Abstract:
Organizational autonomy of the interuniversity athletic department, university responsibility for athletics, and pressure from nonuniversity individuals, groups, and organizations are all concerns related to the department's dependence on various sources in its environment for financial support. The Emerson (1962) power-dependence theory of social exchange relations, and its adaptation to the study of organization-environment relations (Pfeffer & Salancik, 1978; Thompson, 1967), guided an examination of funding and control in Canadian university athletics. This study examined whether athletic departments are perceived to be controlled by the funding sources in their environment according to their relative resource dependence upon those sources. Financial resource dependence and perceived control data were obtained from athletic directors (ADs) at 34 Canadian universities. Significant Spearman rank order correlations reveal the resource dependence-based perceived control of the university central administration, corporate sponsors, and provincial/federal sport organizations and ministries (p< .05). Of these, however, only central administration was perceived to have considerable control over the departments. Nevertheless, ADs should be aware of the resource dependence-based control potential of these other sources.

Abstract Objective The objective of this project was to enable poison control center (PCC) participation in standards-based health information exchange (HIE). Previously, PCC participation was not possible due to software noncompliance with HIE standards, lack of informatics infrastructure, and the need to integrate HIE processes into workflow. Materials and Methods We adapted the Health Level Seven Consolidated Clinical Document Architecture (C-CDA) consultation note for the PCC use case. We used rapid prototyping to determine requirements for an HIE dashboard for use by PCCs and developed software called SNOWHITE that enables poison center HIE in tandem with a poisoning information system. Results We successfully implemented the process and software at the PCC and began sending outbound C-CDAs from the Utah PCC on February 15, 2017; we began receiving inbound C-CDAs on October 30, 2018. Discussion With the creation of SNOWHITE and initiation of an HIE process for sending outgoing C-CDA consultation notes from the Utah Poison Control Center, we accomplished the first participation of PCCs in standards-based HIE in the US. We faced several challenges that are also likely to be present at PCCs in other states, including the lack of a robust set of patient identifiers to support automated patient identity matching, challenges in emergency department computerized workflow integration, and the need to build HIE software for PCCs. Conclusion As a multi-disciplinary, multi-organizational team, we successfully developed both a process and the informatics tools necessary to enable PCC participation in standards-based HIE and implemented the process at the Utah PCC.

Professor Susumu Hara is based at the Department of Aerospace Engineering, Nagoya University in Japan explains that when the Mars rover Opportunity was set to land on that planet in the first weeks of 2004, onlookers held their breath as it dropped from orbit and hurtled toward the red surface. 'Any failure in the calculations or landing systems would mean a harder than expected impact,' he highlights. 'The impacts sustained by a rover such as Opportunity can derail a mission before it even starts, damaging cargo or vital systems required to complete the mission.' Impacts occur during landing but also as the craft enters the atmosphere, when it makes sudden moves, while it is on surface or when debris strikes it. 'Therefore, a system and materials to protect a craft are vital,' outlines Hara. 'Surprisingly, the solutions to this problem are not sophisticated. In fact, most craft still employ devices resembling automobile bumpers, which absorb the energy from an impact by crumpling under the force of said impact.' Unfortunately, these cannot be reused, even during testing phases a new prototype is required after every single test run. Recent missions also employed techniques like airbags or sky cranes. While successful they too have drawbacks. 'Airbags create huge rebounds which can jostle the craft and the contents inside while sky cranes are extremely costly to develop,' Hara says. For this reason, he is dedicated to designing a new highly reliable and cost-effective shock control mechanism.

AbstractObjective:A computer virus outbreak was recognized, verified, defined, investigated, and controlled using an infection control approach. The pathogenesis and epidemiology of computer virus infection are reviewed.Design:Case-control study.Setting:Pharmacy of a tertiary care teaching institution.Results:On October 28, 1991, 2 personal computers in the drug information center manifested symptoms consistent with the “Jerusalem” virus infection. The same day, a departmental personal computer began playing ‘Yankee Doodle,” a sign of “Doodle” virus infection. An investigation of all departmental personal computers identified the “Stoned” virus in an additional personal computer. Controls were functioning virus-free personal computers within the department. Cases were associated with users who brought diskettes from outside the department (5/5 cases versus 5/13 controls, p = .04) and with College of Pharmacy student users (3/5 cases versus 0/13 controls, p = .012). The detection of a virus-infected diskette or personal computer was associated with the number of 5 1/4-inch diskettes in the files of personal computers, a surrogate for rate of media exchange (mean= 17.4 versus 152.5, p = .018, Wilcoxon rank sum test). After education of departmental personal computer users regarding appropriate computer hygiene and installation of virus protection software, no further spread of personal computer viruses occurred, although 2 additional Stoned-infected and 1 Jerusalem-infected diskettes were detected.Conclusions:We recommend that virus detection software be installed on personal computers where the interchange of diskettes among computers is necessary, that write-protect tabs be placed on all program master diskettes and data diskettes where data are being read and not written, that in the event of a computer virus outbreak, all available diskettes be quarantined and scanned by virus detection software, and to facilitate quarantine and scanning in an outbreak, that diskettes be stored in organized files.

Abstract Introduction This study examined risk factors for secondary traumatic stress (STS) in behavioral health clinicians and whether access to the Post-Traumatic Stress Disorder (PTSD) Clinicians Exchange website mitigated STS risk. Methods A diverse sample of clinicians (N = 605) treating traumatized military populations in Department of Veterans Affairs (VA), Department of Defense, and community practice settings were randomized to a newsletter-only control group or the exchange group. The exchange website included resources for treating PTSD and promoting clinician well-being. Online surveys were administered at 0-, 6-, and 12-months postrandomization. Regression analyses were used to examine the link among risk factors, exchange access, and STS. Results Baseline clinician demographics, experience, total caseload, appeal of evidence-based practices (EBPs), and likelihood of adopting EBPs if required were not linked with STS at the 12-month assessment period. Providing care at the VA, more burnout, less compassion satisfaction, greater trauma caseload, less openness to new EBPs, and greater divergence from EBP procedures were linked with greater STS. Only burnout and divergence were associated with STS after accounting for other significant STS risk factors. Exchange and control group clinicians reported similar STS levels after accounting for burnout and divergence. Conclusions Given that burnout was linked with STS, future intervention may use techniques targeting burnout and STS (eg, emotion regulation strategies). Research exploring the link between divergence from EBPs and STS may inform EBP dissemination efforts and STS interventions. Finally, results highlight the need for research optimizing STS intervention efficacy among clinicians treating military populations.

Abstract:
Activity-based costing (ABC) is a method where the costing of the product or service produced is performed on the basis of the activities. This method provides advantages to healthcare managers such as more realistic cost information, elimination of indirect, unnecessary and time-consuming activities and performance measurement. The aim of this study is to examine the applicability of ABC method in health institutions and to compare the calculated costs with the HIN price list. With this aim, the costs of the procedure of IVF center in a university hospital were calculated with the ABC method. Cost calculations for the procedures of Oocyte Pick-Up (OPU), Testicular Sperm Extraction (TESE), Embryo Transfer (ET) and Microinjection as discussed in the study were made with the ABC method and compared against the Healthcare Implementation Notification (HIN) system price lists. The calculated costs were determined according to the effective exchange rate of the dollar. It was revealed that the calculated costs differed from the price list. Finally, the findings suggest that ABC method could provide significant benefit for healthcare managers not only for to understand actual care costs but also to improving the cost control efforts as well as to planning the profitability analysis for hospitals. ABC method would provide a better opportunity and competitive for hospital managers to make a more accurate comparison between their and other health institutions.

This study examined the characteristics of internal accounting control system based on size of human resources assigned to the system to verify whether downward stickiness of total cost and selling and administrative expenses differentially appears according to the level.An entrepreneur who wishes to maximize one's utility has an incentive to externally grow the enterprise beyond the optimal size or to possess idle resources, creating cost sticking phenomenon by possessing surplus resources even when sales volume decreases. Previous studies verified the effect of control system intended to resolve the agency problem on asymmetry of cost. This study expanded the scope of previous studies to consider internal accounting control system as a monitor of the agency problem. Enterprises that effectively operate internal accounting control system are expected to show limited opportunistic incentive of the entrepreneurs because of reduced agency cost.The main purpose for enterprises to establish and operate internal accounting control system is to provide accurate and reliable financial information to investors by preventing or uncovering accounting scandals of the management and possibility of errors. Internal accounting control system of Korea was first introduced in September 2001 through 'Corporate Restructuring Promotion Act', and Financial Supervisory Service announced the regulation on public announcement in September 2002 as a series of relevant measures. The regulation requires enterprises to publicly announce the number of workers in each department in charge of internal accounting control system on the internal accounting control system report. Under the presumption that internal accounting control can be more effectively done by increasing the number of workers in charge of internal accounting control system, the intent of such public announcement is to provide external interested parties with information that can be used to determine whether internal accounting control system of an enterprise is being effectively managed. Accordingly in this study, enterprises with large size of human resources assigned to internal accounting control system were predicted to show relaxed downward stickiness of cost.As a result of verifying 2,532 enterprise-year samples listed on Korea Stock Exchange between 2003 and 2008, enterprises with larger size of human resources assigned to internal accounting control system were found to show relaxed downward stickiness of cost. This means that an enterprise with large number of workers assigned to internal accounting control system can effectively monitor the enterprise, which can efficiently control opportunistic decisions of the entrepreneur such as downward stickiness of cost. As an additional analysis, human resources were classified into departments to which persons in charge of internal accounting are affiliated to examine the effect on downward stickiness of cost. As a result of verification, high ratio of internal accounting personnel assigned to accounting department resulted in relaxed downward stickiness of cost. Accounting departments in charge of preparing for financial statements were more effectively performing monitoring work compared to other departments.

Abstract:
Recent archaeological and geological surveys, combined with a neutron activation analysis, have located a new obsidian source in San Luis in the Department of Santa Bárbara of Western Honduras. From the Early Preclassic through Late Classic periods, the ancient inhabitants of the neighboring region of La Entrada acquired obsidian from San Luis and Source Y directly from outcrops and nearby streams in the form of small cobbles. These cobbles, due to their small size, were employed exclusively for the unspecialized production of percussion flakes, using a bipolar technique, for household consumption. Political boundaries played an important role in the distribution of certain imported commodities having a high exchange value: among others, Ixtepeque obsidian. It is our belief that political control over the distribution of Ixtepeque obsidian was confined more to smaller spatial configurations than to the suggested territorial political control of the segmentary states in the Maya lowlands. The distribution of goods with lower exchange values, such as the San Luis and Source Y obsidian, may have escaped political control. Small obsidian cobbles from San Luis and Source Y were apparently not distributed over long distances, but were rather essentially local resources.

Design/methodology/approach – Discusses and draws conclusions from six recent, important developments: two administrative rulings from the Financial Crimes Enforcement Network (FinCEN), recent remarks by New York State Department of Financial Services Superintendent Benjamin Lawsky, remarks by Mark Wetjen of the Commodity Futures Trading Commission (CFTC), a recent Securities and Exchange Commission (SEC) informational sweep of crowdsales of crypto-equity, and the US Department of Justice proceedings against Trendon Shavers. Findings – Rather than trying to stifle or control virtual currencies, US governmental entities recognize the long-term value of virtual currencies and are trying to create a regulatory regime to foster growth and development, and an atmosphere where institutional and retail investors are protected. Originality/value – Provides an overview of the key United States regulatory issues facing companies engaged in Bitcoin-related businesses.

Abstract Objective To examine the impact of health information exchange (HIE) on reducing laboratory tests and radiology examinations performed in an emergency department (ED). Materials and Methods The study was conducted in an ED setting in Western New York over a period of 2 months. The care of the patients in the treatment group included an HIE query for every encounter, while the care of other patients in the control group did not include such queries. A group of medical liaisons were hired to query the medical history of patients from an HIE and provide it to the ED clinicians. Negative binomial regression was used to analyze the effects of HIE queries on the number of performed laboratory tests and radiology examinations. The log files of the HIE system since 1 year before the ED admission were used to analyze the differences in outcome measures between the 2 groups of patients. Results Ceteris paribus, HIE usage is associated with, respectively, 52% and 36% reduction in the expected total number of laboratory tests and radiology examinations ordered per patient at the ED. Conclusions The results indicate that access to additional clinical data through the HIE will significantly reduce the number of laboratory tests and radiology examinations performed in the ED settings and thus support the ongoing HIE efforts.

Currently, despite the use of renewable energy sources (RESs), distribution networks are facing problems, such as complexity and low productivity. Emerging microgrids (MGs) with RESs based on supervisory control and data acquisition (SCADA) are an effective solution to control, manage, and finally deal with these challenges. The development and success of MGs is highly dependent on the use of power electronic interfaces. The use of these interfaces is directly related to the progress of SCADA systems and communication infrastructures. The use of SCADA systems for the control and operation of MGs and active distribution networks promotes productivity and efficiency. This paper presents a real MG case study called the LAMBDA MG testbed laboratory, which has been implemented in the electrical department of the Sapienza University of Rome with a centralized energy management system (CEMS). The real-time results of the SCADA system show that a CEMS can create proper energy balance in a LAMBDA MG testbed and, consequently, minimize the exchange power of the LAMBDA MG and main grid.

We report the case of a 63-year-old male patient admitted to our emergency department for dyspnoea, peripheral oedema, severe diarrhoea and asthenia. History revealed Crohn’s disease (CD) submitted to several intestinal surgical resections in the previous years. He recently started a treatment with adalimumab for the control of CD. Laboratory tests at the admission revealed severe haemolytic anaemia and thrombocytopaenia. Haptoglobin levels were low, schistocyte count was markedly increased. In the suspect of thrombotic microangiopathy, he was admitted to our internal medicine department where we urgently started plasma exchange (PEX). We observed normal ADAMTS-13 activity in absence of Shiga toxin or enterotoxic Escherichiacoli at stool tests. Despite a diagnosis of atypical haemolytic–uraemic syndrome, we observed full platelet count recovery and schistocytes normalisation after the fourth PEX. We then put a diagnosis of adalimumab-induced thrombocytopaenic microangiopathy. Adalimumab was withdrawn. We did not observe relapses in the following 3 months.

Abstract:
Abstract Objective To observe the therapeutic effects of intraperitoneal injection of antibiotics, intravenous injection of terlipressin, and combined treatment of coloclysis and plasma exchange on hepatic failure(HF), the subjects included 494 inpatient cases of hepatic failure who were treated in Department of Infectious Diseases, Tongji Hospital, Tongji Medical College, Huazhong University of Science and Technology, Wuhan, China from 1997 to 2008. Methods The patients that met the inclusion criteria were divided into intraperitoneal antibiotic injection group, intravenous terlipressin injection group, coloclysis group, plasma exchange group, combination group of coloclysis and plasma exchange in terms of treatment given and a control group was set up for each of the treatment group. In the intraperitoneal injection group, the prognosis and changes in clinical manifestations were observed in HF patients complicated with spontaneous peritonitis (SBP). In terlipressin injection group,HF patients complicated with hepatorenal syndrome (HRS) were observed for prognosis and changes in serum creatinine. In the combination group, the improvement in serum total bilirubin and prothrombin activity were observed. Results Two weeks after intraperitoneal injection of antibiotics, the ease ratios of abdominal pain, pressure pain and rebound tenderness were 87.64%, 82.02% and 82.02% in the intraperitoneal injection group, respectively and the volume of ascites obviously decreased in 69 patients (77.53%). The survival rate in intraperitoneal injection group was significantly higher than in control group (P = 0.004). Four to eight days after the intravenous injection of terlipressin, the survival rate and the rate of serum creatinine decline of the treatment group were significantly higher than those in the control group (P = 0.003, P = 0.000). After 4 weeks of treatment, the ratio of clinical symptoms improvement (acratia, anorexia, abdominal distension, constipation) in coloclysis group were 60.27%, 57.53%, 91.78% and 94.52%, in plasma exchange group were 71.83%, 69.44%, 75% and 72.22%, and in combination group were 82.14%, 79.46%, 92.85% and 95.54%. The serum total bilirubin was decreased and the prothrombin activity increased and the differences were statistically significant as compared with control group (P = 0.000). Conclusions The intraperitoneal injection of antibiotics, intravenous injection of terlipressin and combined treatment of coloclysis and plasma exchange were all effective for the treatment of HF and its complications.

Objective Outbreak detection and disease control may be improved by simplified, semi-automated reporting of notifiable diseases to public health authorities. The objective of this study was to determine the effect of an electronic, prepopulated notifiable disease report form on case reporting rates by ambulatory care clinics to public health authorities. Methods We conducted a 2-year (2012-2014) controlled before-and-after trial of a health information exchange (HIE) intervention in Indiana designed to prepopulate notifiable disease reporting forms to providers. We analyzed data collected from electronic prepopulated reports and “usual care” (paper, fax) reports submitted to a local health department for 7 conditions by using a difference-in-differences model. Primary outcomes were changes in reporting rates, completeness, and timeliness between intervention and control clinics. Results Provider reporting rates for chlamydia and gonorrhea in intervention clinics increased significantly from 56.9% and 55.6%, respectively, during the baseline period (2012) to 66.4% and 58.3%, respectively, during the intervention period (2013-2014); they decreased from 28.8% and 27.5%, respectively, to 21.7% and 20.6%, respectively, in control clinics ( P < .001). Completeness improved from baseline to intervention for 4 of 15 fields in reports from intervention clinics ( P < .001), although mean completeness improved for 11 fields in both intervention and control clinics. Timeliness improved for both intervention and control clinics; however, reports from control clinics were timelier (mean, 7.9 days) than reports from intervention clinics (mean, 9.7 days). Conclusions Electronic, prepopulated case reporting forms integrated into providers’ workflow, enabled by an HIE network, can be effective in increasing notifiable disease reporting rates and completeness of information. However, it was difficult to assess the effect of using the forms for diseases with low prevalence (eg, salmonellosis, histoplasmosis).

Much recent works has focused on the role of oxidative disturbance in the preeclampsia and eclampsia. Preeclampsia remains a leading cause of maternal and fetal morbidity and mortality.Methods: This prospective study was conducted in upper India sugar exchange and maternity hospital, department of Obstetrics and Gynecology, GSVM Medical College, Kanpur in Collaboration with department of pathology, GSVM Medical College, Kanpur (UP), India. All the patients selected for the present study, both from control and study group was in detail with regard the clinical history, general examination, local examination, basic investigation and Specific Investigation, Super Oxide Dismutase (SOD). Blood samples would be collected with the informed consent from control as well as study group for assessment of antioxidant status by determining levels of superoxide dismutase.Results: The mean age and BMI of control as well as study group is almost same. The incidence of pre-eclampsia and eclampsia in primipara was 29.31 and in multiparous it was 9.52 in all groups. It implies that 95% cases will have improvement in SOD value with mean difference in improvement of at least 0.297807. There is statistically significant difference in mode of delivery by LSCS in study group A as compared to study group B and control in PIH patients. The incidence of complications is significantly higher in Study Group A as compared to control group and Study Group B. There is statistically significant difference in the incidence of preterm birth in study group A as well as Study Group B as compared to control. However, there was no statistically significant difference in the incidence of preterm birth after giving intervention to the study group. When cut off value of SOD enzyme as < 0.578U/mg of protein, sensitivity is 66.07% and specificity as 51.85%.Conclusions: Vitamin-c supplementation will only prevent PIT in PIH with already reduced antioxidant status.

Background: Severe neonatal jaundice can cause fatality and serious permanent effect, called kernicterus, in which the brain stem nuclei and basal ganglia are damaged. We are encouraged to work on efficacy of fluid supplementation in addition to photo therapy as a measure to reduce bilirubin level more efficiently.Methods: A randomized control trial study was conducted in Department of Paediatric Medicine, Neonatal Unit, R.G. Kar Medical College & Hospital, Kolkata, from April 2011 to March 2012. A total 100 term neonates presenting with severe non-hemolytic hyperbilirubinemia (>18 mg/dl to <25 mg/dl) were enrolled as study population. The study subjects were divided into two equal groups, study group and control group by randomization. The study group was given IV fluid supplementation in addition to photo therapy. Total serum bilirubin (TSB) level assessed periodically and results compared with control group.Results: The study results reveal a statistically significant association between the percent fall in TSB at 12 (Chi2= 7.18, p=0.000) and 24 hours (Chi2=10.69, p=0.000) in the intervention arm compared to the control group.Conclusions: Fluid supplementation along with double surface phototherapy in term neonates presenting with severe hyperbilirubinemia decreases the rate of exchange transfusion and duration of phototherapy.

This paper aims to provide an overview of how internal auditing has been adopted by companies listed in the STAR segment of the Italian Stock Exchange, and of the choice on its organizational position. After a review of the existing literature, the research aims to analyze the information emerging from corporate governance reports in 2015, in reference to the year 2014, which the companies prepare as part of information provided annually to the market. The sample for analysis is the group of companies (n. 71) belonging to the FTSE ITALY STAR Italian Stock Exchange.The research has shown, as the first element in the survey, that nearly all of the companies have created a dedicated function or outsourced the internal audit activity. The second major finding from the analysis is that in most companies the function depends hierarchically on the board. The third point analysed concerns the figure of responsible for Internal Auditing, as established by the Code of Conduct, it replaces the person responsible for internal control and risk management. The analysis showed that almost all the companies have attributed the tasks and responsibilities of the person responsible for internal control to the Head of the Internal Audit Department, due to the coincidence of the operating environments of the two figures and the strong synergies between the two roles. The last point analyzed covers the case where the internal auditing department is decentralized or, on the contrary, remains within the company. Abstract Objective Poison control centers (PCCs) routinely collaborate with emergency departments (EDs) to provide care for poison-exposed patients. During this process, a significant amount of information is exchanged between EDs and PCCs via telephone, leading to important inefficiencies and safety vulnerabilities. In the present work, we identified and assessed a set of data standards to enable a standards-based health information exchange process between EDs and PCCs. Materials and methods Based on a reference model for PCC–ED health information exchange, we (1) mapped PCC–ED information exchange events to clinical documents specified in the Health Level Seven (HL7) Consolidated Clinical Document Architecture (C-CDA) Standard, and (2) mapped information types routinely exchanged in PCC–ED telephone conversations to C-CDA sections. Results Four C-CDA document types were necessary to support the PCC–ED information exchange process: History & Physical Note, Consultation Note, Progress Note, and Discharge Summary. Information types that are commonly exchanged between PCCs and EDs can be reasonably well represented within these C-CDA documents. Conclusions A standards-based health information exchange process between PCCs and EDs appears to be feasible given a set of clinical data standards that are required for EHR certification in the USA, although the proposed approach still needs to be validated in actual system implementations. Such a process has the potential to improve the safety and efficiency of PCC–ED communication, ultimately resulting in improved patient care outcomes.

This study is to examine the nature of collaboration between the Department of Home Affairs Australia (DHA) and the Directorate General of Immigration Indonesia (DGI) to protect their borders and intercept unauthorized arrivals, and to identify current challenges of postings of Airline Liaison Officers (ALOs) at airports in Indonesia. This study employs the qualitative research method methodology using the document analysis and observation at Jakarta and Bali airports by collecting sources of data from public organizational reports, official documents, books, online publications, or journal articles. The data is analyzed by using the five indicators by O’Leary & Vij: power, communication, perceived legitimacy, trust, and information exchange. The study shows the collaboration between DHA and DGI in posting of ALOs at airports in Indonesia has been running for more than a decade as the extraterritorial immigration control despite legal challenges, sovereignty, non-refoulment principles, human rights violations, and authority overlap. This paper recommends both countries to ratify new arrangements about posting of ALOs subject to the national sovereignty, data sharing, use of technology, Indonesian legislations, and involvement in operation protocols. Abstract Background Legionnaires’ disease (LD) causes significant morbidity and mortality to hospital patients and residents of skilled nursing facilities (SNF). In California, LD is reportable to local health departments via the California Reportable Disease Information Exchange (CalREDIE) surveillance system. Cases are classified as suspected or confirmed using Centers for Disease Control and Prevention (CDC) definitions. The California Department of Public Health (CDPH) Healthcare-Associated Infections (HAI) Program maintains a database of healthcare-associated LD (HA-LD) and consults with local public health departments for single cases and outbreaks. Methods We described characteristics of confirmed HA-LD cases in 2015–2017. We classified HA-LD as definite if patient had continuous exposure in a facility for 2–10 days prior to symptom onset and possible if patient had overnight exposure in a facility for a portion of 2–10 days prior to symptom onset. Results From 2015 to 2017, 125 (8%) of 1,554 confirmed LD cases were HA-LD. Of these, 73 (58%) were definite HA-LD and 52 (42%) were possible HA-LD. The majority of HA-LD cases (N = 99, 79%) occurred in southern California. SNF were associated with 57 cases (46%) and hospitals with 44 cases (35%); 23 cases (18%) had exposures in both SNF and hospitals during the incubation period. Among the definite HA-LD cases, 50 cases (68%) had exposures in a single SNF. The median age of patients with HA-LD was 77 years. The HAI Program consulted with 15 local public health agencies on 33 HA-LD investigations, including 7 outbreaks and 26 single-case investigations. Conclusion HA-LD represented a small but important percentage of LD in California; the majority occurred in SNF. To prevent HA-LD, California hospitals and skilled nursing facilities should implement water management programs, as recommended by CDC and required by the Centers for Medicare and Medicaid Services (CMS) since June 2017. Abstract Introduction/Objective Autoimmune hemolytic anemia (AIHA) is a group of disorders with limited epidemiological and clinical data, characterized by hemolysis due to autoantibodies against red blood cell surface antigens. Cold agglutinins account for about 25% of all AIHA, which can agglutinate erythrocytes at 0-4 °C. Cold agglutinin disease (CAD) is a self limited disease and usually does not cause significant hemolysis. Here, we report a case with an unusually severe course for secondary CAD and role of plasma exchange in the management. Methods A 53-year-old male patient with no known past medical history presented to the emergency department with shortness of breath and syncope after a week of upper respiratory tract infection symptoms. The patient became profoundly anemic (presented with 8.1 g/dL hemoglobin level and progressively decline to 4.2 g/dL) in a short period of time. Upon initial evaluation, his peripheral blood smear demonstrated clumps of RBCs. Direct anti-globulin testing was negative for IgG but positive for C3 which is consistent with CAD. A respiratory PCR panel detected Rhinovirus. Mycoplasma and EBV IgM were negative. Additionally, chest X-ray showed right middle lobe pneumonia that was treated with antibiotics. Patient received multiple transfusions of pre-warmed pRBCS and showed initial improvement but eventually went into respiratory failure and cardiac arrest with return of spontaneous circulation after 8 minutes of CPR. Subsequently, plasma exchange was started. Only after intiation of plasma exchange, the patient’s ongoing hemolysis was stabilized. Conclusion This case report presents a patient with unexpectedly rapid and severe hemolysis from secondary cold agglutinin disease. Interestingly, the case appeared not to be caused by EBV or Mycoplasma pneumonia infection but Rhinovirus. Further studies confirmed patient had no autoimmune disorder or lymphoid malignancy that may had initiated secondary CAD. On the management aspect, pre-warmed RBC transfusions were not sufficient to stabilize the patient’s condition. Plasma exchange was able to control ongoing hemolysis within 2 sessions successfully.

Materials and Methods 24 children with sickle cell anemia and 9 healthy children as control group include to the study. Complete blood count, hemoglobin electrophoresis and biochemical values were eveluated for both groups. At pulmonology department, the carbonmonoxide diffusion test performing for both groups. At the same day spirometric respiratory function evaluation and exercise test performed both groups at department of sports physiology. The data recieved are compared statistics. Results HbS, HbF, SGPT, ferritin, total bilirubine, direkt bilirubine and Fe++ values were high at patient group (p<0.05). Hemoglobin and hematocrit values were low at patient group according to control group as expected (p<0.05). The number of SCA patient who had one-three venoocclusive crises (VOC) were 14 (58.3%), patient who had three or more VOC were 7 (29.2%) and patient who had no VOC were 3 (12.5%). The number of patient who had acute chest syndrome (ACS) were 5 (20.9%) and 19 patients had no ACS (79.1%). Ýmpaired isole carbonmonoxide diffusion test was established at the 62.5% of the patient's. At patient group, spirometric FEV1 and MEF25 measurement were found lower than the control group (p<0.05). At exercise test VO2/HR rate were lower for patient group (p<0.05). VE/VO2 rate (p=0.023) and R (p=0.016) measurement were found higher. Conclusion Pulmonary gas transfer was found difficult in patients with SCA. Respiratory airways established obstructed in spirometric evaluation. Obstructive defficiensies have to be follow up. Oxygen pulse and respiratory exchange rates were determined low and more oxygen usage was observed for aerobic metabolic activity. With these results, ýt can be say that chronic inflamation process at lung due to oxygen radicals and hipoksemia in sickle cell patients, the aerobic respiratory load was increased. Responses in photosynthesis, transpiration, stomatal conductance, chlorophyll fluorescence characteristics and chlorophyll content of four maize (Zea mays L.) genotypes were examined under first phase of salt stress. In the experiment four maize genotypes viz. indigenous yellow pure line, indigenous yellow, hybrid, and indigenous white were tested in two levels of salinity (control: without NaCl application; salinity: 12 dS m-1 by applying NaCl). The experiment was laid out following completely randomized design (CRD) with four replications in the net house of Department of Crop Botany, Bangladesh Agricultural University, Mymensingh. Plants were harvested on day 21 after 7 d application of full salt stress (12 dS m-1). The reductions of shoot fresh masses under salinity were 70, 57, 55 and 61% in indigenous yellow pure line, indigenous yellow, hybrid, and indigenous white, respectively. Some core physiological parameters viz. net photosynthesis rate (Pn), transpiration rate (E), stomatal conductance (gs), and the chlorophyll content decreased in all the maize genotypes except indigenous white under first phase of salt stress. In contrast, the ratio of variable fluorescence to maximal fluorescence (Fv/Fm) of photosystem II significantly decreased only in indigenous yellow pure line under salinity stress. Thus the first phase of salt stress seemed to be deleterious on its response to shoot fresh mass production in all the tested four maize genotypes with the concomitant decrease in rate of photosynthesis, rate of transpiration, stomatal conductance and chlorophyll content in all genotypes except indigenous white. Since the beginning of the 2000s (the time of Russia's accession to the Bologna process), a great deal of experience has been accumulated on the need and effectiveness of the score-rating system of knowledge assessment on the example of various regions, universities, specialties and disciplines. This experience shows both the disadvantages and merits of the ball-rating system, but in general it is obvious that the “traditional” form of knowledge control in modern conditions has lost its relevance and in most cases is ineffective. The experience of using the ball-rating system at the Department “Foundations and Foundations” in assessing the knowledge of students of construction specialties in the discipline “Soil Mechanics” showed its effectiveness and greater objectivity in comparison with the traditional form of control. The main advantages of the system are its objectivity, increasing the motivation of students to systematically work throughout the semester, the ability to automate the process of calculating, calculating rating points and conducting current forms of control. In order to increase the efficiency of the ball-rating system in the conditions under consideration, this process should be made as clear and open to the student as possible through interactive methods of information exchange, clarification of forms and methods of control, methods of calculation and calculation of rating points. In the academic load of the teacher, time must be officially provided for working with automated control systems, setting and counting rating points and setting the parameters of these systems, since they are mandatory components of a score-rating system.

Background: Thyroid dysfunction, a common endocrine disorder that increasing day by day. Hypothyroidism occasionally occurs in diabetic patients especially those with poor glycemic control. Objectives: The objective of the study was to observe the effect of glycemic control on thyroid hormones level in type 2 diabetic patients & to determine selected socio-demographic characteristics of the respondents. Methods: This was a case control study carried out in the Department of Physiology, Sir Salimullah Medical College (SSMC), Dhaka from July 2016 to June 2017. Total sixty subjects of both male and female age ranged from 40 to 60 years were included in this study. Among them thirty were type 2 diabetic patients (both uncontrolled and controlled) and another thirty were included for comparison as control and were matched by age, sex, and body mass index (BMI). Both case and control group were selected purposively based on selection criteria. Serum TSH, FT4 and FT3 levels were measured by chemiluminescent micro particle immunoassay (CMIA) method. Fasting blood glucose level was estimated by glucose oxidase (GOD) method and blood HbA1c level was estimated by ion exchange high performance liquid chromatography (HPLC) method. The statistical analysis was done by ANOVA test, Bonferroni test and Pearson’s correlation coefficient test. Data quality control was done by using a checklist. Results: In this study, mean serum TSH level was significantly higher in uncontrolled diabetic patients than that of controlled diabetic patients. Serum FT4 level was significantly lower in uncontrolled diabetic patients in comparison to that of controlled diabetic patients. Besides these, serum TSH level was positively correlated (r = +0.575), serum FT4 and FT3 levels were negatively correlated (r = ̶ 0.588, r = ̶ 0.527) with HbA1c level in uncontrolled diabetic patients and all these relationships were statistically significant p≤0.05 respectively. Conclusion: The present study revealed that hypothyroidism occurs in type 2 diabetic patients which were found only in uncontrolled diabetic patients due to their poor glycemic control. Ports and industrial waterfronts today face complex problems associated with environmental contamination and habitat loss that affect the ability of local governments to plan and carry out dredging and waterfront development projects. In the USA, separate regulatory programs exist to address source control, cleanup of contaminated sediments, navigational dredging projects, habitat restoration, and shoreline development projects, each with its own set of agencies, permits, and regulatory requirements. In some areas, these overlapping and often conflicting requirements and interests have led to extreme difficulties in completing port development and cleanup projects, even when these projects could have substantial benefits to the environment. This paper presents a shift in regulatory approach toward in-water projects currently being tested by the Department of Ecology and other federal, state, and local agencies in Washington State. Three projects are described that provide regulatory streamlining in exchange for integration of cleanup and habitat restoration needs into Port navigation and waterfront redevelopment projects. In addition to providing significant benefits to both navigation/commerce and the environment, substantial cost savings can be realized in comparison to traditional approaches to these projects.

In the aftermath of India's first nuclear explosion in 1974, U.S. officials concluded that Brazil posed a growing proliferation risk, and they proposed to target Brazil with a new set of nonproliferation policies that included the denial of fuel-cycle technologies. However, Secretary of State Henry Kissinger expressed doubt that such an approach would curb Brazilian nuclear ambitions. Pushing back against influential voices in the U.S. Congress, the State Department, and the U.S. Arms Control and Disarmament Agency, Kissinger argued that Brazil should be allowed to proceed with its plans to master the nuclear fuel cycle in exchange for U.S. assistance and new nonproliferation commitments. He justified this attitude on the grounds of alliance politics (Brazil was too important a pillar of U.S. Cold War policy in Latin America) and the interests of key domestic constituencies (U.S. private companies eyed Brazil's burgeoning nuclear industry). The Brazilian government responded well to Kissinger's approach and would have struck a deal if the transition to the Carter administration had not rendered the bargain untenable.

The United States takes advantage of legal procedures against foreign companies to collect millions of bytes of data, sometimes including sensitive information on them as well as on their partners and markets. Facing this legal offensive, Europe is still struggling to provide responses to protect its companies.

The reaction of the early ripening potato of Vineta variety to different methods of using growth regulators against the calculated background of mineral nutrition on the gray forest-steppe soil of middle Volga was studied. The effectiveness of processing planting tubers and foliar treatment of plants during their vegetation has been established. The experiments were carried out on the control without the introduction of NPK and on a fertilized background, designed to obtain a tuber crop of 40 tons per hectare. The studies were conducted on gray forest medium loam granulometric composition soil of “Zemlyaki” farm in Nizhnekamsk district of the Republic of Tatarstan. Field experiments were carried out in 2019 by employees of Plant Growing and Horticulture Department of Kazan State Agrarian University. The humus content in the arable layer of soil according to Tyurin is 3.35%, pH is 5.7, easily hydrolyzable nitrogen is 112 mg/kg of soil, mobile phosphorus is 156 mg/kg, exchange potassium is 136 mg/kg of soil, molybdenum is 0.07, copper is 0.52, boron - 0.7, zinc - 4.82 mg/kg. The effectiveness of the integrated use of growth regulators (treatment of planting tubers and foliar application of the drug during the growing season) has been established. The yield increase to control against the background without applying basic fertilizers amounted to 4.82 tons per hectare, and against the background of their application - 4.80 tons per hectare.

This study aims to determine the effect of the independent variable other comprehensive income on the dependent variables represented by the financial performance of commercial banks. Researcher has studied the case of Jordanian Commercial Banks during the period 2012 to 2017. The study sample consists of 13 Jordanian commercial banks. The study independent variable was given by the quotient of other comprehensive income on net income. The ratios: financial performance by return on assets, and return on equity were the two dependent variables. Study hypotheses were tested by the simple regression equation and T- test. It was found that there was a statistically significant effect of other comprehensive income on the financial performance as measured by the return on assets and return on equity. This significance can be attributed to the increasing weight of other comprehensive income items which makes the value of other comprehensive income an indicator of profitability and efficiency of banks and means of maximizing their wealth. It is recommended that Amman Stock Exchange, Securities Commission and the Companies Control Department, to urge the companies listed on ASE to increase the awareness of companies’ management about the importance of other comprehensive income concept.

Medical education requires high-quality, up-to-date world-class training and continuing professional development. The purpose of this work is to analyze the experience of using distance learning forms for intern doctors in the specialty "Emergency Medicine". Results and discussion. The Department of Internal Medicine and Emergency Medicine has created an information and educational domain providing the storage, updating and exchange of various educational information that include the website of our department website and the ZOOM platform. The main types of training are: lectures, seminars, practical classes, consultations and independent work. Independent work is provided by the using educational materials that interns receive via the Internet. Interactive communication is also done via e-mail, the Viber platform. The elaborated types of control enable to assess the initial and intermediate levels of knowledge gained through the learning process. Having evaluated the results of distance learning in interns, we can conclude the interns do demonstrate satisfactory level of theoretical knowledge, but their level of practical skills have been assessed as unsatisfactory. This encourages searching for new forms of distance learning. Thus, distance learning of interns in the specialty "Emergency Medicine" is an effective alternative form of theoretical knowledge acquisition. Improving the methods of conducting online classes helps to improve the assimilation of theoretical material. 3. However, since the doctors in the field of «Emergency Medicine» are clinicians, distance education can not be considered a priority, but only as one of the elements of training.

Professional management of paediatric diabetology, according to consensus guidelines, involves the screening of micro-vascular complications at puberty. The subclinical form of peripheral neural dysfunction in diabetic teenagers is reported with a frequency of 50-88%, by different authors. The purpose of this study was to evaluate the frequency of subclinical distal neuropathy (DSMN) in type 1 diabetic pediatric patients during the second decade of life, and its relationship with metabolic control. The Endocrinology Department and the Neurology-Physiology Laboratory of the Pediatric Clinic in Belgrade carried out a longitudinal follow-up study (lasting 18 months, beginning in November 2000) on a selection of patients with poor metabolic control. During routine clinical treatment, patients were evaluated using the electrophysiological diagnostic method on peripheral neural dysfunction, a subclinical form of neuropathy. Metabolic control was manifested through HbA1c levels, measured every 3 months, using ion-exchange chromatography. Finally, here is the data collected from the clinical follow-up investigation of 60 children, aged 13-19 (median 1S.S?2.2), with duration of diabetes from 2-16 years (median b.3?3.b), and on the following therapies: 43 CT-conventional and 17 IIT-intensive, and insulin dose/day, median 1.02 (0.6-2.1) U/kg. Detected DSMN parameters at the beginning and at the end of the study were also noted. DSMN frequency was positive, at 64% for HbA1c of 9.44; DSMN dysfunction was reversed in 5% of the patients, for HbA1c of 10.17; the worst result was the progression of DSMN at 6.7% for HbA1c of 10.52; 6.7% had negative DSMN, with improved metabolic control, for HbA1c of 8.4; 15% of the examinations were unfinished (+/*). ANOVA statistical analysis showed a significant statistical relationship between metabolic control (HbA1c levels) and DSMN neuropathy (sig. 0.043, p<0.05). There was no significant relationship between the reversion of DSMN and improved HbA1c, although a numeric distinction did exist. On the clinical aspect, there was a significant relationship between insulin dosage and age (p<0.01, sig. 0.007). This data demonstrates the influence of metabolic regulation on neuropathy. Better metabolic control can slow the progression of subclinical peripheral neural dysfunction (DSMN) in diabetic children.

AimsIn 2013, we introduced a specialized, centralized, and interdisciplinary team in our institution that applied a standardized diagnostic and treatment algorithm for the management of prosthetic joint infections (PJIs). The hypothesis for this study was that the outcome of treatment would be improved using this approach.Patients and MethodsIn a retrospective analysis with a standard postoperative follow-up, 95 patients with a PJI of the hip and knee who were treated with a two-stage exchange between 2013 and 2017 formed the study group. A historical cohort of 86 patients treated between 2009 and 2011 not according to the standardized protocol served as a control group. The success of treatment was defined according to the Delphi criteria in a two-year follow-up.ResultsPatients in the study group had a significantly higher Charlson Comorbidity Index (3.9 vs 3.1; p = 0.009) and rate of previous revisions for infection (52.6% vs 36%; p = 0.025), and tended to be older (69.0 vs 66.2 years; p = 0.075) with a broader polymicrobial spectrum (47.3% vs 33.7%; p = 0.062). The rate of recurrent infection (3.1% vs 10.4%; p = 0.048) and the mean time interval between the two stages of the procedure (66.6 vs 80.7 days; p < 0.001) were reduced significantly in the study group compared with the control group.ConclusionWe were able to show that the outcome following the treatment of PJIs of the hip and knee is better when managed in a separate department with an interdisciplinary team using a standard algorithm.

High productivity and power of technological and transport mechanisms, large capacity and number of storage equipment, a variety of types of grain crops simultaneously processed at the elevator require the operator to make a lightning-fast decision, the optimal choice of grain transportation routes.One of the first control systems consisted of huge energy-intensive cabinets with starting equipment, allowing remote control of equipment, without the functions of elementary control of their operation.As storage volumes increased, these systems were supplemented with relay cabinets, whose task was to conduct interlocked control of an ever-increasing number of mechanisms, so that at least somehow it was possible to control such a complex. These relay cabinets had a low degree of reliability (contact elements), consumed a large amount of energy, and required the constant presence of large maintenance personnel.The era of microelectronics has come, which completely changed the idea of the capabilities of control systems. At first, it seemed very expensive and not a quick payback. But every year, the cost of automation equipment is decreasing, reliability has grown to high levels and this technique has come to the management systems of grain storages and elevators.Modern automation at the majority of Ukrainian elevators makes it possible to control technological processes. At the same time, many processes are regulated in a mode that requires a sufficiently high qualification of service personnel. The exchange of data on the operation of SMART-INDIVIDUAL equipment is carried out by logical controllers by polling sensors, generating control actions, monitoring equipment operating modes with display in the WEB-interface system. The SMART-INDIVIDUAL system has undeniable advantages over existing centralized control and management systems. SMART technology is inextricably linked with the dispatch control and data collection system - SCADA, designed to monitor and supervise a large number of remote objects or one geographically distributed object. The SMART-INDIVIDUAL system is equipped with a module with a regulation for monitoring parameters, equipment according to the passport characteristics. At the same time, the system maintains an archive and controls the timing of maintenance. The laboratory installation SMART-INDIVIDUAL includes technological and transport equipment, which is most used in the elevator industry: air filter ZEO-FCS, bucket elevator ZEO-BE, scraper chain conveyor ZEO-DC. On the basis of the SMART-INDIVIDUAL software and hardware complex at the Department of Technological Equipment for Grain Production of the Odessa National Academy of Food Technologies, students are trained in the new specialty "IT service of equipment".

The entire Peninsular Malaysia is situated on Sunda Tectonic Plate, which is subjected to motion at a prominent rate. All the geodetic infrastructures, including geodetic markers, benchmarks, Continuous Operating Reference Stations and Cadastral Reference Marks have moved away from their original position, and their existing coordinates are no longer reliable and secure to use. There are 10 geodetic markers around UTM that are subjected to the issue above. This study aims to redefine new geodetic and plane coordinates on UTM geodetic markers. Two units of Trimble NetR9 Geodetic Type Receiver are used to execute 3D GNSS Control Network on geodetic markers as well as Standard Benchmarks. Receiver Independent Exchange data of Continuous Operating Reference Stations and gravimetric geoid of MyGeoid model are retrieved from Department of Survey and Mapping Malaysia. Trimble Business Center, Golden Surfer 8 and StarNet are used as post-processing, geoid interpolation and one-dimensional network adjustment software, respectively. New sets of geodetic and plane coordinates along with orthometric heights are produced for these 10 geodetic markers. Between 1821 and 1842, Charles Bird King painted a series of portraits of Native American diplomats for Thomas L. McKenney, founding Superintendent of Indian Affairs. These pictures were hung in a gallery in McKenney's office in the War Department in Washington, DC, and were later copied by lithographers for inclusion in McKenney and James Hall's History of the Indian Tribes of the United States (1836–44). Significantly, the production and circulation of these portraits straddles a period of tremendous change in the diplomatic interactions between the United States and Native tribes. This essay analyzes a selection of these images for their complex messages about the sovereignty of Indian people and their appropriate interactions with European American culture. Paying particular attention to pictures of leaders of southern nations, including the Cherokee, Creek, and Seminole, I discuss the sitters' strategies of self-fashioning within the context of long-standing cultural exchange in the region. In addition, I offer a reading of the meaning of the Indian gallery as a whole that challenges the conventional wisdom that it is an archive produced exclusively to impose US control on the subjects included, arguing instead for the inclusion of portrait-making within this history of interaction.

The purpose of the research is increasing the efficiency of using liquid and dry biological preservatives during alfal-fa haylage. Conservation based on use of individual strains or lactic acid bacterium community is a promising direc-tion of resource – saving technologies for foraging. Influence of biologies on organoleptic properties, safety, nutri-tional value and microbiological parameters of alfalfa haylage was studied. The experiment was conducted in 2016-2018 in the laboratory of Agro-Biological Department of Tatar Research Institute of Agriculture of the Kazan Scientific Center of the Russian Academy of Sciences. The object of research is influence of biological preserva-tives on the nutritional properties of alfalfa haylage, and Aislu alfalfa breed herbage. Industrial samples of Ferbak-SIL liquid biological preservatives with and dry Bioamide-3 enzyme free were used for haylage. It was found that, on average, for 3 years of the experiment, the use of preparations led to the improvement of bio-conservation of herbage, optimization of microorganisms and improvement of nutritional properties of alfalfa haylage. Adding liquid Ferbak-SIL bio-preservative with enzymes to alfalfa herbage led to an increase in the safety of dry matter by 2.06%, raw protein – by 0.69%, raw fiber – by 0.37%, raw fat – by 0.19%, exchange energy – by 7.10%, and concentration of lactic acid bacteria-by 230.1%, compared to the control variant. Values of similar indicators in the variants of the experiment with the dry biological product Bioamide-3 enzymes free were, respectively, 0,78, 0,51, 0,11, 0,08, 3,13, 170,8 % in relation to control.

Background: Healthcare-associated group A Streptococcus (GAS) infections can cause severe morbidity and death. Invasive GAS is a reportable condition in the 5-county metropolitan area of Denver, Colorado. Prior to August 2018, methodology to identify long-term care facility (LTCF) residency among reported GAS cases was accomplished by reviewing addresses reported electronically, and identification of postsurgical cases and outbreaks relied on reporting by healthcare facilities. We evaluated whether the use of a health information exchange (HIE) to identify healthcare exposures improved our ability to detect and rapidly respond to these events. Methods: In August 2018, we implemented a review of health records available in the HIE accessible by the Colorado Department of Public Health and Environment for all incoming reports of GAS for selected healthcare exposures: LTCF residency, surgery, delivery, wound care, and other relevant exposures. We defined an LTCF-related case as GAS in a current or recent resident (ie, in the 14 days prior to the positive culture) of an LTCF. Postpartum and postsurgical cases were defined as GAS isolated from a sterile site or wound during the inpatient stay or within 7 days of discharge following a delivery or surgical procedure. Outbreaks in each of these settings were defined as 2 or more cases within a 3-month period. We compared the number of cases and outbreaks identified in each category during a 1-year period before and after implementation of the use of the HIE in the case ascertainment process. Results: During August 2017 through July 2018, prior to implementation of the HIE process, we detected 45 LTCF cases and conducted outbreak investigations in 9 facilities. Moreover, 1 postsurgical case and 1 postpartum outbreak were reported by healthcare facilities; none were detected via surveillance. During August 2018 through July 2019, after the implementation of HIE process, we identified 70 LTCF cases and conducted outbreak investigations in 13 LTCFs. We detected 5 postsurgical cases and 3 postpartum cases, which resulted in 2 outbreak investigations. Conclusions: Enhanced GAS surveillance through use of a HIE resulted in detection of more healthcare-associated GAS infections and outbreaks. Timely identification of healthcare-associated GAS infections can allow for prompt response to outbreaks and promotion of proper infection control practices to prevent further cases. Jurisdictions in which GAS is a reportable condition should consider the use of HIEs as part of routine surveillance to identify GAS outbreaks in high-risk settings. Harm minimisation strategies aim to reduce the deleterious health and social consequences to the individual in drug use and abuse and the potential financial burden to the community. Harm minimisation is the guiding principle of the Steroid Peer Education Program (SPEP). This is an outreach program funded by the Department of Human Services (DHS) which provides counselling, education and needle exchange facilities for body-builders who inject with anabolic steroids. In 1998, SPEP became aware that a substantial number of clients were misusing insulin in an attempt to enhance muscle growth. It appears that these athletes were unaware of the dangers of insulin use, such as hypoglycaemia, and the potentially fatal consequences of this practice. Research was carried out which highlighted the need for education material as a harm reduction strategy. Such education material could be used to raise awareness of the dangers of non-medical insulin use and outline how to prevent and treat a hypoglycaemic episode. It appears pertinent to raise awareness of the non-medical use of insulin especially preceding the Sydney 2000 Olympic Games. In addition to the dangers inherent in insulin use as an anabolic enhancement agent, its use cannot be detected by existing control measures. Finally, the long term health consequences of this practice are largely unknown and represent, therefore, an area for future investigation.

Abstract:
The aim: The study focuses on the morphometric features specification concerning the structural components of the hemomicrocirculatory bed of the perivulnar region of the cecum in rabbits when suturing a wound defect with synthetic polyfilament surgical suture Vicryl. Materials and methods: The research was carried out on rabbits (n = 30: 5 animals made up the control group and 25 animals made up the experimental group) and included surgery based on the method of surgical intervention on the large intestine patented by the authors, which was performed in the conditions of the operating room at the Department of Clinical Anatomy and Operative Surgery. When determining the main morphometric parameters, we took biopsy specimens of the cecum and used paraffin and epoxy resin embedding considering the generally accepted methods. Results: The study determined that the implantation of Vicryl surgical suture material in the tissues of the cecum became a trigger for the formation of active inflammatory focus and occurrence of leukocyte infiltrate in the perivulnar region. Conclusions: Arterioles, as a resistive link, capillaries, as an exchange link and venules, as a capacitive link in the hemomicrocirculatory bed, actively participate in response to the inflammatory process and formation of a connective tissue scar. The process of structural organization of microvessels begins in the formed granulation tissue in the perivulnar region of the cecum starting from the 14th day of the experiment.

SummaryConsumer-mediated health information exchange (HIE) is one of the three types of HIE designated by the Office of the National Coordinator. HIE is intended to improve the quality of care while reducing cost, yet empirical support for this claim is mixed. Future research should identify the contexts whereby HIE is most effective.This study was conducted as a pilot two-arm randomized controlled trial. In the intervention arm, 27 veterans were taught how to generate a Continuity of Care Document (CCD) within the Blue Button feature of their VA patient portal and were then asked to share it with their community non-VA provider. In the attention control condition, 25 Veterans were taught how to look up health information on the Internet. The impact of this training on the next non-VA medical visit was examined.Nineteen (90%) veterans in the intervention arm shared their CCD with their non-VA provider as compared with 2 (17%) in the attention control arm (p<0.001). Both veterans and non-VA providers indicated high satisfaction with the CCD. Comparison of medical records between the VA and non-VA providers did not indicate improved medication reconciliation (p=0.72). If veterans shared their CCD prior to their non-VA providers ordering laboratory tests, the number of duplicate laboratories was significantly reduced (p=0.02).In this pilot randomized controlled trial, training 52 veterans to share their CCD was feasible and accepted by both patients and providers. Sharing this document appeared to reduce duplicate laboratory draws, but did not have an impact on documented medication list concordance. Abstract:
The aim of the research is to confirm whether “God’s Not Dead” (2016) production is an adequate portrayal of religious liberty of school officials in present-day K-12 schools in the United States. The author limited the discussion to the essential problem questions inspired by the production, such as teachers’ right to express their religious views both in the formal setting of a class discussion, and out-of-class informal exchange with students and fellow faculty members. The problem was discussed against the backdrop of selected legal and legislative acts that determine the scope of teachers’ religious expression, such as selected courts’ decisions, official federal guidelines issued by the Department of Education as well as the guides published by non-government organizations. The results of the Bible Literacy Project as well as interviews with several school officials were utilized to further verify the basic concepts of the research. The source texts were analyzed using a close reading method. The study seems to prove that due to the complexity of the legal system as well as the discrepancy between the lines of decisions reached in lower courts it difficult to unambiguously assess the probability of the scenario featured in the movie. However, observable trends in case law and governmental control over teachers’ in-class speech appear to imply that the script is definitely accurate in its portrayal of a legal liability of teachers, and possibly less so in respect of the favorable outcome of the dispute.

The COVID-19 disease broke out globally, bringing great challenges to public health security and increasing the difficulties in constructing ecological civilization governance system. Under the leadership of the Central Committee of the Communist Party of China with Comrade Xi Jinping at its core, the Chinese Government and people have fought against the disease with courage and put people’s health and safety first. Through hard work, China has made achievements in controlling the COVID-19 pandemic and accelerated the restoring of work and life back to normalcy. Background: Telemedicine has been posited as a potential means of bolstering radiation therapy delivery in developing nations. World Aid Exchange (WaidX) is an innovative intercontinental telemedicine platform oriented to oncology specialties. This platform, devoted to reducing the digital divide on health practice, provides telecommunication services between health care facilities in developed and developing countries. It conveys the ability to safely share radiologic images and patient medical records for diagnostic and care purposes. It was successfully piloted in Mwanza, Tanzania in 2015. Since then, it has been implemented in varied settings such as Ethiopia, Djibouti and Brazil. After conducting a site visit and a focused needs assessment, we recognized the need for teleconferencing with the Radiation Department of National Center of Oncology, Yerevan, Armenia, to share expertises in general patient management and contouring and planning for radiotherapy. Aim: To develop a TeleRadiotherapy platform that enables: 1: Conference calling for tumor boards to review radiotherapy plans, discuss disease management and conduct remote quality control 2: Real-time sharing of diagnostic images to guide clinical decision making 3: E-contouring activity performed by parties in Yerevan and New York on radiographic images, with minimization of time lag in contouring 4: Generation of a database for clinical data (i.e., radiation dose, toxicity, disease stage) that serves as a departmental registry and a tool for future research use 5: Access to lectures delivered by physicians, nurses, therapists and physicists both in Yerevan and New York on varied aspects of radiotherapy Methods: The initiative was funded through a competitive grant established within the Department of Radiation Oncology at Cornell. The TeleRadiotherapy system is comprised of 2 physical units, equipped to support networking and telephony integration. An application was used to establish a simplified direct connection between mobile phones in New York and fixed phone extensions in Yerevan. A customized version of Veyon was used for remote connection to a contouring station. Zoom was used to establish the teleconference. Remote operators in Weill Cornell Medicine were trained for using the system. Results: The first teleradiotherapy interaction between Yerevan and New York occurred on February 7th, 2018. Demonstration of contouring on the Oncentra treatment planning system in Yerevan revealed ease of use. The brush tool displayed less drag time than the point-by-point contouring tools. Diagnostic images were easily shared without compromise of the image resolution. Conference call quality was high. This conference has opened a series of biweekly chart rounds, between the two institutions. Conclusion: Teleradiotherapy is feasible with excellent voice quality, image sharing capability and real-time contouring. The database is under construction. We are developing a new model for learning, training and collaboration in radiotherapy using WaidX, to enable rapid knowledge and technology transfer for a more equitable access to high-quality cancer care worldwide.

Annotation. One lung ventilation (OLV) is one of the most difficult intraoperative methods of respiratory support for anesthesiologists. OLV should provide the most comfortable surgical field, maintaining proper gas exchange and minimizing damage to both lungs. This anesthetic procedure has a significant inflammatory response, so using perioperative corticosteroid therapy to suppress inflammatory mediators is recommended as an approach to improving prognosis. Therefore, the aim of this study was to determine how the preoperative administration of methylprednisolone affects the systemic proinflammatory response of cytokines during thoracic surgery. The analysis was performed for 80 patients who underwent surgeries in the thoraco-abdominal department of the Shalimov National Institute of Surgery and Transplantology. Patients were divided into 2 groups (study – 40 patients who were administered methylprednisolone 10 mg / kg intravenously during induction of anesthesia and control – 40 patients without methylprednisolone). Before surgery and in the postoperative period on days 1, 3 and 5, the surface phenotype of peripheral blood lymphocytes and the expression of IL-6 by monocytes were determined by flow cytofluorometry. The author's MedStat package was used for statistical analysis (Lyakh Yu.E., Guryanov V.G., 2004–2012). Postoperative indicators of IL-6 monocyte expression in the blood of patients administered methylprednisolone were significantly lower on the 1st and 3rd postoperative day (p<0.001). Thus, preoperative administration of methylprednisolone reduces the release of pro-inflammatory cytokines and improves the condition of patients after thoracic surgery. We consider it expedient to conduct further research on the administration of methylprednisolone for several days in the postoperative period.

Abstract:
Acquired hemophilia A (AHA) is a rare autoimmune disease caused by circulating autoantibodies inhibiting coagulation factor VIII (FVIII), leading to a clinically significant bleeding diathesis. While in half of AHA cases, no underlying cause is identified, in the other half an association with autoimmune diseases, cancer, the use of certain drugs, pregnancy or the post-partum period is found. We present a case with severe bleeding in which AHA was diagnosed and subsequently a smoldering myeloma was found as underlying disease (case 16, Table 1). We conducted a systematic review of the literature in PubMed looking for the association of AHA and plasma cell neoplasm (PCN) (key words used: hemophilia, inhibitor, factor VIII, myeloma, plasma cell disorder or neoplasms, smoldering myeloma, MGUS, monoclonal gammopathy, paraprotein) and identified 15 additional cases. Description of the cases, sequence of occurrence of both diseases, their treatment, evolution and outcome are here analyzed (Table 1). We found that patients having AHA and PCN were more often males (9/16, 56%), had a median age of 61.5 (range 43-87) years at AHA diagnosis. The most common pattern of bleeding was a soft tissue bleeding. In two cases, AHA was diagnosed after excessive post-operative bleeding, in one patient following a life threatening pericardial bleeding and a hemarthrosis. AHA with active bleeding was the presenting sign in 6 cases (38%) (Table 1, cases 4, 5, 6, 10, 13, 16), in search for an underlying diseases, a PCN was diagnosed subsequently. In the other 10 cases, AHA was diagnosed after PCN. In 3 of them, the occurrence of AHA was interpreted to be secondary to multiple myeloma treatment; the implicated drugs were interferon alpha in one and lenalidomide in two cases. Information about the type of monoclonal gammopathy was available in 11 cases; no particular type of paraprotein or clonal light-chain was discernable. Factor concentrates were used in 12 cases to treat relevant bleedings. Further treatment of AHA consisted of immunosuppression with steroids (N=4), steroids in combination with cyclophosphamide (N=5), or cyclophosphamide alone (N=1). Rituximab was used in 3 cases. Plasma exchange to remove FVIII antibodies was performed in 2 cases, and immunoadsorption was done in our patient. Different therapy regimens were used to treat the underlying PCN over the years (Table 1). At the time of reporting 13 patients were alive, while 2 of 3 patients who died, died of bleeding complications. Information on outcome of AHA and/or PCN was available for 9 of 13 survivors: 6 had normal coagulation tests, while FVIII activity was mildly reduced in two. PCN was in complete and partial remission in 7 and 2 cases, respectively (table 1). Management of AHA is based on four pillars: avoidance of procedures that may have induced bleedings, control of bleeding, inhibitor eradication and treatment of the underlying disease. Our case, together with those described in the literature, emphasizes the possibility of PCN as an underlying cause of AHA. Accordingly, diagnostics including protein-electrophoresis, immune-fixation and free light chains should be considered in patients with AHA. In our experience, early intervention with immunoadsorption can be lifesaving in cases with high FVIII inhibitor titers. The occurrence of excessive unexplained bleeding in PCN should raise the suspicion of secondary AHA and trigger the investigation of coagulations tests. Whether PCN treatment alone can control AHA in these cases remains open, 11/16 (69%) of the reported cases received treatment for both diseases. That’s why we’ve only described those, which will be used. The research objective. This article describe the design of a machining module for the FESTO FMS 500 Modular Laboratory System. This module will be physically implemented as part of the revitalization and modernization of the FMS 500 laboratory system at the Department of Automation, Control and Human-Machine Interactions. The statement of basic materials. We follow the trends in industrial automation and the requirements of the 4th Industrial Revolution. This is reason why we use IO-Link technology in combination with Profinet bus for communication with sensors and actuators. Conclusions. The main objective of this article is to approach the design of the Machining Module with Industry 4.0 elements for the Modular Production System FMS 500.
