Abstract:
In total, 67 parallel plots were chosen from the database of 153 phytosociological relevés made in the Douglas-fir and parallel Norway spruce, European Beech and oak-dominated stands to find influences of this introduced tree species on the understorey layer in totally 12 localities in the whole Czech Republic territory. Douglas-fir stands influence their habitats, which was indicated by species composition changes in the ground vegetation, as well as by abundance and dominance of particular species. Douglas-fir cultivation increases species diversity of the stands, but decreases their abundance. Described differences in understorey are not so noticeable when European beech and sessile oak stands are substituted by Douglas-fir once. But even the significant phenomenon of striking nitrophilous species such as Geranium robertianum, Urtica dioica and Galium aparine occurs here. This indicates a high content of available nitrates in the humus and top-soil horizons.

Abstract:
Summary Aim: Whole-body gallium scan was performed to evaluate the usefulness of gallium scan for detecting extrapulmonary tuberculosis (TB) lesions. Methods: Thirty-seven patients with extrapulmonary TB were included in this study. Four patients were found to have two lesions. Totally, 41 lesions were identified, including 19 TB arthritis, 8 spinal TB, 5 TB meningitis, 3 TB lymphadenopathy, 2 TB pericarditis, 1 TB peritonitis, 1 intestinal TB, 1 skin TB and 1 renal TB. Results: Of the 41 extrapulmonary TB lesions, gallium scan detected 32 lesions with a sensitivity of 78%. All the patients with TB meningitis showed negative gallium scan. When the five cases of TB meningitis were excluded, the detection sensitivity of gallium scan increased to 88.9% (32/36). Conclusion: Our data revealed that gallium scan is a convenient and useful method for evaluating extrapulmonary TB lesions other than TB meningitis. We suggest that gallium scan be included in the clinical routine for patients with suspected extrapulmonary TB.

Abstract:
Gallium-67 scintigraphy is useful for detecting active inflammation. We show a 66-year-old female patient with atrial fibrillation and diffuse thickening of the left atrial wall due to acute myocarditis, who presented diffuse abnormal accumulation of gallium-67 in the left atrium on single photon emission computed tomography/computed tomography (SPECT/CT) fusion images. In the second gallium-67 scan 2 months after the first scintigraphy, the abnormal accumulation in the heart was no longer visible. Gallium-67 SPECT/CT images helped understanding the disease condition that temporary inflammation in the left atrium caused atrial fibrillation.

Abstract:
O 67Ga é um importante radiofármaco usado para identificar processos inflamatórios em doenças crônicas, diagnóstico por imagem de tumores em tecidos moles e a possibilidade de avaliar o resultado para intervenção terapêutica. Neste trabalho desenvolveu-se um módulo de processamento de 67Ga, com o objetivo de reduzir as intervenções na célula quente, causadas pela oxidação dos materiais metálicos e desgastes nas mangueiras das bombas peristálticas, que soltavam resíduos e bloqueavam a passagem através das válvulas utilizadas no processo. Utilizaram-se materiais como: acrílico, PVC, PEEK e teflon e vácuo como meio de transferência de fluidos líquidos na maioria dos procedimentos, com estas modificações obteve-se redução no comprimento das mangueiras de transferência, aumentando o rendimento do processo com menos intervenções para manutenção e menos tempo de exposição dos trabalhadores à radiação, garantindo a qualidade e reduzindo-se o tempo do processamento. Utilizando-se um sistema móvel para deslocamento do módulo de processamento, facilitou-se a limpeza e manutenção da célula que opera com material radioativo, atendendo-se a Resolução da Diretoria Colegiada da ANVISA que dispõe sobre as Boas Práticas de Fabricação de Medicamentos (RDC-17).
The 67Ga is an important radiopharmaceutical used to identify inflammatory processes in chronic illnesses, diagnosis by image of tumors in soft tissues and the possibility to evaluate the result for therapeutic intervention. In the present work a module of 67Ga processing was developed with the objective to reduce the interventions in the hot cell, in order to avoid oxidation caused by metallic materials, and consuming in hoses of the peristaltic pumps, that release residues that blocked the valves used in the process. With materials such as: acrylic, PVC, PEEK e teflon and they are used vacuum as method (way) of fluid transferences instead of peristaltic pump in the majority of the procedures, with this improvements the system can make shorter the lengths of transference hoses, increasing the yield in the process with less interventions for maintenance and time exposure of the workers, guaranteeing the quality and reducing the time of the processing. using a mobile system for displacement of the processing module making in the cleanness and maintenance of the cell that works with radioactive material. Reducing the time of exposure dose of the workers in compliance with RDC-17 of ANVISA, which ruling the Good Manufacturing Practice Procedures.

Abstract:
Drug-induced acute tubulointerstitial nephritis (ATIN) is the most common aetiology of ATIN and a potentially correctable cause of acute kidney injury (AKI). An interval of 7–10 days typically exists between drug exposure and development of AKI, but this interval can be considerably shorter following re-challenge or markedly longer with certain drugs. It occurs in an idiosyncratic and non-dose-dependent manner. Antibiotics, NSAIDs, and proton pump inhibitors are the most frequently involved agents, but the list of drugs that can induce ATIN is continuously increasing. The mechanism of renal injury is postulated to involve cell-mediated immunity, supported by the observation that T cells are the predominant cell type comprising the interstitial infiltrate. A humoral response underlies rare cases of ATIN, in which a portion of a drug molecule (i.e. methicillin) may act as a hapten, bind to the tubular basement membrane (TBM), and elicit anti-TBM antibodies. The classic symptoms of fever, rash, and arthralgia may be absent in up to two-thirds of patients. Diagnostic studies, such as urine eosinophils and renal gallium-67 scanning provide only suggestive evidence. Renal biopsy remains the gold standard for diagnosis, but it may not be required in mild cases or when clinical improvement is rapid after removal of an offending medication. Pathologic findings include interstitial inflammation, oedema, and tubulitis. The time until removal of such agents and the severity of renal biopsy findings provide the best prognostic value for the return to baseline renal function. Poor prognostic indicators are the long duration of AKI (> 3 weeks), a patient’s advanced age, and the high degree of interstitial fibrosis. Early recognition and appropriate therapy are essential to the management of drug-induced ATIN, because patients can ultimately develop chronic kidney disease. The mainstay of therapy is timely discontinuation of the causative agent, whereas controversy persists about the role of steroids.
