Abstract:
This chapter examines the challenges faced by the children of low-status immigrants in education and the labor market. While youth in general face more challenges in the early twenty-first-century than their parents and grandparents did, many of these second-generation youth face a special set of hurdles because of their disadvantaged immigrant origins. In education, the second generations originating from low-status groups suffer “ethnic penalties.” One reason is that many adults in positions of authority in school systems and workplaces hold prejudices that lead to subtle or occasionally blatant discrimination against these second-generation youth. The problems in the educational system are compounded by those these youth face when they enter the labor market. In general, they are less likely to be employed than native youth with comparable educational attainment, and sometimes, as in France and Germany, these employment penalties are large.

Although the US health care system offers the very best care to many, the poor and uninsured typically face challenges in accessing timely health care, even when faced with a life-threatening disease such as cancer. Spurred by unmet patient needs and the growing complexity of health care delivery systems, patient navigation seeks to diminish social, economic, cultural, and medical system barriers to timely quality care. This case study discusses the emergence of patient navigation as a strategy for improving cancer outcomes, especially among vulnerable populations. It explores challenges and opportunities related to advancing successful implementation of patient navigation across the cancer care continuum. It seeks to harness and apply the power and energy of patient navigators with the goal of guiding individuals across the health care continuum—from the communities where they live all the way through screening, diagnosis, and treatment at clinical care sites.

This chapter examines the most revolutionary aspect of Africa’s independence: the attempt to build a large number of states during peace, with particular emphasis on the boundaries and state system that the Africans constructed for themselves. After gaining independence in the early 1960s, African leaders were faced with a major dilemma. If the states, with their colonial power gradients, were to change, the new leaders would have to give up many of the newly tasted benefits of power and face considerable uncertainty about their own fate and the fate of their nations. The chapter considers how leaders in independent Africa confronted the problem of how to extend power over their territories given the incomplete and highly variable administrative systems they inherited from the Europeans.

Common endocrine disorders often present with psychiatric symptoms and may be mistaken for primary psychiatric disorders. Dysregulation of the following endocrine systems commonly affects behavior: the adrenocorticoid system, the thyroid system, the parathyroid system, the pancreas and regulation of glucose, and the reproductive system. Symptoms of depression and anxiety are common across the spectrum of endocrine disorders and more severe cases may include psychotic features. Clinicians are often faced with the challenge of identifying an underlying endocrine etiology for behavioral changes based on relatively nonspecific symptoms. A general understanding of the signs and symptoms associated with elevated or decreased hormone levels can assist in the differential diagnosis and ultimately increase the likelihood that a patient receives appropriate treatment. Accurate diagnosis can be especially critical in acute behavioral emergencies.

With his remarkable knowledge of anatomy and his analytical mind, Bell developed into an outstanding clinical neurologist even before the specialty had been invented. Thus it was that in his later years, when finally he returned to the University of Edinburgh as professor of surgery, referrals and requests for consultation were often for him to provide a neurological opinion rather than to perform surgical operations. His clinical observations regarding motor or sensory disturbances involving the face are of particular interest given his interests in the facial expression of emotions and the innervation of the face. Bell described or elaborated on several clinical disorders, although not always recognizing them as distinct entities—Bell’s palsy and spasms; the numb chin; motor neuron diseases; muscular dystrophy; myotonia; various movement disorders (torticollis and writer’s cramp); atlantoaxial dislocation; trigeminal neuralgia; and referred pain.

Establishment of health information systems has been a central objective of health sector reform in nearly all LMICs over the last two to three decades. Historically, reform processes have taken introduction of health information systems as inhertently strengthening health sector performance. But today it is more appropriate to talk of health sector strengthening as co-evolving with health information systems strengthening, each reinforcing the performance and reform agendas of the other. The need to build synergies is heightened as there are a multitude of global and national health reform processes underway, like those assoicated with the sustainable development goals or with universal health coverage and each of these have expanded informational needs, requiring robust, flexible, and evolving health information systems. An understanding of the challenges faced by efforts at health systems strengthening helps provide meaningful inputs into health information systems design and vice versa. Such an understanding will enrich public health informatics as an academic discipline, as an area of practice, and as a policy domain.
