Abstract:
Introduction. Osteomas are a slow growing benign neoplasm of unknown etiology very rarely involving the temporal bone. They develop in the external auditory canal on squamous sections, in the mastoid, middle and inner ear. By bone composition they are divided into spongious (osteoma spongiosum) and compact osteomas (osteoma eburnum); by growth direction, into outward-growing (exosteoma) and in inwardgrowing (endosteoma); into unilateral and bilateral; by size, into small and gigantic; by surface structure, into smooth and multilobular; by number, into solitary and multiple; into symmetrical and asymmetrical. The symptoms of intracanalicular osteomas are the result of auditory canal obstruction. Diagnosis is made based on case history, clinical examination, audiological processing and radiography (temporal bone CT scan), and confirmed by histopathological examination of the bone. In terms of differential diagnosis, they must be distinguished from exostoses, bone tissue proliferation and osteoid osteomata. The progress of the disease is prolonged, as they are slow growing, asymptomatic and benign tumours. Therapy is surgical only. Case Outline. The report presents the case of a 70-year old patient with the osteoma of the right external auditory canal. In our patient, the osteoma arose in the auditory canal, the most frequent localization; it was unilateral, solitary, multilobular and compact. It was discovered accidentally, during otoscopic examination. The clinical diagnosis was confirmed by CT scan of the temporal bone. We applied surgical therapy by retroauricular approach. The removed bone change was about 12 mm high, 13-14 mm deep and about 8 mm wide. Histopathological findings confirmed osteoma. Conclusion. Due to their slow growth, the rate of auditory canal osteomas develop asymptomatically for a long time without the characteristic clinical features. In most cases, they are discovered accidentally during otoscopic or radiographic examination. The method of choice in diagnosis is temporal bone CT scan. Therapy is surgical.

18F fluorodeoxyglucose (FDG) is widely used for PET CT examinations; however, positron-emitting florin generates relatively high gamma radiation (511 keV) raising occupational as well as public safety concerns. This study aimed to measure the rate of radiation emitted from patients that underwent 18FDG PET/CT examination for oncological conditions, approximately 2 hours after the procedure, before and after urination. A total of 100 patients who underwent 18F-FDG PET/CT examination were included in this study. Following imaging, external radiation exposure rate was measured using proportional counter probe at 1-m distance, approximately 2 hours after the completion of imaging procedure, before and after urination. Factors effecting resulting exposure from patients were examined. The mean post-urination activity ranged between 0.2 and 6.3 μSv/h (median, 1.8 μSv/h). Presence of metastasis, tumor type and gender did not have any effect on mean post-urination activity (P>0.05 for all comparisons). Older age, greater BMI and higher administered dose were associated with higher post-urination activity (P < 0.05 for all comparisons). Findings of this study showed that 2 hours after radionuclide injection, activity rate from patients is far below the recommended limits for general population and further decreases after urination. Discharging patients at 2 hours after urination would not seem to pose radiation health risk for relatives, public or other hospital staff.

ABSTRACT This study investigates whether assured disclosures of management's remediation of material weaknesses in internal controls affect positively unsophisticated investor perceptions of disclosure credibility and the likelihood of their investing in a firm. The results indicate that investors perceive assured material weakness remediation disclosures, whether the audit source is external or internal to the firm, to be more credible than unassured disclosures. Specifically, external assurance is seen to be more credible than the assurance provided by internal auditors but that is seen as more credible than no assurance. However, investment likelihood remains the same regardless of assurance source. Furthermore, the results indicate that investor disclosure credibility perceptions and investing likelihood are lower for internally assured pervasive material weakness remediation disclosures than internally assured account-specific remediations and all externally assured remediation disclosures. Finally, mediation results suggest that both internal and external auditor assurance increases investing likelihood indirectly through increased disclosure credibility.

Abstract:
The aim of this study was to report the outcome of the use of an external fixator to treat a pelvic canal stenosis in a 5-month-old female cat. The cat was referred with a history of 3 weeks of intermittent signs of constipation refractory to the medical management, occurring after surgical treatment for a bilateral sacroiliac luxation and sacral fracture. The clinical examination revealed instability of the pelvis and a radiograph showed a pelvic canal stenosis and megacolon. External fixator was the method of choice to be used in this case. The manually applied tension on an external fixator resulted in a widening of the pelvic canal. At 45 days after surgery, there were no signs of constipation, and the radiological examination showed progressive bone healing. At 18 months post-op, the cat had no abnormalities both on the clinical examination and on the radiography. In conclusion, the use of an external fixator led to the widening of the pelvic canal using a minimally invasive procedure. To the authors’ knowledge, this case report represents the first surgical description and clinical outcome of the widening of the pelvic canal in cats using an external skeletal fixator.

We argue that external cues provide affective information that influences processing strategy and, therefore, examination performance. Participants completed 2 supposedly different forms of a midterm examination; in fact, randomly assigned participants completed identical midterm examinations printed on either red or blue paper. Blue paper led to superior performance, especially for difficult questions. We note a method for appropriately adjusting scores to control for form effects while maintaining percentile rankings within and across forms. We discuss implications for test administration, examination form effects, and adjustment of scores.

Attunement to visual information has been suggested to mediate the performance advantage associated with adopting an external focus of attention (e.g., Al-Abood, Bennett, Moreno Hernandez, Ashford, & Davids, 2002; Magill, 1998). We tested this hypothesis by examining the extent to which online visual information underpins the external focus advantage. The study examined skilled golfers on a putting task under one of three attentional focus conditions: control (no instructions), irrelevant (tone counting), and external (movement effect focus), with either full or occluded vision. In addition to task performance, the effect of attentional focus and vision on between-trial movement variability was examined. We found a significant advantage for an external focus of attention in the absence of vision. The results of the movement variability analysis further indicated that external focus was not mediated by the online use of vision. We discuss these findings in the context of traditional cognitive perspectives to external focus effects.

The aim of this small-scale grounded approach qualitative study was to examine the beliefs of selected external examination invigilators (EEIs) and infer the kinds of activities they consider important. The importance of this study rested in the fact that there is a paucity of research which examines the role of EEIs at secondary, further and higher education levels. Therefore, it aids in filling a literary gap and gives them a ‘voice’ in the research literature. Study participants were five EEIs, working in a London secondary school. Purposeful convenience or opportunity sampling was used in their selection. Informal interviews and participant observation were the research methods used. The findings revealed the fact that beliefs of the EEIs guide how they rated their role and that maintaining examination intangibles and procedures is of high importance. Implication of the findings are discussed.

Abstract:
Carcinoma of the external meatus is a rare disease and a challenge to treat. Demographic and clinical characteristics of this disease are discussed for 14 patients treated in the Institute of Otorhinolaryngonlogy and Maxillofacial Surgery during the period 1993-2003. using four different clasiffication of the disease. All patients were treated surgically with postoperative radiotherapy. Five years survival is 100% for the II and II stage of the disease for all clasifications, and 75% for the III stage according to Pitsburg clasiffication. Reccurence of the disease in the first year after treatment is 66,6% and, 84% in the second year. The diagnostic protocol is clinical examination, computed tomography of the tempo ral bone, nuclear magnetic resonance of the temporal bone and endocranium, ultrasound examination of the neck and parotid region and tumor biopsy. The surgical approach could be less radical, through mastoid, with postoperative radiotherapy.

This study examined the concurrent validity of the teacher-made tests taken by 1794 Grade 12 students using raw scores on an external examination conducted by the Kenya National Examinations Council. Although internal examination scores were significantly correlated with scores on the standardized achievement test, the Kenya Certificate of Education, there were significant mean differences among schools. The data provide evidence for the validity of the internal examination as a measure of the same abilities as the Kenya Certificate of Education examination. Implications of this concurrent validity with respect to the quality of the school and the objectivity of the teachers' evaluation were discussed.

A 60-year-old female was found on a physical examination to have bilateral palpable pulsatile neck masses. She denied local pain, cranial nerve compressive symptoms, or symptoms of cerebral ischemia. Duplex ultrasonography demonstrated bilateral 1.5 x 2.2 cm external carotid artery aneurysms. Isolated bilateral external carotid artery aneurysms were confirmed by computed tomography and angiography. The patient has been treated conservatively, and at 4-year follow-up, she remains asymptomatic, and the carotid artery aneurysms are unchanged in size.

Primary and secondary sexual characteristics in northern pike, Esox lucius Linneaus, and muskellunge, Esox masquinongy Mitchill, differ markedly. Differences in the internal urogenital anatomy include the structure of the genital ducts and the angle they form with the ventral surface. Externally, the genital and urinary ducts open directly to the environment in northern pike, whereas in muskellunge they open into a muscular papilla and then through a urogenital pore. Differences in sexual dimorphism between the two closely related esocids constitute a structural basis to explain the known differences in spawning behaviour and reproductive success. Differences in secondary sexual characteristics explain why the methods developed by Demchenko or Casselman to externally sex pike are inapplicable to muskellunge. To develop and test two external sexing techniques for muskellunge, specimens were collected from two regions in Canada from 1980 to 1987. Examination of reproductive products obtained from stripping provided an accurate indicator of sex and sexual maturity, but demonstrated serious limitations. A simple method to sex muskellunge by examination of the external urogenital morphology is described for juveniles and adults. Dissection of fish revealed that with the urogenital examination technique we sexed juveniles with a success rate of 92–98%, and adults with 100% accuracy.

ABSTRACT Background Anal sphincter tone is routinely assessed by digital rectal examination in patients with fecal incontinence, although its accuracy in detecting sphincter defects or separating competent from incompetent muscles has not been established. Objective In this setting, we aimed to evaluate the accuracy of digital rectal examination in grading anal defects in order to separate small from extensive cases as depicted on 3D endoanal ultrasound, using a scoring sphincter defect and correlate anal tone to anal pressures. Methods Women with fecal incontinence were divided into two groups: small or extensive defects according to the ultrasound scoring system. Sensitivity, specificity, positive and negative predictive values of digital rectal examination in grading global and external sphincter defects were calculated. Anal tone at digital rectal examination was compared to resting and incremental pressures. Results A cohort of 76 consecutive incontinent women were enrolled. The median Wexner score was 9. Sixty-eight showed sphincter defects on 3D endoanal ultrasound. Anal tone at digital rectal examination was considered abnormal in 62 cases. Abnormal digital rectal examination showed a sensitivity of 90%, specificity of 27.78% in distinguishing small from extensive defects of both sphincters. Five out of eight women with no sphincter defects had only abnormal squeeze tone at digital rectal examination. Abnormal squeeze tone at digital rectal examination had a sensitivity of 65.31% in distinguishing small from extensive external anal sphincter defects. Digital rectal examination sensitivity increased linearly from small to extensive external anal sphincter defects (P=0.001). Women with abnormal resting tone had lower resting pressures than women with normal tone at digital rectal examination (P=0.0001). Women with abnormal squeeze tone had lower incremental pressures than women with normal tone at digital rectal examination (P=0.017). Conclusion Digital rectal examination had good sensitivity and poor specificity in discerning small from severe global anal sphincter defects. Moreover, digital rectal examination had fair sensitivity and poor specificity in grading external anal sphincter defects, and its best accuracy was on complete external anal sphincter lesions. Anal resting and squeeze tone were correlated to anal pressures.

Abstract:
Chinese plays an important role in human life, so Chinese online examination system must be developed in the light of shortcomings of the traditional Chinese examinations. Data flow diagram and data dictionary in this paper are the basic work of developing Chinese online examination system and lay the foundation for requirement analysis. A hierarchical data flow diagram was built in the data flow diagram, and the top-level data flow diagram consists of two things: the system name and two external entities that are teacher and student, and one-layer data flow diagram was divided into teacher subsystem and student subsystem data flow diagram according to subsystem; extracted one from each category entries in the data dictionary, and teachers (external entities), papers information (data stream), papers generation (processing), submission information tables (data storage) were described namely.

Purpose To evaluate a new clinical test, the resisted supination external rotation test, for the diagnosis of superior labral anterior posterior lesions of the shoulder. Study Design Cohort study (diagnosis); Level of evidence, 2. Methods Forty athletes (average age, 23.9 years) with activity-related shoulder pain were enrolled in the study. The patients underwent 3 different tests designed specifically to detect superior labral anterior posterior lesions (the resisted supination external rotation test, the crank test, and the active compression test); the tests were performed in a random order on the affected shoulder. The results of the tests were compared with arthroscopic findings. Results Out of 40 athletes, 29 (72.5%) had superior labral anterior posterior tears. The resisted supination external rotation test had the highest sensitivity (82.8%), specificity (81.8%), positive predictive value (92.3%), negative predictive value (64.3%), and diagnostic accuracy (82.5%) of all tests performed. Conclusion By re-creating the peel-back mechanism, the resisted supination external rotation test is more accurate than 2 other commonly used physical examination tests designed to diagnose superior labral anterior posterior tears in overhead-throwing athletes. By using this test in the context of a thorough clinical history and physical examination, lesions of the superior labrum can be more reliably diagnosed.

Venous aneurysms are rare causes of neck mass. Among neck veins, aneurysms of the external jugular vein are extremely uncommon. We present a case of a woman with a history of prior internal jugular vein catheterization who presented at a rural primary health care unit with a nontender progressively enlarging swelling in the right supraclavicular region. B-mode and Doppler ultrasound examination revealed a saccular dilatation of the external jugular vein, suggesting a posttraumatic venous aneurysm. Saccular aneurysms of the external jugular vein are uncommon and only rarely lead to serious complications. Access to ultrasound examination can allow early detection of this entity.

We describe a patient with nasopharyngeal lymphoma who was admitted for chemotherapy. At admission, otoscopic examination revealed a large polyp occluding the left external ear canal. The polyp was removed, and histopathologic examination showed the same tumor that was found previously in the nasopharynx. Magnetic resonance imaging showed an infiltrative process that involved the left side of the nasopharynx and extended toward the left middle and external ear. To our knowledge, this is the first case report of a direct extension of nasopharyngeal lymphoma into the middle and external ear.

Leiomyosarcoma arising from the external iliac vein is uncommon. This is a report of a 51-year-old Japanese man with venous leiomyosarcoma originating from a branch vessel of the left external iliac vein. The tumor was found during a medical examination, and the patient had no symptoms. Computed tomography showed a 72 × 49 mm mass adjacent to the left external iliac vein. The tumor was resected en-block along with ligation of the external iliac vein due to strong adhesion with the tumor. Histological examination showed venous leiomyosarcoma, and its origin was thought to be a branch vessel of the left external iliac vein. The patient has remained free from recurrence at 30 months after surgery.

AbstractObjective:We present the first published description of a painful paraganglioma of the external auditory canal. Atypical histopathology made the diagnosis difficult. We discuss the potential pitfalls of clinical diagnosis and treatment of such a case.Clinical presentation:A 49-year-old woman presented with left-sided otalgia, hearing loss and tinnitus. Physical examination revealed a firm swelling arising from the superior portion of the left external auditory canal. A clinical diagnosis of otitis externa was made.Intervention:There was minimal response to medical treatment. The swelling was aspirated, leading to brisk bleeding. A tumour was suspected from the computed tomography scan, and confirmed by a biopsy. The patient underwent excision of the paraganglioma. The histopathology was atypical, making diagnosis difficult.Conclusion:Such unusual masses of the external ear should always be borne in mind, especially when dealing with atypical presentations of commonly encountered diseases. Clinicians should have a low threshold for early intervention with imaging and biopsy.
