Faced with the dual pandemics of obesity and type 2 diabetes mellitus, heath care providers require a broad array of treatment options. Diet, exercise, and medications remain the cornerstones of type 2 diabetes therapy, but long-term results with lifestyle modifications can be disappointing, and, despite an ever-increasing armamentarium of pharmacotherapeutics, adequate glycaemic control often remains elusive. Moreover, most diabetes medications promote weight gain, and using them to achieve tight glycaemic control introduces a proportionate risk of hypoglycaemia. In cases where behavioural/pharmacological strategies prove insufficient, gastrointestinal surgery offers powerful alternatives for obesity and type 2 diabetes treatment (Fig. 13.4.5.1). Among severely obese patients, bariatric operations cause profound, sustained weight loss, ameliorating obesity-related comorbidities and reducing long-term mortality (1–4). Operations involving intestinal bypasses exert particularly dramatic antidiabetes effects. For example, approximately 84% of obese patients with type 2 diabetes experience diabetes remission after a Roux-en-Y gastric bypass (RYGB), maintaining euglycaemia off diabetes medications for at least 14 years (1, 5–8). Mounting evidence indicates that these effects result not only from weight loss, but also from weight-independent antidiabetic mechanisms (9). Whereas diabetes is traditionally viewed as a relentless disease in which delay of end-organ complications is the major treatment goal, gastrointestinal surgery offers a novel endpoint: complete disease remission. Consequently, conventional bariatric procedures and experimental gastrointestinal manipulations are being used worldwide to treat type 2 diabetes in association with obesity, and, increasingly, among less obese or merely overweight patients (8). Gastrointestinal surgery also offers valuable research opportunities to improve knowledge of diabetes pathogenesis and help develop less invasive procedures and novel pharmaceuticals. This chapter discusses the effects of gastrointestinal operations on type 2 diabetes, and focuses on potential antidiabetic mechanisms that mediate those effects.

Actualmente existen tres grupos, conformados por dos sindicatos, y los empleados no sindicalizados; dichos grupos coinciden en esperar cambios positivos en la Institución. Por otra parte, la lucha de poderes entre la Autoridad y el Sindicato Principal, los diferentes grupos de choque que existen, el nacimiento de un nuevo sindicato bajo el ala de la autoridad, hace que se haga más tensa la vida laboral, surgen entonces, las siguientes preguntas: ¿Cómo influyen los sindicatos en el clima organizacional de la Institución?, ¿qué tanto poder e influencia pueden experimentar las autoridades para lograr desequilibrar al personal?. Los cuestionamientos anteriores, nos inducen a pensar que la dinámica organizacional de la Universidad son un reflejo de la micropolítica, entendiéndose como una variable que refleja cómo es que los sujetos integrantes de la organización utilizan estrategias para influir sobre los otros miembros de la organización a través del ejercicio del poder que se refleja en la vida de la Institución, y que trasciende los espacios institucionales formales generando conflictos, propiciando o dificultando las negociaciones generando grupos de interés, etc. Desde un punto de vista metodológico la investigación es de tipo documental-exploratoria-descriptiva, se aplicó una encuesta, cuyo alfa de cronbach fue de 0.941, que indica que el instrumento diseñado es confiable. Como una alternativa para su resolución, el Estado colombiano ha implementado los Mecanismos Alternativos de Solución de Controversias, considerados como una forma de justicia complementaria de la estatal, cuya implementación persigue dos importantes finalidades: la participación de la sociedad en la solución de sus conflictos y la descongestión del aparato jurisdiccional.Dichas herramientas jurídicas han sido concebidos, además, como una forma de acabar con la judicialización del conflicto y, en consecuencia, de restaurar los tejidos sociales, lo que se logra cuando las partes involucradas, de manera voluntaria, deciden poner fin a su controversia haciendo uso de su autonomía privada, logrando así el restablecimiento de las relaciones que se han deteriorado y la consolidación de relaciones futuras entre los miembros de la sociedad. De allí la importancia de conocer el contexto donde surge la justicia informal, los mecanismos alternativos con los que cuenta el País, y el fundamento constitucional y legal que los soportan.

This final chapter introduces a few tissue engineering systems that either have been used clinically or are rapidly approaching clinical use for cartilage, skin, and nerve repair. Some of the elements of each tissue engineering system are presented, together with references to the primary literature. The first example involves engineering of a tissue with an important mechanical function: cartilage. The general approach described for cartilage has been attempted in other structural tissues such as bone, tendon, and skin; there are important common elements among these tissue engineering systems. One common underlying theme, which is emphasized in the first example, is the use of a material as a scaffold for the ingrowth, proliferation, and function of cells, which differentiate into mature functional cells. After being introduced to this first example and provided with an outline of some experimental results, the reader is then presented with a series of questions: some are intended to stimulate discussion in the classroom, while others can be used as homework assignments or as the basis for independent projects. The main objective of these exercises is to assimilate information from previous chapters and apply it in a situation with clinical application: cartilage tissue engineering. The second and third examples involve tissue-engineered skin and repair of nerves lost to disease or trauma. No questions are provided with these examples, but the reader is encouraged to identify the connections to material in the preceding chapters. One of the first tissue engineering products available to patients is Carticel1®, which uses autologous cultured chondrocytes for the repair of cartilage defects in the femoral condyle (spirally curved prominences on the end of the femur that is involved in the knee joint). The procedure has several phases: first, a pea-sized biopsy of healthy knee cartilage is obtained. This tissue is expanded in vitro to obtain a large number of cultured cells that are all derived from the patient. In a second procedure, a surgeon prepares the transplant site by removing diseased tissue and preparing the defect (in the clinical trials of this product, the defect had a cross-sectional area of &lt;1 to 20 cm2, with 90% of the defects &lt;10 cm2).
