The classic Condorcet Jury Theorem comes with demanding assumptions. This chapter shows that similar results can be derived if the assumptions are weakened. First, if the Competence Assumption is weakened by allowing for heterogeneous voter competence, the Asymptotic Result of the jury theorem still obtains (though the Non-asymptotic Result does only under very specific assumptions). Second, the number of alternatives can be more than two for a structurally similar jury theorem, using plurality voting. Third, different decision procedures, such as the Borda count or the Condorcet pairwise criterion, still lead to the Asymptotic Result. While the Borda count and the Condorcet pairwise criterion have a slight epistemic advantage over plurality voting, for large competent groups this difference is negligible.

A spinal epidural abscess is an infection that resides in the epidural space of the spinal canal, and most commonly occurs from hematogenous seeding or direct extension from adjacent structures. Normal skin flora such as Staphylococcus and Streptococcus spp are the most common organisms to cause an epidural abscess, typically when host immunity is compromised or due to barrier disruption. The clinical presentation is heterogeneous, but often will progress over time to spinal cord compression. Intracranial epidural abscess and subdural empyema occur within the skull and are frequently spread via direct extension of infections from contiguous structures or as complications from neurosurgical procedures. Prompt diagnosis and treatment is essential for improving morbidity and mortality.

The sacroiliac joint (SIJ) is often overlooked as a cause of pain, partially because it is not well visualized on standard imaging and partially because other structures may refer pain to it. This chapter reviews the anatomy of the SIJ as well as the diagnosis and differential diagnosis of SI joint dysfunction and pain, including a multitude of physical exam maneuvers such as the FABER, Gaenslen, extension, Gillet’s, sacroiliac shear, thigh thrust, compression, and distraction tests. This chapter briefly reviews the essential elements needed to develop a safe policy for sedation by nonanesthesia practitioners.

Fetuses with congenital head and neck anomalies which will result in impaired spontaneous respiration and difficulty in securing the airway after delivery may suffer asphyxia at birth. The ex utero intrapartum therapy (EXIT) offers such affected babies a chance at survival by allowing airway manipulations to occur after partial delivery of the baby, while the baby remains on utero-placental support. This very unique procedure requires extensive planning and multidisciplinary coordination with a multiplicity of specialties including maternal fetal medicine, pediatric surgery, pediatric otolaryngology, pediatric anesthesiology, neonatology, and pediatric cardiology. The preoperative planning takes place several days prior to the procedure and also involves meeting with the family. It is the intricacies of multidisciplinary team that require EXIT procedures to take place in certain highly specialized tertiary care fetal centers.

This chapter introduces the concepts of natural orifice transluminal endoscopic surgery (NOTES) and single-incision laparoscopic surgery (SILS). The field of NOTES has evolved over the past decade, and this developmental framework is also outlined to help better understand the current state of the field. NOTES describes a minimally invasive approach to surgical diseases in which instruments are passed transluminally to achieve access to the desired body. SILS is a minimally invasive approach carried out as an extension of traditional laparoscopic surgery. The anesthetic implications of NOTES and SILS are explained, including potential complications that are unique to each. Finally, future directions in developmental endoscopy are discussed to give a sense of what types of procedures may become available or commonplace in the coming decade.

The procedures for amending the Convention, for amending Protocols and for negotiating and adopting new Protocols are examined. The extension in scope agreed at the First Review Conference is discussed. The requirement for consensus as a condition precedent to the adoption of new Protocols and the increasing difficulty in securing such consensus as to new Protocols as the number of States party to the Convention increases are both considered.

Abstract:
The increasing utilization of radiation for diagnostic and therapeutic procedures has provided impetus for improved strategies of radiation protection for interventionalists. The associated discomfort, disability, and career-shortening effects of lead aprons for heavy fluoroscopy users have served as an impetus for the development of lighter and more comfortable models. A suspended radiation protection system employs the use of a “weightless” shield resembling a thick large lead apron with head shield and arm shields. The shield moves with the operator like a garment, providing extensive protection without orthopedic strain or discomfort while maintaining full user functionality. Utilization of a suspended radiation protection system provides the operator with optimum radiation protection, without any additional weight, and maintains procedural flexibility.
