Abstract:

Abstract:
Illusory contours are one of the most widely studied kinds of visual illusion. Illusory contours are often understood as an adaptive response to filling-in missing information created from conditions of camouflage. This chapter describes a new class of very vivid illusory contours that appear impossible to understand as forms of rational inference. It presents a set of illusory contours that emerge in conditions for which there is no missing information or need for their synthesis. It argues that such contours provide a valuable testing ground for both specific theories of illusory contour formation, and general theories of perceptual organization. Videos made specifically for this chapter help illustrate the concepts discussed.

Abstract:
This chapter presents a detailed description, discussion, and reinterpretation of the Neolithic causewayed enclosure at Etton in southern Britain. Discussion focuses on the sequences of digging-filling-resurfacing-redigging at the site, and examines the differences between the ditches and the interior filled-pits, as well as the eastern and the western segments of the site, and the distribution of different categories and conditions of artifacts. One result is a rethinking of the existing understanding of the site as connected to structured deposition, ground stone artifacts, and human lives. The proposal is that the reader interprets the site as a textured surface that was cut, resurfaced, and then cut again. The chapter concludes with a discussion of the relevance of this rereading of Etton for our understanding of the pit-houses at Măgura and at similar sites.

The left atrium (LA) plays an important role in cardiovascular performance, not only as a mechanical contributor, elastic reservoir, and a primer for left ventricular filling, but also as a participant in the regulation of intravascular volume through the production of atrial natriuretic peptide.Although LA diameter in the parasternal long-axis view has been routinely employed, LA volume is a more robust marker for predicting events than LA areas or diameters. The assessment of LA performance based on two-dimensional volumetrics, Doppler evaluation of mitral, pulmonary vein flow, and annular tissue Doppler, as well as deformation imaging techniques, may provide incremental information for prognostic purposes and for the evaluation of severity and duration of conditions associated with LA overload.The aims of this chapter are to explain the basics of LA function, and to describe the role of Doppler echocardiography techniques, and how to implement them, for the non-invasive evaluation of LA in clinical practice.

Abstract:
Pericardial disease leading to pericardial effusion (PEF) is a common clinical disorder. The most common causes are viral infections, metastatic cancer, renal disease, and bleeding disorders. PEF that accumulates slowly can become quite large before haemodynamic embarrassment occurs, while PEF that accumulates rapidly from trauma or aortic dissection can be small,yet cause haemodynamic embarrassment. As the PEF increases in size, the pressure in the pericardial space increases, leading to a decrease in atrial and ventricular chamber sizes, and limiting filling of the chambers. Ultimately, cardiac output is decreased,resulting in cardiac tamponade. When the limits of the pericardial stretch are reached, the volume in the pericardial sac becomes fixed. Any additional increase of PEF results in decreased cardiac size and any change in chamber size with respiration results in a paradoxical change in size of the other chambers. Tamponade is divided into three phases based on changes in pericardial and arterial pressure and cardiac output. Doppler echocardiography is the cornerstone of the diagnosis, follow-up, and management of PEF. It provides information about the presence, size, and location of the PEF, its impact on right ventricle, right atrium, and inferior vena cava size, and assesses tamponade physiology. Comorbid conditions may modify the signs of tamponade and need to be considered during the clinical assessment. Tamponade is not an all-or-nothing diagnosis, but instead should be viewed along a continuum of progressively worsening haemodynamics.

Intellectual property right owners are increasingly paying attention to this new African dynamic and have started to invest into African markets. Unfortunately, so have counterfeiters. Taking action against counterfeit and pirated products in Africa is a challenge against which right holders and their representatives often feel overwhelmed. Filling a gap in the market, this is the only book providing a detailed overview of the law and practice in relation to intellectual property rights enforcement in all 54 African countries and through the two main regional intellectual property organisations, the OAPI and the ARIPO. For the first time, a comprehensive manual on the conditions and procedures under which the civil and criminal courts, the police and customs take action with regard to counterfeit and pirated goods is available. Arranged in a systematic manner, the reader will find for every jurisdiction information on the factual background, the organisation of the courts, the legal context, the judicial, civil and criminal enforcement of patents, trade marks and copyrights as well as information on the scope, requirements and practical application about customs recordals. Abstract:
Acute heart failure is defined as the rapid development or change of symptoms and signs of heart failure that requires urgent medical attention and usually hospitalization. Acute heart failure is the first reason for hospital admission in individuals aged 65 or more and accounts for nearly 70% of the total health care expenditure for heart failure. It is characterized by an adverse prognosis, with an in-hospital mortality rate of 4-7%, a 2-3-month post-discharge mortality of 7-11%, and a 2-3-month readmission rate of 25-30%. The majority of patients have a previous history of heart failure and present with normal or increased blood pressure, while about half of them have a preserved left ventricular ejection fraction. A high prevalence of cardiovascular or non-cardiovascular comordid conditions is further observed, including coronary artery disease, arterial hypertension, atrial fibrillation, diabetes mellitus, renal dysfunction, chronic lung disease, and anaemia. Different classification systems have been proposed for acute heart failure, reflecting the clinical heterogeneity of the syndrome; the categorization to acutely decompensated chronic heart failure vs de novo acute heart failure and to hypertensive, normotensive, and hypotensive acute heart failure are among the most widely used and clinically relevant classifications. The pathophysiology of acute heart failure involves several pathogenetic mechanisms, including volume overload, pressure overload, myocardial loss, and restrictive filling, while several cardiovascular and non-cardiovascular causes or precipitating factors lead to acute heart failure through a single of these mechanisms or a combination of them. Regardless of the underlying mechanism, peripheral and/or pulmonary congestion is the hallmark of acute heart failure, resulting from fluid retention and/or fluid redistribution. Myocardial injury and renal dysfunction are also involved in the precipitation and progression of the syndrome.
