ABSTRAKKecemasan dapat menjadi faktor penting dalam keputusan mencari, melanjutkan, ataupun menghentikan pengobatan infertilitas. Tingkat kecemasan pasangan infertil bervariasi dan dipengaruhi mekanisme koping dan penyesuaian yang dilakukan. Penelitian ini bertujuan untuk mengetahui faktor-faktor yang mempengaruhi kecemasan pasangan infertil yang sedang menjalani pengobatan infertilitas di Rumah Sakit Kota Jambi dan Padang. Jenis penelitian ini adalah cross secsional, dengan sampel 76 wanita infertil yang mengalami kecemasan dengan tehnik total sampling. Analisis data dengan uji chi square dan one sample t test.Hasil penelitian terdapat hubungan diagnosis infertilitas (p = 0,012) dan mekanisme koping (p = 0,000) dengan kecemasan pada pasangan infertil. Tidak ada hubungan karakteristik usia (p = 0.318), pendidikan (p = 0,595), pekerjaan (p = 0,824), durasi infertilitas (p = 0,987), riwayat pengobatan (p = 0,449), dukungan keluarga (p = 0,568), dan budaya terkait infertilitas (p = 0,401).Mekanisme koping merupakan faktor paling dominan mempengaruhi kecemasan wanita pasangan infertil yang sedang menjalani pengobatan infertilitas, yaitu wanita pasangan infertil yang memiliki mekanisme koping berfokus pada emosi mempunyai peluang 7,66 kali untuk mengalami kecemasanKata kunci : Kecemasan, Infertil, Pengobatan Infertilitas Analysis of Factors Affecting Anxiety in Infertile Couples Who Are Treating Treatment for Infertility in Jambi City Hospital and Padang ABSTRACTAnxiety can be an important factor in the decision to seek, continue, or stop infertility treatment. The level of anxiety of infertile couples varies and is influenced by coping mechanisms and adjustments made. This study aims to determine the factors that influence the anxiety of infertile couples who are undergoing infertility treatment in Jambi City Hospital and Padang.This type of research is cross sectional, with a sample of 76 infertile women who experienced anxiety with a total sampling technique. Data analysis with chi square test and one sample t test.The results of the study showed a correlation between infertility diagnosis (p = 0.012) and coping mechanism (p = 0.000) with anxiety in infertile couples. There was no correlation between the characteristics of age (p = 0.318), education (p = 0.595), employment (p = 0.824), duration of infertility (p = 0.987), medical history (p = 0.449), family support (p = 0.568), and culture related to infertility (p = 0.401).Coping mechanism is the most dominant factor affecting anxiety of infertile couples who are undergoing infertility treatment, ie infertile couples who have an emotional-focused coping mechanism have a chance of 7.66 times to experience anxietyKeywords: Anxiety, Infertility, Infertility Treatment

Phenomenology Study: Coping Mechanism of Women No Heredity Viewed by Minangkabau Culture. The Minangkabau tribe adheres to the matrilineal kinship system. Based on this system, when a woman suffers infertility, so it means the family has not the next generation, and the line of descent will cut off. This condition makes the women need to have a coping mechanism to avoid psychology problems, including depression, anxiety, stress, neglected, discrimination, under pressure to divorce, and perceive abused. The study aimed to explore the coping mechanism of infertility women in the context of Minangkabau culture. This phenomenology study applied an in-depth interview, which involved ten women who were selected through a snowball sampling technique. Collaizi’s analysis method resulted in six themes. Those themes are through psychological and social change, obtain a response from others, the views of Minangkabau culture, adaptive coping used by women no heredity, the families were main source support, community social support. The findings strengthen the justification to involve psycho-social aspects in the infertility treatment.Keywords: Coping mechanism, infertile women, Minangkabau culture Abstrak Suku Minangkabau menganut sistem kekerabatan matrilineal. Berdasarkan sistem ini, ketika seorang wanita menderita kemandulan, itu berarti keluarga tidak memiliki generasi berikutnya, dan garis keturunan akan terputus. Kondisi ini membuat perempuan perlu memiliki mekanisme koping untuk menghindari masalah psikologi, termasuk depresi, kecemasan, stres, diabaikan, diskriminasi, di bawah tekanan untuk bercerai, dan juga merasa dilecehkan. Penelitian ini bertujuan untuk mengeksplorasi mekanisme koping wanita infertilitas dalam konteks budaya Minangkabau. Studi fenomenologi ini menerapkan wawancara mendalam yang melibatkan sepuluh wanita yang dipilih melalui teknik pengambilan sampel bola salju. Melalui metode analisis dari Collaizi, menghasilkan enam tema. Tema-tema tersebut termasuk Mengalami perubahan psikologis dan perubahan sosial, mendapat respon dari orang sekitarnya, pandangan budaya Minangkabau, koping adaptif yang digunakan oleh perempuan belum mempunyai keturunan, keluarga menjadi sumber dukungan utama, dukungan sosial dari masyarakat. Background: Infertility is one of the major causes of mental health issues among couples. Interestingly, a large percentage of this infertility cannot be always explained in terms of a medical diagnosis. Psychological distress has long been suspected as having an important impact on infertility. However, the exact nature of association between psychological factors and infertility has been a subject matter of scientific debate. For modern urban women, there is an additional obligation of balancing work and home responsibilities; this dual role inevitably leads to stress. In this context, an important yet less-explored area of infertility is the role of work-related stress on female infertility. Aims: This study tries to investigate the mediating role of certain psychosocial constructs viz (a) Self-efficacy related to work-family conflict—measured in terms of “work-family conflict self-efficacy scale” by Cinamon (2003), (b) perceived occupational stress—measured in terms of “work-to-family conflict scale” by Netemeyer et al (1996), (c) decision-making in family—measured in terms of “decision-making scale” by Blood and Wolfe, (d) perceived infertility stress—measured in terms of “fertility problem inventory” by Newton et al (1999) in 25 urban working women of the age range 28 to 40 years who have been experiencing medically unexplained infertility for at least 1 year; the severity of infertility being measured in terms of duration of involuntary childlessness. Results: The analysis reveals that severity of medically unexplained infertility can be explained in terms of “self-efficacy related to work-family conflict” and “perceived infertility stress”; 28.6% of variation in severity of unexplained infertility being contributed by “self-efficacy related to work-family conflict” and 46.9% of the variation in the severity of unexplained infertility being contributed by a conjunction of “perceived infertility stress” and “self-efficacy related to work-family conflict.” Conclusion: The findings imply that urban educated women tend to place more psychological emphasis on “motherhood,” and “perceived deficit in attaining motherhood” seems to be the important source of personal stress for these women in comparison to stressors emanating from occupational hazards and family dynamics related to decision-making.

AbstractIn the mid-twentieth century, the Family Planning Association emerged in Britain as one of the largest providers of infertility diagnosis for men and women. In the early years of the century, women were coming to birth control centers seeking cures for their childlessness, well before those centers began officially offering infertility investigation and treatment. What changed by mid-century was the emergence of a determination not only to welcome infertility patients at birth control clinics but to establish the clinics as centers for infertility research and care. Beginning in the late 1930s, eugenicists concerned with the impact of population decline on the social acceptability of birth control formed an alliance with feminist medical professionals determined both to empower women in involuntarily childless marriages to understand and address the causes of their infertility and to shift the paradigm of infertility treatment away from its over-focus on the female body. This political marriage of convenience gave birth to a large-scale movement to diagnose both male and female infertility through the auspices of the Family Planning Association. The organization's infertility program, in turn, spurred the development of infertility as a field of medical research in postwar Britain and encouraged the expansion of National Health Service provision of infertility care.

In this article, I offer an autoethnographic examination of unexplained infertility. The narrative is driven by a central question: how does unexplained infertility mean in my life? By exploring the how of unexplained infertility, narrative inquiry emphasizes the long-term impacts of a persistent diagnosis. As an unstable diagnosis, unexplained infertility presents a complicated site from which to consider family planning and gender work. Narrative vignettes occurring over a period of years explore identity. These experiences highlight moments of conflict and upheaval where the abstract diagnosis derails heteronormative and cisgendered constructions of fatherhood and masculinity. This infertility account disrupts codes of silence around experiencing infertility.

Abstract:
Background. The inability of couples to achieve pregnancy is a major cause of psycho-social problems in family relationship that could lead to marital disharmony. Objective. The aim of this study was to find out the possible risk factors for female infertility. Methods. A case-control design and a sample size of 400 (200 cases of infertility and 200 controls) were used in the study. Cases and controls were selected at random at the infertility and family planning clinic of the University of Medical Sciences Teaching Hospital Complex, Akure and were subjected to a predesigned interviewer administered questionnaire to collect the data. The cases were classified into primary and secondary infertility; binary and stepwise logistic regressions were used to generate the Odds ratio and 95% confidence interval of the possible risk factors and the level of significance was set at P<0.05. Results. The mean age of the women with infertility was 28.5±5.43 years and the mean age of those in the control group was 29.1±5.62 years. Among the cases, 155 (77.5%) had secondary infertility, while 45 (22.5%) had primary infertility. Significant risk factors for female infertility included presence of fibroids, having had fibroid operation, multiple sexual partners, previous abortion, polycystic ovary syndrome (PCOS), sexually transmitted infection (STI) and post abortion sepsis. Conclusion. The study showed that secondary infertility is still the most prevalent and the risk factors were multi factorial. Efforts should be intensified to reduce infertility due to preventable causes.

Abstract:
Aim: Family function and husband support can impact depression severity in women with infertility disorder. The aim of this study was to assess the impact of family function and husband support on depression severity among women with infertility disorder at the fertility clinics of a University Teaching Hospital, South-South, Nigeria. Methodology: A cross-sectional descriptive study was carried out among 341 female respondents attending the University of Benin Teaching Hospital fertility clinics over a 3-month period, using a semistructured interviewer administered questionnaire. Data obtained were analyzed. Result: Respondents mean age was 36 ± 5.3 years. The overall prevalence for depression in this study was 42.5% with a breakdown of 64.2%, 30.4% and 5.4% of these depressed participants having mild, moderate and severe depression, respectively. Family dysfunction had a statistically significant relationship with severity of depression in women with infertility (p < 0.001). A statistically significant relationship was established between poor husband support and the severity of depression (p < 0.001). Conclusion: Depression is highly prevalent among women with infertility disorder, severe depression was associated with family dysfunction. Good family function would reduce the severity of depression.

Abstract:
Background: Infertility leads to considerable personal suffering and disruption of family life. According to United Nations "Reproductive health is a state of complete physical mental and social well-being and not merely the absence of disease or infirmity in all matters relating to the reproductive system and to its functions and processes". The objective of present study was to find out different causes of female infertility with diagnostic laparoscopy and their comparative frequency in primary and secondary infertility.Methods: It is a prospective study conducted on all infertile women and they underwent diagnostic laparoscopy for primary and secondary infertility during the study period. Couples who had not lived together for at least 12 months, and those with male factor infertility were excluded. Data were collected on a proforma, and analyzed on SPSS package for windows version 10. Frequencies were calculated for laparoscopic findings regarding primary and secondary infertility.Results: Fifty infertile women underwent laparoscopy during the study period, 35 (70%) had primary infertility while 15 (30%) secondary infertility. 10 (28.5%) patients with primary and 3 (20%) patients with secondary infertility had no visible abnormality. The common finding was tubal blockage in 10 (28.5%) and 5 (33.3%) cases of primary and secondary infertility respectively. 9 (25.7%) cases of primary infertility were detected as polycystic ovaries (PCO) and 2 (13.3%) in cases of secondary infertility. Endometriosis was found in 1 case with primary infertility and 2 (13.3%) cases with secondary infertility. Fibroid was found in 3 (8.57%) and 1 (6.6%) cases of primary and secondary infertility respectively.Conclusions: Most common causes responsible for infertility were tubal occlusion and polycystic ovary. Infertile couple should be thoroughly investigated. Laparoscopy in infertility can be used for a definitive diagnosis.

This study aimed to understand the psychosocial effects of temporary infertility by stress in women. This is a descriptive cross-sectional study with qualitative and exploratory approach. The approach used in this study included conducting interviews with ten women who had proven diagnosis of temporary infertility by stress, the analysis of the interviews was based on the perspective of Grounded Theory. After transcription and subsequent analysis of the interviews, explanatory categories that emerged from the speeches of the interviewees were: No biological problems, undiagnosed; Impacts on social relationships; Impacts on the marital relationship; and the dream realized and future action. The results showed that the numbers of years of infertility treatment more positively also negatively influenced the lives of these women generating psychosocial stress. This study allowed the understanding of the psychosocial effects of temporary infertility by stress in women, which may contribute to work teams in developing strategies to support these, however there is the consistent data literature related to the subject, even in modern times whose infertility problem already set as a public health problem with increasing incidence in the world population.
O presente estudo objetivou compreender as repercussões psicossociais da infertilidade temporária por estresse em mulheres. Trata-se de um estudo transversal descritivo com abordagem qualitativa e exploratória. O procedimento metodológico utilizado nesse estudo compreendeu a realização de entrevistas com dez mulheres, que não apresentavam problema que impedisse a gestação mais tinham infertilidade temporária, a análise das entrevistas se baseou na perspectiva da Teoria Fundamentada nos Dados. Após a transcrição e posterior análise das entrevistas, as categorias explicativas que surgiram dos discursos das entrevistadas foram: sem problemas biológicos, sem diagnóstico; repercussões nas relações sociais; repercussões na relação conjugal; e o sonho realizado e providências futuras. Os resultados obtidos permitem concluir que os anos de tratamento de infertilidade influenciaram positivamente, mas também negativamente na vida dessas mulheres, gerando estresse psicossocial. Esse estudo permitiu a compreensão acerca das repercussões psicossociais da infertilidade temporária por estresse em mulheres, que pode contribuir para equipes de trabalho na elaboração de estratégias para o apoio a essas, entretanto não há na literatura dados consistentes relacionados ao tema, mesmo em tempos atuais cujo problema da infertilidade já se configura como um problema de saúde pública com crescente incidência na população mundial.

Parenthood is a life goal for the majority of individuals and couples who have reached a certain life stage. For many couples parenthood is, however, not a reality through expected means. Statistics gathered, in an unpublished information document, by the Port Elizabeth Infertility and Wellness Clinic, it is estimated that 10 to 15% of all married couples in the region experience infertility. After failed infertility treatment, adoption is considered an option for many couples wishing to have children. The social worker plays an integral role in the assessment and preparation for adoption. However, there is no legislated preparation for the fertility treatment process as there is for adoption in South Africa, yet both processes are intended to result in parenthood shadowed by stressful circumstances for prospective parents. The study explored the experiences of couples who have transitioned from being diagnosed as infertile and have successfully adopted. The study was conducted from a qualitative research perspective and employed the exploratory-descriptive strategy of inquiry. Purposive sampling was applied and eight married couples participated in the study. The participants were sourced from a support and information network for adoptive families as well as referrals from social workers working within the adoption field. Semi-structured interviews were conducted by an independent interviewer with thematic analysis being used for data analysis. Trustworthiness constructs were taken into account to ensure reliability and rigour. The major findings revealed the distinctive experiences regarding fertility treatment, the experiences regarding adoption and suggestions from couples for couples facing similar situations as well as for professionals in practice. The study is viewed as significant in guiding professional services to couples, who are intending to achieve parenthood via adoption, by adding to the body of existing knowledge in adoption and medical social work.

Abstract:

This study analyzes the narratives of 38 married, heterosexual, infertile adoptive parents. It asks how parents use narrative to reconcile a presumed reluctance to adopt with presumed satisfaction with adoption, and if there are differences in the narratives of men and women.

Qualitative analysis reveals that participants place recollections of suffering within redemptive sequences, reinterpreting difficult experiences as leading to personal growth and transformation, which in turn leads to greater connection with others. A majority of participants find profound retrospective meaning in a belief in fate and describe a greater appreciation for their children and for life itself as a result of their experiences.

Women were identified as the protagonists of the stories in the majority of cases by both husband and wife. When women were the protagonists men tended to worry about their wives, view her suffering and her path to motherhood as central to the story, and to easily give up control of the adoption process. When men were the protagonist or couples viewed themselves as equal protagonists, the process involved both partners.

The only theme that participants had a difficult time interpreting was that of race. Pals' (2006) transformational processing model was adapted to rate participants' sense of resolution regarding race. For same-race adopters resolution was associated with placing desire for a same-race child in a broader belief system, while for transracial adopters it was associated with comfort with the community of the child's birth, a belief that one could grow into dealing with race, and a belief in fate. Unresolved same-race adopters felt judged, experienced identity threat, and were inarticulate about race. One group of unresolved transracial adopters paid little attention to race at the time of adoption and later struggled with it, while another group had difficulty discussing race at all, minimized its importance, and often contradicted themselves.

Abstract:
Infertility affects 1 in 8 couples and the literature discusses the emotional effects infertility has on an individual. One option for infertility is to attempt medical interventions and the literature in the field does not explain why some people attempt more interventions than others. Using data from the 2002 National Survey of Family Growth (NSFG), Cycle 6, this quantitative study aims to understand the relationship between traditional gender roles and values and the attempted number of medical interventions by individuals within a relationship experiencing infertility. The results from this study indicate that both males and females that are in relationship experiencing infertility are more likely to attempt medical interventions when they highly value the meaning of family, rather than their views on traditional gender roles and values for men and women.

Abstract:
La région AZFc du bras long du chromosome Y est une des régions les moins stables du génome humain. Elle est composée de multiples palindromes entraînant différents types de réarrangements, dont certains seraient associés à l'infertilité masculine. Nous avons recherché les microréarrangements d'AZFc dans 2 populations : 344 hommes infertiles et 136 hommes d'une population contrôle (119 normospermiques et 17 présentant une azoospermie de type obstructif). Nous avons observé une fréquence élevée de remaniements génomiques (8,7%), avec des fréquences de délétions et de duplications similaires dans les deux populations. Les remaniements ont ensuite été caractérisés au niveau des gènes DAZ et CDY1 et au niveau des haplogroupes du chromosome Y. Nous avons également étudié l'expression des gènes de la famille DAZ par RT-PCR [reverse transcription-polymerase chain reaction] quantitative « en temps réel» à partir de 30 biopsies testiculaires présentant des situations d'infertilité masculine bien identifiées. The AZFc region on the Y chromosome long arm is one of the most unstable regions in the human genome. It consists almost entirely of very long repeats, is prone to rearrangement and some of them have been reported to be associated with male infertility. We screened two populations: 344 infertile patients and 136 controls for AZFc microrearrangements and we observed a high frequency of genomic rearrangements with similar frequency of deletions and duplications in both populations. Rearrangements were then characterized using DAZ and CDY 1 genes analyses and Y chromosome haplogroup determination. We also studied DAZ gene family expression by "real time" quantitative RT-PCR [reverse transcription-polymerase chain reaction] from 30 testicular biopsies presenting distinct histological pictures. Different expression profiles were obtained according to the gene tested and the fertility status

This dissertation aims to evidence the assisted reproduction like a way of maintaining "traditional" relationships of the modern nuclear family, centered on love, on affection, on idea of the couple’s "plan", which connects parents to their children, and in blood and family name categories. The research, conducted in 2017 and 2018, in the small town of Marechal Cândido Rondon / PR, focused on the lived experience by eight women ando ne man who used technological reproductive methods such as the artificial insemination and in vitro fertilization. These methods operate aggressively in their bodies of these women by intensive use of medicalization, but does not question how the techniques are disclosed and marketed. As a methodological strategy, in addition to analytical and bibliographic references about the subject, the construction of data was done through unstructured interviews and other sources obtained on websites and social networks like the facebook. The argument is that, for these women and their families, the Assisted Reproduction "corrects" the infertility and yours stigmatizing effects and, above all, leads to the realization of the biogenetic parental project, reinforcing the family nucleus composed by father, mother and biological child.
Esta dissertação tem como objetivo analisar a reprodução assistida como um meio de manter relacionamentos “tradicionais” da família nuclear moderna, centrada no amor, no afeto, na ideia de “projeto” do casal, que liga os pais a seus filhos, e nas categorias do sangue e do nome de família. A pesquisa, realizada nos anos de 2017 e 2018, no pequeno município de Marechal Cândido Rondon/PR, centrou-se na experiência vivida por oito mulheres e um homem que utilizaram meios reprodutivos tecnológicos como a inseminação artificial e a fertilização in vitro. Esses métodos intervêm agressivamente sobre os corpos dessas mulheres pelo uso intensivo de medicalização, não sendo questionado por elas o modo como as técnicas são divulgadas e mercantilizadas. Como estratégia metodológica, além das referências analíticas e bibliográficas sobre o tema, a construção de dados foi feita por meio de entrevistas semiestruturadas e de outras fontes obtidas em sites da internet e nas redes sociais como o Facebook. O argumento apresentado é que, para essas mulheres e suas famílias, a reprodução assistida “corrige” a infertilidade e seus efeitos estigmatizantes e, sobretudo, conduz à realização do projeto parental biogenético, reforçando o núcleo familiar composto por pai, mãe e filho biológico.

ABSTRACT Background: Previous studies suggest that tobacco use affects systems of the human body involved in the reproductive process. Tobacco smoke exposure affects uterine receptivity, which may lead the risk of infertility. This study aimed to investigate the associations between cigarette smoke exposure, family history of infertility, and the risk of infertility among women in reproductive age. Subjects and Method: A case control study was conducted at obstetrics and gynecology polyclinic and Sekar fertility clinic, in Dr. Moewardi hospital, Surakarta, Central Java, from October to November 2019. A sample of 200 reproductive age women was selected by fixed disease sampling. The dependent variable was infertility. The independent variables were age, body mass index (BMI), physical activity, endometriosis, family history of infertility, smoking exposure, and stress. The data were obtained from medical record and questionnaire. The data were analyzed by a multiple linear regression. Results: The risk of infertility increased with age ≥35 years (b= 1.24; 95% CI= 0.18 to 2.30; p= 0.012), BMI <18 or ≥25 (b= 2.76; 95% CI= 1.74 to 3.76; p<0.001), high physical activity (b= 1.44; 95% CI= 0.38 to 2.51; p= 0.009), endometriosis (b= 1.06; 95% CI= 0.06 to 2.05; p= 0.038), tobacco smoke exposure (b= 1.37; 95% CI= 0.30 to 2.43; p= 0.012), and severe stress (b= 1.07; 95% CI= 0.16 to 2.00; p= 0.022). Conclusion: The risk of infertility increases with age ≥35 years, BMI <18 or ≥25, high physical activity, endometriosis, smoke exposure, and severe stress.
