In palliative care issues arise that concern our existence. Why are we here on earth? Why do we have to die? What is the meaning of life? Death applies to all of us and as a nurse, it is important to see death as a natural part of our life process because it is an inevitable part of the nursing profession. Thus, every person is entitled to receive a dignified care at the end of life. Every person is entitled to receive valuable care at the end of life. Due to limited research and thus also varying knowledge from nurses, the necessary persons experiences of existential issues in palliative care are highlighted. Purpose: The purpose is to describe people's experiences of existential issues in palliative care. Method: Qualitative literature study with data gathering from the databases CINAHL, PubMed and Psycinfo. The studies were quality-scrutinized according to the SBU's review template for qualitative studies. The selected studies were analyzed on the basis of Forsberg and Wengström's eight-step model. Result: The analyzes generated five different theme headings; what is the meaning of life, thoughts of religion and spirituality, the existential suffering, a sense of hope, peace and acceptance, and + existential thoughts about death. Conclusion: Knowledge of existential issues and experiences presented in the literature study's result is required to be able to carry out an evidence-based nursing.

Full textBackground: A growing number of women are surviving breast cancer, a result of improved methods of diagnosis, general mammography and more effective treatments. The group of breast cancer survivors has been made visible mainly thanks to the campaign surrounding the Pink Ribbon. Female survivors often suffer from physical and psychosocial problems related to their breast cancer. The authors wanted to describe the existential experiences the women might have, related to their survivorship. Aim: The aim was to describe the existential experiences of women who have survived breast cancer. Method: A literature review based on eleven original articles with qualitative approach, relevant to the purpose. Databases used in the search were PubMed and ProQuest Nursing & Allied Health Source. Result: The results are presented in four different themes and two sub themes. The four themes are Perception of altered self-image, Perception of altered body, Perception of loneliness and isolation and Perception of altered view of life. From the results it emerged that many women were struggling with existential experiences as a result of a changed body and self-image as well as the loss of their way of relating to life and death. Through increased self-awareness and new insights, a new living situation related to illness and survival could be the turning point to a new and often positive view of life. Discussion: In the result discussion the life-world theory and the concept of the lived body, were used as a theoretical framework, since the purpose was to highlight existential experiences of the unique human being. The changing life-world led to a need to reconcile with a new daily life. The transition from illness to health led to existential experiences of an altered body and self-image as well as loneliness, but was followed by a new perspective of life through self-healing and meaning-making.

Full textPain in the end of life means not only the physical discomfort, but can also have psychological, social and existential dimensions. Existential pain plays a major role in palliative care. Dealing with the meaning of life, guilt and death can provide existential pain. The existential needs are as many studies indicate neglected. Health professionals often avoid these issues. The purpose of this study was to describe the existential pain in cancer patients in a palliative setting. Method: A literature review was conducted with qualitative articles. The results showed that existential pain was widespread. The informants experienced loneliness and guilt and the pain was often described in physical terms. The pain was also associated with loss of various functions. There were also thoughts about spirituality and fear of the unknown future. Discussion: It is important that the nurse is attentive to patient’s needs for closeness or lonely moments. Medical professionals can also help patients regain their autonomy by identifying the features that actually exist. Through conversation, the patient may receive strength to live the last days and to face the unknown future. The conclusion is that in order to identify and alleviate existential pain it requires a good knowledge in communication and pain management.

Full textBakgrund: När en människa är drabbad av en livshotande sjukdom, t.ex. cancer, amyotrof lateral skleros (ALS) eller hjärtsjukdom, som är i ett palliativt skede innebär det för de flesta människor en existentiell erfarenhet. Både den sjuka människan och dennes närstående tvingas att fundera på bl.a. sin relation till döden och vad som är meningen med livet.
Syfte: Syftet med litteraturstudien var att beskriva patienters existentiella upplevelser inom palliativ vård och vad som kan utlösa dessa.
Metod: Forskningsmetoden som användes var en allmän litteraturstudie. En systematisk elektronisk sökning genomfördes i databaserna Cinahl, Medline och Pubmed. Sökorden som användes var palliative care, existential, spiritual och nursing i olika kombinationer. Artiklarna har granskats och värderats enligt Forsbergs och Wengströms checklistor för kvalitativa och kvantitativa artiklar.
Resultat: Alla studier som ingår i litteraturstudiens resultat visade att existentiella upplevelser utlösta av olika faktorer var vanligt förekommande hos patienter inom palliativ vård. Existentiell ensamhet orsakades av bl.a. bristande kommunikation med närstående, ökat beroende, förlust av kontroll och bristande bemötande från vårdpersonal. Upplevelsen av skuld utlöstes av tidigare levnadssätt som kunde vara orsaken till sjukdomen och för att religiösa normer inte hade följts. Hjälplöshet upplevdes vid förlusten av förmågan att klara av vardagliga situationer. Brister i kommunikation och bemötande från vårdpersonal medförde upplevelsen av bristande autonomi och självbild liksom maktlöshet. Konflikter i familjen, rollförändringar och tankar på familjens framtid utlöste upplevelsen av oro och otrygghet. När sjukdomen försämrades var det svårt att bevara hoppet liksom att finna meningen med livet.
Konklusion: Det existentiella välbefinnande hos patienter inom palliativ vård kan förbättras om sjuksköterskor har en ökad kunskap om de existentiella upplevelserna och utlösande faktorer som redovisas i litteraturstudiens resultat.

Full textBackground: Next-of-kin to patients in palliative care often experience a difficult life situation. Life is put on trial and next-of-kin experience both physical and mental stresses of various kinds. The National Board of Health and Welfare (Socialstyrelsen) has issued a knowledge support for palliative care, which states that next-of-kin to palliative patients should be offered support in the form of conversations about emotional and existential issues. Despite this, research suggests that next-of-kin still feel that they do not get the support they need to manage difficult emotions and existential musings. Aim: To illuminate the existential experiences of next-of-kin to patients in palliative care. Method: A general literature review based on ten articles comprising qualitative data. Result: Seven patterns were identified: powerlessness – control; anxiety, stress – assurance; hopelessness – hope; loneliness – togetherness; reduced self – personal growth; meaninglessness – meaningfulness and restricted life – complete life. These patterns describe seven continua of existential experiences that next-of-kin can move along, back and forth, over time. Conclusion: Next-of-kin’s existential experiences are affected by both the patient ́s wellbeing, the next-of-kin's own resources and nursing staff support. It is experienced as important to come to acceptance by using ones own resources and with the support of health professionals, to be included in the care of the patient, to get information and knowledge, to be able to maintain everyday life, to get the opportunity to share thoughts and to be met as a unique individual with own interests and needs. Clinical significance: Relief of existential suffering is one of the nurse's most important and difficult tasks. This study contributes to the knowledge of next-of-kin’s existential experiences and discusses how nurses can respond to and support these experiences.

Full textI Sverige avlider över 20 000 personer i cancer varje år. Då sjukdomen har övergått till palliativ vård och närheten till döden ökar kommer insikten att livstiden är begränsad. Detta kan leda till att många brottas med svåra existentiella frågor i livets slutskede. Arbetets syfte var att belysa de existentiella behoven och upplevelserna, samt hur detta hanterades hos cancersjuka i palliativt skede. Studien gjordes som en litteraturöversikt och artiklarna analyserades med innehållsanalys. I resultatet framkom tre olika kategorier; förlust och skuld; värdighet; hantering och motivation. Att leva med obotlig sjukdom medförde att framtiden blev otrygg, beroendet till andra ökade och känslan av samhörighet till resten av familjen minskade, skuldtankar kunde förekomma. Att behålla sin roll, självkänsla och sina valmöjligheter var av vikt för värdigheten. Mening med livet, samhörighet med familjen var stödjande och gav hopp. Genom omprioriteringar, att hitta nya värderingar och underlätta familjens ansvar hanteras situationen. Slutsatsen blev att de existentiella frågorna bör uppmärksammas mer inom den palliativa vården. Öppen kommunikation, lyhördhet mellan vårdpersonal och patient samt ett fungerande teamarbete är viktigt inom den pallaitiva vården

Full textBackground: Most patients who are in the final stages of life experience a threat towards their own existence and in connection with existential thoughts arise. The palliative care aims to alleviate the suffering associated with the existential thought. Caregivers find existential conversations as difficult and want to learn more in order to be a better support to patients. Therefore, we want to highlight patients existential experience to increase the knowledge and understanding of nurses in palliative care. Aim: Aim of this literature study is to describe existential experiences in patients with cancer, at a palliative stage. Method: The design is a literature review that includes 17 articles, using qualitative, quantitative and mixed methods. The data collection was done through searches in the databases CINAHL, PubMed and PsycINFO. Results: The result is presented in two main categories. The existential survival - Coping strategies and Patients experience of health services. With three subcategories each. Relationships and Community, Existence, Death and Life After. Support, Neglect and Improvement. The results show different coping strategies that patients use to manage their incurable progressive disease. Common among the patients was that close relationships was the meaning of life. Maintaining everyday life and living in the present was also considered important. For the patients, an existential crisis was initially seen in conjunction with the information about the diagnosis, which later went on to try to fill the time left with something meaningful. A central and important component for these patients is health care, continuity and person-centered care, which the patients emphasize as the most valuable. Conclusion: Patients use different coping strategies to cope with the existential feelings that arise in connection with the end of life. The strategies varied depending on the patients' individual experiences. More support for these patients is needed and healthcare professionals need knowledge to function as a better support.

Full textPalliativ vård omfattar den vård man ger till en patient som bara har månader eller veckor kvar att leva. Sjuksköterskan skulle tillgodose fysiska, sociala, existentiella samt andliga behov hos den döende patienten. Att lindra existentiell smärta som ångest, oro kunde underlätta den sista svåra tiden för patienten och dess anhöriga. Syftet var att belysa hur sjuksköterskan tolkade och hanterade existentiell smärta hos patienter i livets slutskede. En kvalitativ innehållsanalys valdes för att få ut en så bred och täckande beskrivning som möjligt. Detta gav möjlighet till en exakt beskrivning av det specifika i sjuksköterskans agerande och samla in så rika och förutsättningslösa berättelser som möjligt. Analysen resulterade i sex kategorier såsom, bekräftelse, skapa tillit, att hantera ett lidande, att uppfylla specifika önskemål, att hjälpa patienten att nå en acceptans samt reflektionstid. I resultatet framkom att sjuksköterskans närvaro kring patienten var viktig för att minska dess existentiella lidande. Att man vågade vara närvarande i de svåraste stunderna hos patienten krävde mod. Man behövde inte alltid göra något utan att bara finnas där. I diskussionen kom det fram att det var viktigt att som sjuksköterska ha en god självkännedom och livserfarenhet samt att veta var man själv stod i sina existentiella frågor och sina egna tankar om döden för att kunna närma sig patienten. Slutsatsen blev att för att kunna tolka och hantera ett extistentiellt lidande hos en patient förutsattes det djupare kunskaper i ämnet samt en personlig styrka i att våga vara närvarande i de svåraste stunderna.
Palliative care involves the care you give to a patient who has only months or weeks left to live. Nurse would respond to physical, social, existential and spiritual needs of the dying patient. To alleviate the existential pain of anxiety, concern was eased on the last difficult time for the patient and their relatives. The aim was to highlight how the nurse recognized and dealt with existential pain in patients end-of-life. A qualitative content analysis was chosen to get as broad and wide as possible. This gave the opportunity for a precise description of the specific nature of the nurse's actions and collect as rich and ended stories as possible. The analysis resulted in six categories such as: confirmation, create confidence, to deal with suffering, to meet specific requests, to help the patient reach an acceptance and reflection. The result showed that the nurse's presence around the patient was important in order to reduce its existential suffering. That dared to be present in the most difficult moments in the patient requiring courage. You need not always do anything but just to be there. The discussion has highlighted that it was important that as nurses have a good self-knowledge and life experience and to know where you yourself were in their existential issues and their own thoughts about death in order to move closer to the patient. It was concluded that to interpret and manage an existential suffering in a patient was anticipated that deeper knowledge of the subject and a personal strength in daring to be present in the most difficult moments.

Full textAbstract Palliative care is characterized by a foundation of values with the catchwords empathy, holism and knowledge. The purpose of the values is to promote the individual's quality of life, alleviate suffering and offer an existential support in the end of life. The foundation for a good person-centered care is based on the relationship between caregiver, patient and next of kin are being promoted and being listened to and seen as a person. Existential pain is common in patients in palliative phase such as loneliness, isolation, meaninglessness, separation and death anxiety. The patient gives the expression of needs to be met with respect, empathy and not be abandoned by his caregiver. Several studies have demonstrated the gaps where the health care system can't define and understand the existential needs. The aim of the study was to describe the nurses' perspectives on working with existential pain in patients in the palliative phase. The method chosen for the study was a literature review to examine the state of knowledge in the field. The databases CINAHL complete, PubMed, PsycINFO and SweMed+ were systematically searched with selected keywords, which resulted in 16 articles being identified which corresponded to the study's aim based on the chosen selection criteria. The result showed that nurses saw a responsibility to create a good relationship and communication with the patient and next of kin, in the work with existential pain in patients in the palliative phase. Important components for a deeper relationship were time, timing, attendance and shown empathy. Responsiveness of the nurse was seen as important to be able to interpret and alleviate the patient's existential pain. Many nurses saw the work as meaningful in the situations which they felt that they alleviated the patient's existential pain. The work involved emotional involvement and in some situations where the balance between distance and presence was difficult to identify. The work also opened up for reflection on life and death. Some nurses described the work with existential pain of the patient as too laborious and did not see it as their duty to sit down and talk about death. Some nurses described that they missing tools to be able to act and handle in different situations with existential pain in the patient. Conclusion in this study, the nurses' meetings with patients with existential pain in palliative stage is different as each person is unique with different needs and resources. The work of existential pain in patients involved the nurse emotionally and the balance between the presence and distance were difficult to identify. More education, discussion and training in addressing existential pain are needed to strengthen nurses in their work with the patient in palliative phase.

Full textABSTRACT Background: To suffer from cancer often means a dramatic experience that affects the physical, psychological, social and spiritual/existential dimensions. The purpose of this study was to examine experiences of medical yoga as a part of the rehabilitation for people who suffer from cancer. The purpose was also to describe what perception the people had concerning their psychological and spiritual/existential health, and the eventual significance of the medical yoga related to that. Method: Explorative qualitative method with individual interviews where eight informants participated. The recruitment emanated from a cancer rehabilitation center at the Academic hospital, Uppsala county council. The rehabilitation form was focused on medical yoga. Result: The psychological health was experienced as relatively good but it had varied over time. Therapeutic talks were important regarding the psychological and spiritual/existential health. Regarding the spiritual and existential issues many of the informants thought about their disease, death and the meaning of life. The medical yoga brought positive experiences. All of the informants felt and received a feeling of calmness and relaxation both during and after the yoga sessions. The group training lead to social interaction which was meaningful to several of the informants. Spiritual and existential experiences in connection with the yoga sessions arose. Conclusion: A cancer disease often affects the psychological and spiritual/existential health. The medical yoga was a meaningful part of the cancer rehabilitation and affected their physical, psychological, social and spiritual/existential dimensions. With the informants experiences in mind the medical yoga could be able as an important contribution to the health care and contribute to a better health among persons who suffers from cancer.

Full textThe existential suffering is found in relation to questions about human freedom, meaning of existence, feelings of loneliness and isolation in life and encountering death. These questions are of a universal human character and are therefore to appear irrespective of human confession to or dissociation from spirituality or religiosity. The aim of this review was partly to describe the existential suffering in cancer patients in curative care, partly to describe nurses´ treatments to these patients and the encounters that arise between patients and nurses. The findings emerging in the study revealed that the existential suffering is characterized by changes and is a great part of the total suffering that cancer patients endure. At the same time are nurses, as a result of fear and ignorance, in many cases unable to encounter the patients´ experiences and needs. The theoretical framework used, consist of selected parts of Katie Eriksson´s nursing theory.
