Abstract:
Introduction Some patients with a fishbone as a foreign body of difficult diagnosis may require further investigations. Generally, radiography is used as the first choice for finding the fishbone. Objective The objective of this study is to determine the accuracy of digital radiography for diagnosis of fishbone foreign body in the throat Methods This descriptive experimental study design has three phases. In the first phase, we assessed subject contrast and visibility of fishbone on a homogeneous background; as for the second phase, we evaluated the embedded fishbone in the fresh cadaver's throat. In the last phase, we studied the accuracy of radiography in diagnosing the fishbone foreign body at any site of the cadaver's throat. Results The subject contrast of 15 fishbones ranged from 0.94 to 0.99. All types of fishbone were obvious in the first phase, whereas, in the second phase, visibility of fishbone was varied. The subject contrast and diameter of fishbone did not show statistically significant correlation with visibility (p = 0.09 and p = 0.24, respectively). In the third phase, embedded fishbone in the base of tongue was detected with the highest accuracy (sensitivity of 1.00 (95%CI: 0.44–1.00) and specificity of 0.92 (95%CI: 0.65–0.99)); whereas, the tonsil was of difficult interpretation with poorest diagnostic value (sensitivity of 0.00 (95%CI: 0.00–0.56) and specificity of 1.00 (95%CI: 0.76–1.00)). Conclusion The digital radiography provides the highest accuracy and benefit to the diagnosis of a fishbone foreign body at the base of the tongue; whereas, the tonsil was of difficult interpretation.

We report the imaging and pathologic findings of fishbone-associated actinomycosis of the anterior cervical space in a 57-year-old man, misdiagnosed preoperatively as a malignancy originating from thyroglossal duct cyst. CT revealed an enhancing mass containing a small abscess pocket and two sharp linear calcifications within it, which infiltrated into the strap muscle. Pathologic examination demonstrated two fishbones within the actinomycotic abscess. Fishbone-associated actinomycosis should be considered when a cervical mass contains sharp linear calcifications.

Fishbones are fishery waste that can be processed and utilized into economic value products, namely fishbone meals. One fish whose bones can be used as a fishbone meal is the red snapper (Lutjanus sp). As the primary raw material for making fishbone meals, the bones of red snapper (Lutjanus sp) are rich in calcium mineral salts such as calcium phosphate and phosphorus. Calcium is a mineral that is needed by the human body. Calcium is perfect for babies, children, pregnant women, and the elderly. Calcium deficiency can cause osteoporosis. This study aims to utilize fish bone waste to have economic value and provide information to the public about fishbone meals rich in calcium minerals. This study used an experimental method with two replications. The results showed an increase in the color, texture, and odor of bone meal of red snapper (Lutjanus sp). While the Diversity Analysis (ANOVA) test showed no significant effect on each parameter

Abstract:
A retrospective study was performed on 42 consecutive patients to examine the value of radiography in the management of complaints of fishbone ingestion. All patients underwent an oral examination followed by radiographic examination with plain films, barium swallows, barium and water swallows, and an endoscopic examination. All radiographs were reviewed by two unbiased, experienced radiologists. Of the 7 cases with fishbone found at endoscopic examination, only two (29%) bones were seen by the primary radiologist and 5 (71 %) by the reviewing radiologists. Except for one case in which all three radiologists detected a fishbone that passed during the examination, and thereby was not found on endoscopy, no fishbones were found at the radiographic examination that were not seen on the endoscopic examination. Thus, radiography added no valuable information but only delayed the endoscopic examination with ensuing removal of the foreign body, which had to be performed regardless of the outcome of the radiographic examination. Therefore, we suggest that patients with a short history of complaints of fishbone ingestion, ie, 48 hours or less, should first be evaluated with oral and endoscopic examination.

Most fish processing industries only use fish meat as raw material for industry. Whereas fish bones are contain high mineral content needed by the human body and can be utilized as high calcium fishbone meal. Fishbone meal in this reaseach was made from waste from the fishery industries in Tanjungpinang, Riau Islands. Mackerel fishbone meal was made in several steps consist of preparation of raw materials, boiling of fishbones, washing and cleaning of the bones, presto processing of fish bones with the addition of sodium bicarbonate (NaHCO3), drying, siege and sieving. The purpose of this study is to determine the effect of presto duration and concentration of NaHCO3. This research was conducted by the factorial randomized block design method (RAKF) with two factors: duration of presto (2 hours (P1) and 4 hours (P2)) and concentration of NaHCO3 (0% (N1), 1% (N2) and 2% (N3)). The results showed that the duration of presto treatment and the concentration of sodium bicarbonate (NaHCO3) had a significant effect on the 5% level of yield but not significant on calsium content. The best results showed on 2 hours presto with addition NaHCO3 1%. The best treatment was used as a sample to measure the chemical characteristics of mackerel fish bone meal. Fish bone meal obtained 4.86% water content, 69.84% ash content, 3.72% protein content, 17.21% fat content, 12.56% phosphorus content, and 0.46% crude fiber. The chemical content of fishbone meal has been appropriate with Indonesian Nasional Standard.

The use of plain radiographs to localize a suspected fishbone foreign body is the subject of controversy. Accordingly radiographs of 14 species of fishbone, impacted in a soft tissue phantom, were assessed by a series of observers from the ENT department (consultant surgeons, senior registrars and house officers). The agreement was assessed by graphical description of the data and tested by a Spearman's rank correlation test The overall results showed that, for the clinician, radiography is very useful to detect the bones of: cod, haddock, lemon sole, cole fish, grey mullet and plaice; useful for red snapper, monk fish, gurnard and salmon; and unhelpful in detecting bones from herring, pike, mackerel and trout The use of radiographs to locate these impacted fishbones can be rationalized in the light of these findings.

Fishbone stimulation was simulated using ECLIPSE 100 in autumn semester project, 2013. I used LGR (Local Grid Refinement) Options in ECLIPSE to emulate fishbone needles extending to reservoir for a single vertical well. It was concluded that fishbone stimulation could increase well productivity, reduce skin factor, and reduce potential water or gas coning. ECLIPSE simulation requires some simplification, as the nature of the software itself does not enable for flexible grid shape and size, two important requirements needed for near well bore simulation.BRILLIANT is designed to deal with several physical models calculated simultaneously in the fluid flow from reservoir to well bore. It utilizes flexible grid size and shape and claims to calculate fluid flow equation accurately and effectively. Recently, BRILLIANT is furtherly developed to be compatible with porosity flow option in reservoir (Darcy flow) and update the calculation stability algorithm. Accurate simulation result is expected from BRILLIANT detail simulation for fishbone case. The goal for this master thesis is to perform fishbone simulation using BRILLIANT and compare it with ECLIPSE simulation result. The case conducted in semester project will be simulated using the latest version of BRILLIANT. This comparison result determines direction of the research afterwards. In the case of insignificant difference of both simulation results, it would be confirmed that ECLIPSE could guarantee accurate result for well performance in fishbone stimulation case. In other words, it is unnecessary to simulate reservoir and near well bore details using BRILLIANT. However, different result between ECLIPSE and BRILLIANT simulation would divulge a good recommendation to use BRILLIANT simulation result as an input to ECLIPSE simulation. Result of this research is as follows. BRILLIANT and ECLIPSE simulation gives a different result for fishbone simulation case, thus it is required to upscale BRILLIANT as an input to correct ECLIPSE fishbone simulation. The upscaling technique in ECLIPSE could be conducted by two methods: permeability or skin modification, in order to match BRILLIANT simulation result. Furthermore, controllable and uncontrollable parameters are varied to quantify those effect to fishbone well performance. The parameters include fishbone needles number, fishbone annulus size, fishbone needle length, reservoir thickness, and reservoir heterogeneity. Fishbone stimulation is also compared with the existing conventional fracturing to analyze the most effective method for increasing well performance.

L'interaction onde-particule dans les plasma est un sujet de recherche important, pour la compréhension des phénomènes physiques fondamentaux comme pour l'opération de réacteurs à fusion tels que les tokamaks. Cette intéraction peut être responsable de l'existence de modes instables, comme l'instabilité "fishbone" dans les plasmas de tokamak. Celle-ci est causée par l'interaction résonante entre un mode vivant dans la plasma et une population de particules supra-thermiques. Cette instabilité cause l'éjection d'une partie de ces particules énergétiques. Elle est par ailleurs caractérisée par une diminution de sa fréquence durant son évolution. Dans cette thèse, un modèle pour l'instabilité "fishbone", décrivant le plasma thérmique avec un traitement fluide et les particlules énergétiques avec un traitement cinétique, est développé. Ce modèle est simplifié de manière à permettre la compréhension des mécanismes les plus basiques qui causent la destabilisation du mode, sa diminution en fréquence durant son évolution ainsi que l'éjection de particules qu'il engendre. La théorie linéaire de ce modèle est faite, montrant les conditions qui permettent l'existence de l'instabilité, et permettant la caractérisation de son comportement linéaire. Les résultats analytiques sont ensuite comparés aux résultats linéaires numériques, obtenus grâce à un code développé durant cette thèse et basé sur les hypothèses du modèle, et ils sont en accord. Enfin, ce code est utilisé pour explorer le comportement non linéaire des particules énergétiques. Le mécanisme principalement responsable du changement de fréquence du mode ainsi que de l'éjection des particules est identifié et étudié en detail
The wave-particle interaction in plasmas is an important research subject, for fundamental physical understanding as well as for the operation of fusion devices such as tokamaks. This interaction can cause the existence of unstable modes, such as the fishbone instability that is observed in tokamak plasmas. It results from the resonant interaction between an electro-magnetic wave living in the plasma and a population of supra-thermal particles. This mode causes the ejection of a portion of these energetic particles, and is thus detrimental to the confinment of energy in a tokamak, and it is characterized by a frequency down-chirping, i.e. a decrease of frequency of the mode during its evolution. In this thesis, a model for the fishbone instability is developed, that describes the thermal plasma with fluid equations and the supra-thermal particles with the kinetic Vlasov equation. This model is highly simplified in order to understand the basic mechanisms leading to destabilization, frequency chirping, and particle ejection. The linear theory of this model is then done, showing the conditions that lead to the existence of an instability, and that allow the characterization of its linear behavior. The linear analytic results are then compared to numerical linear results obtained with a code, based on the assumptions of the model, that was developed during this PhD and the results are found to be in good agreement. Finally, the code is used to explore the nonlinear behavior of energetic particles in the later phase of the fishbone instability. The main mechanism responsible for the frequency chirping and energetic particle ejection is identified and studied in detail

Dans les plasmas de tokamak, les particules rapides générées par les réactions de fusion et par les méthodes de chauffage non-inductives peuvent interagir avec les instabilités Magnéto-Hydro-Dynamiques, conduisant potentiellement à leur transport en dehors du plasma de coeur. Cette problématique est importante dans le contexte des plasmas en combustion, où la relaxation collisionnelle des particules alphas est nécessaire pour compenser la perte d’énergie lors la décharge. Le temps de transport résonant des particules rapides étant bien plus petit que leur temps de thermalisation sur le plasma thermique, ces instabilités MHD-cinétiques peuvent engendrer une dégradation de l’efficacité énergétique de la réaction, qui est d’importance cruciale pour les futurs réacteurs commerciaux. Dans cette thèse, nous étudions l’interaction des ions énergétiques avec le mode de kink interne, qui conduit à l’émergence de l’instabilité fishbone. À cette fin, nous utilisons le code non-linéaire hybride XTOR-K, pour simuler les phases non-linéaires du fishbone ionique, durant lesquelles les particules rapides sont transportées. Dans un premier temps, la théorie linéaire de l’instabilité fishbone est reproduite, retrouvant une relation de dispersion similaire à celle du modèle de Porcelli. Un écart est constaté pour les particules très passantes. Un code linéaire a été implémenté pour résoudre de façon non perturbative la relation de dispersion du fishbone obtenue. Les résultats obtenus avec ce code sont cohérents avec ceux d’XTOR-K dans la phase linéaire des simulations, avec des taux de croissance, des fréquences de rotation et des surfaces de résonance similaires. Ces résultats fournisent une vérification linéaire de XTOR-K, permettant son utilisation sur des équilibres plus complexes, et durant des phases non-linéaires. Dans un second temps, une étude paramétrique est fournise par XTOR-K sur la stabilité linéaire du fishbone alpha, avec des équilibres pertinents pour le cas ITER 15 MA. Nos simulations montrent que ce scénario sera probablement instable à l’égard du fishbone alpha, pour des densités de particules alpha réalistes dans ITER. Les résultats non-linéaires obtenus avec XTOR-K sur un équilibre circulaire peu énergétique et sur le cas ITER 15 MA sont ensuite présentés. Ces résultats documentent la dynamique auto-cohérente des particules rapides et des modes MHD lors d’une oscillation fishbone. Le transport résonant de particules rapides est une caractéristique commune dans ces simulations, accompagnée d’une décroissance charactéristique de la fréquence du mode MHD-cinétique. Les différences dans ces simulations sont discutées, ainsi que le régime non-linéaire charactérisant le mode observé sur la base des théories existantes. Durant une oscillation fishbone, dans nos diffrentes simulations, le transport total de particules alpha au coeur du plasma est de l’ordre de 5 à 10% de la population initiale, ce qui montre que la réduction des performances de fusion due à l’instabilité fishbone-alpha est limitée. À partir de ces simulations, un mécanisme expliquant le couplage non-linéaire entre le transport résonant de particules et la décroissance en fréquence du mode MHD-cinétique est présenté
In tokamak plasmas, fast particles generated by fusion reactions and by non-inductive heating techniques can resonantly interact with Magneto-Hydro-Dynamic (MHD) instabilities, potentially leading to their transport out of the plasma core. This topic is important in the context of burning plasmas, where the collisional relaxation of alpha particles is expected to compensate the energy losses. The resonant transport time of fast particles being much lower than their typical relaxation, these Kinetic-MHD instabilities can adversely impact the plasma energy balance, and therefore the fusion performance of future commercial reactors. In this thesis, we study the interaction of energetic ions with the internal kink mode, resulting in the onset of the fishbone instability. To this end, we use the nonlinear hybrid Kinetic-MHD code XTOR-K to simulate the nonlinear phases of the fishbone instability, during which fast particles are being transported. Firstly, the linear theory of the fishbone instability is re-derived, recovering similar expressions with Porcelli’s dispersion relation. Differences arise when considering deeply passing particles. A linear code has been implemented to solve non-perturbatively the fishbone dispersion relation obtained. Results with this code are consistent with those obtained from XTOR-K linear simulations, in terms of mode growth rates, rotations frequencies and resonant surfaces. This provides a linear verification of XTOR-K, that enables its use on complex equilibria and during nonlinear phases. Secondly, a parametric study is provided by XTOR-K regarding the alpha fishbone linear stability with equilibria relevant for the ITER 15 MA case. Our simulations show that this scenario is likely to be fishbone unstable for ITER relevant alpha particle densities. Finally, nonlinear results obtained with XTOR-K in low energy circular equilibria and in the ITER 15 MA case are presented. These results document the self-consistent dynamics of fast particles and MHD modes during one fishbone oscillation. Resonant fast particle transport is a common feature of these simulations, together with frequency chirping of the Kinetic-MHD mode. Differences between these simulations are discussed, as well as the nonlinear regime characterizing the observed mode based on existing theories. During one fishbone oscillation, in our different simulations, the overall alpha particle transport in the core plasma impacts from 5 to 10% of the initial population, which shows that the reduction of fusion performance due to the alpha-fishbone instability is limited. From these simulations, a mechanism explaining the nonlinear coupling between resonant particle transport and mode chirping is presented

This research project focuses on the application of process improvement techniques in a clothing manufacturer to address delay problems in workflow in the factory. The objective of the research is threefold; investigate delays at the beginning of production and make suggestions, show the usefulness of continuous improvement techniques in improving activities in a clothing manufacturer and demonstrate how action research can be used in doing research in production and operation management. Using tools such as flow charts, check sheets, pareto analysis, fishbone diagrams, interviews and the "ask why five times" tool, an investigation into delays led to a second investigation into sewing defects. This established that these sewing defects are caused mainly by time constraints, the malfunctioning of machines, the wrong handling of garments, and previous operations. After an investigation using the above tools it was established that these defects could be addressed by setting realistic targets, doing regular maintenance on machines, cautioning operators to be more careful during their operations, and encouraging regular checks on garments before the next operation. The action learning methodology led to the following lessons; selecting a correct measuring tool is important, that not all tools need to be used, and that it takes time to do a research project using this method.

The integration of Middle and Eastern European states into democratic processes has determined changes in public sector management and with rationalization of sectors of Lithuanian economy the system of justice was also involved: in the year 1993 the Parliament approved the outlines of the reform of the system of justice which set that legal bases of the reform have to be formed, specialists are to be prepared employees selected, the necessary financial and economical basis created, new legal institutions have to be created and the old ones reformed in order to ensure the stable system of justice. The reform has revealed both the advantages and disadvantages of the system of justice: not all the areas of the system of justice have received the appropriate attention. A similar situation occurred when carrying out the outlines of the Reform of the System of Justice approved in the year 1998. An appropriate and effective functioning of legal proceeding has been emphasized in international acts of law and experts’ meetings: democratization of management of prosecution system and improvement of the legal regulation of its management as well as more operative fulfillment of its functions are necessa ry; the reorganization has not only to strengthen prosecutors’ professionalism but also to ensure the adequate working conditions. Vidurio ir Rytų Europos valstybių įsijungimas į demokratinius procesus sąlygojo viešojo sektoriaus valdymo pokyčius, o racionalizuojant Lietuvos ūkio sritis, nebuvo aplenkta ir teisinė sistema: 1993 m. Abstract:

Detta är ett naturvetenskapligt arbete som omfattas både av en kvantitativ och en kvalitativ undersökning. Syftet med arbetet är att undersöka mellanskarvens födoval i sjön Sommen, samt att närmare studera huruvida det unika beståndet av röding och öring ingår i mellanskarvens födoval. Sommens Fiskevårdsområde har fått tillstånd till skyddsjakt på mellanskarv av Länsstyrelsen och det främsta skälet skulle vara att fågeln prederar röding och öring. Genom att analysera mellanskarvens spyboll och jämföra innehållet med referensfiskar fångade från samma sjö ska vi se om beslutet från Länsstyrelsen är befogat. Insamling av spybollar och referensfiskar utfördes i fält medan analys av spybollar utfördes på laboratorium. I resultatet av undersökningen konstaterades att mellanskarven i Sommen nästan uteslutande äter abborre. I de analyserade spybollarna som omfattas av denna undersökning återfanns inte röding och öring. Detta skulle innebära att Länsstyrelsens beslut om skyddsjakt på mellanskarv i Sommen p.g.a. predation av röding och öring inte är befogad.

Abstract The trajectory placement of a horizontal well that crosses a heterogeneous tight gas carbonate reservoir is one of the important elements that contribute to the success of well productivity. Proper placement can yield good production and vice versa improper placement may give low well productivity. An effort was studied to improve the well productivity from an existing single horizontal well that was initially placed in sub optimal location by implementing the Fishbones completion technology (work-over). In this paper, Simulation Opportunity Index (SOI) has been selected as a method to indicate the remaining gas sweet spot throughout the reservoir. SOI integrates 3 independent components extracted from static and dynamic parameters; reservoir permeability-thickness, movable gas, and reservoir pressure from a historically-matched dynamic model. By utilizing SOI, a map of the prospective gas sweet spots can be created; hence low performance existing wells are utilized to exploit the surrounding potential sweet spots using Fishbones completion, which consists of tiny short needles with a maximum effective length of 32 feet placed along the horizontal section. The study reveals that the Fishbones completion application on existing low-productive horizontal well can multiply the well productivity in multi-layer reservoir environment in addition to the significant production gain. Assisted History Matching (AHM) is used to explore the best scenario of Fishbones features combination, such as the number of Subs required as the container room to hold the needles, the optimum needles length, and the optimum lateral drain section to place the Subs and needles. Many sensitivities with the abovementioned variables are run at once, and the analysis is conducted to identify the most impacting parameter to bring the highest well recovery. The use of SOI method to scrutinize the best location of well candidatures for Fishbone application and the required Subs and needles placement is not only able to rejuvenate the performance of the problematic well, but it can aid in generating CAPEX saving and efficient project schedule to manufacture Fishbones with a proper number of Subs and needles compared to blind Fishbone technology installation. A combination of two techniques between reservoir simulation study to generate SOI with the new advanced completion technology called Fishbones applied on low-performance existing horizontal well is the real study integration with the final objective to increase the ultimate gas recovery in a tight gas carbonate reservoir. This is a breakthrough solution to optimize the development of tight carbonate gas fields in addition to the conventional development strategy with normal infill drillings.

Abstract In the last years it has become technically- and economically feasible to drill productivity enhancing fishbone wells in tight and/or thin formations or in areas where hydraulic fracturing is not permitted due to local environmental regulations. Modeling of fishbone wells is complex and currently very time-consuming. A fully integrated workflow has been developed to model fishbone wells, using dynamic models to generate reliable production forecasts. This novel workflow comprises of the implementation of pre-defined well trajectories and number of needles in a box- and/or full-field dynamic model. Subsequently, the number of needles and number of wells to be drilled is optimized using a workflow driven by project economics. The workflow comprises the ability to generate forecasts for different well configurations, such as horizontal well length, well depth, number of needles and needle lengths. The needles are modeled explicitly as multilaterals. The needles cannot be oriented during drilling. To obtain a reliable prediction of the recovery of the fishbone wells, a so-called "needle rotation randomizer" has been developed. The orientation of the needles is important in the prediction of future production, as the ratio between the horizontal- and the vertical permeability (KvKh) can be significant in tight formations. The attractiveness of different well- and needle configurations, as well as the number of fishbone wells is assessed as part of the workflow. This paper addresses how different scenarios, including variation of the number of needle stations (subs) and wells impact project economics by applying a simplified Net-Present-Value (NPV) calculation. Modelling results highlight that more subs will lead to a higher production rate, with corresponding higher NPV's. However, too many subs will lead to lower NPV's, as recovery increase becomes more marginal. It will be demonstrated how modelling can be used to optimize the number of subs. With further full-field dynamic modeling the production forecast can be enhanced by varying well numbers and their spacing the using the optimized number of subs. This paper will showcase how, a fully optimized full field development plan, applying fishbone wells can be generated using the applied workflow in a very short timeframe. The novel workflow involves the generation reliable production forecasts for fishbone wells, employing a "needle rotation randomizer". Secondly, the new workflow enables optimizing the number of subs and wells, using an efficient economic optimization assessment.

Abstract This paper describes a schematic representation of product retirement specification that aids in design for recycling and reuse. In the past decade, a graphical representation of the assembly process, called the assembly fishbone diagram, has effectively assisted engineers to conduct design for assembly (DFA) and process failure modes and effects analysis (FMEA). On the other hand, environmentally conscious manufacturing requires engineers to make advanced planning for product retirement. This study investigates the use of the reverse fishbone diagram to model the disassembly and reprocessing sequence of a product at the end of its useful life. An industry-provided student project guided us to an initial definition of the reverse fishbone diagram that effectively led the students to analyze the recyclability and make practical redesign suggestions. The diagram is continuously adding more rigorous definitions and promises to be a central tool for evaluation of recyclability in a simultaneous engineering setting.

Abstract:
This paper introduces a novel airfoil morphing structure known as the Fishbone Active Camber (FishBAC). This design employs a biologically inspired compliant structure to create large, continuous changes in airfoil camber and section aerodynamic properties. The structure consists of a thin chordwise bending beam spine with stringers branching off to connect it to a pre-tensioned Elastomeric Matrix Composite (EMC) skin surface. Actuators mounted in the D-spar induce bending moments on the spine through an antagonistic pair of tendons in a manner similar to natural musculature systems. Several potential morphing configurations using this concept are introduced. The paper then focuses on a trailing edge morph wherein the compliant spine connects a rigid leading edge D-spar to a solid trailing edge strip. The motivation for exploring this novel morphing architecture is established through analytical aerodynamic comparison to the NACA 0012 airfoil with and without a discrete trailing edge flap. A prototype device is built to explore various aspects of manufacturing this concept, and to prove the large deflection capability of the FishBAC.
