Full textAim: The aim of this study was to compare fracture load and fracture mode of three-unit fixed partial dentures, made in lithium disilicate when the FDPs are made with pressing technique versus milled with a CAD/CAM system.Hypothesis: Pressing technique is better suited with regards to fracture load since milling of lithium disilicate blocks can result in defects and tension build up in the material, leaving the material more prone to chipping/micro-abrasions on the surface that could propagate and thus affecting the fracture load. P-value set to p ≤0.05.Material and methods: 40 FDPs where manufactured with pressing technique versus milled with a CAD/CAM system in each group. Furthermore, each group where subdivided depending on the anatomy. The FDPs thereafter underwent an artificial ageing process consisting of thermocycling and preloading. After the artificial aging process the FDPs were subjected to load to fracture, where the statistical data was collected and analyzed. Also, the fracture mode was observed and analyzed.Results: There was no significant difference regarding the fracture strength between the full anatomy groups. There was also no significant difference regarding the fracture strength between the core-design groups. The hypothesis was therefore rejected.Conclusion: There is no significant difference with regards to how much load is required to fracture FDPs made through pressing technique compared to FDPs made through the use of CAD/CAM technique. There is a clear pattern regarding the fracture patterns, however a conclusion can’t be drawn considering the sample size.

Resumo: O objetivo deste estudo foi avaliar a influência da altura do remanescente coronário e de diferentes tipos de retenções intra-radiculares na resistência à fratura de raízes bovinas restauradas com coroas totais confeccionadas em resina composta laboratorial nas seguintes condições: ausência de remanescente coronário ou presença de um ou dois milímetros e cinco métodos de retenção intra-radicular. Cento e cinqüenta dentes bovinos foram selecionados padronizando-se as dimensões das raízes. As coroas foram seccionadas e as raízes remanescentes receberam tratamento endodôntico e foram divididas, aleatoriamente, em quinze experimentais (n=10): G1 - pino de fibra de vidro (FV) e 0 mm; G2 - FV e 1 mm; G3 - FV e 2 mm; G4 - pino pré-fabricado metálico (PM) e 0 mm; G5 - PM e 1 mm; G6 - PM e 2 mm; G7 - pino de fibra de carbono (FC) e 0 mm; G8 - FC e 1 mm; G9 - FC e 2 mm; G10 - núcleo metálico fundido (NMF) e 0 mm; G11 - NMF e 1 mm; G12 - NMF e 2 mm; G13 ¿ preenchimento do canal radicular com resina composta (RC) e 0 mm; G14 - RC e 1 mm; G15 - RC e 2 mm. As coroas totais foram cimentadas com cimento resinoso e as raízes foram fixadas em resina poliestirênica, simulando-se o ligamento periodontal com material de moldagem à base de poliéter. As amostras foram submetidas ao carregamento tangencial de compressão, sob um ângulo de 135o a 0,5 mm/min até ocorrer a fratura. Os resultados (ANOVA / Tukey a=0,05) demonstraram que na ausência de remanescente, o grupo RC apresentou os maiores valores de resistência à fratura, não diferindo estatisticamente dos grupos restaurados com FC, FV e PM; o grupo restaurado com NMF apresentou os menores valores médios de resistência à fratura. Na presença de 1 mm de remanescente o grupo PM apresentou os maiores valores de resistência à fratura, não diferindo estatisticamente dos grupos restaurados com FV, NMF e RC e o grupo FC apresentou os menores valores médios. Na presença de 2 mm de remanescente, os diferentes tipos de retenções não promoveram diferenças entre os valores médios de resistência à fratura. Para os grupos restaurados com FV, FC e NMF, a altura do remanescente não influenciou nos resultados. Para os grupos PM, a altura de 1 mm proporcionou os maiores valores de resistência à fratura e o grupo com 2 mm de remanescente apresentou os menores valores. No grupo RC, a ausência de remanescente dental coronário promoveu os melhores resultados e o grupo com 1 mm de remanescente apresentou os piores resultados. A análise geral dos padrões de fratura demonstrou que 68% das fraturas atingiram os terços apical ou médio. Os grupos restaurados com pinos de fibra de vidro apresentaram os melhores padrões de fratura, enquanto os restaurados apenas com resina composta apresentaram os piores padrões de fratura
Abstract: The purpose of this study was to evaluate the influence of dental coronal structure height and different types of intraradicular retention on the fracture strength of bovine roots restored with total crowns made of laboratory composite resin under the following conditions: absence of dental coronal structure or presence of one or two millimeters, and five methods of intraradicular retention. One hundred fifty bovine teeth were chosen and the canal size was standardized. The crowns were sectioned and the canals were treated endodontically and the specimens distributed randomly into 15 experimental groups: G1 ¿ fiberglass post (FV) and 0 mm; G2 ¿ FV and 1 mm; G3 ¿ FV and 2 mm; G4 ¿ prefabricated metal post (PM) and 0 mm; G5 ¿ (PM) and 1 mm; G6 ¿ (PM) and 2 mm; G7 ¿ carbon fiber post (FC) and 0 mm; G8 ¿ (FC) and 1 mm; G9 ¿ (FC) 1 mm; G15 ¿ RC and 2 mm. Total crowns were luted with resin cement and the roots were fixed in resin blocks and artificial periodontal ligament was simulated with casting material made of polyether. The specimens were submitted to compressive strength test at 135o and 0.5 mm/min until fracture occurred. The results (ANOVA / Tukey a=0.05), showed that in absence of dental coronal structure, the group RC presented the most fracture strength values, not differing statistically from groups restored by FC, FV and PM, and the group restored by NMF presented the lowest mean values. In the presence of 1 mm, Group PM presented the highest fracture strength values, not differing statistically from groups restored by FV, NMF and RC, and the group restored by FC presented the lowest mean values. In the presence of 2 mm, the different types of intraradicular retentions did not cause differences among mean fracture strength values. In the groups restored by FV, FC and NMF, the dental coronal structure height had no influence on the results. In the PM group, the height of 1 mm, showed the highest and the group with 2 mm presented the lowest mean values. In the group RC the absence of dental coronal structure showed the best results and the group with 1 mm presented the worst results. The general fracture pattern analysis showed that 68% of fractures involved the gingival and apical thirds. Resumo: O objetivo do estudo foi avaliar o comportamento mecânico de uma cerâmica, à base de dissilicato de lítio, em ensaio de flexão com as extremidades fixas. Este estudo particular tem similaridade com caso real de aplicação de prótese em pacientes, o que melhora a avaliação do material quando comparado a ensaios convecionais como o ensaio de flexão em três pontos, com viga simplesmente apoiada. Para isso os corpos de prova foram submetidos à três tipos de envelhecimento: ciclagem mecânica, ciclagem termomecânica, ambas de acordo com os protocolos utilizados na literatura, e ciclagem intrabucal. A ciclagem intrabucal foi realizada por meio da utilização de um dispositivo intrabucal semelhante a uma placa oclusal, na qual foram instalados os corpos de prova, por meio de um dispositivo metálico próprio. Estas placas foram utilizadas em dez voluntários por oito horas, durante o período noturno. As amostras foram posteriormente submetidas ao ensaio mecânico de flexão de três pontos com as extremidades engastadas, para avaliação da resistência residual. Os resultados de resistência residual foram submetidos à análise estatística descritiva (média e desvio padrão) e inferencial, mediante o teste paramétrico de análise de variância (ANOVA) um fator e o teste de Tukey ( = 0,05). Abstract: This study aimed to evaluate the behavior of lithium disilicate ceramic in similar condition to a partial fixed prosthesis of three elements by subjecting to three types of aging (n=10): mechanical cycling, thermomechanical cycling, both according to the protocols used in the literature, and intraoral cycling. The intraoral cycling was accomplished by using an intraoral device, which is similar to an occlusal splint. They were installed with the samples (CP) by means of a metallic device that was fixed in balanced contact condition with the remainder of the occlusal surface of the plate. These plates were used in ten volunteers for eight hours during the evening. After fatigue, the samples were subjected to three-point bending tests with clamped ends to evaluate the residual strength. The residual strength results were submitted to descriptive statistics (mean and standard deviation) and inferential by the parametric analysis of variance (ANOVA), with one factor and Tukey test ( = 0.05). In addition, representative samples of each group were observed in a scanning electron microscope (SEM) for fractographic analysis. The cycling condition influenced the mechanical behavior of ceramic with statistical significance of (p = 0,000), and the intraoral cycling group showed the lowest residual flexural strength. In fractographic analysis, they showed that the origin of fracture was in the tensile zone, but there was a secondary fracture origin represented by a twisted zone. Resumo: O objetivo deste estudo foi avaliar as tensões e deformações in vitro, por meio da fotoelasticidade e da extensometria, em próteses parciais fixas implantossuportadas de três elementos variando o número de pilares: dois e três. O primeiro bloco de poliuretano (modelo mestre 1) recebeu três implantes e o segundo (modelo mestre 2) recebeu dois implantes, todos de hexágono externo. Em seguida, os modelos mestre foram duplicados em resina fotoelástica. Pilares protéticos microunit foram conectados aos implantes com torque de 20 Ncm. Sobre os pilares protéticos dos modelos mestre foram parafusados cilindros plásticos, que receberam estruturas em padrões de resinas, os quais foram fundidos em liga de Ni-Cr (n=10). Quatro extensômetros foram colados na superfície de cada bloco de poliuretano, tangenciando os implantes. Nos modelos mestre e nos modelos fotoelásticos, cada estrutura metálica foi parafusada sobre os pilares protéticos com torque de 10 Ncm. Os dados obtidos na extensometria foram submetidos aos testes estatísticos RM ANOVA e Tukey (5%). As franjas isocromáticas obtidas nos modelos fotoelásticos após cada condição de carga foram registradas com uma câmera digital, visualizadas em um programa de computador e comparadas qualitativamente. Resultados: Houve uma diferença estatística significante entre a quantidade de suporte de uma prótese fixa (p=0,001). Abstract: The present study used photoelastic and strain gauge analysis to compare stress and microstrain to perform an in vitro evaluation of the effect of the numbers of support in a implant-supported fixed partial prostheses with tree elements. The first polyurethane block (master model 1) will contain three implants and the second block (model 2) two of them, all implants have a external hexagon. Then, the master models were duplicated in photoelastic resin. Microunit abutments were connected to the implants and received a 20 Ncm torque. Over the master model plastic prosthetic cylinders were screwed the abutments that received standard patterns cast in Ni-Cr alloy (n=10). Four strain gauges (SG) were bonded in the surface of the block, tangentially to the lateral implants. On the master model and photoelastic model, each metallic structure were screwed onto the abutments with a 10 Ncm torque. The data obtained from the strain gauge were analyzed statistically by RM ANOVA and Tukey's test, with a 5% conventional level of significance. The isochromatic fringes obtained in the photoelastic models after each load condition were recorded with a digital camera, visualized with software to facilitate qualitative analysis. Results: There was statistically significant differences in support of FPD( p=0,001).The results demonstrated that 3 implants has higher stress distribution and in the pontic of FPD we found higher stress (2326,95µε in 2 implants and 1006,57µε in 3 implants).In photoelastic images, 3 implants presented better stress distribution than 2 implants. Resumo: O objetivo desse estudo foi avaliar a adaptação marginal de copings cerâmicos obtidos pelos métodos de moldagem convencional e escaneamento. Os modelos foram separados em 3 grupos para avaliação e todas as coroas foram confeccionadas sobre um modelo mestre previamente fabricado em CoCr. Grupo 1: Com o auxílio de uma moldeira individual cilíndrica de acrílico foram reproduzidas 10 moldagens com silicona de adição (Panasil) no modelo mestre pela técnica de moldagem dupla e replicados dez modelos físicos em gesso pedra Fuji Rock.Sobre esses modelos foi realizado o enceramento dos copings, confecção dos sprues de cera e sobre eles foi realizada a injeção da cerâmica Grupo 2: foram realizadas 10 moldagens diretas com scanner extraoral (Swing Dental Scanner) no modelo mestre. As imagens foram obtidas através do software Exocad e copings em IPS-Emax CAD foram confeccionados em fresadora (Ceramill Motion 2) para posterior sinterização da cerâmica. Grupo 3: foram realizadas 10 moldagens diretas com scanner intraoral (CEREC Bluecam) no modelo mestre. As imagens foram obtidas através do software Exocad. Copings em IPS-Emax CAD foram confeccionados em fresadora (Ceramill Motion 2) para posterior sinterização da cerâmica. The objective of the study was to evaluate the marginal adaptation of ceramic crowns obtained by conventional molding and scanning methods. The models were separated into 3 groups for evaluation and all the crowns were made on a master model previously manufactured in CoCr. For group 1: With the aid of a single cylindrical acrylic tray the master model was molded 10 times with Panasil in the master model by the technique of double molding and replicated ten physical models in stone gypsum Fuji Rock. On these models was done the waxing of the copings, making the sprues of wax and on them was carried out the injection of the ceramics In group 2, 10 direct impressions were performed with the Swing Dental Scanner (Korea) extra oral scanner in the master model. The images were obtained through the software Exocad (GmbH) and copings in IPS-Emax CAD were made in the Ceramill Motion 2 (Amanngirrbach) mill for subsequent sintering of the ceramics. In group 3, 10 direct impressions were performed with the CEREC Bluecam intraoral scanner (Sirona Dental Systems) in the master model. The images were obtained through the Exocad (GmbH) software. IPS-Emax CAD copings were made in the Ceramill Motion 2 (Amanngirrbach) mill for subsequent sintering of the ceramic. Ceramic copings were not cemented in the master model, only positioned and maintained with a constant pressure of 1.4 to 1.5 kg during the measurement, with the aid of an eyeliner adapted for this work, where eight elective points were used as reference to measure the vertical distance between the end line of the master model and the end line of the copings. The margin at each point was measured 3 times with stereo microscope Discovery V20 (ZEISS). Resumo: No processo de fundição pela cera perdida, os materiais envolvidos podem apresentar interações entre si e o de tempo despendido para se obter as estruturas metálicas é oneroso. Assim, o objetivo deste trabalho foi avaliar a influência da propriedade física de resistência à compressão de revestimentos fosfatados indicados para altas temperaturas de fundição, utilizando os métodos de aquecimento convencional (MAC) e rápido (MAR), e analisar a rugosidade de superfície e a alteração dimensional do produto final da fundição (a peça metálica) obtido com as ligas metálicas Ni-Cr (Verabond II) e Ni-Cr-Ti (Fit Cast Titanium). Os revestimentos estudados Castorit Super C, Castorit All Speed, Heat Shock e Micro Fine 1700 foram vazados em uma matriz de silicone que permitiu obter amostras com formato cilíndrico (n=200), medindo 10mm de diâmetro e 20mm de comprimento. A resistência (MPa) deles foi determinada à temperatura ambiente (resistência fria) após 15, 30 e 60 minutos de manipulação, e também imediatamente após serem submetidos aos métodos de aquecimento MAC e MAR (resistência de queima), por meio de uma máquina de ensaio universal, em velocidade de 0,05mm/min e carga de 150kgf. Após inclusão, aplicação dos métodos de aquecimento e fundição às amostras (n=120) para rugosidade de superfície (Ra-μm), foram avaliadas por um rugosímetro Mitutoyo Surftest SJ-401 com cut-off de 0,8mm e três repetições em cada amostra. A alteração dimensional foi determinada pela diferença entre os valores dimensionais da cera e da peça fundida (n=60) utilizando-se o software AutoCad2008. Abstract: In the lost wax casting procedure, the involved materials can present interactions among them and the time used to obtain the metallic structures is onerous. Thus, the purpose of this study was to evaluate the influence of the compressive strength physical property of phosphate-bonded investment materials indicated for high casting temperatures, utilizing conventional heating (CHM) and quick heating methods (QHM), and analyze the surface roughness and the dimensional accuracy of the final casting product (the metallic piece) obtained from Ni-Cr (Verabond II) and Ni-Cr-Ti (Fit Cast Titanium) metallic alloys. Castorit Super C, Castorit All Speed, Heat Shock and Micro Fine 1700 investment materials were poured into a silicone matrix in order to obtain cylindrical samples (n=200), measuring 10mm in diameter and 20mm in length. Their strength (MPa) was determined at room temperature (green strength) after 15, 30 and 60 minutes of manipulation, and also immediately after being submitted to the CHM and QHM heating methods (fired strength), by means of an universal testing machine at 0,05mm/min speed and 150kgf load. After investing, applying the heating methods and casting the samples (n=120) for surface roughness (Ra), the samples were evaluated with a Mitutoyo Surftest SJ-400 profilometer with 0,8mm cut-off, three times in each sample. The dimensional accuracy was determined by the difference among the dimensional values of the wax and of the metallic piece (n=60) through the software AutoCad2008. The mean values obtained were submitted to ANOVA and Tukey tests (p <0.05), and the results showed that the values of compressive strength were different among the investment materials (Heat Shock presenting the highest values) and among the analysis times with a direct relation of time and strength increase, but there was no difference between CHM and QHM. Resumo: A redução de desajustes marginais em estruturas suportadas por implantes osseointegrados é requerida para maior longevidade das fixações. Este trabalho teve o propósito de avaliar o desajuste marginal de infra-estruturas metálicas implanto-retidas fundidas em titânio comercialmente puro (Ti c.p.) pela técnica de fundição-sobre-análogos, comparando os resultados com estruturas obtidas pela incorporação de cilindros cimentados e pela técnica convencional de fundição tipo monobloco. A partir de uma matriz metálica mandibular contendo 05 análogos de abutments tipo Micro Unit, 10 estruturas foram obtidas de modelos elaborados em revestimento pela fundição-sobre-análogos. Estruturas com cilindros incorporados mediante cimentação por agente resinoso e pela técnica convencional em monobloco, foram manufaturadas de outros 20 modelos elaborados em gesso. As leituras de desajuste foram conduzidas num microscópio mensurador com aumento de 120X, tendo sido baseadas no protocolo do aperto de 10 Ncm em um único parafuso distalmente posicionado e avaliação de desajuste no segmento em alça. Pelo Análise de Variância e teste de Tukey foram constatadas diferenças estatísticas significativas (P=0,01) entre os valores médios do grupo cilindro cimentado (27,90±6,61µm), monobloco (332,87±63,91µm) e fundição-sobre-análogos (97,70±40,53µm). As estruturas metálicas implanto-retidas obtidas pela técnica de fundição-sobre-análogos apresentaram melhor adaptação marginal que as fundidas em monobloco, mas com maior desajuste que as estruturas com cilindros cimentados
Abstract: The reduction of marginal gap in osseointegrated implant-supported frameworks is required for longevity of the settings. The purpose of this study was to evaluate the marginal misfit of cast implant-retained frameworks made from commercially pure titanium (CP Ti) comparing: the over-analog casting technique, cemented cylinders technique, and the conventional one-piece casting technique. From a mandibular master cast containing 05 Micro Unit abutment analogs, 10 frameworks were made from investment material for over-analog casting technique. Twenty dental stone casts were used for both cemented cylinder and one-piece casting techniques. A resinous agent was used for the cemented cylinders technique. The marginal gaps were measured using a traveling microscope (×120) by means of single-screw test protocol: tightness of 10 Ncm of distal screw and evaluation of marginal gap in the opposed extension. Mean marginal misfit data were calculated for each framework. Analysis of Variance and Tukey test exhibited statistical significant differences (P=0.01) among the misfit mean values of the cemented cylinders group (27.90±6.61µm), one-piece casting group (332.87±63.91µm) and over-analog casting group (97.70±40.53µm). The over-analog casting technique provided a smaller marginal misfit than one-piece casting technique, however the over-analog casting technique resulted in a higher marginal gap than cemented cylinders technique

Resumo: A longevidade das reabilitações implanto-retidas é comprometida quando tensões provenientes do desajuste marginal e conduzidas às fixações causam dano à junção osso-implante. Este trabalho teve o propósito de avaliar os desajustes marginais e as tensões aferidas nos abutments de infra-estruturas implanto-retidas fundidas em titânio comercialmente puro (Ti c.p.) pelas técnicas monobloco, soldagem laser e fundição-sobre-análogos e também avaliou a influência de processos de eletroerosão nos valores de desajuste e de tensão. A partir de uma matriz metálica mandibular, contendo 05 análogos de abutments cônicos, foram obtidas 10 estruturas para a técnica de fundição-sobre-análogos a partir de modelos fabricados em revestimento. Modelos fabricados em gesso originaram outras 10 estruturas em monobloco e 10 soldadas a laser. As leituras de desajustes foram conduzidas num microscópio mensurador (120X), tendo sido baseadas no protocolo do teste do parafuso único. As tensões (em gf) foram captadas mediante o uso de strain gauges colados nos abutments em formação de ½ ponte de Wheatstone e após o apertamento de 10 Ncm de todos os parafusos protéticos. As medidas de desajuste marginal e tensão foram repetidas após as etapas de eletroerosão da base de assentamento de parafusos e das bordas dos cilindros fundidos. Os testes de Análise de Variância e de Tukey foram utilizados na análise estatística (P<0,05). Após a finalização das etapas de eletroerosão, os valores de desajuste marginal foram significativamente reduzidos no grupo monobloco e os valores de tensão tiveram uma redução significativa nos grupos monobloco e fundição-sobre-análogos. Concluiu-se que as estruturas pela técnica de fundição-sobre-análogos apresentaram menor desajuste marginal e tensão nos abutments que as obtidas em monobloco e soldadas a laser; os processos de eletroerosão foram eficazes na redução do desajuste e da tensão nos abutments de infra-estruturas em monobloco. From a mandibular master cast, containing 05 conical abutment analogs, 10 frameworks were made from investment casts for over-analog casting technique. Plaster casts were used to originate 10 others frameworks gotten by one-piece casting technique and 10 frameworks by the laser welding technique. The marginal gaps were measured using a traveling microscope ('120) by means of one-screw test protocol. The strains (in gf) were identified by means of strain gauges arrangement in ½ bridge de Wheatstone attached to abutments and after the tightening of prosthetic screws at 10 Ncm. The marginal misfit and strain measurement were reproduced after spark erosion steps of screws seat base and cast cylinders fitting borders. Analysis of Variance and Tukey tests were used in the statistical analysis of the results (P<0.05). Following spark erosion steps, marginal misfit values were significantly reduced on the one-piece casting group and the strain values were significantly reduced on the one-piece casting group and over-analog casting group. It was concluded that the frameworks made from over-analog casting technique showed lesser marginal misfit and abutments strain than frameworks gotten by one-piece casting technique and laser welding technique; the spark erosion procedures were efficient in the decrease of marginal misfit and abutments strain of frameworks made from one-piece casting technique. Resumo: O objetivo desse trabalho foi avaliar por meio da técnica extensometria linear elétrica as deformações ocorridas nas réplicas de implantes e nas infraestruturas metálicas de próteses parciais fixas envolvendo o espaço protético dos quatro incisivos superiores com variação na disposição dos análogos e na configuração protética. Foram confeccionadas duas infraestruturas: corpo de prova I, com análogos instalados na posição correspondente aos incisivos laterais e infraestrutura com pôntico na região dos incisivos centrais e, corpo de prova II com análogos posicionados nos incisivos centrais e infraestrutura com cantilever na região dos incisivos laterais. Os extensômetros foram posicionados nas regiões cérvico-vestibular e cérvico-palatina dos análogos e cervical das infraestruturas. Os corpos de prova foram submetidos a carga progressiva de 150N a uma angulação de 45° na superfície palatina do incisivo lateral esquerdo superior, simulando uma oclusão funcional. Como resultados, foi observado que todos os extensômetros registraram deformação com característica de compressão ou tração. As deformações nos análogos foram significativamente diferentes das infraestruturas (p<0,05 ou p<0,01). Em ambos corpos de prova, maiores valores de deformação foram exibidos na região vestibular dos análogos. Não houve diferença significativa entre os extensômetros fixos na mesma posição (análogos e infraestruturas), quando os dois corpos de prova foram comparados. O corpo de prova II apresentou maiores valores de deformação. Pode-se concluir que próteses parciais fixas com extensão em cantilever apresentaram as maiores deformações e o posicionamento dos implantes e desenho protético influenciam na distribuição de tensão e deformação nos componentes do sistema.
Abstract: The aim of this study was to evaluate by means of strain gauges the deformations in metal samples of implants and framework of FPDs involving the four upper incisors of different analogues positions. Thus, two samples were made: framework I with the analogues in the position of the lateral incisors and the pontic in the central incisors; framework II with the analogues in the central incisors and the cantilever in the lateral incisors. The strain gauges were positioned in the analogues on the superior buccal and palatine surfaces. In the framework the strain gauges were fixed in superior surface. The samples received a load of 150 N, at 45 angle, on the palatine surface of the incisor left side, simulating the functional occlusion. The results showed the deformations with compression and traction characteristics. The deformations in the analogues were significantly different from those in framework (p <0.05 or p <0.01). In both samples the highest deformations values were presented in the buccal areas of the analogues. The strain gauges positioned in the same local (analogues and framework) when compared the two samples there were no significant difference. However, the sample II showed the highest deformations values. It was concluded that the fixed partial dentures with cantilever extension showed highest deformation and the positioning of the implants and prosthetic design influence the stress distribution and deformation in the components

Resumo: Objetivo: O objetivo deste estudo foi avaliar a adaptação marginal horizontal de infraestruturas de próteses fixas confeccionadas pelo sistema CAD-CAM com três diferentes sistemas de aquisição de dados. Material e Métodos: Foram confeccionados 50 corpos de prova divididos em 5 grupos, sendo eles, Convencional (Ni-Cr); GI: iTero; GII: CerecBluecam/fresadora industrial; GIII: 3S; GIV: CerecBluecam/ fresadora Sirona, todos com n=10. Os corpos de prova foram numerados e randomizados, realizou-se análise da desadaptação marginal horizontal, esta dividida em positiva para sobre-contorno e negativa para subcontorno. Resultados: A análise intra-examinador revelou não haver diferença estatística entre os dois períodos de análise (t pareado=0.019; p=0.985). Em relação as desadaptações de sobre-contorno (valores positivos) podemos analisar que o grupo controle (Convencional) apresentou estatisticamente (p <0.001) menores valores de desadaptação (82 μm) quando comparado com o grupo CAD/CAM (144 μm). Ao comparar o sistema intra/extra-oral pode-se observar que o grupo intra-oral (iTero) apresentou melhores resultados, sendo essa diferença estatisticamente significante (p <0.001) que o extra-oral (3s). Já avaliando a diferença entre um sistema aberto com um fechado foi observado que o sistema aberto apresentou estatisticamente menor desadaptação (p<0.001). Quando avaliou-se a "quebra" de um sistema fechado (Cerec BlueCam) para sua fresagem a nível industrial notou-se que há desvantagens em tal procedimento, pois houve estatisticamente maiores desadaptações no grupo da "quebra" do sistema (p<0.001). Em contra partida nenhuma diferença estatisticamente significante foi encontrada em todas as análises quando se comparou desadaptação por sub contorno. Significância: Avaliando a discrepância...

Evaluating the "broke" of a closed system (Cerec Bluecam) your industrial milling had statistically (p < 0.001) worse adaptation when compared to the laboratorial milling. About the under extended marginal discrepancy, in all analysis there was not found statistically significant difference. Significance: Assessing the horizontal marginal discrepancy was possible to note that in relation to the overextended the control group had statistically better results than the CAD/CAM systems, however the Cerec Bluecam system by sirona milling had better results than all CAD/CAM system and also the control group. Full textThis study carried out three dogs to gingival displacement comparing the copper band and compound gingival displacement and impression technique with string impregnated in aluminium chloride with gingival displacement technique, and single impression by condensing silicone impression material. The objective was to compare the periodontal biocompability between the two techniques. Blocks specimens including the teeth and the soft tissue were taken immediately, 3, 7, 14 e 21 days, and had histological analysis with stain hematoxilin and eosin. The results showed that all groups had reversible histological consequences.

Resumo: Introdução: O processo de interpretação da cor em restaurações cerâmicas delgadas pré e pós-tratamento é um desafio. Os métodos de avaliação dos resultados são diversos e a literatura não é clara em apontar o mais eficaz e eficiente. A questão é ainda mais relevante diante da variação do cimento resinoso utilizado. Proposição: Após variar o tipo de cimento resinoso utilizado, o objetivo deste estudo foi avaliar o comportamento da cor de restaurações cerâmicas em dentes com demanda estética, ao longo de 1 ano após o tratamento, com o auxílio de diferentes métodos de acompanhamento (espectrofotômetro, fotografias, vídeos e avaliação clínica) em um estudo clínico longitudinal e prospectivo. Materiais e Métodos: Foram selecionados 28 pacientes, perfazendo um total de 204 dentes restaurados (89 facetas laminadas, 57 coroas totais ou parciais e 58 lentes de contato). 76 restaurações foram cimentadas com Rely X veneer, 71 com Variolink veneer e 57 com Rely X U200. Foram realizadas mensurações de cor com espectrofotômetro, fotografias digitais e vídeos gravados no decorrer do tratamento e em controles periódicos (inicial, 30 dias, 180 dias e 1 ano após a cimentação). Grupos formados por observadores externos (pacientes, alunos de 1º e último ano de graduação, pós-graduandos e especialistas) avaliaram, aleatoriamente, as imagens obtidas inicialmente e ao final do tratamento, segundo perguntas sobre a satisfação ou não com o resultado estético do caso. Resultados: Os dados obtidos com o espectrofotômetro foram submetidos a testes estatísticos que revelaram diferença significativa apenas entre os tempos avaliados, contudo, para todos os tipos de restauração foi observada estabilidade de cor após, no máximo, 180 dias da cimentação. Abstract: Introduction: The process of interpretation of color in thin ceramic restorations pre- and post-treatment is challenging. The evaluation methods of the results are diverse and literature is not clear in pointing out the most effective method. The question is even more relevant given the variation of the resin cement used. Purpose: After changing the type of resin cement used, the aim of this study was to evaluate the color behavior of ceramic restorations in teeth with aesthetic demand, over 1 year after treatment, with the help of different monitoring methods (spectrophotometer, photographs, videos and clinical assessment) in a longitudinal and prospective study. Materials and Methods: 28 patients were selected, for a total of 204 restored teeth (89 veneers, 57 crowns and 58 contact lenses). 76 restorations were cemented with Rely X veneer, 71 with Variolink veneer and 57 with Rely X U200. Color measurements were made with a spectrophotometer, digital photos and videos taken during treatment and periodic controls (initial, 30 days, 180 days and 1 year after cementation). Groups formed by outside observers (patients, students of 1st and last year of undergraduate, graduate students, and experts) evaluated, randomly, images obtained initially and after treatment, according to questions on satisfaction or not with the aesthetic outcome of the case. Results: The data obtained with the spectrophotometer were subjected to statistical tests which showed a significant difference only between the time periods; however, for all types of restoration, color stability was observed after a maximum of 180 days after cementation. Resumo: A introdução da zircônia na Odontologia deu-se pela necessidade de melhores propriedades mecânicas dos materiais cerâmicos, pois com a crescente demanda estética, os cerâmicos tornaram-se materiais de largo uso, devido às suas propriedades estéticas e mecânicas. Em busca dessas propriedades, vários estudos analisaram a taxa de sobrevivência de restaurações zircônia-porcelana, relatando como uma das causas de falha a fratura precoce da porcelana de cobertura e o aumento significativo nos relatos clínicos de lascamento da porcelana de recobrimento aplicada sobre "coping" de zircônia. Diante desse problema, o objetivo deste trabalho foi determinar a tensão de falha no lascamento a partir de casos de clínicos e experimentais pertinentes a esse fenômeno em próteses fixas livres de metal .Foram coletados dados de artigos dos últimos 15 anos, por meio das bases de dados Pubmed e Google Schoolar, sendo selecionados para essa revisão os que obtiveram melhor abordagem do assunto, dando ênfase às possíveis causas de falhas nas restaurações cerâmicas dos casos clínicos e os ensaios realizados, visando um melhor entendimento desse fenômeno, via análise quantitativa e qualitativa, para melhor utilização e aproveitamento desse material na Odontologia. Abstract: The introduction of zirconia in dentistry was due to the need for better mechanical properties of ceramic materials, because of an increasing aesthetic demand. Ceramics became widely used materials due to their aesthetic and mechanical properties. Aiming at these properties, several studies have analyzed the survival rate of zirconia-porcelain restorations, reporting as one of the causes of failure. Early porcelain fracture coverage and a significant increase in clinical reports of veneering porcelain applied over "coping "of zirconia. In view of this problem, the objective of this work was to determine the failure causes of chipping from clinical and experimental cases associated to this phenomenon in fixed metal free prostheses. Data from articles of the last 15 years were collected through the Pubmed and Google Schoolar databases. Those who obtained the best approach to the subject were selected for this analysis, emphasizing the possible causes of failures in the ceramic restorations of the clinical cases and the tests performed. Aiming for a better understanding of this phenomenon, via quantitative and qualitative analysis, to better use this material in dentistry. To determine the failure causes, linear elastic fracture mechanics were used and the crack correction factor was determined based on the crack dimensions. Full textScaling and root planning remove cementum present in this area, which would acts as a dentin barrier against penetration of bacterias, toxins and other products coming from the surface of the tooth. The objective of this study was to evaluate in vitro endodontically treated teeth, the resistance of different materials to methylene blue, which could cross the root dentin when submitted to two different situations: a) scaling and root planning b) removal of cementum with drills. It was used 48 human uniradicular teeth, assigned by FOUSPs teeth bank and were endodontically treated and prepared to receive pins. These were modeled with an acrylic resin in order to obtain the metallic intraradicular posts. The teeth were divided into four groups with twelve specimens. Each one of the groups received a different material for cementing: Cement of Zinc® (SS White, Rio de Janeiro, Rio de Janeiro, Brazil), Panavia F® Resin Cements (Kuraray, Kurashiki, Okayama, Japan), Rely X U 100® (3M Dental, Seefeld, Germany) and Rely X Luting 2® (3M Dental, Sant Paul, United States). After this procedure, it was delimited two windows in the cervical region of the root. One of the sides the cementum was removed with a diamond drill and on the other side, with 40 Gracey 5-6 curette strokes (Neumar, São Paulo, Brazil) parallel to the long axis of the tooth. Except on the instrumented regions, the tooth was externally sealed and exposed to a 1% methylen blue solution and pH 7.2 during 48 hours. The samples were scraped from the long axis until the largest post diameter. The impregnated area with the dye were scanned and measured with the Imagelab® program. The comparison between the impregnated area through the tracer agent on the cement film in the cervical and medium thirds revealed statistically significant differences (p<0,05). On the side where the dentin was removed with a drill, there was a statistically significant difference when the three adhesive materials were compared to the Zinc Cement (SS White, Rio de Janeiro, Rio de Janeiro, Brazil). In the comparison between the resin cements there was a statistically significant difference (p<0,05) only between the cements: Panavia F® (Kuraray, Kurashiki, Okayama, Japan) and Rely X U 100® (3M Dental, Seefeld, Germany) for the apical third. On the surface treated with periodontal curettes, it was identified a meaningful statistical difference between the studied groups (p<0,05) only when compared the cements: Rely X U 100® (3M Dental, Seefeld, Germany) with Rely X Luting 2® (3M Dental, Sant Paul, United States) for the cervical third. The conclusion was that when the treatment uses a drill, the cements: Panavia F® (Kuraray, Kurashiki, Okayama, Japan), Rely X U 100® (3M Dental, Seefeld, Germany) and Rely X Luting 2® (3M Dental, Sant Paul, United States) have similar behavior to the cervical and medium third. The unique difference comes from the apical third. On the other hand, the treatments using periodontal curettes, the cements Panavia F® (Kuraray, Kurashiki, Okayama, Japan), Rely X U 100® (3M Dental, Seefeld, Germany) and Rely X Luting 2® (3M Dental, Sant Paul, United States) resulted in similar results for the medium and apical thirds, only differing on the cervical third.

Full textThis study deals with metallographic and electrochemical properties of eight parent alloys used in fixed partial denture (five High Noble, one Noble and two predominantly based alloy), and their corresponding pre- and post-solder realized in conventionnal way. Laser solder have also been realized. In the fisrt part of the study, parent alloys and their corresponding solders were caracterized by considering their physical properties, microstructure and mechanical aspect. Different types of internal defects were noticed in several parent alloys and solder joints. The second main aspect of the study concerns electochemical comportement of parents alloys and their post-solders alloys in diffent in several types of electrolyte simulating articificial saliva. Resumo: O primeiro contato de alunos de graduação com a disciplina de Prótese Parcial Fixa ocorre em aulas teóricas e posteriormente em práticas laboratoriais. Na teoria os docentes utilizam de diversas formas para elucidar um preparo de Prótese Parcial Fixa, fotos e vídeos no passo a passo, guias de silicone, guias de redução, marcações coloridas e até mesmo demonstrações ao vivo. Utilizam- se desses métodos a fim de formar um conceito e fixar uma imagem de um preparo ideal obedecendo aos princípios mecânicos e biológicos das reduções estruturais em prótese fixa. O presente estudo teve como objetivo avaliar as principais dificuldades dos alunos de graduação nos preparos coronários em Prótese Parcial Fixa e sugerir ferramentas de ensino para auxiliar aluno e professor, a elucidar e calibrar os alunos nos desgastes coronários. Participaram da pesquisa ao todo 87 alunos matriculados do 3° ano integral, dos anos 2017 e 2018 da disciplina de Prótese Parcial Fixa, da graduação em Odontologia do ICT. Foi desenvolvido um aplicativo de celular com um banco de dados dos principais preparos coronários e com a possibilidade de comparação do preparo finalizado através de fotos. Abstract: The first contact of undergraduate students with the discipline of Partial Fixed Prosthesis occurs in theory classes and later in laboratory practices. In theory, teachers use a variety of ways to elucidate a Fixed Partial Prosthesis preparation, step-by-step photos and videos, silicone guides, reduction guides, color markers and even live demonstrations. These methods are used to form a concept and to fix an image of an ideal preparation obeying the mechanical and biological principles of the structural reductions in fixed prosthesis. The present study had as objective to evaluate the main difficulties of undergraduate students in coronary arteries in Fixed Partial Prosthesis and to suggest teaching tools to help students and teachers to elucidate and calibrate students in coronary wear. A total of 87 enrolled students from the 3rd full year, from the years 2017 and 2018 of the discipline of Partial Fixed Prosthesis, from the graduation in Dentistry from the ICT participated in the research. A cellular application was developed with a database of the main coronary preparations and the possibility of comparing the finished preparation with photos. Two methodologies were also developed, applied and validated: a color design mannequin that helped the student to visualize the coronary preparations and a practical demonstration designed to compare the methods of theory and practical teaching. 78.2% of the students report the amount of reduction as the main difficulty. Full textThis study objective is to evaluate in vitro, the resistance that different resinous materials have through the fluids passage cross through the dentine in the absence of radicular cementum. 60 human teeth given by FOUSP bank of teeth were used. The teeth were submitted to endodontic treatment according to Paiva's and Antoniazzi´s technique and stored in a temperature of 37°C in humidity 100% during 24 hours for the preparation of the intra-radicular retainer, respecting the 4 mm apical limit. The intra -radicular retainers were modeled with Duralay resin type and melted with silver league and cemented with the following materials: Zinc Oxiphosfate (SS White), Enforce (Dentsply), Panavia F (Kuraray) and Rely X (3M). After the foundation, a window was delimited in the root cervical area where the cementum was removed with a diamond drill. The external part of the root was made waterproof with etile cyanocrylate and cosmetic enamel for subsequent immersion in a 1% blue methylen solution for 48 hours. Elapsed this period, the waterproof layer was removed, the teeth were consumed in the mesio-distal direction and appraised with the Imagelab software. The impregnated area by the tracer agent of the in the film of the cements comparison, in the cervical and medium thirds, revealed significant statistical differences (p <0,01) of Panavia F (Kuraray) in relation to the other three materials, demonstrating the superiority of that material in relationship to the radicular chanal waterproofing system. Its possible to conclude that among the appraised materials, the Panavia F (Kuraray) cement presented significant statistical difference, in the cervical segment as well as the medium third.

O objetivo desse estudo foi avaliar a adaptação marginal de copings cerâmicos obtidos pelos métodos de moldagem convencional e escaneamento. Os modelos foram separados em 3 grupos para avaliação e todas as coroas foram confeccionadas sobre um modelo mestre previamente fabricado em CoCr. Grupo 1: Com o auxílio de uma moldeira individual cilíndrica de acrílico foram reproduzidas 10 moldagens com silicona de adição (Panasil) no modelo mestre pela técnica de moldagem dupla e replicados dez modelos físicos em gesso pedra Fuji Rock.Sobre esses modelos foi realizado o enceramento dos copings, confecção dos sprues de cera e sobre eles foi realizada a injeção da cerâmica Grupo 2: foram realizadas 10 moldagens diretas com scanner extraoral (Swing Dental Scanner) no modelo mestre. As imagens foram obtidas através do software Exocad e copings em IPS-Emax CAD foram confeccionados em fresadora (Ceramill Motion 2) para posterior sinterização da cerâmica. Grupo 3: foram realizadas 10 moldagens diretas com scanner intraoral (CEREC Bluecam) no modelo mestre. As imagens foram obtidas através do software Exocad. Copings em IPS-Emax CAD foram confeccionados em fresadora (Ceramill Motion 2) para posterior sinterização da cerâmica. Os copings de cerâmica não foram cimentados no modelo mestre, apenas posicionados e mantidos com pressão constante de 1,4 a 1,5 kg durante a medição, com a ajuda de um delineador adaptado para esse trabalho, onde oito pontos eletivos foram usados como referência para medir a distância vertical entre a linha de término do modelo mestre e a linha de término dos copings. A margem em cada ponto foi medida 3 vezes com estéreomicroscópio Discovery V20 (ZEISS). A média de desadaptação marginal foi calculada e submetida ao teste de variância de um fator ANOVA com nível de significância de 5% e foram obtidos os seguintes resultados: 66,15µm para o grupo 1; 67,48µm para o grupo 2 e 72,91µm para o grupo 3,verificando assim que esses valores não diferem estatisticamente e todos estão dentro do limite de aceitabilidade clínica
The objective of the study was to evaluate the marginal adaptation of ceramic crowns obtained by conventional molding and scanning methods. The models were separated into 3 groups for evaluation and all the crowns were made on a master model previously manufactured in CoCr. For group 1: With the aid of a single cylindrical acrylic tray the master model was molded 10 times with Panasil in the master model by the technique of double molding and replicated ten physical models in stone gypsum Fuji Rock. On these models was done the waxing of the copings, making the sprues of wax and on them was carried out the injection of the ceramics In group 2, 10 direct impressions were performed with the Swing Dental Scanner (Korea) extra oral scanner in the master model. The images were obtained through the software Exocad (GmbH) and copings in IPS-Emax CAD were made in the Ceramill Motion 2 (Amanngirrbach) mill for subsequent sintering of the ceramics. In group 3, 10 direct impressions were performed with the CEREC Bluecam intraoral scanner (Sirona Dental Systems) in the master model. The images were obtained through the Exocad (GmbH) software. IPS-Emax CAD copings were made in the Ceramill Motion 2 (Amanngirrbach) mill for subsequent sintering of the ceramic. Ceramic copings were not cemented in the master model, only positioned and maintained with a constant pressure of 1.4 to 1.5 kg during the measurement, with the aid of an eyeliner adapted for this work, where eight elective points were used as reference to measure the vertical distance between the end line of the master model and the end line of the copings. The margin at each point was measured 3 times with stereo microscope Discovery V20 (ZEISS). The mean was calculated and submitted to the ANOVA variance test "with a significance level of 5% and the following results were obtained: 66.15μm for group 1; 67.48μm for group 2 and 72.91μm for group 3, thus verifying that these values do not differ statistically and all are within the limit of clinical acceptability.

Full textThe successful treatment of tooth loss is directly associated with a correct and systematic planning. However, in general, the decision-making process is based on the dentist s clinical experience and on publications that usually are no more than clinical observations with low scientific value. The purpose of this study was to evaluate, comparatively, the consequences of tooth loss in subjects presenting one missing tooth, in the posterior region, without treatment and subjects treated with fixed partial denture. Fifty-six subjects with a mean age of 42.86 years, who had lost one posterior tooth for at least 3 years without receiving any prosthetic treatment (Group 1 / n=28), or treated with fixed partial dentures for at least 3 years (group 2 / n=28) were consecutively selected from public and private dental clinics. Subjects were assessed for objective (age, sex, educational level, DMFT index, plaque index, gingival index and occlusal relationships) and subjective variables (satisfaction with the oral condition and chewing ability. The chi-square test and Fischer exact test were applied to analyze categorical parameters, and t test was used for numerical parameters. The significance level was set at 0.05. In general, there was no statistical difference between the two groups. With the parameters analyzed, the results of this study revealed that the absence of a permanent tooth does not cause changes that demand its replacement
O sucesso do tratamento da perda dental está diretamente associado a um correto e criterioso planejamento. Porém, de modo geral, a tomada de decisão por parte do profissional baseia-se em sua experiência clínica e em publicações que muitas vezes não passam de observações clínicas de pouco valor científico. Este trabalho teve como objetivo avaliar, comparativamente, as conseqüências da perda dental unitária, na região posterior, entre indivíduos sem tratamento e indivíduos tratados com prótese parcial fixa. Cinqüenta e seis participantes com idade média de 42,86 anos, apresentando perda de um único dente posterior permanente há no mínimo 3 anos sem tratamento protético (Grupo 1 / n=28), ou com tratamento através de prótese parcial fixa realizado há no mínimo 3 anos (Grupo 2 / n=28), foram selecionados consecutivamente a partir de clínicas odontológicas públicas e privadas. Os participantes foram avaliados sob os aspectos objetivo (idade, sexo, grau de escolaridade, índice CPOD, índice de placa, índice gengival e relações oclusais), e subjetivo (grau de satisfação com a condição oral e capacidade mastigatória). O teste qui-quadrado e o teste exato de Fischer foram aplicados para analisar os parâmetros qualitativos, e o teste t foi utilizado para os parâmetros quantitativos. Fixou-se o nível de significância em 0,05. De modo geral, não houve diferença estatística entre os dois grupos. Com os parâmetros analisados, os resultados deste trabalho revelaram que a ausência de um dente posterior permanente não provoca alterações que tornem indispensável sua reposição

Foi aplicado silano, adesivo e cimento resinoso Variolink II, fotoativado por 40 segundos. As amostras receberam 1000 ciclos térmicos de 5 a 55°C. O ensaio de cisalhamento foi realizado em máquina de ensaios MTS 810, com célula de carga de 10 kN e velocidade de 0,5 mm/min. Para a topografia superficial foram confeccionados um coping de IPS, um do INC e uma coroa de DP, seguindo as recomendações dos fabricantes. Cada coping ou coroa foi cortado com disco diamantado em quatro partes: uma delas não recebeu nenhum tratamento, e as demais receberam um dos tratamentos acima descritos. Fotomicrografias foram obtidas por meio de microscopia eletrônica de varredura e a análise foi feita de acordo com a característica da superfície. Pode-se concluir que: o tratamento associado não ofereceu vantagens em relação ao condicionamento ácido, para os materiais DP e IPS; para o INC o tratamento associado não ofereceu vantagens em relação ao jateamento. Resumo : Próteses sobre implantes osseointegrados requerem critério de fabricação preciso devido a ausência de tecidos periodontais que compensam erros menores de relacionamento. Para tanto, em um arco mandibular edêntulo, construído em aço-cromo, foram fixados cinco análogos de pilares de implante, paralelos entre si, com distância de 10mm medidos de centro a centro. Sobre estes foram parafusados novos cilindros de ouro, com torque de 10 N/cm. Então foram executadas 15 sobre-estruturas metálicas que foram divididas: GC - Adaptação passiva dos cilindros de ouro; GM - monobloco GB - segmentos soldados por brasagem e, GL - segmentos soldados a laser. Observou-se as estruturas parafusadas com parafusos sempre novos sob microscópio de mensuração. A medição foi realizada na vestibular e lingual da interface cilindro/análogo e dados anotados em tabela apropriada. Para o eixo y, a técnica de soldagem a laser (Media=12,41 e Dp=11,12) produziu menos distorção significativamente do que a técnica de brasagem (Media=23,06 e Dp=13,31) seguida pela técnica de monobloco (Media=39,83 e Dp=21,88) p= 0.05. Para o eixo x, somente o grupo Monobloco apresentou diferença significativa para as demais técnicas (Média= 45,66 e Dp=21,56)

So, in an edentulous mandibular arch, constructed with "chromium steel", five analogous were fixed, parallels, with distance of 10 mm measured of center to center. Above these gold cylinders were positioned and screwed with a torque of 10 N/m. In the next follow, fifteen over structures were fabricated and divided in tour groups: CG - Passive adaptation of gold cylinders; MG - Monobloc casting; BG - Casting in segments after brasing; LG - Casting in segments after laser welding. The structures were observed with measures microscope and the data noted in specific table. The measures done in two points (left and right) in the buccal and lingual surfaces of the interface of the cylinder/analogous set in the x and y-axis. The statistics analysis (ANOVA) showed significant statistics differences, on y-axis, among groups: LG group was superior to BG and this was superior to MG group and on x-axis only MG was superior to others groups (when we studied distortion.)

Resumo: Este trabalho buscou estudar as microdeformações geradas ao redor de implantes de hexágono externo durante carregamentos axiais e não axiais, variando-se a angulação dos implantes, utilizando a análise por elementos finitos e a extensometria linear como ferramentas. Inicialmente modelos 3D de diferentes próteses fixas foram construídos a fim de se permitir uma correlação dos resultados encontrados no modelo simplificado com o modelo anatômico e assim validar a geometria da prótese utilizada no estudo. Após a confirmação dos resultados de tensão similar entre as próteses na região dos implantes e do bloco, o modelo da prótese simplificada foi definida como válida. Em seguida, um modelo de bloco de poliuretano foi criado e duplicado. Implantes com conexão de hexágono externo (HE) foram modelados e em um bloco representados perpendiculares à superfície enquanto que em outro bloco foram colocados com inclinação de 17°. Foram modelados também intermediários do tipo mini pilar cônico retos e angulados conforme a inclinação dos implantes. Por último, foi utilizado o modelo de supraestrutura previamente validada para ambos os grupos, na qual a carga foi incidida. Todos os constituintes foram considerados perfeitamente simétricos, sólidos, isotrópicos. Os modelos receberam cargas de 300 N/cm em pontos axiais e não axiais através do software de análise por elementos finitos para se verificar a tensão máxima principal e as microdeformações. Abstract:This Work aimed to study the microstrains generated around external hexagon implants during axial and non-axial loads, varying the angulation of the implants, using finite element analysis and linear strain gauge as tools. Initially 3D models of different fixed prostheses were constructed in order to allow a correlation of the results found in the simplified model with the anatomical model and thus validate the geometry of the prosthesis used in the study. After confirming the results of similar stress between the prostheses in the implants and the block region, the simplified prosthesis was defined as valid. After, a polyurethane block model was created and duplicated. Implants with external hexagon connection (HE) were modeled and inserted perpendiculary into one block while in another block, were placed with the inclination of 17 °. Straight and angled mini conical abutments were also modeled according to the inclination of the implants. Finally, the supra-structure previously validated for both groups was used, through which the load was affected. All constituents were considered perfectly symmetrical, solid, and isotropic. The models received a load of 300 N / cm in axial and non-axial points through the finite element analysis software, to verify the maximum principal stress and microstrains. Then, through the experimental analysis of strain-gauge, two polyurethane blocks were prepared and received three HE implants each, as well as respective mini tapered pillars according to the inclination of the installed implants. Then, a metallic superstructure, identical to the computational model, was cast in Ni-Cr and screwed onto the implants with torque of 10 N / cm. The load of 300 N / cm was applied for 10 seconds at axial and non-axial points. Full textThis study deals with microstructural, structural, chemical and mechanical properties of two predominantly based dental alloys (Co-Cr, Colado CC® and Ni-Cr, Colado NC®). Cast alloys are characterized to analyze microstructure, structure, composition stability, hardness and elastic modulus. Solidification path was determined and microsegregation has been characterized. Influence of recasting has next been studied. Five times recasted alloys in different amounts were processed (0, 25, 50 and 75% of new alloy). It was demonstrated that the examined alloys are characterized by a dendritic microstructure with a large grain size, typical of the cast materials. No notable degenerative changes in microstructure were observed. Al2O3 particles were characterized on Ni-Cr alloy. Occurrence of porosities significantly increase in recast samples but remains clinically acceptable. Alloys demonstrated great compositional stability after 5 recasts. A significant decrease in Vickers hardness values appeared in the fourth recast in both alloys. Alloys showed no significant variation in term of modulus of elasticity, although a slight tendency decrease was observed. The main phase encountered is a face centered cubic solid solution. Full textThe aim of this study was to analyze, through a non linear method of finite elements, a fixed partial denture of four elements, having natural teeth as an abutment located in the mandibular first pre-molar region and an osseointegrated implant located in the mandibular second molar region in the same hemi-arc, when a vertical load of 175N was applied and distributed over the retainer and pontic. Three situations were evaluated: Model 1: fixed denture with rigid connectors; Model 2: fixed denture with a semi-rigid connector between the first and second pre-molar; Model 3: fixed denture with a semi-rigid connector between first molar and second molar. From the results obtained from all models, the bone region that presented the highest stress concentration was close to the mesial region in the cortical bone of the implant. The highest stress levels were found in model 2 and the lowest in model 3. As regards the implant, in models 1 and 3, higher stress levels were also observed in the cervicalmesial region, close to the intermediate retainer abutment union. In model 2, a great increase of stress was observed in the entire cervical region. Moreover, with regard to the connectors, in model 1, there were high stress levels between the pontics and between the pontic and implant retainer; in model 2 the highest stress levels were found between the molars; and in model 3, the highest stress levels were found at the semi-rigid connector.

Full textThe passivity of fixed partial denture (FPD) on implant and the stress distribution to the bone by the infrastructure, contribute not only to osseointegration but also to its maintenance. The stress analysis by photoelasticity allows visualizing the stresses and deformations magnitudes in a model fabricated in photoelastic resin that has a concentration pattern and stress distribution similar to the bone tissue. The aim of this study was to compare the stress distribution of cantilever FDPs with different infrastructure materials (Co-Cr and Zirconia [Zr]) and methods of manufacture (CAD/CAM and conventional casting [one piece, laser welding and welding with TIG]), when submitted to different occlusal loads (punctiform and distributed occlusal). From a master model, 3 photoelastic models were fabricated to simulate the mandibular free end, with the tooth 44 in resin and 2 implants replacing the teeth 45 and 46. The groups of this study included FPDs with infrastructures obtained by conventional casting in one piece (G1), sectioned and laser welded (G2) and TIG (G3) and obtained by the CAD/CAM system in Co-Cr (G4) and Zr (G5). The FDPs were screwed onto the abutments and subjected to punctate and distributed occlusal loading (150 N) for qualitative evaluation of the fringes and quantitative at six different points: apical (P1) and distal (P2) of the tooth 44, apical (P3) and distal (P4) of the implant replacing the tooth 45, apical (P5) and distal (P6) of the implant replacing the tooth 46. The photoelastic analysis showed a higher concentration of stresses in the FPD with one piece infrastructure (G1) and a lower concentration in the prosthesis obtained by CAD/CAM method (G4 and G5). When submitted to distributed occlusal loading, the FPD with Zr infrastructure (G5) showed a considerable increase in stress concentration, followed by one piece (G1) and those that were laser (G2) and TIG (G3) welded. In the cantilever load, the TIG welded Co-Cr infrastructure (G3) showed the highest stress concentration. Within the limitations of this study, it was concluded that FPDs with infrastructures obtained by the CAD/CAM system were associated with lower stress concentrations and that the section of metal infrastructures for later welding may also be a viable alternative, according to the peri-implant stress distribution.

Full textIn cases of partial edentulism, specially in the absence of three or more contiguous posterior teeth, there is not a consensus about the ideal number of implants nor the biomechanical convenience of splinting or not prosthetic crowns. The aim of the present study was to analyze the influence of the splinting of fixed prosthetic restorations and the number of implants on von Mises stress distribution within mathematical two-dimensional models obtained from finite element method. From the digitalized image of a real anatomical piece, corresponding to the sagital slice of the left posterior portion of a mandible, three models were constructed in the following configurations: (1) non-splinted prosthetic crowns on three implants; (2) splinted prosthetic crowns on three implants; and (3) fixed partial denture on two implants, with an intermediate pontic. A 300N static load was divided, in equal magnitude, by 15 application areas on occlusal surfaces and angled in 15º in relation to the long axis of the restorations. The results showed that the model with splinted crowns on three implants presented the lowest stress values, whereas the highest stress values were found in the model with non-splinted crowns. The proportional limit of the analyzed materials and structures was not reached in the three models, fact that did not contraindicate any studied configuration. However, the results suggested that treatment planning may be adequate to anatomical, occlusal and biomechanical specific conditions.

Este estudo teve como objetivo avaliar a efetividade de quatro condicionadores de metal (Metal Photo Primer, MPP; Cesead II Opaque Primer, OP; Targis Link, TL e Siloc, S) aplicados sobre diferentes superfícies das ligas de Ni-Cr, Co-Cr e do metal Ti Cp. Após o polimento metalográfico e jateamento com Al2O3, foram obtidos três grupos experimentais para cada metal : Grupo polimento (P) - controle; Grupo (J50), jateado com Al2O3 de 250 um. A esses grupos foram aplicados os condicionadores e as resinas opaca e de revestimento estético (Artglass). Decorridos 24 horas de armazenamento em água a 37ºC, cada grupo foi dividido em dois grupos e submetidos aos ensaios mecânicos de cisalhamento (0,5 mm/min.), um antes e outro após 500 ciclos térmicos (60ºC e 4ºC). As superfícies fraturadas foram analisadas e o padrão de falhas determinado. Adicionalmente foram feitas análises de MEV e EDS. Seguem os resultados para cada metal: Liga de Ni-Cr: para os Grupos P e J50, não foram observadas diferenças (p>0,05) nos valores médios de tensão de cisalhamento entre os condicionadores OP e TL e entre MPP e S, sendo maiores as médias dos dois primeiros. Não foram encontradas diferenças (p>0,05) entre as médias de tensão para os condicionadores do Grupo J250. No entanto, esses valores foram superiores aos do Grupo J50. Após a termociclagem, as médias para os condicionadores OP, S e TL não apresentaram aumento significante (p>0,05), enquanto a média para o MPP aumentou (p<0,05). The purpose of the study was to evaluated the effect of three adhesive primers (Metal Photo Primer, MPP; Cesead II Opaque Primer, OP; Targis Link, TL) and a bonding system (Siloc, S) on the shear bond strength between a prosthetic composite material (Artglass) to Ni-Cr alloy (Wiron 99), Co-Cr alloy (Wironit), and CP Ti (Tritan). Disk patterns were cast of the 3 alloys. All the disk surfaces were abraded with No. 80, 320, and 600 SiC paper, respectively and three different surface treatment were applied: No. 600 SiC polishing, P; 50 mm Al2O3 sandblasting, S50; 250 mm Al2O3 sandblasting, S250. The composites (Artglass opaque and dentin) were applied using a cylindrical stainless steel split according to manufacturer's instructions then light polymerized. Specimens without primer and bonding system were also prepared as control. The specimens were stored in water a 37oC for 24 h and then shear bond strengths were determined both before and after immersed alternately in water baths at 4oC and 60oC for 15 seconds each for up to 500 thermal cycles before shear mode testing at a crosshead speed of 0.5 mm/min. Data were statistically analyzed using 3-way ANOVA and 1-way ANOVA, followed by Tukey multiple range intervals calculated at the 0.05 significance level. Fractured interfaces were analyzed by use of light microscopy and SEM. Results: Ni-Cr alloy: The bond strength of the P-OP/J50-OP group and P-TL/J50- TL group were statistically comparable (p>0,05) and exhibited greater strength than the P-MPP/J50-MPP group and P-S/J50-S (p<0,05). No significant differences were found among the four conditioners for J250 group (p>0,05), and these group showed the greater strength than the J50 group (p<0,05). Resumo: O presente estudo teve como objetivo avaliar as características mecânicas de próteses parciais fixas adesivas, confeccionadas em resina composta de uso indireto, reforçadas por uma malha de nylon. Para a confecção da matriz dos espécimes e estudo das tensões nas estruturas, foi realizada análise qualitativa pregressa análise de elementos finitos (FEA). A pesquisa in vitro simula uma prótese parcial fixa de três elementos com inserção entre os primeiros pré-molares e os primeiros molares superiores (n = 60; 10 por grupo). Três dos seis grupos passaram por ciclagem mecânica. Os grupos experimentais foram: G1-prótese fixa convencional; G2 - prótese convencional ciclada; G3 - prótese com reforço da malha posicionada na vertical; G4 - Prótese com reforço da malha posicionada na vertical ciclada; G5 - Prótese com reforço da malha posicionada na horizontal; G6 - Prótese com reforço da malha posicionada na horizontal ciclada.Todos os grupos passaram por ensaio de carga máxima a fratura (EMIC). Os resultados não apresentaram diferença estatística entre os grupos. Concluiu-se que a utilização da malha nylon proporcionou estabilidade, mesmo após a fratura da peça, pois as partes não se separam, além disso, os resultados obtidos in vitro puderam validar os dados in sílico
Abstract: The present study aimed to evaluate the mechanical characteristics of adhesive fixed partial dentures, made from composite resin of indirect use, reinforced by a nylon mesh. A finite element analysis (FEA) was used to compile the specimen matrix and to study the stresses in the structures. In vitro research, simulated a fixed partial denture of three elements with insertion between the first premolars and the major maxillary molars (n = 60; 10 per group). Three of the six groups underwent mechanical cycling. The experimental groups were: G1 - conventional fixed prosthesis; G2 - conventional cyst prosthesis; G3 - prosthesis with reinforcement of the mesh positioned vertically; G4 - Prosthesis with reinforcement of the mesh positioned in the cycled vertical; G5 - Prosthetics with mesh reflux positioned horizontally; G6 - Prosthetics with mesh reinforcement positioned horizontally cycled. All groups underwent maximum fracture loading test (EMIC). The results do not present a statistic between the groups. It was concluded that the use of the nylon mesh provided stability, even after fracture of the part, as parts do not separate, besides, the results obtained in vitro could validate the data in silica

Full textIn cases of severe vertical bone loss, especially in the posterior region of the mandible, the use of short implants associated with splinted resin-veneered fixed dental prostheses (FDPs) can be an alternative to bone graft surgery and better force transmission to the adjacent bone, respectively. Among the methods to evaluate the stress distribution around the implants, the digital image correlation (DIC) has advantages as regards the possibility of evaluating the stress distribution continuously using images taken over time, during occlusal loading. Thus, this study evaluated qualitatively and quantitatively the biomechanical behavior of splinted and non-splinted resin-veneered FDPs, screw-retained on implants of different lengths positioned in the posterior region of the mandible with the aid of the DIC method. Four F16 polyurethane resin models were fabricated to simulate half of the mandibular arch containing tooth 44 in resin (Luxatemp) and teeth 45, 46 and 47 replaced by short (4 x 5 mm) and/or conventional (4 x 11 mm) implants. Groups (G) of this study were: G1 (two conventional implants [45 and 46] and one short implant [47]), G2 (one conventional implant [45] and two short implants [46 and 47]) G3 (three short implants) and G4 (three conventional implants). Splinted and non-splinted FDPs were screwed to the implant abutment and subjected to different oclusal loads of 250N: punctiform (45 mesial and 47 distal) and balanced occlusal load. Strains in the horizontal direction (εxx) were calculated based on displacement and compared qualitatively and quantitatively by the DIC software (Davis 8.0, LaVision Inc.). In general, there was a stress concentration at higher intensity in those areas where short implants were placed. In punctiform load on the second molar, there was a compressive and tensile stresses predominance when splinted and non-splinted crowns were used, respectively, and the stresses of greater magnitude were found in the G2, G3 and G4 with non-splinted crowns (p>0.05), showing median values (μS) equivalent to 396.08, 360.45 and 354.04, respectively. In punctiform load involving the second premolar, there was a compressive stress predominance in all planning with highest magnitudes in groups with splinted crowns (p<0.05). The oclusal load distributed simulations also revealed compressive stresses predominance in all planning, with greater magnitude in groups with splinted crowns, especially in G1 (-464.05 ± 106.43) and G3 (-474.39 ± 179.54), both with statistically significant differences when compared to the other groups (p<0.05). Within the limitations of this study, it can be concluded that splinting multiple adjacent implant supported provided a high stress concentration around the implants during punctiform (45 mesial) and balanced occlusal load, reinforcing the need for effective interproximal contact and oclusal adjustment to promote better stress distribution. Additionally, the combination of short and conventional implants seems to be a viable alternative in the rehabilitation of mandibular free end with reduced bone height.

Full textThis study deals with microstructural, structural, chemical and mechanical properties of two predominantly based dental alloys (Co-Cr, Colado CC® and Ni-Cr, Colado NC®). Cast alloys are characterized to analyze microstructure, structure, composition stability, hardness and elastic modulus. Solidification path was determined and microsegregation has been characterized. Influence of recasting has next been studied. Five times recasted alloys in different amounts were processed (0, 25, 50 and 75% of new alloy). It was demonstrated that the examined alloys are characterized by a dendritic microstructure with a large grain size, typical of the cast materials. No notable degenerative changes in microstructure were observed. Al2O3 particles were characterized on Ni-Cr alloy. Occurrence of porosities significantly increase in recast samples but remains clinically acceptable. Alloys demonstrated great compositional stability after 5 recasts. A significant decrease in Vickers hardness values appeared in the fourth recast in both alloys. Alloys showed no significant variation in term of modulus of elasticity, although a slight tendency decrease was observed. The main phase encountered is a face centered cubic solid solution. Full textProsthetic restoration should suffer and apply functional loads over implants in a controlled way with the aim to reduce the occurrence of mechanical and biological failures. The concentration of inadequate stress on the implant-bone interface may cause bone resorption around implants at levels above those considered clinically acceptable. There is no consensus in the literature about the best prosthetic plan for partial rehabilitation with multiple adjacent implants, with the aim to reduce the stress generates over the implantbone interface. Photoelasticity is an experimental technique used to analyzes stress and is highly used due to its relative simplicity and reliability in terms of the clinical agreement of the findings. The purpose of the present study was to assess, using photoelastic analysis, the biomechanical behavior of screwretained fixed partial dentures on morse taper implants with individual or connected crowns, simulating the rehabilitation of posterior areas with or without the presence of a dental element distal to the implants. Furthermore, the analyses considered different esthetic materials used in prosthetic restorations (ceramic or resin), and different static loads were applied over the prostheses. To do this, a photoelastic model was fabricated simulating the missing of the second pre-molar and the first molar. The model was rehabilitated through four types of screw-retained crowns over two adjacent morse taper implants (Neodent): CC connected crowns with ceramic esthetic overlay; IC individual crowns with ceramic esthetic overlay; CR connected crowns with resin esthetic overlay; - IR - individual crowns with resin esthetic overlay. The crowns marginal fit was assessed by reading the vertical misfits on the crownabutment interface using an optic microscope. The qualitative photoelastic analysis was performed under the application of different loads on the crowns occlusal surface: distributed occlusal (1kgf), simultaneous punctiform (1kgf), alternate punctiform over the molar and pre-molar (0.5 kgf). The first analysis was done with an effective proximal contact between the prostheses and the distal tooth. The second analysis was performed after eliminating the crown of the distal tooth in the photoelastic model. Photoelastic records were obtained in each situation that was of any interest to the analysis, with the aim to make it easy to observe and compare the distribution standard of the isochromatic fringes around the implants. According to the obtained results, it is concluded that crown splint promoted better stress distribution around the implants. The metallo-ceramic restorations resulted in less magnitude and stress concentration around the implants compared to metaloplastic restorations. The presence of one tooth with an effective contact point on the crown distal did not change the resulting stress around the implants, but that tooth took part in the stress distribution for the photoelastic model. A bad distributed occlusion over the prosthetic restorations can cause overload on the implants.

Full textIn cases of severe vertical bone loss, especially in posterior region of the mandible, an alternative to bone graft surgery, little used and studied, is the use of implant-supported cantilever fixed partial denture (FPD). Thinking about the framework material that ensures adequate strength to resist the masticatory loads in long term, together with the development of zirconia (Zr) and of CAD/CAM technology, the aim of this study is to evaluate the vertical misfit, torque loss and fracture resistance of cantilever and stress distribution in FPDs screwretained on Morse taper implants, fabricated with different framework materials (Co-Cr [Conventional Casting] and Co-Cr and Zr [CAD/CAM]) and positioned on the posterior region of the mandible. Forty FPDs with distal cantilever were fabricated and distributed into 5 groups (n=8), according to framework material: 1 Co-Cr (Conventional casting laser welding); 2 Co-Cr (Conventional casting TIG welding); 3 Co-Cr (Conventional casting one-piece); 4 Co-Cr (CAD/CAM) and 5 Zr (CAD/CAM). Analysis of vertical misfit was performed in 4 different moments, using stereomicroscopy (50× magnification) and the Sheffield method: T0 Framework on master model; T1 Framework on polyurethane model, before the ceramic veneering application; T2 FPD after application of the ceramic veneering and before the thermomechanical cycling and T3 FPD after thermomechanical cycling. Evaluation of screw loosening was performed before (T2) and after (T3) thermomechanical cycling and was obtained by a digital torque wrench with an accuracy of 0.1 N.cm. To perform the thermomechanical cycling, FPDs were positioned in the testing machine, and a 120 N load was applied on them, seeking to simulate 14 months of use (1,200,000 cycles). Fracture maximum resistance of the cantilever was determined by the flexural test and, therefore, a perpendicular force on the cantilever was applied at a crosshead speed of 0.5 mm/min. A new FDP for each group was fabricated to be used in the evaluation of the stress distribution, when they were submitted to punctiform on the cantilever and oclusal distributed loads (150 N), by the photoelastic analysis. In general, FPDs with one-piece frameworks (G3) presented higher vertical misfits, especially in the element not tightened of the second and third experimental conditions, and were associated with higher stress concentration in both the punctiform (360.07 kPa) and oclusal distributed (302.46 kPa) loads. On the other hand, the frameworks obtained by the CAD/CAM system showed the smaller vertical misfits, being statistically similar (p>0.05) to welded frameworks at various times, and better stress distributions, reaching values less than 280 kPa in the punctiform load and 200 kPa in the occlusal distributed load. Transfer impression (T0 → T1) and thermomechanical cycling (T2 → T3) had little effect on vertical misfit of the groups. The application process of the ceramic veneering (T1 → T2) and its cumulative effect with thermomechanical cycling (T1 →T3) were responsible for considerable changes in vertical misfit of the group, especially in those conventionally cast (G1, G2 and G3). A thermomechanical cycling had considerable effect in the percentage of intragroup torque loss (pMolar=0.039; pPré-molar=0.044), although comparisons between the different groups did not show statistical significance (pMolar=0.116; pPrémolar= 0.334). FPDs with metal frameworks (>410.83±72.26 N) showed considerably higher fracture resistance (p<0.05) than zirconia (277.47±39.10 N) and the first signs of fracture of the ceramic veneering were observed around 900 N. Within the limitations of this study, FPDs with frameworks fabricated using the CAD/CAM system were associated with better cervical adaptation and stress distribution, although the section of the frameworks followed by welding may also be a viable alternative, based on the proximity of the values of vertical misfits and stress distribution. Additionally, FPDs with zirconia frameworks can be used with some degree of security, considering the good marginal adaptation and stress distribution, as well as high values of fracture resistance verified.

Full textA photoelastic stress analysis and a digital image correlation (DIC) method were used to analyze the biomechanical behavior of implant supported prosthesis. This study compared the biomechanical behavior of screwed, splinted and non-splinted, implant supported fixed partial dentures, simulating the rehabilitation of a mandibular posterior region, with and without the presence of a second molar proximal contact. Since complete passivity is difficult to achieve when splinted restorations supported by multiple implants are used, some authors suggest individually restoring adjacent implants to allow for a passive fit in the resulting restorations, thus reducing the strains transferred to the implants. Photoelasticity is an experimental technique for stress analysis and has been constantly used due its simplicity, reliable results and correlation with clinical findings. The digital image correlation method is capable of providing a full-field surface strain measurement, allowing for an accurate analysis. The ideal restoration of a partially edentulous space remains controversial as to the best prosthetic design for a partial rehabilitation with multiple implants in order to minimize the strains transferred to the bone-implant interface. The influence of the veneering materials was also compared (porcelain and resin). An epoxy resin model was made with acrylic resin replicas of a mandibular first premolar and second molar and with threaded implants (Titamax GT - 4.0 x 11.0 mm - Neodent) replacing the second premolar and first molar. Another model was made without the presence of the second molar. Groups were: UC - porcelain-veneered splinted crowns; IC - porcelain-veneered non-splinted crowns; UR - resin-veneered splinted crowns; IR - resin-veneered non-splinted crowns. Strains transferred to the photoelastic model were analyzed after tightening the prosthetic screws. Qualitative and quantitative photoelastic stress analysis was performed after the application of a vertical load (50 N) on the occlusal surface of the crown replacing the first molar. The image correlation analysis was also performed qualitatively and quantitatively, with the load applied in the same position as for the photoelastic analysis, although with greater intensity (250 N). Strains in the horizontal direction (εxx) and shear strains (εxy) were analyzed and compared with the DIC method. Both methods for strain analysis used in this study performed satisfactory and are adequate for the comparisons proposed in this work. According to the results found, it can be concluded that splinting multiple adjacent implant supported crowns provided a better strain distribution around the implants and the supporting structures, especially when the distal proximal contact was absent. The total strains in the model were not influenced by the presence of a distal proximal contact, but the presence of the second molar optimized the strain distribution between the structures. The crowns veneering material was not significant for the strains transferred to the photoelastic model and for compressive strains (εxx). Shear strains (εxy) were influenced by the presence of the second molar, and when it was absent, the use of a veneering material with low elasticity modulus (resin) significantly reduced these tensions.

Na literatura científica estão descritas diversas possibilidades de tratamento e existe uma grande variabilidade de materiais e protocolos instituídos, o que dificulta a comparação e a conclusão relativa a qual das opções é efetivamente a melhor. Objetivo: Realizar uma revisão sistemática sobre avaliação dos tipos de reabilitações fixas dento-suportadas em dentes anteriores, abordando as principais propriedades e comparando de cada uma e delas. Materiais e métodos: A presente revisão bibliográfica foi baseada em informação científica publicada, recorrendo-se às seguintes bases de dados: “MEDLINE/Pubmed”, “B-On”, “EBSCO HOST” e “SCIENCE DIRECT”, através da utilização das palavras-chave: “Cerâmic crowns”; “Metalceramic crowns”; CAD-CAM”; “Fixed partial denture”; “Dental techonology”; ”Dental materials”; “Prótese parcial fixa”; “Cerâmica dentária”; “Procera”; “IPS Empress”; In-Ceram”; “Coroas dentárias”. Conclusões: Para cada situação clínica quando se está perante uma reabilitação com prótese fixa, é importante determinar os objetivos do tratamento com base num diagnóstico preciso e selecionar o tratamento mais adequado, bem como o tipo de material a utilizar, de maneira a que a estética, resistência e durabilidade estejam associadas. O estudo de novos materiais dentários tem proporcionado um melhor tratamento estético e funcional a quem recorre aos tratamentos restauradores com coroas metalocerâmicas ou totalmente cerâmicas. Assim, com o aperfeiçoamento e desenvolvimento de novas técnicas, bem como o avanço da tecnologia, podem obter-se estruturas cada vez mais semelhantes às estruturas dentárias naturais. As coroas metalocerâmicas ou totalmente cerâmicas, dão-nos a capacidade conseguir realizar a substituição de uma ou mais estruturas dentárias, quer seja por perda parcial dos dentes, usando a coroa como reforço, ou por perdas totais dos dentes, com a utilização de pontes ou implantes. No decorrer desta revisão, descreve-se e compara-se as várias propriedades de cada tipo de coroa, bem como dos materiais e técnica CAD-CAM utilizados nas suas confeções. Introduction: The knowledge of the characteristics of the fixed prosthesis PFM and all-ceramic is crucial to obtain a proper oral rehabilitation, as for medical dentist as for the patient to be rehabilitated. In the scientific literature are described several possibilities for treatment and there is a great variability in materials and protocols in place, which makes the comparison and the arrival of conclusions. Objective: Conduct a systematic review of evaluation of the types of tooth-supported fixed restorations in anterior teeth, addressing the main properties and comparing each of them and. Material and methods: The present bibliographic revision has bases in published scientific information. Conclusions: For each clinical situation when one is faced with a rehabilitation with fixed prosthesis, it is important to determine the goals of treatment based on an accurate diagnosis and to select the most appropriate treatment, as well as the type of material to be used, so that the aesthetics, strength and durability are associated. The study of new dental materials has provided a better functional and aesthetic treatment to those who resort to restorative treatments crowns with metal-ceramic or all-ceramic. Thus, with the improvement and development of new techniques as well as the advancement of technology, allows us to structure increasingly similar to natural tooth structure. The ceramic-metal crowns or fully ceramic give us the ability to accomplish replacement of one or more dental structures, whether by partial loss of the teeth, using a crown reinforcement, or total loss of teeth, with the use of bridges or implants. During this revision will be described in detail the various properties of each type of crown, as well as materials and technique used in the CAD-CAM their clothing.

Resumo: Este estudo teve como objetivo avaliar a efetividade de quatro condicionadores de metal (Metal Photo Primer, MPP; Cesead II Opaque Primer, OP; Targis Link, TL e Siloc, S) aplicados sobre diferentes superfícies das ligas de Ni-Cr, Co-Cr e do metal Ti Cp. Após o polimento metalográfico e jateamento com Al2O3, foram obtidos três grupos experimentais para cada metal : Grupo polimento (P) - controle; Grupo (J50), jateado com Al2O3 de 250 um. A esses grupos foram aplicados os condicionadores e as resinas opaca e de revestimento estético (Artglass). Decorridos 24 horas de armazenamento em água a 37ºC, cada grupo foi dividido em dois grupos e submetidos aos ensaios mecânicos de cisalhamento (0,5 mm/min.), um antes e outro após 500 ciclos térmicos (60ºC e 4ºC). As superfícies fraturadas foram analisadas e o padrão de falhas determinado. Adicionalmente foram feitas análises de MEV e EDS. Seguem os resultados para cada metal: Liga de Ni-Cr: para os Grupos P e J50, não foram observadas diferenças (p>0,05) nos valores médios de tensão de cisalhamento entre os condicionadores OP e TL e entre MPP e S, sendo maiores as médias dos dois primeiros. Não foram encontradas diferenças (p>0,05) entre as médias de tensão para os condicionadores do Grupo J250. No entanto, esses valores foram superiores aos do Grupo J50. Após a termociclagem, as médias para os condicionadores OP, S e TL não apresentaram aumento significante (p>0,05), enquanto a média para o MPP aumentou (p<0,05). Liga de Co-Cr, para o Grupo P, não foram observadas diferenças (p>0,05) nos valores médios de tensão entre os condicionadores OP e TL e entre MPP e S, sendo superiores as médias dos dois primeiros. Abstract: The purpose of the study was to evaluated the effect of three adhesive primers (Metal Photo Primer, MPP; Cesead II Opaque Primer, OP; Targis Link, TL) and a bonding system (Siloc, S) on the shear bond strength between a prosthetic composite material (Artglass) to Ni-Cr alloy (Wiron 99), Co-Cr alloy (Wironit), and CP Ti (Tritan). Disk patterns were cast of the 3 alloys. All the disk surfaces were abraded with No. 80, 320, and 600 SiC paper, respectively and three different surface treatment were applied: No. 600 SiC polishing, P; 50 mm Al2O3 sandblasting, S50; 250 mm Al2O3 sandblasting, S250. The composites (Artglass opaque and dentin) were applied using a cylindrical stainless steel split according to manufacturer's instructions then light polymerized. Specimens without primer and bonding system were also prepared as control. The specimens were stored in water a 37oC for 24 h and then shear bond strengths were determined both before and after immersed alternately in water baths at 4oC and 60oC for 15 seconds each for up to 500 thermal cycles before shear mode testing at a crosshead speed of 0.5 mm/min. Data were statistically analyzed using 3-way ANOVA and 1-way ANOVA, followed by Tukey multiple range intervals calculated at the 0.05 significance level. Fractured interfaces were analyzed by use of light microscopy and SEM. Results: Ni-Cr alloy: The bond strength of the P-OP/J50-OP group and P-TL/J50- TL group were statistically comparable (p>0,05) and exhibited greater strength than the P-MPP/J50-MPP group and P-S/J50-S (p<0,05). No significant differences were found among the four conditioners for J250 group (p>0,05), and these group showed the greater strength than the J50 group (p<0,05). Full textThis study evaluated the addition of 0.5mm of diameter Al2O3/GdAlO3 fibers obtained by LHPG (Laser Heated Pedestal Growth) to veneering porcelains of the following materials: porcelain monolayers: VM7 and VM9; Bilayers: In-Ceram Alumina + VM7, In-Ceram 2000 AL Cubes + VM7 and In-Ceram 2000 YZ Cubes for CEREC + VM9 (VITA Zahnfabrik). The specimens (bars) were composed by only the veneer porcelain (monolayer) or both the veneer and the framework (bilayer). The new ceramic composites contained longitudinal fibers in 10 or 17% vol. Specimens without fibers were used as control. A metallic device was developed for the standardization of the fibers position inside the specimens. The means of the fracture toughness, density and flexural strength of the developed fibers were calculated as well as the density and flexural strength of the composites. One way ANOVA and a Tukey test (5%) were applied. Characterizations of fiber/porcelain interface and fractographyc analysis were carried out after scanning electronic microscopy (SEM) of bars. X-ray difratometry and dilatometry were also performed. Vm9 porcelain showed incompatibility with the fibers, probably due to differences between the coefficients of thermal expansion of fibers and porcelain. VM7 monolayers with the addition of 17% of fibers in volume showed significantly higher resistance in relation to control. It was concluded that the inclusion of ceramic fibers of Al2O3/GdAlO3 to veneer porcelain improves its flexural strength.

Full textThis study was composed by two phases that evaluated two gingival displacement (GD) techniques, conventional and cordless: 1- a in vitro study which assess cytoxicity/genotoxicity, and pH of AlCl3-based agents; and 2- a randomized clinical which evaluated the efficacy of these techniques. For in vitro phase, eluates were obtained by dilution of AlCl3-based into culture medium for 1h, 4h, and 24h. pH values were measured by digital pHmeter, cytotoxicity by MTT test and genotoxicity by Micronuclei test. A crossover, double-blind, randomized clinical trial was designed to evaluate clinical, immunologic and individual factors before and after GD by two techniques, conventional (gingival cord + astringent gel) and cordless (astringent-based paste). These factors were evaluated on incisors prepared for metal-ceramic dental crowns before, 1 day post-GD and 10 day post-GD. Interleukin 1β, Interleukin 6 and Tumor Necrosis Factor α concentrations were evaluated on gingival crevicular fluid collected before (control) and 1 day post-GD. In vitro data were analyzed by two-way ANOVA and Tukey test. Clinical data were analyzed by One-way Repeated Measures ANOVA and Tukey test (immunologic factor), Friedman test (periodontal parameters), and Fisher Exact or Qui-square test (pain, stress, and unpleasant flavor during GD). All test were realized considering p<0.05. Mean pH values were statistically higher for Viscostat than Expasyl. Both astringents were non-cytotoxic at 1h. Expasyl was more cytotoxic than Viscostat at 4h and 24h. Expasyl genotoxicity was not different from control group. Neither of the two evaluated techniques resulted in worse periodontal indices. Conventional technique was statistically more stressing than cordless, and similar considering pain and unpleasant taste. GD by the both 11 treatments evaluated led to statistically higher mean concentrations of the three interleukins evaluated. Conventional GD technique resulted in the highest interleukins levels. In vitro, AlCl3-based astringents presented acceptable biological activity with no genotoxic difference between Expasyl and control group. In vivo, cordless technique was less stressful and resulted in lower cytokines levels after GD
Este estudo foi composto por duas fases que avaliaram duas técnicas para afastamento gingival (AG), convencional e sem fio: 1- um estudo in vitro que avaliou acitotoxicidade/genotoxicidade, e o pH de agentes à base de AlCl3; e 2- um ensaio clínico randomizado que avaliou a eficácia destas técnicas. Na fase in vitro, eludatos foram obtidos pela diluição dos eluates agentes à base de AlCl3 em meio de cultura por 1h, 4h e 24h. Os valores de pH foram aferidos por pHmetro digital, citotoxicidade pelo teste MTT e genotoxicidade pelo teste do Micronúcleo. Um ensaio clínico randomizado, cruzado, duplo-cego foi concebido para avaliar fatores clínicos, imunológicos e individuais antes e após AG por duas técnicas, convencional (C: fio gengival + gel adstringente) e sem fio (SF: pasta à base de adstringente). Estes fatores foram avaliados em incisivos preparados para coroas metalo-cerâmicas antes, 1 dia e 10 dias após AG. As concentrações de Interleucina 1β, Interleucina 6 e Fator de Necrose Tumoral α foram avaliadas no fluido gengival crevicular coletado antes (controle) e 1 dia após AG. Os dados do estudo in vitro foram analisados por ANOVA 2-fatores e teste de Tukey. Os dados do ensaio clínico foram analiados por ANOVA para medidas repetidas e teste de Tukey (fator imunológico), teste de Friedman (parâmetros periodontais), e Exato de Fisher ou teste Qui-quadrado (dor, estresse e sabor desagradável). Todos os testes consideraram p<0.05. Os valores médios de pH foram estatísticamente superiores para C que para SF. As técnicas não foram citotóxicas no tempo de 1h. SF foi mais citotóxica que C em 4h e 24h. A genotoxicidade SF foi semelhante à do grupo controle. Nenhuma das técnicas avaliadas resultou em piora nos indices periodontais. Atétnica C foi mais estressante 9 que a SF, e semelhantes considerando dor e sabor desagradável. AG realizado por ambas as técnicas levou a maiores concentrações médias das três citocinas avaliadas. C resultou nos maiores níveis de interleucinas. In vitro, os adstringentes apresentaram atividade biológica aceitável, sem diferença na genotoxicidade entre as técnicas. In vivo, SF foi menos estressante e resultou em menores níveis de citocinas após AG

Full textPara reabilitar a ausência de um elemento dentário posterior, as próteses parciais fixas (PPF) com retentores intracoronários são uma alternativa aos implantes osseointegrados. O objetivo deste estudo foi avaliar a distribuição de tensões nessas próteses com três combinações de materiais: cerâmica de zircônia parcialmente estabilizada por ítria (ZPEI) revestida por cerâmica de fluorapatita (α), cerâmica de dissilicato de lítio (β) ou compósito fibrorreforçado (γ). Na composição α, foram analisadas a presença ou ausência da cerâmica de revestimento na parede cervical das caixas proximais e três variações na área total da seção transversal dos conectores (4 mm de largura x 3,2, 4,2 ou 5,2 mm de altura). Em 8 modelos bidimensionais de elementos finitos, uma carga vertical de 500 N foi aplicada na fossa central do pôntico e as tensões principais máximas (tração) e mínimas (compressão) foram apontadas em MPa. Inicialmente foram avaliados os 6 modelos com PPF de ZPEI e suas variações. Os maiores valores das tensões de tração foram encontrados no terço cervical dos conectores. Quando presente nestas regiões, a cerâmica de revestimento recebeu tensões acima do limite de sua resistência à flexão. Na comparação entre os modelos sem cerâmica de revestimento na parede cervical das caixas proximais, mesmo aquele com conectores de 3,2 x 4 mm, cuja infraestrutura apresentava 2,5 x 3 mm, poderia ser recomendado para uso clínico. Altos valores de tensões de compressão foram registrados entre o terço oclusal e médio dos conectores, correspondente à união entre as cerâmicas, o que poderia ocasionar, devido à flexão, falhas adesivas. Posteriormente, o modelo de ZPEI com a cerâmica de fluorapatita ausente da parede cervical das caixas proximais e área total dos conectores de 4,2 x 4 mm foi comparado aos dois outros materiais com conectores de mesma área. Na PPF de dissilicato de lítio, os valores representaram uma provável violação do limite de sua resistência à flexão. A PPF de compósito fibrorreforçado apresentou tensões bem abaixo do limite de resistência à flexão de sua infraestrutura, mas, como no modelo de ZPEI, tensões compressivas se concentraram com alto valor entre o terço oclusal e médio dos conectores, local de união entre a resina composta e a infraestrutura de fibras. Os resultados mostraram que a cerâmica de dissilicato de lítio e a presença da cerâmica de fluorapatita na parede cervical das caixas proximais deveriam ser contraindicadas para a condição proposta. Parece viável uma área de conectores na infraestrutura de ZPEI com no mínimo 2,5 x 3 mm. A PPF de compósito fibrorreforçado apresenta resistência estrutural para a situação estudada, mas, como também aquelas compostas de ZPEI, aparenta ter como pontos fracos a adesão entre a infraestrutura e o material de cobertura e a própria resistência deste último.
Inlay-retained fixed partial dentures (IRFPD) are an alternative to osseointegrated implants to replace a single missing posterior tooth. The aim of this study was to evaluate the stress distribution within these prostheses with three material combinations: yttria-tetragonal zirconia polycrystal (Y-TZP) ceramic veneered with fluorapatite ceramic (α), lithium-disilicate ceramic (β) or fiber-reinforced composite (γ). In composition α, an analysis was conducted on the presence or absence of the veneering ceramic on the cervical wall of the proximal boxes and three variations in the total area of the connectors cross-section (4 mm in width x 3.2, 4.2 or 5.2 mm in height). In 8 two-dimensional finite element models, a vertical load of 500 N was applied on the central fossa of the pontic. Maximum (tension) and minimum (compression) principal stresses were then shown in MPa. First, the 6 models with Y-TZP fixed partial dentures (FPD) and their variations were evaluated. The highest values of tensile stresses were found at the cervical third of the connectors. When present in these regions, the veneering ceramic received stresses beyond its flexural strength limit. In the comparison among models without veneering ceramic on the cervical wall of the proximal boxes, even the model with connectors of 3.2 x 4 mm, whose framework presented 2.5 x 3 mm, could be recommended for clinical use. High values of compressive stresses were recorded between the middle and occlusal thirds of the connectors, corresponding to the juncture of the ceramics. Owing to flexure, these stresses might lead to adhesive failures. Then, the Y-TZP model without fluorapatite ceramic on the cervical wall of the proximal boxes and total connector area of 4.2 x 4 mm was compared to the other two materials with connectors of the same area. For the lithium-disilicate FPD, the observed values would probably exceed its flexural strength limit. The fiber-reinforced composite FPD presented stresses far below the flexural strength limit of its framework, but, as observed in the Y-TZP model, high values of compressive stresses were concentrated between the middle and occlusal thirds of the connectors, where the juncture of the composite and the fiber framework is located. The results showed that the use of lithium-disilicate ceramics and the presence of fluorapatite ceramics on the cervical wall of the proximal boxes should be contraindicated for the condition proposed. A minimum of 2.5 x 3 mm for the connector area of the Y-TZP framework seems viable. The fiber-reinforced composite FPD presented structural resistance for the situation studied but, as was also the case of the Y-TZP fixed partial dentures, its weak points seem to be the adhesion between the framework and the veneering material, as well as the resistance of the veneering material itself.

Full textThe aim of this study was to compare the fracture strength of abutment teeth, under fixed partial denture (FPD), restored with direct and indirect core, to analyze the type of fracture and to verify its relation to the type of core used. A total of 15 bone blocks taken from the jaw of adult swine, with at least 1th and 3rd molars, that were the abutment teeth for the FPD. The specimens were divided into 3 experimental groups (GR): GR I - (control) dentine core, GR II - cast metallic core in non-precious Cu-Al alloy (NPG, Aalbadent, USA) and GR III - glass fiber post (White post DC, FGM, BRA) and composite resin (Opallis, FGM, BRA). The standardization of coronary core was performed by a matrix of acetate obtained from the preparation of the GR I. The cementation of intraradicular retainers was performed with dual self-adhesive resin cement (RelyX U100, 3MESPE, USA). Infrastructures for FPDs of four elements were waxed, from acetate matrix to standardize the size and shape, casted with non-precious Ni-Cr alloy (Wironia light, Wilcos, USA) and cemented with dual self-adhesive resin cement (RelyX U100, 3MESPE, USA). The bone blocks were embedded in acrylic resin and subjected to compression test by universal testing machine, until fracture. The fractures were classified as repairable (upper or to the bone level) or irreparable (beyond the bone level), after analysis by stereomicroscope and tomography computed. Tests Analysis of variance and Fisher's exact test (p<0.05) were applied for statistical analysis. For the variable fracture strength, there were no statistical differences among the cores tested (p = 0.113), both GR I and the GR III, 100% of the fractures were repairable and GR II, 80% of the fractures were irreparable. There is evidence of association between fracture type and the core type used (p = 0.011). It was possible to conclude that: 1. The type of core used did not influence the fracture strength of the abutment teeth. 2. The type of fracture occurred in the abutments was predominantly repairable, when restored with direct core, and irreparable when restored with indirect core. 3. There is association between core type and the fracture type occurred.
O objetivo deste estudo foi comparar a resistência à fratura de pilares de próteses parciais fixas (PPFS) restaurados com núcleo em dentina, com pino de fibra de vidro e resina composta e com núcleo metálico fundido; analisar o tipo de fratura (reparável ou irreparável) e verificar a sua relação com o tipo de núcleo utilizado. Foram utilizados 15 blocos ósseos extraídos da mandíbula de suínos adultos, com 1º, 2º e 3º molares, que constituíram os pilares para a PPF. Os espécimes foram divididos em 3 grupos (GR) experimentais: GR I - (controle) núcleo em dentina, GR II - núcleo metálico fundido (NMF) em liga não preciosa Cu-Al (NPG, Aalbadent, USA) e GR III - pino de fibra de vidro - PFV (White post DC, FGM, BRA) e resina composta - RC (Opallis, FGM, BRA). A padronização coronária dos núcleos foi realizada através de uma matriz de acetato obtida a partir do preparo do GR I. A cimentação dos retentores intrarradiculares foi realizada com cimento resinoso autoadesivo dual (Relyx U100, 3MESPE, USA). Infraestruturas para PPFs de 4 elementos foram enceradas, a partir de uma matriz de acetato para padronização de dimensão e formato, fundidas em liga não preciosa Ni-Cr (Wironia light, Wilcos, BRA) e cimentadas com cimento resinoso autoadesivo dual (Relyx U100, 3MESPE, USA). Posteriormente, os blocos ósseos foram embutidos em resina acrílica autopolimerizável e submetidos ao ensaio de compressão em máquina de ensaio universal até a fratura. As fraturas foram classificadas como reparáveis (acima ou ao nível da crista óssea) ou irreparáveis (abaixo da crista óssea), após análise por estereomicroscópio e tomografias computadorizadas. Foram aplicados os testes Análise de Variância e Teste Exato de Fisher (p<0,05) para análise estatística. Para a variável resistência à fratura, não foi observada diferença estatística entre os núcleos testados (p=0,113), tanto no GR I quanto no GR III, 100% das fraturas foram reparáveis e no GR II, 80% das fraturas foram irreparáveis. Foi verificada associação entre tipo de fratura e o tipo de núcleo utilizado (p=0,011). Concluiu-se que o tipo de núcleo utilizado não influencia a resistência à fratura dos dentes pilares, entretanto influencia o tipo de fratura gerado.
