Abstract:
Acute gout is a common inflammatory arthritis in the adult population. Epidemiological evidence suggests that the prevalence of gout is steadily on the rise due to longevity, coexisting comorbidities, and iatrogenic causes contributing to hyperuricaemia. Acute gout usually presents as a self-limiting flare of synovitis that occurs due to deposition of monosodium urate crystals. The frequency of flares generally increases over time in patients who continue to have hyperuricaemia and their risk factors for acute gout attacks have not been adequately addressed. Effective treatment of acute gouty

Abstract:
Opioid-induced constipation is a major problem for patients with advanced medical illness despite aggressive use of bowel regimens. Methylnaltrexone is an opioid antagonist with limited ability to cross the blood–brain barrier and thus is thought to induce opioid withdrawal at the level of the gastrointestinal tract without causing central withdrawal and flare of pain. This chapter describes and evaluates a randomized controlled trial of methylnaltrexone versus placebo and an open label extension study for patients with advanced illness and opioid-induced constipation. Primary outcomes are eff

Abstract:
Standard allergy testing relies on identifying the IgE antibody specific for the allergen in question. Two classic methods of doing this are the immediate wheal-and-flare skin prick tests (a small amount of antigen is introduced into the skin and evaluated at 15 minutes for the presence of an immediate wheal-and-flare reaction) and in vitro testing. Allergy testing that does not have a clear scientific basis includes cytotoxic testing, provocation-neutralization testing or treatment, and "yeast allergy" testing. Abstract:
Juvenile polymyositis and dermatomyositis are inflammatory myopathies that affect muscle. Dermatomyositis also affects skin, and can have many extramuscular manifestations. Inflammatory myopathies are uncommon in childhood, with dermatomyositis occurring more than polymyositis. For this reason, published research has concentrated on juvenile dermatomyositis. The spectrum of disease severity ranges from mild cases that can recover completely without treatment, to multisystem inflammation that can be fatal.
