SummaryThis commentary questions a Cochrane review that examined whether first-rank symptoms are a useful diagnostic tool for differentiating schizophrenia from other psychotic disorders. It concludes that first-rank symptoms are not particularly accurate in this role, although they might be useful initial screening questions in community surveys or waiting-room screening.

Abstract:
Based on evidence from patients with epilepsy who develop a schizophrenia-like illness, it is argued that the first-rank symptoms of Schneider are closely related to temporal lobe pathology, particularly in the dominant hemisphere. It is suggested that the first-rank symptoms should be seen as signifiers of temporal lobe pathology, as opposed to being pathognomonic for schizophrenic illness.

Delusional memories have an established place in psychiatric phenomenology and use of the term extends to its inclusion in the Present State Examination (Winget al,1974). For the general case, we prove that there exists a one-to-one correspondence, which is explicitly given, between Pfaffian systems of rank two and Pfaffian systems of rank three. That is, for a Pfaffian system of rank two in dimension five, the covariant system is constructed.
