Abstract:
Essential to both propriety and fashion, hats were a crucial aspect of British female dress and appearance in the late nineteenth and early twentieth centuries. This article shows how British novelists of this period, ranging from mainstream to experimental, understood this importance. With appropriate contextualization, these literary depictions can illuminate how women wore and felt about their hats. Authors such as Frances Hodgson Burnett, Dorothy Whipple and Virginia Woolf used these accessories to explore social respectability and convention, the pleasures and challenges of following fashion, and consumption strategies among women. Despite the era's significant social changes, remarkable continuity exists in these literary representations of hats.

Abstract:
This article reflects the importance of wool in the life of traditional Kyrgyz society. The nomadic way of life provided them with everything they needed for daily life. Livestock, the main economic activity of the Kyrgyz, provided raw materials for livestock: wool, skins, horns, sheep skins and bones. The article describes the various characteristics and features of sheep’s wool. As a traditional knowledge of the Kyrgyz, its use in the treatment of fractures through wool, i. e. The importance of the medical aspect is established. Sheep’s wool is widely used in the manufacture of various household items, in the exterior and interior decoration of traditional yurts, and in the making of hats, clothes, and shoes. The technological method of shearing sheep to obtain high-quality wool, as well as the features of its preparation in the manufacture of Kyrgyz felt carpets (ala kiiz and shyrdak) are described in detail.

The purpose of this research is to know the benefits of tourism coast economy are felt directly by the local people in Meosmansar Subdistrict, Raja Ampat Regency. The case study taken from the local people of Arborek Village which is a tourism village that has hand-crafed in the form of woven hats and bags of leaf mats. The type of data in this study used qualitative data. The data sources used are primary data and secondary data. The data are collected through by observation, indepth interviews with the some of the interviewees and as well as documentation. The technique of determination of the informant used in this research is purposive sampling. The informants which is selected in this study are representative of the local government in this case the Tourism and Culture officer of Raja Ampat Regency as well as the local community whose doing business craftsmen woven hats and bags. The data are collected is analyzed using the concepts of economic benefits by Leiper. The results of this study found the presence of perceived economic benefit directly by the local communities from tourism sector. Abstract:
This article investigates gendered nationalist ideologies and their attendant myths and narratives in present-day Kyrgyzstan through an investigation of clothing items and practices. Clothes “speak volumes,” revealing tensions between gendered narratives of nationhood and various interpretations of what “proper” Kyrgyz femininities and masculinities should be. Clothing thus becomes both a sign and a site of the politics of identity, inscribing power relations and individual strategies of Kyrgyz men and women onto their bodies. Individual clothing choices and strategies take place within the general context of discursive struggles over what authentic and appropriate representations of Kyrgyzness should be. Thus, such clothing items as ak kalpak (conical felt hats) and the practice of Muslim women covering their head (hijab) acquire social and political meanings that stand for wider processes of identity contestations in the country.

Although this essay is about reading texts from the past, I begin with the present, with a US president who was elected despite widespread knowledge that he had infamously boasted about how he often starts kissing beautiful women without consent. “I don't even wait,” he crowed. “When you're a star they let you do it. You can do anything. Grab ’em by the pussy. You can do anything.” The internet was soon flooded with angry feminist responses to his crass admission of sexual violence, retorts that appropriated his use of vulgar language with wit and resistance, the most prevalent being the meme “Pussy Grabs Back!” (see fig. 1). The Pussy Hat Project soon followed. As we know, the pussy hat phenomenon offended some who felt it was racist (not all vulvas are pink) and/or transphobic (not all women have pussies). Nevertheless, a sea of pink pussy hats, worn by persons of many embodiments and identity categories, would be seen in protest, worldwide, at the record-breaking post-inauguration Women's Marches across the globe. If “Grab ’em by the pussy” rightly offended, “Pussy Grabs Back!” gained traction.

Abstract:
This research focuses on designing low-cost aids in teaching materials, to help children with developmental disabilities. Using flash software and power point as interface design with the assistance of interactive device could also be developed teaching materials for children. When detected by a wii remote and infrared (IR emitter) device, corresponding information appears on a screen to increase the interaction of assistive technology aimed at children by adopting an enhanced learning process. There are two cases on this study, they are actually applied on children with developmental disabilities, and the overall equipment cost is approximately US$50.Case 1 is a child wearing hats with reflective stickers, a wii remote with an infrared emitter board in front. When we performed the initial probing of their interactive physical activity, the child was asked to wear hats with reflective stickers, while the teachers asked him to perform continual actions of standing and squatting. Sensors were set up next to the child so that when the child performed the movements, the movements would be sensed by the sensor. The signals were emitted to the computer, and then the corresponding synchronized screen would be displayed immediately. To put it simply, we used the cheap reflective stickers as a mouse, therefore we can design different teaching module courses. Case 2 is one kind of teaching materials for customer design, the participant is also a child with developmental disabilities.In this case, the research took the pictures of the subjects, then edited the images in flash through a mouse. The participating children wore the reflective stickers on their hands, when they moved their hands, their images on the screen would move accordingly as well. In this case, we dressed up the children differently, when the children saw that they were displayed on the screen, and the cursor movements were controlled by their limbs, they felt very happy. In this research, the devices relied upon user-center design, reducing the learning loading from their teaching materials and enhance their learning motivation and interest.

Abstract:
Purpose. The article introduces and analyzes in detail the materials of the Bronze Age from the vicinity of Lake Lop Nor (Xinjiang, China), which are stored in the funds of the National Museum of Korea. The artifacts were obtained by the expedition to Xinjiang under the general guidance of the Japanese explorer and religious public figure, count Otani Kodzui. They are part of a special section of the famous “Otani Collection”. The Korean segment of this collection was completely described in the Republic of Korea in 2016, and now its analysis is presented in Russian for the first time. The materials of the Bronze Age constitute the most archaic part of the Xinjiang collection. This group includes 12 items: 4 woven baskets, 3 wooden masks with carvings of human faces, 2 ritual staves, 2 felt hats and one boot from a pair of leather footwear. Results. Analysis of the artifacts demonstrates analogies with the eponymous and most famous site of Xiaohe culture on the territory of Xinjiang – the Xiaohe burial ground at Lake Lop Nor. However, there were other complexes like Xiaohe which existed in ancient Xinjiang (for example, Beifang burial ground on the Keriya River). The authors note that the ornamental composition on the surface of woven baskets demonstrates similarity to the decor of the Andronovo culture’s ceramic vessels, and the iconography of wooden carvings of human faces is close to the images on the steles and plates of the Okunevo culture of Southern Siberia. Separate objects similar to both Andronovo and Okunevo cultures are also known to have been found on other sites of Xinjiang. Conclusion. The closest analogy to the described items from the funds of the National Museum of Korea is the Xiaohe culture materials. The question about the exact location of the site where the items of Xinjiang collection in Korea come from is still open. It requires additional research.

Purpose – The purpose of this paper is to provide a commentary on “‘I felt I deserved it’ – Experiences and implications of disability hate crime”. Design/methodology/approach – Comparing the findings reported by Richardson and her colleagues with the wider evidence base, it is clear that there is considerable consistency. While scholarship in this area is still nascent, there is sufficient clarity around key issues that should support action to tackle hate crime against people with learning disability and/or autism. Findings – Interventions should not make uncritical assumptions around the vulnerability of people with learning disability and/or autism. Poor service commissioning, design and delivery can play a part in heightening risks. Hate crime is not simply a criminal justice issue, and effective intervention will rely on multi-agency working. Originality/value – The commentary recommends a social model approach towards understanding hate crime and how it may be tackled. It identifies the challenges confronting multi-agency working by situating the analysis against the wider context of public spending cuts and the impact of these on wider societal attitudes towards disabled people.

Purpose – The purpose of this paper is to summarise key findings and recommendations from the “Living in Fear” research project focusing on the experiences of people with learning disabilities and autism related to disability hate crime and the experience of the police in dealing with such incidents. Design/methodology/approach – Methods included: first, a postal survey with 255 people with learning disabilities or autism (or their carers for people with more severe disabilities), of whom 24 also took part in semi-structured interviews; and second, an electronic survey of the knowledge and experience of 459 police officers or support staff. Findings – Just under half of participants had experienced some form of victimisation. The Police reported problems with the definition of disability hate crime and challenges to responding effectively. Social implications – A case study from the research highlights some of the key findings and is linked to implications for people with learning disabilities and autism, carers, police and other agencies. Originality/value – Previous research has highlighted that victimisation is an issue for this group of people, but has never explored the prevalence and nature of such experiences in a representative sample. Neither has previous research brought together the perspectives of so many different agencies to offer recommendations that go across many sectors. The paper will be of interest to people with disabilities and their carers, professionals in health, social care and the Criminal Justice system.

The need for fresh responses to hate crime has become all the more apparent at a time when numbers of incidents have risen to record levels, both within the UK and beyond. Despite progress within the domains of scholarship and policy, these escalating levels of hate crime – and the associated increase in tensions, scapegoating and targeted hostility that accompanies such spikes – casts doubt over the effectiveness of existing measures and their capacity to address the needs of hate crime victims. This article draws from extensive fieldwork conducted with more than 2000 victims of hate crime to illustrate failings in relation to dismantling barriers to reporting, prioritizing meaningful engagement with diverse communities and delivering effective criminal justice interventions. It highlights how these failings can exacerbate the sense of distress felt by victims from a diverse range of backgrounds and communities, and calls for urgent action to plug the ever-widening chasm between state-level narratives and victims’ lived realities.

Weight stigma is reportedly globalizing, but there is limited evidence of how concerns around weight change are impacting those in many places. Additionally, few studies to date have considered how weight stigma operates in the context of interventions that purposefully eschew from a focus on weight itself. We have used qualitative analysis to examine body acceptance and responses to weight stigma among urban Brazilian gorda women participating in two forms of Health at Every Size® (HAES®) interventions: an intensive HAES® program (I-HAES®, n=26), which directly addressed weight stigma and a traditional, less-intensive HAES® program (CTRL, n=13). Individual, semi-structured interviews regarding how participants felt about their bodies were conducted post-intervention. Exploratory content analysis followed an inductive approach. The I-HAES®-group was more prone to accept their bodies, to feel well-being, to do new things, and to give proactive responses to weight stigma, while the CTRL-group internalized and accepted stigma, reported a lack of body acceptance, and indicated they were less able to challenge stigma in their lives. Interdisciplinary, intensive HAES® interventions appear able to meaningfully tackle responses to weight stigma and promote body acceptance, important in contexts where stigma is a major component of how women experience their weight.

Abstract:
Abstract Hate violence which denigrates a person’s social identity whether it involves physical or verbal aggression off or online – is a communicative act. It transmits a message to the victim that they are devalued and unwelcome. It is a marginalising and exclusionary message. Answering back to hate violence by challenging hateful expression is one way of responding. It is a form of ‘civil courage’. Yet why should anybody want to take a stand and speak out – given the risks involved that perpetrators might turn on those who intervene or respond in some other way? This paper proposes that the importance of civil courage goes beyond being the right thing to do, or the humane thing, when a bystander witnesses hate violence off- or online. Instead, if we comprehend hate violence as a communicative act, and if we understand the particular impact of the exclusionary message it sends (and understand how bystander inaction can magnify the felt sense of social exclusion), then we might appreciate the potential value of an act of civil courage in response. There is a moral imperative for civil courage as it answers back to hate violence by sending an inclusionary message to the victim – as reasoned in this paper.

Journalists have long experienced a love-hate relationship with government public information officers (PIOs) in setting the public agenda, particularly at the federal level. Three surveys of journalists and PIOs examined whether government controls impact journalists’ gatekeeping roles. Results indicated that journalists felt PIO controls are increasing, that reporters covering federal agencies express the most frustration, and that PIOs at the federal level indicate the greatest need for controls over journalists.

Background & Objectives: Hospital-acquired infections (HAIs) are a common problem and challenge faced by hospitals in all countries around the world. Nurses are an important part of the healthcare team that plays a unique role in the control of HAIs. Compliance on the part of healthcare workers (HCWs) including nurses with standard precautions has been recognized as being an efficient means to prevent and control HAIs. The present study was conducted with objective of understanding the level of Knowledge, attitude and practice (KAP) regarding HAIs among nursing students in the Western Region of Nepal.Materials & Methods: This descriptive cross-sectional study was conducted among nursing students posted in different wards of two hospitals of Pokhara. A self administered questionnaire containing different set of questions regarding knowledge, attitude and practice on HAI were used for data collection.Results: Among the total participants in the study 97% of the participants considered that prevention of HAIs were a valuable part of nurses’ role but only 89% had received formal training regarding hand hygiene. The results show that 74% of the participants had good knowledge regarding HAIs. 82% of the participants felt that they would be less likely to transmit infection to the patient if they performed hand-hygiene. 66% of them identified that hand hygiene agents were not readily available in current settings. Regarding practice, only 6% performed hand hygiene before patient contact.Conclusion: The nursing students had good knowledge regarding HAIs that was reflected in their attitude and practice on hand hygiene for the prevention of HAIs. However there is the need of regular training and performance feedback regarding hand hygiene and the hospital environment should be hand-hygiene friendly with easily accessible to sinks and other facilities.

Based on a survey of 593 lesbian, gay, bisexual, and transgender (LGBT) people in the United Kingdom, this study shows that direct anti-LGBT hate crimes (measured by direct experiences of victimization) and indirect anti-LGBT hate crimes (measured by personally knowing other victims of hate crime) are highly prolific and frequent experiences for LGBT people. Our findings show that trans people are particularly susceptible to hate crimes, both in terms of prevalence and frequency. This article additionally highlights the negative emotional and (intended) behavioral reactions that were correlated with an imagined hate crime scenario, showing that trans people are more likely to experience heightened levels of threat, vulnerability, and anxiety compared with non-trans LGB people. The study found that trans people are also more likely to feel unsupported by family, friends, and society for being LGBT, which was correlated with the frequency of direct (verbal) abuse they had previously endured. The final part of this study explores trans people’s confidence levels in the Government, the police, and the Crown Prosecution Service (CPS) in relation to addressing hate crime. In general, trans people felt that the police are not effective at policing anti-LGBT hate crime, and they are not respectful toward them as victims; this was especially true where individuals had previous contact with the police. Respondents were also less confident in the CPS to prosecute anti-LGBT hate crimes, though the level of confidence was slightly higher when respondents had direct experience with the CPS. The empirical evidence presented here supports the assertion that all LGBT people, but particularly trans individuals, continue to be denied equal participation in society due to individual, social, and structural experiences of prejudice. The article concludes by arguing for a renewed policy focus that must address this issue as a public health problem.

Abstract:
Background. The incidence of healthcare-associated infections (HAIs) in the public health sector in South Africa is not known due to the lack of a surveillance system. We report on the challenges experienced in the implementation of a surveillance system for HAIs in intensive care units (ICUs). Methods. A passive, paper-based surveillance system was piloted in eight ICUs to measure the incidence of ventilator-associated pneumonia, catheter-associated urinary tract infection, and central line-associated bloodstream infection. Extensive consultation with the ICU clinical and nursing managers informed the development of the surveillance system. The Plan-Do-Study-Act method was utilized to guide the implementation of the surveillance. Results. The intended outputs of the surveillance system were not fully realized due to incomplete data. The organizational culture did not promote the collection of surveillance data. Nurses felt that the surveillance form added to their workload, and the infection control practitioners were unable to adequately supervise the process due to competing work demands. Conclusions. A manual system that adds to the administrative workload of nurses is not an effective method of measuring the burden of HAIs. Change management is required to promote an organizational culture that supports accurate data collection for HAIs.

When I returned to the United States after four years in England, I felt as if the country had shifted under my feet. Remember 1991? George Bush was triumphantly concluding the Gulf War and seemed electorally invincible; Bill Clinton was stumbling in the early primaries; no-one had heard of Clarence Thomas or Anita Hill; Waco was just another town in Texas; OJ was still married. Don't worry about the far right, I would say with the confidence and accuracy of a BBC meteorologist, they've always been there, and periodically they reappear to scare us. I would cite The Order, Posse Comitatus, or Aryan Nation as the Michigan Militia of their time. Then Waco happened, and I was stunned. Then Oklahoma City happened, and I was aghast. Politicians have been ‘running against Washington’ and voters ‘throwing the bums out’ for as long as I can remember. The rise of cults, militias, and other extremists is something very different with little relation to left or right, Republican or Democrat. With militiamen spouting hate and calling it love, with ‘patriots’ taking up arms against the government, with religious zealots dying in infernos, one might justifiably ask what else is at hand. Where is America now? Ask another American and you'd get another answer, but here's mine: America is at (or near) a place where I cannot burn the nation's flag and call it legal protest, but the Ku Klux Klan can burn a cross in a public space and call it religion. If I feel a little unsteady on my feet, I guess I haven't regained my land legs yet.

SUMMARYInformation is lacking on the integrated evaluation of mortality rates in healthcare-associated infections (HAIs). Our aim was to differentiate the risk factors responsible for the incidence from those for the case-fatality rates in association with HAIs. We therefore examined the time trends of both incidence and case-fatality rates over a 20-year period at a tertiary-care teaching medical centre in Taiwan and the mortality rate was expressed as the product of the incidence rate and the case-fatality rate. During the study period the overall mortality rate fell from 0·46 to 0·32 deaths/1000 patient-days and the incidence rate fell from 3·41 to 2·31/1000 patient-days, but the case-fatality rate increased marginally from 13·5% to 14·0%. The independent risk factors associated with incidence of HAIs were age, gender, infection site, admission type, and department of hospitalization. Significant prognostic factors for HAI case-fatality were age, infection site, intensive care, and clinical department. We conclude that the decreasing trend for the HAI mortality rate was accompanied by a significant decline in the incidence rate and this was offset by a slightly increasing trend in the case-fatality rate. This deconstruction approach could provide further insights into the underlying complex causes of mortality for HAIs.

Background: Ethical issues associated with nurses’ interactions with industry have implications for the safety, quality, and cost of healthcare. To date, little work has explored nurse–industry interactions and their associated ethical issues empirically. Design and participants: A phenomenological study was conducted to explore registered nurses’ interactions with industry in clinical practice. Five registered nurses working in direct patient care were recruited and individual, in-depth interviews were conducted. The University’s Committee on Human Research approved the study. Findings: Nurses frequently interacted with industry in their practice and felt ambivalent about these interactions. Nurses described systemic cuts to multiple “goods” central to nursing practice, including patient support, but paradoxically relied on industry resources to deliver these “goods.” They relied on a particular conception of trust to navigate these interactions but were left to do so individually on the basis of their experience. Conflicts of interest arose as a result of multiple competing interests, and were frequently mediated through nurses’ superiors. Conclusion: Nursing as a profession requires a guiding narrative to aid nurses in interpreting and navigating interactions with industry. A conception of trust that incorporates both the work of caring and attention to social justice could form the basis of these interactions, but would require that nursing take a much more critical stance toward marketing interactions.

- Psychoanalytic theory seems best suited to explain why psychoanalysis won't work. As theory was taught when Hans Loewald wrote his early papers, practitioners were led to expect favorable responses that made no sense. He recognized that this was the result of neglecting the process aspect of Freud's theory. Proposing that psychic structure is in continuous process within the mind and with the outside world, Loewald was able to bridge the gap between analyst and patient, past and present, old and new meaning, intrapsychic and interpersonal. He portrayed analytic process as working the same way as ordinary mental process. Although this was probably our most profound elaboration of Freud's work, and one of the few plausible theories of therapeutic action, Freudian analysts felt threatened, and denounced Loewald as sentimentalizing treatment, infantilizing patients and betraying intrapsychic causality. Interpersonal analystis claimed him for their own. In December 1890, when Samuel Iseman stopped in Sumter, South Carolina, he hoped to turn his luck around. The traveling grocery salesman's trip so far had not gone well. Rain had delayed his train by two hours. When Iseman finally arrived, he had time to “drum” up business with only one retailer before drying off and retiring for the evening. The next day was no better; his expense money was dangerously low. He dunned two area merchants for payment on their accounts, eager to refill his coffers, but received only the runaround for his efforts. He checked in on groceryman (and postmaster) Moses W. Harrell in nearby Timmonsville, who made good his account, with one exception. The grocer took issue with a recent delivery of hams that had come from Iseman's Charleston wholesale fi rm, 120 miles south. Harrell complained to Iseman that the hams were of poor quality and that he had sold only two or three. Harrell asked for a refund. The traveling man felt badly that the grocer had taken the meat because Iseman had talked them up. When the hams failed to satisfy, both the salesman's word and the confidence he had earned from Harrell were on the line.

e17034 Background: Modest prospective data reports the impact of biopsies on patients (pts) with respect to adverse events (A/E) and anxiety. With increasing use of biopsies in clinical practice and trials, BIOPSY was designed to prospectively assess this. Methods: Pts undergoing a biopsy for diagnosis and/or research at Princess Margaret Cancer Centre were eligible. Surveys at 1 wk prior to biopsy and 1 wk and 1 mth post-biopsy assessed anxiety, A/E and pt experience. Anxiety was measured with a modified hospital anxiety and depression scale (HADS) score. Results: From 3/2015 to 1/2017, 51, 47 and 39 pts completed 1, 2 and 3 surveys respectively. Median age was 61 years. 40 biopsies were for research (78%) and 11 for diagnosis (22%). 46 pts were involved in a clinical trial (90%). 47 pts had a gynecological cancer (92%). 45 pts felt well-informed of the possible risks of the biopsy (88%). 23 pts noted the biopsy would not impact them directly (45%). 48 pts supported the use of tissue for future unrelated research (94%). Feelings of anxiety about the biopsy improved with time (table 1). 28 pts had pain 1 wk post-biopsy (60%) which was a moderate or major problem in 9 pts (19%). 2 pts reported reduced dignity with the biopsy (4%). 12 pts at 1 wk (26%) felt another biopsy would be a moderate-major issue. At 1 mth 3 of these pts reported a biopsy was now a mild problem. No significant A/E occurred. At baseline only 2 pts felt a biopsy would deter them from entering a clinical trial (4%). 42 pts at 1 wk (89%) and 30 pts at 1 mth (77%) felt they would consent to another biopsy for research. On univariate analysis pts with at least moderate pain post-biopsy at 1 wk or 1 mth (p=0.03) or an elevated HADS score at 1 mth (p=0.02) appeared less likely to agree to a further research biopsy but numbers were small. Conclusions: Anxiety and pain are potentially modifiable A/E that impact whether a pt will agree to a further biopsy. Overall, most pts tolerated their biopsy well, supported its use for future unspecified research and were open to further biopsies. [Table: see text]

Covid-19 can affect the psychological development of adolescents, causing anxiety that can not be controlled. One of the factors that influence anxiety is knowledge.Purpose: the purpose of this paper was to know the relationship between knowledge levels with anxiety levels in adolescents on the Covid-19 pandemic.Method: the study utilized a descriptive analytic with total sampling design. Respondents in this paper were all students of XII class in Balikpapan Adventist Senior High School totaling 60 people. Data collection using knowledge questionnaire adopted from the WHO surveyand Hamilton Anxiety Rating Scale (HARS) questionnaire.Result: there were 9 respondents (15%) with standard knowledge felt mild anxiety, 33 respondents (55%) with good knowledge felt mild anxiety, 4 respondents (6.6%) with standard knowledge felt moderate anxiety, 9 respondents (15%) with good knowledge felt moderate anxiety, 1 respondent (1.7%) with standard knowledge felt severe anxiety, 4 respondents (6.7%) with good knowledge felt severe anxiety. The results of the chi-square test obtained p-value of 0.135 < α (0,05).Conclusion: There is no relationship between the level of knowledge and the level of anxiety in adolescents at Balikpapan Adventist High School. Natural environments consist of multiple objects, many of which repeatedly occupy similar locations within a scene. For example, hats are seen on people’s heads, while shoes are most often seen close to the ground. Such positional regularities bias the distribution of objects across the visual field: hats are more often encountered in the upper visual field, while shoes are more often encountered in the lower visual field. Here we tested the hypothesis that typical visual field locations of objects facilitate cortical processing. We recorded functional MRI while participants viewed images of objects that were associated with upper or lower visual field locations. Using multivariate classification, we show that object information can be more successfully decoded from response patterns in object-selective lateral occipital cortex (LO) when the objects are presented in their typical location (e.g., shoe in the lower visual field) than when they are presented in an atypical location (e.g., shoe in the upper visual field). In a functional connectivity analysis, we relate this benefit to increased coupling between LO and early visual cortex, suggesting that typical object positioning facilitates information propagation across the visual hierarchy. Together these results suggest that object representations in occipital visual cortex are tuned to the structure of natural environments. This tuning may support object perception in spatially structured environments. NEW & NOTEWORTHY In the real world, objects appear in predictable spatial locations. Hats, commonly appearing on people’s heads, often fall into the upper visual field. Shoes, mostly appearing on people’s feet, often fall into the lower visual field. Here we used functional MRI to demonstrate that such regularities facilitate cortical processing: Objects encountered in their typical locations are coded more efficiently, which may allow us to effortlessly recognize objects in natural environments.

The ‘time squeeze’ is a phrase often used to describe contemporary concerns about a shortage of time and an acceleration of the pace of daily life. This paper reviews analysis of the Health and Lifestyle Survey (HALS), 1985 and 1992, and draws upon in-depth semi-structured interviews conducted with twenty British suburban households, in order to shed light on ‘senses’ of time squeeze. 75% of HALS respondents felt at least ‘somewhat’ pressed for time, with variables of occupation, gender, age and consumption significantly increasing senses of being ‘pressed for time’. This is not surprising given theories of the ‘time squeeze’. However, identification of variables only offers insights into isolated causal effects and does little to explain how or why so many respondents reported feeling ‘usually pressed for time’. Using interview data to help interpret the HALS findings, this paper identifies three mechanisms associated with the relationship between practices and time (volume, co-ordination and allocation), suggesting that ‘harriedness’ represents multiple experiences of time (substantive, temporal dis-organisation, and temporal density). In conclusion, it is argued that when investigating ‘harriedness’ it is necessary to recognise the different mechanisms that generate multiple experiences of time in order for analysis to move beyond one-dimensional interpretations of the ‘time squeeze’, and in order to account for the relationship between social practices and their conduct within temporalities (or the rhythms of daily life).

223 Background: To better understand adult acute myeloid leukemia (AML) survivorship and health related quality of life, we piloted a survey exploring patient reported outcomes for patients in first complete remission (CR) to determine if patients feel the survey is relevant to their overall well-being. Methods: A cross-sectional survey measuring: quality of life, physical, role, emotional, cognitive, social functioning (EORTC QLQ-C30 v 3.0); physical, psychological, social, and spiritual well-being (Quality of Life-Cancer Survivor (QOL- CS) scale); fatigue (Functional Assessment of Chronic Illness Therapy – Fatigue Scale (FACIT-Fatigue)); anxiety and depression (Hospital Anxiety and Depression Scale (HADS)); sociodemographic and 5 open-ended questions. Results: 18 participants completed the survey; mean age was 57.2 years. Nine patients were in CR for <2 years, and 9 were in CR for ≥2 years. Participants scored well on the EORTC QLQ-C30 and reported symptoms ranging from 11% (constipation) to 83% (fatigue). The FACIT-Fatigue (worst 0-best 52) mean score was 28.7 and median score was 33.5 (normal ≥30). On the HADS anxiety scale, 2 participants scored in the abnormal range. One scored in the abnormal range on the depression scale. On the QOL-CS, participants scored above 6 out of 10 in all domains, with exceptions of the psychological subscales of distress and fear (Table 1). Most participants felt the survey was completely or mostly relevant (88.8%) in understanding their quality of life. Most felt the length was optimal (77.8%). Conclusions: Most survivors scored well on quality of life and well-being, but with ongoing fatigue, distress, and fear of future tests and recurrence. Few consistently scored worse on the scales. There may be a population coping less well and it would be informative for survivorship programs to prospectively characterize their medical and psychosocial-spiritual needs. [Table: see text]

When Mrs. T asked Dorothy, “What will the next one look like?” Dorothy replied, “Well, if you wanted to go from [the first person] all the way to four … you would put a head, then the body, hands, then the feet. And then you would put a hat, and another hat, and another one, and another one …. Background: Over the past 10 years, a rural health system has tried 10 different interventions to reduce hospital-associated infections (HAIs), and only 1 intervention has led to a reduction in HAIs. Reducing HAIs is a goal of nearly all hospitals, and improper hand hygiene is widely accepted as the main cause of HAIs. Even so, improving hand hygiene compliance is a challenge. Methods: Our facility implemented a two-phase longitudinal study to utilize an electronic hand hygiene reminder system to reduce HAIs. In the first phase, we implemented an intervention in 2 high-risk clinical units. The second phase of the study consisted of expanding the system to 3 additional clinical areas that had a lower incidence of HAIs. The hand hygiene baseline was established at 45% for these units prior to the voice reminder being turned on. Results: The system gathered baseline data prior to being turned on, and our average hand hygiene compliance rate was 49%. Once the voice reminder was turned on, hand hygiene improved nearly 35% within 6 months. During the first phase, there was a statistically significant 62% reduction in the average number of HAIs (catheter associated urinary tract infections (CAUTI), central-line–acquired bloodstream infections (CLABSIs), methicillin-resistant Staphylococcus aureus (MRSA), multidrug-resistant organisms (MDROs), and Clostridiodes difficile experienced in the preliminary units, comparing 12 months prior to 12 months after turning on the voice reminder. In the second phase, hand hygiene compliance increased to >65% in the following 6 months. During the second phase, all HAIs fell by a statistically significant 60%. This was determined by comparing the HAI rates 6 months prior to the voice reminder being turned on to 6 months after the voice reminder was turned on. Conclusions: The HAI data from both phases were aggregated, and there was a statistically significant reduction in MDROs by 90%, CAUTIs by 60%, and C. difficile by 64%. The Interpersonal Model of Depression (IMD) based on the Theory of Human Relatedness (Hagerty, Lynch-Sauer, Patusky, & Bouwsema, 1993) is evaluated among adults with Down syndrome. One hundred subjects participated, with 32% having elevated depression scores and 40% stating they felt lonely. The relationship between depression, perceived social support, loneliness, and life satisfaction is statistically significant, F(6, 172) = 4.36, p < .001. Loneliness, social isolation, loss of sense of well-being, self-hate, and social withdrawal are important interpersonal manifestations and represent increasing levels of depression. Social and emotional loneliness are two dimensions of loneliness. The IMD provides a framework to assess depression in this population. Research on the efficacy of depression treatment based on the IMD is needed.

Abstract:
Hans Albrecht Bethe, who died on 6 March 2005 at the age of 98 years, was one of the greatest physicists of the twentieth century, a giant among giants whose legacy will remain with physics and the wider science community for years to come. He was universally admired for his scientific achievement, his integrity, fairness and for deeply felt concern for the progress of science and humanity which made him the ‘conscience of science’. Be the studied theoretical physics with many of the greatest minds within the physics community including Sommerfeld, Ewald and Bohr. His Jewish background made a career in Germany all but impossible, and after a brief spell in England between 1933 and 1935 he emigrated to the USA. He took up a post at Cornell University, where he remained, except for his work at Los Alamos and several sabbaticals, until the end of his career. Hans Bethe was a universalist who contributed to scientific research for more than seven decades. He was awarded the Nobel Prize for his work on energy production in stars. Many other of his discoveries would have been worthy of a Nobel Prize, for instance his work on the Lamb shift or his work on the nuclear many–body problem. Like many of his colleagues who had contributed to the development of nuclear weapons, Hans Bethe devoted much of his time and energy to the control of these weapons, to nuclear disarmament and to the promotion of greater understanding between East and West, most notably through his activities within the framework of the Pugwash Movement.

Background: Hospital-acquired infections (HAIs) are a leading cause of healthcare morbidity and cost for the health community. It is widely recognized that hand hygiene is the leading contributor infections, but hand hygiene still remains a major problem for nearly all healthcare systems. A longitudinal study was conducted over a 4-year period in a community-based health system. Methods: An electronic hand hygiene reminder system was installed in 2 different facilities including both critical care and noncritical units. This system collects data on individual healthcare provider hand hygiene and provides a real-time voice reminder in the event that a provider forgets to perform hand hygiene. The primary study was designed to investigate the impact of a real-time voice reminder to improve hand hygiene. A baseline period of hand hygiene was established prior to the interventions after installing the system without any access to data reporting or the voice reminder. Each of the hospitals had the voice reminder turned on and off 3 times. The baseline HAI rates were established by comparing in each facility for the 12 months prior to the implementation of the system. During the study period, there were no significant changes to other common infection control practices. Results: In both facilities, every time the voices were turned on, hand hygiene improved significantly and each clinical unit saw a >200% improvement in hand hygiene within 3 months of turning the voice reminder. HAIs fell by a statistically significant in all clinical areas by 51%. After a period of stabilization, the voice reminder was turned off hand hygiene compliance fell and HAI rates then increased. The voice reminder was then turned back on and off 2 more times. In every case, hand hygiene rates fell back to the baseline and HAIs returned to their baseline. When the voice reminder was then turned back on, HAIs dropped to 54%–81% of the baseline in each of the clinical units. The system also captured individual providers’ hand hygiene performance data and displayed it in a simple and engaging way, allowing managers easily understand who was struggling with hand hygiene. These data were then leveraged through a series of competitions to systematically drive hand hygiene performance improvement. These included traditional interventions to address an education issue in addition to interventions to identify workflow problems. Conclusions: Using this highly targeted approach, the leadership were able to efficiently drive sustained hand hygiene improvement and a further reduction in HAIs.Funding: NoneDisclosures: None

ZusammenfassungDerzeit existieren für Kopf- und Halskarzinome (HNSCC) keine evidenzbasierten Leitlinien für das Re-Staging oder die Tumornachsorge nach primärer Radio(chemo)therapie. In Deutschland ist die hochauflösende Sonografie mit Verwendung von Farbduplexverfahren neben der Computertomografie (CT) und der Magnetresonanztomografie (MRT) ein bewährtes Verfahren zur Therapiekontrolle (Re-Staging) im Rahmen der Tumornachsorge nach der Primärtherapie von Kopf-Hals-Karzinomen. Die PET/CT gilt seit Kurzem als Standard. Es fehlt an klaren Empfehlungen, wie sich die unterschiedlichen in der Routine angewendeten Verfahren ergänzen. Außerdem fehlt es an validen Selektionsalgorithmen für die Indikation zur PET/CT. Indikationen für und wider eine Salvage Neck Dissection werden häufig als Einzelfallentscheidung getroffen. Dieser Artikel soll die gängigen Re-Staging Modalitäten darstellen und ein aktuelles Studienkonzept kurz erläutern.

Este trabalho pretende evidenciar aproximações entre propostas éticas de Hans Jonas (Alemanha,1903-EUA,1993) e Pierre Aubenque (França, 1929-). Ambos corroboram a tese que defende a prudência como a virtude da ética que se faz necessária nesses tempos de hegemonia tecnocientífica e economia baseada no consumo. Estes eminentes pensadores do século XX também apresentam críticas semelhantes em relação ao pensamento moderno, à ética kantiana e à supremacia da razão calculante. Ambos se inspiraram no imperativo categórico de Kant e elaboraram imperativos categóricos para uma nova ética, que o francês denominou ecoética, e o alemão de ética do futuro.Abstract: This work pretends to evidence approximations between the Hans Jonas’s (Germany, 1903-USA, 1993) and Pierre Aubenque’s (France, 1929-) ethical proposals. Both corroborate the thesis that defends the prudence as the necessary moral virtue in this technical-scientific hegemony and economy based on consumption time. These XX century’s eminent philosophers also show similar critiques to the modern thinking, to the Kantian ethics and to the calculating reason supremacy’s. Both felt inspired in the Kant’s categorical imperative to elaborate new categorical imperatives to a new ethics to our time, that the French one called Ecoethics and the Germany one, ethics of future. BackgroundHierarchical cumulative scales are common and informative in psychology. The General Health Questionnaire (GHQ) does not appear to have been subjected to an analysis that examines the hierarchical and cumulative nature of its items. We report an analysis of data from the 30-item GHQ (GHQ-30) as part of the Health and Lifestyle Survey (HALS).MethodData from 6317 participants who completed the GHQ-30 as part of the HALS were analysed using the Mokken Scaling Procedure (MSP), which is a computer program that searches polychotomous data for hierarchical and cumulative scales on the basis of a range of diagnostic criteria.ResultsA final scale consisting of nine items from the GHQ-30 was obtained that, according to the criteria for a Mokken scale, was a reliable and very strong scale. The least difficult item in the scale is ‘been (un)able to face up to your problems?’ and the most difficult item is ‘felt that life isn't worth living?'ConclusionsItems from the GHQ-30 form a short hierarchical and cumulative scale. The majority of these items also appear in the GHQ-12. The nine-item GHQ shows better distribution properties than the GHQ-30 and compares very favourably with the GHQ-12.

ABSTRACT Many musicians contributed to the early spread of Schenkerian ideas in the U.S., but one played a crucial and unparalleled role during the earliest decade, the 1930s: Hans Weisse (1892––1940), one of Schenker's most esteemed students. Although Jewish, Weisse was not a political or war refugee, as were later Schenker éémigréés. Instead, he came to the U.S. to teach, in the fall of 1931; by the time his former colleagues were finding their ways from Europe, Weisse had already become an American citizen. He died prematurely in 1940, but by then his impact on the pedagogy of music theory and analysis was already being felt, not only directly, at institutions where he taught, but through his many students who were active in disseminating the Schenkerian approach. As the Schenker enterprise now enters its eighth decade in the U.S., a better understanding of its early and largely unexplored foundations becomes ever more essential, and the contributions of Weisse provide a logical point of departure. Heretofore his role has remained largely undocumented. Investigation of three aspects of Weisse's accomplishment and legacy help shed light on his pivotal role as a music educator and thinker in this country: the record of his highly active professional life as a teacher of music; the theoretical positions and pedagogical strategies revealed by his few preserved writings and remarks; and the enduring influence of his teachings. Through the work of his students, it can be seen that his contribution was decisive in laying the foundation for future Schenkerian work.

Compulsive sexual behaviour is a disorder causing substantial social damage. Understanding the experiences of individuals with this disorder can help prevent and mitigate its consequences. Researchers conducted an independent content analysis of the data obtained through qualitative methods in the cities of Shiraz and Tehran in Iran. A total of 19 women aged 20–35 years receiving treatment for their sexual compulsivity as well as a therapist were interviewed. Five main categories emerged from the data, including the resentful emotional duality (having contradictory or paradoxical emotions simultaneously, such as love versus hate and rage versus fear) felt towards their father and men in general, an early aberrant social experience (premature exposure to sexual relations) and an abnormal perception of love. It appears that the weak cultural and educational structures in place and interpersonal communication disorders contribute significantly to the formation of hyper-sexuality.

Introduction : Hospital acquired Infections (HAIs) are called those infections that were not present at the time of patient’ hospitalization in a hospital and have been acquired after hospitalization. Nurses are an important part of the any healthcare team who play a unique role in the control of Hospital acquired infections. Objective : The aim of this study was to investigate the level of knowledge, attitudes, and practices of staff nurses about preventing the spread of hospital acquired infections (HAIs) at tertiary care Hospital of Dhaka city. Methods and Materials : This descriptive cross-sectional study was done among nurses having two years experience from two tertiary level hospitals in Dhaka city during January to June 2017. Self administered questionnaire containing different set of questions regarding knowledge, attitude and practice on HAI were used as a tool for data collection. Questionnaire was supplied to all staff nurses available at different in-patient wards of these two hospitals. Only 234 staff nurses who completed and returned the questionnaire were included in this study. Data were analyzed using Microsoft excel 2013 software. Result : Staff nurses were found to have good knowledge, moderately positive attitude but poor practice in prevention of hospital acquired infections. About 95% of the participants considered that prevention of HAIs were a valuable part of their role. About 65% of the staff nurses had received formal training regarding hand hygiene. The 100 % of participants felt that they would be less likely to transmit infection to the patients if they performed hand-hygiene. About 64% of them argued that hand hygiene agents were not readily available in current settings. Regarding practice, only 6% performed hand hygiene before patients contact and 27% of the staff nurses reported that they often forgot to perform hand hygiene. Conclusion : The finding of this study revealed a good knowledge of infection prevention among the majority of participants with relatively minimal level of practice. For strengthening the knowledge, attitude and practice towards HAIs, there is in need of developing regular training program and monitoring on performance feedback regarding hand hygiene is recommended. Scholars have long argued that economic downturns intensify racial discord. However, empirical support for this relationship has been mixed, with most recent studies finding no evidence that downturns provoke greater racial animosity. Yet most past research has focused on hate crimes, a particularly violent and relatively infrequent manifestation of racial antipathy. In this article, we reexamine the relationship between economic downturns and racial acrimony using more subtle indicators of racial animosity. We found that during economic downturns, Whites felt less warmly about Blacks (Studies 1 and 2), held more negative explicit and implicit attitudes about Blacks, were more likely to condone the use of stereotypes, and were more willing to regard inequality between groups as natural and acceptable (Study 2). Moreover, during downturns, Black musicians (Study 3) and Black politicians (Study 4) were less likely to secure a musical hit or win a congressional election.

Anxiety is the narrowing of the field of perception but still may be directed to others. This studyaimed to identify the anxiety levels of students who have single parents in the exam. Method: Descriptiveresearch design. The population was 31 students who have single parents. The sample was 31 students.The Sampling technique was total sampling. The Instrument used Hars questionnaires. Result: Theresults showed that 87% students experiencing anxiety in susceptible adaptive anxiety which makes thechild’s motivation to achieve the goal. Based on the results, there were three things that stand out,namely the onset of anxiety disorders of sleep (not sleep soundly) 30%, impaired intelligence (poormemory, difficulty concentrating) 26%, and anxiety (a bad feeling before the exam) 17%. Discussion: Itwas expected that counseling can determine the level of anxiety felt by the class XII students who havesingle parents and allows students to uncover the feelings of the problem so that the learning processwill increase.

Self-identified involuntary celibates, or “incels,” have congregated online in recent years as a way to discuss and promote a particular patriarchal and blatantly misogynist ideology that blames women, specifically, and feminist society, broadly, for the unmet sexual desires of men who feel entitled, based on gender, to sexual experiences. Thus, incel ideology is an obvious example of severe sex-negativity. While incel ideology is commonly filled with hate speech and threats of violence, there are very few, fortunately, who go on to commit extreme violence. The present study examines feelings of hopelessness and helplessness among seven incel offenders who committed or clearly attempted to commit multiple murder. Although these offenders invariably felt hopelessness and helplessness across major areas of life functioning, they grossly misattributed blame to women for their overall misery. Findings provide valuable insights into the psychology of an extreme form of sex-negativity that extends a mindset of revenge rape to pseudocommando-style mass murder.
