Abstract:
Purpose The concept of Takaful has a long history. It is linked with the era of Prophet Muhammad 1,400 years ago. The globalization and development of socio-economic systems have made business activities more complex in response to emerging human needs and requirements. Similarly, Takaful insurance has fully commercialized and become an important indicator of the international financial market. The purpose of this study is to understand the Takaful mechanism and progression of its procedures to date since its inception. Design/methodology/approach This study seeks to examine the origin, evolution and historical developments of Takaful mechanism, operations, models and governing framework with extant literature review from previous studies and current practices. Findings The modern Takaful insurance first began in Sudan back in 1979. The Takaful operations must abide by the Sharia laws and work under the supervision of the Sharia Supervisory Board. Since its evolution, Sharia scholars have introduced various Takaful models that are going to be explained in this study. Moreover, several Islamic organizations, including the “Islamic Financial Services Board” and the “Accounting and Auditing Organization for Islamic Financial Institutions,” have provided guidelines and supervision to develop and strengthen the Takaful industry further. The study acknowledges Takaful as a growing insurance industry with huge potential and promising future in both Pakistan and the international market. Practical implications During the analysis, various deficiencies and loopholes were identified, which are responsible for the unmatched growth of conventional insurance. They can be eliminated with the joint efforts of industrial players, Sharia scholars and Takaful insurance companies. Hence, Islamic scholars and academic researchers are encouraged to develop and modify the current practices of Takaful mechanism according to current market demands and consumer approach. The research efforts will help Takaful operators to develop more innovative Takaful products adhering Sharia compliance. Consequently, it will help to access more consumer market and further enhances the Takaful growth. Originality/value This study is an effort to provide a basic understanding of the mechanism of Takaful models. The study helps to comprehend how Takaful models have evolved and been modified over the course of time. Moreover, it provides a base for further development and improvement in current practices of Takaful models, which will result in increased progress for the Takaful industry.

Abstract:
Background Although confirmed disability progression (CDP) is a common outcome in multiple sclerosis (MS) clinical trials, its predictive value for long-term outcomes is uncertain. Objective To investigate whether CDP at month 24 predicts subsequent disability accumulation in MS. Methods The Comprehensive Longitudinal Investigation of Multiple Sclerosis at Brigham and Women’s Hospital includes participants with relapsing-remitting MS or clinically isolated syndrome with Expanded Disability Status Scale (EDSS) scores ≤5 (N = 1214). CDP was assessed as a predictor of time to EDSS score 6 (EDSS 6) and to secondary progressive MS (SPMS) using a Cox proportional hazards model; adjusted models included additional clinical/participant characteristics. Models were compared using Akaike’s An Information Criterion. Results CDP was directionally associated with faster time to EDSS 6 in univariate analysis (HR = 1.61 [95% CI: 0.83, 3.13]). After adjusting for month 24 EDSS, CDP was directionally associated with slower time to EDSS 6 (adjusted HR = 0.65 [0.32, 1.28]). Models including CDP had worse fit statistics than those using EDSS scores without CDP. When models included clinical and magnetic resonance imaging measures, T2 lesion volume improved fit statistics. Results were similar for time to SPMS. Conclusions CDP was less predictive of time to subsequent events than other MS clinical features.

Abstract:
In the review, the author returns to the topic of uremia and uremic toxins, their importance for practitioners in the treatment using renal replacement therapies, gives a modern look at their classification, place during the onset and development of pathological processes in the progression of chronic kidney disease. However, current guidelines and studies for the treatment of chronic kidney disease indicate a lack of attention to the role and importance of uremic toxins in the predialysis stages of uremia treatment, in particular to the possible damaging effects of substances retained in the body with reduced glomerular filtration, directly to the renal function. The tables with the list of uremic toxins according to their classification are presented. References are made to the results of clinical and laboratory studies of uremic toxins, their impact on the general clinical picture of uremia and ways of their influence on the progression of chronic kidney disease and the further progression of the clinical picture of uremia. Attention is drawn to the fact that substances recognized as uremic toxins are present in healthy individuals without manifestations of their negative effects, and therefore the opinion is expressed about the need to study the physiological significance of these solvents under normal glomerular filtration. The question arises about the consideration of the factors of uremic toxins impact as a point of application in terms of the progression of chronic kidney disease and the use of this knowledge in renoprotective therapy in the predialysis stages of chronic kidney disease.

Abstract:
Overview: The standard of care for both newly diagnosed and recurrent glioblastoma (GBM) patients has changed significantly in the past 10 years. Surgery followed by radiation and concurrent and adjuvant temozolomide is now the well-established standard treatment for newly diagnosed GBM. More recently, bevacizumab has become a mainstay of treatment for recurrent GBM. However, despite these advances and significant improvements in patient outcomes, the management and treatment of GBM patients remains a challenging and frustrating endeavor. Difficulties in interpretation of imaging changes after initial treatment, as well as the effects of antiangiogenic agents like bevacizumab on MRI characteristics, can make even the determination of disease progression complicated in multiple situations. Although a high percentage of patients benefit from antiangiogenic therapy in terms of radiographic response and progression-free survival, the effects of bevacizumab on prolonging overall survival remain controversial. Furthermore, tumor progression after treatment with antiangiogenic agents carries a particularly poor prognosis and there is a general lack of effective therapies for this group of patients. These limitations in terms of standard treatments contrast with a relative wealth of new information regarding the molecular underpinnings of GBM. Data from several large-scale efforts to molecularly profile GBM tumors including The Cancer Genome Atlas (TCGA) project have helped define specific molecular subtypes of GBM with distinct biology and clinical outcomes. These findings are helping to refine our understanding of the molecular heterogeneity and pathogenesis of these tumors and provide a basis for the future development of rational and targeted therapies for specific tumor subtypes.

Abstract:
The rapid expansion of computer technology have forced several business sectors to integrate with the continuous development of techniques in order to assist them in various tasks. Many fields have happily embraced the technology implementing it in numerous ways, however the development speed have proven difficult to keep up with. The insurance industry have struggled with ridding themselves of old and monolithic legacy systems with a haphazard construction. These systems are costly, cumbersome and often reliant on a “third party” centered structure creating such flaws as data leaks and monopolisation. Blockchain is a distributed ledger operating over a peer-to-peer basis, with the intention to unshackle contemporary system from their dependence towards central authorities. Additionally, the peer-to-peer architecture introduced a new form of transparency which differs from contemporary solutions used in centralised systems, beyond the peer-to-peer architecture, the blockchain also incorporated consensus algorithms, allowing peers to verify one another to achieve consensus regarding the validity of each block. This resulted in a "trustless system" considering no single party in the community is dependent on the credibility of a central authority. In order for the blockchain technology to be applicable on the market it must overcome obstacles such as privacy and the new EU data protection regulation “General protection data regulation”. However a vague definition of personal data have caused ambiguity which appears to be irreconcilable with the blockchain technology. Research have therefore shifted its focus to explore opportunities for the technology to collaborate with techniques from centralisation in order to overcome its obstacles.

Abstract:
It is critical to determine whether ERM creates value to ensure that the significant resources deployed in ERM programmes result in tangible corporate value. This thesis analyses the valuation implications of ERM maturity using The Risk and Insurance Management Society Risk Maturity Model (RIMS RMM) data collected from 2006 to 2011 using a 5-point maturity scale. The results, measured by Tobin’s Q, suggest that firms that have reached mature levels of ERM exhibit higher firm values, demonstrating a statistically significant positive relation to the magnitude of 25 percent. Upon decomposition of the maturity score, the most meaningful aspects of ERM relate to top-down executive engagement and the resultant cascade of ERM culture throughout the firm. In addition to serving as one of the first studies to establish strong empirical evidence for ERM increasing firm value, the thesis extends its contribution to examine the firm performance implications of ERM maturation under different models, specifically identifying characteristics that engender or inhibit the aforementioned valuation implications. A discussion of how an ERM programme could significantly affect an organisation’s policies and relationship with stakeholders is also explored. ERM is theoretically projected to be a means to help strengthen the relationship that companies have with regulators, reinforce or repair relationships with shareholders, strengthen interactions with supply chain stakeholders, enhance reputational relations with clients and improve auditor partnerships. The thesis concludes by arguing that ERM has now reached a critical mass, such that further adoption and development of the ERM discipline is now inevitable.

Abstract:
Myasthenia gravis (MG) is an autoimmune disease whose name means "grave muscular weakness". MG is a rare disease affecting only 200-400 persons per million and the characteristic symptoms include muscle weakness, particularly in highly active voluntary muscles. MG affects the neuromuscular junction in an antibody-mediated manner, resulting in impaired nerve-muscle cell communication in affected individuals. Specifically, two main proteins are targeted: nicotinic acetylcholine receptors (ACh receptors) and a muscle-specific tyrosine kinase (MuSK). Previous studies have discovered the mechanism of MG pathogenesis but the exact mechanisms which cause the failure to maintain self-tolerance have not been discovered. Based on current knowledge of MG, this paper will explore potential causes of the disease and provide numerous hypotheses directed at future research opportunities.

Abstract:
Dementia progression is characterised by a lengthy pre-symptomatic phase, where pathology may be accumulating, followed by a more rapid decline evidenced by onset of clinical symptoms and eventually functional impairment. The rate of decline in symptoms from cognitive impairment to dementia greatly varies between individuals, complicating prognosis and the assessment of much needed disease-modifying drugs. As a result there is a huge demand for greater understanding of the between-subject variability in progression and a need to understand biomarkers and risk factors for predicting future cognitive decline. In cohorts derived from two of the largest NHS foundation trust mental health service providers in the UK and multiple Alzheimer’s cohorts, this thesis explores methods for modelling disease progression and investigates the relationship between blood based proteins, genetic variants, health indicators and potential repurposing medications with cognitive decline. Through applying approaches that tackle three areas that require consideration for modelling of cognitive decline and disease progression, this thesis identified associations of antidepressant medication and psychotic symptoms associated with faster cognitive decline in dementia, in two separate cohorts.

Abstract:
The use of the present progressive to express future is a grammatical construction imported into Portuguese from English. In this construction here called Future Present Progressive (FPP), the verbal predicate expresses the present tense, while a temporal adverb locates the event in the future. The assimilation of this construction to Brazilian Portuguese (BP) enriches the language’s inventory of periphrastic expressions of the future tense. This work studied the internal structure of FPP constructions in BP and the relationship between its components. Lexical and grammatical aspects were considered to investigate why utterances such as: ‘Estamos nos mudando em duas semanas’  (We are moving in two years) are grammatical, while utterances as: ‘*Estou morando no Candá ano que vem’  (*I am living in Canada next year) are not, although both expressions are grammatical in the present progressive in BP. The research was conducting with naturally occurring language, collected from the Twitter microblogging service, using a methodology, which one might call digital linguistics. The data was treated using the resources of the multi-lingual database TypeCraft, and is freely available at: http://typecraft.org/TCEditor/1730/. Our analysis of FPPs focuses on the relationship between lexical and grammatical aspect and suggests an extension of the semantic features that define different aspects. It furthermore highlights the role that the stage-level predicate ‘estar’ plays in deriving the semantics of this construction. Our research suggests that, besides the combination between lexical and grammatical aspect, different semantic mechanisms can also influence the compatibility pattern of verbs and syntactic constructions. Our research also shows that the FPP, although a borrowing from English, has morphosyntactic and semantic properties that makes it an integral part of the BP grammar.

Abstract:
This dissertation puts forward an analysis of auxiliary verbs in general, and of Portuguese auxiliary verbs in particular. The proposal is to universally characterize auxiliary verbs as a class of verbal elements which (i) belong to the verbal category ; (ii) have a unvalued verbal feature [uV] ; (iii) do not assign theta roles ; and (iv), together with the main verb, compose one single phrasal domain. The presence of [uV] guarantees that the auxiliary verb will select a verbal projection as its complement, within which is the target that will value the uninterpretable [uV] feature by means of the operation Agree (Chomsky 2000, 2001). The four characteristics proposed to describe auxiliary verbs allowed for the derivation of criteria of auxiliarity, a reinterpretation of the criteria usually discussed in the literature. Eight necessary and sufficient criteria define an auxiliary verb in Portuguese. The composition of THE Portuguese auxiliary class with its four members ser, ter, estar and ir resulted from the application of these eight criteria. Passives were analysed by means of the smuggling system, proposed by Collins (2005) and the auxiliary ser was considered to be a functional verb which values the [perfective] feature of the passive participle. A non-canonical passive was found to co-exist with canonical passives, one which shows the auxiliary ter. We argue that ter is the computational product of features of the auxiliary ser with features of the functional node v*. For the analysis of perfective ter and progressive estar, we have adopted a decompositional view along the lines of Kayne (1993), which derives these verbs from the incoporation of prepositional elements to the features of an abstract auxiliary verb. So, three of the Portuguese auxiliary verbs result from syntactic computation: passive ter is the manifestation of the features [vSER - v*]; perfective ter is the manifestation of the features [vAUX - PDEPOIS]; and progressive estar includes the features [vAUX - PDENTRO]. And the auxiliary verb ir is treated as a modal head, which, when combined with features of the head T, generate a future reading.

Abstract:
The construction of meaningful future self-representations is an important part of identity development. For adolescents living with a progressive life-threatening neurodegenerative illness, the projection of self into the future requires the movement into the unknown and uncertain realm of an illness trajectory. The purpose of this study was to examine how adolescents who are living with a progressive life-threatening neurodegenerative illness construct meaningful future self-representations. A narrative approach was utilized as the method of inquiry. Ten adolescents between the ages of 12 and 20 who had been diagnosed with a progressive life-threatening neurodegenerative illness participated in a semi-structured narrative interview. Within the interviews three lines of inquiry were followed, which explored future thinking, possible selves, and continuity in the stories of the selves over time. Data analysis occurred across four readings of the narrative transcripts. The analysis included a stepping-in and stepping-back process, allowing for individual stories to remain contextualized and for collective themes to emerge across the narratives. Major findings confirm future thinking as a contextually based activity, a construction of a range of possible selves both with and without the illness, and an ability to oscillate between these two versions of future selves. As well, the adolescents demonstrated personal continuity in the selves of the past, present, and the future. As an overarching theme, future thinking emerges as a required activity for reasons of coping, personhood, and decision-making. This research makes contributions to developmental science as well as pediatric palliative care as it extends current literature pertaining to future thinking and possible selves, thereby adding to the conceptual and theoretical knowledge base. As well, the findings highlight the importance of developing a healthcare practice that is responsive to the developmental need to create a future self and to explore all aspects of what is possible, as well as impossible.

Abstract:
This thesis introduces the idea of progressive autonomy, namely future-oriented self-governance, based on the pursuit of desired goals that one has established for oneself. As the thesis shows, focus on this sort of activity, as well as its value and importance, has been largely left out of the existing literature on autonomy. In contrast, this activity is central to progressive autonomy, which, as this thesis puts forth, enables the individual to actively determine the course of his life. Throughout the process, the individual is author of his own narrative, which, as the ongoing fulfillment of desired goals, he experiences as both meaningful and worthwhile. The thesis does not frame the narrative of the progressively autonomous individual as merely a story or an account of events. Rather, it does so as a sequence events the progressively autonomous individual is directly responsible for bringing about, as well as concretely situated in. He is, in other words, part and parcel of his narrative, as opposed to, say, a novelist who physically stands apart from the events he imagines and writes. Finally, the thesis demonstrates that, though the progressively autonomous individual is motivated by his own desires, he is more than just a pleasure seeker; he is strategically morally responsible. Such responsibility involves and is characterized by rejecting certain motives that undermine the actual achievement of desired goals, while affirming other motives that facilitate this achievement. This, as the thesis shows, renders the progressively autonomous individual more responsible than one who partakes in this rejection or affirmation, without regard to how either will impact one’s future.

Abstract:
This thesis explores the relationship between architecture and the blind people. For better clarity and understanding of the topic, The work has been divided into five distinctive parts. In the first, introduction part, I approach space sensation of a blind person from the physiological and psychological angle. This analysis is very important for the understanding of imagination, which the blind person has. Another important fact to understand is the way in which the blind person gets the information about the space and what is his ability to process it. Further, I explain the basic principles of the blind person’s movement through space and describe instructions of how to help blind people in the concrete spatial situations. In the next part, I give a short information about the historical progress of the relation between the society and the blind people. An important fact is that, as late as 200 years ago, the blind people were considered as “sub men” and did not have the full freedom privilege. This relation has been modified over time and nowadays, the blind people have the freedom of movement and all the rights to enjoy their lives. To meet the needs of the blind people, many associations have been created that act on their behalf. The associations have different modes of functioning, but their common goal is to help blind people with their social rehabilitation, looking for employment and removing architectural barriers. The core of my work is divided into two parts. In the first one, I focus on the problematic of the blind people and architecture from the functional aspect. The functional aspect is vital for the blind people. It corresponds to their safety in the environment. In this chapter, I treat in detail basic elements for designing buildings, for specifying barriers and principles of design for blind people. Furthermore, I describe the help tools the blind people need in order to move in the space. I evaluate the future of these tools and indicate the rapid technological progression which makes the life of blind people more comfortable. In the next part, I focus on the problematic of the blind people and architecture from the aesthetic aspect. This question has not been much explored yet, but is, nonetheless, very important. In this part of my work, I use more my personal experience and the knowledge coming from the exploration of space. Aesthetical values are subjective for all the people, with or without a handicap. Therefore, in this final chapter, I try to note the facts that can be used in the process of the aesthetical perception and to reflect on them. In the domain of aesthetical research, there also exist tools and institutions that help blind people in their comprehension of architecture. The division of my work emerged from the compilation of diverse facts in the process of writing. This thesis has an informative and practical character. It is intended for people who want to understand and to be informed about how to design architecture for the blind people. It will be directed towards people who build this kind of architecture and need an overview of how to make it. It is also intended for people interested in learning more about architecture.

Abstract:
Abstract In this paper, the establishment of a prognostic approach based on a nonlinear degradation model for reliability assessment focusing on health state and remaining useful life estimation is considered. A model able to describe the non-linearity of degradation to predict future damage progression for real-time application has to be defined. Real-time data are generated during operation, so incomplete data about failure and usage up to the end of life are expected. For the accurate prediction of system lifetime, estimation of future degradation from the point of assessment is required. At this point, the unavailable data are numerically calculated by integrating linearized gradients adaptively by considering nonlinearity in current degradation. The coefficients used to define future degradation gradients are identified according to different states assuming future linear degradation increments. These coefficients are determined using an optimization-based algorithm simultaneously with the calculation of consumed lifetime by extending the previously established state machine lifetime model. For performance evaluation of the approach, the effectiveness of predicting remaining useful life using tribological experiments data is investigated. The results show the potential of this approach to deal with nonlinearity in the degradation progression.

Abstract:
Based on weighted possible-world semantics, Belle and Lakemeyer recently proposed the logic DS, a probabilistic extension of a modal variant of the situation calculus with a model of belief. The logic has many desirable properties like full introspection and it is able to precisely capture the beliefs of a probabilistic knowledge base in terms of the notion of only-believing. While the proposal is intuitively appealing, it is unclear how to do planning with such logic. The reason behind this is that the logic lacks projection reasoning mechanisms. Projection reasoning, in general, is to decide what holds after actions. Two main solutions to projection exist: regression and progression. Roughly, regression reduces a query about the future to a query about the initial state while progression, on the other hand, changes the initial state according to the effects of actions and then checks whether the formula holds in the updated state. In this paper, we study projection by progression in the logic DS. It is known that the progression of a categorical knowledge base wrt a noise-free action corresponds to what is only-known after that action. We show how to progress a type of probabilistic knowledge base wrt noisy actions by the notion of only-believing after actions. Our notion of only-believing is closely related to Lin and Reiter's notion of progression.

Abstract:
For decades, the technical problem of selecting optimal transmission gear ratio has been researched for various ground vehicles based on different selection criteria depending on a particular vehicle application; criteria of terrain mobility, traction and acceleration performance, engine power utilization and fuel efficiency have been widely in use. Innumerable analytical and experimental research results and data were implemented in thousand different transmission designs. Today, this unique information about the number of gears in transmission, and value of gear ratios make a field for (i) a research analysis of engineering efficiency of different transmission designs, which were in operation for decades, and (ii) developing more efficient analytical methodologies to select the number of gears and transmission gear ratios and, furthermore, continuously, in-real time control power transfer from the vehicle energy source to the wheels. This paper considers the first, of the above-listed problems in detail with applications to various terrain vehicles and then specifically to off-road wheeled vehicles. The analysis presented in the paper, envelops simple random samples of up to 50 vehicles. It starts from WW2 military vehicles, goes to off-road trucks of 1980s and finally compares modern dump trucks and other terrain vehicles of several major world OEMs. The paper presents an analytical method, computational algorithm and results of a study in which, the efficiency of conventional analytical methodologies are evaluated using actual data on fuel consumption and characteristics of transmissions, vehicle engines, driveline and running gear systems and payloads. To serve this purpose, actual data of each vehicle is compared with analytical data of the vehicle, computed using the conventional methods, with focus on gear/velocity ratios and average fuel consumption at each transmission gear. The fuel consumption analysis was carried out by computing vehicle transport capacity as a function of the average velocity and mass of the payload for each vehicle. The result shows a distinct change of behavior in gear design methodology between post war and present day vehicles. It was a determined divergence from the initial trends, which were based on either the geometrical progression method or arithmetic method for selecting the number of transmission speeds and the values of gear ratios. This resulted in not only having a wide range of speed characteristics of automatic transmission over a few manual gears, but also, as discovered in this study, lead to increased fuel consumption of some vehicles in all range of speeds. The WW2 vehicles designed with manual transmission have gear ratios are closely aligned to analytically calculated geometric progression. Same behavior is observed in the off-road vehicles of 1980’s. Here, with a manual transmission, the trend is more towards less number of gears and with large interval between speed ratios. This of course gives a better fuel efficiency, but leads to trade off in lower average vehicle velocity. The transmission design for modern day dump trucks is also very close to the geometric progression approach. The other modern off-road trucks, as discovered in the analysis, follow an arithmetic progression. Although this results in smooth transmission, but fuel efficiency is compromised significantly, compared to dump trucks. It is important to note that a design based on geometric progression, would result in same speed distribution with less number of gears and better fuel efficiency. For a modern day terrain trucks, to have an optimum combination of both characteristics, it is important to consider all the parameters affecting velocity ratios and fuel consumption and incorporate an efficient analytical methodology to stay competitive, in the rapidly evolving market of all terrain vehicles.

Abstract:
Globally, studies show that women in the labor force are paid less, hold fewer senior positions and participate less in science, technology, engineering and mathematics (STEM) fields. A 2019 UNESCO report found that women represent only 29% of science R&D positions globally and are already 25% less likely than men to know how to leverage digital technology for basic uses. As the use and development of Artificial Intelligence (AI) continues to mature, its time to ask: What will tomorrows labor market look like for women? Are we effectively harnessing the power of AI to narrow gender equality gaps, or are we letting these gaps perpetuate, or even worse, widen? This collaboration between UNESCO, the Inter-American Development Bank (IDB) and the Organisation for Economic Co-operation and Development (OECD) examines the effects of the use of AI on the working lives of women. By closely following the major stages of the workforce lifecycle from job requirements, to hiring to career progression and upskilling within the workplace - this joint report is a thorough introduction to issues related gender and AI and hopes to foster important conversations about womens equality in the future of work.

Abstract:
This report consists of climate statistics for air pressure, temperature, relative humidity, precipitation, snow depth, wind, sunshine and global radiation for the normal period covering the years 1991–2020. In addition for the first time oceanographic statistics for sea water level, sea water temperature and sea ice are included. Based on the station and gridded data the new 1991–2020 normal period is approximately 0.6 °C warmer in Finland compared to the previous 1981–2010 period. The new normal period is approximately 1.3 °C warmer when compared to the 1961–1990 period. Climate and oceanographic statistics can be used for example when anomalies for current conditions need to be calculated and put in a historical context. In addition they help in decision-making when it is needed to approximate conditions months into the future. And lastly normal periods provide a way to monitor the progression of climate change. The calculation of the normal period statistics have been carried out following the World Meteorological Organization (WMO) guidelines. Like with previous normal periods, some stricter guidelines were followed nationally, for example in the way missing observations were handled. The period 1991–2020 is both nationally and internationally the official normal period before the next one covering 2001–2030 is taken into use.

Abstract:
Review question / Objective: Is acupuncture a safe and effective therapy for constipation associated with Parkinson’s disease? Our aim is to assess the effectiveness and safety of acupuncture for constipation associated with PD and give guidance to future research direction. Condition being studied: Parkinson’s disease (PD) is a prevalent degenerative disease of nervous system characterized mainly by static tremor, bradykinesia, myotonia, postural gait disorders and other non-motor symptoms. According to variations on race, ethnicity, age and sex, the incidence of PD ranges from 8 to 20.5 per 100, 000 individuals annually. One global research shows that there were 6.1 million individuals suffer from PD in 2016 and will be 12 million patients around the world. According to several outcomes of case-control studies, the prevalence of constipation in PD varies from 28% to 61%. Constipation, as a common gastrointestinal disease which refers to the clinical presentation of reduced spontaneous complete bowel movement, dyschezia, feeling of incomplete defecation and outlet obstruction, is demonstrated to antedate the motor symptom and it's severity is related to the progression of PD. Acupuncture has been proved to act on the pathogenesis of constipation associated with PD. The proposed systematic review we're about to present is the first advanced evidence-based medical evidence in this area.

Review question / Objective: This study is the protocol for a systematic review to evaluate the efficacy and safety of HuaYu TongFu Method combined with acupuncture in the treatment of Acute Exacerbation of Chronic Obstructive Pulmonary Disease. we conducted a systematic review and meta-analysis of published randomized clinical trials (RCTs) of such combined therapy in the treatment of AECOPD, It provides a reliable scientific basis for clinicians to use this approach to treat AECOPD. Condition being studied: Chronic obstructive pulmonary disease is the third leading cause of death worldwide. AECOPD is the most common cause of hospitalization and death in patients with COPD. As lung function deteriorates and the disease progresses, the risk of alveolar hypoxia and consequent hypoxemia increases. Inflammation plays an important role in the progression of AECOPD. Modern medicine mainly treats AECPD by anti-inflammatory, relief of airway spasm, glucocorticoids, inhalants and other methods. Long-term application can easily lead to bacterial flora imbalance and drug resistance in patients. Comparatively, traditional Chinese medicine and acupuncture therapy are safe and effective. To assess the therapeutic efficacy and safety of HuaYu TongFu Method combined with acupuncture in AECOPD, we created a protocol for a systematic review to inform future clinical applications.

Baldwin, Gretchen. Considering the Future of Gender and Peace Operations: Strategic Debates and Operational Challenges. Stockholm International Peace Research Institute, December 2022. http://dx.doi.org/10.55163/lcsv1618.

Full text
Abstract:
The women, peace and security (WPS) agenda in peace operations has had myriad successes as well as setbacks in implementation. The rise of ‘gender-sensitive’ or ‘gender-responsive’ approaches to peacekeeping signals progress in policy language, but in practice, peacekeepers can struggle to comprehensively implement gender analyses or deliver on WPS tasks. Based on a review of existing academic and policy literature, this paper identifies five strategic debates central to WPS agenda implementation: participation beyond numbers, men’s roles, defining ‘gender’, balancing gender ‘inside and outside the fence’, and non-traditional security challenges. It then identifies six key operational challenges to implementation: resourcing, accountability, recruitment barriers, scale, siloing versus mainstreaming, and securitization. With the approach of the 25th anniversary of the WPS agenda in 2025—possibly the most significant yet for the integration of gender and peace operations—stakeholders should step up efforts to ensure the sustainability of the agenda’s implementation, in line with its most progressive reading.
