Abstract:
The article discusses the comparatively young form of written Romani literary self-expression as an example of “minor literature” in Deleuze and Guattari’s sense.[1] The focus here is on producing a classifying survey of the literary production of Romani writers in France and Spain, with the article outlining the different aesthetic fields and literary forms evident in French and Spanish Romani literature. The comparative approach reveals thatdespite regional and national differences, these minor literatures demonstrate several aesthetic similarities typical of Romani literature that could ultimately come to define the transnational, cross-border characteristics of Romani literature. Furthermore, I show that there are literary tendencies in contemporary Romani literatures that go beyond the usual forms of establishing literary self-expression in diasporic cultural productions or aesthetic appropriation of major society’s literary traditions, so that Romani literatures in French and Spanish should, I argue, also be seen as part of world literature.
 1 It is important to emphasize that the potentially offending implications of the evaluative use of the term “minor” is by no means hinted at in Deleuze and Guattari: The French “literature mineure” does not indicate lower aesthetic qualities or literary inferiority to majority literature but rather describes a literature produced by writers not (exclusively) belonging to the nation-state in which they live. At the same time, it should be mentioned that the term “small literature,” in contrast to minor literatures, means literary expressions from small nations or/and in small languages like, for example, in Bulgarian, Estonian, or Luxembourgish (cf., Glesener 2012).

Abstract:
Comparative literature has always pursued literary studies in a transnational framework. But for much of its history it has been a “modest intellectual enterprise, fundamentally limited to Western Europe, and mostly revolving around the river Rhine (German philologists working on French literature). Not much more,” as Franco Moretti pithily sums it up (54). The rise of postcolonial theory in the wake of Edward Said's and Gayatri Spivak's influential work vastly expanded comparatist horizons, as did the attention to minority literatures that spread outward from the study of American literature and culture in the 1990s. In 1993 Charles Bernheimer's report to the American Comparative Literature Association, “Comparative Literature at the Turn of the Century,” criticized the elitist and exclusionary tenor of earlier reports on the state of the discipline by Harry Levin (1965) and Tom Greene (1975). Instead, it emphasized “tendencies in literary studies, toward a multicultural, global, and interdisciplinary curriculum” and called for an expansion from comparative literature's traditional focus on a mostly western European and North American canon of works to a truly global conception of Goethean Weltliteratur, for inclusion of previously marginalized minority literatures from around the world, and for connections to media studies, other humanities disciplines, and the social sciences (47).

Abstract:
The paper presents a review of the main issues and methods of studying modern French literature and comparative studies.
 The authors outline the diferences between European approaches, now taken with focus rather on all-European common principles than cultural distinctions, and American tendencies that reflect the priority of feminist and post-colonial methods of comparative studies. Attention is paid to the French peculiarities concerning the replacement of the term ‘influence’ by ‘intertextuality’, and to the role of intermedial and interdisciplinary comparative studies.
 Among the outlined concepts and issues are research ethics in comparative studies; non-essential writers and genres (F. Lavokat); relation of comparative studies to the concepts of European and world literature (A. Tomiche); the role and place of comparative studies in literature and culture (F. Toudoire-Surlapierre), accuracy and universality of defining the discipline (B. Franco), the study of links between literature and art (G. Steiner).
 Attention is also paid to the discussions on the concept of ‘world literature’ (in particular to the views of P. Kazanova) that concern the term ‘world literature’ as it is interpreted by American researchers and ‘European literature’ used by French ones. Other issues are the concept of ‘cultural transfer’; the content of hermeneutic practice in comparison; the role of analysis and ‘defamiliarization’ (introduced by V. Shklovsky); comparison as an object of criticism, a tool of analytics, and methodological necessity; the transversality as the coexistence of diferent comparative methods. The comparative approach has been shown as ontological and culturological vision, a special method of research with a basis in comparison and opposition of the interconnected systems covering translation studies, mythology, imagology, geocriticism, post-colonial and gender studies, research of cultural transfer specified as multicomparativism.

Abstract:
This article aims to examine the Polish literary reception of the French Revolution during the period of Romanticism. Its main focus is on how Polish writers displaced their more immediate experiences of revolutionary events onto a backdrop of ‘ancient revolutions’, in which revolution was described indirectly by drawing on classical traditions, particularly the history of ancient Greeks and Romans. As this classical tradition was mediated by key works of German and French thinkers, this European context is crucial for understanding the literary strategies adopted by Polish authors. Three main approaches are visible in the Polish reception, and I will illustrate them using the works of Zygmunt Krasiński (1812–1859), Juliusz Słowacki (1809–1849) and Cyprian Norwid (1821–1883). My comparative study will be restricted to four works: Krasiński's Irydion and Przedświt (Predawn), Słowacki's Agezylausz (Agesilaus) and Norwid's Quidam.

Abstract This article discusses the work of the early Irish comparatists Hutcheson Macaulay Posnett, who in 1886 published the first monograph in English in comparative literature. By bringing into discussion Posnett’s lesser-known journalistic publications on politics, the essay argues that his comparative project was importantly determined by the contemporary challenges of British imperial politics and by his own position in the British Empire. The article investigates several aspects of Posnett’s work in the context of British colonialism: his understanding of literature and literary criticism, his perception of the English and French systems of national literature, and his understanding of world literature and classical literature. Recognising the imperial and colonial context of Comparative Literature additionally highlights the development of literary comparisons, which have marked subsequent discussions in the discipline.

The article draws on the Deleuzoguattarian conceptualisation of minor literature in order to analyse the literature of Brina Svit. The concepts of (de)territorialisation (schizophrenia), minor, and other attributes of minor literature are employed in the comparative analysis of different literary elements of four of Brina Svit’s novels. The article outlines the ‘line of flight’ (defined as deterritorialisation, de-oedipisation, politicalness, and collectiveness) that manifests itself from Svit’s novels written in Slovenian to the novels written in French. The literature of Brina Svit is placed in between minor literature and minority literature.

Abstract:
In this article we present the ongoing theoretical discussions concerning the relations between anthropology and literature in France. We recall the historical relationship of a part of French anthropology and the world of literature. We then try to show how the anthropology of literature began by using the model of the anthropology of art, mainly concentrating on literary works as individual creations specific to the style or the cosmology of a given writer. We explore a new perspective on the analysis of the social and symbolic meanings of literary worlds, putting the emphasis on what is called ‘ethnocriticism’ in France. In order to understand better the influence of literature and literary motives on contemporary cultural practices, and to grasp the relation of literary works with the outside world and with everyday life, we propose to build up a comparative approach of literary works and rituals. Through different novels or other literary works, we address possible developments of contemporary anthropologies of literature in France.

Abstract:
This thesis takes into consideration the presence of computers and electronic equipment in French literary and multimedia discussions, beginning in the first chapter with the foundation of the Oulipo group in 1960 and taking as a starting point the group's conceptions of the computer in relation to literature. It proceeds in the second chapter to explore the materialities and physical factors that have informed the evolution of ideas related to the composition and reading of digital texts, so as to illuminate some of the differences that may be purported to exist between e-literatures and traditional print works. Drawing on Roland Barthes' 'Between Work and Text,' the chapters gradually progress into an exploration of spatiality in digital and interactive literatures, taking into account the role of exhibitions in accommodating and diffusing these forms in France, notably the 1985 exhibition 'Les Immatériaux,' to whose writing installations the third chapter is dedicated. The first three chapters thus focus on computer assisted reading and writing prior to 1985. The chapters that form the second half of the thesis deal with more recent years, exploring online and mobile application works, reading these as engendering their own distinct physical spaces that extend beyond the 'site' of the work - both the website or display and the tactile materials on which the work is operated - creating in relation to the reading what Roberto Simanowski terms a 'semiotic body'. The fourth chapter takes into consideration the role of the reader's body in Annie Abrahams' 'Séparation' and Xavier Malbreil's 'Livre des Morts'. The fifth chapter explores gesture as a mode of reading and reinscription in the online, interactive works of Serge Bouchardon. Finally, the sixth chapter looks at mobile application narratives, spampoetry and email art, offering ways of reading the new spatialities these forms generate. The work as a whole aims to offer some perspectives for considering digital literatures as capable of creating complex spatial experiences between work and text.

Abstract:
This dissertation examines how literary form adapts to emergent print environments by identifying common strategies for incorporating the act of reading into the situation of the text. In my analysis of original textual forms, I investigate the material specificity of constitutively modern practices of reading and subjectivity, focusing on how innovative publications structure these practices by involving the reader in the process of production. This project assembles six pioneering writers across literary traditions, genres, and periods, from the 1830s to the 1910s, in three chapter pairings: novelistic episodes of Honoré de Balzac’s Comédie humaine and prose poems of Charles Baudelaire’s Spleen de Paris in nineteenth-century Parisian periodicals; the prose poetry books, Une saison en enfer by Arthur Rimbaud and Spring and All by William Carlos Williams; and genre-bending texts from the œuvres of Stéphane Mallarmé and Vladimir Mayakovsky, including the typographically irregular page spreads of Un coup de dés jamais n’abolira le hasard and Vladimir Mayakovsky: A Tragedy (Vladimir Maiakovskii: Tragediia). My discussion locates reflexive conceptions of modern literature in constructions of the reading subject, while extending the performative framework of textual modes of address to new media and digital technologies—social interfaces that mediate subjectivity by structuring practices of reading.

Os salões eram considerados importantes locais de sociabilidade, onde eram travadas relações, alianças e casamentos desde o século XVIII. A crônica social, por sua vez, carrega a representação dessas sociabilidades para as páginas dos jornais e os torna espaço de reconhecimento da alta sociedade, descrevendo modas, frequentadores, acontecimentos e o próprio ambiente mundano. Através da análise comparativa desses textos produzidos por Marcel Proust e João do Rio, a presente pesquisa visa investigar esse gênero de grande destaque nos jornais e revistas da Belle Époque, tanto na França como no Brasil. Para isso, propomos, em primeiro lugar, uma contextualização histórico-cultural da alta sociedade, dos salões e da imprensa dos dois países. Em seguida, procuramos explorar os traços jornalísticos e literários presentes nas crônicas sociais. Como os escritores se serviam dos protocolos do suporte impresso? De que maneira os aspectos ficcionais são inseridos nessas seções dos periódicos? E por que? Como João do Rio produzia crônicas de salões em um país cuja população era majoritariamente formada por analfabetos? Esses foram alguns dos questionamentos levantados. Por meio de leituras sistemáticas, análises textuais e de gênero levando em conta o contexto dos jornais Le Figaro e Le Gaulois, no caso de Proust, e O Paiz, no caso de João do Rio, foi possível concluir que a crônica social é composta de uma tensão entre a tentativa de representar o esplendor dos salões da alta sociedade carioca e parisiense e a necessidade de responder às exigências midiáticas. Pode-se dizer que ambos compreendem o gênero e fazem bom uso tanto das regras midiáticas como de seus aspectos literários. Marcel Proust e João do Rio, apesar de estarem inseridos em cenários tão diversos, descrevem o ritual dos salões, cuja liturgia é conhecida e reconhecida pelos leitores e ainda inserem esse universo, ao mesmo tempo real e imaginado, entre as colunas econômicas e políticas.<br>The salons were considered important sociability places, where relationships and alliances were settled and marriages arranged since the 18th century. The social article, in its turn, lead the representation of this sociability to the newspapers pages, turning them into a space of recognition for the high society, describing the fashion, the public, the events and the very worldliness ambiance. Through the comparative analysis of the texts by Marcel Proust and João do Rio, the present research intends to investigate this genre of great preeminence in newspapers and magazines form the Belle Époque in France and Brazil. In order to do so, we provide, in the first place, a historical and cultural context of the high society, the salons and the press from both countries. Then, we will exploit the journalistic and literary traits in the social articles. How did the writers take advantage of the printed media protocols? How fictional traits were inserted in these sections of the newspapers and why? How did João do Rio write salon articles in a country whose population was predominantly illiterate? Those were a few of the questions raised. Through systematic readings and analysis of text and genre on the newspapers Le Figaro and Le Gaulois regarding Proust, and O Paiz in the case of João do Rio, we were able to infer that the society pieces were made from a tension between the attempt to represent the Carioca and Parisian high societies salons splendor and the need to supply the demands of the media. We can state that both authors understand the genre and make good use of its rules and literary traits. Marcel Proust and João do Rio, despite being inserted in such different contexts, describe the salons rituals whose liturgy is know and acknowledged by its readers and yet place them into this universe, at the same time real and imagined, between economy and politics columns.

Dans plusieurs romans québécois récents, nous observons une tendance chez les auteurs féminins à exploiter le corps de la femme dans des écrits, souvent à connotation érotique. Ce corps devient un outil de séduction et sert au personnage féminin dans une quête quelconque (attention, affection, admiration, etc.) Cette approche surprend si nous considérons la démarche entreprise par les féministes des années 1970-80 pour libérer la femme de l'emprise qu'exercaient les hommes sur son corps et pour la faire passer d'objet à sujet, entre autres au sein d'une littérature féministe. Nous abordons donc, dans cette analyse d'oeuvres de nouvelles écrivaines, la conception du personnage féminin ainsi que son rôle, pour savoir si ce dernier renvoie à une fonction de sujet où s'il exerce plutôt un retour à la fonction d'objet, idéologie véhiculée par les valeurs patriarcales. À l'aide de romans contemporains, Borderline de Marie-Sissi Labrèche, Putain de Nelly Arcan et Ainsi font-elles toutes de Clara Ness, nous constatons que les personnages féminins donnent l'impression d'être sujets par la façon dont ils usent de leur corps pour se forger une identité et une personnalité forte. Par contre, les romancières récupèrent des idées misogynes entourant la femme, la ramenant à sa condition de femme-objet dont le corps sert au plaisir des hommes. Des études sur l'érotisme et la pornographie permettent de déterminer les limites et les différences entre ce qui est conçu pour le regard masculin et ce qui renvoie aux intérêts féminins. De plus, les théories sur l'agentivité qui étudient le pouvoir et le contrôle qu'exercent les personnages féminins sur leur propre vie nous permettent de mieux délimiter les frontières entre femme-objet, femme-objet "volontaire" et "assumée" et femme-sujet.

Orientador: Daniela Mantarro Callipo<br>Banca: Carla Cavalcanti e Silva<br>Banca: Marcos Antônio de Moraes<br>Banca:<br>Resumo: Utilizando o livro Táxi e crônicas no Diário Nacional, organizado por Telê Porto Ancona Lopez (2005), pretende-se estudar o gênero crônica, bem como apresentar hipóteses que elucidem a importância das alusões aos literatos franceses Marcel Proust, Julien Benda e Blaise Cendrars, presentes em 14 das 173 crônicas de Mário de Andrade que compõem a obra. Antonio Candido (1977, p.12) afirma que já no século XVIII, a língua francesa era a ―língua universal das pessoas cultas. Um século mais tarde, estava no ―clímax de seu prestígio e de sua função civilizadora, tornando-se ―ensino obrigatório e indispensável‖. No Brasil, os escritores acompanhavam as discussões literárias e filosóficas que ocorriam na França, pois a Independência e o Romantismo tornaram urgente a afirmação da nossa nacionalidade e a ex-colônia passou a nutrir uma espécie de desprezo por toda e qualquer forma de vínculo que pudesse colocar a literatura brasileira em situação de submissão à portuguesa. A França, grande centro irradiador de cultura da época, passou a ser a fonte de inspiração para os escritores que liam e admiravam as obras de Balzac, Victor Hugo e Dumas Fils. Esta irradiação chegou ao século XX, tornando-se presente na obra de Manuel Bandeira, Oswald de Andrade e Sérgio Milliet. Abstract:
In this dissertation, I examine the evolution of the Gypsy trope in Romantic French literature at a time when nostalgia became a powerful aesthetic and political tool used by varying sides of an ideological war. Long considered a transient outsider who did not view time or privilege the past in the same way Europeans did, the Gypsy, I argue, became a useful way for France's writers to contain and tame the transience they felt interrupted nostalgia's attempt to recapture a lost past. My work specifically looks at the development of this trope within a thirty-year period that begins in 1823, just before Charles X became France's last Bourbon king, and ends just after Louis-Napoleon declared himself Emperor of France in 1852. Beginning with Quentin Durward (1823), Walter Scott's first historical novel about France, and the French novel that looked to it for inspiration, Victor Hugo's Notre-Dame de Paris (1831), I show how the Gypsy became a character that communicated a fear that France was recklessly forgetting and destroying the monuments and narratives that had long preserved its pre-revolutionary past. While these novels became models in how nostalgia could be deployed to seduce France back into a relationship with a particular past, I also look at how the Gypsy trope is transformed some fifteen years later when nostalgia for Napoleon nearly leads France into two international conflicts and eventually traps the French into what George Sand called a dangerous "bail avec le pass&eacute." In new readings of Prosper Mérimée's Carmen (1845) and George Sand's La Filleule (1853), I argue that both authors personify the dangers of recapturing the past, albeit in two very different ways. While Mérimée makes nostalgia and the Gypsy accomplices, George Sand gives France an admirable Gypsy heroine, a young woman who offers readers a way out of nostalgia's viscous circle. I conclude by arguing that nostalgia and this Romantic trope found their way back into France at the dawn of a new millennium, and the Gypsy has once again been typecast in art and politics as deviant for refusing to dwell in or on the past.<br>Romance Languages and Literatures

L'importance de l'oeuvre de Gaston Miron dans la littérature québecoise et celle de Manuel Bandeira dans la littérature brésilienne n'est plus a démontrer. Cependant, l'orientation qu'ont reçue ces oeuvres a été principalement de nature sémiotique et thématique. La présente étude vise a démontrer que par l'approche pragmatique---qui met en évidence les marques discursives pertinentes---la poésie se révèle porteuse d'un type d'engagement de nature socio-culturelle, comme chez le poète brésilien, ou politique, chez le poète québecois. Ainsi, la matérialité du discours, dont relèvent les ressources stylistico-rhétoriques (les déictiques, les pronoms personnels, les temps verbaux), ainsi que les éléments de l'oralite (la parataxe, l'ellipse et la repetition), manifestent une subjectivité sur le plan de l'énonciation discursive. En outre, ce plan est tissé par différents niveaux, que ce soit phonique, syntaxique, lexical, sémantique et rythmique; par la façon dont ils sont agencés, communique une plethore d'émotions, résultant en un éthos qui essaie de rejoindre le destinataire, de façon a susciter chez celui-ci une réaction et, par conséquent, une transformation culturelle et sociale.

AbstractThis chapter analyzes literary advice culture from a transnational-comparative perspective. It sheds light on the reception of the American poetics of creative writing in contemporary France by examining the specific case of Outils du roman: Avec Malt Olbren sur les pistes et exercices du creative writing à l’américaine (2016, Tools of the Novel. Exploring American Creative Writing with Malt Olbren) by the experimental prose-writer François Bon. This text represents a broader dynamic in which French authors of literary advice resort to a repertoire of American writing techniques in an attempt to revive French literature. To conceptualize this process of transfer, I use Deleuze and Guattari’s concept of “minor literature.” This notion conveys how literary advice in France must constantly position itself vis-à-vis its American counterpart, but also how it appropriates and transforms this same body of ideas and techniques. More generally, this chapter makes a case for an increased consideration of supranational transfers in the domain of literary advice when studying processes of local literary change.

Abstract:
Objectives. To evaluate the effectiveness and comparative effectiveness of pharmacologic and nonpharmacologic therapies for the acute treatment of episodic migraine in adults. Data sources. MEDLINE®, Embase®, Cochrane Central Registrar of Controlled Trials, Cochrane Database of Systematic Reviews, PsycINFO®, Scopus, and various grey literature sources from database inception to July 24, 2020. Comparative effectiveness evidence about triptans and nonsteroidal anti-inflammatory drugs (NSAIDs) was extracted from existing systematic reviews. Review methods. We included randomized controlled trials (RCTs) and comparative observational studies that enrolled adults who received an intervention to acutely treat episodic migraine. Pairs of independent reviewers selected and appraised studies. Results. Data on triptans were derived from 186 RCTs summarized in nine systematic reviews (101,276 patients; most studied was sumatriptan, followed by zolmitriptan, eletriptan, naratriptan, almotriptan, rizatriptan, and frovatriptan). Compared with placebo, triptans resolved pain at 2 hours and 1 day, and increased the risk of mild and transient adverse events (high strength of the body of evidence [SOE]). Data on NSAIDs were derived from five systematic reviews (13,214 patients; most studied was ibuprofen, followed by diclofenac and ketorolac). Compared with placebo, NSAIDs probably resolved pain at 2 hours and 1 day, and increased the risk of mild and transient adverse events (moderate SOE). For other interventions, we included 135 RCTs and 6 comparative observational studies (37,653 patients). Compared with placebo, antiemetics (low SOE), dihydroergotamine (moderate to high SOE), ergotamine plus caffeine (moderate SOE), and acetaminophen (moderate SOE) reduced acute pain. Opioids were evaluated in 15 studies (2,208 patients).Butorphanol, meperidine, morphine, hydromorphone, and tramadol in combination with acetaminophen may reduce pain at 2 hours and 1 day, compared with placebo (low SOE). Some opioids may be less effective than some antiemetics or dexamethasone (low SOE). No studies evaluated instruments for predicting risk of opioid misuse, opioid use disorder, or overdose, or evaluated risk mitigation strategies to be used when prescribing opioids for the acute treatment of episodic migraine. Calcitonin gene-related peptide (CGRP) receptor antagonists improved headache relief at 2 hours and increased the likelihood of being headache-free at 2 hours, at 1 day, and at 1 week (low to high SOE). Lasmiditan (the first approved 5-HT1F receptor agonist) restored function at 2 hours and resolved pain at 2 hours, 1 day, and 1 week (moderate to high SOE). Sparse and low SOE suggested possible effectiveness of dexamethasone, dipyrone, magnesium sulfate, and octreotide. Compared with placebo, several nonpharmacologic treatments may improve various measures of pain, including remote electrical neuromodulation (moderate SOE), magnetic stimulation (low SOE), acupuncture (low SOE), chamomile oil (low SOE), external trigeminal nerve stimulation (low SOE), and eye movement desensitization re-processing (low SOE). However, these interventions, including the noninvasive neuromodulation devices, have been evaluated only by single or very few trials. Conclusions. A number of acute treatments for episodic migraine exist with varying degrees of evidence for effectiveness and harms. Use of triptans, NSAIDs, antiemetics, dihydroergotamine, CGRP antagonists, and lasmiditan is associated with improved pain and function. The evidence base for many other interventions for acute treatment, including opioids, remains limited.

Abstract:
Objective. This evidence report synthesizes the available evidence on radiation therapy for brain metastases. Data sources. We searched PubMed®, Embase®, Web of Science, Scopus, CINAHL®, clinicaltrials.gov, and published guidelines in July 2020; assessed independently submitted data; consulted with experts; and contacted authors. Review methods. The protocol was informed by Key Informants. The systematic review was supported by a Technical Expert Panel and is registered in PROSPERO (CRD42020168260). Two reviewers independently screened citations; data were abstracted by one reviewer and checked by an experienced reviewer. We included randomized controlled trials (RCTs) and large observational studies (for safety assessments), evaluating whole brain radiation therapy (WBRT) and stereotactic radiosurgery (SRS) alone or in combination, as initial or postoperative treatment, with or without systemic therapy for adults with brain metastases due to non-small cell lung cancer, breast cancer, or melanoma. Results. In total, 97 studies, reported in 190 publications, were identified, but the number of analyses was limited due to different intervention and comparator combinations as well as insufficient reporting of outcome data. Risk of bias varied; 25 trials were terminated early, predominantly due to poor accrual. Most studies evaluated WBRT, alone or in combination with SRS, as initial treatment; 10 RCTs reported on post-surgical interventions. The combination treatment SRS plus WBRT compared to SRS alone or WBRT alone showed no statistically significant difference in overall survival (hazard ratio [HR], 1.09; confidence interval [CI], 0.69 to 1.73; 4 RCTs; low strength of evidence [SoE]) or death due to brain metastases (relative risk [RR], 0.93; CI, 0.48 to 1.81; 3 RCTs; low SoE). Radiation therapy after surgery did not improve overall survival compared with surgery alone (HR, 0.98; CI, 0.76 to 1.26; 5 RCTs; moderate SoE). Data for quality of life, functional status, and cognitive effects were insufficient to determine effects of WBRT, SRS, or post-surgical interventions. We did not find systematic differences across interventions in serious adverse events radiation necrosis, fatigue, or seizures (all low or moderate SoE). WBRT plus systemic therapy (RR, 1.44; CI, 1.03 to 2.00; 14 studies; moderate SoE) was associated with increased risks for vomiting compared to WBRT alone. Conclusion. Despite the substantial research literature on radiation therapy, comparative effectiveness information is limited. There is a need for more data on patient-relevant outcomes such as quality of life, functional status, and cognitive effects.

Abstract:
Background. Untreated maternal mental health disorders can have devastating sequelae for the mother and child. For women who are currently or planning to become pregnant or are breastfeeding, a critical question is whether the benefits of treating psychiatric illness with pharmacologic interventions outweigh the harms for mother and child. Methods. We conducted a systematic review to assess the benefits and harms of pharmacologic interventions compared with placebo, no treatment, or other pharmacologic interventions for pregnant and postpartum women with mental health disorders. We searched four databases and other sources for evidence available from inception through June 5, 2020 and surveilled the literature through March 2, 2021; dually screened the results; and analyzed eligible studies. We included studies of pregnant, postpartum, or reproductive-age women with a new or preexisting diagnosis of a mental health disorder treated with pharmacotherapy; we excluded psychotherapy. Eligible comparators included women with the disorder but no pharmacotherapy or women who discontinued the pharmacotherapy before pregnancy. Results. A total of 164 studies (168 articles) met eligibility criteria. Brexanolone for depression onset in the third trimester or in the postpartum period probably improves depressive symptoms at 30 days (least square mean difference in the Hamilton Rating Scale for Depression, -2.6; p=0.02; N=209) when compared with placebo. Sertraline for postpartum depression may improve response (calculated relative risk [RR], 2.24; 95% confidence interval [CI], 0.95 to 5.24; N=36), remission (calculated RR, 2.51; 95% CI, 0.94 to 6.70; N=36), and depressive symptoms (p-values ranging from 0.01 to 0.05) when compared with placebo. Discontinuing use of mood stabilizers during pregnancy may increase recurrence (adjusted hazard ratio [AHR], 2.2; 95% CI, 1.2 to 4.2; N=89) and reduce time to recurrence of mood disorders (2 vs. 28 weeks, AHR, 12.1; 95% CI, 1.6 to 91; N=26) for bipolar disorder when compared with continued use. Brexanolone for depression onset in the third trimester or in the postpartum period may increase the risk of sedation or somnolence, leading to dose interruption or reduction when compared with placebo (5% vs. 0%). More than 95 percent of studies reporting on harms were observational in design and unable to fully account for confounding. These studies suggested some associations between benzodiazepine exposure before conception and ectopic pregnancy; between specific antidepressants during pregnancy and adverse maternal outcomes such as postpartum hemorrhage, preeclampsia, and spontaneous abortion, and child outcomes such as respiratory issues, low Apgar scores, persistent pulmonary hypertension of the newborn, depression in children, and autism spectrum disorder; between quetiapine or olanzapine and gestational diabetes; and between benzodiazepine and neonatal intensive care admissions. Causality cannot be inferred from these studies. We found insufficient evidence on benefits and harms from comparative effectiveness studies, with one exception: one study suggested a higher risk of overall congenital anomalies (adjusted RR [ARR], 1.85; 95% CI, 1.23 to 2.78; N=2,608) and cardiac anomalies (ARR, 2.25; 95% CI, 1.17 to 4.34; N=2,608) for lithium compared with lamotrigine during first- trimester exposure. Conclusions. Few studies have been conducted in pregnant and postpartum women on the benefits of pharmacotherapy; many studies report on harms but are of low quality. The limited evidence available is consistent with some benefit, and some studies suggested increased adverse events. However, because these studies could not rule out underlying disease severity as the cause of the association, the causal link between the exposure and adverse events is unclear. Patients and clinicians need to make an informed, collaborative decision on treatment choices.
