Degree Project in nursing 15 credit pionts, Malmö University; Faculty of Health and Society, Department of Care Science, 2019.Background: The palliative care is built on four cornerstones; symptom relief, multi-professional collaboration, communication and support for related parties. In palliative care issues arise that concern our existence. Why are we here on earth? Why do we have to die? What is the meaning of life? Death applies to all of us and as a nurse, it is important to see death as a natural part of our life process because it is an inevitable part of the nursing profession. Thus, every person is entitled to receive a dignified care at the end of life. Every person is entitled to receive valuable care at the end of life. Due to limited research and thus also varying knowledge from nurses, the necessary persons experiences of existential issues in palliative care are highlighted. Purpose: The purpose is to describe people's experiences of existential issues in palliative care. Method: Qualitative literature study with data gathering from the databases CINAHL, PubMed and Psycinfo. The studies were quality-scrutinized according to the SBU's review template for qualitative studies. The selected studies were analyzed on the basis of Forsberg and Wengström's eight-step model. Result: The analyzes generated five different theme headings; what is the meaning of life, thoughts of religion and spirituality, the existential suffering, a sense of hope, peace and acceptance, and + existential thoughts about death. Conclusion: Knowledge of existential issues and experiences presented in the literature study's result is required to be able to carry out an evidence-based nursing.

Full textBackground: A growing number of women are surviving breast cancer, a result of improved methods of diagnosis, general mammography and more effective treatments. The group of breast cancer survivors has been made visible mainly thanks to the campaign surrounding the Pink Ribbon. Female survivors often suffer from physical and psychosocial problems related to their breast cancer. The authors wanted to describe the existential experiences the women might have, related to their survivorship. Aim: The aim was to describe the existential experiences of women who have survived breast cancer. Method: A literature review based on eleven original articles with qualitative approach, relevant to the purpose. Databases used in the search were PubMed and ProQuest Nursing & Allied Health Source. Result: The results are presented in four different themes and two sub themes. The four themes are Perception of altered self-image, Perception of altered body, Perception of loneliness and isolation and Perception of altered view of life. From the results it emerged that many women were struggling with existential experiences as a result of a changed body and self-image as well as the loss of their way of relating to life and death. Through increased self-awareness and new insights, a new living situation related to illness and survival could be the turning point to a new and often positive view of life. Discussion: In the result discussion the life-world theory and the concept of the lived body, were used as a theoretical framework, since the purpose was to highlight existential experiences of the unique human being. The changing life-world led to a need to reconcile with a new daily life. The transition from illness to health led to existential experiences of an altered body and self-image as well as loneliness, but was followed by a new perspective of life through self-healing and meaning-making.

Full textPain in the end of life means not only the physical discomfort, but can also have psychological, social and existential dimensions. Existential pain plays a major role in palliative care. Dealing with the meaning of life, guilt and death can provide existential pain. The existential needs are as many studies indicate neglected. Health professionals often avoid these issues. The purpose of this study was to describe the existential pain in cancer patients in a palliative setting. Method: A literature review was conducted with qualitative articles. The results showed that existential pain was widespread. The informants experienced loneliness and guilt and the pain was often described in physical terms. The pain was also associated with loss of various functions. There were also thoughts about spirituality and fear of the unknown future. Discussion: It is important that the nurse is attentive to patient’s needs for closeness or lonely moments. Medical professionals can also help patients regain their autonomy by identifying the features that actually exist. Through conversation, the patient may receive strength to live the last days and to face the unknown future. The conclusion is that in order to identify and alleviate existential pain it requires a good knowledge in communication and pain management.

Full textBakgrund: När en människa är drabbad av en livshotande sjukdom, t.ex. cancer, amyotrof lateral skleros (ALS) eller hjärtsjukdom, som är i ett palliativt skede innebär det för de flesta människor en existentiell erfarenhet. Både den sjuka människan och dennes närstående tvingas att fundera på bl.a. sin relation till döden och vad som är meningen med livet.
Syfte: Syftet med litteraturstudien var att beskriva patienters existentiella upplevelser inom palliativ vård och vad som kan utlösa dessa.
Metod: Forskningsmetoden som användes var en allmän litteraturstudie. En systematisk elektronisk sökning genomfördes i databaserna Cinahl, Medline och Pubmed. Sökorden som användes var palliative care, existential, spiritual och nursing i olika kombinationer. Artiklarna har granskats och värderats enligt Forsbergs och Wengströms checklistor för kvalitativa och kvantitativa artiklar.
Resultat: Alla studier som ingår i litteraturstudiens resultat visade att existentiella upplevelser utlösta av olika faktorer var vanligt förekommande hos patienter inom palliativ vård. Existentiell ensamhet orsakades av bl.a. bristande kommunikation med närstående, ökat beroende, förlust av kontroll och bristande bemötande från vårdpersonal. Upplevelsen av skuld utlöstes av tidigare levnadssätt som kunde vara orsaken till sjukdomen och för att religiösa normer inte hade följts. Hjälplöshet upplevdes vid förlusten av förmågan att klara av vardagliga situationer. Brister i kommunikation och bemötande från vårdpersonal medförde upplevelsen av bristande autonomi och självbild liksom maktlöshet. Konflikter i familjen, rollförändringar och tankar på familjens framtid utlöste upplevelsen av oro och otrygghet. När sjukdomen försämrades var det svårt att bevara hoppet liksom att finna meningen med livet.
Konklusion: Det existentiella välbefinnande hos patienter inom palliativ vård kan förbättras om sjuksköterskor har en ökad kunskap om de existentiella upplevelserna och utlösande faktorer som redovisas i litteraturstudiens resultat.

Full textBackground: Next-of-kin to patients in palliative care often experience a difficult life situation. Life is put on trial and next-of-kin experience both physical and mental stresses of various kinds. The National Board of Health and Welfare (Socialstyrelsen) has issued a knowledge support for palliative care, which states that next-of-kin to palliative patients should be offered support in the form of conversations about emotional and existential issues. Despite this, research suggests that next-of-kin still feel that they do not get the support they need to manage difficult emotions and existential musings. Aim: To illuminate the existential experiences of next-of-kin to patients in palliative care. Method: A general literature review based on ten articles comprising qualitative data. Result: Seven patterns were identified: powerlessness – control; anxiety, stress – assurance; hopelessness – hope; loneliness – togetherness; reduced self – personal growth; meaninglessness – meaningfulness and restricted life – complete life. These patterns describe seven continua of existential experiences that next-of-kin can move along, back and forth, over time. Conclusion: Next-of-kin’s existential experiences are affected by both the patient ́s wellbeing, the next-of-kin's own resources and nursing staff support. It is experienced as important to come to acceptance by using ones own resources and with the support of health professionals, to be included in the care of the patient, to get information and knowledge, to be able to maintain everyday life, to get the opportunity to share thoughts and to be met as a unique individual with own interests and needs. Clinical significance: Relief of existential suffering is one of the nurse's most important and difficult tasks. This study contributes to the knowledge of next-of-kin’s existential experiences and discusses how nurses can respond to and support these experiences.
