Full textInformation today is a valuable resource for organizations which become more and more dependent on their information systems. Information subject to various threats and the need to be protected in order that organizations can effectively run their business. A systematic information security helps organizations to achieve and maintain a sufficient level of information security. The study aims to investigate how information is managed within organizations in general. A case study has been performed in Folktandvården (the Public Dental Service), Region Jönköping County to investigate how the organization handle information security. In addition, the study aims to evaluate awareness of information security among employees at the business and to present proposals on how to improve handling of personal data. Mixed techniques have been used to gather information. Literature studies in the field of information security has been implemented. The empirical data collected through a questionnaire, interviews and written questions sent by e-mail to managers in Folktandvården. This study uses the standards SS-ISO / IEC 27001:2014 and SS-ISO / IEC 27002:2014 to evaluate the information in Folktandvården, Region Jönköping County and to get a picture of how information is managed within organizations. Organizations can maintain the security of their information by implementing an information security management system (ISMS) that preserves the confidentiality, integrity and availability of information. Information security and ISMS application differs between organizations, which could be affected by the organization's needs and goals, size and structure. Case study results show that Folktandvården, Region Jönköping County implements an active management of information. The organization manages most of the specifications in the standards. However this study proposes to organize more training programs for information security awareness. These programs should be updated regularly in order to continue to be in line with organizational policies and procedures. It is recommended that the organization performs information classification fully in accordance with the model it has. Additionally, it is recommended to develop the planning of continuity for information. The results from the questionnaire show that the employees are aware of how they handle information security incidents and they think that the systems are available for authorized access. Several of the proposals presented by this study have been heeded and will lead to further work in Folktandvården. Organizations' personal information should be protected by applying the rules in accordance with applicable regulations. A responsible person in the organization should provide guidance to employees about their responsibility for the handling of personal data.

Full textIntroduction: Tuberculosis is one of the world's most widespread infectious diseases. One third of the world’s population is expected to carry the bacterium without being ill (latent tuberculosis, LTBI). Nearly ten percent of the carriers develop active tuberculosis sometime during their lifetime. Europe has a comparatively low incidence of tuberculosis and in Sweden the number of cases of tuberculosis has decreased since the mid-40s. In Sweden, the increased number of immigrants in recent years is seen as an explanation for a temporarily increased prevalence of tuberculosis. Early diagnosis and treatment of tuberculosis are key factors in preventing the spread of infection and giving the individual the opportunity to manage their health, which is important from a public health perspective. The county councils/regions have chosen different strategies for whom to be included in screening for tuberculosis in health surveys of immigrants. Aim: The aim of the study was to investigate the prevalence and follow-up of latent tuberculosis among immigrants who have undergone a health examination at health centers in the County of Jönköping. Method: The study was a retrospective observation study based on quantitative approach. Data was collected through journal review and 361 records were included in the analysis. The presence of latent and active tuberculosis was correlated with gender, age and country of origin. Results: The results of the study showed that the prevalence of LTBI in the study population was 9.4%. The results showed that LTBI was more common in men and individuals > 35 years. However, no differences were seen by country of origin. Conclusion: The study is of limited size and thus generalisability. LTBI is relatively common among immigrants. Further studies are needed to identify optimal routines for screening and preventive measures in the immigrant group.

A big problem with the system today is that many non-acute patients are looking for help in the emergency rooms instead of getting proper help at the primary care. This does not benefit the financial state for Swedish hospitals where emergency care is significantly more expensive than primary care. In 2016, Region Jönköping took an initiative to improve the process by redistributing the non-acute patients from the emergency room back to primary care. Purpose: The purpose of the study is to implement a process-based evaluation of the improvement process ER to primary care. By charting their work, we will try to identify the strengths and weaknesses to provide suggestions of improvements in order to achieve the objectives of the project. Method: The research is based on a qualitative single case study focusing on evaluation studies. Empirical data has been collected through semi-structured interviews and documents. Conclusion: The improvement process ER to primary care is divided into three steps; planning, implementation and monitoring. A chosen team for the project use various tools in the work for improvement. In order to develop and improve this process, the project team has to design a standardized approach that is accepted by all the entities involved in it. Recommendations for such an improved process is to introduce multiple communication channels, provide increased support and expand the knowledge of those working in the process. By marketing the competence within the primary care, the distribution of the patients can get more efficient.

Full textBackground and discussion: Public organizations have in recent decades been subjected to change, and this is due to an increased demand for efficiency. The control instrument (management tool) from industrial settings has been taken in response to this efficiency and one of them is Lean, in Healthcare named “Lean Healthcare”. Lean can be seen as an idea that has been translated from the manufacturing industry to healthcare, and the theoretical framework translation model has been used to understand this in this paper. Purpose: The purpose of the study is to describe and explain how different healthcare organizations translate Lean. This is to provide knowledge of what Lean stands for in Healthcare, as well as the difficulties and opportunities that exist. Method: A qualitative case study has been conducted to understand how Lean is translated into Healthcare. Data has been collected through semi-structured interviews at the clinic level, which meant that we had contact with doctors, nurses, physicists, specialist nurses, midwifes and administrative management. We have also taken note of the documents that enabled us to have a broad understanding of how the translation looked which has given us an understanding of the opportunities and difficulties that existed. Conclusions: We have seen how the idea, Lean, has undergone various phases in both of the clinics, where it was translated into something local. We can conclude that Lean in Healthcare is something that stands for efficiency, the process of thinking, collaboration and problem solving, standardization, visualization, leaders investing in their employees, and measurement of quest for survival. Furthermore, we note that Lean in Healthcare creates opportunities as it leads to more effective treatment, better availability of personnel, shorter lead times, improved patient flow, better work, better structure and a better working environment. Difficulties arise as proven by finding balance and alignment, difficulties with professions, difficulties related to the industrial context and the complexity of healthcare where unpredictable events are common.

Full textThe health care system has undergone major changes over the years and is today a central part of our welfare society. The trend is to direct citizens towards the Primary Health Care sector, making the health care system face new challenges. The primary challenges are related to the allocation of resources and guiding priorities for this. Today, health care is increasingly controlled by economic incentives, where the link to compensation is based on, compensation models. How the compensation model is designed varies between different principal stakeholders whom are designing their care choice model after its own priorities of primary health care objectives. The aim of the current study is to analyse whether the different compensation models in primary care supports the goal to work towards and what experience the different operators, politicians, administrative management and profession, have of compensation models ' effects. To study how the compensation model supports business goals in primary care two organizations has been selected, County Council of Jönköping and Blekinge on which a comparative research design has been applied. Data for the theoretical frame of reference is based on published articles, books and other information material from the authorities. Primary data has been gathered from six qualitative, semi-structured interviews. Secondary data for the study consists of data collected from each region/County Councils. To use a compensation model for the allocation of resources in primary health care is considered to be a good instrument in which monetary remuneration justifies. Objectives as availability, equal treatment, high quality care, person-centred care, patient safety, along with cost-effective care can sometimes counteract with each other. Good cost control is often a priority when governing health care operations, monitoring and control the business needs much administration, thus undermining the objective of cost effective care. Compensation model is effective way to control primary health care since monetary compensation is involved but the model is not used primarily to steer towards the target but for cost control. The experience of the effects of the compensation model is that they sometimes act counterproductive towards goal for the health care system and you might receiving unwanted effects as manipulation of data and focus what you can do to generate more compensation. A vast amount of administration is needed to follow up compensation models.

Full textHealthcare associated infections (HAIs) are one of the greatest threats to patient safety. Hand hygiene is the most important component in the prevention efforts. Measurements to standard precautions are important for adherence to the basic rules of hygiene and clothing. An improvement work focused on hygiene observers in Region Jönköping County, was implemented in which three hall gatherings was replaced by 17 meetings with fewer participants. The aim of the improvement work is to increase the number of measuring devices. The purpose of the study of the improvement work is to describe the hygiene observers experiences concerning deficiencies in the basic hygiene practices and the new meeting structure and to identify factors that influence the extent to which measurements will be carried out. Method: PDSA, Nolan's improvement model, survey, qualitative data analysis in the form of focus group interviews. The result of the improvement work indicates more units performing measurements. Results of the study of the improvement work shows lack of management support and inadequate knowledge among staff about how infections spread. Other factors are lack of disinfection before patient care, overuse of gloves and poor adherence to dress rules. Conclusions: Management support is crucial for adherence to measurement. Improvements in number of HAI, costs and patients ' suffering increases as hygiene measurements are carried out.
