Abstract:
The period leading to death is characterized by increasing prevalence and intensity of physical, psychological, existential, and social concerns, and it is often a challenging time for patients, their families, and health-care providers. This chapter specifically addresses the most prevalent symptoms and concerns encountered when managing the actively dying patient. Symptoms affecting dying patients’ comfort, including pain, dyspnoea, delirium, terminal secretions, and refractory symptoms and suffering require different clinical management as death approaches. Other topics included are recognizing the dying phase; communication with and preparation of patient, family, and staff; anticipating dying-advanced care planning and approach to resuscitation; addressing psychosocial and existential concerns; and approach to difficult end-of-life scenarios. This chapter aims to provide the health-care practitioner with a good overview and approach to the whole-person care needs of the dying patient and their family and carers to enable health practitioners to feel comfortable in providing this important care with confidence.

Abstract This book contains 5 chapters that presents a simple, straightforward discussion of the principles and processes involved in the production of grain yield by agronomic crops, and how these processes underlie and influence management decisions. The focus is on grain crops, principally maize and soybean, although the general principles apply equally well to cereals, grain legumes and oil crops. Management decisions define all cropping systems - what (crop species, variety), where (climate), when (planting date), and how (row spacing and population density) are the fundamental choices. Knowledge of the fundamental processes responsible for plant growth and the accumulation of yield simplifies the decision-making process and leads to improved management decisions, higher grain yields, and cropping systems that are efficient, resilient and sustainable. The contents include basic plant growth processes (e.g. photosynthesis, respiration, evapotranspiration); growth and production of yield; crop management (seed quality, variety selection, planting date, row spacing); and crop production in the future (climate change, GMOs, precision agriculture and new crops). This books is intended for researchers in crop science, agronomy and plant science, and crop production practitioners. This book will enable readers to make better, more informed management decisions; decisions that will help maintain a well-fed world in the future.

Abstract:
Parenteral nutrition (PN) is a technique of artificial nutrition support, which consists of the intravenous administration of macronutrients, micronutrients, and water. PN has become integrated into intensive care unit (ICU) patient management with the aim of preventing energy deficits and preserving lean body mass. The addition of PN to enteral nutrition is known as supplemental PN. Parenteral feeding should be considered whenever enteral nutritional support is contraindicated, or when enteral nutrition alone is unable to meet energy and nutrient requirements. International guidelines differ considerably regarding the indications for PN. Thus, the ESPEN guidelines recommend initiating PN in critically-ill patients who do not meet caloric goals within 2–3 days of commencing EN, while the Canadian guidelines recommend PN only after extensive attempts to feed with EN have failed. The ASPEN guidelines advocate administering PN after 8 days of attempting EN unsuccessfully. Several studies have demonstrated that parenteral glutamine supplementation may improve outcome, and the ESPEN guidelines give a grade A recommendation to the use of glutamine in critically-ill patients who receive PN. Studies on IV omega-3 fatty acids have yielded promising results in animal models of acute respiratory distress syndrome and proved superior to solutions with omega -6 compositions. The discrepancy between animal models and clinical practice could be related to different time frames.

Exploring the environmental effects of animal agriculture, fishing, and hunting, Eating Earth exposes critical common ground between earth and animal advocacy. The first chapter (animal agriculture) examines greenhouse gas emissions and climate change, manure and dead zones, freshwater depletion, deforestation, predator control, land and use--including the ranching industries public lands subsidies. Chapter two first examines whether or not the consumption of fish is healthy and outlines morally relevant aspects of fish physiology, then scrutinizes the fishing industry, documenting the "silent collapse" of ocean ecosystems and calling attention to the indiscriminate nature of hooks and nets, including the problem of bycatch and what this means for endangered species and fragile seascapes. Chapter three outlines the historic link between the U. S. Government, wildlife management, and hunters, then systematically unravels common beliefs about sport hunting, such as the belief that hunters are essential to wildlife conservation, that contemporary hunting qualifies as a tradition, and that hunting is merciful, economical, or rooted in "fair chase." At the end of each chapter, Kemmerer examines possible solutions to problems presented, such as sustainable meats, organic and local, grass fed, aquaculture, new fishing technologies, and enhanced regulations. Eating Earth offers a concise examination of the environmental effects of dietary choice, clearly presenting the many reasons why dietary choice ought to be front and center for environmentalists.
