from the experiment show that the devised strategy is relevant, feasible, and reliable to indentify indicators of program results which are relevant, feasible and important to the stakeholders concerned by the FLUPPY program. 45 outcome indicators have been identified by the actors involved in the Fluppy program, a social skills development program for kindergardeners in schools with a high poverty index. All these indicators had an average score varying between 3.43 and 4.86 for relevance, between 2.57 and 4.5 for feasibility and between 3.29 and 4.64 regarding the importance given by the actors. The discussion also served to further analyze the results and to highlight improvements needed to establish the five elements implemented during the development of the strategy.

Full textIn the past few decades, in Quebec, there are significant increases in the prevalence of at-risk-for overweight and overweight among children and adolescents. Moreover, the majority of children and adolescents do not meet daily recommendations for vegetables and fruits (VF) and dairy products (DP) consumption. Therefore, the Quebec education program has encouraged schools to develop new competencies to improve the adoption of healthy lifestyle habits. The Team Nutriathlon, a computer-assisted school-based nutrition intervention, has been created to support this initiative. This program aimed to increase the consumption and variety of VF and DP in children. The aims of this study were (1) to evaluate the impact of Team Nutriathlon, a school-based nutrition intervention, on VF and DP consumption in children, and (2) to evaluate factors that influence the success of the program among high school students. The results of the study show that during and immediately after the program, children in the intervention group consumed more servings of VF and DP compared to the control group. No effect has been shown ten weeks after the program. Students reveal that the use of technology for recording the number of servings, team work, teacher’s and family’s implication and the use of strategies were facilitating factors contributing to the success of the program. Students also revealed that the lack of attendance with the data collection outside of school hours, the user code dysfunction and the incompatibility of the Web platform with students technologic tools (tablet) were barriers for the success of the program.

Full textBackground: In West Africa, interventions targeting female sex workers (FSW) are crucial to impact on the HIV dynamics. However, the contents and efficacy of these interventions are unclear, and identifying the most at risk FSW in order to adapt these interventions remain challenging, partly because of the limitations of self-reported sexual behaviours. We therefore designed a comprehensive dedicated intervention targeting young female sex workers, and assessed its impact on HIV incidence in Burkina Faso and the reliability of interview data. Methods: From September 2009 to September 2010 we conducted a cross sectional study in Ouagadougou, Burkina Faso. Then HIV-uninfected FSW aged 18-25 years were enrolled in a prospective interventional cohort. The participants were followed quarterly for a maximum of 21 months. The intervention group received a package which combined prevention and care within the same setting, and consisting of peer-led education sessions, psychological support, sexually transmitted infections and HIV care, general routine health care, and reproductive health services. At each visit, behavioural characteristics were collected and HIV, HSV-2 and pregnancy were tested. High-risk behaviour was defined as the first occurrence of any biological event resulting from unsafe sex, including unexpected pregnancy or HSV-2 or T. vaginalis infection. We used random logistic models to assess the relationship between socio-demographic characteristics and the residual high risk behaviours during the intervention.We compared the cohort HIV incidence with a Bernoulli modelled expected incidence in the absence of intervention, using data collected at the same time from FSW clients and key parameters from the litterature. Results: We screened 609 FSW including 188 (30.9%) professionals. Their median age was 21 years [IQR, 19-23], and the prevalence of HIV was 10.3% among professionals and 6.5% among non-professionals. Overall, 277 (45.6%) women reported high-risk behaviours (41.2% among professionals and 47.5% among non-professionals), which were driven mainly by non-consistent condom use with regular partners. In multivariable analysis, before the intervention, HIV infection was associated with older age (AOR=1.44; 95%CI: 1.22-1.71), with being married/cohabiting (AOR=2.70; 95%CI: 1.21-6.04), and with T. vaginalis infection (AOR=9.63; 95%CI: 2.93-31.59), while previous HIV testing was associated with a decreased risk (AOR=0.18; 95%CI: 0.08-0.40).The 321 HIV-uninfected FSW enrolled in the cohort completed 409 person-years of follow-up. No participant seroconverted for HIV during the study while the expected modelled number of HIV infections were 5.05 (95%CI, 5.01-5.08) during the same follow-up (409 person-years) or 1.23 infection per 100 person-years (p=0.005). This null incidence was related to a reduction in the number of regular partners and regular clients, and to an increase in consistent condom use with casual clients (AOR =2.19; 95%CI, 1.16-4.14, p=0.01) and with regular clients (AOR=2.18; 95%CI, 1.26-3.76, p=0.005). However, the incidence of residual risk was high, at 26.7/100 person-years (95% CI, 24.1-33.7). The residual risk was higher among FSW living in couple (adjusted odd ratio [AOR] =7.47, 95% CI, 1.70-30.80) and among those for whom sex work was not the main source of income (AOR =5.53, 95% CI, 1.75-16.84). The latter also tended not to report high-risk behaviours during face –to–face interview. Conclusions: This study highlights the need for targeted interventions among young FSW focusing particularly on non-professionals, sexual behaviours with regular partners and regular HIV testing. The ANRS 1222 study intervention package which combined peer-based prevention and care within the same setting markedly reduced HIV incidence among young female sex workers in Burkina Faso, through reduced risky behaviours

Full textThis study focuses on the implementation of a mindfulness training program for 15 young elite golfers, aged 17 to 20, from a Quebec-City golf academy. The program spanned over 6 weeks, and included formal and daily meditative practice of approximately 10 minutes with audio support and weekly monitoring of participant progress. The objectives were to assess whether this program increased mindfulness in everyday life and in golf, and to check whether this increase persisted over time. A single-subject A-B multiple-baseline design was used. Golfers were assessed with the French computerized versions of the Mindful Attention Awareness Scale (MAAS) and Mindfulness Inventory for Sport-Golf (MIS-Golf). After establishing a baseline, the assessments were carried out on a weekly basis during the intervention and follow-up phases. The tendency to be mindful in daily life was measured, as well as the 3 dimensions of mindfulness in the specific context of golf: awareness, acceptance and refocusing. Visual inspection of the graphs and four series of mixed-model regression analyses were performed. The results revealed (significant) partial low to moderate evidence supporting the effectiveness of the program regarding mindfulness in everyday life, as well as the "acceptance" dimension of mindfulness in golf. This mindfulness training program is considered to be viable, and its initial results are promising. Future research should optimize some of its qualities and refine the study of the effects of the program.

Full textAbstract: Introduction: Rotavirus is the main cause of acute gastroenteritis (AGE) among young children worldwide. In 2011, the monovalent rotavirus vaccine (RV1) was introduced into the Quebec universal immunization program to reduce morbidity related to rotavirus gastroenteritis (RVGE). This thesis aimed to examine AGE and RVGE hospitalization rates before and after implementation of the program in young children from the Eastern Townships (impact study) and the vaccine coverage, and to assess vaccine effectiveness (VE) of the RV1 (effectiveness study). Methods: The pairing of a tertiary hospital database with the regional immunization registry allowed to extract a cohort of children born at the Centre hospitalier universitaire de Sherbrooke (CHUS), living in Eastern Townships and aged less than five years during the study period from June 2004 to May 2014 (n= 37,757). This cohort was retrospectively followed-up to examine AGE and RVGE annual hospitalization rates of pre- (2004/2005-2010/2011) and post-program years (2011/2012-2013/2014), globally and according to several socioeconomic characteristics. Moreover, RV1 VE was calculated using three children cohorts: (1) vaccinated children born in 2011-2013 (n=5,033), (2) unvaccinated children born in 2011-2013 (n=1,239), and (3) unvaccinated children born in 2008-2010 (n=6,436). Results: AGE hospitalization rates evolved from 81/10,000 children aged less than five years in pre-program period to 46/10,000 in post-program period (relative reduction=43%, p<0.001). Following implementation of the program, vaccine coverage rapidly increased to reach 81%. Despite similar vaccine coverage among different groups, lowest relative reductions were observed in disadvantaged groups. Adjusted VE of a complete series was 62% (95% confidence interval [CI]: 37%-77%) and 94% (95% CI: 52%-99%) against AGE and RVGE hospitalizations, respectively. Children living in neighbourhoods with higher rates of low-income families had lower VE against AGE hospitalizations (30% vs. 78%, p=0.027). Conclusion: Three years following its introduction into the universal vaccination program, RV1 significantly reduced severe gastroenteritis in young children in the Eastern Townships. This vaccine was highly effective to prevent RVGE hospitalizations, particularly among the most well-off. Further studies in similar setting are needed to determine factors related to lower VE among vulnerable groups.
