Full textSoft tissue sarcomas are mesenchymal origin neoplasms collectively classified according to histological characteristics and biological behaviour similiarities. Various neoplasms are included in this major group, as fibrosarcoma, hemangiosarcoma, peripheral nerve sheath tumor, myxosarcoma, liposarcoma, leiomyosarcoma, rhabdomiosarcoma, malignant fibrous histiocytoma, synovial cell sarcoma and undifferentiated sarcoma. The microvessel density measure has been applied to investigate tumor angiogenesis in many neoplasms. The objectives of this study were to evaluate de microvessel density in canine and feline soft tissue sarcomas and compare the general vessel measurement area and intense vascular areas hot spot. Those data were compared to usually applied prognostic factors like mitotic index, necroses presence and amount and cellular differentiation. Soft tissue sarcomas were collected in Laboratório Regional de Diagnóstico da Universidade Federal de Pelotas from 1978 to 2008 among canine and feline necropsies and biopsies. In a total of 1668 neoplasms cases during this period, 100 were soft tissue sarcomas, 87 canine and 13 feline. Sex prevalence was not observed in the analyzed species. The major prevalence in canine were hemangiosarcomas (n=34) and fibrosarcomas (n=20), being the mongrel dogs (n=35) most. The large breeds presented major frequence (n=22), followed by medium size (n=16) and small sized breeds (n=9). . In feline population, there were not accentuated prevalence, exciding hemangiosarcomas (n=4) and fibrosarcomas (n=4). In those 100 cases, 39 paraffin blocks were obtained, being 36 canine and 3 feline, mostly mongrel specimens, with 9,21(±2,99) medium age. The general vascular and hot spot areas were significantly correlated (r2=0,98; p<0,01). The mitotic index between areas were in those areas (r2=0,95, p<0,01). Hemangiosarcomas presented major vascular media in the techniques, followed by muscular sarcomas and fibrosarcomas (p<0,05). The microvessel density in soft tissue sarcomas do not present correlation to other prognostic factor usually applied. Hot spot areas can be utilized to determine the vascular degree and mitotic index in soft tissue sarcomas.
Os sarcomas de tecidos moles são neoplasmas de origem mesenquimal classificados coletivamente devido a características histológicas e comportamento biológico similares. Vários neoplasmas estão incluídos neste grande grupo, como fibrossarcoma, hemangiossarcoma, tumor de bainha de nervo periférico, mixossarcoma, lipossarcoma, leiomiossarcoma, rabdomiossarcoma, histiocitoma fibroso maligno, sarcoma sinovial e sarcoma indiferenciado. A mensuração da densidade vascular tem sido utilizada para investigar a angiogênese tumoral em diferentes neoplasmas. Este estudo teve como objetivo a determinação da microdensidade vascular de sarcomas de tecidos moles caninos e felinos através da comparação entre a mensuração geral vascular e áreas de intensa proliferação hot spot. Estes dados foram comparados a fatores prognósticos usualmente empregados, como índice mitótico, presença e quantidade de necrose e diferenciação celular. Os sarcomas de tecidos moles foram resgatados dos arquivos do Laboratório Regional de Diagnóstico da Universidade Federal de Pelotas entre 1978 a 2008 dentre necropsias e biópsias caninas e felinas. Num total de 1668 neoplasmas deste período, 100 eram sarcomas de tecidos moles, sendo 87 caninos e 13 felinos. Não se observou prevalência de sexo nas espécies analisadas. Em caninos, a maior prevalência foi de hemangiossarcomas (n=34) e fibrossarcomas (n=20), sendo os sem raça definida (n=35) os mais acometidos. Os animais de porte grande apresentaram maior freqüência (n=22), seguidos pelos de porte médio (n=16) e de porte pequeno (n=9). Em felinos não houve uma prevalência acentuada, destacando-se os hemangiossarcomas (n=4) e fibrossarcomas (n=4). Destes 100, obtiveram-se blocos de 39 casos, sendo 36 caninos e 3 felinos, em sua maioria sem raça definida nas duas espécies, com idade média observada foi de 9,21(±2,99 anos). A mensuração vascular geral e de áreas hot spot apresentaram correlação significativa (r2=0,98; p<0,01). O índice mitótico nas duas áreas observadas foram significativas (r2=0,95, p<0,01). Os hemangiossarcomas apresentaram maior média vascular nas duas técnicas, seguidos pelos sarcomas musculares e fibrossarcomas (p<0,05). Houve diferença entre os hemangiossarcomas e os fibrossarcomas, e os sarcomas musculares não diferiram desses (p<0,05). A microdensidade vascular em sarcomas de tecidos moles não apresentou correlação com outros fatores empregados usualmente. A visualização de campos hot spot pode ser utilizada para determinar o grau vascular e índice mitótico.

Full textAdvanced soft tissue sarcoma (STS) patients have a dismal prognosis. Efforts to increase benefit from chemotherapy are needed and dose-dense chemotherapy could be an option, since this approach has demonstrated survival benefit in breast cancer. On the other hand, the identification of a prognostic marker is essential to stratify which patients could benefit most from intensified strategies. Ezrin is a member of the ERM (ezrin, radixin, moesin) cytoeskeleton-associated protein family associated with invasion and metastasis, and has been pointed as important prognostic marker in sarcomas. The objective of this study was to explore the role of dose-dense doxorubicin- and ifosfamide-based chemotherapy in advanced high grade STS patients. Primary endpoint was response rate and secondary endpoints were overall survival (OS), progression free survival (PFS), toxicity profile, quality of life and pain control evaluation. Tumor ezrin immunoreactivity was an exploratory endpoint as a predictor of response to chemotherapy and as a prognostic factor in this population, trying to find which patients could benefit most from this intensified strategy. This prospective, single arm, phase II study included 21 advanced STS patients. Median age was 37 years (23-60y) and lower limbs were the most frequent primary site. Synovial, leiomyo and unclassified sarcoma were the most common histologies. Protocol consisted of 6 cycles of sequential dose-dense doxorubicin 30 mg/m2 D1-3 and ifosfamide 2.5 g/m2 D1-5 every 14 and 21 days, respectively, followed by seven days of hematopoietic support. The median doxorubicin and ifosfamide dose-intensities were, respectively, 42 mg/m2/week and 3.63 g/m2/week (93% and 87% of planned, respectively) and 15 patients (71%) received all cycles. Grade 3/4 toxicities occurred in 19 patients and 77/105 cycles, febrile neutropenia in 5 patients (six cycles) and three LVEF drops of at least 10%, one symptomatic. No renal toxicity was observed what could occurred due to the two-hour-schedule of ifosfamide. Responses were evaluated by RECIST criteria and three patients presented partial response (response rate of 14%). Six responses were necessary to the inclusion of the target population, however, this was not observed and the study was closed. Three deaths were probably related to toxicity. After a median follow-up was 11 months, PFS and OS were 8.1 months and 20.1 months, respectively. Patients with synovial sarcoma and those younger than 45y presented better survival at univariate analysis. Ezrin expression was positive in 10 patients (47%) and a trend was observed for a correlation between positive ezrin expression and longer survival (p= 0.1191). There was a statistically significant correlation between positive ezrin expression and synovial hystology (Fishers exact test, p= 0.0325). This sequential dosedense doxorubicin/ifosfamide-based chemotherapy protocol was toxic, response rate was low in advanced STS patients and can not be considered for routine practice outside clinical trials

O NGF (fator de crescimento neural) é uma proteína endógena secretada no sistema nervoso central e periférico, desempenhando diversos papéis, tais como neuroproteção, desenvolvimento e diferenciação neuronal, crescimento de neuritos e plasticidade sináptica. Sintetizado no neocórtex e no hipocampo, o NGF atua em populações de neurônios colinérgicos, sensoriais e simpáticos através da interação com receptores TrkA e p75NTR. Está relacionado com neuropatologias como doença de Alzheimer, hiperalgesia e com as neuropatologias autonômicas e sensoriais hereditárias tipos IV e V, onde foram identificas as mutações S187N e R221W, respectivamente. Apesar do vasto conhecimento da ação das vias relacionadas ao NGF e seus receptores, os mecanismos moleculares de interação, bem como da ativação das sinalizações internas dos receptores, ainda não estão totalmente esclarecidos. Através da abordagem de análise de modos normais, foram investigadas as alterações estruturais dos mutantes em relação ao tipo selvagem de NGF sob a ótica de estrutura de proteínas. Diferenças significativas na correlação de movimentos globais, flexibilidade do complexo, interações moleculares e na liberdade conformacional em ambas as mutações foram encontradas. Foram observadas variações estruturais e limitações conformacionais nos receptores como reflexo da ação dos mutantes de NGF, indicando que, mesmo após a interação, neurotrofina e receptor formam um complexo ativo. Desse modo, as mutações no NGF reduzem a capacidade de formação do complexo ativado através de alterações estruturais e conformacionais, causando prejuízo às funções biológicas derivadas da interação.
NGF (nerve growth factor) is an endogenous protein secreted in the central and peripheral nervous system, playing different roles, such as neuroprotection, development and neuronal differentiation, neurite outgrowth and synaptic plasticity. Synthesized in the neocortex and hippocampus, NGF acts on populations of cholinergic, sensory and sympathetic neurons through interaction with TrkA and p75NTR receptors. It is related neuropathologies such as Alzheimer's disease, hyperalgesia and hereditary sensory and autonomic neuropathologies types IV and V, which were identified the mutations S187N and R221W, respectively. Despite the vast knowledge of the action of pathways related to NGF and its receptors, the molecular mechanisms of interaction as well as the activation of the internal signalling pathways of the receptors are still not fully understood. Through the normal modes analysis approach, the structural changes of the mutants compared to wild type NGF were investigated from the perspective of protein structure. Significant changes in global movements correlation, complex flexibility, molecular interactions and conformational freedom were found for both mutations. Structural changes and conformational limitations in the receptors were observed, as reflecting the action of NGF mutants, indicating that even after the interaction neurotrophin and receptor form an active complex. Thus, mutations in the NGF reduce the ability of activated complex formation by structural and conformational changes, causing impairment to biological functions derived from the interaction.

Full textIn adult individual, angiogenesis occurs particularly in pathological situations as tumors growth, development of metastasis and in the wound healing process. The vascular endothelial growth factor (VEGF) is the main agent involved in tumor angiogenesis. Therefore, the aim of the present study was to evaluate the circulating levels of VEGF in the serum of dogs suffering from soft tissue sarcoma and its relation with the blood cells, and also the expression of VEGF and the micro vascular density in tumors specimens. A group of health animals was used as control. The control group and treatment group were composed of 30 male dogs 25 dogs (18 males and 7 castrated females) respectively. The blood collection was carried once in the control group and three times in the animals with sarcoma (timely, pre-surgery, two weeks and three months post-surgery) following the same protocol. The blood was processed and a quantitative method ELISA (enzime-linked immunosorbent assay) was used to determinate of the levels of VEGF. The expression of the VEGF and the microvascular density were investigated by means of the immunohistochemical test using anti-VEGF and anti-factor VIII antibodies respectively. There was no difference between serum level of VEGF of the controls animals and serum level of VEGF in the animal with soft tissue sarcoma in pre-surgical time. Serum level of VEGF in pre-surgical time revealed discrete increased in relation the two weeks and three months of post-surgery. There was a positive correlation between serum VEGF and neutrophils counting and negative correlation between the VEGF and amount of haemoglobin in the animals with sarcoma. The animals with hemangiopericytoma showed a trend to higher levels of VEGF in relation to the malignant peripheral nerve sheath tumor. The expression of the VEGF was detected in 65% of the cases; the hemangiopericytoma is the one that expressed greater intratumoral amount of VEGF. There was no difference in the microvascular density between the negative and positive tumors to the VEGF. The results suggest contribution of the circulating blood cells for the levels of serum VEGF of the of the dogs with of soft tissue sarcomas, tumors cells and other cells types seem to be responsible for tumor angiogenesis, but they don\'t contribute for rise serum level of VEGF.

Full textINTRODUCTION: Surgery combined with adjuvant radiotherapy provides high rates of limb sparing and local control for adult extremity soft tissue sarcomas, but is still associated with high rates of local morbidity and distant recurrences. The role of adjuvant or neoadjuvant chemotherapy is still controversy and target of clinical investigations. The identification of molecular predictive factors of response to chemotherapy could select patients who have benefits or not with its use. OBJECTIVES: to identify gene expression profiles that discriminate tumors with respect to response to neoadjuvant chemotherapy. Analyze the preliminary results of a protocol of neoadjuvant chemotherapy in soft tissue sarcomas. METHODS: samples were collected from subjects of a single-arm prospective clinical trial that investigates the effectiveness of a neoadjuvant doxorubicin and ifosphamide-based chemotherapy regimen in high grade extremity soft tissue sarcomas in adults. Gene expression profiles were determined by the analysis of cDNA microarrays. RESULTS: 14 patients were included in the clinical trial and six samples were used in the molecular study. 222 sequences differentially expressed between responders and non responders were identified. Among the genes with higher differences in expression, we have identified genes involved with TGF signaling pathway, angiogenesis, extracelular matrix degradation and development. The objective response rate to neoadjuvant chemotherapy was 28,6%, the amputation rate was 7,1%, and the wound complication rate was 23%. Grades 3 and 4 complications have occurred in 50 % of the cases, but no deaths or modifications on surgical intent related to chemotherapy complications have occurred. CONCLUSIONS: tumors considered responders to neoadjuvant chemotherapy showed a gene expression profile significantly different from non responders, especially with respect to the TGF signaling pathway. The neoadjuvant regimen tested has showed to be effective and safe to be considering for a phase III clinical trial
