Abstract:

The purpose of this thesis was to write a grant to obtain funding for a comprehensive transitional patient care program and family caregiver support services. Through process analysis, the grant writer developed the Next Step Hospital to Home Caregiver Support Program (NSHCP). This program was developed to bridge the gap in the delivery of services of transitional patient care and family caregiver support services.

California Healthcare Foundation (CHCF) was identified as an appropriate funding agency. Their focus on ideas and innovations that improve quality medical care, increase efficiency, and lower the costs of medical care is compatible with the mission of the NSHCP. In collaboration with CHCF and Anaheim Regional Medical Center, the grant writer developed the NSFCP in an effort to provide quality transitional care and caregiver support services. The actual submission and/or funding of this grant was not a requirement for the successful completion of this project.

Abstract:
It was unclear what the actual role of the Family Readiness Group (FRG) was in helping the spouses of U.S. Navy submariners (SMSs) in learning to live the submariner-family lifestyle. Submarines deploy in regular cycles regardless of world conflict. Submariners and their spouses are isolated from each other during deployments, communities of submariner spouses are smaller than other Navy communities, and spouses must acquire unique social capital to manage unique challenges. The purpose of this study was to explore how SMSs experience or perceive the FRG role in their social construction and adaptation to the SMS lifestyle. The examination was guided by Ingram, Schneider, and DeLeon's social construction and policy design theory. Data were collected using an open-ended survey distributed to 83 SMSs through an online survey platform. Data were coded for themes and subthemes using an iterative process including values and process coding. Key results were that SMSs construct themselves differently than how they are constructed by policy principals. Among SMSs, benefits and burdens perceived to be distributed by the FRG program are different than the distribution of actual benefits and burdens. These differences influenced participants' engagement with the FRG program. More research is needed to define this influence and to explore the origins of relationships that increase lifestyle capital. The implication for social change is that a better understanding of the nature of SMS lifestyles can contribute to better policy decisions and improved program design, leading to better outcomes for military spouses.

Haber, Meirav. "Program Evaluation of a Pilot Project Using the Family Art Assessment to Support Clinical Treatment." Digital Commons at Loyola Marymount University and Loyola Law School, 2012. https://digitalcommons.lmu.edu/etd/101.

Full text
Abstract:
This study explores the use of Helen Landgarten’s family art assessment in helping to inform therapists’ work with families. The study endeavors to evaluate a pilot project in which director of the Helen B. Landgarten Art Therapy Clinic, Dr. Paige Asawa, MFT, ATR-BC, implemented the Landgarten family art assessment at a local clinic with five families and involved family track clinicians through observation and discussion to inform their clinical treatment of families. Through program evaluation using a survey and an art-based research procedure, the study investigates family track clinicians’ experiences, recommendations, and opinions of the family art assessment. Ten family track clinicians participated in program evaluation in this study. A quantitative and qualitative analysis of participants’ survey answers and art responses served to solidify whether these clinicians found the family art assessment project beneficial to their clinical work with families. A synthesis of the literature, survey analysis, and art analysis reveals the value of having art therapists conduct family art assessments as an informative consultation for clinicians assessing and treating families. Study results may contribute to more formal inclusion of the family art assessment in assessment procedures at this local clinic. These results hold valuable implications for redefining the role of the art therapist as assessment expert, using art therapy to promote collaboration among mental health professionals, and ultimately improving the quality of clinical family care.

Ona, Samsiya. "Evidence-Based Preventive Healthcare in the CWB Family Support Homes: The Healthy Learners Pilot Program." Thesis, Harvard University, 2015. http://nrs.harvard.edu/urn-3:HUL.InstRepos:17295881.

Full text
Abstract:
Zambia is one of the poorest countries in the world. The school aged population experiences high mortality and morbidity rates from malnutrition, chronic and acute infections. Our long-term goal is to develop an ongoing preventive health program to supplement the CWB educational mission. The pilot program included health screenings, detailed health status and needs survey, interviewing local experts and key stakeholders and a health promotion workshop for the schoolteachers. Follow-up screenings were also conducted in Summer 2013. We screened 455 children and surveyed 223 children with their guardians during the pilot study. The most common diagnoses were GU infections, URTIs, diarrhea and fungal infections. Among the 455 children screened, there were 51 cases of S. haematobium infection. A striking finding from the health status survey is the lack of knowledge on health and sanitation topics and the urgent need to integrate health and nutrition education in the school curriculum. The teachers’ health promotion workshop was effective in increasing teachers’ health literacy. The HLP pilot helped initiate a program to provide school-based healthcare to children with otherwise minimal access to care and to train their teachers in important basic public health topics.

Sewell, Margaret G. "Parenting stress and program support acceptance among Mexican American Head Start mothers." Diss., The University of Arizona, 2004. http://hdl.handle.net/10150/280749.

Full text
Abstract:
This study examined conditions under which low income mothers engage in voluntary supportive relationships with community-based early childhood programs. Parenting stress and support processes were examined in a population of low-income Mexican American Head Start mothers. A new construct, program support acceptance, describes mothers' appraisal of the program as a potential source of parenting support. Research questions related to (a) the relationship between Time 1 (fall) parenting stress and mid-year program support acceptance, (b) the relationship between mid-year program support acceptance and Time 2 (spring) parenting stress, and (c) the contribution of program support acceptance to longitudinal change in parenting stress. The study also considered contextual variables (acculturation, maternal education, stressful life events, partner status, and extended household) as predictors of parenting stress and program support acceptance, and potential moderation of parenting stress and support linkages by acculturation. Finally, the study considered the theoretical issue of whether parenting stress serves to motivate or inhibit support-seeking (reflected in program support acceptance) as a coping behavior. The guiding theoretical framework for the study was based on Abidin's parenting stress model (1983, 1992), and Lazarus and Folkman's general model of stress, appraisal and coping (1984), as well as family support literature based in ecological systems theory. Mean levels of parenting stress declined significantly from Time 1 to Time 2 for mothers in the Head Start program. The contextual variables did not significantly predict Time 1 parenting stress, and marginally predicted program support acceptance. Higher Time 1 parenting stress scores significantly predicted lower levels of mid-year program support acceptance. Higher program support acceptance scores at mid-year significantly predicted lower Time 2 parenting stress in bivariate analysis, but dropped to non-significance after controlling for Time 1 parenting stress. Acculturation did not moderate linkages between parenting stress and program support acceptance. However, several relationships were significant for immigrant generation mothers which were not significant for later generation mothers. In the immigrant group, being partnered predicted higher program support acceptance, and program support acceptance contributed significantly to lower Time 2 parenting stress. Higher maternal education level was associated with lower Time 2 parenting stress.

Abstract:
The goal of this study was to learn more about the experience of Canadian military fathers who participated in a fathers' support group entitled "With Dads Around, Kids Are Sound." An explorative, semi-structured, qualitative design was applied to gain understanding of the participants' perspectives. The sample consisted of seven military fathers. The researcher also interviewed three wives in an attempt to corroborate some of the findings. The two main reasons that brought fathers into the group were role conflict and wanting to prevent family problems. Based on comments received, the program was found to be effective in supporting military fathers. Key elements noted by participants include normalization, peer support, and learning ways to prevent dysfunctional family patterns. Recommendations to improve the program include making it a continuous opportunity---including a specific chapter on military lifestyle---and being more flexible with the content to allow for more peer support-type discussion. Implications for social work practice, policy, and research are also discussed.

Cook, Natalie E. "Aligning Cultural Responsiveness in Evaluation and Evaluation Capacity Building: A Needs Assessment with Family Support Programs." Thesis, Virginia Tech, 2016. http://hdl.handle.net/10919/64424.

Full text
Abstract:
Family support programs serve vulnerable families by providing various forms of support, such as education, health services, financial assistance, and referrals to community resources. A major feature of evaluation involves assessing program effectiveness and learning from evaluation findings (Mertens and Wilson, 2012). Collaboration and cultural responsiveness are important topics in evaluation which remain largely distinct in the literature. However, evaluation capacity building provides a context for exploring possible intersections. Data about seven programs were collected via semi-structured interviews and document analysis. This study revealed that the program leaders feel that their programs are unique, complex, and misunderstood. The findings also suggest that program leaders believe that evaluation is important for program improvement and funding. Although participants did not anticipate evaluation capacity building and did not readily express a desire to develop their own evaluation skills, participants from all seven programs enthusiastically expressed interest in evaluation capacity building once explained. Although participants did not discuss cultural responsiveness as it relates to race, they expressed a need to overcome a community culture of reluctance to participate in programs and aversion to educational pursuits. Given the programs' shared population of interest, similar outcomes, and common challenges, evaluation capacity building in a group setting may give Roanoke family support program leaders the evaluation knowledge, skills, and peer support to engage in program evaluation that is both collaborative and culturally responsive.

Full text
Abstract:
This quasi-experimental study compared family formation and perceived partner support among pregnant adolescents in a prenatal care program. Participants were assigned to either an intervention group utilizing centering pregnancy (CP) prenatal care and case management, or to a comparison group receiving case management only. Partners were invited to participate in CP group sessions. This study included 173 predominantly minority pregnant adolescents ages 15–18 years who were enrolled in a prenatal program and followed one month postpartum. Family formation included living and relationship arrangements. Perceived partner support included six domains of perceived social provisions. Data were collected through participants’ self-reports using computer-assisted self-interviews. Changes in family formation and perceptions of partner support from baseline to postpartum did not differ between intervention and comparison groups. Male partners who attended at least one CP session were perceived as more supportive at both the beginning and end of the program than partners who did not attend any sessions. After combining groups, pregnant adolescents reported a significant shift in family formation and increased monetary support from partners from baseline to postpartum. Partner support is important for ensuring positive pregnancy outcomes. Additional strategies are needed to engage young fathers who do not readily provide support during pregnancy.

Abstract:

The purpose of this project was to design a family support program, identify potential funding sources and write a grant to fund a program to reduce child maltreatment recurrence (CMR) for LIFT a non-profit agency located in Los Angeles, California. Research indicates risk factors such as neglect, poverty, single parent households, parental substance use and history of child welfare services, place children at increased risk of CMR and that comprehensive interventions are needed to address the multiple issues associated with CMR. The proposed program aims to provide supportive services to help families achieve greater economic self-sufficiency, leading families to be able to provide for the basic needs of their children and reducing the likelihood of CMR. Actual submission and/or funding of the grant was not a requirement for the completion of this project. Implications for social work practice are discussed.

Digital Commons at Loyola Marymount University and Loyola Law School, 2018. https://digitalcommons.lmu.edu/etd/492.

Full text
Abstract:
A qualitative study of the experiences and observations of 4 art therapists and 2 program directors who facilitated Camp Suzanne, a week-long art-based therapeutic program for incarcerated mothers and their children in a federal prison in California. Research on psychotherapy, art therapy, and family therapy in prison environments, with a focus on parent-child dyads, Attachment Theory, and various techniques for creating sustainable therapy with separated family units, including tele-mental health and evidence-based military protocols, informed the interviews. The research participants were interviewed individually and created art regarding the subjects of Attachment Theory with incarcerated-mother-child dyads and longevity considerations for the program. Emergent themes in the data included the impact of art-making on attachment and a variety of observable attachment styles, as well as obstacles to both attachment and longevity of Camp Suzanne. Some of the obstacles addressed include systemic challenges, continuity of care, location concerns, external support (for facilitators and for incarcerated-mother-child dyads), as well as preparatory support (psychoeducation). Various implications of these obstacles are discussed.

Full text
Abstract:
The Family‟s Health Support Centers were created in 2008 to broaden the coverage, scope and solvability of Basic Attention‟s programs. Ten years after its creation, monitoring mechanisms, measurement of programs and results achieved by NASF are still scarce. The National Improvement Program of Access and Quality of Basic Attention (PMAQ-AB), launched in 2011, aims to motivate managers and health teams to improve quality of services offered to SUS‟ users and to strengthen Basic Attention. The goal of this research was to analyze the organization and the professional practice of NASF in Brazil, based on data collected in the second cycle of PMAQ-AB (2013-2014). To this quantitative and descriptive study, we used the secondary database from the external evaluation of AB teams that participated in PMAQ-AB. Two instruments were used for the external evaluation: 1) Attention Basic teams (Family‟s Health and Parameterized Teams); and 2) Family‟s Health Support Centers – NASF. From the first, we picked three domains from the II module: adjustment of team‟s composition to the territory needs; access from teams to NASF; and articulation of actions from technical-pedagogical support, as well as clinical-care. From the second, the following domains: Perennial education; organization of matrix support to the EAB; and demand and shared attention management. For data analysis we used SPSS version 22.0. Results pointed to the insertion of new professionals at AB, especially Physical Therapist, Physical Education professional and Veterinarian. It was evident the lack of joint action planning between NASF and eSF, although literature references many difficulties in this activity. The main points of contact between teams: in scheduled days or thought the phone, since e-mail was barely used. Among shared activities, residential visits and individual appointments were highlights for eSF. To NASF professionals: discussions of cases and visits with team professionals. Discussions about monitoring and evaluation of results from shared attention had a small impact in the team‟s routine. Target audience supported by NASF: adults and senior citizens, especially activities designed to nutritional attention and people with chronic diseases.

The study aimed to develop an individualized digit-based family support program and to evaluate its feasibility. A case study method was adopted. Eight parents having young ASD children were involved. Questionnaires, archival records, and telephone interviews were utilized for data collection. As a result, a family support program, which was digit-based and individualized, was developed. The program consisted of eight systems with four elements interwined. The eight systems were the contents-building system, the management system, the self-paced-learning system, the data-linking system, the searching system, the guiding system, the program-evaluation system, and the self-amending system. The four elements were the administrator, the webmaster, learners, and the hardware of the website. Five groups of contents provided by the program, including the information regrading ASD and ASD children, instruction strategies and related issues, behavioral management, supporting service resources, and referral, respectively. The contents were selected based on the eight participants’ needs. The referal information and supporting service resources information were most desirable, the behavioral management information the least. For evaluating the individualization of the program, the following four indices were chosen: being user-determined, suiting user’s convenience, providing multiple choices, and being adjustable. Regarding the feasibility of this program, a rather acceptable outcome was preliminarily found. The program was evaluated by its form, contents, conformance to the individualization, and effects on the participants and their ASD children. Both the form and the contents of the program were appreciated by all the participants. The individualization were conformed well at two of the indices-suiting user’s convenience, being adjustable, but only partially at the other two indices—providing multiple choices, being user-determined. However, the effects of the program on the participants and their children were found limited, probably, due to the short-period implement of the program. Nevertheless, some of the participants indicated that they gained improvement on knowledge or skills because of the program and that they had applied the strategies, learned from the program, in managing their children’s behavioral problems and received good results.

Using a Family-Centered Positive Behavior Support Program to Reduce Noncompliance Behaviors of A Student with Autism Spectrum Disorder Abstract The purpose of this action research was to develop and understand the implementing process and effects of the family-centered positive behavior support program designed for reducing noncompliance behaviors of a student with autism spectrum disorder. The researcher and the mother of the participant developed and implemented the program in a collaborative partnership. By conducting functional assessment and family assessment, the researcher and the parent got the information to design a contextually appropriate positive behavior program. The data were collected by observation, interviews and checklists for the researcher to know the changes of the noncompliance behaviors and positive behaviors, and the growth of both the parent and the researcher. The conclusions of this study were as follows: 1. The family-centered positive behavior program could reduce the rate of noncompliance behaviors for the participant with autism spectrum disorder. 2. The family-centered positive behavior program could help the participant with autism spectrum disorder to increase positive behaviors. 3. The development and implementation of the research could comply with the principles of the family-centered positive behavior. 4. The family-centered positive behavior program could contribute to a positive change in the attitude of the parent and the researcher’s personal growth. According to the findings and conclusions, suggestions were raised for parent, teachers, and future study. Weatherspoon, Joni. "Analysis of high-risk hispanic families in a family support program : characteristics, factors affecting parenting skills over time, and age-paced newsletter efficacy." Thesis, 2004. http://hdl.handle.net/1957/31097.

Full text
Abstract:
Two studies investigated the characteristics of high-risk Hispanic parents with young children who were enrolled in a primary prevention home visitation program targeting first birth families. In the first study, more and less acculturated high-risk Hispanic families were compared to high-risk non-Hispanic White families on differences in demographic characteristics, risk factors associated with poor child outcomes, and parent behaviors related to infant health. Using the General Linear Model Univariate procedure, these characteristics were then related to level of parenting skills, measured after approximately one year of service. A total of 1005 families were included in the study. There were important differences in parent psychosocial characteristics, health risks, and outcomes among Hispanic families. Hispanic mothers who were more acculturated, as measured by use of English in the home, were more likely to be at risk for substance abuse, mental illness, and family conflict. They were more likely to smoke and less likely to breastfeed. Less acculturated Hispanic mothers were less educated and more likely to be isolated. Controlling for pretest scores and education level, there were significant between group differences on parenting skills after one year of service, however the effect was small and overshadowed by the significant interaction effect between partner status and household category. Scores for single Spanish-speaking Hispanic mothers were significantly lower than scores of Spanish-speaking Hispanics in partner households. Risk factors that were positively associated with program progress included breastfeeding and mother's education level. Smoking, mental illness, and unrealistic expectations for the child were negatively associated with program progress. The second paper addressed the efficacy of an age-paced parenting newsletter as part of a home-visiting program for Spanish-speaking Hispanic parents. Family Service Workers serving these families delivered the newsletter and used it as a curriculum guide and teaching tool for families. Parents reported reading the newsletter, learning from it, and changing their behavior in response to newsletter content. Home visitors reported that it was a useful tool to reach high-risk parents with important parenting information. They found that it was particularly useful to combat inappropriate or unhealthful parenting advice from well-meaning family members and friends.

Abstract:
Home First is an Ontario transition management approach that attempts to reduce the pressure on hospital and Long Term Care (LTC) beds through early discharge planning, the provision of timely and appropriate home care, and the delay of LTC placement. The purpose of this qualitative descriptive study was to obtain descriptions from South Eastern Ontario Home First clients and their family caregivers of their experiences with and thoughts about care transitions, the provision of care, and the Home First approach. The goal was to enable insight into the Home First approach, care transitions, and the provision of care through access to the perspectives of study participants. Nine semi structured interviews (and one or more follow-up calls for each interview) with Home First clients discharged from hospitals in South East Ontario and their family caregivers were conducted and their content analyzed. All participating Home First clients were pleased to be home from hospital and did not consider LTC placement a positive option. All had family involved with their care and used a mix of formal and informal services to meet their care needs. Four general themes were identified: (a) maintaining independence while responding (or not) to risks, (b) constraints on care provision, (c) communication is key, and (d) relationship matters. Although all Home First clients participating in the study were discharged home successfully, a sense of partnership between health care providers, families, and clients was often lacking. The Home First approach may be successfully addressing hospital alternative level of care issues and getting people home where they want to be, but it is also putting increasing demands on formal and informal community caregivers. There is room for improvement in how well their needs and those of care recipients are being met. Health professionals and policy makers must ask caregivers and recipients about their concerns and provide them with appropriate resources and information if they want them to become true partners on the care team.
