Abstract:
Patients with dissociative (nonepileptic) seizures (DS) commonly, although not always, present with comorbid somatic and psychological symptoms. This chapter reviews the epidemiology, diagnosis, and treatment of other functional somatic symptoms and disorders in relation to DS with a particular emphasis on other functional neurological symptoms such as movement disorder and sensory disturbance. Clinical assessment is presented as the start of treatment in patients with multiple functional symptoms including DS. The chapter discusses the importance of making a diagnosis of functional disorders, especially those in neurological settings, using positive evidence from the physical examination, such as Hoover’s sign for limb weakness or the tremor entrainment test for tremor. Practical methods of explaining these diagnoses to patients are reviewed, emphasizing the importance of transparency, educational materials, and presenting a model that focuses on mechanism more than etiology. The chapter concludes with a review of the evidence for treatment of a range of functional disorders often comorbid with DS.

Abstract:
This chapter summarizes the early postoperative and long-term complications of biliopancreatic diversion with duodenal switch (BPD-DS) procedures. Perioperative mortality for BPD-DS is currently around 0.1%, with an early complication rate of 7%. The standard follow-up required for the prevention of nutritional deficiencies as well as the management of short- and long-term complications specific to metabolic surgeries are also discussed. The data and clinical views expressed in this chapter are based to a large extent on the authors’ experience with 5,000 BPD-DS surgeries in our institution over the last 25 years, and which have been reported extensively in the literature. In our experience, severe protein malnutrition occurred in 10% patients, of whom 5% required readmission and 1.5% a surgical revision to correct the problem.

Abstract:
This chapter reviews the archetypical chromosome disorder, namely Down syndrome (DS; trisomy 21), and the various different chromosomal forms that may be the basis of it: standard trisomy 21, translocation trisomy, both de novo and inherited, and other rare forms. The concept of dosage imbalance as the basis of the pathogenesis is reviewed, and the “DS critical region” on chromosome 21 is examined. Reproductive risks associated with each of these DS types are discussed. The chapter considers the other full autosomal trisomies, T13 and T18, and also (mosaic) T9. Triploidy, as the basis of hydatidiform mole, is reviewed. Also reviewed are the influence of parental, mostly maternal, age, in the genesis of these aneuploidies, and the effect of secular change on these observations. Tables provide precise age-related risk figures for recurrence risk of T21 and more general figures for other trisomies.

Abstract:
By tracing historically the evolution of Korean state-led capitalism and comparing it with other economies, this book criticizes prevalent theories including neoliberalism, the developmental state, and institutionalism, while proposing a theoretical alternative focusing on endogenous changes and institutional adaptability through elite competition within the state. Unlike the arguments of the neoliberals, the state can still play an active role in reconstituting the national economy in globalization. The Korean state successfully fosters economic growth by nurturing industrial commons even in globalization, rather than change toward a neoliberal free market system. In order to better account for sustainable economic growth over a long time, this book emphasizes institutional adaptability through elite competition, rather than offering neoliberal celebrations of the free market and the statist emphasis on the stringent Weberian state. The Korean economy, as well as the East Asian developmental state (DS) economies, could have sustainable development over a long period, not because of an apparent and standardized growth formula, or because of some institutional elements of a stringent Weberian state, but because they have adjusted their methods and strategies of development through competition among elites inside and outside the state, as new challenges, never met with an apparent solution, have continuously emerged. In order to better account for the evolution of state-led developmentalism in Korea, as well as in other countries, this book proposes changes by competition among elites within as well as outside the state, which causes changes in developmentalism and more flexible adjustments in new contexts.

Abstract:
Systemic lupus erythematosus (SLE) is a systemic autoimmune disease with various clinical manifestations. The hallmark of SLE is the presence of antibodies against nuclear constituents, such as double-stranded (ds)DNA, histones, and nucleosomes. Local deposition of antinuclear antibodies in complex with nuclear autoantigens induces serious inflammatory conditions that can affect several tissues and organs, including the kidney.The levels of antinucleosome and anti-dsDNA antibodies seem to correlate with glomerulonephritis and these autoantibodies can often be detected years before the patient is diagnosed with SLE. Apoptotic debris is present in the extracellular matrix and circulation of patients with SLE due to an aberrant process of apoptosis and/or insufficient clearance of apoptotic cells and apoptotic debris. The non-cleared apoptotic debris in patients with SLE may lead to activation of both the innate (myeloid and plasmacytoid dendritic cells) and adaptive (T and B cells) immune system. In addition to the activation by apoptotic debris and immune complexes, the immune system in SLE may be deregulated at the level of (a) presentation of self-peptides by antigen-presenting cells, (b) selection processes for both B and T cells, and (c) regulatory processes of B- and T-cell responses. Lupus nephritis may be classified in different classes based on histological findings in renal biopsies. The chromatin-containing immune complexes deposit in the capillary filter, most likely due to the interaction of chromatin with the polysaccharide heparan sulphate. A decreased renal expression of the endonuclease DNaseI further contributes to the glomerular persistence of chromatin and the development of glomerulonephritis.Current treatment of lupus nephritis is not specific and aims to reduce the inflammatory response with general immunosuppressive therapies. However, research has revealed novel potential therapeutic candidates at the level of dendritic cells, B cells, and T cells.
