Ras, Tasleem. "An audit of geriatric stroke rehabilitation services at a post-acute hospital (Booth Memorial) in urban Cape Town, South Africa." Master's thesis, University of Cape Town, 2009. http://hdl.handle.net/11427/12604.
Full textBooth Memorial Hospital (BMH), situated at the foot of Table Mountain in urban Cape Town, South Africa, is an NGO-run hospital in a Public- Private Partnership with the Western Cape Department of Health. The essential service being offered is sub-acute health care, with state hospitals referring patients needing terminal, rehabilitation or convalescent care for a variety of illnesses. Post-acute stroke rehabilitation forms an integral part of the general service being offered by BMH. This dovetails with the state-run rehabilitation hospital in the sense that younger patients able to undergo intense rehabilitation are sent to the state-run hospital, while BMH generally cares for those patients who may not be able to withstand the rigours of an intense rehabilitation program. In our setting, this means that most of the patients seen at BMH for post-stroke rehabilitation are elderly. The elderly patient who has suffered a stroke is vulnerable in many respects. The most obvious vulnerability refers to the physical, emotional and psychological derangements that follow strokes in this age group. In addition, due to rampant poverty prevalent in the community we serve, social and economic factors place this group of patients at an even greater disadvantage. It is an imperative for social survival and an acceptable quality of life that this group of patients retain as much of their independence and empowerment after the stroke as is possible. The current situation of overflowing old-age homes, increasing prevalence of elder abuse and neglect, and the deficiency of community structures to care for the elderly compel health care providers to ensure that a post-stroke rehabilitation service operates with maximal efficiency, given the paucity of resources endemic in our health system. The above factors provided an impetus for this study. A need arose to assess the quality of the stroke rehabilitation services at BMH, as the starting point to what is hoped will be a continuous Quality Improvement Cycle. An additional factor compelling the examination of the quality of service being offered is that the Geriatric population is underserved in the present health system, competing for scarce resources with other vulnerable groups such as children, Cancer patients, and people living with HIV/AIDS. By focussing on a service that is almost exclusively aimed at the elderly, we hope to be able to advocate for greater resource allocation to this age group, given that receiving good health care is essential to the quality of life of the ageing population. This assessment of the current services looked at the Structure, Process of care and Clinical Outcomes of stroke rehabilitation services being given to patients over the age of 65 years. The findings were evaluated against internationally accepted norms of post-acute stroke care, and are presented in this study.

Full textENGLISH ABSTRACT: It is very common to talk about or refer to people as “People Living with HIV/AIDS” but never “A Church with Aids”. Responding to HIV/AIDS has earned the JL Zwane Memorial Church this name or rather this label. Members of the congregation, many of them young, were dying and remaining silent, not doing anything, would have meant contributing to the disaster that had struck. Something drastic had to be done to respond to the pandemic or else the whole community would have perished. This was not easy as it meant risking losing those already in the church. To many people, HIV/AIDS was seen as a punishment from God to those who have sinned. For this reason many did not want to have anything to do with People Living with HIV/AIDS. The whole response is driven by the needs of the community i.e. those infected and those affected. This means listening to people tell their stories and then respond accordingly. Listening is the key in the response as the whole response revolves around people and their experiences.

Full textThis work is founded on the study of three funerary complexes of outlying suburbs located in the capital cities of Mediomatrici (Metz-Divodurum), Nervii (Bagay-Bagacum) and Morini (Thérouanne-Tervanna). Our study is based on a corpus composed of more than 480 funerary structures, which has been amended by the data issued from the other funerary discoveries carried out in these three cities, from the 18th century to nowadays. Based upon that documentation, we propose an analysis of funerary practices in use in the North of Gaul and more particularly in the province of Gallia Belgica and of their evolution in the first three centuries of our area. The first part of this work is related to the organization of the funerary areas in an urban context through the analysis of different parameters: places and settlement dynamic of funerary areas, internal organization and space structuring as well as the mode of distribution of the structures (functional, chronological and social). The following chapters are related to the understanding and interpretation of the different funerary practices and gestures, which may have been observed during the study of the structures of our corpus. To do that, we have tried to reconstruct the different steps of the funerary process and to replace the whole practices in the order in which they took place. First, we took an interest in the data connected with the stage of the body cremation, data observed during the excavation of the funerary pyres, through the examination of the pit structure, the methods of treatment of the body as well as the role and the origin of the grave goods used around the pyre. Then, we moved on to the place of final deposition of the remains by presenting the constituent elements of the grave, the different types of funerary structures we have been confronted with as well as the varied modes of deposition of the bones and their organization within the sepulchral pit. A specific attention has also been given to the steps which are relative to the transfer and to the modes of deposition of the bones of the deceased in the grave as well as the practices and body movements which follow that. Finally, in a last chapter, we have analyzed the different deposits of the funerary goods in order to define as well as possible the different gestures which were pointed out through the archaeological remains, and which were made at the time of the composition of the grave but also throughout the funeral or during the memorial feasts where respects to the deceased were paid. This overall study is based on a detailed catalogue of the tombs which is composed of two volumes (volume 1: Metz “Avenue André Malraux”, volume 2: Bavay “La Fache des Près Aulnoys” and Thérouanne “Les Oblets”) displaying by chronological phases the observations made during the excavation work, the inventory of the grave goods, as well as the results of the studies of animal and human bones rests

Currently, there has not been any related study concerning the influence on the work satisfaction and the morale by military funeral services. In addition, the number of related references is very limited. Therefore, in this essay, method of quantitative research along with qualitative analysis is used to comprehend the satisfaction from funeral services of career soldiers and officers. The correlation between the work satisfaction and the morale is also analyzed. In this essay, some viewpoints are demonstrated as references to enhance military morale. In the part of quantitative research, a purposive sampling method was used to sample career soldiers and officers on a naval base in Taiwan. Each of the 200 subjects was given a questionnaire to fill out and a total of 200 questionnaires were returned. After two incomplete ones were taken out, 198 were valid (198 out of 200). As to the part of the qualitative analysis, a purposive sampling method was used when the data was collected. Five career soldiers and officers (three with experience handling funerals and two without) were interviewed in depth. They provided their personal feelings and suggestions toward the military funeral services. In addition, a professional in funeral services (a mortician) is interviewed. Through discussions on the complicated issues of the services, the study was completed. While analyzing the collected data, I tried to take down every word from the subjects so that the perception and current situation of the subjects could be truly presented. Results from the analysis: 1.Career soldiers and officers were satisfied with the honoring ceremonies and family allowances. 2.Career soldiers and officers in general had negative impressions of civil funeral services. 3.Career soldiers and officers were positive about gaining professional capabilities and job achievement from work. 4.Career soldiers and officers were all positive about the morale in each unit. 5.However, all the subjects agreed that implementing military funeral services could help enhance work satisfaction and morale. Suggestions: 1.The contents and related organizations of military funeral services should be publicized entirely. Thus, the services could be provided to career soldiers and officers at the crucial time when they would be needed. 2.Subsidies and compensations for military funeral services should be increased to adequately assist soldiers, officers and their families through difficult times. 3.The military should establish a service mechanism with the objective being to “Protecting soldiers’ interests”, and assist them in handling funeral issues (such as consultation, evaluation of civil funerary services, as well as price assessment and comparison). 4.The army should pay more attention to the work satisfaction of career soldiers and officers. 5.They should attach importance to their morale as well. 6.Strengthen welfare and services in the military so that the work satisfaction and morale can be enhanced.
