Abstract:
It was made an organized study of the functional condition of hepato-biliary system beside 200 patients with fatty hepatosis and fatty hepatosis with concomitant sugar diabetes. In verification of the diagnosis were used results of the biochemical blood`s study, ultrasonography study, computer and MR tomography. All patients were conducted with duodenal tubing with following biochemical bile study. Breach of the functional condition liver was revealled beside majority sick fatty hepatosis, moreover degree of the breaches becomes above under accompanying sugar diabetes. Unidirectional change in biochemical bile characteristic wasn`t noted beside patient fatty hepatosis and fatty hepatosis with accompanying sugar diabetes, corresponding to I stage cholelithiasis.

Abstract:
Background. Spread of childhood obesity is associated with social and economic factors of modern lifestyle that alter food preferences and lead to sedentary life. Nowadays fatty hepatosis is considered a hepatic manifestation of the metabolic syndrome, which is linked to the rising prevalence of obesity among the population. There is evidence of a correlation between metabolic disorders and oxidative stress reactions.Aims. To study characteristics of lipid peroxidation system and antioxidant defense processes in adolescents with fatty hepatosis.Materials and methods. We examined 15 adolescents with obesity and fatty hepatosis, 20 adolescents with obesity and without fatty hepatosis, and 20 apparently healthy adolescents, who formed the control group. Spectrophotometric and fluorometric methods were used in the research.Results. We found that some components of antioxidant status of adolescents with obesity and fatty hepatosis were decreased, both in comparison with the group with obesity without hepatosis and control group.Conclusions. It is recommended that patients with obesity and fatty hepatosis take antioxidants in addition to metabolic therapy.

Abstract:
The purpose — to study the features and relationship of blood uric acid and metabolic atherogenic disorders in adolescents with fatty hepatosis. Material and methods. The study of atherogenic disorders included the following groups: 1 — adolescents with fatty hepatosis and hyperuricemia (n = 44), 2 — adolescents with fatty hepatosis and normal blood uric acid levels (n = 76), 3 (comparison group) — adolescents without acute and chronic diseases (n = 220). The study results showed higher values of average body mass index, waist circumference, triglycerides, total lipid peroxidation, glucose, HOMA index, insulin, C-reactive protein, and lower values of high-density lipoprotein cholesterol and 25-hydroxyvitamin D in the group of adolescents with fatty hepatosis and hyperuricemia, compared to the group with fatty hepatosis and normal blood uric acid levels and the comparison group. A correlation analysis in a group of adolescents with ultrasound signs of fatty hepatosis revealed a positive correlation of the uric acid level in the blood with the HOMA index (r = 0.45; p < 0.001), waist circumference (r = 0.45; p < 0.001), body mass index (r = 0.35; p < 0.001), insulin (r = 0.34; p = 0.007), glucose (r = 0.31; p < 0.024), total lipid peroxidation (r = 0.41; p < 0.015), triglycerides (r = 0.36; p = 0.014), and C-reactive protein (r = 0.31; p = 0.007), and a negative correlation with the 25-hydroxyvitamin D level (r = — 0.38; p = 0.001). Conclusions. The study showed the possibility to use blood uric acid levels in fatty hepatosis in adolescents as an indicator of atherogenic metabolic disorders: insulin resistance, lipid peroxidation, dyslipidemia, chronic inflammation, and 25(OH)D insufficiency.

Abstract:
Relevance. High milk productivity of cows after calving is the cause of liver damage in the form of hepatosis and premature retirement of animals. In such cases, pathoanatomical examination of the liver reveals simultaneously disorders in the form of protein and fatty dystrophies with subsequent destruction of cellsand circulatory disorders. Hepatoses can also cause such postpartum complications in cows as retention of placenta .Methods. For the prevention of hepatosis, the drug "Ovostim-ct" is proposed, which contains cytotoxic serums against the tissues of the liver, ovary, spleen and muscle. In order to prevent liver damage and postpartum retention of placenta , the drug is used 1–1.5 months before calving at a dose of 1 ml per 100 kg of live weight subcutaneously or intramuscularly.Results. The use of the hepatoprotector has reduced the number of cases of hepatosis and retention of placenta in cows after calving. Also, “Ovostim-ct” had a beneficial effect on the biochemical parameters of the blood serum of cows: there was an increase in the concentration in the bloodof total protein by 6.7%, albumin — by 15.6% and glucose — by 45.8%.

Abstract:
This article shows the results of studies in the course of which in a comparative aspect the LEP polymorphism was studied in subpopulations of Holstein dairy cattle in cows with fatty hepatosis and in healthy animals. This is the first time such studies are being conducted. In this case, a hepothesis about the relationship of genetic markers, in particular the leptin gene, with the risk of developing and progression of fatty liver disease in a model of dairy cows when determining the frequency of occurrence of genotypes with LEP polymorphisms in groups of animals with hepatosis and healthy animals was studied. The aim of the research was to establish the possible influence of the genotype at the leptin locus (polymorphisms Y7F, A80V, R25C) on the frequency of occurrence of fatty hepatosis in dairy cows. In the course of studies on the A80V, R25C, Y7F loci of the leptin gene, 180 cows were genotyped, where 80 animals were diagnosed with fatty hepatosis and 100 animals were healthy. In the group of healthy Holstein cows, compared with the group of animals with pathology, the RC genotypes (R25C polymorphism) were significantly more frequent, the frequency of the CC genotype (R25C polymorphism), on the contrary, was much higher in the group of animals with fatty hepatosis. Similar trends were noted for another polymorphism (A80V).

Abstract:
To determine the features of disorders and the relationship of the blood lipid spectrum, indicators of peroxidation and insulin resistance in adolescents with fatty hepatosis, a single-stage study was conducted of a sample of 58 adolescents from Arkhangelsk aged 16–17 y. o. who have instrumental signs of fatty hepatosis according to ultrasound diagnostics. The comparison group consisted of 219 adolescents from Arkhangelsk who did not have acute and chronic diseases at the time of the examination. The results of the study showed higher average values of total lipid peroxidation, HOMA index, insulin, glucose, triglycerides, total cholesterol, low-density lipoprotein cholesterol and lower high-density lipoprotein cholesterol in the group of adolescents with fatty hepatosis than in the comparison group and the presence of a correlation of the HOMA index with the level of total lipid peroxidation, triglycerides and the presence of dyslipidemia (r = 0.53; 0.44; and 0.31; p < 0.05). Thus, when detecting fatty hepatosis in adolescents during diagnostic and corrective measures, it is necessary to take into account the possibility of formation and interrelation of the following pathogenetic factors of non-alcoholic fatty liver disease: insulin resistance, lipid peroxidation and dyslipidemia.

Abstract:
The relevance of this study lies in the lack of reliable information on the key aspects of the molecular mechanisms of fatty hepatosis, a common mammalian disease, and its dangerous complications, such as fibrosis, cirrhosis, and hepatocellular carcinoma. These circumstances substantially complicate the prompt diagnosis of this hepatopathology and reduce the effectiveness of treatment. The metabolism of the heme derivative bilirubin is unique and inherent in the liver, which undergoes transformation due to hepatocyte enzyme systems to form neutral compounds, such as glucurono-conjugates and sulphoconjugates, which undergoes changes and requires analysis in fatty hepatosis in animals. The aim of the study was to determine the characteristic changes in the content of unconjugated bilirubin and its sulphoconjugate, glucuronide, monoglucuronide, monoglucoside, and diglucuronide in the blood, bile, and liver of rats under the modelling of fatty hepatosis and the administration of rehabilitation therapy. For this, the thin-layer chromatography method was used. It was found that under tetracycline-induced fatty hepatosis in laboratory rats, the concentration of unconjugated bilirubin in the blood decreased by 39.3%, bilirubin glucuronide by 44.4%, and the total fraction of bilirubin monoglucuronide and monoglycoside by 78.9%. Oral administration of the milk phospholipid complex in the composition of the BAS “FLP-MD” to sick animals ensured the maintenance of these indicators at the control level. The content of bilirubin glucuronide and bilirubin diglucuronide decreased in the liver tissue of the treated rats. When these animals were administered a phospholipid-containing bioadditives, the level of unconjugated bilirubin in the liver tissue increased by 22.2%, but the content of its conjugated forms decreased. The content of all the studied bilirubin fractions decreased in the bile of sick rats during self-rehabilitation. The components of the BAS “FLP-MD” in fatty hepatosis in rats did not affect the reduction of unconjugated bilirubin in bile but eliminated the inhibitory effect of the modelled pathology on the content of all conjugated forms of bilirubin. The findings can be used as biochemical markers to monitor the pigment metabolism in the development of fatty hepatosis in mammals

A ten-day course of karsil (silimarin, legalon) or silibor therapy improved the absorption (as indicated by ueviridin test) and oxidative-antitoxic (as shown by antipyrin test) hepatocyte functions in patients with non-insulin-dependent diabetes mellitus with hepatosis. A similar ten-day course of Liv-52 did not change the studied liver parameters. lnductothermal exposure of the splenic area (a 20 min session daily for 5 days) improved the absorption and antitoxic functions of the liver in diabetics. A positive effect of splenic inductothermia persisted for at least two weeks. The peak values of ultrasonic amplitude histogram of the liver reflect the time course of liver status in the course of karsil and silibor therapy and splenic inductothermal exposure.

An interesting and unexplored to date is the hepatoprotective effect of hypoglycemic drugs, analogues of glucagon-like peptide-1, which include liraglutide. Knowing the positive effect of liraglutide on plasma lipid fractions and a general decrease in adipose tissue, a decrease in glucagon production and an increase in insulin synthesis, a beneficial effect on the cardiovascular system and an anti-inflammatory effect, there is reason to believe that this drug may be useful in the complex therapy of liver diseases, such as fatty hepatosis, cirrhosis and non-infectious hepatitis, as its action affects the pathogenetic links of this group of diseases. 40 In this article, we analyze the possibilities of using Saxenda, the main active ingredient of which is liraglutide, to potentiate the hepatoprotective effects of the Hepaton-vet drug in groups of rats with induced hepatopathy, evaluating the effect of these drugs on blood biochemical parameters.

Most of the diseases associated with carbohydrate and fat metabolism disorders (type 2 diabetes mellitus, obesity, metabolic syndrome) lead to changes in the structure and function of liver cells, and the formed liver dysfunction negatively affects the further progression of the disease. The process of liver damage develops with varying intensity and does not immediately lead to irreversible consequences; therefore, dysfunction should be detected as early as possible, and the data obtained should be used to assess the current state and predict their reversibility.Study purpose. To create a quantitative assessment method that allows to assess the current functional state of the liver and to determine the reversibility of existing functional disorders and the severity of structural changes, to assess the prognosis of the course of the disease and the effectiveness of restorative measures.Instruments and Data Collection Procedure. Examination of patients, in addition to conventional methods, included an assessment of the absorptive-excretory function of the liver and biliary tract patency using a scintillation gamma camera (Siemens Symbia T16) with subsequent processing on the SUPER-SEGAMS computer system (Hungary). Freshly prepared Bromesida, 99mТс was administered intravenously at the rate of 1.1 MBq per kg of the patient's body weight, with a normal content of bilirubin in blood. Series of scintigrams allow to assess the passage of the drug visually through the blood-liver-ducts-intestine system, to characterize the anatomical features and organic changes in the biliary system. Quantitative analysis of the "activity-time" curves obtained from the areas of interest (the right lobe of the liver - 2 zones, the left lobe of the liver, the common bile duct, the intestinal area, the heart area) makes it possible to study the absorptive-excretory function of the liver.Results. By the method of mathematical modeling, a formula was obtained - the index of the functional activity of hepatocytes:IFAH= (-1,1564 + 0,0653 × BMI - 0,0144 × Tmax) × 100,where:IFAH - index of functional activity of hepatocytes (liver cells);BMI - body mass index (kg/m2)Tmax - indicator of the absorption function of the liver (min) - the time to reach the maximum accumulation of the radiopharmaceutical in the liver. It is an indicator of the function of polygonal liver cells (normal = 8-12 min).Functional activity of hepatocytes: from 0 to 9.9 - normal functional activity of hepatocytes; from 10 to 19.9 - the risk of developing functional disorders; from 20 to 29.9 - reversible dysfunction of hepatocytes (steatosis); more than 30 - irreversible (organic) liver dysfunction (steatohepatitis). With negative IFAH values - the influence of extrahepatic factors, such as diseases that accelerate metabolism at the cellular level (thyrotoxicosis), taking drugs in violation of the prescriptions before these studies.The originality and novelty of the technique are confirmed by the patent – “A method for diagnosing fatty hepatosis” (patent RU 2 578 080 C2 dated February 19, 2016).The method was tested in clinical practice and the data obtained confirmed the high diagnostic accuracy (95%) of the proposed method for calculating the index of IFAH.Conclusion: the discussed method allows doctors to evaluate not only the current functional state of the liver, but also to determine the reversibility of existing functional disorders and the severity of structural changes; also could be used to evaluate the prognosis of the course of the disease and the effectiveness of restorative measures. It is characterized by relative ease of implementation. Only one parameter — Tmax is required, after which the study can be completed.
