Abstract:
Adverse events are an unfortunate reality of caring for patients in our current healthcare system. Preventing and mitigating these events are an important part of quality improvement. First, an understanding of what events occur and how often they are occurring is critical to planning improvements. Incident reporting systems are one way of gathering this information. Then, events should be categorized and analyzed for improvement. The failure mode and effects analysis (FMEA) and bow-tie diagram are two tools for this purpose. Once an event has occurred, consideration should be given to the caregivers as well as the patient when managing and resolving adverse events. Prevention requires strong analysis of events and recognition of both latent (system) and human causes. Interventions have different degrees of effectiveness, ranging from highly effective forcing functions, to marginally effective encouraging statements. There are four steps to event management: mitigation, immediate management, refractory management, and follow-up.

Historians of the British Civil Wars are increasingly taking notice of these bloody conflicts as a critical event in the welfare history of Europe. This volume will examine the human costs of the conflict and the ways in which they left lasting physical and mental scars after the cessation of armed hostilities. Its essays examine the effectiveness of medical care and the capacity of the British peoples to endure these traumatic events. During these wars, the Long Parliament’s concern for the ‘commonweal’ led to centralised care for those who had suffered ‘in the State’s service’, including improved medical treatment, permanent military hospitals, and a national pension scheme, that for the first time included widows and orphans. This signified a novel acceptance of the State’s duty of care to its servicemen and their families. These essays explore these developments from a variety of new angles, drawing upon the insights shared at the inaugural conference of the National Civil War Centre in August 2015. This book reaches out to new audiences for military history, broadening its remit and extending its methodological reach.

Abstract:
This chapter initially considers methodological issues that arise in assessing the effectiveness of compulsory community treatment. It then reviews the evidence from uncontrolled, controlled, and randomized studies of various types of intervention. Although uncontrolled studies suggest some reduction in health service use following compulsory community treatment, this is not generally confirmed in studies with matched or randomized controls. Although proponents of compulsory community treatment argue that it is less coercive than the alternatives of compulsory admission to hospital or arrest, research findings suggest that it remains an unproven way of reducing either procedure. Even where changes in outcome have been shown, such as decreased criminal victimization, it is not clear whether these are due to the legislative framework or to a greater intensity of contact.

Abstract:
This chapter provides a summary of the Clinical Antipsychotic Trials of Intervention Effectiveness—Alzheimer’s disease (CATIE-AD), a multicenter trial that investigated whether atypical antipsychotics are an effective treatment for psychosis, aggression, or agitation in outpatients with Alzheimer’s disease. The chapter briefly reviews the study design, as well as implications and limitations. A relevant clinical case concludes the chapter. In summary, atypical antipsychotic use for up to 36 weeks did not lead to clinical improvement based on time to discontinuation, or symptom reduction. Risk of discontinuation due to adverse events and side-effects with worse with antipsychotic treatment compared to placebo. In patients with psychosis, agitation, or aggression due to Alzheimer’s disease, the efficacy of atypical antipsychotics is questionable and their use comes with considerable risks of side effects and adverse events.

For the past ten years, the prevalence of suicide completions among service members has increased, with rates greatly exceeding those seen within the US civilian population. In response, the Department of Defense, Department of Veterans Affairs, and each branch of the military collaboratively implemented strategic approaches to improving existing suicide prevention programs and invested in the development of innovative programs. These approaches include consolidating suicide event reporting into a central database, reducing the stigma associated with help-seeking behaviors, enhancing resilience among service members, improving aspects impacting service members’ overall quality of life through an expansion of benefits and services, and improving the physical, psychological, social, and spiritual well-being of service members. This article explores the evolution of these suicide risk prevention efforts in consideration of the effectiveness of different strategies used by military branches and veteran programs.
