The chapter describes the study by Wright and colleagues examining the impact of aggressive cancer care on bereaved family members’ perceptions of end-of-life care. Family members of patients with advanced-stage lung or colorectal cancer were interviewed after their loved ones died to elicit perceptions of care. This chapter presents family-member reported rating of quality of end-of-life care and the association of such rating with aggressive cancer care, including intensive care unit admission within 30 days of death, and no hospice or late hospice admission. It also reviews family members’ perception of whether patients received end-of-life care that was concordant with their wishes. This chapter highlights the downstream impact of aggressive cancer care on family members and the resulting implications for end-of-life care.

Advance care planning is a complex process whereby an individual reflects on future care options at the end of life after reflecting on his or her values and goals for care. These values, goals, and preferences are then communicated to key stakeholders in the process (i.e., proxy and surrogate decision-makers, family members, and health care providers). It is unclear how well the completion of advance directives and a written outcome of advance care planning affect desired patient outcomes. In this chapter, a critical review is provided of a mortality follow-back survey that evaluates the association of advance directives with quality of end-of-life care from the perspective of bereaved family members. Study strengths and limitations are described, as are directions for future research.

This chapter focuses on new technology and its impact on acting as an occupation. It begins by describing how the advent of film transformed patterns of employment in the commercial entertainment industry. Returning to the theme of cultural hierarchy, it goes on to argue that even as the legitimate theater drifted toward the periphery of the nation's cultural life, the old theatrical elite continued to claim the right, through the mechanism of the Actors' Equity Association (AEA), to speak for the entire acting community. After examining working conditions in the motion picture studios, it turns its attention to the Equity campaign to organize the film industry, asserting that its architects were less concerned with negotiating a standard contract than with imposing their authority upon the men and women of the silver screen. The chapter argues that an overwhelming majority of motion picture actors reacted with hostility to what they saw as the AEA's attempt to “Broadwayize” Hollywood, interpreting it as a threat to their collective autonomy and a denial of the specificity of their work. By refusing to obey the strike call in the summer of 1929, they were declaring their independence from the traditions of the legitimate stage.

Migraine is one of the most common, disabling neurological disorders in women of reproductive age. It is a disabling condition that can lead to a compromised health-related quality of life. Often `migraineurs' are unable to carry out everyday tasks due to a reduction in functioning and productivity. This burden impacts not only at work, but also on social and family life. Migraine affects not only the physical but also mental and social health. Chapter 5 discusses the available evidence of a correlation between migraine and pre-eclampsia. Pre-eclampsia complicates 3-5% of all pregnancies and remains a leading cause of maternal and perinatal morbidity, and mortality. Although the primary mechanisms of both migraine and pre-eclampsia are not yet well understood, they do share some common pathogenetic aspects. However, studies investigating the clinical association between migraine and pre-eclampsia are sparse. The majority suggest a close association between headaches and hypertensive disorders of pregnancy: gestational hypertension, pre-eclampsia, and eclampsia. There also appears to be a significant increase in the incidence of severe pre-eclampsia in women suffering from migraine. Clinical manifestations of both migraine and pre-eclampsia appear to result from an interaction of the mind and body. A relation with stress has been identified. Further robust research is needed to elucidate the psychosomatic contributions to the pathogenesis of migraine and pre-eclampsia, and the clinical application of their relationship in improving materno-fetal health.

The book “Arca Project: Solidarity strengthening dreams and hopes in Altaneira-Ceará” describes the history of two Non-Governmental Organizations (NGOs), known as Associação Raízes Culturais de Altaneira (ARCA) and Fundação Educativa e Cultural Arca (Fundação Arca), linked especially by the spirit of solidarity. While the Association, formed mainly by family farmers, works with income generation and solidarity economy projects, the Arca Foundation develops projects in line with local and regional education and culture. The work begins by portraying the path of Carlos Alberto Tolovi in a social project of struggle for housing in an old favela that was located near the center of São Paulo, when he served as a seminarian and also as a priest, trying to show how much this his path contributed to the formation of the Arca Project. After describing how ARCA has established, the author continues to present the evolution of the work carried out by the Association and the context that subsequently led to the creation of the Arca Foundation. The narrative of the history of these two non-profit entities, known together as Projeto Arca, refers to the discussion of key concepts that inspire collective work, the questioning of the reality of social injustices and inequalities that characterize capitalist society and represents the hope of a culture, where the dignity of human life and social relations are more important than the economic one, aimed mainly at maximizing individual profit.

Chesterfield Smith was one of the boldest lawyers of the twentieth century. A child of a poor, broken household but also a child of a politically connected family, Smith grew up aimless. His World War II combat experience changed him. He returned an ambitious and impatient man who had learned from the European theater what systemized hate and prejudice could do. Smith rose fast, building his small firm to a goliath, leading the Florida Bar, and masterminding the creation of a new state constitution. As president of the American Bar Association during Watergate, his was one of the earliest voices calling for Nixon to obey the law or resign. At home, Smith urged his lawyers to improve the practice of law, and the world around them, by “doing good.” Smith’s larger-than-life personality and drive to improve his surroundings irritated some and inspired many.

Catecholaminergic polymorphic ventricular tachycardia (CPVT) is a rare inherited disorder associated with syncope and sudden death manifesting in the young during sympathetic activation. The electrocardiogram is normal and the heart is structurally normal. The diagnosis is usually made with an exercise stress test that shows a typical pattern of onset and offset of adrenergically induced ventricular arrhythmias. Molecular screening of RyR2, the major CPVT gene, is recommended whenever the suspicion of CPVT is high. If a disease-causing mutation is identified, cascade screening allows pre-symptomatic diagnosis among family members. All affected subjects should be treated with beta blockers (nadolol or propranolol). Preliminary data support the association of beta blockers with flecainide. After a cardiac arrest, an implantable cardioverter defibrillator (ICD) should be implanted, but it is accompanied by a disquietingly high incidence of adverse effects. After syncope on beta blocker therapy, left cardiac sympathetic denervation is most effective, preserves quality of life, and does not preclude a subsequent ICD implantation.

Impulse control disorders (ICDs) are common disabling disorders that have impulsive behavior as a core feature. They emerge early in life and run a chronic lifelong course. They are assumed to lie at the severest end of a continuum of impulsivity that connects normal with pathological states. People with ICDs experience a drive to undertake repetitive acts. Although the consequences are damaging, performance of the impulsive act may be experienced as rewarding, or alternatively may relieve distress, implicating dysfunction of the neural circuitry involved in reward processing and/or behavioral inhibition. Clinical data are increasingly pointing toward an etiological association between some ICDs, such as pathological gambling and addiction, and others, such as trichotillomania and compulsive disorders. Comorbidity with other psychiatric disorders is also common, and hints at overlapping psychobiological processes across several diagnostic groups. The results of neurocognitive studies suggest that impulsivity is multidimensional and comprises dissociable cognitive and behavioral indices governed by separate underlying neural mechanisms. For example, trichotillomania may primarily involve motor impulsivity, whereas problem gambling may involve reward impulsivity and reflection impulsivity. Exploring neurocognitive changes in individuals with ICDs and other mental disorders characterized by poor impulse control, and among their family members, may help to elucidate the underpinning neurocircuitry and clarify their nosological status.

Using Analytical Psychology as a theoretical basis, the present study aimed at clarifying and understanding the meanings that the man, who does not work, nor has an income of his own, attributes to himself, to his situation and to the social expectations related to working. Another objective was to elucidate which would be, in that case, the existing factors of investment and/or disinvestment in the work. Therefore, the study explored subjective aspects, using the qualitative approach and employing the Life History interview as a research tool. The research included four participants living in the state of São Paulo, Brazil. The inclusion criteria required that the participants be men, in the age group of approximate 30 years, without any paid work nor any type of income for at least five months, and financially dependent on their family members or spouses. The results show that the perception of work is an elementary configuration in the life trajectory. Work may signify a constant obligation — an imposition that endures — or be a meaning in transformation — leading to resignifications. The association between work and identity affirmation — as well as conscious and unconscious motivations — stands out. The research also made it possible to infer the existence of complexes resulting from the work experience. The survey of the subjective experiences linked to an increasingly prevailing conjuncture in the current society points to the intense affective load related to work. In this context, the assistance of the clinical psychologist becomes relevant.

In the mid-twentieth century the social sciences significantly expanded, and played a major role in shaping British intellectual, political, and cultural life. Central to this shift was the left-wing policy maker and sociologist Michael Young. In the 1940s Young was a key architect of the Labour Party’s 1945 election manifesto, Let Us Face the Future. He became a sociologist in the 1950s, publishing a classic study of the London working class, Family and Kinship in East London, with Peter Willmott in 1957, and the 1958 dystopian satire, The Rise of the Meritocracy, about a future society in which status was determined entirely by intelligence. Young also founded dozens of organizations, including the Institute of Community Studies, the Consumers’ Association, and the Open University. Moving between politics, academia, and activism, Young believed that the social sciences could help policy makers and politicians understand human nature and build better social and political institutions. This book examines the relationship between social science and public policy in left-wing politics between the end of the Second World War and the end of the first Wilson government through the figure of Michael Young. It shows how Young and other researchers and policy makers challenged Labour values like full employment and nationalization, and argued that the Labour Party should put more emphasis on relationships, family, and community. Showing that the social sciences were embedded in the politics of the post-war left, this book argues that historians and scholars should take their role in British politics and political thought seriously.

Teaching Moral Sex is the first comprehensive study to focus on the role of religion in the history of public sex education in the United States. It examines religious contributions to national sex education organizations from the late nineteenth century to the early twenty-first century, highlighting issues of public health, public education, family, and the role of the state. It details how public sex education was created through the collaboration of religious sex educators—primarily liberal Protestants, along with some Catholics and Reform Jews—with “men of science,” namely, physicians, biology professors, and social scientists. Slominski argues that the work of early religious sex educators laid foundations for both sides of contemporary controversies regarding comprehensive sexuality education and abstinence-only education. In other words, instead of casting religion as merely an opponent of sex education, this research shows how deeply embedded religion has been in sex education history and how this legacy has shaped terms of current debates. By focusing on religion, this book introduces a new cast of characters into sex education history, including Quaker and Unitarian social purity reformers, the Young Men’s Christian Association, military chaplains, the Federal Council of Churches, and the National Council of Churches. These religious sex educators made sex education more acceptable to the public and created the groundwork for recent debates through their strategic combination of progressive and restrictive approaches to sexuality. In sickle cell anaemia (SCA) a single mutation in the haemoglobin beta-globin gene is responsible for a pleomorphic phenotype leading to acute and chronic life-threatening complications. Healthcare management programmes, patient and family education, infection prophylaxis (especially in childhood), and long-term treatment for some patients (such as hydroxyurea) have significantly improved survival, giving rise to some new long-term issues.Sickle cell-associated nephropathy (SCAN) leads in some cases to chronic renal failure with a significant impact on survival. SCAN is characterized by an increased effective plasma renal flow and glomerular filtration rate, glomerular hypertrophy, and damaged vasa recta system leading to albuminuria and impaired urinary concentration.Early onset of hyperfiltration occurs in 60% of SCA patients often associated with microalbuminuria. SCAN risk factors are still under investigation, but may be related to chronic haemolysis at an early time point. Other lesions in patients with sickle cell anaemia include papillary necrosis, and recurrent acute kidney injury in association with crises or infections.ACEI are recommended if there is proteinuria. There is no current agreement on whether angiotensin-converting enzyme inhibitors (ACEI) should be introduced earlier, but systematic screening for microalbuminuria and hypertension, and avoidance of nephrotoxic agents are strongly advised.Patients with sickle cell trait (carriers for sickle cell anaemia) are prone to microscopic haematuria and abnormalities of the vasa recta have been described. A very rare tumour, renal medullary carcinoma, is largely restricted to this group (in whom it is still extremely rare). Increased risk of other renal problems is still largely hypothetical rather than proven.The prevalence of nephropathies in other sickle cell diseases (in particular haemoglobin SC disease) is much lower.

Abstract:
Born in Carthage, North Carolina, Lucean Arthur Headen (1879-1957) grew up amid former slave artisans. Inspired by his grandfather, a wheelwright, and great-uncle, a toolmaker, he dreamed as a child of becoming an inventor. His ambitions suffered the menace of Jim Crow and the reality of a new inventive landscape in which investment was shifting from lone inventors to the new “industrial scientists.” But determined and ambitious, Headen left the South, and after toiling for a decade as a Pullman porter, risked everything to pursue his dream. He eventually earned eleven patents, most for innovative engine designs and anti-icing methods for aircraft. An equally capable entrepreneur and sportsman, Headen learned to fly in 1911, manufactured his own “Pace Setter” and “Headen Special” cars in the early 1920s, and founded the first national black auto racing association in 1924, all establishing him as an important authority on transportation technologies among African Americans. Emigrating to England in 1931, Headen also proved a successful manufacturer, operating engineering firms in Surrey that distributed his motor and other products worldwide for twenty-five years. Though Headen left few personal records, Jill D. Snider recreates the life of this extraordinary man through historical detective work in newspapers, business and trade publications, genealogical databases, and scholarly works. Mapping the social networks his family built within the Presbyterian church and other organizations (networks on which Headen often relied), she also reveals the legacy of Carthage's, and the South's, black artisans. Their story shows us that, despite our worship of personal triumph, success is often a communal as well as an individual achievement.
