Abstract:

The various treatment options for the aesthetic and functional rehabilitation of partially edentulous patients are conventional fixed partial dentures, implant supported fixed partial dentures and removable partial dentures. In some cases, removable partial dentures may be the only choice which is available other than implants and fixed partial denture. Removable cast partial dentures are used as definitive removable prostheses when indicated, but location of clasps is not aesthetically pleasant. So, when patient is concerned about aesthetics, flexible partial dentures which is aesthetically superior may be considered. But for the success of flexible removable partial denture, proper diagnosis, treatment planning, insertion, wearing and maintenance of this prosthesis is very important.

Abstract:

The aim of this clinical study was to analysis the mandibular movement in patients after insertion of fixed partial dentures using Cadiax Compact II and compares the Horizontal condylar angles and Bennette angles before and after insertion of posterior fixed partial denture.Material and methods: Mandibular movement of thirty two patients, sixteen males and sixteen female planed for posterior fixed partial dentures), had been recorded using Cadiax Compact II® for Horizontal Condylar Inclination and Bennette Angle. Three records had been obtained R1 before the abutments teeth preparations of the Fixed Partial Dentures, R2 after the cementation of the Fixed Partial Dentures and R3 after one month of the Fixed Partial Dentures cementation.Results: Statistical highly significant difference was found between R1&R2 for Bennette angles in both males and females, and significant difference in group R1&R3 for Horizontal condylar Inclination in females only. The other groups show no statistical significant difference.Conclusion: Within the limit of this clinical study, the insertion of fixed partial denture have no changing effect on the Horizontal Condylar inclination in males but in females have mild change. For Bennette angle there is highly significant change in both males and females immediately after the fixed partial denture cementation, but after one month this change disappeared.

<strong>Abstract</strong> The presence of a gap in the anterior tooth region, whether due to trauma or a congenitally missing tooth, can cause both psychological distress and functional challenges. Traditional treatment options, such as implants, removable partial dentures, and fixed partial dentures, are available, but some patients may be reluctant to undergo implant placement or use removable dentures. They may prefer a fixed partial denture (FPD), yet hesitate to have adjacent healthy teeth prepared, especially in anterior cases. In such situations, a porcelain-fused-to-metal fixed partial denture, like a Maryland Bridge, offers an ideal solution. This case report details the creation of a Maryland Bridge with specific modifications tailored to the patient's needs.

Background: The impact of edentulism on oral health status is an important public health issue. Conventional complete dentures, implant-supported removable dentures and implant-supported fixed dentures are evidence-based treatment IRPtions in the treatment of complete edentulism. Quality of life may be compromised due to various reasons such as functional problems, diseases, nutritional deficiency, physiological and psychosocial problems in individuals using prostheses.Objectives: Our aim in this study is to evaluate patient satisfaction of completely edentulous patients treated with conventional complete dentures, implant-supported removable dentures and implant-supported fixed dentures, oral health-related quality of life and to compare the effects on quality of life.Methods: In this study, total of 140 patients involved, 49 female and 91 males, aged between 40 and 90 who were completely edentulous and treated at least 6 months ago in the Prosthetic Dentistry Clinic.Patients were divided into 3 groups as conventional complete denture (CD), implant retained overdenture (IRP) and implant supported fixed prosthesis (ISFP). By Oral Health Impact Profile (OHIP-14), Oral Health-Related Quality of Life-United Kingdom (OHQoL-UK) questionnaires oral health-related quality of life was measured analyzed and obtained parameters were compared.Results: When all findings were evaluated, the oral health-related quality of life of the group treated with implant-supported fixed prosthesis were found to be higher than the conventional complete denture and implant-supported fixed prosthesis group. In addition, it was determined that implant-supported fixed and removable treatments had positive effects on patient satisfaction, quality of life compared to conventional complete denture treatment.Conclusions: Support from dental implants while making a prosthetic treatment plan in edentulous patients will contribute positively to the stability and retention of the prosthesis. This will increase the quality of life of the person by supporting patient satisfaction, adaptation to the prosthesis and nutrition

Abstract:

The aim of this systematic review was to evaluate the functionality of fixed and removable partial dentures as test interventions in relation to variations in the opposing dentition and their prosthetic restoration. The abstracts identified in the respective databases were screened independently by two investigators. RCTs and uncontrolled studies were considered, provided the patients were included consecutively and the confounding variables were adequately monitored. Seventeen papers were included. The study and publication quality was assessed using a “biometric quality” tool showing an overall poor quality. The reported outcomes, such as survival rates, were in each case obtained from a single study. Two possible trends could be deduced for the endpoint longevity: (a) the first trend in favor of removable partial dentures, compared to fixed partial dentures, with a fully edentulous opposing arch fitted with a removable prosthesis; (b) the second trend in favor of implant-supported partial dentures, compared to conventionally fixed partial dentures, with natural opposing dentition or with a removable partial denture in the opposing arch. No evidence could be generated as to whether, and if so how, variations in the opposing dentition have a bearing on the decision to fit a partially edentulous arch with a fixed or removable partial denture.

Abstract:

Abstract Background: The development of carious lesions is dependent on the presence of acid-producing microflora in the mouth. The activity of these plaque bacteria is dependent on sugar, which diffuses from the saliva following carbohydrate intake. The sugar serves as a substrate for fermentation, producing organic acids that demineralize tooth enamel. Elimination of the sugar from the mouth is accomplished by dilution with newly secreted saliva, a process generally referred to as sugar clearence. Objectives: Determine if fixed partial prosthodontic treatment affects the length of salivary sugar clearance. Materials and methods: Fifteen male subjects age between 21 and 32, and requiring 10 porcelain fixed partial dentures were included in this study. Before receiving their dentures, the subjects, rinsed with a 10% sucrose solution for 30 seconds then gave saliva samples. The prpcedure was repeated one month after the dentures were put in place. The length of sugar clearance was calculated. Results: Salivary sugar was cleared in 14.89±0.89 minutes before the denture application and in 18.86± 0.90 minutes after denture treatment. The initial sucrose concentration was 113.63±1.29 mmol/L before the dentures and 142.99±10.82 mmol/L after the dentures. Conclusion: The use of fixed partial dentures does not affect the time required to clear salivary sugar or the level of the initial sucrose concentration.

Although only one, tooth loss, can cause a wide range of problems. They are difficulty of chewing, shifting teeth, andaesthetics problems. There are two categories of dentures, namely removable and fixed dentures. Lately, the paradigmof tooth loss rehabilitation has changed. Removable denture has been used in a long period, but this alternativetreatment has several disadvantages. Fixed denture is preferred especially for a single tooth loss because of thecomfort and resemblance to natural teeth. Currently, implant supported fixed denture is an option in replacing thefixed denture. Its advantages are no teeth reduction needed and ultimate emerging profile. In these three cases,reported comfort of using dental implants, especially in the case of single tooth loss. At control stage, the function ofmastication and aesthetic factors was also uncomplaint.

Abstract:

Introduction. Stomatoprothetic dentures are one of the factors of oral health and life quality of people of all ages. The aim of the paper is to make a connection between the oral health quality and quality of life on one side and the type of denture on the other. Material and Methods. This research study was conducted on the sample of 360 patients singled out in three numerically identical groups as follows: group I - patients treated with fixed dentures, group II - patients treated with mobile dentures, and group III - patients treated with both fixed and mobile dentures. The oral health quality was observed via five parameters: anamnestic data, symptoms of ill-functioning of basic functions in stomatogenic system (chewing and speech), extra oral examination, intraoral examination, and dental abilities. For some oral health quality parameters, the index of quality was determined. Results. Analyses of our three target groups of patients indicated that the patients from group I (treated with fixed dentures) suffered the least negative effects in line with the observed parameters; they are followed by patients from group III (combined dentures) and patients from group II (mobile dentures), respectively. Conclusion. Our research study showed that some oral health parameters have different impacts on health and life quality in patients treated with different stomatoprosthetic dentures.

Abstract:

Abstract The aims of this study were to evaluate the treatment need of fixed bridges according to the distribution of pontics in dentition in different age groups, and to investigate the primary and late complications and survival of the conventional fixed metal ceramic prostheses, as well as patients' satisfaction with the prosthetic treatment. The whole material consisted of the patients treated with fixed metal ceramic prostheses by undergraduate students at the Institute of Dentistry during the years 1984 - 1996. There were altogether 772 patients, 460 women (60 %) and 312 men (40 %). Their mean age was 47 years (23 - 81 years). Altogether 944 single metal ceramic crowns and 543 fixed bridges (1374 abutments and 807 pontics) were prepared. It can be concluded that the fixed bridges are most often prepared to replace upper first premolars and lower first molars also in the future. The most usual primary complications related to fixed bridges occurred during preprosthetic endodontic treatment of abutment teeth and during the preparation of the root canals. Previous restoration of the prepared tooth does not have any marked effect on the prognosis of single crowns with dowels, although anatomically complicated upper lateral incisors and upper first premolars need special attention in the treatment planning. Patients were satisfied with aesthetics and function of the fixed metal ceramic prostheses. Late complications found in clinical examinations were few, and the survival rate for the fixed metal ceramic bridge prostheses was calculated to be 84 % after 10 years, long fixed bridges having a lower survival than the shorter ones. The treatment need for conventional fixed bridges seems to be highest among patients over 50 years of age in the future. Age does not influence the longevity of the fixed prostheses, but basic circumstances of the mouth, especially low secretion of saliva affected by diseases and/or medications and high scores of lactobacilli and streptococcus mutans of the saliva seem to decrease the survival.

Abstract:

Background Dental prostheses have seen a major change since the advent of dental implants in 1965. The osseointegration property of titanium fixtures enables dentists to place a dental prosthesis at almost everywhere intra-orally provided there is sufficient bone to support it. The novel treatment modality had attracted attention of clinicians who had made multiple innovations on their use. Meanwhile, there has also been a shift of focus in the medical field from disease-centered outcome to subjective patient-centered outcome in the measurement of health status. Quality of Life (QoL) measurements are one of the subjective measurements of health status. They provided a means for clinicians to compare different prostheses, including dental prostheses. Aim and Objectives This systematic review seeks to identify evidence that proves the superiority of the new treatment modality in improving QoL or Oral Health Related QoL and to briefly discuss their implications for public health policies in Hong Kong. Methods Researches which compare conventional removable dental prostheses and the fixed alternatives in terms of improving QoL were identified. The evidence was scrutinized, critically analyzed and a summary was made. Results Current evidence is only strong enough to support the superiority of the new implanted supported prostheses over the conventional type for patients without lower teeth. Discussion and Conclusions It is concluded in this systematic review that the new implant-fixed or supported dental prostheses are superior to conventional removable dental prostheses for patients without lower teeth. However, it is so far inconclusive as to whether dental implants have greater impact on QoL of partially dentate patients than conventional removable dental prostheses. Abstract:

Developing conservative FPD designs in term of the preservation of tooth structure is not just a matter of cutting from the crown to form abutment. These designs need to prove that they show benign behaviour under occlusal forces. The aim of this study was to develop new single layer non-metal 3-unit posterior FPD designs as an alternative to the traditional FPD design without compromising the structure integrity of the natural dentition. The group designs were traditional, two different partial coverage crowns and two different inlay retained FPD. The CEREC CAD/CAM system was employed using a modified designing method and two resin bonded non-metal materials were used for the fabrication of these bridges. A series of tests were conducted in order to identify I) the volume of tooth structure loss with different designs 2) the ability of the CAD/CAM system to produce full-thickness FPD restorations 3) the fracture resistance of these restoration and 4) 2D and 3D FEA models were later developed involving some highly innovative steps. Micro CT, MIMICS and FreeForm DeskTop were used to create the 3D model. For meshing these models, due to the complexity of the 3D model, a sectional meshing technique was used by mechanical APDL and Workbench Ansys Y 12. The FEA data were compared with the results from the practical experiments. XIX Abdulsalam AI-Zahawi Benign non-metal fixed partial denture The results of the in vitro tests revealed that the four conservative designs showed less tooth structure loss compared with the traditional design and the inlay design was less than the partial coverage crown retained FPD. The CEREC CAD/CAM system was able to produce the suggested FPD designs in a full-thickness as an alternative to producing framework FPD. The fracture resistance test revealed that there is no significant influence of designs on the fracture strength excluding design V. The results showed that no fractures of the bridges in design V were recorded during the test. The FPD fabricated from the low elastic modulus material PEEK showed a higher fracture strength than one with the high elastic modulus ceramic. Part of the 2D FEA results showed agreement with the practical test results especially that the site of high stress concentration and the fracture site were at the gingival side of the connector. Nevertheless the 2D FEA results showed that there were many differences with the 3D FEA results. These differences include the overestimation in the stress level in the 2D FEA compared with 3D FEA model and the high stress located at the inlay cavity wall, which was difficult to see in the 2D FEA. 3D FEA results were in agreement with fracture strength tests results and these include that there is no influence of the partial coverage crown retained FPDs on the stress compared with the traditional design, the fracture site and the location of the higher stress were unaffected by the designs. Although both inlay retained FPD designs show more conservative preparations than partial crown retainers, they were found to be more destructive to the abutment teeth structure under a concentrated load. The clinical significant of this research project is that it is possible to create more conservative FPD designs without compromising the structure integrity of the restored dentition. xx

Abstract:

Ceramics have been increasingly becoming the prevalent materials of choice for such major prosthetic restoration as crowns and bridges. Their performance has been under much scrutiny as the key factors involved are investigated and identified for ensuring clinical success. However, long-term function in the oral cavity (mastication), and such problems as manufacturing defects, bruxism, or an impact accident, can lead to porcelain fracture. This has become a major barrier limiting more extensive exploitation of ceramic materials to fully replace metals in major dental restorative procedures such as multi-unit fixed partial dentures (FPDs), where tensile stresses may be substantial. This thesis aims to apply the eXtended Finite Element Method (XFEM) to prediction of crack initiation and propagation in all-ceramic dental prostheses. XFEM analyses were conducted to not only help quantify stress concentration and distribution, but also inherently accommodate the failure criteria from a fracture mechanics perspective. The modelling validation was performed by comparing the XFEM simulation with the in-vitro experiments. Fractographic analysis has also been introduced to define the cause of failures, as well as to identify the role of residual stresses, and pre-existing flaws on the longevity of dental restorations. Through the eight research projects conducted in this thesis, including thermal induced fracture of core-veneered ceramic structures, fracture behaviours of inlay and onlay FPDs, XFEM based topology optimization of all-ceramic structures, and fracture simulations of caries restorations and crowns, XFEM has been demonstrated to be an effective tool for analysing the fracture behaviours and optimising ceramic prostheses for enhancing longevity of various restorations. The outcomes generated in this thesis provided dentists and dental technicians with fundamental understanding in the fracture mechanics for a range of all-ceramic dental prostheses, thereby obtaining clinical guide for enhancing the fracture strength of the structure.

Entretanto, poucos são aqueles que avaliam a mastigação de pacientes parcialmente edêntulos, comparando a função mastigatória após a reabilitação por meio de diferentes próteses parciais, o qual foi o objetivo deste ensaio clínico pareado. Para tanto, foram selecionados 12 voluntários (8 gênero feminino, idade média 62.6 ± 7.8 anos), apresentando edentulismo total superior e parcial inferior (classe I de Kennedy), os quais receberam, de forma sequencial, próteses parciais removíveis (PPR), PPR com encaixe implanto-retido (PPRI) e prótese parcial fixa sobre implantes (PPFI). Todos os tratamentos foram realizados na mandíbula e utilizados por 2 meses antes da avaliação mastigatória, enquanto a maxila recebeu uma nova prótese total que foi utilizada durante todo o estudo. A mastigação foi avaliada por meio da mensuração da performance mastigatória (PM), índice de trituração dos alimentos (ITA), força máxima de mordida (FMM), espessura dos músculos masseter e temporal, movimento mastigatório, limiar de deglutição (LD), habilidade mastigatória, estado nutricional, qualidade de vida (QV) e satisfação do paciente. Foi realizada a análise exploratória dos dados e aplicada análise de variância para medidas repetidas seguido de teste de Tukey-Kramer para as comparações múltiplas entre os tratamentos. Todas as análises foram realizadas utilizando SAS software (release 9.1, 2003; SAS Institute Inc., Cary, USA) (p ? 0.05). A PM melhorou consideravelmente após o uso de PPRI e PPFI com aumento de 85% e 87%, respectivamente. Resultados similares foram observados em relação ao ITA e à FMM com aumento (p ? 0.0001) de 91% e 62% no ITA de 79% e 62% na FMM após o uso de PPRI e PPFI, respectivamente. Independente do tipo de prótese, o uso de PPRI e PPFI aumentou a espessura do masseter durante a contração voluntária máxima (p ? 0.0001) e alterou o movimento mastigatório, reduzindo o tempo total do ciclo, bem como a duração das fases de abertura e fechamento (p ? 0.05). A habilidade mastigatória melhorou após o uso da PPRI e PPFI, independente do alimento avaliado. O LD foi alterado pelo tratamento reabilitador, com redução no número de ciclos e tamanho da partícula triturada, sendo os menores valores observados com o uso da PPFI. Houve aumento no consumo de fibras (p = 0.007), cálcio (p = 0.001) e ferro (p = 0.02) após o uso de PPFI, além da redução no consumo de alimentos com altos níveis de colesterol (p = 0.02). A satisfação aumentou (p ? 0.05) e o impacto da saúde oral na QV reduziu, tanto no score geral (p = 0.04) quanto no domínio de dor física (p = 0.02) após o uso da PPFI. A reabilitação de pacientes parcialmente edêntulos com PPRI e PPFI melhorou significativamente a função mastigatória e a magnitude do efeito relacionou-se ao tipo de prótese<br>Abstract: Studies revealed the masticatory improvement after the use of prosthesis over dental implants. However, few are those who evaluated the chewing of partially edentulous patients, comparing the masticatory function after treatment with different partial dentures, which was the aim of this paired clinical trial. Therefore, 12 volunteers (8 females, mean age 62.6 ± 7.8 years) presenting total edentulism in maxilla and partial edentulism in the mandible (Kennedy class I) were selected and received, in a sequential way, a conventional removable partial dentures (RPD), implant-retained partial dentures (IRPD) and implant fixed partial denture (IFPD). All treatment were assembled in the mandible and used for 2 months, while the edentulous maxilla received a new complete denture which was used throughout the study. Mastication was assessed by measuring masticatory performance (MP), food comminution index (FCI), maximum bite force (MBF), masseter and temporal muscle thickness, chewing movements, swallowing threshold (ST), masticatory ability, nutritional status, quality of life (QOL) and patient satisfaction. Data were analyzed and repeated measures analysis of variance was applied followed by Tukey-Kramer multiple for comparisons between treatments. All analyzes were performed using SAS software (release 9.1, 2003, SAS Institute Inc., Cary, USA) (p ? 0.05). MP greatly improved after IRPD and IFPD use with an increase of 85% and 87% respectively. Similar results were observed in respect to FCI and MBF with an increase (p ? 0.0001) of 91% and 62% in FCI and of 79% and 62% in MBF after the IRPD and IFPD use, respectively. Regardless the prosthesis type, the use of IRPD and IFPD increased the masseter thickness during maximum voluntary clenching (p ? 0.0001) and altered the chewing movements, reducing the total cycle time, as well as the duration of opening and closing phases (p ? 0.05). MA improved after IRPD and IFPD use, irrespectively of the food rated. ST was affected by prosthetic treatment, showing a reduction in the number of chewing cycles and in the size of the comminuted particle, with the lowest values observed after IFPD use. There was a raise in fiber (p = 0.007), calcium (p = 0.001) and iron (p = 0.02) intake after the IFPD use and a reduction in the intake of food with high cholesterol levels (p = 0.02). Patients satisfaction also increased (p ? 0.05) and the impact of oral health on QOL decreased in overall score (p = 0.04) and in the physical pain domain (p = 0.02) after the IFPD use. Além disto, julgar a qualidade da adaptação das peças depende da confiabilidade dos métodos existentes para determinação da passividade. O objetivo deste estudo foi determinar o desajuste marginal de infra-estruturas metálicas implanto-retidas tipo monobloco antes e após o fresamento corretivo destas peças empregando retificador manual. Avaliou-se ainda a hipótese de que os procedimentos de fresamento corretivo podem ter influência sobre a confiabilidade do teste do parafuso único utilizado para averiguação de adaptação marginal de peças múltiplas sobre implantes. Foram confeccionadas 40 infra-estruturas metálicas pela técnica do monobloco, sendo 20 fundidas em titânio comercialmente puro (Ti c.p.) e outras 20 fundidas em liga de cobalto-cromo (Cr-Cr). Para tal, utilizou-se matriz metálica simulando a disposição clínica de implantes osseointegrados na região interforames mentonianos contendo 5 análogos de pilares cônicos para peças múltiplas padrão Branemark (Micro-Unit - Conexão) e cilindros protéticos calcináveis compatíveis. As peças confeccionadas em Ti c.p. e em Co-Cr, foram subdivididas em 2 subgrupos (n=10) de acordo com a seqüência de retificação a que foram impostas: Seqüência A ¿ fresamento corretivo das bordas de adaptação dos cilindros protéticos seguido do fresamento corretivo da base de assentamento da cabeça do parafuso protético ao pilar; e, Seqüência B ¿ fresamento corretivo da base de assentamento da cabeça do parafuso protético ao pilar seguido do fresamento corretivo das bordas de adaptação dos cilindros protéticos. A conduta forneceu um total de 4 grupos experimentais (Titânio A e B; Co-Cr A e B). As leituras de desajuste marginal, realizadas sobre a matriz metálica e com auxílio de um microscópio mensurador, foram baseadas na técnica do parafuso único e realizadas em cada estrutura antes e após cada etapa de retificação corretiva. As médias de desajuste marginal de cada estrutura foram calculadas e consideradas como uma unidade para a análise estatística. O teste de Friedman foi aplicado com nível de significância de 5% para avaliar o efeito dos diferentes passos de retificação sobre o desajuste marginal. Nos Grupos Ti A e B, assim como no Grupo Co-Cr A, não foi observada diferença significativa entre as condições experimentais (Grupo Ti A, p=0,9048; Grupo Ti B, p=0,273 e Grupo Co-Cr A, p=0,4966). Porém, diferenças estatisticamente significativas foram observadas no Grupo Co-Cr B, após a retificação corretiva das bordas dos cilindros fundidos (p=0,0017). Foi concluído que os procedimentos de fresamento corretivo não mostraram efeito significativo sobre a adaptação marginal das pecas de titânio. Contudo, em um grupo de peças fundidas em Co-Cr, a retificação das bordas dos cilindros mostrou efeito adverso sobre a passividade. Os procedimentos de fresamento corretivo parecem não apresentar influência significativa sobre a confiabilidade do teste do parafuso único, porém, devem ser realizados cuidadosamente, necessitando de aperfeiçoamentos para uma utilização segura<br>Abstract: The fabrication of a passively fitting framework over abutments and fixtures, maintaining its structural integrity and with a low costs, is the ultimate goal of the construction process of a multi-unit implant-support prosthesis. Furthermore, the accuracy and reliability of the passive fit determination methods have a crucial hole on framework misfit analysis. The objective was to evaluate the passive fit by the single-screw test method of cast commercially pure titanium (CP Ti) implantsupported frameworks after the corrective finishing of cast cylinders fitting borders and screw seats. The hypothesis tested was that finishing procedures could affect the single screw test measurements reliability. Were fabricated 40 frameworks by onepiece casting method, being 20 CP Ti frameworks and others 20 Co-Cr frameworks. A metallic index containing 05 Branemark-type multi-unit abutment analogs and respective calcinable cylinders were used to frameworks fabrication. The CP Ti and Co-Cr frames were assembled in 2 sub-groups (n=10) accordingly with the finishing sequence proposed: Group A - corrective finishing of cast cylinder borders followed by corrective finishing of cast cylinder screw seats - and Group B - corrective finishing of cast cylinder screw seats followed by corrective finishing of cast cylinder borders-. This conduct produce 4 experimental groups (CPTi A and B; Co-Cr A and B). The marginal gaps, assessed over the metallic matrix using a traveling microscope, were carried out before and after each corrective finishing step accordingly with one screw test protocol. The marginal misfit means of each framework was considered as a unit to statistical analyses. The Friedman¿s test was used with significance level set as 5% to analyze the effect of the different corrective finishing steps over frameworks passive fit. In the CPTi A and B Groups, and in the Co-Cr A Group, there was no significant differences between experimental conditions (Group CPTi A, p=0.9048; Group CPTi B, p=0.273 and Grupo Co-Cr A, p=0.4966). However, in the CPTi B Group, a significant difference was found after cast cylinder fitting borders corrective finishing (p=0.0015). It was concluded that the finishing process does not show any effect over Ti framewoks misfit values. However, to a group of Co-Cr frames, the fitting borders corrective finishing has a deleterious effect over passivity. Abstract The replacement of missing teeth with an implant-supported prostheses eliminates the necessity of preparing the adjacent teeth to support and retain a conventional fixed partial denture, and may be similar in expense to the patient in many situations. Unfortunately, the implant-supported denture presents a biomechanical design problem. Screw loosening is perhaps the most common cause of structural failure. It occurs under repeated loading at stress levels below the ultimate strength of the material. With an increased understanding of the mechanical principles related to screw tightening, various mechanical explanations and assumptions have been made (Fig. 1). However, little information has been published with regard to a general model that can be used to predict the screw joint tightening and force transformation.
