Abstract:
This case is illustrative of migraine during pregnancy. In pregnancy, due to changes in the level of estrogen, migraines are often more frequent in the first trimester. The natural history for migraine, especially migraine without aura, is that 70% get better by the second trimester. A general strategy for management of migraines during pregnancy is illustrated. In general, prophylactic medications are not used during pregnancy, and migraines are treated symptomatically. Tables of the usual migraine medications to treat both the acute headache and to prevent migraine are included, along with their pregnancy classification and breast feeding safety. The chapter is meant to be a practical guide to migraine management during pregnancy and postpartum.

Abstract:
One in three women will present with problems, usually bleeding and/or pain, in early pregnancy prior to their planned maternity booking appointment at the end of the first trimester. The mainstay of investigation is pelvic ultrasonography. However, often the diagnosis is not clear and further ultrasound scans or repeated measurements of serum human chorionic gonadotropin concentrations are required. Most women with bleeding in early pregnancy will have an ongoing pregnancy and can be reassured. However, around 15% of pregnancies will miscarry in the first trimester. Management of miscarriage is often conservative but there are both surgical and medical, using misoprostol, management options. Importantly, approximately 1 in 80 pregnancies are ectopic pregnancies. These can be difficult to diagnose and are associated with increased maternal morbidity and mortality. Management is usually by laparoscopic salpingectomy or systemic methotrexate administration. As ectopic pregnancy can present to disparate medical practitioners it is important to have an awareness and suspicion of this condition as well as considering the possibility of pregnancy in all women of reproductive age.

General pregnancy care topics are also addressed, including types of obstetric providers; common lab tests including the complete blood count, blood type, antibody testing, and HbA1C; and imaging including the first-, second-, and third-trimester ultrasounds as well as specialized evaluations such as a nuchal translucency and fetal magnetic resonance imaging (MRI). Last, there is a explanation with examples of how to document a pregnancy history by gravidity and parity.

This chapter also reviews the possible management referrals that may be made if the patient chooses to continue the pregnancy, including perinatology, specialized imaging, cardiology, neonatology, fetal surgery and interventions, pediatric surgery, pediatric subspecialties, pathology, and palliative care. Abstract:
Multiple sclerosis (MS) often affects women of childbearing age. There are many issues to consider when counseling women about their disease and treatment during this time. The Pregnancy in Multiple Sclerosis (PRIMS) study, published in 1998, is the best large-scale prospective study published to date. Based on this trial, and those that followed, it is recognized that the rate of relapse in MS decreases during pregnancy, especially during the third trimester, but there is a significant increase in relapse rate in the first three months postpartum. If relapses do occur during pregnancy, women are often treated with methylprednisolone, but this is generally avoided in the first trimester. Disease-modifying therapies (DMTs) are usually discontinued during preconception, pregnancy, and while breast-feeding. DMTs are classified under different FDA pregnancy categories based on human and animal data. Breast-feeding may influence postpartum relapse rate, but the true effect continues to be debated.
