Abstract:
This article discusses the growing body of evidence on the importance of health for looked after children and young people and evaluates a project, Hearty Lives (Liverpool), designed to produce positive results. Unhealthy weight and lifestyles are significant issues for young people in care, whether living at home with parents under the supervision of social services, with foster carers or in residential homes, as there is a close relationship between food, nutrition and family connectedness. Following the principles of Appreciative Inquiry, the evaluation uses a case study approach to explore the learning and experiences of those involved in the intervention. The learning gained provides useful insights for practitioners and organisations who are interested in developing similar projects elsewhere.

Abstract:
In the UK, the last 15 years have seen a profound change in the way that lesbians and gay men have been socially and politically located and acknowledged. This is evidenced by recent legislative changes that have given protection to lesbians and gay men and placed a duty on public bodies to provide equitable services. For a number of years lesbians and gay men have been specifically targeted, recruited and utilised as adopters of children in public care. With these changes has come the realisation that a number of complexities in adoption practice have been insufficiently addressed. Brown and Cocker (2008) have argued that in the assessment of prospective lesbian and gay adopters, struggling with complexities is crucial for safe and effective assessment of suitability to be realised. This appreciation of the complexity of practice has been articulated in the Independent Inquiry into the Circumstances of Child Sexual Abuse by Two Foster Carers in Wakefield (Parrott et al, 2007). The report noted that the ‘homosexuality’ of the foster carers became the primary focus of social work anxiety. This happened at the expense of holistic considerations of both the carers' potential and their actual foster care practice. Although these findings relate to foster care, they are transferable to the adoption field. The authors of the Inquiry emphasised the importance of ‘discrimination’ in practice, arguing that ‘discrimination’ was essential for discerning and analytical assessment, support and supervision. This article by Christine Cocker and Helen Cosis Brown offers a conceptual framework as well as practice tools to facilitate such discriminatory, effective, discerning and comprehensive assessments of lesbian and gay prospective adopters. The paramountcy of the child's welfare has to remain central to developments in adoption practice. This article maintains this centrality.

Abstract:
This article reports findings from an independent audit and evaluation of an innovative children's services programme (CSP) funded 2009-2011 by twelve Local Authorities under the auspices of the North-East of England's Improvement & Efficiency Partnership (NEIEP) to manage improvements and efficiencies across the children's services sector focusing upon messages for social workers. The overall purpose of the CSP was to make demonstrable progress in tackling the challenges of child poverty by delivering high quality support to the growing number of children with social care/health needs through regional improvement. The key objectives of the CSP included supporting workforce reform and integrated working; development of personalised services; family support to reduce the need for residential care; and provision of tools to aid commissioners with needs analysis. The audit identified key outputs, for example, improvements to best practice on Whole Family approaches, safeguarding and leadership training evolved through a skills framework; and included a regional model of social work supervision training along with a provision of options to increase the range and quality of foster care placements. The evaluation considered actions arising from the above findings, including demonstrated improvements to inter-disciplinary working and pooling resources to produce better outcomes for families; setting up a data-base to improve the balance between fostering, residential care and family support; and creating opportunities for social workers to explore the practical implementation of using personal budgets.

Abstract:
Objective Patient care delivered by residents is an educational opportunity to foster autonomy. These services, however, may not be billed without direct faculty supervision. This quality improvement project analyzes descriptive data, procedures, and billing from an academic otolaryngology–head and neck surgery department’s inpatient consult service. Methods This prospective cohort study collected descriptive and billing data on all consults over 30 consecutive days. Data collected described bedside procedures and operative interventions. Encounters were Current Procedural Terminology coded and equivalent work relative value unit (wRVU) calculated. Codes submitted for billing were reviewed to identify opportunities for inpatient billing improvement. Results Ninety-eight new consults were included, and 105 bedside procedures were performed. Flexible laryngoscopy (n = 27), I&D (incision and drainage; n = 11), and suturing (n = 11) were the most performed bedside procedures. Operative intervention was required in 15 encounters. The inpatient consult team provided the equivalent of 391.39 wRVU. Seventy-three percent of operative compared to 3% of bedside procedures were submitted for billing. Discussion The productivity of our team approximated 61.8% of a full-time general otolaryngologist but with decreased billing submissions. Balance between resident autonomy, education, quality patient care, and the ability to capture service revenue is complex. Strategies other institutions have used to capture missed billing opportunities have included a hospitalist model, mid-level providers, and resident billing. Implications for Practice This study characterizes an academic inpatient consult service. Results describe a case for improving the structure of the inpatient consult service, caution that studies collecting data via department billing may underrepresent services, and suggest alternative service structures to overcome identified billing limitations.

Abstract:
The article introduces the significance of social worker's assistance to prospective foster parents: a case of an X Lithuanian town. The best place for the child development and improvement is the family. For some reasons not all the children are raised in families. Some children have no possibility to live with their biological families, therefore according to the established legislation, they live in foster homes under the supervision of foster parents or are adopted. Children adoption in our country is regulated by different acts of legislation. The activity of the ombudsperson for children’s rights is governed by the Law on the Ombudsman for Children Rights Protection of the Republic of Lithuania (2000), Civil Code of the Republic of Lithuania, Regulations of organization and management of children guardianship (2002), etc. Research <strong>aim</strong> is to analyze the social service worker's assistance to the prospective foster parents. The following objectives to reach our <strong>goal</strong> are presented: to analyze theoretical conception of adoption procedure and enforcement; to discuss the system of the social service workers’ assistance to prospective foster parents; to investigate foster parents' approach towards the social service worker's assistance and to reveal the main problems. <strong>Research methods</strong>: analysis of the scientific literature – scientific analysis of adoption issues. The stages of adoption procedures and the requirements overviewed and analysed are provided; document analysis: documents, acts of legislation, laws and regulations of child adoption regulations are analysed and overviewed; a qualitative study: semi-structured interview, analyzing the foster parents' approach towards the social service workers' assistance; problems and probable solutions are identified. The research results are obtained by content analysis. <strong>The article presents the following generalizations:</strong> 1) adoption is considered as a social process when specific rights and responsibilities are provided to foster parents in order to guarantee the child's safety. In case of inter-country adoption, the adoption opportunities are estimated, the child is prepared for the adoption and the family for the child is selected. At that period foster parents face internal and external factors. Seeking to grant the children rights and smooth adoption process, professional social workers should coordinate it; 2) institution of the Ombudsman for Children Rights of the Republic of Lithuania and State Child Rights Protection and Adoption Service under the Ministry of Social Security and Labour are also engaged in the adoption and foster care; 3) the research results reveal that the biggest problems for prospective foster parents are extensively long adoption period, lack of social service workers' assistance and poor knowledge on the available assistance.

Abstract:
Background: We study the role of individual and team-level motivation in explaining large-scale primary care performance improvements in El Salvador, one of the top-performing countries in the Salud Mesoamerica Initiative. Methods: Case study with outlier sampling of high-performing, community health teams in El Salvador. Design includes scoping review of literature, document review, non-participant observation, and qualitative analysis of in-depth interviews following a realist case study protocol. Results: The interplay between program interventions and organizational, community and policy contexts trigger multi-level motivational mechanisms that operate in complex, dynamic fashion. Interventions like performance measurement and team-based, in-kind incentives foster motivation among individual members of high-performing teams, which may be moderated by working conditions, supervision practices, and by the stress exerted by the interventions themselves. Individuals report a strong sense of public service motivation and an overarching sense of commitment to the community they serve. At the interpersonal level, the linkage between performance measurement and in-kind incentives triggers a sense of collective efficacy and increases team motivation and improvement behaviors. The convening of learning forums and performance dialogue increases the stakes for high-performing teams, helps them make sense of performance data, and leads to performance information utilization for healthcare improvements. Closeness to communities creates strong emotional linkages among team members that further increases collective efficacy and social identity. Such changes in individuals, team, and organizational behaviors can contribute to improved delivery of primary care services and explain the gains in performance demonstrated by the program. Conclusions: This case suggests that primary care systems that rely on multi-disciplinary teams for the provision of care can benefit from performance measurement and management interventions that leverage individual and team-level motivation. Realist evaluation can help prioritize policy-relevant research and enhance the design and evaluation of large-scale performance reforms in primary care systems in low- and middle-income settings.

Abstract:
This paper is an account of the principles and practices of treatment offered at the Cassel Hospital, London, with a particular focus on the Inpatient Families Unit. The Cassel Hospital is an internationally renowned therapeutic community, the operation of which is based on psychoanalytic principles and which has operated within the British National Health Service for nearly 50 years. An account of the historical development of the hospital is given as well as a description of its structure and function. The following three innovative structures are elaborated: a complex network within which patients can develop, Cassel-style nursing care, and nurse-therapist supervision. Theoretical underpinnings are outlined, which together with two case studies facilitate an appreciation of the capacity of the therapeutic network to foster the successful treatment of a range of severely disordered individuals and families. Such treatment may approach a level perhaps otherwise unattainable and which is widely applicable in the public hospital and clinic settings in Australia.

Abstract:
Kata Kunci: adopsi anak, pekerjaan sosial ABSTRACT Adoption is one of the efforts made by married couples to be able to have the presence of a child in their family. In Indonesia, the adoption procedures are clearly regulated in several laws and government regulations. But in its implementation, there are still various problems of adoption that occur, including a lack of knowledge in community regarding the legal adoption procedures, the absence of valid data of number adoptions in Indonesia, and the ongoing dispute between foster parents and biological parents due to adoption procedures carried out in private adoption. The method used in writing this article is a literature study method. The results of the study are the need for efforts to increase information on adoption to the community through the ease of accessibility of adoption services in each local authority. This can be achieved through good integration between the government, children's social institutions, courts, and social workers. In addition, the adoption services provided must be ensured to reach the stage of supervision, assistance, and strengthening of the care of prospective adoptive parents to ensure the fulfillment of the needs and welfare of children in the future. Abstract:
Every year, thousands of children end in foster care. It is estimated that approximately 24,000 foster youths age-out of the system each year. The improvement of outcomes of foster youths when they age-out of the system has been a concern among government agencies, policy makers, and advocates. Although research in the foster care area has studied the outcomes of foster youths when they age-out of the system, the role of the foster parents in improving these outcomes, especially in the area of career development, has not received the same attention. The purpose of this study was to explore the perception of foster parents of youths with and without disabilities about their foster youth's career self-efficacy beliefs. A total of 157 foster parents registered in the Iowa Foster and Adoptive Parents Association (IFAPA) and Nebraska Foster and Adoptive Parent Association (NFAPA) email databases responded to an online survey. The survey included a demographic questionnaire which presented questions about the extent of their role as foster parents and the Career Decision Making Self-Efficacy Scale - Short Form (CDMSE-SF) that measured the foster parents' perception of the career self-efficacy of their foster youth. After conducting a MANOVA and ANOVA analyses, a highly perceived self-efficacy belief was found among the foster parents in general. However, when the group of foster parents who reported having a youth with disability and those who do not have a youth with disability were compared, the perceived career self-efficacy beliefs were higher among the foster parents of youths without disabilities. Additionally, the foster parents identified services and/or training they understand are important for them and the foster youth such as more orientation about disabilities, the parent's role in Individual Educational Plan (IPE) meetings, and the opportunity for the foster youth of having employment experience before they leave the foster care system. Considering the important role of the family in the career development of youths and the development of expectations, these findings are important not only to the rehabilitation counseling professionals, but to many individuals in the helping professions, educators, researchers, and policy makers.

Abstract:
This study examined former foster youth’s perceptions of the skills and services they received while they were in the foster care system. More specifically the efficacy of these services, and what services and skills they believed would have been helpful to assist them in their transition into independent living. A qualitative design was used, and semi-structured face-to-face and telephone interviews were conducted with ten former foster youth who aged out of foster care at age eighteen through twenty-one. Two males and eight females from diverse backgrounds participated in this study. Participants were recruited through availability and snowball sampling at community agencies, college campuses, and the community. This study found that participants experienced a difficult time transitioning from foster care to independent living, felt unprepared after leaving foster care, needing additional services, and expressed wanting to have received more in depth financial management skills. This study recommends providing additional independent living skills, social support, and participation in the Independent Living Program as part of foster youth’s case plan when preparing to transition out of foster care into independent living.

Abstract:
This study examined the outcomes for young adults in Extended Foster Care. Extended Foster Care is a relatively new program within the Child Welfare system, which provides services for former foster youth between the ages of 18 and 21 to become more successful, independent young adults. Before the implementation of Extended Foster Care in 2012, most of the foster children were left to fend for themselves after they turned 18, which resulted in negative outcomes. Extended Foster Care is a seemingly helpful program; however, due to its new existence there has been very limited research regarding the outcomes of these young adults in Extended Foster Care. This study used secondary data from the San Bernardino County Legislative, Research and Quality Support Services Unit. After data retrieval, independent t-tests and chi-squared tests were conducted using SPSS version 21. The independent variable of days spent in Extended Foster Care was compared to the following dependent variables: disability status, education status, housing status, and employment status and its barriers. The results of this study showed that the longer a young adults stays in Extended Foster Care, the higher their likelihood is of positive housing, employment, and education status. This study suggests it is critical to continue to provide Extended Foster Care services and further research needs to be conducted to ensure program quality and to continue to improve the services to this vulnerable, young adult population.

Abstract:
AbstractThe wide implementation of patient safety improvement efforts continues to face many barriers including insufficient involvement of all stakeholders in healthcare, lack of individual and organizational learning when medical errors occur and scarce investments in patient safety. The promotion of systems-based approaches offers methods and tools to improve the safety of care. A multidisciplinary perspective must include the involvement of patients and citizens as fundamental contributors to the design, implementation, and delivery of health services. The patient journey is a challenging example of using a systems approach. The inclusion of the patient’s viewpoint and experience about their health journey throughout the time of care and across all the care settings represents a key factor in improving patient safety. Patient engagement ensures that the design of healthcare services are aligned with the values, the preferences, and needs of the patient community and integrates the real-life experience and the skills of the people to enhance patient safety in the patient journey. The utmost priority to implement patient engagement is the training of patients. Therefore, training for both patients/families/advocates and health professionals is the foundation on which to build active engagement of patients and consequently an effective and efficient patient journey. The chapter offers examples of successful training courses designed to foster strategic alliances among healthcare professionals and researchers with patients and their organizations. Training of patients constitutes the first step to develop shared knowledge, co-produced projects, and the achievement of active multilevel participation of patients for the implementation of patient safety in the patient journey.

Abstract:
AbstractIn 2020, the UN launched the “Decade of Action” to achieve the Sustainable Development Goals (SDGs) by the year 2030. As the SDGs are interdependent, intersectional and interdisciplinary, so must be their solutions. This chapter argues that the best way to identify, develop, and scale solutions of such quality is (digital) entrepreneurship, building on the principles of open innovation, cutting-edge technologies, and social business. The COVID-19 pandemic in early 2020 in particular serves as a stark reminder of the interconnected nature of the SDGs and the challenges we face in achieving them. In this article, we explore the third SDG (SDG-3), “Good health and well-being”. We show the potential for digital entrepreneurship to foster the rise of new forms of digital health care and to accelerate the digitalization of the healthcare sector. Due to both perceived and real issues of regulatory compliance, user experience, and long investment/equipment use cycles, SDG-3 has been one of the slowest to adopt innovative solutions by far. We discuss specific areas, such as blended reality or quantum computing, for emerging and future digital health applications. In this chapter, we provide: the “memoreBox” of social start-up RetroBrain R&D, a special edition of gamelab.berlin’s app “Singleton”, and D-Wave’s free access to its cloud quantum computing services. All these examples of digital entrepreneurship utilize in whole or in part a combination of open innovation, future and emerging technologies, and social business, thus supporting our rationale. The article closes with recommendations for different stakeholders of entrepreneurial ecosystems, demonstrating both the necessity and the potential of digital entrepreneurship for the SDGs and the “Decade of Action”.

The mission of the emergency medical services is to promote and support a system that provides timely, professional and state-of-the art emergency medical care, including ambulance services, to anyone who is victim of a sudden injury or illness, at any time and any location. A medical emergency has five different phases, namely: population awareness and behaviour, occurrence of the problem and its detection, alarming of trained responders and help rendered by bystanders and trained pre-hospital providers, transport to the nearest or most appropriate hospital, and, if necessary, admission or transfer to a tertiary care centre which provides a high degree of subspecialty expertise. In order to meet these goals, emergency medical services must work aligned with local, state officials; with fire and rescue departments; with other ambulance providers, hospitals, and other agencies to foster a high performance network. The term emergency medical service evolved to reflect a change from a straightforward system of ambulances providing nothing but transportation, to a complex network in which high-quality medical care is given from the moment the call is received, on-scene with the patient and during transportation. Medical supervision and/or participation of emergency medicine physicians (EP) in the emergency medical service systems contributes to the quality of medical care. This emergency medical services network must be capable to respond instantly and to maintain efficacy around the clock, with well-trained, well-equipped personnel linked through a strong communication system. Research plays a pivotal role in defining necessary resources and in continuously improving the delivery of high-quality care. This chapter gives an overview of the different aspects of emergency medical services and calls for high quality research in pre-hospital emergency care in a true partnership between cardiologists and emergency physicians.

Abstract:
Spread across Brazil and attaining an unparalleled political force, therapeutic communities are as inescapable in the debate on drug policy as they are complex to define. Although they are not a Brazilian creation, they have been operating in that country for decades, and their dissemination intensified in the 1990s. In 2011, they were officially incorporated into Brazil's Psychosocial Care Network (Rede de Atenção Psicossocial, or RAPS). Since then, therapeutic communities have been at the center of public debates about their regulation; about how they should—or even if they should—be a part of the healthcare system; about the level of supervision to which they should be submitted; about their sources of funding, particularly whether or not they should have access to public funding; and, most importantly, about the quality of the services they offer and the many reports of rights violation that have been made public. However, a well-informed public debate can only flourish if the available information is based on sound evidence. The SSRC’s Drugs, Security and Democracy Program is concerned with the policy relevance of the research projects it supports, and the debate around therapeutic communities in Brazil points to a clear need for impartial research that addresses different cross-cutting aspects of this topic in its various dimensions: legal, regulatory, health, and observance of human rights, among others. It is in this context that we publish this working paper series on therapeutic communities in Brazil. The eight articles that compose this series offer a multidisciplinary view of the topic, expanding and deepening the existing literature and offering powerful contributions to a substantive analysis of therapeutic communities as instruments of public policy. Although they can be read separately, it is as a whole that the strength of the eight articles that make up this series becomes more evident. Even though they offer different perspectives, they are complementary works in—and already essential for—delineating and understanding the phenomenon of therapeutic communities in Brazil.

Perceptions of community pharmacists, patent and proprietary medicine vendors, and their clients regarding quality of family planning services: The IntegratE Project. Population Council, 2021. http://dx.doi.org/10.31899/rh17.1016.

Full text
Abstract:
The IntegratE Project is a four-year initiative (2017–21) implemented by the Population Council and partners that seeks to increase access to contraceptive methods by involving the private sector (community pharmacists [CPs] and patent and proprietary medicine vendors [PPMVs]) in family planning (FP) service delivery in Lagos and Kaduna States, Nigeria. The project aims to establish a regulatory system with the Pharmacists Council of Nigeria to ensure that CPs and PPMVs provide quality FP services, comply with FP regulations, and report service statistics to the Health Information Management System (HMIS). To achieve this, the project is implementing: a pilot three-tiered accreditation system for PPMVs; a supervisory model to ensure standard drug-stocking practices; building the capacity of CPs and PPMVs to provide a wider range of FP services and data report to the HMIS. This brief focuses on quality of care received by women voluntarily seeking FP services from CPs and PPMVs. CPs and PPMVs and their clients appear to be satisfied with the FP services offered by CPs and PPMVs; on-going learning opportunities, and a supportive supervision system that is properly coordinated should be sufficient to maintain the quality of services offered by CPs and PPMVs.
