Abstract:
Objective: To determine an appropriate survey instrument to evaluate the impact of organizational structures on the work environment of intensive care nurses. Background: Internationally the demand for intensive care is increasing. Solely increasing bed capacity is not sustainable. Large capacity multi-specialty Intensive Care Units are emerging as the preferred organizational model with benefits resulting from optimizing operational synergies and economies of scale. The impact of this organizational transition on intensive care nurses is not well understood. An appropriate survey instrument for intensive care nurses is required. Design: Integrative literature review. Data Sources: CINAHL, PubMed, EMBASE and OVID Nursing databases searched for studies published between 2005 and 2013. Review methods: An integrative review and quality assessment of the studies was undertaken to select nurse outcome measures associated with organizational structures across a range of acute and critical care settings. Congruence between nurse outcome measures and nurse survey instruments tested in the literature was assessed to select instruments for further psychometric evaluation. Results: Thirty-one cross sectional quantitative studies, from fourteen countries, were reviewed. Twenty one nurse outcome measures associated with organizational factors were identified and a total of twenty five survey instruments used in the studies reviewed. Assessment of congruence and psychometric properties determined that a combination of two instruments is required to comprehensively assess the organizational environment of nurses working in intensive care units. Conclusion: The environment of nurses working in intensive care is effectively evaluated with an instrument that combines subscales from the Practice Environment Scale-Nurse Work Index and Maslach’s Burnout Inventory.

BackgroundMatching demand and supply in nursing work continues to generate debate. Current approaches focus on objective measures, such as nurses per occupied bed or patient classification. However, staff numbers do not tell the whole staffing story. The subjective measure of nurses’ perceived adequacy of staffing (PAS) has the potential to enhance nurse staffing methods in a way that goes beyond traditional workload measurement or workforce planning methods.ObjectivesTo detect outcomes associated with nurses’ PAS and the factors that influence PAS and to review the psychometric properties of instruments used to measure PAS in a hospital setting.Design and methodsA scoping review was performed to identify outcomes associated with PAS, factors influencing PAS and instruments measuring PAS. A search of PubMed, Cumulative Index to Nursing and Allied Health Literature (CINAHL), Business Source Complete and Embase databases identified 2609 potentially relevant articles. Data were independently extracted, analysed and synthesised. The quality of studies describing influencing factors or outcomes of PAS and psychometric properties of instruments measuring PAS were assessed following the National Institute for Health and Care Excellence quality appraisal checklist and the COnsensus-based Standards for the selection of health Measurement INstruments guidelines.ResultsSixty-three studies were included, describing 60 outcomes of PAS, 79 factors influencing PAS and 21 instruments measuring PAS. In general, positive PAS was related to positive outcomes for the patient, nurse and organisation, supporting the relevance of PAS as a staffing measure. We identified a variety of factors that influence PAS, including demand for care, nurse supply and organisation of care delivery. Associations between these factors and PAS were inconsistent. The quality of studies investigating the development and evaluation of instruments measuring PAS was moderate.ConclusionsMeasuring the PAS may enhance nurse staffing methods in a hospital setting. Further work is needed to refine and psychometrically evaluate instruments for measuring PAS.

Background Various factors in hospitals can adversely affect patients’ outcomes, including faulty communication between nurses and physicians. Whether specific communication elements (timeliness, accuracy, openness, understanding) can influence adverse outcomes is unknown. Objectives To determine the relationships between patients’ outcomes and (1) nurses’ perceptions of elements of communication between nurses and physicians and (2) characteristics of the practice environment. Methods A cross-sectional survey design was used. Information on ventilator-associated pneumonia, bloodstream infection associated with a central catheter, and pressure ulcers was collected from 25 intensive care units in southeastern Michigan. Simultaneously, 462 nurses in those units (response rate, 53.3%) were anonymously surveyed. The Conditions for Work Effectiveness Questionnaire-II and the Practice Environment Scale of the Nursing Work Index were used to measure characteristics of the practice environment. The Intensive Care Unit Nurse-Physician Questionnaire was used to measure communication between nurses and physicians. Statistical tests included correlation and multiple regression. Analyses were conducted at the unit level. Results Unit response rates varied from 6% to 100%. Together, variability in understanding communication and capacity utilization were predictive of 27% of the variance in ventilator-associated pneumonia. Timeliness of communication was inversely related to pressure ulcers (r= −0.38; P=.06), and workplace empowerment and scores on the Acute Physiology and Chronic Health Evaluation III were positive predictors of ventilator-associated pneumonia (R2=0.36; P=.005). Conclusions Not all elements of communication were related to the selected adverse outcomes. The connection between characteristics of the practice environment at the unit level and adverse outcomes remains elusive.

Abstract Background The NHS Long Term Plan (2019) sets specific targets for multidisciplinary services for frail older people over the next ten years. Delivery and sustainability is crucially dependant on the capability and capacity of gerontological nursing. High cost cities such as London experience high staff vacancy rates in acute care older adult services. Aim The study took a regional approach, working with NHS Trusts to develop a multicomponent intervention to increase retention and competencies of early career nurses working in gerontological services. The study examined the acceptability and feasibility of the intervention and tested a quasi-experiential evaluation design. Ethical approval was obtained from the University Ethics committee. Methods A co-design approach with stakeholders, early career nurses, educationalists and nurse managers, produced a multicomponent intervention: education module (masters level), gerontological competency booklet, external clinical learning opportunities, career coaching and mentorship delivered over a six-month period. The evaluation involved a mix-methods pre-post survey and focus group interviews. Results Twenty-nine early career nurses were recruited from five Trusts. The multicomponent intervention was well received, but there were difficulties facilitating external learning opportunities and providing career mentors. The primary outcome was intention to remain in gerontological nursing (measured using a point Likert scale). Pre-post the intervention this remained high (mean score 6 IQR 5-7), p=0.78. There was a significant increase in gerontological knowledge: at baseline the median score was 87 (IQR 81-102) compared to 107 (IQR 98-112) post-intervention, p=0.006. In focus groups participants identified three main mechanism of action for ECHO: building gerontological knowledge and skills; professional identity as older adult nurse; and networking to broaden horizons. Conclusions The study has demonstrated the potential of Trusts to work collaboratively with education providers to deliver a model of career-education pathway that may help attract and retain early career nurses to work in gerontology.

Proportion of health care workers of those who believe that the work in progress threatens their health and safety is higher than the average in other sectors. Workers in health care facilities are especially exposed to violence in the workplace [1]. The aim of the study was the evaluation of the presence of violence at work and determination of its effect on the working ability of health care sector workers. The study included the survey of 132 emergency department employees (doctors, physician assistants, nurses) from Kurzeme Region hospitals (Latvia). Questionnaire of violence in the workplace of the health care sector workers and the work ability index assessment questionnaire were used for the data collection. It was found that medical staff of health care establishments experience mostly verbal abuse (97%), less from physical attacks (36%), and intimidation/bullying (30%). The study found that medical personnel are aware of the changes implemented in the workplace (29%), there has been no change (33%), while the impact of changes implemented by medical staff at the workplace has not been experienced (47%). For personnel who have suffered from violence in the workplace, working capacity is generally lower than for the personnel that has not experienced violence in the workplace.

Objective: No studies were found in the literature which compared the capacity of nurses and nursing students to assess patient problems using the clinical cases followed during internship. Therefore, the aim of this study was to formulate a method for comparing these skills in cases followed during a practical clinical internship.Methods: The sample studied was made up of students of the degree course in nursing during their internship and by community nurses, both trained in using assessment. Each student identified a patient and carried out an assessment of the problems according to the functional patterns of M. Gordon; the nurses also simultaneously carried out the same activity without comparing their work with that of the students. A method was formulated for evaluating the correctness of the two evaluations.Results: The results relative to the assessment showed a percentage of correctness of 85.77% for the students and 91.28% for the nurses with a statistically significant difference (p = .027).Conclusions: The results obtained demonstrated that the students in the last year of their degree course in nursing had developed a good capacity of assessment during their internship in clinical practice in the community in line with the capacity of the nurses who taught them.

Background: Pleural disease incidence is increasing and there has been a move towards outpatient management. Pleural clinics have been developed for patients to be assessed and have pleural procedures for diagnostic and symptomatic purposes. Aim: The study aimed to assess the impact of nurse practitioner (NP) involvement in a pleural procedures clinic on clinic capacity, and staff and patient experience. Methods: A mixed-methods service evaluation was completed in a medium-sized general hospital. The number of patients seen in clinic over a 6-month period at the beginning of NP involvement was compared with the number of patients seen 1 year later. Semi-structured interviews of key staff stakeholders were conducted. Findings: Clinic capacity increased from 4.57 to 5.86 patients per list. NP involvement increased access, improved patient care, reduced work burden for other staff and improved educational opportunities. Conclusion: The impact of NP involvement in the pleural clinic was seen as positive and increased clinic capacity. The same model of practice could be considered elsewhere.

Abstract:
This study investigates the engagement of healthcare professionals in continuing professional development in public secondary hospitals in Lagos State, Nigeria. A survey research design and a multistage sampling technique were adopted for the study. The sample consisted of 356 healthcare professionals (112 physicians, 32 pharmacists and 212 nurses). The data were collected using a self-developed structured questionnaire. The questions were built based on the literature review with modified questions adapted from widely used the questionnaires on hospital survey on patient safety, ambulatory and community pharmacy, nursing safety practices and CPD professional and skill competency and World Health Organization Regional Guidelines for Continuing Medical Education. Healthcare professionals in adult general outpatient departments were only included in the study. Findings revealed that physicians engagement in informal CPD (conferences, seminars, in-service training) is (M = 3.09), and formal CPD (conduct of research, publications or peer journal review) (M = 2.11). Pharmacists engagement in informal CPD is (M = 3.10) and formal CPD (M =1.89), Nurses’ engagement in informal CPD is (M =2.30) and CPD (M = 1.78). These evidence indicates that the respondents tend to lean more on less time-demanding and work-based capacity building approach, than they engaged mostly in informal CPD activities than they engaged in a methodical educational and professional advanced method of capacity building. The study recommends improved Government funding for training, policy improvement on organizational support and working conditions for study leave opportunities, and aligning the metrics of evaluating and scoring CPD engagement with the need assessment gaps in healthcare system in renewing practicing license of healthcare professionals. Introduction: In many rural and remote communities in BC, family physicians who are providing excellent primary and emergency care would like to access useful, timely, and collegial support to ensure the highest quality of health services for their patients. We undertook a real-time virtual support project in Robson Valley, located in northern BC, to evaluate the use of digital technologies such as videoconferencing for on demand consultation between family physicians at rural sites and emergency physicians at a regional site. Telehealth consults also occurred between rural sites with nurses at community emergency rooms consulting with local on-call physicians. Our aim was to use telehealth to facilitate timely access to high quality, comprehensive, coordinated team-based care. An evaluation framework, based on the Triple Aim sought to: 1) Identify telehealth use cases and assess impact on patient outcomes, patient and health professional experience, and cost of health care delivery; and 2) Assess the role of relationships among care team members in progressing from uptake to normalization of telehealth into routine usage. Methods: Using a participatory approach, all members of the pilot project were involved in shaping the pilot including the co-development of the evaluation itself. Evaluation was used iteratively throughout implementation for ongoing quality improvement via regular team meetings, sharing and reflecting on findings, and adjusting processes as required. Mixed methods were used including: interviews with family physicians, nurses, and patients at rural sites, and emergency physicians at regional site; review of records such as technology use statistics; and stakeholder focus groups. Results: From November 2016 to July 2017, 26 cases of telehealth use were captured and evaluated. Findings indicate that telehealth has positively impacted care team, patients, and health system. Benefits for care team at the rural sites included confidence in diagnoses through timely access to advice and support, while emergency physicians at the regional site gained deeper understanding of the practice settings of rural colleagues. Nevertheless, telehealth has complicated the emergency department work flow and increased physician workload. Findings demonstrated efficiencies for the health system, including reducing the need for patient transfer. Patients expressed confidence in the physicians and telehealth system; by receiving care closer to home, they experienced personal cost savings. Implementation saw a move away from scheduled telehealth visits to real use of technology for timely access. Conclusion: Evidence of the benefits of telehealth in emergency settings is needed to support stakeholder engagement to address issues of workflow and capacity. This pilot has early indications of significant local impact and will inform the expansion of emergency telehealth in all emergency settings in BC.

Trata-se de uma pesquisa quantitativa, com delineamento transversal a partir do levantamento de dados survey, que objetivou avaliar a capacidade para o trabalho de enfermeiros do Hospital de Clínicas de Porto Alegre RS e suas características laborais, hábitos de vida e as medidas de promoção da capacidade para o trabalho. Definiu-se como população do estudo enfermeiros em atividade no hospital. A amostra foi composta por 195 enfermeiros selecionados aleatoriamente. Foi utilizado o instrumento Índice de Capacidade para o Trabalho, validado no Brasil. A leitura dos dados foi realizada pelo Software Sphinx, sendo os mesmos convertidos para o SPSS, V.15 para análises estatísticas. Foram realizadas análises estatísticas descritivas e analíticas; utilizou-se confiança de 95%, a significância de 5%. Os dados mostram que 94,5% dos enfermeiros do hospital são do sexo feminino; com idade média de 42,6 anos (dp=8,5); 66,5% casados ou com companheiros; 76,7% possuem pós – graduação; 92,8% atuam diretamente na assistência. Quanto ao turno de trabalho 36,0% são da noite; 28,4% da manhã e 20,8% da tarde. Trabalham no hospital em média há 14,0 anos (dp=9,4), e no turno atual, em média, há 9,4 anos (dp=8,7), 87,4% dizem sentir-se satisfeitos freqüentemente, 56,8% sentem se valorizados, 85,2 %, não trabalha em outra instituição e estão 70,0% satisfeitos com o seu salário. O escore médio de ICT dos enfermeiros atingiu 41,8 pontos e houve correlação significativa com a remuneração (p-valor<0,05). Há evidências de diferença estatística do ICT médio relacionado à satisfação com o local de trabalho (p-valor=0,001), sentimento de valorização por parte da instituição (p-valor=0,003), e turno de trabalho (p-valor= 0,032). Os aspectos laborais que apresentaram associação com o ICT médio foram: sobrecarga de trabalho (p-valor=0,001), reconhecimento do trabalho real (p-valor=0,003), reconhecimento profissional (p-valor=0,001), comunicação no ambiente de trabalho (p-valor=0,042), possibilidade de tomar decisões com tempo suficiente (p-valor=0,005), possibilidade de fazer melhorias no esquema de trabalho (p-valor=0,001), e número suficiente de pessoas na escala (p-valor=0,050). Constatou-se diferença significativa para o ICT médio do grupo de enfermeiros que realiza programas e atividades em família (p-valor=0,009). Houve diferença significativa para o ICT médio entre os tipos de doenças digestivas, psiquiátricas e geniturinárias (p-valor<0,05). Em relação às medidas sugeridas para melhorar a capacidade para o trabalho, o grupo de enfermeiros com ICT moderado relatou maior número de medidas quando comparado com o grupo que apresentou escores bom e ótimo. Os grupos com capacidade para o trabalho bom ou ótimo apresentaram semelhanças em relação às medidas relatadas. A população de enfermeiros apresentou perfil sociodemográfico e estilo de vida diferenciado, com elevado padrão de saúde e capacidade para o trabalho.
This is a quantitative research, with a cross-sectional delineation from the compilation of the survey data, which aimed to evaluate the work ability of nurses of the Hospital de Clínicas de Porto Alegre RS and labor characteristics, lifestyle and promotional measures that promote the capacity to work. The study’s population was the nurses in activity at the hospital. The sample consisted of 195 randomly selected nurses. The study used the Work Ability Index, validated in Brazil. The reading of data was performed by Sphinx Software, and they were converted to SPSS V.15 for statistical analysis. There were made descriptive statistics and analytical analysis, we used 95% confidence, the significance of 5%. The data show that 94.5% of hospital nurses are female, with an average age of 42.6 years (SD = 8.5), 66.5% married or with partners, 76.7% have a post - graduate, 92.8% work directly with assistance. As for the shift work, 36.0% work at night, 28.4% in the morning and 20.8% in the afternoon. Working in the hospital for an average of 14.0 years (SD = 9.4), and the current turn, on average, for 9.4 years (SD = 8.7), 87.4% say they feel pleased often, 56.8% feel valued, 85.2%, does not work at another institution and 70.0% are satisfied with their salary. The average score of WAI of nurses reached 41.8 points and had significant correlation with payment (p <0.05). There are evidences of statistical difference of the medium WAI related to satisfaction with the workplace (p = 0.001), feelings of appreciation from the institution (p = 0.003), and shift work (p = 0.032). The labor aspects that were associated with the WAI average were: work overload (p = 0.001), recognition of the real work (p = 0.003), professional recognition (p = 0.001), communication in the workplace (p-value = 0.042), possibility to make decisions with enough time (p = 0.005), possibility to make improvements to the work scheme (p = 0.001), and enough people working (p-value = 0.050). It was found a significant difference for the medium WAI of the group of nurses who carry out family programs and activities (p = 0.009). There was a significant difference in the medium WAI between types of digestive diseases, psychiatric and genitourinary (p <0.05). Regarding the measures suggested to improve the ability to work, the group of nurses with moderate WAI reported more measures when compared with the group that the scores showed good or excellent. The groups with the ability to work good or excellent had some similarities in the measurements reported. The population of nurses showed a sociodemographic profile and distinctive lifestyle with high standard of health and work ability.
Esta es una encuesta cuantitativa, de corte transversal de los datos de la encuesta, que tiene por objeto evaluar la capacidad para el trabajo de las enfermeras del Hospital de Clínicas de Porto Alegre, RS y características del trabajo, estilo de vida y medidas de promoción capacidad de trabajo. Se define como el estudio de las enfermeras de la población en la actividad hospitalaria. La muestra consistió de 195 enfermeras seleccionadas al azar. El estudio utilizó el Índice de la Capacidad de Trabajo(ICT), validado en Brasil. La lectura de los datos se realizó mediante la Esfinge de software, y se convierten en V.15 SPSS para el análisis estadístico. Se analizaron con estadística descriptiva y analítica, que utilizó el 95% de confianza, la significación del 5%. Los datos muestran que el 94,5% de las enfermeras de hospicio son mujeres, con una edad media de 42,6 años (SD= 8,5), el 66,5% casados o con los socios, el 76,7% tenían un post - grado , el 92,8% trabaja directamente con la atención. En cuanto al cambio son 36.0% de la noche, el 28,4% y 20,8% en la tarde por la mañana. Trabajo en el hospital por un promedio de 14,0 años (SD=9,4), y el turno actual, en promedio, 9,4 años (SD=8,7), 87,4% dicen que se sienten felices con frecuencia 56,8% se sienten valorados, el 85,2%, no funciona en otra institución y el 70,0% están satisfechos con su salario. Los aspectos del mercado de trabajo que se asociaron con la media de los ICT fueron: sobrecarga de trabajo (p = 0,001), el reconocimiento del trabajo real (p = 0,003), el reconocimiento profesional (p = 0,001), la comunicación en el lugar de trabajo (p-valor = 0,042), capacidad de tomar decisiones con suficiente tiempo (p = 0,005), la capacidad de introducir mejoras en el esquema de trabajo (p = 0,001), y bastante gente en la escala (valor de p = 0.050). Se encontró una diferencia significativa para el grupo ICT promedio de enfermeras que lleva a cabo programas y actividades de la familia (p = 0,009). Hubo una diferencia significativa entre los tipos ICT promedio de las enfermedades digestivas, psiquiátricas y genitourinarias (p <0,05). Para las medidas sugeridas para mejorar la capacidad de trabajo, el grupo de enfermeras com ICT informó de las medidas más moderadas en comparación con el grupo que mostró resultados buenos o excelentes. Los grupos con la capacidad de trabajo bueno o muy bueno había algunas similitudes en las mediciones reportadas. La población de enfermeras mostró diferencias sociodemográficas y de estilo de vida con el máximo nivel de capacidad de la salud y el trabajo.

Abstract:
Nurses' perceptions of their work environment and empowerment in relationship to patient satisfaction are well explored in the literature. Empowerment has been linked to improved job satisfaction and improving the elements of empowerment cam improve overall patient satisfaction. This project examined 2 nursing units, 1 with lower patient satisfaction scores (Unit A) and 1 with higher patient satisfaction scores (Unit B), in order to compare their structural empowerment scores as measured by the Conditions of Work Effectiveness Questionnaire II (CWEQ-II). A total of 29 nurses who worked full-time, part-time, or per diem on 1 of the 2 units were recruited to participate in the project (11 from Unit A and 18 from Unit B). Using Kanter's theory as a framework, the nurses were asked to complete the CWEQ-II and overall scores as well as subscale scores were calculated. Scores were estimated using a Mann-Whitney U test given the lack of normality and linearity of the data. Both units had a moderate overall empowerment level (Unit A: 19.55; Unit B: 21.47). Unit A had a significant difference in Access to Resources: Acquiring Temporary Help (z = -2.07, p < 0.05) as compared to Unit B. In comparing nurses with a Bachelor's and higher nursing degree to nurses with less than a Bachelor's degree, there was a significant difference in Access to Resources: Acquiring Temporary Help (z = -3.115, p < 0.05) and overall Resource Subscale (z = -2.157, p < 0.05). The project demonstrates the need for organizations to create a workplace that promotes empowerment among nurses as a strategy to improve patient and organizational outcomes. A work environment that promotes a nurse's control over his or her practice and decision-making ability may lead to higher patient satisfaction and may become the role model of a nursing practice environment as a result.

This thesis presents the experimental work on testing the seasonal performance of a 40 kW air-to-water propane heat pump, as a part of the research project Next Heat Pump Generation working with Natural fluids (NxtHPG) that has been carried out in ‘Applied thermodynamics and refrigeration Division’ in the department of Energy Technology, KTH. The thesis work involves three parts: preparation of the HP unit test rigs, experimental campaign and the evaluation of test results. In the first part, the set-up of the measurement device and necessary modifications to the heat pump unit as well as the test rigs have been completed to create an accurate measuring environmental for the experimental campaign. During the second stage, two series of experimental campaign have been done in the heating mode of the heat pump unit, and satisfactory results have been obtained in parts of the test conditions. Finally, the evaluation on the system performance as well as the behaviors of the components is discussed. The variation between experimental results and the simulation conducted by IMST-ART model is within reasonable range, proving that the heat pump unit has been working in good conditions. The compressor is proved to have been performing as manufacturer expected. However, some further investigations on the behaviors of the heat exchangers and expansion valve, such as superheat oscillation, are recommended be carried out in the future work. The completion of the rest of tests in which minus air temperature should be maintained and the ones with the desuperheater working should be done as well.

Abstract:
The purpose of this study was to examine the effect of program type and selected predictor variables on the vocational placement and feelings of self-esteem of disabled male workers. It was predicted that: (1) a physical capacity assessment program (PCA), designed by the investigator, would result in a higher rate of successful vocational placement than conventional placement techniques, in a shorter period of time and with a better vocational match; (2) increased measures of self-esteem would be experienced by the PCA group and at a more significant level than conventional vocational programs; (3) there would not be a significant interaction between the vocational placement variables and the independent variables of age, degree of disability and injury type; and (4) there would not be a significant effect of selected moderator variables between groups on the vocational placement variables. Fifty disabled workers were randomly selected from the active caseloads of the Vocational Rehabilitation Department of the Workers' Compensation Board of British Columbia for participation in the study. Their average age was 33.32 years, and their average length of time on wage loss was 245 days. All subjects were injured as the result of an industrial accident, were no longer undergoing medical treatment, had been cleared for a return to work but were suffering from a residual disability that prevented them from returning to the work force. Subjects were randomly assigned to one of two treatment conditions involving vocational counselling with job search training (JS) and vocational counselling with physical capacity assessment (PCA). Following completion of the program, subjects began job search activities and were monitored for a period of 6 months or until successful vocational placement was achieved. Data analyses showed that: there was a significant main effect between groups for success in vocational placement with PCA placing more subjects than job search; there were no significant differences between groups on measures of self-esteem following completion of the programs; there was a significant interaction effect of age, degree of disability and type of injury with success in vocational placement and; there was a significant interaction effect of several moderator variables including marital status and length of time on wage loss with success in vocational placement and time to vocational placement. The results tend to support the use of physical capacity assessment for the guantification of individual capacity to perform work. Further, the results suggest that the use of PCA information in the identification and selection of appropriate vocational alternatives can significantly enhance the success of vocational placement disabled workers. Recommendations were made for future research.

Major depressive disorder (MDD) is a relevant condition to consider regarding Work Capacity Evaluation (WCE) because of its high prevalence, strong impact on short-term work disability, and low rate of treatment. The challenge that faces occupational therapists (OTs) is that there is no specific guideline and process to follow when conducting Work Capacity Evaluation with clients suffering from MDD. The researcher had also noted conflicting opinions with regard to the outcome of Work Capacity Evaluation (WCE) and recommendations in occupational therapy reports. The researcher is of the opinion that indeed occupational therapy assessments need to be clear about what to measure, and selection of appropriate standardised measures and non-standardised assessments is needed. Therefore, the research question was, what are the perceptions and practices of occupational therapists in determining work capacity of employees suffering from major depressive disorder? The aim of this study was to describe the perceptions and practices of occupational therapists in determining work capacity of employees suffering from major depressive disorder. The literature showed that Work Capacity Evaluation assists to determine the employee’s returnto- work. The Code of Good Practice: Dismissal contained in the Labour Relations Act of 1998, stipulates that incapacity on the ground of ill health or injury may be either temporary or permanent. The nature of this study was a mixed method design including both qualitative and quantitative approaches. The phenomenological strategy was used. The study had a sample size of 68 participants, practising as occupational therapists. Purposive sampling was used. The data were collected in four distinct phases, referred to as sequential exploratory strategy with elements of embedded design. Phase one, used descriptive open-ended questions; phase two was a close-ended questionnaire to confirm findings in phase one; phase three consisted of focus group interviews and phase four was member checking to confirm findings in phase three. During data analysis, five themes related to employees suffering from MDD emerged and they were identified as: (1) the content of comprehensive assessment for work capacity evaluation, (2) the process of work capacity evaluation, (3) the competency requirements of the occupational therapist, (4) occupational performance and (5) formulating return-to-work decision. Recommendations related to the five themes were formulated and suggestions for future research proffered. The study clarified and confirmed that occupational therapists have a major role to play in performing work capacity of employees suffering from major depressive disorder, and that they need to be competent in performing these evaluations. ABSTRACT Background: Work fatigue experienced by nurses can hurt the service provided by the hospital. The workload was difference in the capacity or ability of worker with job demand that must be faced. Interpersonal interaction is a relationship, communication, and interaction with other people. The purpose of this study was to determine relationship between workload and interpersonal interactions with subjective work fatigue in inpatient nurses at dr. M Yunus hospital, Bengkulu, Indonesia. Subjects and Method: This was a cross sectional conducted at dr. M Yunus Bengkulu from August to September, 2020. A sample of 128 nurses in the inpatient room of Dr. Hospital. M Yunus Bengkulu selected by total sampling. The dependent variable was subjective work fatigue. The independent variables were workload and interpersonal interactions. The instrument used was a questionnaire. The data was analyzed by chi square. Results: There was a statistically significant relationship between workload (OR= 9.31; 95% CI= 3.86 to 22.85; p< o.001) and interpersonal interactions (OR= 7.11; 95% CI= 3.05 up to 16.76; p< 0.001) with subjective work fatigue. Conclusion: Workload and interpersonal interactions are factors that can cause subjective work fatigue in the inpatient nurses at RSUD Dr. M Yunus Bengkulu. Suction anchors are used within mooring systems for a range of deep water floating structures. The number of suction anchor systems has increased rapidly over the last two decades due to ease of installation and a well-defined design approach. As with other offshore installations, anchor systems may be subjected to re-assessment during their operational lifetime, due to changes in loading, codes or other issues. Such assessment was performed recently for a floating storage unit (FSU) presently in service in the Norwegian Sea, where the geotechnical design of the anchors was re-evaluated. The re-assessment of this storage unit allowed for a number of changes compared to the original design, including updated loads and load histories as well as changes to the safety factors. The geotechnical aspects of the design were also updated, including more refined analyses and a detailed review of the design soil profiles based on back-analysis of installation data. This paper presents the work done related to the geotechnical holding capacity of the anchors during the assessment project. The work performed highlights the benefit of efficient FE analysis and a thorough evaluation of the soil properties and engineering behaviour.

Thermoplastic composite pipes (TCPs), due to their superior properties including corrosion resistance, thermal insulation, fast installation, etc., are increasingly used as the subsea pipelines. The TCP is made from thermoplastic resins and reinforced by continuous fibers. Because of the brittle nature of carbon fibers and glass fibers, the dimensions of dent are not suitable for assessment of impact resistance of a TCP. In the present work, a procedure was proposed using the internal pressure capacity as an indicator to evaluate the lateral impact resistance of a TCP. First, the internal pressure capacity of an intact TCP was evaluated. Second, a quasi-static simulation was conducted by applying a lateral compression force on the intact TCP using a rigid ball, until one of the composite plies in the reinforcement layer failed. The quasi-static simulation provided an initial estimate of the minimum energy that causes the start of damage of the TCP. Third, the impact simulations were performed by using a rigid ball hitting the TCP and, then, the internal pressure capacity of the damaged TCP was evaluated. Finally, the internal pressure capacity of the damaged pipe, compared with that of the intact pipe, was used as an indicator to evaluate the lateral impact resistance of the TCP. In this study, a glass-fiber reinforced polyethylene (PE) pipe of an inner diameter of 150 mm was modeled by ABAQUS to illustrate the procedure. A theoretical method was proposed to calculate the impact energy of a dropped object in a shallow water. The example studied in the present work showed that the modeled TCP was not strong enough to survive the lateral impact caused by the dropped object and should be buried to a certain depth beneath the seabed if used as a subsea pipeline.

Aim/Purpose: This research paper reports on the evaluation of a mental health self-management program provided to immigrant nurses working at various rural South Australian aged care services. Background: The residential aged care staffing crisis is severe in rural areas. To improve immigrant nurses’ employment experiences, a mental health self-management program was developed and conducted in rural and regional health care services in South Australia. Methodology: A mixed approach of pre- and post-surveys and post workshop focus groups was utilized with the objectives of exploring the experiences of 25 immigrant nurses and the impact of the mental health program. Feminist standpoint theory was used to interpret the qualitative data. Contribution: A new learning environment was created for immigrant nurses to learn about the theory and practice of maintaining and promoting mental health. Findings: Statistical tests showed a marked difference in responses before and after the intervention, especially regarding knowledge of mental health. The results of this study indicated that a change in thinking was triggered, followed by a change in behaviour enabling participants to undertake self-management strategies. Recommendations for Practitioners: Include expanding the workshops to cover more health care practitioners. Recommendations for Researchers: Feminist researchers must actively listen and examine their own beliefs and those of others to create knowledge. Extending the program to metropolitan areas and examining differences in data. E technology such as zoom, skype or virtual classrooms could be used. Impact on Society: The new awareness and knowledge would be beneficial in the family and community because issues at work can impact on the ability to care for the family, and there are often problems around family separation. Future Research: Extending the research to include men and staff of metropolitan aged care facilities.

The aim of this work was to evaluate the effect of inlet air temperature and gum Arabic concentration on yield of spray drying of soymilk and powder quality (moisture content, water activity and antioxidant capacity). Since soymilk had a significant lipid content, gum Arabic played a significant role as an emulsion stabilizer, improving process yield and preserving antioxidant capacity. However, temperature did not affect antioxidant capacity. The optimal condition to obtain higher antioxidant capacity was: 30% of gum Arabic and 160°C. The powder obtained under optimized condition was characterized regarding to bulk density, particle size distribution and morphology.Keywords: soymilk; spray drying; ferric reduction power FRAP; DPPH scavenging ability; scanning electron microscopy.

Abstract:
This paper presents an overview of a joint industry project carried out in the period 2001 – 2003 to address the Fatigue Capacity of FPSOs. Numerical investigations and fatigue testing is performed in order to improve the accuracy, robustness and efficiency of finite element modeling and hot spot stress evaluation for typical FPSO details. Design criteria will be developed for fatigue crack growth from the root of fillet welds in typical details in FPSOs including fillet welds around attachments and fillet welds around pipes penetrating deck plates. Fatigue analysis methods for FPSOs with guidelines on determination of side pressures in the water line will be developed. This work will be supported by assessment of actual measurement data from the field and testing in a tank. Methods for planning in-service inspection of FPSOs based on a fracture mechanics and reliability analysis will be developed. Also a procedure for repair of a fatigue crack while the FPSO is on station in the field will be developed.

A new Offshore Standard (DNV-OS-E406) [1] has been developed for the design of Free Fall Lifeboats (FFLB) used for emergency evacuation from offshore structures. This standard aims at ensuring safe evacuation, focusing on the design at an early stage. Load (CFD) and structural (FE) analyses are used to assist in determining whether or not the free fall lifeboats comply with the Target Safety requirements as set forth by the Standard. The present paper describes the methodology used when assessing the structural capacity of FFLB. The structural integrity is established throughout every phase of a lifeboat drop, linking computational fluid dynamics (CFD) and finite element analysis (FEA). Loads computed from CFD on a finite volume mesh are interpolated to a finite element model where the structural analysis can be performed in order to obtain stresses, strains and deflections, and finally assess the structural capacity of the lifeboat. Emphasis in this work is given to the hydrodynamic model and loads, while structural analyses are discussed more in depth by Brinchmann et al. [2].

Abstract Being able to estimate the tensile strain capacity (TSC) of vintage girth welds is sometimes necessary for the integrity management of vintage pipelines. Assessing girth weld integrity could be a top priority after a confirmed ground movement event. Decisions may also be needed about the disposition of a girth weld when weld anomalies are found. Typical fitness-for-service (FFS) procedures, such as API 1104 Annex A and API 579/ASME FFS-1, generally target materials under nominally elastic conditions and strain demands less than 0.2%. These procedures may produce overly conservative results when the strain demand exceeds 0.2%. This paper summarizes the development and validation of a TSC estimation tool for vintage girth welds under PRCI funding. The work consisted of three components: the development of a TSC model for vintage girth welds, the implementation of the model into a software tool, and the experimental validation of the performance of the tool using curved wide plate (CWP) tests. The TSC model was developed following the procedures established through a previous PRCI-PHMSA cofounded work. Finite element analyses (FEA) were performed to obtain a crack-driving force database while considering the salient features of vintage girth welds, such as larger weld caps and weld strength mismatch levels. The TSC model was then derived from the crack-driving force database using apparent toughness values representative of vintage girth welds. A graphical user interface (GUI) and a user manual were developed to facilitate the application of the TSC model. The software tool produces TSC estimates based on geometry, material, loading, and flaw characteristics of a girth weld. For inputs that might not have readily available values, recommended values are provided. The tool allows the evaluation of the impact of various input parameters on TSC. The performance of the TSC estimation tool was evaluated against eight purposely designed CWP tests. Accompanying small-scale material characterization tests, including chemical composition, round bar tensile, microhardness, and Charpy impact tests, were performed to provide additional inputs for the evaluation of the tool. The tool is shown to provide reasonably conservative estimates for TSC. An example problem is presented to demonstrate the application of the tool. Gaps and future work to improve the tool are highlighted at the end of the paper.

Soils containing inorganic compounds are frequently encountered by transportation agencies during construction within the right-of-way, and they pose a threat to human health and the environment. As a result, construction activities may experience project delays and increased costs associated with management of inorganic compounds containing soils required to meet environmental regulations. Recalcitrance of metal-contaminated soils toward conventional treatment technologies is exacerbated in clay or organic content-rich fine-grained soils with low permeability and high sorption capacity because of increased treatment complexity, cost, and duration. The objective of this study was to develop an accelerated in situ electrochemical treatment approach to extract inorganic compounds from fine-grained soils, with the treatment time comparable to excavation and off-site disposal. Three reactor experiments were conducted on samples collected from two borehole locations from a field site in Illinois that contained arsenic (As)(~7.4 mg/kg) and manganese (Mn)(~700 mg/kg). A combination of hydrogen peroxide (H2O2) and/or citrate buffer solution was used to treat the soils. A low-intensity electrical field was applied to soil samples using a bench-scale reactor that resembles field-scale in situ electrochemical systems. For the treatment using 10% H2O2 and citrate buffer solution, average removal of 23% and 8% were achieved for Mn and As, respectively. With 4% H2O2 and citrate buffer, 39% and 24% removal were achieved for Mn and As; while using only citrate buffer as the electrolyte, 49% and 9% removal were achieved for Mn and As, respectively. All chemical regimes adopted in this study reduced the inorganic compound concentrations to below the maximum allowable concentration for Illinois as specified by the Illinois Environmental Protection Agency. The results from this work indicate that electrochemical systems that leverage low concentrations of hydrogen peroxide and citrate buffer can be effective for remediating soils containing manganese and arsenic.

Addiction Recovery Agency (Ara) and Beacon Counseling Trust (BCT) provide critical safer gambling education and treatment services for the West of England, North West England, and Wales. Their respective dedication to the safer gambling field and commitment to enhancing system integration led to a natural partnership between the two organisations. Drawing from Ara and BCT’s significant expertise, they partnered to develop a suite of safer gambling programmes. As the suite of innovative programmes grew, they recognised a need to articulate and share their leadership in transforming the safer gambling landscape in England and Wales. The Safer Gambling Movement describes Ara and Beacon’s leadership in developing a grassroots movement to build momentum for a national public health approach in Great Britain by first building this capacity in England and Wales. GREO was brought on as the evaluation partner to help create a theory of change to describe this work and lay the foundation for future evaluations.
