Abstract Thyrotoxic periodic paralysis (TPP) is a life-threatening neuromuscular complication of thyrotoxicosis characterized by muscle weakness and hypokalemia and with an unclear etiopathogenesis. However, the 17q24.3 locus had been genetically linked to TPP, in which the genetic variant rs312691 (TC genotype) in long intergenic noncoding RNA (lincRNA) CTD-2378E21.1 is located downstream of inward-rectifier potassium (Kir) channel genes [KCNJ2 and its antisense KCNJ2 (AS-KCNJ2)]. A TPP patient with a suppressed thyroid-stimulating hormone level, a high free thyroxine level of (5.8 ng/dL), and low serum potassium level of (2 mEq/L) was evaluated for Kir channel expression during and after recovery from thyrotoxicosis. We observed that circulating lincRNA and Kir expression varied in accordance with thyroid status and TC genotype. To endorse this association of a lincRNA-rs312691 variant with a genetic risk of TPP, an additional series of 37 patients with TPP and 32 patients with thyrotoxic without paralysis (TWP) were assessed. We verified that the risk of minor allele C was greater in TPP than in TWP (odds ratio, 5.289; P = 0.0062), and protective major allele T was more frequent than observed in the 1000 genome controls (odds ratio, 11.90; P < 0.0001). AS-KCNJ2 was downregulated during thyrotoxicosis in the TWP controls carrying allele T and were upregulated in those with TPP with risk allele C. Moreover, KCNJ2 (Kir2.1) expression was reduced during thyrotoxicosis and restored in euthyroid status. We further excluded any other coding variant by performing targeted exome sequencing mutational screening in 17q24.3. Our data suggest that high lincRNA AS-KCNJ2 and CDT-2378E21.1 expression, possibly driven by the triiodothyronine regulatory mechanism, reduces the Kir2.1 expression observed during thyrotoxicosis. This finding could contribute to the understanding of the reduced inward-rectifying current observed during muscle weakness in genetically susceptible TPP patients.

This paper combines a derivative-free hybrid optimization algorithm, generalize pattern search (GPS), with Treed Gaussian Processes (TGP) to create a new hybrid optimization algorithm. The goal is to use the method for top design of satellite system, in which the objective or constraint functions usually are computationally expensive black-box functions. TGP model partitions the design space into disjoint regions, and employs independent Gaussian Processes (GP) in each partition to represent the time consumption of true problem responses. Utilizing the TGP, we generate the new “promising” points, which are the combination of model-predicted values and estimated model errors. Then, these points are used to guide GPS search in the design space efficiently. The hybrid optimization method is applied to top design of multi-satellites cooperated observation. The results demonstrate that the proposed method can not only increase the chance of obtaining optimal solution but also cut down the cost of function evaluations.

AbstractA printed antenna array consisting of 10 wideband symmetrical pentagonal dipoles is presented. The feed network of impedance transformers is employed to provide appropriate amplitude and phase distribution necessary to obtain flat top beam pattern. The measured results demonstrate excellent radiation characteristics including 38° flat gain region with maximum ripple of 3.5 dB at the centre frequency. Furthermore, the proposed antenna that is placed in corner reflector with angle of 60° has good gain (17 dBi) and side lobe suppression (18.9 dB). Although it is designed at the centre frequencyfc=12 GHz, it was shown to be capable of shaping a good flat top radiation pattern within a fractional bandwidth at least 6 % of centre frequency. Also, proposed antenna features cheap, simple and easy fabrication that makes it suitable for mass production.

Abstract:
The effects of two structurally similar pyrazine derivatives, tetramethylpyrazine (TMP) and tetraethylpyrazine (TEP) on the contractile responses of dog saphenous vein to KCl (via membrane depolarization), phenylephrine (PHE, α1-adrenergic agonist), and B-HT 920 (α2-adrenergic agonist) were investigated. The relaxant or inhibitory effect of TMP and TEP was most potent on KCl-induced responses and least potent on PHE-induced responses. Their effect on KCl-induced responses was more prominent at 30 mM KCl than at 100 mM KCl. In Ca2+-free medium, PHE and B-HT 920 elicited transient responses, which were also markedly and reversibly inhibited by TMP and TEP. Similar results were also obtained when prostaglandin F2α was used as an agonist. In all four types of contractile responses involving different receptors, the inhibitory effect of TEP was consistently more potent than that of TMP. We conclude that both TMP and TEP behave as a nonselective smooth muscle relaxant having similar and multiple actions including their general interference with the processes involving both Ca2+ entry and intracellular Ca2+ release.Key words: vascular smooth muscle, calcium channels, tetramethylpyrazine, tetraethylpyrazine, α-adrenoceptors, saphenous vein.

AbstractSmartdielectric resonator antenna (DRA) having beam control mechanism is anew area to be explored by antenna researchers. Proposed new geometry DRA has low loss, design flexibility, high efficiency, compact size and desired radiated beam control. Developing beam control in new geometry DRAs is investigated for the first time in this letter. Unique technique for beam control and beam width control is proposed using pit top and mount top DRA. Gain is controlled from 5.0 to 9.98 dBi and beam is controlled from ±30° to ±70° in broadside radiation pattern. U shape pit DRA has maximum directive gain of 9.98 dBi and efficiency 98% at 5.8 GHz frequency. Measured and simulated results of radiation pattern and reflection coefficient are found to be in close proximity. Hardware of U shape pit top DRA, mount top DRA, left side arc top DRA, right side arc shape top DRA is developed and investigated. Mobile and cellular communication network need wide coverage, hence large beam width is required. Narrowing of beam width at higher order mode is also achieved.

Abstract:
<p><strong>Background: </strong>Human-milk is a complex extracellular fluid with different biochemical composition compared with blood. The present study assessed the levels of markers of oxidative stress in human-milk and plasma of healthy lactating mothers.</p><p><strong>Methods:</strong> Forty lactating mothers (22-36 years) volunteered to participate in this study. They were non-smokers and apparently normal nursing mothers who had normal delivery without postnatal complications. Blood and breast-milk samples were collected from the lactating mothers between 6th and 18th weeks after delivery. Total antioxidant potential (TAP), total plasma peroxides (TPP) and malondialdehyde (MDA) were determined in both human-milk and blood of the mothers using spectrophotometric methods. Oxidative stress index (OSI) was determined as the percent ratio of the TPP and TAP.</p><p><strong>Results</strong>: The results showed that TAP, TPP and OSI increased significantly (p&lt;0.001) in human-milk compared with the plasma. The plasma / human-milk ratios of TAP, TPP and OSI were 1:3, 1:5 and 1:2 respectively. The level of MDA was significantly (p=0.01) lower in the breast-milk, compared with the plasma. In the plasma, levels of OSI correlated significantly with TAP (r= -0.46; p= 0.015) and TPP (r=0.90; p &lt;0.001) while in the breast-milk, OSI correlated significantly with only the TAP (r = -0.76; p&lt;0.001).</p><p><strong>Conclusion:</strong> It could be concluded that the higher TPP in the human milk would be beneficial against pathogens, while the bio-accumulated antioxidant molecules regulate free radical load to avert the consequences of oxidative stress in the breast tissue.</p>

This paper inspects the influence of a spring attachment provided on the top elastic layer on the stability of a pre-twisted, rotating sandwich beam having viscoelastic supports at the root under the impact of a periodically varying axial load. The spring is deployed on the beam to achieve more strength to weight ratio without compromising the stability. The beam is exponentially tapered, and a tip mass is at the free end to represent the rotating members in various types of machinery as closely as possible. The ruling equations and inter-related boundary conditions are attained by applying Hamilton’s principle. To obtain the solution, a matrix equation was developed through the assumed-mode variational method. The resulting matrix equation was converted to a coupled Hill’s equation of parametric vibration through the modal matrix corresponding to the free vibration problem. Finally, static and dynamic stability graphs were obtained for several system parameters such as position and length of the attached spring on the top elastic layer, the mass of the spring attachment, stiffness of the spring attachment, angle of pre-twist, tip mass, taper parameter, temperature gradient parameter, setting angle, viscoelastic spring stiffness, etc. to analyze their impact on the system’s stability. Saito and Otomi conditions were used to obtain dynamic stability plots. Greater stability is achieved due to the spring attachment on the top of the top elastic layer.

Abstract Background Thrombospondin-1 (TSP-1) is considered to control the von Willebrand factor (VWF) multimer size by reducing interchain disulfide bridges, whereas ADAMTS13 is the major VWF cleaving protease. However the exact contribution of each of these to VWF size control is not known. In this study we investigate the relationship between ADAMTS13 antigen levels and TSP-1 levels in diseased plasmas. Methods Antigen levels of TSP-1 and ADAMTS13 were determined in 21 plasma samples derived from 11 patients with TTP, and compared to normal control, 20 ITP samples and 20 SLE samples. TSP-1 antigen was measured by a commercial kit (Cytimmune, Rockville, MD), which can detect both bound and free TSP-1 in plasma. ADAMTS13 antigen was measured in a newly developed sandwich ELISA using 3 monoclonal antibodies to ADAMTS13. All donors were Chinese. Results The average TSP-1 antigen level in normal plasma was 18.34±7.24 μ g/ml (mean ± SD, n=30) and the mean ADAMTS13 level in Chinese normals (CN) was 601±145 mU/ml (n=26) compared to the level set at 1 U/ml in pooled (n=20) normal human plasma from Caucasian origin. In TTP plasma, before plasma exchange (pre-PE), 5.79±2.35 μg/ml TSP-1 was detected (n=11, p&lt;0.01 versus CN), which upon PE increased to 9.06±6.52 μ g/ml (n=10, p&gt;0.05 versus pre-PE). The mean antigen level of ADAMTS13 was 98.7±82.8 mU/ml (n=11, p&lt;0.01, versus CN) and 449±232 mU/ml (n=10, p&lt;0.05 versus pre-PE), before and after PE, respectively. Furthermore TSP-1 antigen levels and ADAMTS13 titres in these patients plasma correlated linearly with an R=0.7 (n=21). To determine whether this significant decrease in both TSP-1 and ADAMTS13 antigen levels is restricted to TTP, plasma from ITP and SLE patients was analyzed next, however no significant decrease of TSP-1 was found. In contrast, the average antigen level of ADAMTS13 was raised to 830±229 mU/ml (n=20, p&lt;0.01 versus CN) and 721±192 mU/ml (n=20, p&lt;0.05 versus CN), for ITP and SLE plasmas, respectively. Conclusion Both ADAMTS13 and TSP-1 antigen levels are significantly reduced in TTP plasma and hence the combined effect may be responsible for the presence of ultra-long VWF multimers in TTP plasma. Interestingly ADAMTS13 antigen level appears to be increased in SLE/or ITP parients, in contrast to previous reports that showed reduced activity, whereas no changes in TSP-1-levels were found in those patients.

Abstract:
This paper focuses on three fundamental notions-realities which describe human life and constitute at the same time its foundation and the space of participation in it. The triad culture – education – truth describes the most essential human experiences, simultaneously, defining the direction of their deepening and creative developing. Certainly, in the times like these when everything is questioned notions-experiences that belong to this triad are not problem-free. What is more, they are deemed to be superfluous, especially in ultra-left-wing ideologies in which suicidal tendencies are easily discernible. Taking this into consideration, it is hardly possible to avoid polemic attitude towards numerous questions even those most essential since in their semantic field the greatest confusion may be detected. That is why there are numerous attempts to answer the question how, in the face of such a dynamic spiritual condition of the world, those fundamental issues can be dealt with within the frames of anthropology, and how they can be justified from the anthropological point of view. This reflection is rather philosophical in character, however, it is also inspired by the fundamental Christian truth which says about the incarnation of the Son of God and about the salvation of man made by Him.

Abstract:
The fouling behavior of a steam generator (SG) tube was investigated at different positions after 500 h of immersion in high-temperature water. A triple-layer structure of fouling appeared at both the crevice position and the free span position, namely, the large, dispersedly distributed deposition layer on the top; the small and faceted outer layer; and the relatively continuous inner layer. There was no obvious positional effect on the thickness of the inner layer. However, in the crevice position, the density of the deposited particle and the thickness of the outer layer was much higher than those of the free span position. The tube support plate (TSP) made of 410 stainless steel contributed significantly to the fouling behavior of the SG tube in the crevice between the SG tube and the TSP.

This article is the result of theological research on the subject of life. The phenomenon of life is multifaceted, but often it cannot be defined because it is a mystery. The author of the article, based on the truth that man is a biological-psycho-spiritual unity, interprets these spheres in the key of St. Bonaventure’s idea, describes them as vestigium, umbra, and imago Dei. The issue of spiritual life in the category of Imago Dei is analyzed in detail. The author tries to answer the questions that arise by using not only theological fields: or is there an openness to transcendence only in a man? What does it mean to be an image of God? How to achieve a full life? The sphere of bios, psyche, and zoe in the human person are permeating each other, but this spiritual life transcends the previous two planes. This distinguishes man from other creatures and gives him a unique character. God, in His goodness and freedom, grants man life, creates him as a free being and in His image. But the perfect image of the Father is Jesus Christ. In His Incarnation, He showed us the fullness of humanity and through imitation and union with Christ, man can become conformed to the image of God. In this way, it is finally possible to obtain full participation in the communion with God in the Holy Trinity.

Background Utility of the sentinel lymph node (SLN) biopsy in some malignancies has been reported, however, research on that of gallbladder cancer (GBC) is rare. We evaluated the relationship between cystic duct node (CDN) and downstream lymph node (LN) status. CDN was defined as a SLN in this study. Results Thirty-eight patients were classified into T2, including T2p (n = 18) and T2h (n = 20), and 42 patients into T3, including T3p (n = 22) andT3h (n = 20). The incidence of LN metastasis was significantly higher in hepatic-side than peritoneal-side in both T2 and T3 (P = 0.036 and 0.009, respectively). In T2, 14 T2p had negative CDN and downstream LN, however, three T2h had negative CDN and positive downstream LNs (defined as a skipped LN metastasis) (P = 0.043). In T3, patients with skipped LN metastasis were significantly higher in T3h (n = 11) than those in T3p (n = 2) (P<0.001). There was no recurrence of the local lymph node. Disease-free survival in the T2p and T3p were significantly better than those in the T2h and T3h (P = 0.005 and 0.025, respectively). Conclusion The concept of SLN can be applicable to T2p GBC, where the downstream LNs dissection can be omitted.

AbstractA wideband bandpass filter (BPF) with an adjustable notched-band and high selectivity is proposed. The proposed BPF consists of a multi-mode resonator (MMR), twoλ/2 resonators, and I/O feed lines with 50 ohm characteristic impedance. The MMR, connected as a whole by a wide stub, is composed of one I-shaped resonator and two open-loop resonators. Tightly coupling is built between MMR andλ/2 resonators. I/O feed lines are directly connected with twoλ/2 resonators, respectively. Due to the use of tapped-line coupling, one transmission zero (TZ) is formed near low-edge of aim passband. High-edge of passband with one attendant TZ can be tuned to desired location by adjusting bottom-side position of used wide stub or bottom-side length of I-shaped resonator in MMR. The top-side length of I-shaped resonator is applied to improve upper stopband performance and shift undesired resonant mode of MMR near high-edge of aim passband to proper frequency point. The notched-band in aim passband is dominated by top-side position of wide stub in MMR. Good agreement is observed between simulated and measured results.

Abstract This paper presents a novel design approach of a compact dual-band monopole antenna with an overall size of 18.9x13x1.6mm3. The proposed antenna is composed of a fractal ring shaped patch fed by a Y-shaped transmission line on the top side of the substrate and a second fractal ring along with a U-shaped ground plane on the bottom side. The second fractal ring, identical to the radiating ring, is loaded and a rectangular slot is etched at the top side of the ground plane respectively, to achieve dual-band characteristic and improve the impedance matching. The effect of standard ground-plane (SGP) of a laptop computer is incorporated in the design; the antenna is mounted on a SGP in order to investigate its performance. The antenna covers widely the frequency bands of the WLAN 2.4 GHz (2.2–2.52 GHz) and WiMAX 3.5 GHz (3.32–4.35 GHz), and exhibits an omnidirectional radiation pattern in the H-plane and a monopole like radiation pattern in the E-plane. A good agreement between the simulated and measured results indicates that the proposed dual-band antenna design is suitable for WLAN/WiMAX applications.

Annexin VI (AnxVI) of molecular mass 68-70 kDa belongs to a multigenic family of ubiquitous Ca2+- and phospholipid-binding proteins. In this report, we describe the GTP-binding properties of porcine liver AnxVI, determined with a fluorescent GTP analogue, 2'-(or 3')-O-(2,4,6-trinitrophenyl)guanosine 5'-triphosphate (TNP-GTP). The optimal binding of TNP-GTP to AnxVI was observed in the presence of Ca2+ and asolectin liposomes, as evidenced by a 5.5-fold increase of TNP-GTP fluorescence and a concomitant blue shift (by 17 nm) of its maximal emission wavelength. Titration of AnxVI with TNP-GTP resulted in the determination of the dissociation constant (Kd) and binding stoichiometry that amounted to 1.3 microM and 1:1 TNP-GTP/AnxVI, mole/mole, respectively. In addition, the intrinsic fluorescence of the membrane-bound form of AnxVI was quenched by TNP-GTP and this was accompanied by fluorescence resonance energy transfer (FRET) from AnxVI Trp residues to TNP-GTP. This indicates that the GTP-binding site within the AnxVI molecule is probably located in the vicinity of a Trp-containing domain of the protein. By controlled proteolysis of human recombinant AnxVI, followed by purification of the proteolytic fragments by affinity chromatography on GTP-agarose, we isolated a 35 kDa fragment corresponding to the N-terminal half of AnxVI containing Trp192. On the basis of these results, we suggest that AnxVI is a GTP-binding protein and the binding of the nucleotide may have a regulatory impact on the interaction of annexin with membranes, e.g. formation of ion channels by the protein.

Abstract:
Objective: Thyrotoxic periodic paralysis (TPP) is a muscular disorder characterized by sudden episodes of muscle weakness and hypokalemia in the setting of thyrotoxicosis. We aimed to report our experience with TPP in West Texas and compare its clinical presentation to that of patients admitted for complicated thyrotoxicosis. Methods: Retrospective review of records of adult patients with admission diagnosis of hyperthyroidism, thyrotoxicosis, and/or discharge diagnosis of periodic paralysis seen at our institution in a 6-year period. Results: Patients admitted for complicated thyrotoxicosis were more commonly females of a mean age of 44 years. Patients with TPP were more commonly Hispanic males of a mean age of 27 years. Despite no significantly different thyroid hormone levels, patients with TPP presented with less-severe signs and symptoms of hyperthyroidism, as reflected by lower Burch-Wartofsky score on admission (19 vs. 35; P<.001) and lower occurrence of atrial fibrillation in the TPP group (0% vs. 36%; P<.001). Finally, 89% of TPP patients presented with corrected QT (QTc) prolongation, whereas only 19% of thyrotoxic patient presented with a prolonged QTc. Conclusion: Hispanic patients with TTP seems to have relative resistance to the actions of thyroid hormones and commonly present with QTc prolongation, a risk factor for cardiac arrhythmias.
