With the advent of major reform in health care delivery systems, team-based approaches to primary care are becoming standard, inviting expansion of the role of the psychiatrist as part of those teams. Teams of interdisciplinary professionals expand access to primary care and the connection to specialty care for those who need the most support. This chapter describes the typical roles in integrated primary care teams, including those filled by a psychiatrist. The authors discuss the differences between traditional mental health and primary care cultures and adapting to the needs of primary care through transdisciplinary collaboration. An overview of the potential levels of collaboration is provided. The authors describe their own long-term experience with integrated primary care in the Department of Family Medicine at the University of Rochester. The chapter concludes with a discussion about the future of psychiatric care within primary care teams as well as population management for those with comorbid illness.

This chapter focuses on the Congressional hearings conducted in 1982 and 1984–1985 to address the issue of franchise relocation. It first considers the so-called community protection acts that were introduced during the early 1980s in the wake of franchise relocations in the National Football League (NFL) and the lingering ill-will triggered by the expansion Washington Senators' move to Texas. It then looks at the legal wrangling between the NFL and Al Davis over his relocation of the Oakland Raiders to Los Angeles, along with legislators' push for franchise expansion and their doubts about revenue sharing in the NFL and Major League Baseball (MLB). It also examines the United States Football League's (USFL) antitrust suit against the NFL accusing it of being an illegal monopoly and using predatory tactics to thwart the USFL.

This chapter examines the issue of franchise relocation. Legislators had two main concerns throughout the series of hearings: to procure teams for their constituents while avoiding losing teams via relocation. The legislators' concerns were imbued with an element of reality, at least. Cities with multiple Major League Baseball (MLB) teams usually had one team that was struggling, and legislators held a different attitude to such teams relocating than they would with regard to later relocations of prosperous teams. This chapter first considers three options for acquiring a big-league team: purchase an existing team, hope for an expansion team in an established league, or enter a team into a new league. It then discusses the economics of franchise relocations, along with the early histories of franchise turnovers in professional sports leagues, including the National Football League (NFL) and its predecessor, the American Professional Football Association. It also looks at Columbia Broadcasting System's (CBS) purchase of the New York Yankees during the 1964 season that sparked fears of an unfair alliance.

This chapter focuses on the Congressional hearings conducted in 1959 and 1960 to address the issue of expansion and prospective new leagues. All of the league constitutions contained clauses pertaining to relocating or selling existing franchises and creating new franchises. These clauses typically required a supermajority, in some cases unanimity, of owners to approve franchise relocation or sales and expansion. However, incumbent owners were lukewarm about franchise expansion and hostile towards new leagues. This chapter begins with a discussion of tactics employed by team owners to prevent the entry of a new league, including territorial rights. It then considers the demise of the Continental League due to a number of hurdles, such as getting television contracts, providing pension benefits equivalent to Major League Baseball's (MLB) scheme, and and getting stadiums. It also examines the expansion policies of the National Basketball Association (NBA) and the National Football League (NFL), along with the legal wrangling between the NFL and the American Football League (AFL) over expansion.

This chapter examines the economics of antitrust, with particular emphasis on how antitrust law affects professional team sports. In the late 1800s, Americans worried about the growing concentration of power in the hands of a few producers such as Standard Oil, American Tobacco, and other large firms that consolidated their holds over industries by merging and acquiring other companies. Other industrial leaders sought to fix prices above those obtained under competition. The Sherman Antitrust Act, enacted in 1890, contains provisions addressing “contract,” “conspiracy,” and “trade and commerce.” This chapter first considers how courts applied the Sherman Act to cases involving professional team sports before discussing the characteristics of professional sports leagues, how owners of professional sports teams reported profits and losses, the issue of player salaries and exploitation, and competitive balance and revenue sharing in professional leagues. It also describes franchise relocation and expansion and how television created demand in sports.

We review major findings relating the organizational context to team creativity, using Amabile and colleagues’ (1996) framework for assessing the work environment for creativity as a guide. We find that research addressing the organizational context remains relatively concentrated in a few areas, particularly resources and work group support, and that other areas such as autonomy and certain organizational impediments have received only sparse attention. We offer researchers both a primer on the more and less studied areas of organizational context for team creativity and suggest some expansion of the work environment framework. Specifically, we suggest the addition of social networks, based on current trends in the organizational creativity literature.

Medicaid’s intergovernmental design with a generous federal matching rate and substantial state discretion has fostered gradual but steady expansions of the program over time. Gradual growth creates a favorable political environment in which each expansion creates a political constituency for Medicaid—among provider groups and enrollees—who then fight against retrenchment. Yet, as program expenditures continue to increase, especially during fiscally distressed times, the partisan divide over the future direction of Medicaid becomes more stark. These are the political dynamics that will define how states struggle with the Affordable Care Act (ACA) Medicaid expansion and also how states attempt to provide long-term care financing and services under their Medicaid programs.

The case-study chapters looked at all presidential powers within a given constitution. This looks at one presidential power—term limits—across all constitutions to provide one final test for the book’s power asymmetry argument. I conduct a statistical test to show how power asymmetry, this time measured by approval rates, lead to presidential success in relaxing term limits. Across Latin America since the 1980s, term limits became one of the most active and contentious areas of constitutional change. The chapter reveals, first, that not all popular presidents pursue efforts to change term limits, but those who do have a higher chance of prevailing the more popular they are. Second, the only political actors capable of preventing the expansion of term limits, other than the courts, is the ruling party. Popular presidents interested in expanding presidential power thus face incentives to weaken the same political machine that brought them to office.

This conclusion discusses the aftermath of the Congressional hearings. During the hearings, the owners' general prerogatives survived essentially intact, although free agency of some sort was imminent in all sports by 1976. Legislators did not repudiate the reserve clause, the reverse-order draft, or territorial rights, despite their qualms regarding these institutions. The legislators and their aides missed some opportunities to subject the team financial data from the 1950s to analysis, which could have shed light on such questions as the effects of revenue sharing. Some fans gained when their hometown landed an expansion or existing franchise, while other fans lost when legislators did not prevent franchise relocation. Congress has held several hearings in the intervening decades since 1989. The professional sports leagues have also evolved. Technology has altered the landscape.

This chapter documents the league's continued efforts at expansion. Although some of the eight franchises were still struggling to earn sufficient revenues, the NBA's stability and the success of its twenty-four-second shot clock encouraged NBA owners. Some began considering relocating their teams to larger cities. Their improved product on the court and growing prosperity spurred other businesspeople to begin seeking teams of their own, leading to increases in franchise values and demands for expansion teams. Moreover, new players from this generation were elevated to nigh-legendary status among pro basketball fans. The league's general stability and incipient prosperity also bolstered player salaries and emboldened players to seek more rights.

This book tells the story of the Mt Stromlo Observatory in Canberra which began with W.G. Duffield's idealism and vision in 1905. The Observatory began life as a government department, later becoming an optical munitions factory producing gun sights and telescopes during the Second World War, before changing its focus to astrophysics – the new astronomy. 
 
 In the ensuing years programs were introduced to push the Observatory in new directions at the international frontiers of astronomy. The world wars were important ‘pacemakers’ of welfare state development in Germany—first and foremost via special wartime or post-war benefit regimes. Veterans’ pensions and reinsertion after World War I and compensation of various war victims after World War II massively increased social spending for decades. Whenever war did have a significant impact on the core welfare state programmes (i.e. the big social insurance schemes), it was through indirect and long-term rather than direct, short-term dynamics. Labour mobilization via the involvement of trade unions and the significant expansion of wartime social assistance and social services during World War I, for example, paved the way for the expansion of the welfare state in the Weimar Republic (such as unemployment insurance in 1927). Social policy during World War II targeted benefits towards soldiers’ families and ethnic German victims, but it was far from the ‘dictatorship of favours’ Götz Aly describes.

This chapter introduces the edited volume,The Oxford Handbook of Philosophy and Psychiatry. Published in 2013, the centenary of Karl Jaspers'General Psychopathology, the chapter draws lessons from the last hundred years for the coming century. No predictions are made. Instead, five 'conditions for flourishing' are set out: 1) Particular Problems - the importance of focussing on well-defined particular problems rather than general theory building, 2) Product- orientation - remaining always responsibly product oriented in the specific sense that both sides (philosophers and practitioners) put in the work necessary to 'go deep' with each other's fields, 3) Partnership - working in partnerships of one kind or another (ranging from team work through to doubly qualified researchers), 4) Process - constant reflection on process based on peer review but leaving scope for the occasional rogue voice to cut innovatively against the grain, and 5) Q - a condition of a different kind, Q is an empirically derived measure of the balance between in-group cohesion and out-group openness required to support creativity. Illustrations are given of how these five conditions for flourishing have underpinned the rapid expansion of philosophy and psychiatry in the closing decades of the twentieth century, and, correspondingly, are also reflected in the Handbook as a whole. Overviews and commentaries on individual contributions to the Handbook are given in extended editorial introductions to each of its eight sections.

This chapter is a general overview of the economic aspects of professional team sports leagues as well as the American economy. The NBA's turbulent birth as the BAA demonstrated that professional sports team owners' twin advantages of price-setting power over ticket prices and enhanced bargaining power over players were not sufficient conditions to ensure profitability. Price-setting power without sufficient demand could still lead to losses. The challenge was to increase demand, which would have led to higher ticket prices, more attendance, and greater revenues and profits. Greater profits would have enabled owners to pay higher salaries and to improve conditions, helping to erase any fly-by-night image. Thus the chapter looks at the issues surrounding profits, player salaries, technology, expansions, discrimination, and so on; as well as how the American economy performed during the years 1945–61 and how it affected attendance and demand for professional team sports leagues.

Central banks have increasingly used communication to guide market actors’ expectations of future rates of interest, inflation, and growth. However, aware of the pitfalls of (financial) central planning, central bankers until recently drew a line by restricting their monetary policy interventions to short-term interest rates. Longer-term rates, they argued, reflected decentralized knowledge and should be determined by market forces. By embracing forward guidance and quantitative easing (QE) to target long-term rates, central banks have crossed that line. While consistent with the post-1980s expansion of the temporal reach of monetary policy further into the future, these unconventional policies nevertheless mark a structural break—the return of hydraulic macroeconomic state agency, refashioned for a financialized economy. This chapter analyses the theoretical and practical reasoning behind this shift in the governability paradigm and examines the epistemic and reputational costs of modern central bank planning and the non-market setting of long-term bond prices.

In the past, states with access to cheap and abundant sources of energy were able to develop radical new technology that paired core innovations with new fuels. Given the strong relationship between global warming and fossil fuels, in this chapter we ask whether the emergence of unconventional fuels and extraction methods are likely to make a significant difference for the relative international standing of the United States now and China perhaps later. Should the expansion of less expensive but older sources of energy, thanks to fracking technologies, help states maintain or regain systemic leadership? Our answer is that fracking will be advantageous in the short term but much less so when it comes to longer-term considerations such as global warming and systemic leadership. More likely, this innovation will only protract the transition away from fossil fuels.

Chapter 7 introduces commercial banks as creators of money and integrates them into the general equilibrium framework. The motivation to deviate from the standard approach that neglects commercial banks and entrusts all money creation to a central bank is twofold. First, apart from currency, central banks do not provide money directly but rather supply reserves that enable banks to create deposits. After the Great Recession, this transmission process staggered: increases in reserves outpaced increases in deposits. Any analysis of the monetary expansions starting in 2008 would remain incomplete and unsatisfactory unless it took account of this fact. Second, central banks normally control an overnight interbank interest rate that differs from the market interest rate on bonds. Considering an interbank market and its relationship with the bond market makes it possible to derive a term structure of interest rates. This is important because inverse term structures are good predictors for recessions.

This chapter details the series of political events from the proclamation of Ferdinand Marcos as the sixth president of the Philippines to his usurpation of power in 1972. Marcos' dream of a march to greatness started well. Expansive public works projects as well as intensified tax collection led to a measure of prosperity in the 1970s. He was reelected in 1969, becoming the first president to win a second term. However, during this second term, militancy also increased among student groups. On July 7, 1972, the constitutional convention voted to change the form of government from presidential rule to a parliamentary system. This allowed Marcos, whose last and second term would have ended in December 1973, to seek a third term. On September 22, Marcos signed the martial law decree, which was dated September 21. As justification, he claimed that an “armed insurrection and rebellion” was being waged against the government by “lawless elements” and “terroristic organizations.”5He would wield absolute power for the next fourteen years and eight months.
