Our aim was to re-evaluate the role of CNEP in RDS using a modified CNEP chamber. The new CNEP system comprised a thin (0.25 mm) rubber neck seal, facilities for routine and emergency patient access and an air controlled incubator heater. The protocol of the randomised controlled trial of CNEP in RDS is presented. There were 122 matched pairs in the trial. Abstract Respiratory distress syndrome (RDS) is a disease characterized by neonatal respiratory failure. It is principally caused by a deficiency of pulmonary surfactant, which is a lipoprotein mixture essential for reducing surface tension at the air-liquid interface of the alveolus. Prematurity is the major risk factor predisposing to RDS. Several pieces of evidence suggest the role of genetic factors in the susceptibility to this multifactorial disease.
