Based on fishbone diagrams, the problem was obtained that the implementation of handover of patient patients was not optimal.Results: To optimize the implementation of handover, implementation was carried out in the form of improving SPO, improving nurses' effective communication with SBAR and making role play video handover.Conclusion: The result is the handover can be optimal with the role and function of management of the head nurse especially a directing role. Recommendations given by follow up and revising the hand over standars, make a pilot project nursing ward and evaluate the implementation of the patient's handover through regular supervision.Keywords: Fishbone Diagram; Handover; Head Nurse; Optimization; DirectingPendahuluan: Serah terima perawat antar shift jaga merupakan salah satu komponen penting dalam mencapai keselamatan pasien dan mempertahankan kualitas asuhan.Tujuan: Meningkatkan optimalisasi fungsi pengarahan kepala ruang dalam pelaksanaan serah tertima antar shift di RS X Jakarta.Metode: Menggunakan study analysis situasi dengan pendekatan diagram tulang ikan, dimulai dengan mengisi kuesioner, observasi dan wawancara dan kemudian dianalisis menggunakan diagram tulang ikan. Berdasarkan diagram tulang ikan, masalah diperoleh bahwa pelaksanaan serah terima pasien pasien belum optimal.Hasil: Untuk mengoptimalkan pelaksanaan serah terima, implementasi dilakukan dalam bentuk meningkatkan SPO, meningkatkan komunikasi efektif perawat dengan SBAR dan membuat role play video serah terima.Simpulan: Hasilnya adalah serah terima dapat dioptimalkan dengan peran dan fungsi manajemen kepala ruang khususnya fungsi pengarahan. Rekomendasi yang diberikan dengan menindaklanjuti dan merevisi SOP serah terima, membuat ruang percontohan dan mengevaluasi pelaksanaan serah terima pasien melalui pengawasan rutin.

The paper is intended to analyze trucks rear-end accident by applying fishbone diagram and fault tree analysis methods and study the countermeasures. And applying fishbone diagram to analyze the cause of trucks rear-end collision accidents systematically and logically and classify the accidents cause. The paper drew the fault tree of trucks rear-end collision accidents, sought the way of controlling the accidents through least path-set of fault tree, and found the main factors that influence the truck traffic safety according to the result of structure importance level. According to the analysis of the fishbone diagram and fault tree results, the paper researched the preventive measures of truck traffic accidents, and draws up safety checklist to ensure the effective implementation of the measures.

Abstract:
This study aims to determine the potential hazards that have the highest risk value as many as 3 types of work accidents in labs in the Private Vocational Laboratory in the Loceret Nganjuk area in accordance with the Risk Rating Number and provide recommendations for improvement on the types of activities that have the highest risk value as many as 3 types of work accidents in activities practicum at the Private Vocational Laboratory in the Loceret Nganjuk area using the Fishbone Diagram. The research method uses the Hazard Identification And Risk Assessment (HIRA) method and Fishbone Diagram. From the results of the study, the types of activities that have the highest Risk Rating Number are tripping Spare Parts / workshop tools with a Risk Rating Number of 8, tripping handtool / work tools with a Risk Rating Number of 8 and breathing in smoke with a Risk Rating Number of 8. Of the three types These activities are categorized as medium priority risk level / significant risks entered in index III B, namely the occurrence of unwanted potential. Then an analysis is carried out using a Fishbone Diagram by providing recommendations, namely conducting activities in a laboratory carefully and always using PPE as well as complying with the rules and knowing the functions of each - each Spare Part, work equipment and sharp-smelling objects. Pengendalian kualitas dalam proses produksi makanan, mempunyai kebutuhan yang berbeda bagi setiap produsen dan pelanggan. Untuk memenuhi kebutuhan produsen, perusahaan dapat membuat barang berkualitas sesuai keinginan produsen. Salah satu komponen dalam pembuatan makanan diantaranya yaitu pembuatan tepung roti, Akan tetapi saat pembuatan tepung roti tidak terlepas dari ketidaksesuaian saat aktivitas proses produksi dan salah satu faktor lainnya adalah proses pengemasan yang menyebabkan tekstur tepung tidak renyah. Proses pengawasan kualitas produk menjadi prioritas dan krusial yang harus dilakukan di setiap proses produksinya. Tujuan penelitian ini adalah untuk mengendalikan kualitas tepung roti. Adapun metode yang digunakan untuk menganalisa masalah yang terjadi yaitu diagram pareto dan fishbone diagram. Dapat disimpulkan bahwa defect terbesar adalah kerapatan kemasan, yang disebabkan oleh beberapa yaitu faktor lingkungan terdiri dari penerangan yang baik, faktor material terdiri dari cup mudah rusak dan tinta cepat luntur, faktor manusia yaitu operator belum sesuai dengan standar operasional prosedur, faktor metode yaitu saat aktivitas pencetakan tidak maksimal dan proses seal tidak rapat, dan faktor mesin terdiri dari mesin cetak yang kurang maksimal dan mesin seal kurang panas. Dengan demikian saran perbaikan yang perlu dilakukan dalam pembuatan kemasan tepung roti, yaitu (1) diberikan penerangan yang layak di area lingkungan kerja, (2) penggantian material kemasan yang kuat agar cup tidak mudah jebol sehingga tinta tidak cepat luntur (3) operator mengikuti standar operasional agar lebih teliti (4) metode kerja saat pengemasan perlu kalibrasi isi tepung agar tidak berlebih, (5) optimalkan saat pencetakan agar proses seal lebih rapat

Abstract:
Research on the Quality of Student Worksheets (LKPD) using the fishbone diagram strategy aimed to determine the quality of the product in the form of Student Worksheets which were used as teaching material media to help students in developing writing. For this reason, students tended not to have difficulties when learning the writing descriptive text skills. This LKPD used fishbone diagram strategy that resembled a fishbone used in speaking and writing skills. This research was a research and development (R&D). The data analysis technique was quantitative description. The results showed that Student Worksheet writing descriptive text using the fishbone diagram strategy was feasible to be used as a companion to teaching materials. The validation results from material experts stated that LKPD obtained a percentage of 97.6% and 91.7%, while the validation from linguists achieved feasibility with a percentage of 88% and 86%. The feasibility test for field experts (teachers) got a percentage of 93.68% and 94.73% and the trials on grade VII students of SMP Negeri 1 Kota Dumai were obtained with a percentage of 84.62%.

PT XYZ is one of the companies engaged in the coal mining and energy in South Sumatra Province. Mining activities at the Pit A are still focused on stripping overburden using the Komatsu PC-3000E power shovel digging tool. The company has established a 513,333 BCM production target of overburden stripping in November 2020. While the total overburden production obtained is 488,162 BCM. The existence of factors that affect the productivity of the tool will determine the achievement of production targets, so it is necessary to evaluate production by identifying the factors causing the problem. One of method that can be used to determine the root cause of the problem is by using a fishbone diagram. A Fishbone diagram is a method used to identify the root cause of a quality problem. The purpose of this research is to calculate overburden production in November 2020, identify factors that influence the achievement of overburden production targets using a fishbone diagram, and make recommendations for improvement plans based on factors that affect the achievement of overburden production targets. Based on the results of the fishbone diagram analysis using the 4M pattern consisting recommendations for improvement plans can be designed for each factor causing the problem which is machine, methods, materials, and manpower.

Abstract:
Post-harvest is an activity processed from harvesting to becoming a product. Post-harvest handling aims to maintain the quality and minimize the loss of the agricultural product. Post-harvest loss of okra is the reduction of the okra harvest amount from picking up to transporting process. Many factors can cause the post-harvest loss of okra. This study aimed to determine the causal sources of the okra loss. This study used FMEA and fishbone diagram. Fishbone diagram was used to identify the causes of the okra loss in a fishbone skeleton. FMEA was used to give the value on the causes of the okra loss by assessing the severity, occurrence and detection to get the RPN value. The RPN value was used to rank the potential causes of the loss. From the result of the study, it was found that there were 4 main factors causing the okra loss which had the RPN value above the RPN critical value. Abstract:
The creative industry is one of the industries that currently supports the Indonesian economy. The purpose of this study is to formulate the distribution model of Jember's creative industry towards the international market. This study is quantitative-descriptive research conducted in Jember using SMEs as population. Furthermore, samples were taken by using a non-probability sampling technique with a convenience sampling method. The analytical tool used in this study was a fishbone diagram. The results of the fishbone diagram analysis showed several causes of SMEs growth problems such as the lack of HR skills in foreign languages, the inadequate of sophisticated technology in business, and the minimal amount of business capital. The inappropriate adoption of the technology was then identified as the root cause of the problem. Thus, the distribution model for the creative industry to increase competitiveness in the international market is the technology application in every element of the SMEs. This paper uses a quality management tool called the fishbone diagram (Ishikawa diagram), which is based on non-numeric data, to show what solutions are to solve an analyzed problem: the reducing pollution from road transportation in agglomerated urban areas. This study aims to identify the possible, main and secondary causes that could generate the studied problem. The factors that determine the reducing pollution from road transportation are presented. Factors are grouped into several main categories. In the paper a new model of the fishbone diagram which highlights main and secondary categories of potential causes that determine the problem solving is presented. The diagram allows for a clear definition of the problem studied and provides a visual graphical chart of the factors that determine the studied problem. The fishbone diagram provides guidelines for city development to protect the environment and optimizing the level and type of road transport to reduce the pollution phenomenon. The paper aims to group the factors that determine the reducing pollution from road transportation in agglomerated urban areas on main categories, so that appropriate action will subsequently be taken by the main responsible actors. The paper presents four main factors: policies and legislation, local authority management, car manufacturers and car users.

Saraswati Coconut Product (PT.SCP) ini merupakan salah satu perusahaan manufaktur yang memproduksi briket arang (charcoal). Kegiatan produksi briket arang berada di Briquettes Department Production. Produksi briket dilakukan secara Continue Process, digunakannya Continue Process karena proses produksi dilakukan tanpa henti (24 jam) dan sistem produksi yang diterapkan Briquettes Department di PT. SCP adalah Make To Order. Produksi briket tidak terlepas dari masalah yang berkaitan dengan tidak seimbangnya aliran produksi. Pada aliran produksi briket PT. Saraswati Coconut Product terdapat antrian material yang menghambat aliran produksi hingga terjadi bottleneck. Akibatnya proses produksi briket arang menjadi lebih lama dari waktu normal produksi. Untuk mengetahui penyebab permasalahan bottleneck secara detail, maka perlu dilakukannya identifikasi penyebab masalah dari sistem produksi briket dengan menggunakan metode diagram fishbone (tulang ikan). Diagram fishbone (Diagram Tulang Ikan) adalah diagram sebab akibat yang dapat digunakan untuk mengidentifikasi potensi masalah kinerja. Diagram tulang ikan menyediakan struktur untuk diskusi kelompok sekitar potensi penyebab masalah tersebut. Tujuan dari analisa ini mengetahui penyebab bottleneck dan tindakan perbaikannya. Hasil observasi lapangan dapat diketahui bahwa penumpukkan material (bottleneck) terjadi pada jalur aliran bahan stasiun pretel dan stasiun drying. Penumpukkan material terjadi karena hasil cetakan briket dari stasiun pretel harus menunggu terlebih dahulu sebelum melalui proses pengeringan pada oven. Adapun rata-rata waktu proses oven untuk mengeringkan briket dari tujuh (7) oven yang ada di stasiun drying adalah 66 jam. In the development of a system, quality assurance is needed. A system can be said to be of good quality if it meets the needs of the user. Quality can be measured from various points of view. One point of view to measure software quality is to focus on service management systems. To determine the quality of a system, it is necessary to analyze the quality of the system. The school library system at MTs Muhammadiyah 8 Ujungpangkah has implemented a lot of service management systems to support the process of activities in the library. However, it is not yet known whether the system is in accordance with the assessment standards or not.The author uses the ISO 20000 standard to assess the requirements for obtaining a certificate in the library application by conducting various analyses. This study aims to analyze the application of the library system with ISO 20000. The analytical research methods used include determining clauses, checklists, Likert scale, maturity levels, and fishbone diagrams. The references that I take are based on the results of the analysis of the writing team that I have made including graphs, diagrams, clause scores, checklist scores, Likert scales, maturity levels, and fishbone diagrams. Based on the results of the author's analysis, the school library application system at MTs Muhammadiyah 8 Ujungpangkah does not meet the ISO 20000 standard so that there is still much that needs to be improved in the library system. The Likert scale score is only 11%, while to meet the ISO 20000 standard the Likert scale score must reach at least 60%. The overall problem with this analytical assessment that results in the Library System application not passing the ISO 20000 quality standard is the lack of a Likert scale score and fishbone diagram. With fishbone diagram, the main root cause is found with organitation structure that is not clear to monitor the application working. The hypothesis in this study can be the basis for future research so that the application can meet the feasibility test of the ISO 20000 quality standard.

Makassar adalah salah satu kota yang saat ini sedang berkomitmen menjadikan kota makassar bebas sampah atau dengan slogan makassar tidak rantasa. penelitian ini bertujuan untuk mengetahui implementasi kebijakan. Untuk mengukur pelaksanaan kebijakan makassar tidak rantasa penelitian menggukana teori fishborn , konsep dasar dari diagram fishbone adalah permasalahan mendasar diletakkan pada bagian kanan dari diagram atau pada bagian kepala dari kerangka tulang ikannya. Implementasinya masih kurang dan masyarakat harus turun tangan juga membantu pemerintah dalam mewujudkan makassar menjadi kota bersih dimana masyarakat harus menjada kebersihan lingkungan masing – masing, aktifkan gotong royong di setiap kelurahan – kelurahan kota makassar tersebut agar semuat turut terlibat dalam peogram kerja MTR atau makassar tidak rantasa dan penyebab utama agar implementasi kebijakan tersebut bisa berjalan sesuai yang di inginkan di mana soal peralatan kebersihan yang harus lengkap di setiak kelurahan yang berada di masing-masing kecamatan kota makassar. Insiden cedera tekan akan menunjukkan kualitas pelayanan suatu rumah sakit dan memberikan dampak terhadap pasien, keluarga dan petugas yang merawatnya. Tujuan penulisan untuk menganalisis faktor- faktor yang mempengaruhi terjadinya peningkatan angka insiden cedera tekan di rumah sakit. Metode yang dipakai menggunakan studi kasus analisis situasi dengan pendekatan diagram fishbone dengan mengelompokan kedalam 5 tools manajemen man, money, method, material, machine untuk mendapatkan akar masalah. Masalah utama yang didapatkan adalah belum optimalnya pelaksanaan pencegahan cedera tekan di rumah sakit. Hasil yang didapatkan adalah perlu dikembangkannya suatu teknologi sebagai inovasi alternatif. Berdasarkan hal tersebut dikembangkan inovasi yang disebut electronic_Pressure Injury Alarm (e_PIA). Inovasi e_PIA yang telah dikembangkan, perlu disempurnakan dan di uji lebih lanjut agar dapat diterapkan untuk membantu menurunkan angka insiden cedera tekan. The paper presents the results of a study concerning the use of the fishbone (Ishikawa) diagram in analyzing the causes (factors) that determine the success of the research and development projects. The studied problem was “the successful development of R & D projects” and this constitutes the head of the fishbone (Ishikawa) diagram skeleton. This study aims to identify the possible, main and secondary causes that could generate the studied problem. For the proposed problem the type factors will be identified: man, methods, machines, management, materials, and others. It will draw a fishbone diagram because it has several advantages. The diagram allows for a clear definition of the problem studied and provides a visual graphical diagram of the factors that determine the problem - the success of a research and development project. It is proposed the steps of realization the diagram. They are determined many possible causes and these are grouped into seven categories of causes for the problem: Man, Methods, Materials, Machines and equipments, Management, Political factors, Social and Economic factors. This paper proposes a new diagram model (5M+P+SE), a fish skeleton that is composed of seven main branches, thus: five type M branches, one P-type branch and one SE-type branch. The development of the Ishikawa diagram in a detailed form for determining the possible causes of a problem has the advantage of giving the possibility of identifying and analyzing all the factors connected to the problem. Knowledge the success factors is important in order to create all the favorable conditions for a research and development project to be successful. Finally, the proposals for the successful development of a R & D project will materialize in practical solutions.

Metode dasar yang digunakan adalah metode deskriptif dengan teknik pengambilan data menggunakan teknik observasi, wawancara, kuesioner, serta dokumen dan pencatatan. Data yang digunakan adalah data primer dan sekunder. Penelitian dilakukan di Kabupaten Ngawi. Metode analisis data yang digunakan dalam penelitian ini adalah analisis pendapatan usahatani, analisis efisiensi usahatani, serta analisis fishbone diagram. Berdasarkan hasil analisis menunjukan bahwa pendapatan usahatani padi konvensional lebih tinggi apabila dibandingkan dengan usahatani padi organik dengan nilai Rp. 18.131.817,66,-/ha/MT berbanding dengan Rp. 11.330.399,35,-/ha/MT. Analisis Efisiensi menunjukan bahwa kedua jenis usahatani telah efisien namun nilai efisiensi usahatani padi konvensional lebih tinggi apabila dibandingkan dengan usahatani padi organik dengan nilai 3,09/MT berbanding dengan 2,01/MT. Berdasarkan analisis fishbone diagram menunjukan bahwa faktor yang paling berpengaruh adalah dari faktor manusia yaitu dengan permasalahan penerimaan uang yang terlambat.Kata Kunci : Efisiensi Usahatani, Fishbone Diagram, Padi Organik, Pendapatan, Petani.

Bank Indonesia (BI) is the Central Bank of the Republic of Indonesia which is responsible for achieving and maintaining the stability of the rupiah value, such as maintaining the stability of volatile food. Volatile food can be maintain using cluster development, one of them is the shallot commodity cluster. This study aimed to determine the development of shallot commodity clusters in the context of controlling inflation at the Tegal Bank Indonesia Representative Office. Data collection techniques used were observation, interviews, and literature study. The data analysis technique used is descriptive qualitative analysis through a fishbone diagram instrument. The results of the fishbone diagram analysis showed the factors that influence the shallot commodity on inflation control in Brebes Regency are farmers, upstream factors, environmental factors, and downstream factors. Shallot is the main contributor to inflation in Brebes district, in 2016 these commodities contributed to inflation by 0.33%, in 2017-2019 commodities contributed to deflation of -0.26%, -0.22274% and -0.0883%. The conclusion is the development of shallot commodity cluster in the context of controlling inflation in the Tegal Bank Indonesia Representative Office using a fishbone diagram instrument considered to be good enough.

This study aims to determine the assessment of the quality of administration staff services at the Education Office of South Sumatra Province (Dinas Pendidikan Provinsi Sumatera Selatan) based on the analysis results of five dimensions of service quality in the SERVQUAL (service quality) model, named Tangible (Physical Evidence), Empathy (Care), Realibility, Responsiveness (Quick Response) and Assurance (Certainty). The collected data was analyzed using a questionnaire in the form of a SERVQUAL scale that describes the gap (GAP) of the expected service quality between expectations and reality (perceptions) received by the Educators and Education Personnel of the South Sumatra Province Education Office. The results of the study indicate that the quality of personnel administration services has not been a satisfactory because of the dimensions that have a minus or negative value, named: Realibility -0.15 and Responsiveness -0.46. Therefore, the South Sumatra Provincial Education Office(Dinas Pendidikan Provinsi Sumatera Selatan) needs to improve the quality of administrative services so that it is optimal and changes the gap to a positive value. Recommendations for improvement are made using the Fishbone Diagram and the Kaizen 5W 1 H method. The fishbone chart is useful for showing the main factors that affect quality and have an impact on service quality. If the problems and causes are known for certain, then the corrective actions and steps will be easier to conduct. To create a Fishbone Diagram, the Kaizen Method Tools are used by asking several questions on the basis of 5W + 1H (what, who, why, where, when and who). Keywords: : SERVQUAL, GAP analysis, Administrative Services Abstrak Penelitian ini bertujuan untuk mengetahui penilaian kualitas pelayanan administrasi kepegawaian Dinas Pendidikan Provinsi Sumatera Selatan didasarkan pada hasil analisa lima dimensi kualitas jasa dalam model SERVQUAL (service quality) yaitu Tangible (Bukti Fisik), Emphaty (Kepedulian), Realibility (Keandalan), Responsiveness (Keandalan/Cepat Tanggap) dan Assurance (Kepastian). Data yang terkumpul dianalisa menggunakan kuesioner berupa skala SERVQUAL yang menggambarkan kesenjangan (GAP) kualitas pelayanan yang diharapkan antara (ekpetasi) dan kenyataan (persepsi) yang diterima oleh Tenaga Pendidik dan Tenaga Kependidikan Dinas Pendidikan Provinsi Sumatera Selatan. Hasil penelitian menunjukkan bahwa kualitas pelayanan administrasi kepegawaian belum memuaskan karena ada dimensi yang bernilai minus atau negative yaitu : Realibility -0,15 dan Responsiveness -0,46. Oleh karena itu Dinas Pendidikan Provinsi Sumatera Selatan perlu meningkatkan kualitas pelayanan administrasi agar optimal dan merubah Gap menjadi bernilai positif. Rekomendasi perbaikan dilakukan dengan Diagram Fishbone dan Metode Kaizen 5W 1 H. Diagram tulang ikan (fishbone chart) berguna untuk memperlihatkan faktor-faktor utama yang berpengaruh pada kualitas dan mempunyai akibat pada kualitas pelayanan. Kajian ini bertujuan untuk mengidentifikasi faktor-faktor yang berpengaruh dalam meningkatkan efektivitas pengelolaan sistem jaringan irigasi. Kajian ini menggunakan metode deskriptif berdasarkan kerangka fishbone dan regresi linier berganda. Diagram fishbone digunakan untuk menemukan kemungkinan penyebab masalah dalam pengelolaan irigasi. Faktor-faktor penting dari diagram fishbone lantas dipertajam dengan metode analisis regresi linier berganda untuk mengetahui seberapa besar faktor tersebut mempengaruhi manfaat dari sistem irigasi. Variabel independen yang digunakan dalam regresi adalah: prasarana fisik, produktivitas tanaman, sarana penunjang, organisasi personalia, dokumentasi kegiatan dan kondisi kelembagaan. Data didapat dengan mengumpulkan informasi primer dan sekunder dari pengelola jaringan irigasi dan perkumpulan petani pemakai air di lapangan. Hasil penelitian menunjukkan bahwa faktor-faktor yang berpengaruh signifikan dalam meningkatkan efektivitas pengelolaan sistem jaringan irigasi adalah prasarana fisik, organisasi personalia dan sarana penunjang. Ke depan, perlu dibuat sistem yang dapat mengukur perubahan kapasitas pengelolaan terkait dengan operasional dan pemeliharaan (OP) dari waktu ke waktu.

Quality is a factor that can increase product competitiveness. In improving products quality is necessary to control the quality (Quality Control) of the process activities. This study aims to determine the quality control of granulated sugar packaging process by using the seven tool method consisting of check sheet, histogram, Pareto chart, flow chart, scatter diagram, control chart and fishbone diagram. From 30 days observation concluded that the highest type of damage consist of metal detection by 145 bags and torn packaging by 50 bags. Then, from the results of control chart known that out of control process still happen. Both types of damage then revised, and process capability analysis is carried out, so can be concluded that the process is not capable in making the product according to specifications. Therefore, an analysis using fishbone diagram shows that the causes of damage are human, working methods, and machines. From these factors, Standard Operating Procedures can be done as well as designing new machines in packaging process

PG Kremboong merupakan salah satu perusahaan gula pasir di bawah naungan PTPN X yang sudah berusia 172 tahun. Kegiatan produksi yang berjalan secara terus-menerus dan umur pabrik yang sudah sangat tua menjadi salah satu faktor yang dapat menyebabkan penurunan efektivitas mesin produksi. Oleh karena itu, penelitian ini bertujuan untuk mengukur tingkat efektivitas mesin produksi gula terutama di stasiun gilingan PG Kremboong dan memberikan alternatif strategi perbaikan. Metode yang digunakan pada penelitian ini yaitu overall throughput effectiveness (OTE), fishbone diagram dan analytical hierarchy process (AHP). Metode OTE digunakan untuk mengukur tingkat efektivitas stasiun giling PG Kremboong dan six big losses pada setiap mesinnya. Fishbone diagram digunakan untuk mengetahui faktor yang berpengaruh. Metode AHP digunakan untuk menentukan tingkat prioritas perbandingan berpasangan antar faktor, subfaktor dan alternatif strategi perbaikan. Hasil penelitian menunjukkan nilai OTE sebesar 84,41% dengan bottleneck indicator sebesar 0.0256. Rata-rata kerugian yang terjadi pada setiap mesin yaitu breakdown losses 0,048%, setup and adjustment losses 0,159%, idle and minor stoppages 12,127%, dan reduced speed losses 11,749%. Faktor yang menjadi prioritas utama pada metode AHP yaitu material (0,543), method (0,275), machine (0,118) dan man (0,063). Prioritas alternatif strategi perbaikan untuk meningkatkan efektivitas stasiun giling di PG Kremboong terdiri atas perbaikan perencanaan produksi (0,606), peningkatan pengawasan dan pengecekan mesin (0,234), dan memberikan pelatihan perbaikan mesin (0,161). The fishbone diagram is a widely utilized patient safety tool that helps to facilitate root cause analysis discussions. This tool was expanded by the authors to reflect the contributions of both systems and individual cognitive errors to diagnostic errors. We describe how two medical centers have applied this modified fishbone diagram to approach diagnostic errors in a way that better meets the patient safety and educational needs of their respective institutions.

Machine failure on the M-145 bucket elevator contributed to downtime in January 2021, as much as 605 minutes or 6.75 hours, starting from January 17-20 2021 with a total of 17 production downtimes. The purpose of this study was to analyze the damage to the M145 elevator to determine the root cause of the damage and in this study used the fishbone diagram method. From the results of research conducted there was damage to the M145 elevator that the cause of the overflow was due to the peeling of the rubber coating on the top pulley of the upper motor pulley so that the belt became loose and caused friction on the conveyor belt, the tightness of the conveyor belt which was rarely checked, from the beginning of the construction of the bucket which caused the material to be indirect. lifted to the top but first stirred at the bottom so that it inhibits the speed of the bucket elevator. Sampel diambil dengan teknik <em>cluster random sampling</em> sebanyak 2 kelas, yaitu kelas XI IPA 2 menggunakan <em>PBL</em> dengan teknik <em>fishbone</em> <em>diagram</em> dan kelas XI IPA 3 menggunakan <em>PBL</em> dengan teknik <em>concept mapping</em>. Pengambilan data kreativitas dan gaya belajar dengan angket, hasil belajar kognitif dikumpulkan dengan menggunakan tes, hasil belajar afektif dikumpulkan dengan teknik observasi, dan hasil belajar psikomotorik dikumpulkan dengan teknik observasi dan tes. Uji normalitas dengan menggunakan uji <em>Shapiro Wilk. </em>Uji homogenitas menggunakan Uji <em>Levene Statistic.</em> Uji hipotesis menggunakan Anava tiga jalan dengan desain faktorial 2x2x2 mengunakan <em>Software </em><em>SPSS 1</em><em>8</em> <em>for</em> <em>Windows.</em> Kesimpulan penelitian ini sebagai berikut: 1) ada perbedaan hasil belajar antara siswa yang diberi pembelajaran Biologi menggunakan model <em>PBL</em> disertai teknik <em>fishbone</em> diagram dan <em>concept mapping</em> pada aspek kognitif dan psikomotorik, sedangkan untuk hasil belajar afektif tidak ada; 2) tidak ada perbedaan hasil belajar antara siswa yang memiliki kreativitas tinggi dan rendah pada aspek kognitif, afektif dan psikomotorik; 3) ada perbedaan hasil belajar kognitif antara siswa yang memiliki gaya belajar kinestetik dan visual, tidak ada perbedaan pada aspek afektif dan psikomotorik; 4) ada interaksi antara pembelajaran Biologi menggunakan model <em>PBL</em> disertai teknik <em>fishbone</em> diagram dan <em>concept mapping </em>dengan kreativitas terhadap hasil belajar psikomotorik, sedangkan untuk hasil belajar kognitif dan afektif tidak ada; 5) ada interaksi antara pembelajaran Biologi menggunakan model <em>PBL</em> disertai teknik <em>fishbone diagram</em> dan <em>concept mapping</em> dengan gaya belajar terhadap hasil belajar psikomotorik, sedangkan pada aspek kognitif dan afektif tidak ada; 6) tidak ada interaksi antara kreativitas dan gaya belajar terhadap hasil belajar kognitif, afektif, dan psikomotorik; 7) tidak ada interaksi antara model <em>PBL</em> disertai teknik <em>fishbone</em> <em>diagram</em> dan <em>concept mapping</em>, kreativitas dan gaya belajar siswa terhadap hasil belajar kognitif, afektif, dan psikomotorik.</p>

PurposeThis research proposes a text analytics–based framework that examines the utility of online customer reviews in evaluating automobile manufacturers and discovering their consumer-perceived weaknesses.Design/methodology/approachThe proposed framework integrates aspect-level sentiment analysis with the house of quality (HoQ), TOPSIS, Pareto chart and fishbone diagram. While sentiment analysis mines and quantifies review-embedded consumer opinions on various automobile attributes, the integrated HoQ-TOPSIS analyzes the quantified opinions and evaluates the manufacturers. The Pareto charts assist in discovering consumer-perceived weaknesses of the underperforming manufacturers. Finally, the fishbone diagram visually represents the results in the form with which the manufacturing community is acquainted.FindingsThe proposed framework is tested on a review data set collected from CarWale, a well-known car portal in India. Selecting five manufacturers from the mid-size car segment, the authors identified the worst-performing one and discovered its weak attributes.Practical implicationsThe proposed framework can help the manufacturers in evaluating competitor; identifying consumers' contemporary interests; discovering own and their competitors' weak attributes; assessing the suppliers and sending early warnings; detecting the hazardous defects. It can assist the component suppliers in devising process improvement strategies; improving their customer network; comparing them with competitors. Objective Matrix adalah suatu sistem pengukuran produktivitas parsial untuk memantau produktivitas di setiap bagian perusahaan sesuai dengan tingkat kepentingan elemen tersebut, yang kemudian dilakukan evaluasi menggunakan diagram fishbone. Hasil analisis data menunjukkan terjadi fluktuasi produktivitas di bagian pengecoran PT Apie Indo Karunia dengan nilai produktivitas tertinggi terjadi pada bulan Juli 2018 dan nilai produktivitas terendah terjadi pada bulan Juni 2018. Evaluasi dengan diagram fishbone menunjukkan penurunan produktivitas disebabkan oleh beberapa faktor yaitu tenaga kerja, metode, mesin dan lingkungan, dengan diperoleh 10 usulan perbaikan untuk meminimalisir terjadinya penurunan produktivitas.

PT. KPS is one steel company in Indonesia that produces steel products in the form of slab and plate that is marketed in the country up to foreign countries. Cold leveller machine is a machine that serves to flatten the surface of the plate. Problems occurred in the machinery that is often experienced a breakdown so that the plate with uneven surfaces more and more because of the time machine working on the wane. This research aims to quantify the value of the overall equipment effectiveness (OEE), calculating the factor of six big losses and figure out the root cause of the problem, the dominant factor of the six big losses with the fishbone diagram. The OEE value obtained on the engine cold leveller is 82%. The biggest factor that causes the low effectiveness of the machine that is the reduced speed losses amounting to 11.59% and equipment failure losses amounted to 6.04%. The proposed corrective actions is prioritizing the main TPM pillar 3 that is autonomous maintenance, quality maintenance, training and education.Keyword : fishbone diagram, overall equipment effectiveness (OEE), pareto, the six big losses, total productive maintenance (TPM)PT. Permasalahan yang terjadi pada mesin tersebut yaitu sering mengalami breakdown sehingga plate dengan permukaan yang tidak rata semakin banyak karena waktu bekerja mesin semakin berkurang. Penelitian ini bertujuan untuk menghitung nilai overall equipment effectiveness (OEE), menghitung nilai faktor six big losses dan mengetahui akar penyebab masalah yang dominan dari faktor six big losses dengan fishbone diagram. Nilai OEE yang didapat pada mesin cold leveller adalah sebesar 82%. Faktor terbesar yang menyebabkan rendahnya efektifitas mesin yaitu reduced speed losses sebesar 11,59% dan equipment failure losses sebesar 6,04%. This paper aims to present the application of a fishbone sequential diagram in air traffic management (ATM) incident investigations performing as a key connection between safety occurrence analysis methodology (SOAM) and accident/incident data reporting (ADREP) approaches. SOAM analysis is focused on organizational cause detection; nevertheless, this detection of individual causes from a complete incident scenario presents a complex analysis, and even more, the chronological relationship between causes, which is lacking in SOAM, should be tracked for post-investigation analysis. The conventional fishbone diagram is useful for failure cause classification; however, we consider that this technique can also show its potential to establish temporal dependencies between causes, which are categorized and registered with ADREP taxonomy for future database creation. A loss of separation incident that occurred in the Edmonton area (Canada) is used as a case study to illustrate this methodology as well as the whole analysis process.

Currently parking system that is running in Pt. Sanggraha Daksamitra still uses a conventional parking system. When the driver wants to do parking, driver first presses the analog button on the parking gate to get a parking ticket. Lack of maintenance on the parking gate causes vehicle jams in the parking entrance area. To minimize errors that occur, the fishbone diagram analysis method is used to analyze the problem. After the problem was found, a parking reservation was developed that used qr-code as a scanner identification in the parking system. The system produced in this study is expected to facilitate parking drivers who want to park their vehicles in the parking area of Pt. Sanggraha Daksamitra. The results of this system will be tested using blackbox testing.

Dalam proses produksi, perusahaan tidak jarang mengalami masalah kecacatan produk yang jumlahnya sangat besar. Kecacatan produk menyebabkan nilai jual dari perusahaan menurun dan terjadi keterlambatan pengiriman karena produk cacat memerlukan proses ulang untuk perbaikan. Penelitian ini dilakukan untuk mengetahui jenis kecacatan produk yang paling dominan menggunakan diagram pareto dan mengetahui apakah jenis kecacatan dominan tersebut terkendali atau tidak menggunakan peta kendali p software R sehingga dapat dianalisis penyebabnya menggunakan fishbone diagram dan diberikan usulan perbaikan untuk meminimalisir jumlah kecacatan tersebut. Metode yang digunakan yaitu pengendalian kualitas dengan Statistical Process Control (SPC). Berdasarkan hasil penelitian, jenis kecacatan produk kain oxford kontruksi TR30/TR16 124 44 62 bulan Juli 2021 yang paling dominan yaitu flek obat sebesar 1361 yard atau sebanyak 25%. Dari peta kendali atribut p, diketahui bahwa jenis kecacatan flek obat tidak terkendali. Setelah dianalisis akar masalah menggunakan fishbone diagram, diketahui faktor-faktor penyebab dari flek obat yaitu tenaga kerja, metode, lingkungan kerja, bahan baku (grey) dan mesin.

PT Amanah Insanillahia is a company involved in the bottled water industry. Based on the preliminary study, it shown the quality control only conducted to check the number of defective products produced without further evaluation and analysis. Therefore, there is a need to analyze the data of defective products produced and evaluate the results. The purpose of this study is to analyze the nonconformity product of the bottled water 600 ml brand PRIM-A. The p-chart is used in this research to analyze the number of defective products. Then, the fishbone diagram is used to analyze the causes of defective products. The results show that there is no data out of the control limits and mostly the data near in the central line of the p-chart. Using the fishbone diagram, it concluded that factors of man, machines, materials, methods, and environment are the causes of nonconformity products of the bottled water 600 ml brand PRIM-A. Risk identification and assessment can be analysed using many risk management tools. Fishbone diagram is one of these techniques which can be employed, for the identification of the causes behind the construction failure, which has become a phenomenon that often gets repeated in several projects. If these failures are not understood and handled scientifically, it may lead to disputes between the project parties. Additionally, the construction failure also leads to an increase in the project budget, which in turn causes a delay in the completion of the projects. Punching shear in reinforcement slab may be one of the reasons for construction failures. However, there are many doubts about other causes that lead to this failure as well as the role of these causes in the construction failure. Also, there are many causes linked to this failure of which some fall on the designer and the others fall on the contractor. Thus, this research aims to determine the causes of punching shear failure in the concrete slab and its role in the failure using a logic managerial analysis. For this purpose, the applicability of the Fishbone diagram has been extended, for the analysis of probability as well as the impact of the risk of punching shear, thus elucidating the risk score of each category without ignoring the global risk. In this direction, interviews and questionnaires are conducted with numerous experts specialize in both the design and execution field of construction projects for identifying the most important causes that lead to the occurrence of punching shear failure. Further, the Fishbone diagram for punching shear’s risk illuminated that impact of some of the primary and secondary causes such as planning, designing, and maintenance is more than the expectation. Therefore, the concentration in these areas should be carried out by taking into consideration the adapt risk response plan to prevent or mitigate these risks.

Permasalahan yang difokuskan diantaranya sistem ekonomi yang belum menunjang, kurangnya pengetahuan produk investasi di pasar modal syariah, persepsi masyarakat kurang memadai karena kurangnya literasi, trust issues terhadap produk syariah dan minimnya regulasi. Dari permasalahan tersebut dibuatkan rencana tindak kerja sebagai solusi yang akhirnya dijadikan strategi pengembangan pasar modal syariah di Indonesia. Diagram fishbone digunakan untuk mengetahui strategi dalam optimalisasi pengembangan pasar modal syariah yang berawal dari masalah perkembangan pasar modal syariah di Indonesia. The research method used is a literature study based on journals. Data collection through Focus Group Discussion (FGD). And then, the data that has been collected was analyzed using the analysis of Urgency, Seriousness, and Growth (USG). Based on the USG analysis, the results of the problems that occur that can hinder the development of the Islamic capital market in Indonesia are obtained. The problems that are focused on include an unsupported economic system, lack of knowledge of investment products in the Islamic capital market, inadequate public perception due to lack of literacy, trust issues on sharia products and lack of regulation. From these problems, a work action plan was made as a solution which eventually became a strategy for developing the Islamic capital market in Indonesia. The fishbone diagram is used to determine the strategy in optimizing the development of the Islamic capital market which started from the problem of the development of the Islamic capital market in Indonesia. The fishbone diagram shows the problems that become a reference in the strategy for developing the Islamic capital market in Indonesia.

Tepung mocaf adalah tepung yang terbuat dari bahan baku singkong yang dimodifikasi dengan proses fermentasi. Kualitas suatu produk sangat penting bagi perusahaan agar proses produksi berjalan dengan baik dan tidak memiliki hambatan. Tujuan dari penelitian ini adalah untuk mengevaluasi proses produksi dan pengemasan produk tepung mocaf di PT Rumah Mocaf Kabupaten Banjarnegara Jawa Tengah. Metode penelitian dilakukan dengan cara pengamatan langsung, wawancara dengan karyawan, pengambilan data proses produksi dan pengemasan produk, dan identifikasi cacat produk. Analisis jumlah dan cacat tertinggi menggunakan diagram pareto dan analisis penyebab cacat produk menggunakan diagram fishbone. Berdasarkan hasil analisis identifikasi cacat produk, dapat diketahui bahwa cacat tepung mocaf terdapat pada kesalahan proses pengeringan, pengupasan kulit singkong dan pencucian, fermentasi, penggilingan. Hasil analisis pareto menunjukkan bahwa jenis cacat yang menyebabkan kerusakan pada tepung mocaf adalah adanya bau asam dengan presentase 42% (cacat tertinggi), warna tepung tidak putih 25%, kadar air tinggi 21% dan bentuk tepung agak kasar 13%. Hasil analisis jenis cacat kemasan pada proses pengemasan tepung mocaf adalah kerusakan saat penyimpanan seperti terbentuk lubang pada kemasan, tanggal produksi atau tanggal kadaluwarsa yang tidak terlihat, kemasan tidak tertutup rapat dan kemasan rusak pada saat di-sealer. Faktor penyebab cacat tepung mocaf berdasarkan analisis diagram fishbone yaitu dari bahan baku, mesin, metode, pekerjaan dan lingkungan.

Abstract Information obtained from the daily inpatient census in the form of data that will be processed into information needed by the hospital, so that the daily inpatient census activities must be carried out correctly in accordance with procedures so that the data generated is accurate and can be used as a basis for decision making for management. This study aims to analyze the factors causing the inactivity of inpatient daily census activities correctly using a fishbone diagram. This research is a qualitative descriptive study conducted at Dr. Saiful Anwar Malang in February - March 2020. The subjects of this study were the daily census officer of the inpatient room, and the daily census officer of the medical record section. Data was taken by interview and observation methods. Data were analyzed using fishbone diagrams. The results of this study indicate that the factors that cause the inability of the inpatient daily census to be conducted correctly are Man (misperception of the inpatient census daily officer between the understanding day of care with the length of stay, educational background of the officer not from the medical record, and the officer has never attended training on the census), Money (there is no funding budget for training implementation, Materials (census information system on billing system is not implemented, census recapitulation is still manual using paper forms and Microsoft excel), Method (Daily SOP for inpatient census has not been revised), Machine (census application on billing system is not implemented) Fishbone diagrams where the in-patient census daily activities are carried out correctly are mostly caused by Man factors and Materials factors.

Kebocoran oli pada shockbreaker kendaraan roda dua bagian depan berdampak pada ketidak nyamanan dalam berkendara, terutama kondisi jalan yang berlubang atau jalan berbatu. Kehandalan shockbreaker yang menurun, akan berdampak juga pada kondisi mesin maupun komponen motor itu sendiri dan pengendara. Penelitian ini bertujuan untuk menentukan akar penyebab terhadap kebocoran oli pada shockbreaker dan metode perawatan yang harus dilakukan, supaya kejadian yang sama berkurang atau tidak terulang kembali. Dalam penelitian ini untuk mempermudah proses yang dilakukan menggunakan metode fishbone diagram. Adapun hasil penelitian kebocoran yang terjadi pada shockbreaker bagian depan disebabkan oleh kontaminasi debu, saat shockbreaker bekerja terdapat kontaminasi debu dan kontak dengan seal bagian atas, sehingga mengakibatkan gesekan dan merusak seal shockbreaker. Scratch pada poros disebabkan gesekan seal yang rusak, dan oli yang berada di tabung keluar dan menimbulkan kebocoran. Agar kejadian kebocoran tidak terulang kembali adalah mengganti secara berkala oli shock breaker dan seal, supaya keandalan komponen shock breaker meningkat, serta kenyamanan pengemudi terjaga
