Gluckman, Sir Peter, Mark Hanson, Chong Yap Seng, and Anne Bardsley. Copper in pregnancy and breastfeeding. Oxford University Press, 2015. http://dx.doi.org/10.1093/med/9780198722700.003.0024.

Full text
Abstract:
Copper is an essential trace mineral with a central role in oxidation#amp;#x2013;reduction reactions. It is involved in connective tissue formation and disulphide bonding of keratin. Clinical deficiency for copper is uncommon but has been increasing in prevalence, resulting from malnutrition or lack of trace metals in the food source, even in Western diets. Secondary copper deficiency can be induced by excess intake of zinc or iron and is sometimes seen in individuals taking supplements of these minerals. Copper requirements are increased in pregnancy, and suboptimal supply may have adverse effects on developing tissues and organ systems including the lung, skin, bones, and immune system. Despite the increased need, supplementation is generally not recommended in pregnancy. An increased intake of foods high in copper, such as nuts, and a proportionate lowering of low-copper foods (e.g. fats/oils) may be advisable in women with potentially marginal copper status.

Gluckman, Sir Peter, Mark Hanson, Chong Yap Seng, and Anne Bardsley. Vitamin B12 (cobalamin) in pregnancy and breastfeeding. Oxford University Press, 2015. http://dx.doi.org/10.1093/med/9780198722700.003.0013.

Full text
Abstract:
Vitamin B12 is required for the synthesis of fatty acids and myelin and so is crucial for normal neurological function and maintenance of the CNS. In conjunction with folate, it is involved in red blood cell formation and DNA synthesis, and in embryogenesis, it is important for proper neural tube formation and brain development. Maternal intake during pregnancy is important, as only newly absorbed vitamin B12, and not that stored in the maternal liver, is concentrated in the placenta. Despite the active transfer during pregnancy, the vitamin B12 content in the newborn is low, and the infant is dependent on breast milk for ongoing needs. Pregnant and lactating women should ensure that their diet contains sufficient (animal) sources of vitamin B12; those consuming vegan or strict vegetarian diets should either take vitamin B12 supplements or seek foods that have been fortified with vitamin B12.

Abstract:
The aetiopathogenesis of gout is initiated by urate overproduction and uric acid under-excretion, leading to hyperuricaemia. Foods such as seafood, red meat, beer, and sugar-sweetened beverages contribute to overproduction. Under-excretion is mediated by renal and gut uric acid transporters such as SLC2A9, ABCG2, and URAT1. In hyperurcaemia, there is formation of monosodium urate (MSU) crystals in joints, with acute gouty arthritis mediated by the innate immune system occurring in response to these crystals. Factors such as urate concentration, proteins present in synovial fluid, temperature, and pH control crystal nucleation and growth. Activation of the inflammasome by MSU crystals and production of interleukin-1ß‎ is central to acute gouty arthritis. Advanced gout occurs when there is persistent gouty arthritis and tophus with the tophus being an organized immune tissue response to MSU crystals that involves both innate and adaptive immune cells. Progression through the gout checkpoints (hyperuricaemia, MSU crystal formation, and immune response) is governed by inherited genetic variants, lifetime environmental exposures, and their interaction.

Takemoto, Hiroyuki, Yoshi Kawamoto, and Takeshi Furuichi. The formation of Congo River and the origin of bonobos: A new hypothesis. Oxford University Press, 2018. http://dx.doi.org/10.1093/oso/9780198728511.003.0016.

Full text
Abstract:
The Congo River functions as a strong geographical barrier for many terrestrial mammals in the Congo Basin, separating forest habitat into right and left banks of the river. However, there has been little discussion on the biogeography of the Congo Basin because the history of the river has been obscured. Based on the recent information of the sea-floor sediments near the mouth of the river and the geophysical survey on the continent, this chapter proposes a plausible hypothesis on the Congo River formation and presents a consequent hypothesis on the divergence of bonobos (Pan paniscus) from other Pan populations. The present hypothesis is also helpful for understanding the distribution of other primates and other mammals in the basin. Furthermore, this hypothesis suggests that all hominid clades, including human, chimpanzee and gorilla, except bonobo, evolved in the area north or east of the Congo River.

Abstract:
The 3,144–amino acid huntingtin protein (HTT) folds in water into a structure consisting of compact, organized domains interspersed with intrinsically disordered protein (IDP) elements. The IDPs function as sites of post-translational modifications and proteolysis as well as in targeting, binding, and aggregation. Although the dominant structural motif of HTT is the α‎-helix–rich HEAT repeat, the expanded polyglutamine (polyQ) toxicity responsible for Huntington’s disease is most likely played out within intrinsically disordered HTT exon 1–like fragments consisting of the 16– to 17–amino acid N-terminal HTTNT segment, the polyQ segment, and a proline-rich segment. The physical behavior of HTT exon 1 fragments is dominated by interactive, polyQ repeat length–dependent structural transitions responsible for membrane and protein–protein interactions and the formation of tetramers, higher oligomers, amyloid fibrils, and inclusions. Understanding the basis of this solution behavior may be the key to disease mechanisms and molecular therapeutic strategies.

Daudon, Michel, and Paul Jungers. Uric acid stones. Edited by Mark E. De Broe. Oxford University Press, 2018. http://dx.doi.org/10.1093/med/9780199592548.003.0202_update_001.

Full text
Abstract:
Uric acid (UA) stones are typically red-orange and often appear as sand/ gravel though they may be large. They are totally radiolucent. They account for about 10% of all kidney stones in most countries, and up to 20% in some populations. It is twice as frequent in males, prevalence increases with age, and it is two to three times higher in patients with type 2 diabetes or with features of the metabolic syndrome. Factors that induce the formation of UA stones are a low urine volume, hyperuricosuria, and, more importantly, a permanently low urine pH (< 5). Indeed, below its pKa of 5.35 at 37°C, UA is in non-dissociated form, whose solubility is at best 100 mg/L, whereas urinary UA excretion normally exceeds 600 mg/day and may exceed 1g/day.Because UA solubility increases up to approximately 500 mg/L at urine pH > 6, urine alkalinization, with a target pH of 6.5–7, is the cornerstone of medical treatment. This most often allows dissolution of existing stones and prevention of recurrent stone formation so that urological intervention is infrequently needed. The preferred agent for alkalinization is potassium citrate (30–60 mEq/day in divided doses), because potassium urate is twice more soluble than sodium urate. However, in patients with poor gastric tolerance to potassium citrate or contraindication to potassium supplements, sodium bicarbonate is an acceptable alternative. Limitation of animal proteins, purine-rich foods (including beer), alcoholic drinks and acidified beverages (sodas) are useful measures, together with large fluid intake (> 2–2.5 L/day). Allopurinol may be indicated in cases of symptomatic hyperuricaemia. Regular observance of alkalinisation, with periodic controls of urine pH by the patient, is needed to prevent the rapid formation of UA stones. Patients affected by UANL, especially if overweight, should be evaluated for type 2 diabetes or glucose intolerance and managed accordingly.
