Abstract:

Abstract:

Abstract:
Determination of the provenance of material culture by means of chemical analysis has a long and distinguished history in archaeology. The chemical analysis of archaeological objects started in the intellectual ferment of late-eighteenth-century Europe (Caley 1948, 1949, 1967; Pollard 2013), almost as soon as systematic (gravimetric) means of chemical analysis had been devised (Pollard in prep.). Many of the leading scientists of the day, such as Vauquelin, Klaproth, Davy, Faraday and Berzelius, carried out analyses of archaeological objects as part of their interests in the contents of the ‘cabinets of curiosities’ of the day (Pollard&Heron 2008). The subject moved frommere curiosity to systematic and problemorientated study with the work of G¨obel (1842),Wocel (1854), Damour (1865) and Helm (1886), who essentially formulated the idea of ‘provenance studies’—that some chemical characteristic of the geological rawmaterial(s) provides a ‘fingerprint’ which can bemeasured in the finished object, and that if an object from a remote source is identified at a particular place, then it is evidence of some sort of direct or indirect contact and ‘trade’ between the two places.

Abstract:
ABSTRACTObjective:To examine the attitudes of Iranian nurses toward caring for dying patients. Methods:Nurses' attitudes toward death and caring for dying patients were examined by using two types of questionnaires: the Death Attitude Profile–Revised (DAP-R) and Frommelt's Attitude towards Caring for Dying Patients (FATCOD), both with a demographic survey. Results:The results showed that most respondents are likely to view death as a natural part of life and also as a gateway to the afterlife. The majority reported that they are likely to provide care and emotional support for the people who are dying and their families, but they were unlikely to talk with them or even educate them about death. They had a tendency not to accept patients and their families as the authoritative decision makers or involve families in patient care. Nurses' personal views on death, as well as personal experiences, affected their attitudes toward care of the dying.Significance of results:Lack of education and experience, as well as cultural and professional limitations, may have contributed to the negative attitude toward some aspects of the care for people who are dying among the nurses surveyed. Creating a reflective narrative environment in which nurses can express their own feelings about death and dying seems to be a potentially effective approach to identify the factors influencing their interaction with the dying. Continuing education may be required for Iranian palliative care nurses in order to improve the patients quality of care at the end of life.

Abstract:
Recent guidebooks for the Westerner traveling to Northern India generally refer the prospective visitor to a common range of cities around Delhi – Agra, Jaipur, and Udaipur; within these, the Taj Mahal, Jaipur's Pink City and nearby Amber Fort, and Udaipur's glamorous lake palaces usually merit must-see status. Until its refurbishment a few years ago, the Albert Hall Museum, an elaborate structure with old-fashioned interiors and a location a kilometer south of Jaipur's city center, ranked as a second- or even third-tier tourist attraction; travel guides from recent years mention it with indifference, describing its collections as “dusty” and “fine, if carelessly exhibited” (Bindloss and Singh 170), or even suggesting that “a slow circular turn around the building in a car will suffice” (Frommers 520). Yet a century ago the Museum proudly occupied a primary place in British travel guides to India. It opened with ceremony and fanfare in 1887, and by 1898 almost three million Indian and over ten thousand European visitors had passed through its doors (Hendley, Report 9). A striking example of colonial architecture, constructed of white stone with numerous courtyards, covered walkways, and ornamented domes (Figure 1), it was regarded as perhaps the most noteworthy edifice within a noteworthy Indian city. Thomas Holbein Hendley, resident Surgeon-Major in Jaipur, chief curator for the 1883 Jaipur Exhibition, and the Albert Hall Museum's Secretary and tireless champion, recommended that travelers in Jaipur for a single day make two visits, both morning and evening, to the site, and that those with an additional day to spend in the city schedule a third visit. Murray's Handbook for Travellers in India, Burma and Ceylon concurred, describing it as “a beautiful museum – an Oriental South Kensington, suitably housed” (174), and just after the turn of the century, English journalist Sidney Low recalled that it was “the best museum, with one exception, in all India, a museum which, in the careful selection and the judicious arrangement of its contents, is a model of what such an institution ought to be” (114).

Abstract:
The Lorentzian interpretation of the special theory of relativity explains all the relativistic effects by true deformations of rods and clocks in absolute motion against a preferred reference system, and where Lorentz invariance is a dynamic symmetry with the Galilei group the more fundamental kinematic symmetry of nature. In an exactly nonrelativistic quantum field theory the particle number operator commutes with the Hamilton operator which permits to introduce negative besides positive masses as the fundamental constituents of matter. Assuming that space is densely filled with an equal number of positive and negative locally interacting Planck mass particles, with those of equal sign repelling and those of opposite sign attracting each other, all the particles except the Planck mass particles are quasiparticles of this positive-negative-mass Planck mass plasma. Very much as the Van der Waals forces is the residual short-range electromagnetic force holding condensed matter together, and the strong nuclear force the residual short range gluon force holding together nuclear matter, it is conjectured that the Higgs field is the residual short range gravitational force holding together pre-quark matter made up from large positive and negative masses of the order ±1013 GeV. This hypothesis supports a theory by Dehnen and Frommert who have shown that the Higgs field acts like a short range gravitational field, with a strength about 32 orders of magnitude larger than one would expect in the absence of the positive-negative pre-quark mass hypothesis.

ABSTRACT THE DIFFERENCE ATTITUDE ON PALIATIVE NURSING ACTION BETWEEN STUDENTS WHO HAD ENROLLED AND HAVE NOT ENROLLED THE PALLIATIVE SUBJECT AT UNAI Nurhayati Siagian Faculty of Nursing, Universitas Advent Indonesia Siagian_nurhayati@yahoo.co.id Introduction: This study is conducted in relation to the fact where many students are not exposed to true palliative care because they have not got the Palliative nursing material. This is important because they are going to deal directly with the Palliative patients when they have practiced at the hospital as the result they have to know how to deal with and to look after the Palliative patients, whereas the Palliative Nursing subject is newly implemented in nursing curriculum in Indonesia. Method: The research method used in this study was descriptive quantitative. The population were third year and fourth year students of Bachelor of Nursing program at Universitas Advent Indonesia. The samples were 84 students (42 students had enrolled Palliative Nursing subject and 42 students have not enrolled the Palliative Nursing subject), which were determined by purposive sampling. The instrument and material are used the Frommelt's Attitude Towards Care of the Dying (FATCOD) Score questionnaire consisting of 24 questions and used Likert scale. Results: There was a significant difference on the attitudes on Paliative Nursing actions between students who have not enrolled and who had enrolled the Palliative subject, with the significance results of (2 tailed) 0.019 <0.005 which meant that there was a significant difference. Discussion: After the research is conducted, it is found that there was a significant difference attitudes of the students who have not enrolled the Palliative Nursing subject when they have taken care the Palliative patients. The results of this study are expected to be useful for the community, especially to the students, lecturers and researchers in order to improve the Palliative nursing quality in Indonesia. Abstract:
Untuk mengukur sikap perawat terhadap pasien menjelang ajal digunakan kuesioner Frommelt’s Attitude Toward Care of the Dying (FATCOD) scale. Kata kunci: kematian; menjelang ajal; sikap perawatNURSE’S BEHAVIOR TOWARD DEATH AND DYING PATIENT ABSTRACT Caring for a dying patient is an unpleasant experience because nurses can feel frightened, anxiety, sadness, and frustration. The purpose of this study was to determine the correlation between nurse’s behavior towards death and dying patient. Data collection was conducted from October - November 2019 to 95 nurses at Adven Hospital of Bandung using a convenience sampling technique and descriptive correlation method using Frommelt's Attitude toward Care of the Dying (FATCOD) scale and Death Attitudes Profile-Revised (DAP-R) questionnaire. The results of this study indicate that the majority (87.4 %) of nurses have a negative dying attitude, and 81.1 % of nurses have a negative death attitude. F-test and t-test show that the work unit and palliative training have a significant correlation (sig <0.05) to nurses’ dying attitude. Therefore the authors suggest that nurses need to take palliative training to enhance nurses' positive dying attitudes and death attitudes. Keyword: death; dying; nurse attitude

Abstract:
Abstract Introduction: Polyglandular Autoimmune Syndrome Type II (PAS-2) is a rare disorder characterized by two or more endocrine diseases (primary adrenal insufficiency, autoimmune thyroid disease, type 1 diabetes). Of these, the most common is autoimmune thyropathy, followed by Type 1 Diabetes (1). These endocrinopathies rarely have concurrent onset. This case reports a 22-year-old male, recently recovered from severe pneumonia, who presented to the Emergency Department in acute adrenal crisis and was diagnosed with PAS-2. Case: A 22-year-old male with past medical history of celiac disease presented with abdominal discomfort, nausea, vomiting, fatigue and dizziness for 1–2 weeks. Review of systems included a 20-pound weight loss over several months. Recent history included a hospitalization at another facility three weeks prior for pneumonia and septic shock requiring admission to the ICU and vasopressor treatment. He was not discharged on any medications. He was afebrile with heart rate of 105/min, blood pressure 78/48 mm Hg and BMI of 17.2. Physical exam revealed dry mucous membranes and mild diffuse abdominal tenderness. Skin was warm and dry without hyperpigmentation. Laboratory values included sodium 123 (135 - 146 mmol/L), potassium 6.8 (3.3 - 4.8 mmol/L), glucose 47 (70 - 100 mg/dL), TSH 37.67 (0.3 - 5.00 uIU/mL), and FT4 0.7 (0.7 - 1.7 ng/dL). He was started on fluids and intravenous hydrocortisone. Cortisol and ACTH levels drawn prior to the initiation of steroids resulted at 0.6 (6–20 mcg/dl) and 977 (9 - 46 pg/mL) respectively. Additional labs included: aldosterone < 1 ng/dL, 21 hydroxylase antibody positive, TPO antibody > 1000 (0 - 100 Units) and GAD-65 antibody > 47 IU/mL (<5 IU/mL). Levothyroxine was initiated after hydrocortisone. Blood glucose was elevated during hospitalization, peaking at 227 mg/dL. He was discharged on prednisone, fludrocortisone and levothyroxine. At 2 week follow up, he reported overall improvement in health and was pleased with a weight gain of 12 lbs. Blood glucose remained mildly elevated (123 - 143 mg/dL). Conclusion: The patient had pneumonia and septic shock septic at an outside hospital three weeks prior to presentation. There was no record of steroid administration or suspicion of adrenal insufficiency. We postulate that his severe illness contributed to significant depletion of his adrenal reserve, and therefore he presented to our facility a short time later in overt adrenal crisis. Adrenal crisis is unusual to be the first presentation of PAS-2. It is important to have a high index of suspicion for adrenal insufficiency and PAS-2 in patients presenting with severe illness or hypotension who have known autoimmune disorders. Reference: 1. Abstract:
Background:Adiponectin is a cytokine mainly secreted by the adipose tissue1, whose circulating levels are paradoxically low in subjects with obesity and associate with a beneficial metabolic profile2., Recent studies have shown that adiponectin levels are elevated in both serum and synovial fluid collected from patients with rheumatoid arthritis (RA)3,4. Moreover, adiponectin is able to induce the production of interleukin (IL)-6, tumor necrosis factor (TNF), CXCL1 and CXCL8 by lymphocytes from healthy subjects5, and of IL-6 and CXCL8 by fibroblast-like synoviocytes (FLS) from patients with RA6. However, it is not clear if adiponectin is able to initiate the inflammatory processes associated with the preclinical phase of RA.Objectives:We aim to determine if adiponectin is able to induce inflammatory responses in peripheral blood mononuclear cells (PBMCs) and FLS from non-inflamed subjects. Methods:Human PBMCs were collected from healthy donors, whereas non-inflamed FLS from non-arthritic patients who underwent diagnostic arthroscopy due to previous trauma. PBMCs (1× 105cells/well in 96-well plate) and FLS (5000 cells/well in 96-well plate) were stimulated using 5 μg/ml recombinant human total adiponectin protein, and the supernatants were collected 48 hours after stimulation. Phytohemagglutinin (PHA) and TNF were used as positive controls to activate PBMCs and FLS, respectively. Using multiplex assay and ELISA, we screened the production of 13 chemokines and 12 cytokines from healthy human PBMCs and non-inflamed FLS.Results:Adiponectin was able to stimulate a distinct profile of chemokines and cytokines in PBMCs and FLS. In both healthy PBMCs and non-inflamed FLS adiponectin induced the production of CXCL1, CXCL5, CXCL8, CCL2 and IL-6. Moreover, CCL3, CCL20, CCL4, CCL17, TNF, IL-10 and GM-CSF were induced by adiponectin only in healthy PBMCs, whereas CXCL10, CCL5 and CCL11 only in non-inflamed FLS (Fig. 1 and 2).Figure 1.Adiponectin induces the production of various chemokines from PBMCs (A) and FLS (B). CXCL8 (#) was measured using ELISA, and other chemokines were measured using multiplex assay. The fold change of CXCL1 in FLS (†) was not calculated because its level before stimulation was undetectable. Conclusion:We here report that adiponectin has pro-inflammatory properties as it induced chemokine and cytokine production from human healthy PBMCs and non-inflamed FLS. As adiponectin is able to induce pro-inflammatory responses from non-inflamed cells, we suggest that this adipokine might be implicated in the preclinical phase of RA pathogenesis.References:[1]Makki K, Froguel P, Wolowczuk I. Abstract:
Background:Long non-coding (lnc-)RNA are regulatory molecules transcribed from DNA similar to mRNA and interact directly with DNA, RNA and proteins. Some lncRNAs have been shown to contain micro (mi-)RNAs in their sequence that can be released by splicing and lead to active miRNA molecules, e.g. lncRNA H19 includes two miRNAs 675-3p and -5p in its sequence.Adipose tissue derived factors (adipokines) are involved in inflammation processes and osteoarthritis (OA) development. The proinflammatory adipokine visfatin has been shown to alter osteogenic differentiation (OD) of pluripotent mesenchymal stem cells (MSCs) and reduces elastic fiber expression, increases matrix mineralization and proinflammatory cytokine and chemokine production(1).Objectives:We evaluated a novel effect of visfatin on lncRNA H19 in MSCs during OD. The goal was to explore the kinetics of the visfatin effect during OD with regard to H19 regulation and to investigate H19 downstream mechanisms leading to the observed altered MSC differentiation and osteoblast activity. Methods:MSCs isolated from OA hip or knee bone (phMSC) and commercially obtained healthy human (h-)MSCs were differentiated towards osteoblasts with/without visfatin, resistin, leptin, TNF and Wnt/TGFβ1 pathway inhibitors. Supernatants were collected at days 2, 7, 9, 14 and 21 of OD, cell lysates at day 2, 7, 9, 14 and matrix mineralization assays conducted at day 21. H19 and miRNA expression was evaluated by real-time PCR after mi-/RNA isolation. IL-6 was analyzed by ELISA.Results:H19 was continuously upregulated in unstimulated controls as expected during OD but also when stimulated with other adipokines. In contrast, stimulation with visfatin significantly decreased H19 (day 2 to 14 of OD, hip-phMSCs: p = 0.0097, knee-phMSCs: p=0.0075, h-MSC: p = 0.044). Visfatin increased matrix mineralization and IL-6 production as expected (hMSC: p = 0.03, phMSC: p = 0.013)(1). TNF stimulation during OD did not lead to a downregulation of H19 nor increased matrix mineralization, thus showing that the effects were visfatin-dependent. H19s endogenous miRNA 675-5p was changed in parallel with H19, increased during control OD and significantly down-regulated by visfatin (e.g. day 14 p = 0.015). However, H19s endogenous miRNA 675-3p was inversely regulated, downregulated during control OD while visfatin stimulation attenuated this effect (e.g. day 14 p = 0.025). Altered Wnt-signaling and involvement of the TGFβ1 pathway could not be observed.Conclusion:H19 is upregulated during OD and may therefore play a regulatory role in the process of osteogenesis. Visfatin stimulation of MSCs during OD showed pro-inflammatory effects, increased matrix mineralization while reducing elastic fiber production(1). These effects were associated with a downregulation of H19, a specific visfatin effect not triggered by other adipokines or TNF. The H19 sequence includes two endogenous micro-RNAs 675-3p and 5p. We demonstrated miRNA 675-5p to be regulated in parallel to H19, whereas miRNA 675-3p was inversely regulated and increased continuously upon visfatin stimulation. Based on these results, we hypothesize that visfatin provides a specific stimulus for the splicing of miRNA 675-3p from H19, in turn leading to H19 reduction. miRNA 675-3p thus represents an effector mechanism of visfatin that contributes to the observed functional effects in differentiating MSCs.References:[1]Tsiklauri, L.et al.Visfatin alters the cytokine and matrix-degrading enzyme profile during osteogenic and adipogenic MSC differentiation.Osteoarthr. Abstract:
Abstract “Lambeth Palace is my Washpot. Over Fulham have I cast my breeches.” So declared the novelist and secularist H. G. Wells in a letter to his mistress, Rebecca West, in May 1917. His claim was that, because of him, Britain was “full of theological discussion” and theological books were “selling like hot cakes”. He was lunching with liberal churchmen and dining with bishops. Certainly, the first of the books published during Wells’s short “religious period”, the novel Mr. Britling Sees It Through, had sold very well on both sides of the Atlantic and made Wells financially secure. Geoffrey Studdert Kennedy (“Woodbine Willie”) wrote that, “Everyone ought to read Mr. H. G. Wells’s great novel, Mr. Britling Sees It Through. It is a gallant and illuminating attempt to state the question, and to answer it. His thought has brought him to a very real and living faith in God revealed in Jesus Christ, and has also brought relief to many troubled minds among the officers of the British Army.” Yet, Wells’s God was explicitly a finite God, and his theology was far from orthodox. How can we account for his boast and for the clerical affirmation which he certainly did receive? This article examines and re-evaluates previous accounts of the responses of clergy to Wells’s writing, correcting some narratives. It discusses the way in which many clergy used Mr. Britling as a means by which to engage in a populist way with the question of theodicy, and examines the letters which Wells received from several prominent clerics, locating their responses in the context of their own theological writings. This is shown to be key to understanding the reaction of writers such as Studdert Kennedy to Mr. Britling Sees It Through. Finally, an assessment is made of the veracity of Wells’s boasting to his mistress, concluding that his claims were somewhat exaggerated. “Lambeth Palace is my Washpot, Over Fulham have I cast my breeches.” It is a gallant and illuminating attempt to state the question, and to answer it. His thought has brought him to a very real and living faith in God revealed in Jesus Christ, and has also brought relief to many troubled minds among the officers of the British Army.”

Abstract:
Background:Adiponectin is a major adipokine with pleiotropic effects on inflammatory conditions including rheumatoid arthritis (RA). Adiponectin generally has anti-atherogenic effects, and its serum level inversely correlates with body mass index (BMI) and visceral fat area (VFA). On the other hand, several studies have indicated a deleterious role of adiponectin in RA progression [1]. Recently, both low BMI and increased serum adiponectin have been reported as poor prognostic factors of RA [2, 3]. However, large-scale surveys have not been done focusing on both BMI and serum adiponectin, and it is unclear which factor provides further contribution to RA disease activity. In addition, the effects of biological disease-modifying antirheumatic drugs (bDMARDs) and Janus kinase (JAK) inhibitors on serum adiponectin are largely unknown. Objectives:To clarify the relationship among serum adiponectin, body composition, current disease activity and therapeutic agents of RA.Methods:We conducted a cross-sectional study in RA patients under treatment with agents including bDMARDs and JAK inhibitors. A total of 351 subjects from the Kyoto University RA Management Alliance cohort (KURAMA) were enrolled. We classified the participants into five body composition groups (overweight with or without visceral adiposity, normal with or without visceral adiposity, and underweight), according to the cut-off points for obesity and visceral fat used in Japan: BMI, 18.5 kg/m2for underweight and 25.0 kg/m2for obesity, and VFA, 100 cm2for visceral adiposity. Differences of continuous variables among the five groups were assessed by the Steel-Dwass test or one-way analysis of variance (ANOVA). We adopted a multiple standardized linear regression model to analyze effects of serum adiponectin level on DAS28-ESR.Results:Serum adiponectin levels (20.9±12.5 vs. 14.7±8.4 µg/ml, p < 0.001) and DAS28-ESR (3.04±1.0 vs. 2.63±0.9,p= 0.017) in the underweight group were significantly higher than those in the others. In multiple regression analysis, serum adiponectin level, but not BMI, was positively correlated with DAS28-ESR (estimate = 0.0127,p= 0.0258). Subanalysis also showed that the use of bDMARD or JAK inhibitor did not have an obvious influence on circulating adiponectin. Conclusion:In the multiple regression analysis we revealed a positive and independent correlation between serum adiponectin and DAS28-ESR in Japanese RA patients. Thus, serum adiponectin is an potential marker reflecting high disease activity of RA regardless of current medications.References:[1]Frommer KW, Zimmermann B, Meier FM, Schroder D, Heil M, Schaffler A, et al. Abstract:
