Abstract:
Engaging family advisors in pediatric quality improvement (QI) efforts is well-studied in intensive care but less understood in other settings. The purpose of this study was to assess the perceived impact of including a family advisor as a colead on a QI initiative that successfully improved the family-centered timing of routine morning blood tests performed on pediatric inpatients. Five structured written reflections from core QI team members were analyzed using inductive thematic analysis and 3 major themes were identified. The first found that a family advisor’s presence from the beginning of a QI initiative helps inform project design. The second determined that family partners working with residents fostered a better shared understanding of the role of trainees and caregivers in improving the quality of care. The third found that a family partner is an effective change agent to enact practice improvement, support professional development, and enhance resident education. Our qualitative analysis showed that engaging a family advisor as a colead influenced the design, implementation, and post-intervention impact of the initiative and improved family-centered outcomes.

Abstract:
The establishment of the Court for Children in the juvenile justice system is to provide legal protection for children. This court consists of a Magistrate and two advisors which one of them is a woman as provided in section 11 (2) of the Child Act 2001. Basically, the role of the advisor of the Court for Children is to advise the Magistrate relating to the orders (punishment) to be imposed on children who are in conflict with the law and to advise the parents or guardians whenever necessary. However, Section 11 of the Child Act 2001 is the only provision as regards the advisor of the Court for Children in Malaysia. There are no other guidelines or regulations on the mechanism of execution of duties of the advisor, its appointments, and training requirements. Thus, this study aims to identify the issues and challenges relating to the position and roles of the advisor. An analysis is made of legal documents and academic journals. Semi-structured interviews were also conducted with the Social Welfare Department (JKM), Ministry of Women, Family and Community Development (KPWKM), Legal Affairs Division (BHEUU), Prime Minister Department (JPM), and five Children Court advisors (who are representing the urban and rural areas). The study found that the Children Court advisor was recognized in 1947 via the Juvenile Court Act 1947. However, there are several issues and challenges to be addressed including the jurisdiction, governance structure, a mechanism of execution of duties, appointment criteria, the appointment process, modules, and training for Children Court advisors. The absence of guidelines, specific regulations, or manuals has limited the functions and roles of the Children Court advisor. This study suggests a better legal framework for the Children Court advisors to increase their credibility and professionalism. Thus, they can play an effective role in the juvenile justice system in Malaysia.

We follow one advisor and five family firm succession cases over 4 years to capture emerging emotions during the succession process. Using inductive analysis, we investigate how the advisor’s mediation of these emotions affects individual-level satisfaction with the succession process. Interviews, observation, meeting minutes, and archival data reveal an iterative process: the advisor first unearths the incumbent’s and successor’s emotions to surface emotional tensions before alleviating them. Unearthing and alleviating emotions speeds role adjustments and advances succession, especially when the incumbent becomes “stuck” in the process. Emotion mediation and role adjustment appear to foster individual-level satisfaction with the succession process.

This study reviews the literature on advising family firms and integrates past contributions in such a way that scholars can use the findings as a foundation to launch future research. It reviews 105 articles and other research contributions published since 1983. The findings suggest that although advisors play a crucial role within family firms, there remains to date a lack of rigorous academic research. These past contributions are integrated into a framework that provides a first step toward developing theoretical arguments and charting directions for future research that address the nature of the family firm–advisor relationship.

220 Background: Patient, family and public engagement builds strong, sustainable partnerships benefitting the planning, delivery and evaluation of health services. With growing international evidence and increased organizational investment supporting this type of engagement, evaluating the experience of these individuals is essential to ensuring meaningful participation and value. This study describes the development and psychometric properties of the Advisor Experience Survey in order to assess on-going patient, family and public advisor engagement at the system-level. Methods: The development and psychometric validation consisted of five steps: 1) literature review to identify relevant engagement experience items; 2) modified Delphi process where relevant engagement experience items were rated by experts; 3) cognitive interviews to ensure that items were clear and understandable; 4) pilot of survey; and 5) exploratory and confirmatory factor analysis (EFA; CFA) to determine construct validity. Results: The 13-item survey was disseminated to eligible patient, family and public advisors between April and September 2016 using FluidSurveys. All items were rated on a 5-point Likert scale. A total of 126 responses were included in the EFA and CFA. The final confirmatory solution consists of 3-factors (Support and Resources, Engagement Opportunities and Engagement Experience), including 9-items (X2= 31.4, df = 24; AGFI = 0.92; CFI = 0.99; SRMR = 0.02 and RMSEA = 0.05; 95% C.I. = 0.00-0.09). The 3-factor solution explains a total of 92% of the variance of the advisor engagement experience. Conclusions: The 3-factor Advisor Experience Survey with 9-items is acceptable for measuring the patient, family and public advisor engagement experience as it demonstrates good internal consistency and construct reliability. Cancer Care Ontario utilizes the 3-factor solution to report patient, family and advisor engagement experience at the system-level on a quarterly basis.

Consulting and working as an advisor to family businesses are awesome responsibilities. The interventions and advice we give will in some instances have an impact on a family for generations. As a result, it is critical to understand what factors create success as well as failure. In most instances, understanding our failures provides the richest opportunity to create future benefits for our clients. Accepting this challenge of learning from our mistakes requires courage and leadership to explore and understand our own depths and the five common shadows facing family business consultants and advisors. For those of us who take the risk and accept the challenge, the benefits are enormous—not only for ourselves and our profession but more importantly for our clients who are the recipients of our work.

Family enterprise advisors work on complex and unique problems for their family enterprise clients. Little attention has been given to these professionals and their abilities to provide innovative solutions. In this study, our aim is to understand more about family enterprise advisors ( N = 231). To achieve this objective, we hypothesize that the effects of advisor goal orientation (i.e., learning orientation, proving orientation, and avoidance orientation) on adaptive behaviors (i.e., personal bricolage and individual innovative behavior) are mediated by the quality of feedback received from clients. The results indicate that quality of feedback partially mediates the relationships between goal orientation and these behaviors. We conclude by providing a practitioner model explaining how advisors may adapt to different family enterprise client role environments.

Implementation of law number 22 in the year of 1999 on regional autonomy has changed the management of government of Indonesia, both on the national scale and on a regional one. One of thegovernment institutions that got its impact is Family Planning Program. This program has implications, not only to advisor’s attitudes and work behaviors, such as job performance and working motivation but also to government commitment on the reception and implementation of this program namely formation of family planning institutions at theregional level.One of the attitudes indicated by the advisor in the form of complaints related to salary wages and operating costs are not considered balance with the workload that causes themotivation and performance of work decrease. Other phenomena are to turn over the workers in the family planning and other government institutions. Accumulation of these could be the cue of advisors’ weak commitment towards their institutions. Implications: This study can provide an understanding of job satisfaction and increase the extension's commitment in performing its duties.

Abstract:
Health care consumers are under-represented in literature when defining patient and family engagement. The proportion of people living longer is rapidly growing. Future research is needed to evaluate which strategies of patient and family engagement are most useful in real-world health care settings for patient and families. The purpose of this study was to describe the lived experiences of patient/family advisors working within patient family advisory councils at an academic medical center in the Midwestern United States. The conceptual framework is based on Greenleaf's servant leadership and Bass's transformational leadership. The research questions examined how patient/family advisors describe patient and family engagement, their experiences from the advisor program, and what is most meaningful to them. A phenomenological design was employed with a purposeful sample of 19 interview respondents drawn from 5 different advisory councils. Data analysis consisted of interpretive phenomenological analysis and a detailed, in-depth account of participant experiences. Transcripts from semi structured face-to-face interviews were collected, coded, validated by member checking, and triangulated with emergent themes. Emergent themes included patient/family advisors' descriptions of patient and family engagement within the patient family advisory councils and organizational efforts most meaningful to patient/family advisors. The results of this study may help create social change by improving the standards and quality of patient and family engagement by preparing health care professionals to better meet the needs of health care consumers.

Background: Family firms have a strong impact on the world’s well-being, in terms of social stability, international wealth creation, and global development. Advisors appear essential to ensure the family firms’ prosperity and success, representing a potential source of advantage. There are several types of advisors working for family firms. Our study focuses on formal advisors (i.e. professionals, legally hired and paid by family firms), that either provide process or content advisory. Process advisors focus more on family dynamics and processes (e.g. relational and emotional aspect), while content advisors provide strategic services and specialized knowledge. Research Purpose: To understand how the approaches of advising family firms differ among formal business advisors.Research Problem: When advising family firms, two main type of issues can emerge. Soft issues regard personal and relational features, like emotions, while hard issues are emitted from the business, and are related to profitability and finance. In our critical literature review, we identify a gap in the existing knowledge regarding how advisors deal with soft issues when advising family firms. Although the unique characteristics and issues of family firms lead them to seek for specialized advice, they seem to be dissatisfied with the advisors’ ability to address soft issues. On the other side, content advisors show frustration in dealing with them. Research Questions: (1) How do content advisors perceive and address family firms’ soft issues? (2) How do process advisors perceive and address family firms’ soft issues? Method: Ontology – Relativism; Epistemology – Social constructionism; Methodology –Exploratory Qualitative Interview-based Study; Data collection – In-depth Interviews; Sampling – Two Purposive Criterion Samples of Content (i.e. accountants) and Process Advisors (i.e. family business advisors) (seven respondents each one); Data Analysis – Content Analysis (creation of a tree-diagram based on codes, sub-categories, categories, main categories)Conclusion: We develop a model describing how content and process advisors perceive and address soft issues, pointing out the identified differences in their approaches. Furthermore, our results suggest the existence of two level of soft issues, which adds new insights to the existing academic knowledge. Managerial Implications: We develop six main managerial implications for both content and process advisors, which we recommend as inputs facilitating the advisors’ work with family firms.

An intervention that may be applied by non-psychotherapist caretakers, i.e., substance dependence advisors, working in therapeutic communities, who will participate in this research. Therapeutic communities for substance addicts are important resources, described by the Brazilian National Anti-Drug Department, in the drug-fighting network, however, as most of the substance dependence services, they require interventions in the family level. Due to this need and the complexity of the phenomenon, substance dependence experts have oriented approaches in this scope. By means of a qualitative research, we will seek to investigate, among the caretakers involved in this research (former substance addicts recovered for at least one year), the functioning patterns of their family systems in the generational and transgenerational levels. We will seek to develop along with the participants the understanding of the relationship patterns in their original and current families, with the purpose of valuing the competences and rescuing the resilient aspects of these caretaker families systems. Therefore, our purpose is to create, together with these caretakers, an intervention instrument, easily applicable by non-psychotherapists working with families involved with the problems of substance addiction, to empower them to fight the drug-addiction cycle<br>O presente estudo tem como objetivo apresentar uma proposta de intervenção nos sistemas familiares com membro adicto. Uma intervenção que possa ser aplicada por cuidadores não-psicoterapeutas, ou seja, conselheiros em dependência química, que trabalhem em comunidades terapêuticas, os quais serão os participantes desta pesquisa. As comunidades terapêuticas para dependentes químicos são importantes recursos, descritos pela Secretária Nacional Antidrogas, na rede de atenção ao enfrentamento das drogas, porém, assim como a maioria dos serviços para dependência química, carecem de intervenções em nível familiar. Em virtude dessa carência e da complexidade do fenômeno é que especialistas em dependência química têm orientado abordagens neste âmbito. Por meio de uma pesquisa qualitativa, procuraremos investigar, entre os cuidadores envolvidos nesta pesquisa (ex-dependentes químicos recuperados há pelo menos um ano), os padrões de funcionamento de seus sistemas familiares no plano geracional e transgeracional. Buscaremos desenvolver, junto aos participantes, a compreensão dos padrões de relacionamento em sua família de origem e atual, com o intuito de valorizar as competências e resgatar os aspectos resilientes dos sistemas familiares destes cuidadores. Nosso objetivo, então, é criar, junto com estes cuidadores, um instrumento de intervenção, facilmente aplicável por não-psicoterapeutas que trabalhem com famílias envolvidas com a problemática da dependência química, para o empoderamento destas no enfrentamento do ciclo da drogadicção

Os objetivos centrais da pesquisa são caracterizar a base produtiva e os mercados de hortifrutícolas orgânicos, nesta região, destacando as contradições e as relações presentes entre os elos da cadeia. Tem-se como hipótese que a base produtiva de FLV orgânicos não consegue atender a demanda de mercado existente em Goiânia, devido a problemas de ordem técnica, econômica e organizacional. O marco referencial teórico foi elaborado a partir da produção científica desenvolvida sobre a Agroecologia, sendo que o aporte metodológico para levantamento e análise dos dados da pesquisa está fundamentado na teoria do Diagnóstico dos Sistemas Agrários. Os resultados da pesquisa indicam que a produção orgânica de frutas, verduras e legumes na região de influência econômica de Goiânia é feita por dois grupos de agricultores filiados à Associação para o Desenvolvimento da Agricultura Orgânica de Goiás e à Associação Agroecológica Vale Vivo. A atual produção orgânica enfrenta problemas de ordem tecnológica, organizacional e econômica. O mercado dos produtos orgânicos está restrito às feiras e às cestas de alimentos orgânicos e, as demandas dos mercados varejistas e da CEASA/GO não são atendidas. A resolução destes problemas passa pela ampliação e pelo acesso aos programas e às políticas públicas disponíveis para o desenvolvimento da Agroecologia e da agricultura familiar.

Les approches de conseil agricole favorisant le renforcement des compétences des producteurs sont au centre des préoccupations des acteurs du développement agricole, particulièrement au Sud. Le conseil à l'exploitation familiale (CEF) est une approche de conseil de gestion, basée sur des méthodes d'apprentissage et d'aide à la décision, favorisant l'évolution des producteurs dans leur réflexion stratégique. L'objet de cette thèse est l'analyse de l'évolution de la gestion stratégique des producteurs à travers l'accompagnement fourni par le CEF. Nous avons choisi d'analyser les processus d'apprentissage induits par le CEF en caractérisant l'évolution de la gestion stratégique de l'exploitation agricole chez dix-neuf producteurs du Sud-Bénin. Nous proposons d'analyser l'évolution de la réflexion stratégique comme un processus complexe, non linéaire et récursif, assimilable à un processus d'apprentissage. Nous analysons l'évolution de la réflexion stratégique à travers l'analyse de la construction de la proactivité dans le CEF, la proactivité étant définie comme la combinaison de la vision stratégique du producteur (incluant la source de changement perçue par le producteur) et de la mise en œuvre d'actions stratégiques. Pour caractériser la situation des producteurs avant le CEF, nous analysons la situation des producteurs selon trois niveaux : leur proactivité, leur système d'activités en lien avec leur niveau de ressources, et leurs pratiques de planification de gestion de l'exploitation agricole. Nous avons choisi d'adopter une posture interprétativiste d'analyse du discours des producteurs et des changements qu'ils attribuent au CEF. Les résultats de ce travail montrent tout d'abord que les trois niveaux de caractérisation des producteurs (la proactivité, le niveau de ressources et les pratiques de planification) ne sont pas directement liés. Notre travail montre ensuite que le CEF fait évoluer la proactivité des producteurs dans un temps court, en un an, principalement en révélant des marges de manœuvre existantes, en faisant évoluer leur vision de l'avenir et leur perception de leur pouvoir sur le changement. Enfin, les résultats valident la récursivité du processus d'apprentissage, le profil de proactivité au départ étant le facteur le plus important influençant la construction de la réflexion stratégique durant cette première année de participation au CEF. De nouvelles perspectives de recherche sont proposées, comme l'analyse de ces processus sur le temps long, l'analyse de la durabilité de la proactivité et de son lien avec la pérennité du projet des producteurs. Ces résultats alimentent également les réflexions plus opérationnelles sur le CEF, à la fois sur le ciblage de l'approche, sur l'adaptation des méthodes et des outils à l'accompagnement de cette réflexion stratégique, sur le renforcement de capacités des professionnels du CEF pour appréhender ces différentes dimensions des processus d'apprentissage<br>Agricultural advisory approaches enabling producers' capacity building are the center of interest of many development actors, especially in developing countries. Management Advice for Family Farms (MAFF – Conseil à l'exploitation familial / CEF) is a management advice approach, based on learning methods and decision support tools, helping producers to evolve on their strategic thinking. The aim of this research is to analyze the evolution of producers' strategic management through MAFF. We chose to analyze the learning processes induced by MAFF by characterizing the evolution of strategic thinking for nineteen producers in Southern Bénin. We consider the evolution of strategic thinking as a complex, non linear and recursive process, similar to a learning process. The evolution of strategic thinking is identified through the evolution of producer's proactivity through MAFF, proactivity being defined as the combination of producers' strategic vision (including their perceived source of change) and the strategic actions undertaken to reach this strategic vision. In order to describe the producers' situation before MAFF, we chose to focus on three main levels: their proactivity, their activity systems (including their resources for different types of capitals) and their planning practices for the management of their farm. From an interpretativist point of view, we analyzed producers' speech and representations, and the changes their spontaneously attributed to MAFF after one year of participation. The results of this research show first that the three levels of characterization of the producers' situation before MAFF started (proactivity, activity system and resources, planning practices) are not directly linked. This work also shows the evolution of producers' proactivity through MAFF in a limited time (one year), mainly by changing their perception on their perceived constraints and resources, revealing flexibility, changing their vision of the future and their perception of their power on their environment and change. Finally, those results substantiate the recursivity of the learning process, proactivity profile before MAFF being the main influence factor of strategic thinking building during this first year of participation to MAFF. Those results open new research perspectives, to analyze this strategic thinking building process on the longer term, the analysis of the persistence of proactivity, and the link between proactivity, performance and the sustainability of producers' projects. Those results also raise questions at the operational level, on the inclusivity of advisory approaches, on the adaptation of methods and tools to trigger this strategic thinking building, and on the necessary capacity building for all advisory system professionals, to comprehend the various dimensions of the producers' learning processes and to take into consideration producers' strategic thinking

The chapter explores Colson’s sources of income under the Old Regime and how this was related to his position in society. Although he drew revenues from familial property he owned in Varennes and a variety of other investments, his main sources of income came from his position as legal advisor and financial administrator to the Ravary wine-making family and, above all, to the Longaunay family of nobles. The chapter focuses, in particular, on the complex relations linking him to the Marquise, Marquis, and Comte of Longaunay in his role of overseeing the exploitation of the family’s lands and seigniorial dues in Normandy and Berry and serving as point man in the many lawsuits in which the family found itself entangled. It also examines the wide variety of his other responsibilities for this family beyond finances, and his close relations with Roch Lemaigre, the local intendant living near the family’s possessions in Berry in the small town of Levroux.

In many fast-growing cities around the country, up to three- quarters of the land zoned for residential use is reserved for detached, single-family dwellings at suburban densities. This is both a climate justice and racial justice issue as it has the doubly negative impact of artificially constraining housing supply and driving up costs, forcing many lower and middle income families farther away from job centers and imposing on them long, costly, and carbon-intensive com- mutes. Single-family zoning was also used as an explicit tool to segregate the U.S. by race starting in the 1920s and, in the process, denied countless people of color access to home- ownership, the most powerful wealth-building tool available to U.S. families. This is a significant factor in the stark racial disparities in household wealth that we see today.This paper outlines the findings of a nationally cited report on single-family zoning released by the Seattle Planning Commission, which advises the City Council and Mayor on land use and housing policy and of which the author is a member. It also reviews a collaboration between the com- mission and a graduate research-based architectural design studio and seminar co-taught by the author. This collabo- ration re-envisions urban, single-family neighborhoods to be more equitable, sustainable and livable while engaging students in a national policy dialogue in the process. The results of the studio will advance the commission’s efforts to advise Seattle’s elected officials in revising public policy to be more aligned with the city’s climate and racial justice goals.

"Divorce is considered a form of family disintegration that leads to the demolition of the family and family pillars after its construction through the marriage contract and then the termination of all social ties between husband and wife and often between their relatives. Divorce rates have risen to frightening levels that threaten our Islamic societies. Among the most important causes of divorce in our society are the following: The failure of one or both spouses in the process of adapting to the other through the different nature of the spouses and their personalities, the interference of the parents, the lack of harmony and compatibility between the spouses, the bad relationship and the large number of marital problems, the cultural openness, the absence of dialogue within the family. Several parties have sought to develop possible solutions to this dangerous phenomenon in our society, including: Establishment of advisory offices to reduce divorce by social and psychological specialists, and include the issue of divorce within the educational and educational curricula in a more concerned manner that shows the extent of the seriousness of divorce and its negative effects on the individual, family and society, and the development of an integrated policy that ensures the treatment of the causes and motives leading to divorce in the community, as well as holding conferences. Scientific and enlightening seminars and awareness workshops and the need for religious institutions and their media platforms to play a guiding and awareness role of the danger and effects of divorce on family construction and society, and to educate community members about the dangers of divorce and the importance of maintaining the husband’s bond and stability. As well as reviewing some marriage legislation and regulations, such as raising the age of marriage and reconsidering the issue of underage marriage, which is witnessing a rise in divorce rates. Among the proposed solutions is the demand to withdraw the power of divorce from the man's hands and place it in the hands of the judge, to prevent certain harm to women, or as a means to prevent the frequent occurrence of divorce. The last proposition created a problem that contradicts the stereotypical image of divorce in Islamic law, for which conditions and elements have been set, especially since Islamic Sharia is the main source of personal status laws in most Islamic countries. Therefore, the importance of this research is reflected in the study of this solution and its effectiveness as a means to prevent the spread of divorce, and not deviate from the pattern specified for it according to Sharia."

This study aimed to review the first-time advisory experience of husbands during labor time of primigravida wife. Subjects and Method: A scoping review method was conducted in eight stages including (1) Identification of study problems; (2) Determining priority problem and study question; (3) Determining framework; (4) Literature searching; (5) Article selection; (6) Critical appraisal; (7) Data extraction; and (8) Mapping. The search included ScienceDirect, Wiley Online Library, ProQuest, and grey literature through Google Scholar search engine databases. The inclusion criteria were English/ Indonesian-language and full-text articles in peer-reviewed journals published between 2009 and 2019. A total of 543,111 articles were obtained by the searched database. After the review process, six articles were eligible for this review. The data were reported by the PRISMA flow chart. Results: Six articles from developed countries (Australia, Sweden, Singapore, Israel, and England) met the inclusion criteria with qualitative and quantitative (cross-sectional) studies. Three main aspects discussed were support, challenges, and psychological conditions of experience of husbands during labor time of pregnancy. Support of husbands was identified as physical and moral. Challenges faced by husbands included lack of preparedness, knowledge, and encouragement from health care professionals. Psychological conditions of satisfaction and concern were found in husbands’ transition to fatherhood. Conclusion: Responsibility, emotion, experience, and barrier of husbands are related to maternal health problems. Better involvement of fathers will be able to enhance better quality of relationships and family health through understanding, experience, and assistance, especially in the childbirth process. This paper describes the summer research experiences of a high-school chemistry and biology teacher and a middle-school science teacher at the Milwaukee School of Engineering (MSOE). The first project involved researching B Complex Vitamins at MSOE’s Center for Biomolecular Modeling, developing molecular models using rapid prototyping technology (Z Corp 3D Printer) and creating curriculum modules for teaching the role of B Complex Vitamins to chemistry students in public high schools. A Javascript/HTML for interactive and dynamic presentation for understanding of thiamine (Vitamin B1) via web was written and implemented with Jmol software. A visual inspection of the family of Vitamin B Complex created and the curriculum modules developed during this project provide exciting and effective learning avenues for students in biology and chemistry classrooms. The second project dealt with the study of bioheat transfer and its simulation using MATLAB, and creation of a curriculum module that illustrates heat transfer principles reinforced by implementing the simulation. The teachers enjoyed the interaction with their advisors and the positive environment for their professional development. Details of their projects and experiences along with the evaluation of the program are presented in this paper. The teachers were pleased to be involved in connecting physics, biology, engineering and math into real projects that will motivate the students in their classes to pursue careers in STEM fields.

The prevalence of inherited thrombotic syndromes in the general population (1 in 2,500/5,000) appears to be higher than that of inherited bleeding disorders. We have reviewed the problems of their diagnosis and propose a simple screening procedure. The most important candidates far. screening are patients with unexplained venous thromboembolism at ages ofless than 40 years, particularly when thrombotic episodes are recurrent.Screening must start from collectionof the clinical and family history of the propositus and from the exclusion of common acquired forms of thrombophilia. A negative family historydoes not exclude inherited thrombophilia, because the defects have oftena low penetrance and fresh mutationsmay have occurred in the propositi. The test chosen for laboratoryscreening of inherited thrombotic syndromes must be limited in number, easy todo and, more importantly, their results should be clinically relevent Which defects should be screened and what type of methodology should be used? Since a number of laboratories may still not have the facilities to perform protein C functional assays, they are advised to set up at least an immunoassay, since type I deficiencies are much more frequent than type II deficiencies. The tests included in the second step of the screening are aimed at detectingthe less common or less well established causes of thrombophilia, and should be carried out when the clinical history suggests the existence of inherited thrombophilia and yet the first step has failed to reveal any laboratory abnormality. Defective plasminogen activation can be evaluated by measuring plasminogen activator activity with the simple fibrin plate assay carried out before and after stimuli such as venous occlusion and/or DDAVP infusion. The parallel measurement of plasminogen activator inhibitor allows to distinguish cases of detective plasminogen activation due to high inhibitor levels. The measurement of heparin cofactor II should also be included in this battery of second-step screening tests.Using this screening procedure in95 propositi with juvenile venous thromboembolism, we have identified 7 kindreds with antithrombin III deficiency (5 type I and 2 type II) (7.5%),7 kindreds with protein C deficiency (1 type II) (7.5%), 5 kindredswith protein S deficiency (5%), 1 withhypoplasminogenemia (1%) and 1 with dysfibrinogenemia Milano II (1). Theremaining undiagnosed cases might bedue to as yet unidentified deficiencies or abnormalities of other antithrombotic mechanisms such as,for instance, endothelial thrombomodulin or the fibrinolysis enhancing property of the protein C-protein S system.

SSHAP convened a virtual roundtable of expert advisors on Friday 12 March 2021 to discuss the outbreak of Ebola in Guinea declared on 14 February 2021. At the time of writing (19 March 2021), there have been 18 cases (14 confirmed, 4 probable), 9 deaths (including 5 in the community; CFR 50%) and 6 recoveries. Six of the 7 first cases identified were from the family of the first case, a 51-year-old nurse from Gouecke who died in N’Zérékoré on 28 January. Vaccination was launched on 23 February, and as of 17 March, 3,492 people had been vaccinated. The last new case was reported on 4 March 2021.

In Kenya, although 45 percent of maternal deaths occur within the first 24 hours after childbirth and 65 percent of maternal deaths occur during the first week postpartum, health-care providers continue to advise on a first check-up six weeks after childbirth. The early postpartum period is also critical to newborn survival, with 50–70 percent of life-threatening newborn illnesses occurring in the first week. Yet most strategies to reduce maternal and perinatal morbidity and mortality have focused on pregnancy and birth. In addition to the heavy workload of providers who do not assess the mother post-delivery when she may bring her infant for immunization, lack of knowledge, poverty, cultural beliefs and practices perpetuate the problem. The only register that exists for mothers post-delivery is for family planning, thus perpetuating the lack of emphasis on the early postpartum period with no standardized register to record care given. To address this gap in service delivery, the Population Council defined the minimal services a mother and baby should receive from a skilled attendant after birth. As stated in this brief, the development of a standardized postpartum register is one step toward advocating for providing early postpartum care among health-service providers.
